Abstract

Vaccination remains key to reducing the risk of COVID-19–related severe illness and death. Because of historic medical exclusion and barriers to access, Black communities have had lower rates of COVID-19 vaccination than White communities. We describe the efforts of an academic medical institution to implement community-based COVID-19 vaccine clinics in medically underserved neighborhoods in Philadelphia, Pennsylvania. Over a 13-month period (April 2021–April 2022), the initiative delivered 9038 vaccine doses to community members, a majority of whom (57%) identified as Black. 5. Gemcitabine-induced digital ischaemia in a patient with metastatic breast cancer

Abstract

A woman in her 50s with HER2 (human epidermal growth factor receptor 2) positive, estrogen/progesterone receptor negative, metastatic invasive ductal carcinoma of the breast, presented with acral cyanosis and severe throbbing pain after recent administration of gemcitabine. She was treated with aspirin, heparin, amlodipine, topical nitroglycerin and analgesics. Gemcitabine was discontinued permanently. She had a gradual recovery except for a small necrotic area over the right 4th digit. However, surgical intervention was avoided.

Abstract

Adrenocortical carcinoma (ACC) is a rare form of cancer with an annual incidence of two per million. The risk of venous thromboembolism increases sevenfold in patients with cancer. This case report describes an iliac vein deep vein thrombosis (DVT) as an atypical presentation of an ACC and highlights the value of further imaging investigation in patients with unprovoked DVTs.

Abstract: Oral disintegrating tablets (ODT) offer an attractive choice for Gastroesophageal Reflux Disease (GERD) patients suffering from dysphagia. In chronic condition, GERD patient suffers from severe erosive esophagitis. Thus patients feel difficulty and pain during swallowing, which results in patient in-compliance toward medication of tablets or capsules- especially in geriatrics and pediatric patients. These symptoms of GERD patients have attracted the formulation scientists in improving the formulation methodology for such patients. Orally disintegrating tablets could increase the therapeutic impact and drug compliance in these patients. The aim of this compilation is to provide a more convenient way to develop an oral disintegrating drug delivery system of proton pump inhibitors in patients suffering from odynophagia, associated with chronic Gastroesophageal Reflux Disease (GERD). Oral disintegrating tablets (ODT), when placed on the tongue, can quickly disintegrate and release the medicament. It later dissolves or disperses in saliva without any additional water. The saliva containing drug can easily be swallowed and descends into the stomach leading to maximum absorption from the mouth, throat, and upper esophagus. The patient compliance and bio-availability of Oral disintegrating tablets (ODT) are high compared to other conventional tablets.

19. The Prevalence of Blood Borne Diseases in Blood Donors of Peshawar, Khyber Pakhtunkhwa, Pakistan

Abstract

Background: The goal of the study was to investigate the burden of transfusion- transmitted infections (TTIs) hepatitis B virus (HBV), hepatitis C virus (HCV), human immunodeficiency virus (HIV), syphilis, and malarial parasite (MP) in ABO Blood Groups and Rh Type System among voluntarily blood donors in Khyber Pakhtunkhwa (KPK), Pakistan. It is a retrospective single center cross sectional study. This study was conducted from June 2020 to September 2021 (16 months) at the frontier foundation thalassemia center Peshawar KPK. Donors were physically healthy and fit for donation. Donors with physical disabilities and/or having co-morbid conditions were excluded from the report. Methods: All the samples were screened for anti-HIV, anti-HCV, HBsAg, Syphilis, and Malarial Parasite via ELISA kit and Immune Chromatographic Technique (ICT), respectively. A total of 6311 blood donations were evaluated. The majority of the donations (92%) were from (VNRBD) voluntary non-remunerated blood donation, while only 8% came from replacement donors. Results: Amongst 6311 blood donations, 1.50 % (n = 95) were infected at least with one pathogen, HBV positive cases were 0.855 % (n = 54), HCV positive cases were 0.316% (n = 20), syphilis positive were 0.30% (n = 19) and MP positive cases were only 0.031% (n = 2). HBV, HCV, syphilis and malaria infections rates were found to be low as compared to the previous data published, while no case was reported for HIV. The study also revealed the distribution pattern of the aforementioned pathogens in blood groups and the Rh type system of the reactive samples. Conclusion: The lower reported in our study indicates the awareness among the people of Peshawar about TTIs and their precautions. The prevalence rate that we are reporting is less than previously published articles in the same domain.

Abstract

Introduction Acute kidney injury (AKI) has been identified as one of the most common and significant problems in hospitalized patients with COVID-19. However, studies examining the relationship between COVID-19 and AKI in low- and low-middle income countries (LLMIC) are lacking. Given that AKI is known to carry a higher mortality rate in these countries, it is important to understand differences in this population. Methods This prospective, observational study examines the AKI incidence and characteristics of 32,210 patients with COVID-19 from 49 countries across all income levels who were admitted to an intensive care unit during their hospital stay. Results Among patients with COVID-19 admitted to the intensive care unit (ICU), AKI incidence was highest in patients in LLMIC, followed by patients in upper-middle income countries (UMIC) and high-income countries (HIC) (53%, 38%, and 30%, respectively), whereas dialysis rates were lowest among patients with AKI from LLMIC and highest among those from HIC (27% vs. 45%). Patients with AKI in LLMIC had the largest proportion of community-acquired AKI (CA-AKI) and highest rate of in-hospital death (79% vs. 54% in HIC and 66% in UMIC). The association between AKI, being from LLMIC and in-hospital death persisted even after adjusting for disease severity. Conclusions AKI is a particularly devastating complication of COVID-19 among patients from poorer nations where the gaps in accessibility and quality of healthcare delivery have a major impact on patient outcomes.

Abstract

This paper intends to examine public secondary school head teachers competencies and their influence on teacher’s instructional decisions as well the overall school culture. This research paper endeavor to unravel secondary school principal’s managerial competencies and their influence on the performance of secondary school principals and such influence will help to stimulate and transform sophisticated school environment that will make strengthen student’s cognitive, affective as well psychomotor domains. Current study will cover following objective: “To determine the effectiveness of head teacher’s managerial skills on teacher’s academic performance at secondary school level”. Research design utilized for this study is of descriptive qualitative nature. Respondents of the study were secondary school principals/head Masters and secondary school teachers. Population of this research composed of all the 83 females’ secondary and 37 higher secondary schools. Sample of the study extracted from the population were 60 principals/ head masters and 120 secondary school teachers. Through proper consultation the researchers developed data collection instrument titled: “Principals’ Managerial Skills Scale”. Mean, Standard deviation was employed as a statistical data analysis tools. Data analysis was followed by conclusion to render realistic recommendations for the establishment of sophisticated school management.

29. Modified-ZnO-mediated dye detoxification by a heterogeneous Fenton process

Abstract

The continuous and unchecked discharge of effluents by the dyeing industry into water bodies has led to the rising importance of water treatment. This study focuses on the successful abatement of the auramine O (AM) dye in an aqueous system through a heterogeneous Fenton process using ultrasonication. Zinc oxide (ZnO) nanoparticles (NPs) were obtained by using a straightforward precipitation method and then modified with glycine (M-ZnO). The NP size sharply decreased upon modification with glycine. X-ray diffraction was used to determine the basic crystal properties, such as crystallinity. It was observed that the size of NPs sharply decreased from 24.6 nm (pure zinc oxide) to 17.3 nm upon modification of zinc oxide with glycine. A bandgap of 3.4 eV was determined for M-ZnO using a UV–visible spectrophotometer. Thermal gravimetric analysis revealed the excellent thermal stability of M-ZnO compared with that of zinc oxide. The catalytic activity for AM abatement was determined using a Fenton process at pH 6. Detoxification assays showed that pure hydrogen peroxide (H2O2) and pure zinc oxide exhibited 38 and 37% AM dye degradation, respectively. Meanwhile, M-ZnO showed 97% AM dye degradation in 240 min. A plausible mechanism is proposed for AM abatement using zinc oxide.

Abstract

BACKGROUND: Evidence of the effectiveness of the WHO-recommended design of longer individualized regimens for multidrug- or rifampicin-resistant TB (MDR/RR-TB) is limited.OBJECTIVES: To report end-of-treatment outcomes for MDR/RR-TB patients from a 2015–2018 multi-country cohort that received a regimen consistent with current 2022 WHO updated recommendations and describe the complexities of comparing regimens.METHODS: We analyzed a subset of participants from the endTB Observational Study who initiated a longer MDR/RR-TB regimen that was consistent with subsequent 2022 WHO guidance on regimen design for longer treatments. We excluded individuals who received an injectable agent or who received fewer than four likely effective drugs.RESULTS: Of the 759 participants analyzed, 607 (80.0%, 95% CI 77.0–82.7) experienced successful end-of-treatment outcomes. The frequency of success was high across groups, whether stratified on number of Group A drugs or fluoroquinolone resistance, and ranged from 72.1% to 90.0%. Regimens were highly variable regarding composition and the duration of individual drugs.CONCLUSIONS: Longer, all-oral, individualized regimens that were consistent with 2022 WHO guidance on regimen design had high frequencies of treatment success. Heterogeneous regimen compositions and drug durations precluded meaningful comparisons. Future research should examine which combinations of drugs maximize safety/tolerability and effectiveness.

39. ROLE OF ENTREPRENEURIAL PASSION IN LINKING ENTREPRENEURIAL EDUCATION AND ENTREPRENEURIAL INTENTIONS

Abstract

This study aims to explore the influence of entrepreneurial passion in linking entrepreneurial education and intentions in the higher education institutions (HEIs) as entrepreneurial passion is positive intense spirits that entrepreneurs experience toward their work to ensure creativity and innovation. Literature revealed that entrepreneurial education plays a crucial role in promoting entrepreneurial intentions in HEIs among the students. Still, the link between entrepreneurial intentions and education is stronger for students who have higher entrepreneurial passions. Present study used mixed-method approach by obtaining data from surveys in HEIs. The results indicated that students who have higher entrepreneurial passions are more possible to have higher entrepreneurial intentions. Students who have taken more entrepreneurial courses report higher levels of entrepreneurial intentions, suggesting that entrepreneurial education play crucial role in promoting entrepreneurial intentions among students. The HEIs should, design courses and programs that promote entrepreneurial passion, by providing theoretical knowledge and practical skills. This may help to create a culture of entrepreneurship in HEIs but also help to promote the development of successful entrepreneurs in the future.

41. GREEN TRANSFORMATIONAL LEADERSHIP & GREEN PERFORMANCE: GREEN MINDFULNESS AS MEDIATOR

Abstract

The current study aims to investigate dynamic relationship between green transformational leadership (GTFRS), Green Performance (GPRF) as well as Mediation Effect of green mindfulness (GMFN) among employees working in USTB, Bannu Medical College and new Green and clean Bannu Project initiative teams in Bannu region. For this purpose, data was collected from 200” project managers/executives in target area and obtained data was entered into SPSS to examine the hypothesized relationship. Findings depict that beta value for the Green Transformation leadership is (B=.484) which confirm that one percent increase in GTRFS will leads to improve (48%) of Project Performance. Further, it was evident from regression analysis that GTRFS has a positive and significant effect on GMFN. Finally, the mediation results evident that Beta value for GTRFS reduced from (β=.462 to β = 0.366) and results in 2nd model also denote significant for GTL after adding GM, that confirm the partial meditational role of GM between employees GTL capabilities and project performance that helps in offering certain valuable recommendations.

Abstract

Groundnut (or peanut, earthnut, monkey nut) is taxonomically classified as Arachis hypogaea and it belongs to family Fabaceae (Leguminosae). In Pakistan, the groundnut is mainly cultivated in rain fed areas, mostly in Punjab, also in some areas of Sindh and North-West Frontier Province. A field experiment was carried out at the Horticultural Research area, Sindh Agriculture University, Tandojam during the year 2021. Two ground nut cultivarrs (“Golden” and “Bari-2011”) were used and sulphate of potash (SOP) was used @ 60, 80, 100 kg ha-1as treatments. Different growth and yield related parameters were studied in this experiment, such as days to flowering, plant height (cm), canopy width (cm), pods plant-1, kernels plant-1, 100 kernels weight, pod yield ha-1, edible portion (%) and non-edible portion (%). Randomized Complete Block Design (RCBD) was used with three replications. Results revealed that the potassium applied @ 80 kg ha-1 significantly affected for canopy width (1213.5 cm), pods plant-1 (52.27), kernels plant-1 (100.99), weight of 100 kernels (58.07 g), pod yield (1590.4 kg ha-1) and edible portion (63.50 %). To compare cultivars, Bari-2011 had superior results for early flowering (28.37 days), plant height (18.68 cm), canopy width (1102.9 cm), pods plant-1 (48.00), kernels plant-1 (94.43), weight of 100 kernels (55.26 g), yield (1543.6 kg ha-1) and edible portion (65.31%) as compared to Golden. The results revealed that potassium application at 80 kg ha-1 produced better results for growth and pod yield related attributes of the groundnut. Abstract

Background: One of the most common cancers in the world is c1olorectal cancer. Surgery is the sole curative option, and post-operative morbidity and mortality should be kept to a minimum to enhance results. Of course, the whole surgical and medical team is extremely frustrated when post-operative complications finally result in patient death. Because colon cancer patients tend to be older, it is to be expected that a growing percentage of patients have co-morbidity, making any procedure riskier. Patients may lose their lives even after successful surgery as a result of co-morbidity-related consequences. The main method of therapy for colorectal cancer is still surgery. However, the procedure entails a high risk of morbidity and death and uses a substantial amount of medical resources. Postoperative complications are common in patients who have had colorectal surgery, and they put them at increased risk for morbidity, mortality, poor oncologic outcomes, and a reduced quality of life. Objective: To find out associated risk factors of the patients related to colorectal cancer and the outcome of resection surgery of colorectal cancer. Materials and Methods: After receiving ethical permission, the department of surgery at Rangpur Medical College Hospital carried out this longitudinal form of descriptive study between July 2019 and June 2020. After explaining the nature and goal of the study to the participants, signed informed permission was acquired. A pre-made questionnaire was used to interview a total of 33 patients. Each patient had a complete physical examination, a careful history review, and any necessary investigations. Based on pre-existing co-morbidities, surgical technique, and associated complications, 30-day postoperative mortality and morbidity were assessed. The case questionnaire contained the collected data. ANOVA, Fisher exact, and the appropriate statistical tests (Chi-square) were run. Software called SPSS (version 22.0) was used to examine the data.

Abstract

Background: This study aimed to investigate the long-term effect of Type 2 diabetes on the quality of life of 100 patients in the Department of Diabetes and Endocrinology at the Hayatabad Medical Complex in Peshawar, Pakistan. The patients’ demographic information, medical history and associated comorbidities, and quality of life were collected and recorded. The quality of life was assessed using the World Health Organization Quality of Life (WHOQOL-BREF) tool. The results showed that Type 2 diabetes had a significant negative impact on the physical, psychological, and social domains of the patients’ quality of life. There were significant differences between males and females in terms of physical health, psychological health, and social relationships. In addition, elderly patients reported lower scores on general health and physical activities than younger participants. These results demonstrate that Type 2 diabetes has a long-term negative effect on the quality of life of those affected and should be taken into consideration when developing treatment and prevention strategies. Objectives: The main objective of this study was to investigate the long-term effect of Type 2 diabetes on the quality of life of 100 patients in the Department of Diabetes and Endocrinology at the Hayatabad Medical Complex in Peshawar, Pakistan.

Abstract

Background: The term "upper gastrointestinal bleeding" refers to bleeding from the gastrointestinal system that occurs above the Treitz ligament. It is one of the gastroenterological problems that is seen most often in clinical settings by gastroenterologists. Objective: To assess the frequency of gastric Varices amongst patients with upper gastrointestinal bleeding. Methodology: Our study was descriptive-cross sectional study, carried out at the Gastroenterology Department…….hospital Peshawar for duration of six months from August 2022 to January 2023. A careful upper-gastrointestinal endoscopy was carried out. Endoscopic findings of gastric varices in patients were recorded on a designed proforma for our research. All the documented data was analyzed by using 23. Results: In our current study a total of 120 patients were included. The male were 67 (55.83%) whereas female patients were 53 (44.17%). The mean age (SD) was 36 (9.11) years. The overall frequency of gastric varices amongst patients with gastrointestinal bleeding was 24 (20%). Conclusion: Our study concludes that frequency of gastric varices amongst patients with upper-gastrointestinal bleeding is very high. Multiple centre studies with large sample size should be conducted to get better results.

Abstract

The study aims to analyze the Financial Development, Economic Development, Governance, and consumption of Fossil Fuels impact on Environmental Quality. Panel data of 40 developing countries for the period of 1996-2019 were examined by applying the System Generalized method of moments (GMM) technique of Blundell and Bond (1998). The results show that financial and economic development in developing countries is at the stake of environmental degradation. The governance impact is negative but low compared to other variables in the study, which shows that governance in these developing economies is not up to the mark and needs continuous improvement to maximize the impact of governance on environmental quality. Fossil fuels consumption in the sample countries showed a devastating effect on environmental quality. Financial reforms are needed to encourage and give incentives to the firms to adopt environmentally friendly technologies, which will result in development in a more sustainable way in developing economies. Easy access to low-interest loans in developing economies will help farmers and live stockholders to adopt new technologies and rethink their approaches to using fertilizers and livestock production to mitigate the emissions of CH4 and N2O. Renewable energy sources (solar panel-generated energy in industries and households, electric cars, etc.) in these developing economies will help to tackle the increase in greenhouse gases emissions because of fossil fuel consumption.
