Abstract

Screw compressors belong to the group of positive displacement machines, which are widely used in various industries due to the advantages of these compressors compared to other industrial compressors. The distance between the moving and fixed parts of the rotating equipment is inevitable. This distance causes current leakage in all types of compressors, which reduces the efficiency of the compressor. In this research, the effect of grooving the flow passage on the flow characteristics of the passing fluid has been investigated using ANSYS CFX software. The simulation quality of positive displacement machines strongly depends on the correct prediction of the leakage current between the existing switches. It seems that creating a groove in the path of the passing current increases the thickness of the boundary layer, which acts as a barrier against the return leakage current between the rotor and the stator in the compressor. In this paper, a simplified model of the problem, which is a grooved cylinder in which a piston rotates, is investigated. First, the flow of fluid from a cylinder whose inner surface is smooth is examined, and then in boundary conditions and similar geometric dimensions, the effect of different types of grooves with different geometries on the flow characteristics will be studied. The variable is the change in drag force on the inner surfaces of the cylinder. The results show that in the best case, ie a cylinder with a single helical groove, the drag force compared to smooth geometry is 1.79 times and the flow rate is reduced by 13.21%. It is expected that the results of this study can be generalized to reduce leakage current between the rotor and the screw compressor housing.

3. Psoas Muscle Index – Could It Be an Indicator of Postoperative Complications in Colorectal Cancer? Case Presentation and Review of the Literature

Abstract

Introduction: Colorectal cancer is the third most commonly diagnosed cancer worldwide, and its incidence is rising in developing countries. Studies have shown an association between sarcopenia and various poor short-term outcomes in cancer patients who underwent surgery. In this case study, we sought to highlight the importance of a modern prognostic factor, the psoas muscle index (PMI), in colorectal cancer. Case Presentation: We present the case of a 59-year-old male patient with sigmoid cancer. We calculated the PMI as the sum of the areas of the left and right psoas muscle at the level of the L3 vertebra, divided by the square of the patient’s height, using computed tomography measurements. We also measured the density of the proas muscles. Prior to surgery, the left psoas muscle had an area of 14.52 cm2, and the right 14.54 cm2, with a PMI of 9.4967 cm2/m2. The mean density of the psoas muscles was 43.60 UH. The surgery consisted in a rectosigmoid resection with termino-terminal colorectal anastomosis. The histopathological examination confirmed moderately differentiated adenocarcinoma, stage IIIB (pT3N1M0). The patient left the hospital eight days after surgery in good general condition. Long-term follow-up was performed by phone and CT scans at 1, 6, and 12 months. Conclusions: CT-measured PMI may be a potential prognostic imaging marker in patients with colorectal cancer that can be easily and routinely assessed preoperatively.

Abstract

Aim The Enhanced Recovery After Surgery (ERAS® ) Society guidelines aim to standardise perioperative care in colorectal surgery via 25 principles. We aimed to assess the variation in uptake of these principles across an international network of colorectal units. Method An online survey was circulated amongst European Society of Coloproctology members in 2019/20. For each ERAS® principle, respondents were asked to score how frequently the principle was implemented in their hospital, from 1 ('rarely') to 4 ('always'). Respondents were also asked to recall whether practice had changed since 2017. Subgroup analyses based on hospital characteristics were conducted. Results Of hospitals approached, 58% responded to the survey (195/335), with 296 individual responses (multiple responses were received from some hospitals). The majority were European (163/195 [83.6%]). Overall, respondents indicated they 'most often' or 'always' adhered to most individual ERAS® principles (18/25 [72%]). Variability in uptake of principles was reported, with universal uptake of some principles (e.g., prophylactic antibiotics; early mobilisation) and inconsistency from 'rarely' to 'always' in others (e.g., no nasogastric intubation; no preoperative fasting and carbohydrate drinks). In alignment with 2018 ERAS® guideline updates, adherence to principles for prehabilitation, managing anaemia, and postoperative nutrition appears to have increased since 2017. Conclusions Uptake of ERAS® principles varied across hospitals, and not all 25 principles were equally adhered to. Whilst some principles exhibited a high level of acceptance, others had a wide variability in uptake indicative of controversy or barriers to uptake. Further research into specific principles is required to improve ERAS® implementation.

5. Experimental methods for determining the characteristic quantities of unconventional naval propellers

Abstract

Basin research involves the practical realization of the scale structure and the analysis of the results according to the studied models and working frequency. The obtained measurements are close in value to those of the numerical simulations, this confirming the numerical results obtained with Ansys Fluent. The analysis carried out on the non-conventional naval propulsion includes all the calculation elements to be able to expand the understanding of this type of propulsion to be able to find the most suitable ones for use in the naval field and not only. The research carried out in this paper presents numerous elements of analysis of the unconventional naval propulsion system. By plotting and validating the thrust functions for a blade, a complete analysis of how this propulsion works is achieved.

7. Analytical Study of Dynamic and Vibrational of Composite Shell with Piezoelectric Layer using GDQM Method

Abstract

Piezoelectric materials, due to their electromechanical coupling properties, are widely used as actuators and sensors in intelligent structures to control vibrations and bends of multilayer sheets with piezoelectric layers. In this paper, the response of free vibrations of a multilayer composite shell with the new Generalized Differential Quadrature Method (GDQM) for different boundary conditions is investigated. The governing equations are obtained by assuming first-order shear theory and using Hamilton's principle. The generalized quadrature differential method is used to solve the obtained equations. To use this method, coding has been done in MATLAB software. Due to the same thickness of the layers, as the number of composite layers increases and the total thickness is constant, the thickness of each layer decreases, and consequently the thickness of the piezoelectric layer decreases. Comparing the results of this method with the work of other researchers shows that this method has good accuracy.

8. Analytical Study of the Penetration of Long Rod Projectiles with Conical and Blunt Nose in Normal and Oblique Ceramic Targets

Abstract

The Mechanics of penetration investigate the interactions between projectiles and targets. The Mechanics of penetration were initially developed with military motives and were used in the design and manufacture of durable military vehicles, warships, protective vests, bulletproof panels, satellite protective equipment, nuclear reactor protection used. In this paper, the penetration of cylindrical and conical-nosed long rods projectiles in normal and the diagonally ceramic target is studied by analytically and numerical methods; And the Interface defeat e phenomenon, in which the projectile is eroded so that the material in the target surface is radial and outward flow. A comparison was also made between the oblique and normal penetration of long rods cylindrical and conical-nosed projectiles, which illustrates the importance of collision angles. The analytical relations related to the reduction of velocity and erosion of the projectile mass were investigated concerning the extraction time and kinetic energy loss of the projectile. Transition from the interface defeat phase to the penetration phase is also described in detail, and the variation of the critical speed suffering from the change in the angle of impact has been investigated. Each of the results of the analytical work and numerical simulations has been evaluated by experimental work carried out in previous studies and the comparison of the results shows the acceptable accuracy for the obtained relationships.

Abstract

Malignant tumors of the oral cavity have a growing incidence, most being squamous cell carcinomas, generally called oral cancers (OCs), clinically detected at various stages of natural evolution. The increased incidence in Romania in recent years and the lack of conclusive data have led to the development of this study. The main purpose of this study was to assess the molecular profile of tumors, the types of blood vessels associated with the tumor, and expression of tumor immunomarkers. Regarding morphological findings, focal epithelial hyperplasia, dysplastic lesions, typical mitoses, perineural invasion, parakeratosis and keratosis beads, intracytoplasmic keratinization were observed. Microvascular density was higher in the tumor area compared to the peritumoral area. Lymphovascular invasion was identified in 13% of cases, which also presented regional lymph node metastases. Podoplanin expression was identified in 79% of cases which were tested positive for the D2-40 immunomarker. All p53-positive cases co-expressed epidermal growth factor receptor (EGFR), half of the EGFR-positive cases co-expressed p53, and co-expression of CD117 and p63 was identified in 80% of EGFR-positive∕cytokeratin 5 (CK5)-positive cases being proposed the basal-like subtype of OCs, defined as EGFR-positive∕CK5-positive, CD117-positive and p63-positive. Results support the need for molecular classification of OCs based on of tumor immunomarker expression and gene analysis.

Abstract

Introduction. With syntropy of CVD and AH, the pathology of the digestive system can exacerbate the course of AH and develop special conditions for increasing the risk of cardiovascular complications. Pharmacotherapy of HD may affect the course and progression of gastroesophageal reflux disease. Material and methods. The study was conducted using the PubMed, Z-library, NCIB, Medscape, Mendeley databases using the keywords: “syntropy”, “comorbidity”, “arterial hypertension”, “acid-dependent diseases”, “gastric ulcer”, “gastroesophageal reflux disease”. Results. AH and CVD are quite often observed together, having, moreover, a conditioned commonality of the mechanisms of formation. The proximity of the location with the heart, the commonality of innervation, in the pathology of the esophagus, can create viscero-visceral reflexes and provoke arrhythmias, imitate coronary heart disease. Pathology of the upper gastrointestinal tract can lead to functional disorders of the cardiovascular system, manifested through the autonomic nervous system. The combined course of AH and CVD, observed, according to various sources, from 11.6% to 50%, is not just random, but may have pathogenetic and etiological patterns. Comorbidity of CVD and AH can enhance the development of the main pathological process and lead to disruption of adaptive mechanisms. Conclusions. The syntropy of AH and CVD requires thorough scientific studies to discover and elucidate the pathogenetic features of the course and methods of better drug therapy.

13. A laboratory platform for studying rotational dust flows in a plasma crystal irradiated by a 10 keV electron beam

Abstract

A novel laboratory platform has been designed and built for the irradiation of a plasma crystal (PC) with an electron beam (e-beam) having an energy around 10 keV and a current of tens of milliamperes. The pulsed e-beam collimated to a few millimeter-size spot is aimed at a crystal made of dust particles levitated in a radio-frequency (RF) plasma. The platform consists of three vacuum chambers connected in-line, each with different utility: one for generating free electrons in a pulsed hollow-anode Penning discharge, another for the extraction and acceleration of electrons at $$\sim 10$$ ∼ 10 kV and for focusing the e-beam in the magnetic field of a pair of circular coils, and the last one for producing PCs above a RF-driven electrode. The main challenge is to obtain both a stable e-beam and PC by insuring appropriate gas pressures, given that the e-beam is formed in high vacuum ($$\lesssim 10^{-4}$$ ≲ 10 - 4 Torr), while the PC is produced at much higher pressures ($$\gtrsim 10^{-1}$$ ≳ 10 - 1 Torr). The main diagnostics include a high speed camera, a Faraday cup and a Langmuir probe. Two applications concerned with the creation of a pair of dust flow vortices and the rotation of a PC by the drag force of the e-beam acting on the strongly coupled dust particles are presented. Abstract

Background: No evidence currently exists characterising global outcomes following major cancer surgery, including esophageal cancer. Therefore, this study aimed to characterise impact of high income countries (HIC) versus low and middle income countries (LMIC) on the outcomes following esophagectomy for esophageal cancer.Method: This international multi-center prospective study across 137 hospitals in 41 countries included patients who underwent an esophagectomy for esophageal cancer, with 90-day follow-up. The main explanatory variable was country income, defined according to the World Bank Data classification. The primary outcome was 90-day postoperative mortality, and secondary outcomes were composite leaks (anastomotic leak or conduit necrosis) and major complications (Clavien-Dindo Grade III - V). Multivariable generalized estimating equation models were used to produce adjusted odds ratios (ORs) and 95% confidence intervals (CI95%).Results: Between April 2018 to December 2018, 2247 patients were included. Patients from HIC were more significantly older, with higher ASA grade, and more advanced tumors. Patients from LMIC had almost three-fold increase in 90-day mortality, compared to HIC (9.4% vs 3.7%, p < 0.001). On adjusted analysis, LMIC were independently associated with higher 90-day mortality (OR: 2.31, CI95%: 1.17-4.55, p = 0.015). However, LMIC were not independently associated with higher rates of anastomotic leaks (OR: 1.06, CI95%: 0.57-1.99, p = 0.9) or major complications (OR: 0.85, CI95%: 0.54-1.32, p = 0.5), compared to HIC.Conclusion: Resections in LMIC were independently associated with higher 90-day postoperative mortality, likely reflecting a failure to rescue of these patients following esophagectomy, despite similar composite anastomotic leaks and major complication rates to HIC. These findings warrant further research, to identify potential issues and solutions to improve global outcomes following esophagectomy for cancer.

Abstract

Background: The incidence of rectum cancer is rising worldwide. New prognostic factors need to be applied to improve patients’ prognosis. The incidence of sarcopenia has been observed in various malignancies. The quantity of skeletal muscle mass is reflected in the psoas muscle index (PMI), suggesting the presence of sarcopenia. Our aim is to investigate the association of postoperative prognosis of patients with rectum cancer with the occurrence of preoperative sarcopenia with quantitative and qualitative reduction of skeletal muscle mass. Material and method: We performed a multidisciplinary research and we examined patients diagnosed with rectal cancer. The PMI was determined at the lower edge of the L3 vertebral body, using CT scan. We present our research partial results within three cases. Results: The PMI of three male patients with rectum cancer who received neoadjuvant chemoradiotherapy was: Case 1 has a PMI of 7.378 cm2/m2, the mean psoas muscle density was 50.14 UH. Case 2 has a PMI of 6,942 cm2/m2 and a mean psoas density of 43.81 UH. Case 3 has a PMI of 8.4885 cm2/m2 and an average density of 43.85 UH in the right and left psoas. Conclusions: The PMI value can be calculated with simple radiologic measurements, thus determining the diagnosis of sarcopenia. Calculations are performed using standard preoperative CT-scans. A PMI value above the threshold limit is promising in the short term, but in the long term it is not responsible for the deterioration of patients’ general condition and tumor recurrence.

Abstract

This study's objective was to separate cellulose, hemicellulose, and lignin after high-pressure supercritical carbon dioxide pretreatment for further valorization. The study investigated the supercritical carbon dioxide pretreatment of apple orchard waste at temperatures of 160-200 °C, for 15-45 min, at a pressure of 10 MPa. Response Surface Methodology (RSM) was used to optimize the supercritical process and to improve its efficiency. The change of functional groups during different pretreatment conditions was examined by Fourier transform infrared (FTIR) spectroscopy. Scanning electron microscopy (SEM) and X-ray diffraction (XRD) confirmed the structural changes in the biomass structure before and after pretreatment. A new ultra-high performance liquid chromatography (UHPLC) coupled with an evaporative light scattering detector (ELSD) method was developed and validated for the determination of carbohydrates in the liquid fraction that resulted after pretreatment. Abstract

Vineyard solid wastes are regarded as an alternative source of renewable raw materials for the production of hemicellulosic sugars and their conversion into furfural. In this work, a liquid hemicellulosic fraction was extracted from vineyard wastes in order to produce a higher furfural yield. A simple ultra-high-performance liquid chromatography with diode array detection (UHPLC-DAD) method was used for determining furfural from vineyard wastes. The UHPLC-DAD method was validated for the determination of furfural from vineyard wastes. The described method shows good linearity with a coefficient of determination higher than 0.998. The limits of detection (LOD) and quantification (LOQ) were 0.3 ��g mL���1 and 1.0 ��g mL���1, respectively. The recoveries determined by the spiking solutions varied between 95.1 and 100.8%. The relative expanded uncertainty of the method was evaluated based on a bottom-up approach and was 13.5%.

Abstract

Venous thromboembolism (VTE) in children is a rare condition. An increased incidence has been observed in the last few years due to several factors, such as increased survival in chronic conditions, especially chronic kidney disease (CKD), use of catheters, and increased sensitivity of diagnostic tools. VTE includes deep vein thrombosis (DVT) and pulmonary embolism (PE). VTE in children is associated with a two to six times higher mortality risk and a 5–10% prevalence of post-thrombotic syndrome. Overall, 5% of VTE episodes in children are associated with chronic kidney disease. The etiology of VTE in chronic kidney disease covers a wide range of pathologies. Various types of thrombotic complications may occur during long-term use of a chronic dialysis catheter. VTE occurs in 3% of children with nephrotic syndrome (NS). The risks for VTE and arterial thromboembolism (ATE) were particularly high in the first 6 months after the onset of NS. Other causes of VTE are graft rejection due to thrombosis of vascular anastomoses after kidney transplantation (3%) and autoimmune diseases (lupus nephritis, antiphospholipid syndrome). In this state-of-the-art overview, we have reviewed the physiologic and pathologic mechanisms underlying pediatric thrombosis and updated current diagnostic and treatment options, emphasizing personal experience as well.

Abstract

Hemoglobin-based oxygen carriers (HBOCs) have been proposed and tested for several decades for the treatment of hemorrhage. We have previously proposed replacing hemoglobin (Hb) in HBOC with the oxygen-carrying protein hemerythrin (Hr), from marine worms, showing that Hr-based derivatives can perform at least as well or even better than Hb-based HBOC in a range of in vitro assays involving oxidative and nitrosative stress as well as in top-up animal models, where small amounts of Hr- or Hb-HBOC were injected into rats. Here, these experiments are extended to a hemorrhage experiment, in which Hr polymerized with glutaraldehyde, alone or conjugated with human serum albumin, is administered after a loss of 20–30% blood volume. The performance of these preparations is compared with that of Hb-based HBOC measured under the same conditions. Polymerized Hr is found to decrease the survival rate and can hence cannot be used as an oxygen carrier in transfusions. On the other hand, an Hr-albumin copolymer restores survival rates to 100% and generally yields biochemical and histological parameters similar to those of glutaraldehyde-polymerized bovine hemoglobin, with the exception of an acid-base imbalance. The latter may be solved by employing an allogeneic albumin as opposed to the human albumin employed in the present study.

Despite the availability of improved antiviral therapies, infection with Hepatitis B virus (HBV) remains a3 significant health issue, as a curable treatment is yet to be discovered. Current HBV vaccines relaying on the efficient expression of the small (S) envelope protein in yeast and the implementation of mass vaccination programs have clearly contributed to containment of the disease. However, the lack of an efficient immune response in up to 10% of vaccinated adults, the controversies regarding the seroprotection persistence in vaccine responders and the emergence of vaccine escape virus mutations urge for the development of better HBV immunogens. Due to the critical role played by the preS1 domain of the large (L) envelope protein in HBV infection and its ability to trigger virus neutralizing antibodies, including this protein in novel vaccine formulations has been considered a promising strategy to overcome the limitations of S only-based vaccines. In this work we aimed to combine relevant L and S epitopes in chimeric antigens, by inserting preS1 sequences within the external antigenic loop of S, followed by production in mammalian cells and detailed analysis of their antigenic and immunogenic properties. Of the newly designed antigens, the S/preS116–42 protein assembled in subviral particles (SVP) showed the highest expression and secretion levels, therefore, it was selected for further studies in vivo. Analysis of the immune response induced in mice vaccinated with S/preS116–42- and S-SVPs, respectively, demonstrated enhanced immunogenicity of the former and its ability to activate both humoral and cellular immune responses. This combined activation resulted in production of neutralizing antibodies against both wild-type and vaccine-escape HBV variants. Our results validate the design of chimeric HBV antigens and promote the novel S/preS1 protein as a potential vaccine candidate for administration in poor-responders to current HBV vaccines.

Abstract

An 8-year-old female spayed German Shepherd cross was presented for acute onset of respiratory distress. Four days before presentation, the owner noticed a reduced appetite and reluctance to move. Clinical examination identified muffled lung sounds and a left base, diamond-shaped systolic murmur graded 4/6. Echocardiography identified pleural and pericardial effusion, ascites and a myxoid mass (39 mm/18.9 mm) obstructing the right ventricular outflow tract and interfering with the pulmonary valve function. Given the poor prognosis, the dog was euthanatised, and a postmortem examination was performed. Grossly, a mass with a heterogeneous appearance was identified below the pulmonary valve leaflets. Based on histopathological and immunohistochemical findings, a diagnosis of intracardiac myxosarcoma affecting the subvalvular region of the pulmonary artery was made. To the author's knowledge, this is the first report of right ventricle out flow tract myxosarcoma in the canine species.

Abstract

Introduction. Although SARS-CoV-2 infection is more common in adults, many cases have been reported in the pediatric population. Case presentation. An 8-month-old infant from twin pregnancy, confirmed with COVID-19, is hospitalized with nasal obstruction, serous rhinorrhea, rare cough and watery stools. The epidemiological link is known, both the parents and the maternal grandparents of the infant being confirmed with COVID-19. Biologically, he presented with thrombocytosis and discrete inflammatory syndrome, and the lung radiograph did not show any lesions. At the same time, the twin sister was hospitalized, who presented a similar symptomatology, with a negative SARS-CoV-2 PCR test, but the lung radiograph showed specific lesions of COVID-19. During hospitalization, 2 more SARS-CoV-2 PCR tests were performed, but with negative results. The evolution of the twins was favorable under symptomatic treatment, respectively antibiotic and symptomatic treatment. Conclusion. SARS-CoV-2 infection may be asymptomatic, especially in infants and newborns, and has a lower prevalence.

Abstract

Background and Objectives: During pregnancy, women undergo various physiological and anatomical changes that are accentuated as the pregnancy progresses, but return to their previous state a few weeks/months after the pregnancy. However, a targeted therapeutic approach is needed. Most of the time, during this period, these changes precipitate the appearance of pain, musculoskeletal pain being the most common. Pregnant women should avoid treating musculoskeletal pain with medication and should choose alternative and complementary methods. Exercise along with rest is the basis for treating chronic musculoskeletal pain. Side effects of physical therapy are rare and, in addition, it is not contraindicated in pregnant women. The benefits of this type of treatment in combating pain far outweigh the risks, being an easy way to improve quality of life. The objective of this article is to discuss the management of musculoskeletal pain during pregnancy, to identify the main musculoskeletal pain encountered in pregnant women along with drug treatment, and to expose the beneficial effects of alternative and complementary methods in combating pain. Materials and Methods: A literature search was conducted using medical databases, including PubMed, Google Scholar, and ScienceDirect, using the keywords “changes of pregnancy”, “musculoskeletal pain”, “pregnancy pain”, “pain management”, “pharmacological approach”, “alternative and complementary treatment” and specific sites. Information was collected from studies whose target population included pregnant women who complained of musculoskeletal pain during the 9 months of pregnancy; pregnant women with other pathologies that could increase their pain were not included in this review. Results: The articles related to the most common non-obstetric musculoskeletal pain in pregnancy along with pharmacological treatment options and alternative and complementary methods for musculoskeletal pain management during pregnancy were selected. Conclusions: The results were used to guide information towards the safest methods of therapy but also to raise awareness of the treatment criteria in order to compare the effectiveness of existing methods. Treatment must consider the implications for the mother and fetus, optimizing non-pharmacological therapeutic options.

34. A Conservative Combined Laser Cryoimmunotherapy Treatment vs. Surgical Excision for Basal Cell Carcinoma

Abstract

Surgical excision is the standard treatment for basal cell carcinoma (BCC), but it can be challenging in elderly patients and patients with comorbidities. The non-surgical guidelines procedures are usually regarded as monotherapy options. This quasi-experimental, non-randomized, comparative effectiveness study aims to evaluate the efficacy of a combined, conservative, non-surgical BCC treatment, and compare it to standard surgical excision. Patients with primary, non-ulcerated, histopathologically confirmed BCCs were divided into a conservative treatment (129 patients) and a standard surgery subgroup (50 patients). The conservative treatment consisted of ablative CO2 laser, cryosurgery, topical occlusive 5-fluorouracil, and imiquimod. The follow-up examinations were performed 3 months after remission, then every 3 to 6 months, and were extended with telephone follow-ups. Cosmetic-self assessment was recorded during a telephone follow-up. Subjects from the conservative subgroup presented a clearance rate of 99.11%, and a recurrence rate of 0.98%. No recurrences were recorded in the surgical group, nor during the telephone follow-up. There were no differences regarding adverse events (p > 0.05). A superior self-assessment cosmetic outcome was obtained using the conservative method (p < 0.001). This conservative treatment is suitable for elders and patients with comorbidities, is not inferior to surgery in terms of clearance, relapses, or local adverse events, and displays superior cosmetic outcomes.

The problem at the European level related to the dependence on battery suppliers from Asia directly correlates with the need to use batteries as energy storage media for energy from renewable sources (photovoltaic and wind), and leads to the need for research into the possibilities for their reuse, remanufacturing or recycling (at the end of their life or purpose of use), and reintroduction, either fully or partially, back into the economy. This article presents possibilities for increasing the protection of the integrity of the cells that form a battery in the event of an impact/road accident, by the numerical analysis of a topographically optimized battery module case. The proposed solution/method is innovative and offers a cell protection efficiency of between 16.6–60% (19.7% to 40.7% if the mean values for all three impact velocities are considered). The efficiency of a cell’s protection decreases with the increase in impact velocity and provides the premise for a greater part of the saved cells to be reintegrated into other energy storage systems (photovoltaic and/or wind), avoiding future problems relating to environmental pollution.

Abstract

Background The Oesophago-Gastric Anastomosis Audit (OGAA) is an international collaborative group set up to study anastomotic leak outcomes after oesophagectomy for cancer. This Delphi study aimed to prioritize future research areas of unmet clinical need in RCTs to reduce anastomotic leaks. Methods A modified Delphi process was overseen by the OGAA committee, national leads, and engaged clinicians from high-income countries (HICs) and low/middle-income countries (LMICs). A three-stage iterative process was used to prioritize research topics, including a scoping systematic review (stage 1), and two rounds of anonymous electronic voting (stages 2 and 3) addressing research priority and ability to recruit. Stratified analyses were performed by country income. Results In stage 1, the steering committee proposed research topics across six domains: preoperative optimization, surgical oncology, technical approach, anastomotic technique, enhanced recovery and nutrition, and management of leaks. In stages 2 and stage 3, 192 and 171 respondents respectively participated in online voting. Prioritized research topics include prehabilitation, anastomotic technique, and timing of surgery after neoadjuvant chemo(radio)therapy. Stratified analyses by country income demonstrated no significant differences in research priorities between HICs and LMICs. However, for ability to recruit, there were significant differences between LMICs and HICs for themes related to the technical approach (minimally invasive, width of gastric tube, ischaemic preconditioning) and location of the anastomosis. Conclusion Several areas of research priority are consistent across LMICs and HICs, but discrepancies in ability to recruit by country income will inform future study design.

Abstract

Since the prevalence of heart failure (HF) increases with age, HF is now one of the most common reasons for the hospitalization of elderly people. Although the treatment strategies and overall outcomes of HF patients have improved over time, hospitalization and mortality rates remain elevated, especially in developed countries where populations are aging. Therefore, this paper is intended to be a valuable multidisciplinary source of information for both doctors (cardiologists and general physicians) and pharmacists in order to decrease the morbidity and mortality of heart failure patients. We address several aspects regarding pharmacological treatment (including new approaches in HF treatment strategies [sacubitril/valsartan combination and sodium glucose co-transporter-2 inhibitors]), as well as the particularities of patients (age-induced changes and sex differences) and treatment (pharmacokinetic and pharmacodynamic changes in drugs; cardiorenal syndrome). The article also highlights several drugs and food supplements that may worsen the prognosis of HF patients and discusses some potential drug–drug interactions, their consequences and recommendations for health care providers, as well as the risks of adverse drug reactions and treatment discontinuation, as an interdisciplinary approach to treatment is essential for HF patients.

Abstract

Hemoglobin- (Hb-) based oxygen carriers (HBOC) have for several decades been explored for treatment of hemorrhage. In our previous top-up tests, HBOC with lower in vitro prooxidant reactivity (incorporating a peroxidase or serum albumin to this end) showed a measurable but small improvement of oxidative stress-related parameters. Here, such HBOCs are tested in a hemorrhage set-up; ovine hemoglobin is also tested for the first time in such a setting, based on in vitro data showing its improved performance versus bovine Hb against oxidative and nitrosative stress agents. Indeed, ovine Hb performs better than bovine Hb in terms of survival rates, arterial tension, immunology, and histology. On the other hand, unlike in the top-up models, where the nonheme peroxidase rubrerythrin as well as bovine serum albumin copolymerized with Hb were shown to improve the performance of HBOC, in the present hemorrhage models rubrerythrin fails dramatically as HBOC ingredient (with a distinct immunological reaction), whereas serum albumin appears not feasible if its source is a different species (i.e., bovine serum albumin fares distinctly worse than rat serum albumin, in HBOC transfusions in rats). An effect of the matrix in which the HBOCs are dissolved (PBS versus gelofusine versus plasma) is noted.

Abstract

Nonlinear vibration problems are generally of great importance in physics, mechanical structures, and other engineering research. This type of equation is very difficult and time-consuming. Researchers, therefore, focus on analytical and numerical methods; In this paper, using the new Newton-Harmonic Balance analytical method, which includes a combination of Newton methods and the harmonic balance method, the common Homotopy Perturbation method is used to solve the nonlinear equation of human tympanic membrane vibration and are compared and validated.
