2. Early clinical exposure as a highly interesting educational program for undergraduate medical students: an interventional study

Background Training professional medical experts is so much dependent on the efficacy of the medical curriculum. Bearing this in mind, we aimed to evaluate the attitude of the undergraduate medical students toward the Early clinical exposure (ECE) program as a facilitator transition to the clinical phase. Methods This quasi-experimental study was conducted on undergraduate medical students at the Mashhad University of Medical Sciences, Mashhad, Iran who were transferring from the pre-clinical course to the externship course from 2021 to 2022 by census method (i.e. all eligible students were included and no sampling was performed). An eight-session ECE intervention was performed on the participants by two professors of the Internal medicine department of Ghaem Hospital, Mashhad University of Medical Sciences, Mashhad, Iran. The participants’ attitude toward the program and the program quality was assessed with the valid and reliable scale developed by Mirzazadeh et al. (Cronbach’s alpha = 0.72). Statistical analyses were performed by SPSS software (version.16) with a statistically significant level of less than 0.05. Results A total of 118 undergraduate medical students were enrolled in the study. Our results revealed that this program could familiarize (n = 95,81.2%)the students with the role of basic sciences knowledge in clinical settings, and 104(88.9%) participants believed that this intervention could motivate them toward learning more. The data revealed that this program was highly interesting for international students. There was a significant differentiation between Iranian and international students in familiarity with doctoring skills in medicine(P Conclusions The ECE program had a positive impact on the students’ satisfaction with medical education, and it also enhanced their understanding of the role they will play as future physicians. Therefore, we recommend that this program be implemented as a part of the medical education curriculum in medical universities.

After more than 2 years of the coronavirus disease 2019 pandemic caused by severe acute respiratory syndrome coronavirus 2, several questions have remained unanswered that affected our daily lives. Although substantial vaccine development could resist this challenge, emerging new variants in different countries could be considered as potent concerns regarding the adverse effects of reinfection or postvaccination. Precisely, these concerns address some significant and probable outcomes in vaccinated or reinfected models, followed by some virus challenges, such as antibody-dependent enhancement and cytokine storm. Therefore, the importance of evaluating the effectiveness of neutralizing antibodies (nAbs) elicited by vaccination and the rise of new variants must be addressed.

6. Preparation and photodeposition of Fe–S/TiO2@PEG nanoparticles for methylene blue and Evans blue

Here, iron, sulfur and poly(ethylene glycol) doping to TiO2 nanoparticles toward the effect on photodegradation of the methylene blue (MB) and Evans blue (EB) was investigated. The present nanostructured photocatalysts displayed notable catalytic activity for the decomposition of colorants in water under visible light irradiation. The photocatalytic reaction constants of different samples were determined for EB and MB to be 0.007, 0.008, 0.009 and 0.01, 0.026, 0.021 1/min, respectively. The values of optical band gap for pure TiO2, Fe–S/TiO2, and Fe–S/TiO2@PEG were estimated to be 3.21, 2.75, and 2.81, respectively. X-ray analysis was performed and correlated with BET, Fe–SEM, and TEM results. The lattice structure was studied by W–H (Williamson–Hall) and H–W (Halder–Wagner) methods with a different assumption in the isotropic and homogenous nature. The results revealed that the SSP model shows the most accuracy and adaption to determine the lattice structure.

7. Structure and Properties of Carbon Steel Cast by the Ablation Process

Ablation casting is a relatively new casting technique which can improve the mechanical properties of castings. In this paper, the microstructure and mechanical properties of plain carbon steel castings produced by conventional casting and ablation casting techniques were compared. It was found that ablation increased the ultimate strength from 638 to 1094 MPa and tensile elongation increased from approximately 13 to 16% as a result of severe microstructural changes from ferrite and pearlite to acicular ferrite, bainite, and martensite. Ablation increased the core hardness of the casting from 182 to 467 HB and the surface hardness from 205 to 508 HB.

Pemphigus is a severe, potentially life-threatening autoimmune blistering mucocutaneous disease which establishes with autoreactive IgG antibodies that target cellular adhesions, precisely extracellular domains of keratinocyte proteins. Several genetic and environmental elements are believed to contribute to the pathogenesis of the disease. The extent to which the initiation and progress of this autoimmune blistering disease may be influenced by the expression of human endogenous retroviruses (HERVs) remains to be elucidated. In this study, we evaluated the expression of HERV groups (K, W, and H) in pemphigus vulgaris (PV) patients in comparison to controls. Peripheral blood samples were collected from 24 PV patients and the corresponding age- and sex-matched healthy controls to extract total RNA for evaluation of HERV-K (HML-2), HERV-W, and HERV-H, env gene expression profile by qPCR. The mRNA expression level of HERV-K, HERV-W, and HERV-H were significantly upregulated in PV patients in comparison to healthy controls (P 0.0001). The difference in expression of studied HERVs groups between men and women was no significant (P 0.05). Although rituximab taking patients had a decreased expression level of studied HERVs, the results were not significant (P 0.05). According to our obtained data, HERVs expression could be measured as a possible diagnostic tool for detection of PV and monitoring of the treatment.

Innate and adaptive immune systems have a crucial role in initiating and progressing some pregnancy disorders such as preeclampsia (PE), which is one of the pregnancy-specific disorders that could result in neonatal and maternal morbidity and mortality. The dysregulation of the spiral artery and inadequate trophoblast invasion lead to PE symptoms through producing various inflammatory cytokines and anti-angiogenic factors from the placenta. T lymphocytes play a special role in the epithelium and stroma of the human endometrium. CD4+ T helper (Th) cells, Th1/Th2, and Th17/T regulatory (Treg) balance mainly contribute to the establishment of a pregnancy-favorable environment. This review examined the dysregulation of some cytokines produced from T cells, the dysregulation of the transcription factors of Th cells, the expression of chemokine receptors on T cells, as well as the effects of some factors including vitamin D on the activity of T cells, and finally, the dysregulation of various miRNAs related to T cells, which could cause PE.

Background Skin cancer is the most common cancer worldwide and commonly classified into malignant melanoma (MM) and Nonmelanoma skin cancers (NMSCs), which mainly include basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). The extent to which Long Interspersed Element-1 (LINE-1, L1) ORF1p is expressed in cutaneous malignancies remains to be evaluated. This study aimed to assess LINE-1 ORF1p immunoreactivity in various skin cancer subtypes. Method The expression level of LINE-1 ORF1p was evaluated in 95 skin cancer specimens comprising 36 (37.9%) BCC, 28 (29.5%) SCC, and 31 (32.6%) melanoma using the tissue microarray (TMA) technique. Then the association between expression of LINE-1 encoded protein and clinicopathological parameters was analyzed. Results We showed that LINE-1 ORF1p expression level was substantially higher in BCC and SCC patients compared with melanoma samples (p p = 0.004). In SCC samples, a lower level of LINE-1 ORF1p expression was associated with age younger than the mean (p = 0.041). At the same time, no significant correlation was found between LINE-1 ORF1p expression and other clinicopathological parameters (all p > 0.05). Conclusions According to our observation, LINE-1 ORF1p immunoreactivity in various skin tumor subtypes extends previous studies of LINE-1 expression in different cancers. LINE-1ORF1p overexpression in NMSCs compared with MM can be considered with caution as a tumor-specific antigen for NMSCs.

12. Explaining Status of Entrepreneurship Indicators and Predicting Them in Rural Residences (Case Study: Ravansar Township)

Today, entrepreneurship is one of the most important strategies for the development of local communities that in order to develop it, basic studies in this field are essential. Given the importance of entrepreneurship in the economic and social development of todaychr('39')s societies, it is important to explain the status of entrepreneurial indicators among villagers, as well as identify factors that can be effective in strengthening entrepreneurial indices. The present quantitative study, conducted via descriptive-analytical method, is aimed at investigating and assessing the state of entrepreneurship indicators and identifying factors affecting those indicators in rural residences of Ravansar Township. The data gathering tool was a questionnaire. The statistical population of the study consists of all heads of rural households in Ravansar Township as 5668 individuals. To achieve the research objectives, comparison tests, frequency distribution table, Pearson correlation coefficient and Spearman and multivariate regression were used in SPSS software. The results regarding enjoyment of heads of households from the entrepreneurship indicators indicated that the degree of enjoyment of 39.2% of the participants were at low and very low levels, 45.5% of them were at the moderate level, and about 15.3% of them were at high and very high levels. All in all, the mean calculated for villagers’ enjoyment of entrepreneurship indicators was about 2.74% which is a level lower than the moderate one. The results of multivariate regression indicated that five identified variables could explain about 70% of the variance of entrepreneurship indicators in the studied area. The identified variables and the explained percentage of each of them are as follows: socioeconomic status (0.391), access to facilities and equipment (0.315), social capital (0.244), performance of management institutions (0.240), and ownership of capital resources (0.191) respective to their significance. Results the relationship between entrepreneurship indices and the variables studied showed in among the 11 variables studied, the relationship between entrepreneurship and gender variables; Internet access, age, household size, and distance to essential centers were not statistically significant and the mentioned variables have not related to entrepreneurship. © 2018 Geographical Researches

Abstract. The purpose of the study was to determine the effect of dietary iron on distribution of zinc, manganese, copper, calcium and magnesium in the body of Wistar rats. Commercial Sangak, an Iranian traditional flat bread was used in this study. It was prepared by additionof yeast and sourdough. Different doses of iron (35, 70, 140, and 210 mg/kg per diet) were added to bread vehicle for 30 days with or without baking soda (250 mg/kg per diet). The concentrations of the above elements were determined by graphite furnace and flame atomic absorption spectroscopy. Serum zinc and manganese concentrations were significantly lower (P < 0.05) in rats that received higher concentrations of iron compared to the controls. Serum iron, and copper concentrations increased significantly (P < 0.05) with an increase in dietary iron compared to the controls. Increments of dietary concentrations of iron raised the concentration of iron, zinc and manganese in the feces, and lowered the concentration of copper and calcium in the feces (P < 0.05). Diets that contained baking soda showed a statistically significant increase in phytic acid (P < 0.05). Lower serum iron was seen in rats that received baking soda with diet of the control group. Conversely, feces iron and zinc were increased in this group. It is concluded that iron absorption might interfere with zinc and manganese because of similar physicochemical properties. Knowledge about these interactions is essential when supplementation of some elements is recommended in populations with a high risk of some deficiencies such as iron and zinc.

Background Alongside many complications in understanding the etiology of Preeclampsia (PE), several determinants, such as the imbalanced proportion of anti-angiogenic/proangiogenic T-cell subsets, especially CD4+ (Th17/Treg), as well as alterations in the expression profile of related cytokines, miRNAs, and transcription factors might have been implicated in PE pathogenesis. Material and method After sample collection and preparation, CD4+ cells were isolated from PE and non-PE pregnant woman and were cultured. Furthermore, analysis such as flow cytometry, real-time PCR, western blotting, and ELISA were performed to assess determinants related to PE manifestation, including sFlt-1, sEng, STAT-3, RORγt, SMAD-7, Foxp3, IL-17, IL-22, Ets-1, and miRNA-326. Results Our results showed that the miRNA-326 expression level increased in CD4+ Cells and Th17 in PE patients which downregulated Ets-1 expression that acts as a negative control for Th17 development. Furthermore, we showed that the number and expression level of Th17 s and transcription factor RORγt escalated, respectively. While Treg and its related transcription factor (Foxp3) demonstrated a decrease. Flow cytometry analysis illustrated that the Th17/Treg ratio increased in PE. Additionally, we demonstrated that expression and concentration levels of cytokines (IL-17 and IL22) and anti-angiogenic molecules (sEng and sFlt-1) soared in isolated CD4+ cells from PE patients, which could be correlated with PE pathogenicity. Conclusion In conclusion, we comprehensively evaluated immunological factors and molecules involved in PE manifestation. Interestingly, the CD4+ T-cell subset could be an extra source of antiangiogenic factors for the maintenance of this hypertension disorder.

15. Effects of Simple and Microencapsulated Lactobacillus acidophilus With or Without Inulin on the Broiler Meat Quality Infected by Avian Influenza Virus (H9N2)

The aims of the present study were to investigate the effects of simple and microencapsulated Lactobacillus acidophilus as probiotic with or without inulin as prebiotic on meat quality of broiler infected by avian influenza virus (H9N2). Two hundred-day-old chicks were randomly allocated into 14 groups based on simple, microencapsulated probiotic and prebiotic and based on vaccination and challenge with H9N2 virus. Groups 1-7 contained 20 chicks, and groups 8-12 and 14 contained 10 chicks. Group 13 was derived from group 1 with 10 chicks at challenge day with avian influenza virus (AIV). Half of the groups were vaccinated by H9N2 vaccine on day 5. All groups except the negative control and positive vaccine control were challenged with 106.5 EID50 of low-virulence H9N2 AIV at day 21. Each bird was received 109 CFU of simple or microencapsulated probiotic on days 0 and 17 by gavage. Prebiotic as dose as 0.1 % of feed weight was used daily. Increase in water-holding capacity, dry matter, ash and protein content, and decrease in dripping loss plus beneficial changes in lightness and redness of breast meat were detected in response to probiotic especially microencapsulated synbiotic. In conclusion, probiotic alone or with prebiotic was able to improve the physicochemical properties of chicken breast muscle in both healthy and AIV-infected chickens.

Detecting coordination pattern and coordination variability help us to find how joints organize collaboratively to perform sit-to-stand (STS) under restricted visual input. This experiment aimed to compare the coordination of the trunk, hip, knee, and ankle and its variability between individuals with long- and short-term restricted visual input during STS. Forty-five female children participated in this study, including fifteen congenitally blind (CB) children and 30 healthy children. The healthy children were divided randomly into two groups: one group in which the participants were instructed to keep their eyes open (EO) and another to keep their eyes closed (EC) for 20 min before the test. In the standing phase, CB children had a decreased ankle-knee vector angle on the nondominant (ND) side compared to that of healthy children. In the sagittal plane, a small coefficient-of-correspondences (CoC) was observed at seat-off (hip-trunk CoC on the dominant (D) side and ankle-hip CoC on the ND side) and in the preparation phase (ankle-hip CoC on the ND side and bilateral hip CoC). In the frontal plane (at the end: ankle-knee, in the standing phase: bilateral hip) a high CoC was observed (in the standing phase: knee-trunk CoC on the D side). The EC group had smaller CoCs at initiation event (knee-trunk and bilateral knee CoCs on both sides), the end event (ankle-knee and ankle-hip CoCs on the ND side), and in the standing phase (bilateral hip CoC) in the frontal plane than the other groups. The findings reveal that vector and CoC variables are altered because of long- and short-term restricted visual data and should be a focus in rehabilitation programs.

Neuroendoscopic surgery has been performed as an effective method for intracerebral hemorrhage (ICH). This study describes the know-how of constructing the ICH cadaver model and the training on the main neuroendoscopic procedures for ICH. During the training, operation time of twenty trainees in main stages of craniotomy and corticotomy (stage 2), and hematoma evacuation under endoscopy (stage 3) was recorded. To distinguish factors influencing trainees' surgical proficiency, operation time was calculated according to seniority, experience in neuroendoscopic surgery and training sequence. Questionnaire about validity of model was conducted eventually. Ten ICH cadaver models with bilateral hematoma were constructed. Seven trainees worked with seniority5 years and eleven had experience in neuroendoscopic surgery. Operation time ranged from 20.6 to 33.4 min in stage 2 and 18.5 to 24.9 min in stage 3. In stage 2, less operation time was needed for trainees with seniority5 years comparing to trainees with seniority ≦5 years (22.56 ± 1.29 vs 29.25 ± 3.02 min, p 0.01). In stage 3, significant difference of operation time was found between trainees with experience in neuroendoscopic surgery and trainees without the experience (20.08 ± 1.22 vs 22.02 ± 1.82 min, p = 0.014), and the same between trainees in latter group and in former group (19.75 ± 0.80 vs 22.54 ± 1.45 min, p 0.01). Questionnaire feedback proved high degree of satisfaction about the training model. Therefore, the ICH cadaver model can assist neurosurgeons with neuroendoscopic treatment learning sessions. Simulation and improvement in neuroendoscopic surgical techniques for ICH treatment were possible with the help of ICH cadaver model.

Extracorporeal shock wave lithotripsy (ESWL) is mainly an alternative for other therapeutic methods such as surgery and endourology to treat urinary tract calculus. Although it is safe and effective, it has undesirable effects on renal function. Diagnostic techniques such as color Doppler ultrasonography create a new attitude toward renal function. The aim of this study was to evaluate renal vascular resistance change before and after extracorporeal shock wave lithotripsy.During the present study, vascular resistive index (RI) of renal intralobar artery was measured before, 30 min, and 1 week after ESWL using Doppler ultrasonography.Thirty minutes after ESWL, RI was significantly increased from primary value of 0.62 ± 0.05 to 0.66 ± 0.06 (p = 0.0001). There was no correlation between increase of RI and patients' age. Following up the patients revealed that mean RI did not return to pretreatment level after 1 week (p0.05). The RI level in the old patients (3 patients who were 60 years or older) was higher than that of the younger ones (19 patients who were younger than 60 years) after 1 week (0.76 ± 0.05 vs. 0.64 ± 0.06). There was no meaningful relationship between ESWL voltage or number of shocks and RI variation before and after ESWL.Following ESWL, patients are at risk of renal tissue damage due to increase of primary RI level. Measuring RI variations using ultrasound techniques after ESWL may provide helpful information to clinical detection of renal tissue damage.

This study was a screening survey to determine the occurrence of aflatoxin M1 (AFM1) in 82 cheese samples composed of white cheese (45 samples) and Lighvan cheese (37 samples) obtained from supermarkets and retail outlets in Rafsanjan city of Iran. The competitive enzyme immunoassay method was used for determination of the toxin in the samples. Aflatoxin M1 was detected in 39 (47.6%) samples, consisting of 29 (64.4%) white cheese (mean: 135 ng/kg; range: 93.3–309 ng/kg) and 10 (27%) Lighvan cheese samples (mean: 90.8 ng/kg; range: 70.5–203 ng/kg). According to Iranian national standard limit for AFM1 in cheese (200 ng/kg), 9 samples (20%) of white cheese and 1 sample (2.70%) of Lighvan cheese had levels above the limit. It was concluded that the contamination of the samples with AFM1 in such a level could be considered as a serious public health problem.

Very little is known about parasitic diseases of European pond turtles (Emys orbicularis) in Iran. The objective of this study is to examine parasitic fauna of European pond turtles collected from Fars province, southwest Iran. Carcasses of turtles (n = 52) which died during dredging procedure are collected from earthen fishery basins in Zarghan region. They have been died earlier during dredging procedure in different farms. Three species of helminths in total were found in gastrointestinal tract, including two nematodes (Serpinema microcephalus and Falcaustra araxiana) and one digenean trematod (Telorchis assula). Large intestines of all examined turtles were infected by F. araxiana (100 %, Mean intensity = 18) and this nematode were also found in gastric nodules. Nine turtles (17.3 %, 3 male, 6 female, Mean intensity = 3) were infected with Serpinema microcephalus. T. assula were found in 25 turtles (48.07 %, 5 male, 20 female, mean intensity = 5). Helminths were not found in any examined organs and no ectoparasite found eighter. F. araxiana is the most prevalent nematode in European pond turtles. Detection of Serpinema.microcephalus is in agreement with the fact which this parasite is common parasite of turtles in all over the world. T. assula might be transmitted between variety of reptiles so presence of the trematod should be considered as a risk factor for other reptiles.

Percutaneous nephrolithotomy (PCNL) can be done in patients who have previously had open nephrolithotomy. Currently, dilatation of the nephrostomy tract is achieved using an Amplatz renal dilator or telescopic metal dilators in these patients. The aim of this study was to compare the safety and efficacy of the "one-shot" procedure in patients with previous open nephrolithotomy with those of the current telescopic technique.Thirty-one patients with past history of open surgery for kidney stone disease underwent PCNL at our institution from February 2006 to March 2007. Patients were randomly divided into two groups according to dilation technique used: group A (telescopic procedure) and group B (one-shot procedure). X-ray exposure, blood loss, and complications were evaluated.No significant difference in hemoglobin decrease was seen in the two groups. The stone-free and complication rates were the same in the two groups. Fluoroscopy time during the one-shot procedure was significantly shorter than that of the telescopic procedure (P0.000).Our experience indicated that the one-shot procedure is feasible in patients with previous open nephrolithotomy. It is as safe and effective as the telescopic procedure, with significant reduction in x-ray exposure.

The contact dynamics in a low-voltage circuit breaker contact assembly have been modeled using a multiphysics approach. The approach considered the effects of current constriction and Lorentz forces acting to separate the contacts, while incorporating the presence of the arc established between the open contacts in providing a path for continued current flow. Contact movement, due to these forces as well as additional tripping mechanism action, was determined. The results compare well with high-speed x-ray video images captured during an actual short circuit test.
