1. Global longitudinal strain: an early marker for cardiotoxicity in patients treated for breast cancer

Abstract

Patients treated with anthracyclines and trastuzumab are at increased risk of developing heart failure. Early diagnosis and treatment may prevent irreversible left ventricular (LV) dysfunction. This study investigates whether subclinical deterioration of global longitudinal strain (GLS) is a more reliable early predictor for LV dysfunction than three-dimensional (3D) LV ejection fraction (LVEF).Adult patients receiving anthracyclines and trastuzumab for breast cancer who had serial echocardiographic follow-up were included in this retrospective study. The primary endpoint was the necessity to temporarily pause chemo- or immunotherapy due to declining LVEF (decline in 3D LVEF of 10 percentage points to 53%). Linear mixed-effects models were used to assess the longitudinal evolution of 3D LVEF and GLS over time.Fifty-one women were included, mean age 54 (50.5-57.6) years, with a total of 216 follow-up echocardiograms (mean follow-up 1.1 ± 0.45 years). GLS and 3D LVEF were significantly correlated (Spearman's rho: -0.36, p 0.001). A decrease in GLS significantly predicted a lower LVEF on the subsequent echocardiogram [ß -0.6, 95% confidence interval (CI) (-1.0 to -0.2), p 0.006]. Conversely, prior LVEF did not significantly predict GLS on the subsequent echocardiogram [ß -0.04, 95% CI -0.1 to -0.01, p = 0.12]. Nine patients reached the primary endpoint. On average, patients who reached the primary endpoint had a relative decrease of 15% GLS at day 205 and an absolute 10% decrease of LVEF to LVEF 53% at day 235.GLS is able to identify subclinical LV dysfunction earlier than 3D LVEF measurement in women undergoing treatment for breast cancer with anthracyclines followed by trastuzumab.

Abstract

The aim of this study was to evaluate and compare the diagnostic value of a Modified Alvarado Score (MAS) ≥7 for acute appendicitis in both Human Immunodeficiency Virus (HIV)-negative (HIVneg) and positive (HIVpos) patientcohorts. This retrospective study included all HIV-tested patients undergoing appendectomy at a regional hospital from March 2010 to March 2011. Abstract

Thoracic aortic aneurysms are present in the majority of patients with Aneurysms-Osteoarthritis syndrome (AOS). Although these aneurysms are generally asymptomatic, the consequences can be catastrophic when an acute aortic dissection or rupture occurs. Elective valve-sparing aortic root replacement can be performed to prevent aortic events, but every operation has the risk of procedural complications. This subchapter discusses the indications, outcome, and recommendations for elective valve-sparing aortic root replacement in patients with AOS.

Because this syndrome has only been discovered recently and the full spectrum of the disease is not entirely elucidated yet, inevitably current clinical recommendations are mainly based on expert consensus. Several aspects of the multidisciplinary treatment for patients with AOS are outlined in the various subchapters following this chapter.

Abstract

Aneurysms-Osteoarthritis syndrome (AOS) is an aggressive connective tissue disorder characterized by arterial tortuosity, aneurysms, and osteoarthritis. Dissections can occur at smaller aortic diameters compared to other connective tissue disorders, such as Marfan syndrome, meaning that vigilant monitoring is warranted. Disorders associated with AOS include hypertrophy of the left ventricle, bicuspid aortic valve, persistent ductus arteriosus, and arterial aneurysm formation elsewhere in the body. This chapter describes imaging techniques used to evaluate cardiovascular abnormalities in patients with AOS.

Abstract

Visceral and iliac aneurysms are relatively rare yet potentially catastrophic when rupturing. Although most visceral and iliac aneurysms are degenerative, they can also be encountered in the setting of connective tissue disorders, such as Loeys-Dietz syndrome and vascular-type Ehlers-Danlos syndrome. Aneurysms-Osteoarthritis syndrome (AOS) is also known to be associated with aneurysms and arterial tortuosity in large and medium-size vessels throughout the arterial tree. As described in Chapter 2 , one-third of AOS patients had aneurysms in other thoracic or abdominal arteries, predominantly involving the pulmonary, splenic, iliac, and mesenteric arteries. This subchapter discuss endovascular and surgical treatment options and their results for patients with visceral and iliac aneurysms associated with AOS.

Abstract

Recently, pathogenic SMAD3 variants were found to cause Aneurysms-Osteoarthritis syndrome (AOS). AOS is inherited as an autosomal dominant disorder and is found to be responsible for 2% of familial thoracic aortic aneurysms and dissections. Aneurysms, dissections, and tortuosity throughout the arterial tree are the main cardiovascular features. Aortic aneurysms are most commonly located in the aortic root at the level of the sinus of Valsalva. The progression rate of the aortic dilatation is variable but can be rapid and unpredictable. Aortic dissections most commonly occur in the fourth decade of life, sometimes in relatively mildly dilated aortas. Other cardiac features include mitral valve prolapse and congenital heart defects. This chapter describes in detail all the cardiovascular features associated with AOS.

Abstract

When a patient presents with an aortic aneurysm or dissection at a relatively young age, it is key to determine which form of heritable thoracic aortic disease is the cause since diagnostic and treatment guidelines differ per diagnosis. However, since many clinical features overlap between the various heritable thoracic aortic diseases, detailed knowledge about the clinical features and genetics is essential for the correct diagnosis. In this chapter five disease entities, other than Aneurysms-Osteoarthritis syndrome, which are well known causes of heritable thoracic aortic disease, will be discussed namely: Marfan syndrome, Loeys-Dietz syndrome, Ehlers-Danlos syndrome, bicuspid aortic valve and Turner syndrome.

Abstract

In a previous study we identified 206 patients with colorectal adenocarcinoma in the Northern Cape province of South Africa, diagnosed between January 2002 and February 2009. The age-standardised incidence was 4.2/100 000 per year world standard population. This is 10% of the rate reported in First-World countries. In high-incidence areas, the rate of abnormal mismatch repair gene expression in colorectal cancers is 2 - 7%.The aim of this study was to determine the prevalence of hMLH1- and hMSH2-deficient colorectal cancer in the Northern Cape.Formalin-fixed paraffin wax-embedded tissue blocks from 87 colorectal adenocarcinomas identified in the previous study were retrieved. Standard immunohistochemical staining methods were used to detect the expression of hMLH1 and hMSH2 (i.e. products of the hMLH1 and hMSH2 genes) in the tumours using heat-induced antigen retrieval and diaminobenzidene as a chromogen. Results. In 8 blocks there was insufficient tumour tissue and in 1 case the immunohistochemical staining failed, probably owing to poor fixation, leaving 78 cases for analysis. In 11 cases hMLH1 was deficient and in 6 cases hMSH2 was deficient. Overall, 21.8% of cancers were deficient for hMLH1 or hMSH2.Presuming that 80% of all hMLH1 deficiencies are due to hypermethylation of the gene, we found 10.5% of colorectal cancers in an area with a low incidence of colorectal cancer to be deficient in the product of the mismatch repair gene/s. This is approximately three times the reported rate in high-incidence areas.

Abstract

A high-frequency power transformer using multilayer printed circuit board (ML-PCB) technology is presented for applications in switched-mode power supplies operating at frequencies up to several megahertz. The mechanical configuration of laboratory prototypes is discussed, as well as the electrical, parasitic, and thermal behavior. The focus is on the leakage inductance, since the analysis of other aspects is relatively simple. Test results show that the transformer has high efficiency, low leakage inductance, good thermal behavior, and good line insulation properties. The topology enables the designer to make a trade-off between leakage inductance and interwinding capacitance. Due to the well-defined geometry, parasitic interwinding capacitance and leakage inductance are reproducible and can be computed relatively easily. >

Abstract

This study examines propagation of 70-fs pulse of 800 nm wavelength in fused silica and sapphire samples in the normal dispersion regime over longitudinal distances up to 500 /spl mu/. The input intensity was varied from 2 up to 30 TW/cm/sup 2/. The beam width was 50-/spl mu/ FWHM and the envelope was assumed to be of Gaussian shape. The numerical results were compared with experimental data obtained with pump-probe technique.

Abstract

This article describes the effect of water stress on the room temperature chlorophyll fluorescence lifetime of plants of wheat (C3) and maize (C4). In addition, net CO2 assimilation rate (P N ), stomatal conductance and the fluorescence quenching coefficientsqP andqNP at steady state conditions were recorded. The overall fluorescence decay of the control plants can be described by an average decay time of 1 ns for both plant types. Water stress did not modify this parameter in the case of wheat, whereas a shortening of the decay was observed for waterstressed maize plants (τ=0.45 ns). This shortening in the chlorophyll fluorescence decay was accompanied by an increase in the nonphotochemical quenching (qNP). The photochemical quenching (qP) and therefore the electron transport via photosystem II remains unaffected by water stress. The most pronounced effect of the stress for both plant types was a decrease inP N due to a closure of the stomata.
