Background: The epidemiology of burns varies depending on the socio-economic factors and cultural characteristics of each region. There is a need for data on the etiology of burn traumas in each region in terms of precautions to be taken to prevent or reduce these traumas. Our aim in this study is to analyze the demographic and epidemiological aspects of elderly Turkish and Syrian patients who were treated in our Burn Center between 2012 and 2021.Materials and Methods: After local ethics committee approval, the records of a total of 21,531 patients who were hospitalized in our hospital's burn center between January 2012 and December 2021, over a 10-year period, were reviewed. In total, demographic and epidemiological data of 155 patients (136 Turkish, 19 Syrian) were recorded. Outpatients, referred to another center, or patients with chronic burns were excluded from the study. Age, gender, burn causes, burn localizations, burn severity (grouped as mild, moderate, severe and critical according to total body surface area (TBSA) percentage), time of admission, place of residence of the patients, duration of follow-up in the hospital and results were recorded. Etiology of burns were classified as scalding, flame, electrical, chemical burns. TBSA was calculated according to the rule of nines. Burn localizations were classified as head and neck, trunk (anterior and posterior), upper extremity and lower extremity, buttocks and genital area. The admission dates of the patients were classified as autumn, winter, spring and summer. The region where the patients lived was evaluated in two groups as urban and rural.Results: A total of 155 patients, 97 (62.7%) female and 58 (37.3%) male, were included in the study. The most common cause was scalding injuries. When we classified the burn degrees of the patients according to the total body area surface, we found that the highest rate of burns was moderate (10-30% TBSA). The most frequently burned anatomical parts of the body were thearms (51.6%) and legs (50.3%) in the extremities. It was seen that 136 of the patients were Turkish and 19 were Syrian. In the analysis of the causes of burns, it was found that chemical and electrical burns were never seen in Syrians, while flame and scalding burns were statistically significantly higher in Turks. When the burned body areas were examined, it was determined that the most common burns were on the extremities in both races. It was observed that 15 (9.7%) patients died in the follow-ups. Among the deaths, it was determined that the most common death occurred under the age of 65 with 80% (p=0.027).Conclusions: In this study, we examine burn injuries in the elderly population, we can say that the need for medical care is higher and more important in this special patient group compared to younger patients, when factors such as the regional population, the number of Syrian refugees, and living conditions are taken into account. Due to the systemic effects of aging and comorbid diseases in geriatric burn patients, we can achieve reduced mortality and morbidity with a multidisciplinary approach at every step., ÖzetAmaç: Yanık epidemiyolojisi, her bölgenin sosyo-ekonomik etkenlere ve kültürel özelliklerine bağlı olarak değişiklik göstermektedir. Bu travmaların önüne geçilmesi veya azaltılması için alınması gereken tedbirler açısından her bölgede yanık travmalarının etiyolojisine yönelik verilere ihtiyaç vardır. Bu çalışmadaki amacımız 2012- 2021 yılları arasında Yanık Merkezimizde tedavi gören yaşlı Türk ve Suriyeli hastaların demografik ve epidemiyolojik olarak incelenmesidir.Materyal ve Metot: Kurum onayı alındıktan sonra Ocak 2012 ile Aralık 2021 tarihleri arasında, 10 yıllık dönemde, hastanemiz yanık merkezinde yatarak tedavi gören toplam 21531 hastanın kayıtları incelendi. Toplamda 155 hastanın (136 Türk, 19 Suriyeli) demografik ve epidemiyolojik verileri kaydedildi. Ayakta tedavi gören, başka bir merkeze sevk edilen veya kronik yanığı olan hastalar çalışma dışı bırakıldı. Çalışmaya alınan hastanın yaşı, cinsiyeti, yanık nedenleri, yanık lokalizasyonları, yanık şiddeti (toplam vücut yüzey alanı (TBSA) yüzdesine göre hafif, orta, şiddetli ve kritik olarak gruplandırıldı), başvuru zamanı, hastaların ikamet ettikleri yer, hastanede takip süresi ve sonuçları kayıt edildi. Yanık nedenleri; haşlanma, alev, elektrik, kimyasal yanık olarak sınıflandırıldı. TBSA dokuzlar kuralına göre hesaplandı. Yanık lokalizasyonları baş-boyun, gövde(ön ve arka), üst ekstremite ve alt ekstremite, kalça ve genital bölge olarak sınıflandırıldı. Hastaların başvuru tarihleri sonbahar, kış, ilkbahar ve yaz olarak sınıflandırıldı. Hastaların yaşadıkları bölge kentsel ve kırsal olmak üzere iki grupta değerlendirildi.Bulgular: Çalışmaya 97’si (%62.7) kadın, 58’i (%37.3) erkek olmak üzere toplam 155 hasta dahil edildi. En sık görülen neden haşlanma yaralanmalarıydı. Bunu alev yanıkları ve elektrik yanıkları takip ediyordu. Hastaların yanık derecelerinin toplam vücut alan yüzeyine göre sınıflandırdığımızda en yüksek oranda orta (%10-30 TBSA) derecede yanık olduğunu gördük. Yine en sık yanan vücut anatomik bölgesi ise kol(%51.6) ve bacak(%50.3) olmak üzere ekstremitelerde olduğu görüldü. Hastaların 136’sı Türk, 19’u Suriyeli olduğu görüldü. Yanık nedenlerinin analizinde ise kimyasal ve elektrik yanıklarının Suriyelilerde hiç görülmediği, alev ve haşlanma yanıklarının ise Türklerde istatistiksel olarak anlamlı şekilde daha fazla olduğu saptandı. Yanık vücut alanları incelendiğinde ise her iki ırkta da en sık ekstremite yanıklarının olduğu tespit edildi. Takiplerde 15 (%9.7) hastanın öldüğü görüldü. Ölümler arasında ise en sık ölümün %80 oranla 65 yaş altında gerçekleştiği saptandı (p=0.027). Sonuç: Yaşlı nüfustaki yanık yaralanmalarını incelediğimiz bu çalışmada bölgesel nüfus, Suriyeli mülteci sayısı, yaşam koşulları gibi etkenler göz önüne alındığında genç hastalara kıyasla bu özel hasta grubunda tıbbi bakım ihtiyacının fazla ve önemli olduğunu söyleyebiliriz. Geriatrik yanık hastalarında gerek komorbid hastalıklar gerekse yaşlanmanın sistemsel etkisi nedeniyle her adımda multidisipliner yaklaşım ile azalan mortalite ve morbiditeyi yakalayabiliriz.

ÖZ Amaç: Bu çalışma, hemipleji tanılı bir bireyde Paralimpik Boccia sporunun etkilenen elin fonksiyonel kullanımı, depresyon ve mental durum üzerindeki etkilerini incelemek amacıyla gerçekleştirilmiştir. Gereç ve Yöntem: Araştırma kapsamında Eggers sınıflandırma sistemine göre devre 3’te yer alan iskemik orta serebral arter tutulumlu kronik hemipleji tanılı sağ eli etkilenmiş olan 69 yaşındaki erkek danışana uluslararası olimpiyatlarda yer alan Engelli Boccia Spor uygulamasına dahil edilmiştir. Birey Modifiye Ashworth skalasına göre 2. evrededir. Aktif dirsek ekstansiyon ve fleksiyonu yaparak objelere uzanabilmekte ve kavrama hareketlerini hafif tonus artışıyla gerçekleştirebilmektedir. Huzurevinde gerçekleştirilen araştırmada danışanın depresyon durumunu değerlendirmek için Geriatrik Depresyon Ölçeği; el fonksiyon değerlendirmesinde Jebsen el fonksiyon testi, mental durum için standart mini-mental test kullanıldı. Paralimpik Boccia sporu haftada 4 gün, 45 dakikalık seanslar halinde toplam 8 hafta süreyle uygulandı. Müdahale sonrasında testler tekrarlandı. Sonuçlar: Müdahalenin öncesinde bireyde orta derecede depresyon olduğu görüldü. Müdahale sonrası geriatrik depresyon ölçeği sonucunda olumlu gelişme kaydedildi. Mental fonksiyon açısından yalnızca bir puanlık artış sağlandı. Jebsen el fonksiyon testinde ise Boccia Sporu öncesi ve sonrası skorlar karşılaştırıldığında, testleri tamamlama sürelerinin azaldığı görüldü. Tartışma: Paralimpik Boccia Sporunun hemipleji tanısı almış bu bireyin, mental ve psikolojik durumunu geliştirdiği ve üst ekstremitenin fonksiyonel kullanımı üzerinde olumlu etkilerinin olduğu ve bu spora tamamlayıcı rehabilitasyon uygulamaları içinde yer verilebileceği sonucuna varılmıştır. Ayrıca sporun bir aktivite olarak kullanımı bu bireylerin sosyal katılımları üzerinde de olumlu etkiler meydana getirecektir., Purpose: This study was carried out to examine the effects of Paralympic Boccia on the functional use of the affected hand, depression, and mental status in an individual with hemiplegia. Material and Methods: Within the scope of the research, a 69-year-old male client with a diagnosis of chronic hemiplegia with ischemic middle cerebral artery involvement, which is in level 3 according to the Eggers classification system, was included in the Disabled Boccia Sports practice in the international Olympics. This individual, who was in stage 2 on the modified Ashworth scale, can reach objects by active elbow extension and flexion and can perform grasping movements with a slight increase in muscle tonus. In the study which was carried out in a nursing home, Geriatric Depression Scale was used to evaluate the depression status of the client, the Jebsen hand function test was used for hand function evaluation, and standard mini-mental test was used for mental status. Paralympic Boccia sport was performed 4 days a week, in 45-minute sessions, for a total of 8 weeks. After the intervention, the tests were repeated. Results: It was observed that the individual had moderate depression before the intervention. A positive improvement was noted as a result of the geriatric depression scale after the intervention. Only a onepoint increase was achieved in terms of mental function. In the Jebsen hand function test, when the scores before and after Boccia Sport were compared, it was observed that the time to complete the tests decreased. Discussion: It has been concluded that Paralympic Boccia Sport has improved the mental and psychological state of this individual diagnosed with hemiplegia and had positive effects on the functional use of the upper extremity, and this sport can be included in complementary rehabilitation practices. In addition, the use of sports as an activity will have positive effects on the social participation of these individuals.

Objective: Mobile applications have been used frequently in post-operative orthopedic rehabilitation in recent years. However, no systematic review has emphasized the importance of mobile applications in the rehabilitation process after total knee arthroplasty (TKA). This systematic review and meta-analysis aimed to evaluate the effectiveness of mobile application-based rehabilitation practices in patients with TKA. Material and Methods: PubMed, Web-of-Science, Scopus, ScienceDirect and Cochrane databases were searched. The Physiotherapy Evidence Database (PEDro) and the Revised Cochrane risk-of-bias tool randomized trials 2 (RoB2) tools were used to demonstrate the methodological quality and risk of bias. Results: A total of 584 articles were screened. Finally, six papers were included in the systematic review. PEDro scores ranged from 4 to 7 (median: 5.5), indicating fair to good methodological quality. All studies were classified as \"some concerns\" in RoB2. Mobile application-based rehabilitation demonstrated better scores on pain, range of motion (ROM), objective and subjective function, satisfaction and compliance in general. Meta-analysis proved that mobile application-based telerehabilitation demonstrated better results on subjective function (ES:0.57, 95% CI: 0.11–1.02). Conclusion: Compared to conventional rehabilitation, application-based telerehabilitation provides more effective results in function, pain and ROM. Furthermore, mobile application-based rehabilitation should also be considered regarding patient satisfaction and compliance.

Objective: Diabetes is a chronic disease whose prevalence increases with age and causes severe damages in all systems of the body. The aim of our study was to assess the effects of proprioceptive exercises on balance and sensory function in the geriatric adults with Type 2 diabetes mellitus. Material and Methods: Forty individuals aged 65 and over with Type 2 diabetes mellitus were divided as a study group (n=20) and as a control group (n=20). All patients were evaluated at the baseline and after 8 weeks training. Balance was mea-sured using Berg Balance Scale and Nintendo Wii system. Balance and gait were evaluated by Tinetti Balance and Gait Test. Semmes-Wein-stein monofilament test was used for assessing tactile sensory, diapa-son for vibration and active matching test for joint position sense. Study group received 45 minutes long proprioceptive exercise training 3 times/week for 8 weeks and the control group only received diabetic patient education. Background Age-related neurodegeneration, sarcopenia, and ectopic fat accumulation are conditions with shared pathways that remain poorly understood. We have measured muscle volume and fat accumulation in masseter and tongue muscle, and aim to explore their association with the total grey matter volume using MRI in older adults recently diagnosed with Alzheimer's disease (AD) and Dementia with Lewy bodies (DLB). Methods In this cross-sectional study, people newly diagnosed with mild AD (n=33) and DLB (n=20) underwent structural head MRI. Muscle volume and intramuscular fat (iMAT) of the tongue and masseter were computed using Slice-O-Matic software for segmentation. Total grey volume and hippocampal volumetric segmentation were performed with the FreeSurfer image analysis suite version 6.0. Independent regression models were employed to analyse the associations. Results Tongue iMAT was higher and total grey volume lower in DLB compared to AD. In the DLB group, tongue muscle was positively associated with total grey matter volume Est 0.92 (SE 0.24 p-value 0.002), left- Est 0.01 (SE 0.0028 p-value 0,002), and right- Est 0.0088 (SE 0.0027 p-value: 0.005) hippocampal volume. There were no statistically significant results for AD. Conclusion Tongue muscle volume was positively associated with hippocampal and total grey volume in DLB. Longitudinal designs are required to explore the extent and significance of this association.

Intisari Latar belakang : Kecenderungan pasien geriatri mengkonsumsi banyak obat menimbulkan resiko timbulnya efek yang tidak diinginkan akibat obat. Salah satu acuan yang dapat digunakan untuk mengevaluasi penggunaan atau peresepan obat pada geriatri adalah Beers Criteria , panduan yang spesifik untuk pengobatan pada populasi geriatri. Tujuan : Penelitian ini bertujuan untuk mengetahui persentase jumlah penggunaan obat berdasar Beers Criteria (Beers List) . Metode : Metode yang digunakan adalah observasional dengan rancangan deskriptif dan pengambilan data dilakukan secara prospektif terhadap resep pasien geriatri rawat inap RSUD dr. Soekardjo. Hasil : Hasil penelitian menunjukkan bahwa 40 pasien (61,0%) dari total 65 pasien geriatri rawat inap menggunakan obat yang tidak tepat berdasarkan Beers Criteria . Dari 40 pasien tersebut terdapat 59 obat yang masuk kedalam Beers Criteria dengan 5 jenis obat yang memiliki rekomendasi dari bukti ilmiah yang kuat untuk dihindari, yaitu kategori 1 digoxin 10 item obat (17,5%), alprazolam 2 item obat (3,5%), ketorolac 8 item obat (14,0%), kategori 2 tramadol 2 item obat (3,5%) dan kategori 3 ranitidin 35 item obat (61,4%). Kesimpulan : Sehingga dapat disimpulkan dari penelitian ini yaitu pada pasien geriatri yang di rawat inap di RSUD dr. Soekardjo terdapat ketidaksesuain pengobatan (PIMs; potentially inappropriate medication ) berdasar Beers Criteria . Kata kunci : Geriatri, Beers Criteria , PIMs, ketidaksesuaian pengobatan. Potentially inappropriate medication among geriatric inpatients Abstract Background : The pathology and physiology in geriatrics tend to make them consume more medications that carry a greater risk of experiencing side effects and adverse drug interactions. One of the references to evaluate drug use or prescription in geriatrics is the Beers Criteria , which describe a new type of basic guidelines, a specific guide to the treatment for geriatric population. Objective : This study aimed to determine the percentage of drug use according to the Beers Criteria. Methods : The method used was observational descriptive design in which data was taken retrospectively from the prescriptions for geriatric inpatients. Results : Forty patients (61.0%) out of a total of 65 identified geriatric inpatients experienced a potentially inappropriate drug use based on the Beers Criteria. In 40 patients, 59 drugs were found to meet the Beers Criteria with 5 types of drugs to be avoided based on the recommendation of strong scientific evidence, including 10 items of Digoxin (17.5%) 2 items of Alprazolam (3.5%), and 8 items of Ketorolac (14.0%) all in category 1, 2 items of Tramadol (3.5%) in category 2, and 35 items of Ranitidine (61, 4%) in category 3. Conclusion : The geriatric inpatients in dr. Soekardjo Hospital experienced PIMs (potentially inappropriate medications) according to the Beers Criteria.

Ketorolak merupakan NSAID yang utamanya dieliminasi melalui ginjal yang membutuhkan penyesuaian dosis pada pasien geriatri dengan penurunan fungsi ginjal. Penelitian ini bertujuan untuk mengetahui profil rasionalitas pendosisan ketorolak, menganalisis hubungan antara rasionalitas pendosisan dengan efektivitas terapi serta kejadian efek samping pada pasien geriatri rawat inap dengan penurunan fungsi ginjal. Penelitian dilakukan dengan rancangan cross-sectional. Pengambilan data secara retrospektif melalui penelusuran rekam medis pasien geriatri rawat inap RSUD Benyamin Guluh periode 2015-2020. Data yang diamati berupa regimen pengobatan, serum kreatinin, efektivitas terapi dan efek samping. Rasionalitas pendosisan dinilai berdasarkan kesesuaian dosis dengan referensi/formula Guisti Hayton. Efektivitas terapi tercapai jika penurunan VAS 0,05) maupun kejadian efek samping.

Çalışmanın temel amacı Muratpaşa merkezinde yaşayan 65 yaş üstü bireylerin ikamet ettikleri konutların fiziksel uygunluğunu incelemek ve konutlarında onları sınırlayan yapısal özellikleri ve nedenlerini tespit etmek, yerel yönetimler ve kent sakinleri için konutlar açısından Demans Dostu Çevre oluşturma sürecinde bir yol haritası oluşturmaya katkı sunmaktır.Çalışma kapsamında, birey-mekân uyumunu ele almak üzere tasarlanan araştırma modelini uygulamak için `Yaş Dostu Kentlere Doğru: Hızla Yaşlanan Bir Toplumda Yaş Dostu Çevreler ve Hizmetler Yaratmak` araştırmasının mikro verileri kullanılarak ikincil veri analizi gerçekleştirilmiştir.İkincil veri analizi kapsamında, Muratpaşa ilçesinde ikamet eden yaşlıların konut içi alanlarının ve genel konut alanın risk değerlendirmeleri yapılmış ve mahallelerin nüfus yoğunluklarına göre demans dostu çevre bağlamında konut-içi erişilebilirlik üzerine risk haritalandırmaları gerçekleştirilmiştir. Birey- konut ilişkisi bağlamında, bireylerin hanehalkı yapıları ve demans risk durumlarına dayalı olarak konut riskleri arasındaki ilişki incelenmiştir.Yapılan analizler ile kent merkezi içerisindeki konut alanlarının demans dostu çevre değerlendirmesi sonucunda riskleri barındırdığı tespit edilmiştir. Türkiye'de özellikle yerinde yaşlanma kavramı önerileri sosyo-politik düzlemde sıklıkla gündeme getirilirken yaşlı bireylerin var olan koşullarına ilişkin bir değerlendirme yapılmamaktadır. Özellikle demans gibi yalnızca bireyi değil aileyi ve toplumu etkileyen hastalıklarda da yerinde yaşlanmanın önerilebilir olması için bireylerin ilk engel ile karşılaştıkları konutlarının erişilebilirliğinin sağlanması önemlidir. Kapsayıcı tasarım ilkelerine bağlı kalınarak kent alanlarının iyileştirilmesi ve kilit hizmetlere erişimin kolaylaştırılması gerekmektedir. The main aim of the study is to examine the physical accessibility of housing, to determine structural features and causes of the limiting older adults in their homes for individual's living in the center of Muratpaşa district and to contribute to create a road map in the process of creating Dementia Friendly Environment for local governments (municipalities) and citizens.Within this objective, secondary data analysis has been performed by using micro data of the research titled `Establishing Age Friendly Environments and Services in a Rapidly Aging Society` to address the person-environment fit model for testing the empirical model.Based on the secondary data analysis, risk assessments of each component of houses were analysed. According to the population densities of the neighborhoods, risk mapping was conducted on the accessibility of houses in the context of dementiafriendly environment. In the context of the person-environment fit model, the relationship between the risk of environment and characteristics of person's household structures and dementia risk situations was examined.With analyzes, it has been determined that the residential areas within the city center contain risks as a result of dementia-friendly environmental assessment. In Turkey, when the concept of aging in place discussing in the socio-political level, usually ignored conditions of the older adults. Especially for dementia that is not only effect the individual but also the household and society, it is necessary to ensure accessibility of the houses where individuals face the first obstacle in order to be able to offer aging in place. Amaç: Yerelleşme politikaları çerçevesinde yaşlılığa yönelik yerel politikaların değerlendirilmesi, uygulamada karşılaşılan sorunlar/olanakların ortaya konması ve yerel yönetimlerde yaşlı bakım hizmetlerine yönelik önerilerin geliştirilmesi amacıyla yapılmıştır.Yöntem: Araştırmada, nitel araştırma metoduna dayalı bir yöntem izlenmiştir. Araştırmanın yöntemi, uygulamalı gerontolojiye dayalı olarak sosyal politika yönelimli betimleyici durum araştırmasıdır. Araştırma Antalya iline bağlı 19 ilçe belediyesiyle yapılmıştır. İlçelerin tamamı araştırmaya dâhil edilmiştir. Bulguların analizinde betimleyici nitel analiz yöntemi kullanılmıştır.Bulgular: Araştırma sonuçları 19 ilçeden 11'inin yaşlılara yönelik gerontolojik bakım hizmetinin olduğunu, sekizinin ise yaşlılara yönelik herhangi bir hizmetinin olmadığını göstermiştir. Belediyelerin gerontolojik bakım hizmetlerinin Kurum Hizmeti, Evde Bakım Hizmeti ve Evde Sağlık Hizmeti adı altında toplandığı görülmüştür. İlçe belediyeleri gerontolojik hizmetlere ilişkin genel olarak organizasyon, denetim, alt yapı, donanım eksikliği, bütçe yetersizliği gibi sorunlar yaşamaktadır.Sonuç: İlçe belediyelerinin genelini hizmetleri profesyonel olarak yürütmediği ve yaşlı çalışmaları konusunda fikir olarak desteğe ve yönlendirmelere ihtiyaçları olduğu anlaşılmıştır.Anahtar Kelimeler: yaşlılık, bakım hizmetleri, yerel yönetimler. Objective: In the framework of localization policies, evaluation of local policies towards aging has been done with the aim of developing suggestions for elderly care services in local administrations by revealing the problems / possibilities encountered in practice.Methods: In the research, a method based on qualitative research method was followed. The method of the research is a social policy oriented descriptive case study based on applied gerontology. The study was conducted with 19 district municipalities of Antalya province. All districts were included in the study. In the analysis of the findings, descriptive qualitative analysis method was used.Results: The results of the study showed that 11 out of 19 districts had gerontological care services for elderly people and eight of them did not have any services for the elderly. It was seen that gerontological care services of municipalities were gathered under the name of Corporate Service, Home Care Service and Home Health Service. District municipalities generally face problems such as organization, supervision, infrastructure, lack of equipment and lack of budget.Conclusion: It is understood that the district municipalities do not carry out the services professionally in general and they need support and guidance as an idea about the elderly work.KeyWords: oldage, care services, local governments. 105

Yaşı ilerledikçe çoğalan yaşam tecrübeleri göz önüne alındığında, insanın ömür boyu öğrenen bir varlık olduğu söylenebilir.Araştırmada Tazelenme Üniversitesi öğrencilerinin aldıkları eğitim ile yaşam durumları arasındaki ilişki tespit edilmeye çalışılmıştır. Tazelenme Üniversitesine kayıtlı öğrencilerin yaşamındaki değişiklikler, yaşam boyu öğrenme kavramının uygulanışı, yeni öğrendikleri bilgileri ile hayatlarında neler değiştiği yada değişmediği incelenmiştir.Araştırma nicel araştırma tekniği ile projeye katılanlar arasından seçilen deneklerin, araştırmacının kendisi tarafından geliştirilen standart soru kağıdına verdikleri cevaplarla değerlendirilmiştir.Yaşam boyu öğrenme kapsamında ülkemizde yeni bir model olan Tazelenme Üniversitelerinin topluma katkısı ve işleyişi konusunda çalışılmış ve bu alanda ilk olma niteliği taşıyan bilgiler edinilmiştir.Anahtar Kelimeler: Yaşam Boyu Öğrenme, Tazelenme Üniversitesi, Gerontoloji, Eğitim, Öğrenme. At every age, there is potential for development. At every age, one's behavior and experiences can change. This can be brought about by an individual orinitiated by others through intervention (Tufan, 2017).There is therefore no such thing as a decline in life. Some losses occur during the life course; at the same time new benefits are also gained. For example, the accumulation of experience over lifetimes and the connection between life experience and wisdom can be found in nearly all cultures. When considering the ever-growing life experience that comes as one ages, it can be said that the human is a being that continues to learn for all of life.This research attempted to determine the relationship between the education that students received at Refreshment University and their life situations. Changes in the lives of the registered students at Refreshment University, their application of the concept of lifelong learning, new information learned and details of their life situation that changed or did not change were examined.Using qualitative research techniques, the researcher developed standardized questions that were then asked to research subjects selected from project participants; their answers were recorded and evaluated.Refreshment University, a new model in our country for lifelong education, has been studied in relation to its functioning and contributions to society. BACKGROUND: The purpose of this study was to evaluate the etiologic and prognostic factors of open eye injuries in geriatric patients in the Southeastern region of Anatolia. METHODS: Forty-five geriatric patients who underwent surgery for an open eye injury in our clinic between the years of 2008 and 2012 were evaluated retrospectively. Age, gender, cause and the mechanism of the trauma, visual acuity (VA), and the time between the trauma and the surgery were obtained from files and evaluated. RESULTS: The mean age of the patients was 70.4±8.2 (65-90) years. Thirty-four of the cases were male and 11 were female. The most frequent mechanism of trauma was a wood strike, while the second most common one was injury with a knife. Corneoscleral penetration was the most frequently observed trauma. The mean VA of the patients was 2.26±0.65 at admission, and was 1.53±0.99 logarithm of the minimum angle of resolution at the final evaluation. The most frequent complications of trauma were iris prolapse and hyphema. There was a significant correlation between the first and final VA. CONCLUSION: Penetrating ocular injuries are seen less frequently among geriatric patients, and their prognosis may be worse due to less-efficient wound site healing and differences in scleral rigidity. The most important factor affecting the final VA measurement was the VA of the patient at admission. AMAÇ: Güneydoğu Anadolu Bölgesi’nde yaşayan geriatrik hastalarda açık göz yaralanmasının etiyolojik faktörleri ve prognozu etkileyen faktörleri irdelemek. GEREÇ VE YÖNTEM: 2008-2012 yılları arasında kliniğimizde açık göz yaralanması nedeni ile ameliyat edilen ve takibi yapılan 65 yaş ve üstü 45 olgu geriye dönük olarak değerlendirildi. Dosya bilgilerinden olguların yaşı, cinsiyeti, travmanın oluş nedeni ve şekli, görme derecesi, travmanın oluş zamanı ile cerrahi arasında geçen süre değerlendirildi. BULGULAR: Hastaların ortalama yaşı 70.4±8.2 (65-90) yıl idi. Olguların %75.6’sı (n=34) erkek ve %24.4’ü (n=11) kadın idi. Travmanın oluş şekli en sık olarak odun çarpması ve ikinci sıklıkta bıçak değmesi olarak tespit edildi. Korneaskleral penetrasyon en sık olarak izlendi. Görme keskinlikleri müracaat anında 2.26±0.65 logMAR ölçüldü. Travmaya en sık eşlik eden komplikasyon iris prolapsusu ve hifema olarak izlendi. İlk görme keskinliği ile nihai görme keskinliği arasında korelasyon izlendi. TARTIŞMA: Geriatrik olgularda oküler penetran yaralanmalar daha az görülmekle beraber yara yeri iyileşmesi zorluğu ve değişen skleral rijidite nedeniyle daha kötü prognozlu olabileceği düşünüldü. Nihai görme keskinliğini etkileyen en önemli faktörün hastanın müracaat anındaki görme keskinliği olduğu görüldü.

Background Although there is evidence that tailored implementation strategies can be effective, there is little evidence on which methods of tailoring improve the effect. We designed and evaluated five tailored programs (TPs) each consisting of various strategies. The aim of this study was to examine (a) how determinants of practice prioritized in the design phase of the TPs were perceived by health care professionals who had been exposed to the TPs and whether they suggested other important determinants of practice and (b) how professionals used the offered strategies and whether they suggested other strategies that might have been more effective. Methods We conducted a mixed-method process evaluation linked to five cluster-randomized trials carried out in five European countries to implement recommendations for five chronic conditions in primary care settings. The five TPs used a total of 28 strategies which aimed to address 38 determinants of practice. Interviews of professionals in the intervention groups and a survey of professionals in the intervention and control groups were performed. Data collection was conducted by each research team in the respective national language. The interview data were first analyzed inductively by each research team, and subsequently, a meta-synthesis was conducted. The survey was analyzed descriptively. Results We conducted 71 interviews; 125 professionals completed the survey. The survey showed that 76 % (n = 29) of targeted determinants of practice were perceived as relevant and 95 % (n = 36) as being modified by the implementation interventions by 66 to 100 % of professionals. On average, 47 % of professionals reported using the strategies and 51 % considered them helpful, albeit with substantial variance between countries and strategies. In the interviews, 89 determinants of practice were identified, of which 70 % (n = 62) had been identified and 45 % (n = 40) had been prioritized in the design phase. The interviewees suggested 65 additional strategies, of which 54 % (n = 35) had been identified and 20 % (n = 13) had been prioritized, but not selected in the final programs. Conclusions This study largely confirmed the perceived relevance of the targeted determinants of practice. This contrasts with the fact that no impact of the trials on the implementation of the recommendations could be observed. The findings suggest that better methods for prioritization of determinants and strategies are needed. Trial registration Each of the five trials was registered separately in recognized trial registries. Details are given in the respective trial outcome papers. Studiens bakgrunn: Behovet for kompetent fagpersonell er økende innen hele det geriatriske feltet. Kvalifiserte og dyktige sykepleiere på sykehjem er avgjørende for en fremtidig god omsorg for skrøpelige gamle. Det foreligger relativt lite forskning om hva erfarne sykepleiere i norske sykehjem anser som meningsfullt i arbeidet. Hensikt: Hensikten med studien var å undersøke hva erfarne sykepleiere på sykehjem oppfatter som særlig meningsfylt i arbeidet. Metode: Studiens design er beskrivende og fortolkende. Åtte sykepleiere uten videreutdanning, med minimum fem års arbeidserfaring fra sykehjem ble intervjuet. Resultater: Sykepleierne ga uttrykk for at de hadde et svært meningsfylt arbeid. Særlig ble muligheten for nærhet og langvarig kontakt med pasientene og deres pårørende fremhevet som positive sider ved arbeidet. Morgenstellet ble fremholdt som en særdeles viktig anledning til samhandling med pasienten, til observasjon, lindring og til å skape velvære. Mulighetene sykehjemsarbeidet gir for et variert arbeid, til spesialisering og individuell utforming av yrkesrollen ble høyt verdsatt og erfares som meningsfulle sider ved arbeidet. Konklusjon: Utvikling og oppbygging av fagkompetanse i tråd med egeninteresser ansees som særlig meningsfullt. Det kan forventes ytterligere spesialisering av sykepleierollen på sykehjem i framtiden. Denne studien gir grunn til å anta at dette er en utvikling sykepleiere vil oppleve som positiv.

Bakgrunn: Samhandling i helse- og omsorgstjenesten har vært en utfordring i mange år, både nasjonalt og internasjonalt, og er fremdeles en utfordring i videreutvikling av helsetjenesten. En alderspsykiatrisk sykehusavdeling tok i 2010 initiativ til utvikling av et aksjonsforskningsprosjekt mellom sykehuset og 5 sykehjem. Ett av tiltakene i aksjonsforskningsprosjektet har vært å gjennomføre et tverrfaglig samhandlingsmøte på aktuelle sykehjem det har vært en utskrivelse til etter to uker. Hensikt: Studiens hensikt er å undersøke tverrfaglig helsepersonells erfaringer fra deltakelse på samhandlingsmøtene. Teoretisk forankring: Studien tar utgangspunkt i faglitteratur og tidligere forskning om samarbeid og samhandling mellom tverrfaglig helsepersonell fra ulike tjenestesteder. Metode: Studien har en beskrivende design og anvender kvalitativ metode med tematisk innholdsanalyse. Studien er en del av et større medvirkningsbasert aksjonsforskningsprosjekt, hvor data er samlet inn ved hjelp av selvrapporteringsskjema (loggskjema). Resultat: Hovedfunnet i studien er at samhandlingsmøte er et møte ansikt til ansikt mellom tverrfaglig og tverretatlig helsepersonell, som bidrar til gjensidighet, fellesskap og utvidet forståelse som kan føre til nye handlinger. Konklusjon: Den detaljerte informasjons- og erfaringsutvekslingen som foregår mellom tverrfaglig helsepersonell i samhandlingsmøte bidrar til utvidet forståelse for den enkelte person. Helsepersonell lærer av hverandre gjennom å dele av sin erfarings- og kunnskapsbase. Sammen utvikler helsepersonell nye individuelle personsentrerte tiltak som kan bidra til at personen beholder sin identitet og verdighet. Erfarings-, kunnskaps- og informasjonsutvekslingen kan bidra til å styrke kontinuiteten i tjenestene i overgangen mellom alderspsykiatrisk sykehus avdeling og sykehjem.

Bu çalışma, geriatrik yaş gruplarında su içi ve kara egzersizlerinin fonksiyonel kapasite ve yaşam kalitesine etkisini karşılaştırmak amacıyla yapıldı. Çalışmaya 31 geriatrik yaş grubunda birey katıldı. Katılımcılar kendi istekleri doğrultusunda 15 kişilik havuz egzersiz grubu (grup1) ve 16 kişilik kara egzersiz grubu (grup2) olarak ikiye ayrıldı. Çalışmanın ilk haftasında grup 2'den bir katılımcı egzersizleri yapamayacağını söyleyerek ayrıldı. Her iki grup için haftada 3 günden oluşan 6 haftalık egzersiz programları belirlendi. Egzersiz programları öncesinde ve sonrasında tüm bireylere fonksiyonel açıdan değerlendirme testleri ve yaşam kalitesi anketi uygulandı. Yapılan değerlendirmelerde geriatrik yaş gruplarına, Whoquol – old yaşam kalitesi anketi, fonksiyonel erişme testi (FRI), gözü açık - kapalı tek ayak üstünde durma, time up and go (TUG), borg skalası, kas testi ve 6 dk. yürüme testleri yapıldı. Banyaknya obat yang dikonsumsi oleh pasien geriatri dengan penyakit kronis meningkatkan penurunan kepatuhan. Ada beberapa cara dalam meningkatkan kepatuhan diantaranya dengan menggunakan Pill box dan Medication reminder chart. Penelitian ini bertujuan untuk mengetahui perbandingan efektifitas penggunaan Pill box dengan M edication reminder chart dalam meningkatkan kepatuhan pasien geriatri dengan hipertensi di Kota Batam. Rancangan Penelitian ini adalah studi experimental; Randomized Controlled Trial dengan desain penelitian pre test dan post test control group. Pengambilan sampel pada penelitian ini dilakukan dengan teknik random alokasi sampling. Pasien akan dikelompokkan menjadi dua kelompok subjek yang berbeda, yaitu kelompok yang mendapat intervensi berupa Pill box dan Medication reminder chart . Data dikumpulkan secara prospektif sesuai dengan kriteria inklusi padap asien geritri rawat jalan, dengan diagnosa penyakit hipertensi di dua rumah sakit Kota Batam, penelitian dilakukan pada bulan Januari sampai April 2016. Pengukuran tingkat kepatuhan menggunakan kuesioner MMAS-8 dan pill count dan outcome klinik pasien diukur dengan melihat tekanan darah sistolik dan diastolik yang disesuaikan dengan diagnosa dokter yang tertera di rekam medik. Pengukuran dilakukan sebelum dan sesudah perlakuan. Data dianalisis secara statistik menggunakan deskripitif, uji bivariat . Hasil penelitian ini menunjukkan bahwa Pill box dapat meningkatkan kepatuhan ( P= 0,000) dan menurunkan tekanan darah sistolik dan diastolik secara signifikan ( P= 0,000 dan P= 0,002), sedangkan pada Medication reminder chart hanya efektif dalam meningkatkan kepatuhan ( P= 0,000). Terdapat perbedaan yang signifikan antara Pill box dan Medication reminder chart dalam meningkatkan kepatuhan ( P= 0,008) dan pada penurunan tekanan darah sistolik dan diastolik ( P= 0,006 dan P= 0,016).

Miksoma odontogenik tipe periferal adalah miksoma yang berlokasi pada jaringan lunak, tumbuh lambat, kurang agresif dan mempunyai tingkat rekurensi yang rendah dibandingkan dengan miksoma odontogenik tipe sentral. Miksoma odontogenik periferal bisa terjadi pada setiap dekade kehidupan, paling banyak terjadi pada dekade keempat. Tujuan penulisan studi kasus ini adalah melaporkan keberhasilan eksisi miksoma odontogenik periferal yang terjadi pada pasien geriatri pasca stroke non hemoragik. Seorang pasien wanita usia 74 tahun, terdapat benjolan di gingiva rahang atas kiri, timbul sejak 3 bulan yang lalu, tidak sakit, tidak mudah berdarah, tetapi mengganggu pengunyahan. Pasien memiliki riwayat stroke non hemoragik yang terkontrol. Eksisi lesi dan kuretase tulang dilakukan dengan anestesi umum. Hasil pasca operasi, setelah dilakukan follow-up selama 6 bulan, tidak ada keluhan pasien terkait dengan penyakit yang diderita, tidak dehisensi, tidak kambuh dan prognosis baik. Eksisi miksoma odontogenik periferal yang terjadi pada pasien geriatri pasca stroke non hemoragik bisa dilakukan, tetapi harus dengan persiapan perioperatif yang optimal, meliputi konsultasi ke bagian neurologi, kardiologi dan rawat bersama dengan bagian penyakit dalam sub bagian geriatri, untuk meminimalkan interaksi obat-obatan yang diberikan dan mencegah komplikasi pasca operasi. ABSTRACT: Excision Peripheral Odontogenic Myxoma of the Maxillary Sinistra on Post Stroke Non-Hemorrhagic Patients under General Anesthesia. Peripheral odontogenic myxoma is a myxoma located on soft tissue, growing slowly, less aggressive and owning low recurrency rate compared to central odontogenic myxoma. Peripheral odontogenic myxoma may occur in every decade of life, mostly in the fourth decade. The aim of this report is to expose the successful excision of peripheral odontogenic myxoma on post non-hemorrhagic stroke of geriatric patient. A seventy-four-year-old woman had a pedunculated mass on the left maxillary gingivitis. It had been growing for 3 months, non-tender, non-bleeding but causing chewing inconvenience. She had a controlled non-hemorrhage stroke. An excision of lesion and bone curettage was conducted under general anesthesia. Six months after the operation, the follow-up showed no further complaints concerning her disease, no dehiscence, no recurrence, and the prognosis was good. It can be concluded that the excision of peripheral odontogenic myxoma on post non-hemorrhagic stroke of geriatric patients was feasible. However, it must be conducted under adequate perioperative preparation, which consists of neurology and cardiology consultation and joint treatment between internal department and geriatric sub-department to minimize drug interaction and to prevent post-operative complication.

Deliryum, akut başlangıçlı ve değişmiş mental durum ile karakterize bir klinik sendromdur. Deliryum hastaneye yatırılan yaşlıların % 14- 56’sında görülür. Bu yazıda, hipokalsemiye bağlı deliryum gelişen 78 yaşında bir kadın olgu sunuldu ve konu güncel literatür bilgileri ile tartışıldı, Delirium is a neuropsychiatric syndrome characterized by the acute onset and fluctuating course of globally altered mental status. Delirium occurs in 14–56% of elderly hospitalized medical patients. In this case, 78 year-old female in a delirium state due to hypocalcemia discussed by the current literature.

INTRODUKTION: Akut hoftenær fraktur er hos ældre i plejebolig en hyppig årsag til indlæggelse. Udskrivning foregår få dage efter operation med forøget risiko for død. MATERIALE OG METODER: I et followupdesign undersøges 211 ældre fra plejebolig, der havde været indlagt til operation af hoftenær fraktur. Geriatrisk Team på Ortopædkirurgisk Afdeling besøgte den ældre senest dagen efter udskrivning. Antallet af opfølgende besøg afhang af behov for behandling og rehabilitering. Interventionen fokuserede på ernæring, væsketerapi og blodtransfusion. RESULTATER: Intensiveret geriatrisk intervention i plejebolig reducerede tredivedages- mortaliten (oddsratio (OR) = 0,08; 95% konfidensinterval (KI): 0,01; 0,52) og reducerede tremåneders- mortaliten (OR = 0,04; 95% KI: 0,004; 0,47). Væske terapi og kosttilskud var associeret med en reduceret Ugeskr Læger 172/25 21. juni 2010 VIDENSKAB 1903 sandsynlighed for død (OR = 0,12; 95% KI: 0,02; 0,67)/(OR = 0,23; 95% KI: 0,07; 0,79). Akut genindlæggelse blev reduceret (OR = 0,20; 95% KI: 0,04;0,91). Postoperativ blodtransfusion var associeret med forbedret funktionsniveau (β = 8,22; 95% KI: 1,44; 15,0). KONKLUSION: Tværfaglig geriatrisk intervention havde en positiv effekt hos ældre i plejebolig og forøgede overlevelsen inden for de første tre måneder efter indlæggelse samt reducerede akut genindlæggelse inden for samme periode. Der er endnu usikkerhed omkring effekten af væsketerapi og blodtransfusion i plejebolig, hvilket kræver yderligere undersøgelser.

Introduktion: Antallet af hoftefrakturer er stigende hos de 85+- årige, og det medfører et stort sengedagsforbrug. Årsagen er multifaktorielt betinget og skyldes oftest faldulykker og osteoporose. Materiale og metoder: Geriatrisk Teamfunktion på Ortopædkirurgisk Afdeling, geriatrisk rehabilitering samt følge-hjem-funktion (GO) sammenlignes med traditionel ortopædkirurgisk behandling (ORT) hos 65+-årige hoftepatienter hvad angår: hæmoglobindifference præoperativt og tre til seks måneder efter fraktur, iværksat osteoporosebehandling, indlæggelsestid, boligændring, genindlæggelser og død inden for seks måneder samt incidens af ny fraktur inden for to år. Resultater: Den mediane indlæggelsestid blev reduceret fra 15 til 13 dage. Signifikant flere påbegyndte behandling med calcium m/D-vitamin og bisfosfonat, og der sås en tendens til en reduceret risiko for ny fraktur inden for to år (incidensrateratio (IRR) = 0,72 [95% konfidensinterval (KI): 0,39-1,32]). Der var ingen reduktion i genindlæggelser (oddsratio (OR) = 1,09 [95% KI:0,71-1,67]) og mortalitet (OR = 1,22 [95% KI: 0,71-2,11]). Konklusion: En kombineret geriatrisk ortopædkirurgisk indsats over for ældre patienter med hoftefraktur synes at øge såvel kvalitet som effektivitet i patientforløbene. Konceptet kan med fordel videreudvikles og kombineres med andre tiltag, der er rettet mod at accelerere patientforløb.
