The isocitrate dehydrogenase enzyme 2 (IDH2) gene is mutated in ∼5% of patients with myelodysplastic syndrome (MDS). Enasidenib is an oral, selective, mutant IDH2 inhibitor approved for IDH2-mutated (mIDH2) relapsed/refractory acute myeloid leukemia. We designed a 2-arm multicenter study to evaluate safety and efficacy of (A) the combination of enasidenib with azacitidine for newly diagnosed mIDH2 MDS, and (B) enasidenib monotherapy for mIDH2 MDS after prior hypomethylating agent (HMA) therapy. Fifty patients with mIDH2 MDS enrolled: 27 in arm A and 23 in arm B. Median age of patients was 73 years. The most common adverse events were neutropenia (40%), nausea (36%), constipation (32%), and fatigue (26%). Hyperbilirubinemia from off-target UGT1A1 inhibition occurred in 14% of patients (8%; grades 3 and 4), and IDH-inhibitor–associated differentiation syndrome (IDH-DS) in 8 patients (16%). In the combination arm, the overall response rate (ORR: complete remission [CR] + marrow CR [mCR] + partial remission) was 74%, including 70% composite CR (CRc: CR + mCR). Median time to best response was 1 month (range, 1-4), and a median of 4 cycles was received (1-32). The median overall survival (OS) was 26 months (range, 14 to not reached). In the enasidenib monotherapy cohort after HMA failure, ORR and CRc were both 35% (n = 8), with 22% CR (n = 5). Median time to first response was 27 days, and time to best response was 4.6 months (2.7-7.6 months). A median of 7 cycles was received (range, 1-29), and the median OS was 20 months (range, 11 to not reached). Enasidenib is an effective treatment option for mIDH2 MDS, both in combination with azacitidine for treatment-naïve high-risk MDS, and as a single agent after prior HMA therapy. The article widely reviewed the variation of the heat transfer characteristics and fluid flow of various nanofluids based on physical and chemical parameters like velocity, geometry, viscosity, friction factor, and pressure drop. It also shed light on the stability of these nanofluids in various conditions. The article mainly focuses on the effects on Reynolds number and Nusselt number, thermal changes in the environment and the cooling solution used for nanofluids, and the dependency of concentration of nanoparticles in the working fluid. Apart from this, it also discusses the geometry in which the fluid is kept and the motion or forces it experiences and simulations to observe and analyse the flow of fluid and heat through these nanofluids. Also, this article presents the improvement in the pool boiling heat transfer rates through nanofluids with twisted tapes and corrugated patterns such as corrugated double-tube exchangers. This article concluded with the results obtained from experimental analysis and numerical methods. According to the study, as nanofluids get bigger, their velocity increases. When particle size is increased from 10 nm to 100 nm, the alumina-water nanofluid’s velocity rises by 22.22%. For Al2O3/water nanofluid with a particle size of 10 nm, the rate of expansion in wall shear stress when concentration is raised from 0% to 5% is 75%. The geometry of the tubes affects the properties of heat transport. When a triangular tube having a twisted tape is utilized in the system, the Nusselt number is enhanced by 34.7% and 52.5% in turbulent and laminar flow respectively.

Posit number system has been used in many applications, especially the deep learning. Because of how well its nonuniform number distribution aligns with deep learning's data distribution, deep learning's training process can be sped up. The hardware multiplier is typically built with the widest mantissa bit-width available due to the flexibility of posit numbers' bitwidth. Such multiplier designs consume a lot of power since the mantissa bit-width is not necessarily the maximum value. This is especially true when the mantissa bit-width is tiny. The mantissa multiplier is still built to have the widest bit-width feasible, but it is broken into numerous smaller multipliers. At run-time, just the necessary tiny multipliers are turned on. The regime bitwidth, which can be used to determine the mantissa bit-width, controls those smaller multipliers. This design technique is applied to 8-bit, 16-bit, and 32-bit posit formats.

In this study an attempt was made to formulate cosmetic gel using Maranta arundinacea (arrowroot powder) Arrowroot contains flavonoids which is known for its anti-inflammatory properties, antibacterial properties from long time. It has earned a reputation as an elegant, lightweight, all-natural absorbent powder with added skin-nourishing, reduce oiliness, anti-inflammatory properties, anti-irritant properties and for treating wounds from historical times. The rhizome of the plant was processed to obtain arrowroot powder. The cosmetic gel was developed using two polymers carbopol 934 and sodium carboxy methyl cellulose (sodium CMC) in different ratios as gelling agents using dispersion method. The formulated gel was evaluated for various physicochemical factors like physical appearance, homogeneity, spread ability, pH and viscosity measurements. Cosmetic gel prepared using carbopol 934 by dispersion method in 1: 5 ratio showed better results in all parameters compared with sodium carboxy methyl cellulose. The best formulation F3 was tested for its antimicrobial property against Staphylococcus aureus by comparing the zone of inhibition with the standard drug (Azithromycin) and blank gel. F3 formulation showed a zone of inhibition of 15.2 mm above the minimum inhibitory concentration. Stability studies for 3 months gave promising results. Thus, cosmetic gel of arrow root with Carbopol 934 in the ratio of 1:5 was found to be ideal gel for acne treatment and skin rejuvenating purpose. Web content mining retrieves the information from web in more structured forms. The page rank plays an essential part in web content mining process. Whenever user searches for any information on web, the relevant information is shown at top of list through page ranking. Many existing page ranking algorithms were developed and failed to rank the web pages in accurate manner through minimum time feeding. In direction to address the above mentioned issues, Lancaster Stem Sammon Projective Feature Selection based Stochastic eXtreme Gradient Boost Clustering (LSSPFS-SXGBC) Approach is introduced for page ranking based on user query. LSSPFS-SXGBC Approach has three processes for performing efficient web page ranking, namely preprocessing, feature selection and clustering. LSSPFS-SXGBC Approach in account of the numeral of operator request by way of an input. Lancaster Stemming Preprocessed Analysis is carried out in LSSPFS-SXGBC Approach for removing the noisy data from the input query. It eradicates the stem words, stop words and incomplete data for minimizing the time and space consumption. Sammon Projective Feature Selection Process is carried out in LSSPFS-SXGBC Approach to select the relevant features (i.e., keywords) based on user needs for efficient page ranking. Sammon Projection maps the high-dimensional space to lower dimensionality space to preserve the inter-point distance structure. After feature selection, Stochastic eXtreme Gradient Boost Page Rank Clustering process is carried out to cluster the similar keyword web pages based on their rank. Gradient Boost Page Rank Cluster is an ensemble of several weak clusters (i.e., X-means cluster). X-means cluster partitions the web pages into ‘x’ numeral of clusters where each reflection goes towards the cluster through adjacent mean value. For every weak cluster, selected features are considered as the training samples. Subsequently, all weak clusters are joined to form the strong cluster for attaining the webpage ranking results. By this way, an efficient page ranking is carried out through higher accurateness and minimum time consumption. The practical validation is carried out in LSSPFS-SXGBC Approach on factors such ranking accurateness, false positive rate, ranking time and space complexity with respect to numeral of user query.

9. Are lipid ratios better indices for evaluating hyperlipidemia? A prospective observational study from a rural region of North Kerala

Introduction and Aim: Historically lipid profile has been used to evaluate hyperlipidemia. More recently lipid ratios have been used particularly in studies from the west. The aim of this study is to evaluate dyslipidemia using both the conventional lipid profile and lipid ratios and to assess its relationship with fatty liver disease. Materials and Methods: Fasting lipid profile, lipid ratios such as the Castelli Risk Index-I (CR-I TC/HDL), Castelli Risk Index II (CR-II LDL/HDL), atherogenic coefficient (non-HDL/HDL) were assessed in our study population of 150 subjects. Any changes noted in serum lipids were correlated with ultrasonography of the liver. Results: Significant changes in lipid ratios such as CR-II was noted in the age group 50-59 years and was statistically significant (p=0.053).CR-I, CR-II and AC indicated a cardiovascular risk prevalence of 22.6%,36.7% and 50.7% respectively. The two ratios which were significantly altered across the various subtypes of fatty liver were LDL/HDL(p=0.018) and non-HDL/HDL (p=0.00). Conclusion: This study emphasizes the importance of lipid ratios in early screening.

COVID-19 has overwhelmed health services globally. Oral antiviral therapies are licensed worldwide, but indications and efficacy rates vary. We aimed to evaluate the safety and efficacy of oral favipiravir in patients hospitalised with COVID-19.We conducted a multicentre, open-label, randomised controlled trial of oral favipiravir in adult patients who were newly admitted to hospital with proven or suspected COVID-19 across five sites in the UK (n=2), Brazil (n=2) and Mexico (n=1). Using a permuted block design, eligible and consenting participants were randomly assigned (1:1) to receive oral favipiravir (1800 mg twice daily for 1 day; 800 mg twice daily for 9 days) plus standard care, or standard care alone. All caregivers and patients were aware of allocation and those analysing data were aware of the treatment groups. The prespecified primary outcome was the time from randomisation to recovery, censored at 28 days, which was assessed using an intention-to-treat approach. Post-hoc analyses were used to assess the efficacy of favipiravir in patients aged younger than 60 years, and in patients aged 60 years and older. The trial was registered with clinicaltrials.gov, NCT04373733.Between May 5, 2020 and May 26, 2021, we assessed 503 patients for eligibility, of whom 499 were randomly assigned to favipiravir and standard care (n=251) or standard care alone (n=248). There was no significant difference between those who received favipiravir and standard care, relative to those who received standard care alone in time to recovery in the overall study population (hazard ratio [HR] 1·06 [95% CI 0·89-1·27]; n=499; p=0·52). Post-hoc analyses showed a faster rate of recovery in patients younger than 60 years who received favipiravir and standard care versus those who had standard care alone (HR 1·35 [1·06-1·72]; n=247; p=0·01). 36 serious adverse events were observed in 27 (11%) of 251 patients administered favipiravir and standard care, and 33 events were observed in 27 (11%) of 248 patients receiving standard care alone, with infectious, respiratory, and cardiovascular events being the most numerous. There was no significant between-group difference in serious adverse events per patient (p=0·87).Favipiravir does not improve clinical outcomes in all patients admitted to hospital with COVID-19, however, patients younger than 60 years might have a beneficial clinical response. The indiscriminate use of favipiravir globally should be cautioned, and further high-quality studies of antiviral agents, and their potential treatment combinations, are warranted in COVID-19.LifeArc and CW+.

13. The catalytic domains of Streptococcus mutans glucosyltransferases: a structural analysis

Streptococcus mutans, found in the human oral cavity, is a significant contributor to the pathogenesis of dental caries. This bacterium expresses three genetically distinct types of glucosyltransferases named GtfB (GTF-I), GtfC (GTF-SI) and GtfD (GTF-S) that play critical roles in the development of dental plaque. The catalytic domains of GtfB, GtfC and GtfD contain conserved active-site residues for the overall enzymatic activity that relate to hydrolytic glycosidic cleavage of sucrose to glucose and fructose, release of fructose and generation of a glycosyl-enzyme intermediate in the reducing end. In a subsequent transglycosylation step, the glucosyl moiety is transferred to the nonreducing end of an acceptor to form a growing glucan polymer chain made up of glucose molecules. It has been proposed that both sucrose breakdown and glucan synthesis occur in the same active site of the catalytic domain, although the active site does not appear to be large enough to accommodate both functions. These three enzymes belong to glycoside hydrolase family 70 (GH70), which shows homology to glycoside hydrolase family 13 (GH13). GtfC synthesizes both soluble and insoluble glucans (α-1,3 and α-1,6 glycosidic linkages), while GtfB and GtfD synthesize only insoluble or soluble glucans, respectively. Here, crystal structures of the catalytic domains of GtfB and GtfD are reported. These structures are compared with previously determined structures of the catalytic domain of GtfC. With this work, apo structures and inhibitor-complex structures with acarbose are now available for the catalytic domains of GtfC and GtfB. The structure of GtfC with maltose allows further identification and comparison of active-site residues. A model of sucrose binding to GtfB is also included. The new structure of the catalytic domain of GtfD affords a structural comparison of the three S. mutans glycosyltransferases. Unfortunately, the catalytic domain of GtfD is not complete since crystallization resulted in the structure of a truncated protein lacking approximately 200 N-terminal residues of domain IV.

RATIONALE: Allergic asthma is linked to impaired bronchial epithelial secretion of IFNs, which may be causally linked to the increased risk of viral exacerbations. We have previously shown that allergen immunotherapy (AIT) effectively reduces asthma exacerbations and prevents respiratory infections requiring antibiotics; however, whether AIT alters antiviral immunity is still unknown. OBJECTIVES: To investigate the effect of house dust mite sublingual AIT (HDM-SLIT) on bronchial epithelial antiviral and inflammatory responses in patients with allergic asthma. METHODS: In this double-blind, randomized controlled trial (VITAL [The Effect of Allergen Immunotherapy on Anti-viral Immunity in Patients with Allergic Asthma]), adult patients with HDM allergic asthma received HDM-SLIT 12-SQ or placebo for 24 weeks. Bronchoscopy was performed at baseline and at Week 24, which included sampling for human bronchial epithelial cells. Human bronchial epithelial cells were cultured at baseline and at Week 24 and stimulated with the viral mimic polyinosinic:polycytidylic acid (poly(I:C)). mRNA expression was quantified using qRT-PCR, and protein concentrations were measured using multiplex ELISA. MEASUREMENTS AND MAIN RESULTS: Thirty-nine patients were randomized to HDM-SLIT (n = 20) or placebo (n = 19). HDM-SLIT resulted in increased polyinosinic:polycytidylic acid–induced expression of IFN-β at both the gene (P = 0.009) and protein (P = 0.02) levels. IFN-λ gene expression was also increased (P = 0.03), whereas IL-33 tended to be decreased (P = 0.09). On the other hand, proinflammatory cytokines IL-6 (P = 0.009) and TNF-α (tumor necrosis factor-α) (P = 0.08) increased compared with baseline in the HDM-SLIT group. There were no significant changes in TSLP (thymic stromal lymphopoietin), IL-4, IL-13, and IL-10. CONCLUSIONS: HDM-SLIT improves bronchial epithelial antiviral resistance to viral infection. These results potentially explain the efficacy of HDM-SLIT in reducing exacerbations in allergic asthma. Clinical trial registered with www.clinicaltrials.gov (NCT 04100902).

The Smart Infant Baby Monitoring System is an IOT-based project. The goal of this research is to lower SIDS. The primary factor in infant mortality is sudden infant death syndrome. Infants' breathing, temperature, heart rate, and digestion are abnormal. If people neglect infants, they are very likely to die. With the aid of IOT, this clever initiative will enable parents to provide assistance from anywhere, whether they are at home or at work. The idea of wanting and caring for infants, along with the need for sensors and cloud technologies, plays a vital role. The Internet of Things (IOT) and modules like the Raspberry Pi, temperature sensor, MAX30100 sensor, and sound sensor are all integrated into this project.. A temperature sensor measures the body's temperature, a MAX30100 measures the infant's heart rate and SPO2, a sound sensor detects crying, and a camera in a Raspberry Pi module measures body position. With the help of all those data, parents may check on their child's health from anywhere in the world. When the modules notice something abnormal, they immediately send their parents an SMS using cloud technology. By doing this, we can lower the number of SIDS.

With the goal of achieving universal health coverage (UHC), Karnataka, a national leader in healthcare, launched the Arogya Karnataka scheme in March 2018. It was later integrated with the government of India's national health protection scheme Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB-PMJAY) to help with financial protection for accessing curative care at the secondary and tertiary levels through collaboration with the public and private sectors. The AB-PMJAY is a rare opportunity to enhance the health of hundreds of millions of Indians and get rid of a significant cause of the country's poverty. The study intends to define the State's distinctive characteristics with relation to AB-PMJAY-ArK and to highlight the State's top initiatives through the scheme. Karnataka has a unique scheme even if it is integrated due to various striking features. The country has an opportunity to address persistent and deeply ingrained issues with governance, quality assurance, and stewardship owing to AB-PMJAY-ArK. The complete approach used by the AB-PMJAY-ArK cell of the department of health and family welfare, government of Karnataka has enhanced the performance of public health institutions and can be adopted as a model by all the Indian States. Access to care, compliance, and timely assistance have all drastically enhanced. This has improved national health indices, but more crucially, it has led to the development of a bigger and stronger public health system, leading the road to UHC.

For WLAN/WIMAX applications, a brand-new tree-shaped metamaterial-loaded microstrip antenna is suggested. The reduced ground plane size and 4.4 dielectric constant (r) and 0.02 loss tangent (δ) dielectric are used to manufacture the 15 × 16 × 1.6 mm3 microstrip antenna. Two X-shaped slots are added to achieve the characteristics needed for WIMAX applications at 5.5 GHz. Additionally, a split-ring resonator is added to the structure to increase its bandwidth. It runs for WLAN applications with a center frequency of 5.8 GHz. The proposed structure’s measured impedance bandwidth is 45.39% with SRR and 53.48% without SRR, respectively. The proposed antenna is capable of satisfying the major requirements of modern wireless devices such as multiband operation, compact size, large bandwidth, and planar structure. The best outcomes are attained with the aid of parametric analysis of feed width, ground height, and slot width. All electromagnetic simulations were performed using CST Studio software. The measured results and the simulation agree. The waveguide extraction approach is used to demonstrate the SRR’s permeability property. The suggested antenna had adequate impedance matching, was small, and had a wide bandwidth.

Introduction: Medicinal plants, minerals and some marine products can potentially treat medical conditions in the long term based on error and trial methods. Surangusa parpam, one of the Siddha herbo-mineral formulations, is mentioned in the classical Siddha text and indicated for respiratory illnesses like cough, cold, fever, bronchial asthma and intestinal tuberculosis. Objectives: This study was designed to evaluate the anti-inflammatory activity of the trial drug Surangusa parpam. Materials and Methods: The trial drug Surangusa parpam is prepared as per literature and inflammation is induced in the experimental animal’s carrageenan-induced paw oedema model. The trial drug Surangusa parpam and standard indomethacin are given orally for the anti-inflammatory effects. The anti-inflammatory effects were measured at regular intervals, and the percentage of the inhibition of paw oedema was to compare the effect of the trial medicine with the control and standard. Results: The trial drug showed significant improvement in the percentage inhibition of paw oedema in graded doses compared to control in the carrageenan-induced paw oedema model. 15 and 35 mg/kg of the trial drug showed significant inhibition of paw oedema at 55.9 % and 58.9 % compared with the control. Conclusion: Anti-inflammatory activity is seen with the graded dose of the trial drug. It can be concluded that the herbo-mineral formulation showed significant anti-inflammatory activity.

This communication presents the significance of a four-port 2×2 multiple input multiple output antenna operating in mid-band frequency for 5G handheld device application. A microstrip-fed reference truncated decagonal monopole antenna operating at 3.9 GHz and a modified split-ring resonator (SRR)-based low-pass filter that excites at a cut-off frequency of 6.5 GHz are designed. On introducing a stub line in the SRR structure, the proposed filter shows a good out-of-band rejection beyond 6.5 GHz. By integrating the filter into the reference monopole, a wideband filtering monopole with improved stopband performance is obtained. A four-port MIMO array is constructed by orthogonally positioning the antenna with a center-to-center separation of 0.25λ0. To improve isolation, a pair of inverted L-stubs of varying lengths is introduced. The closed loop formed by the interconnection of stubs enhances isolation within the frequency range of 3–3.5 GHz. The fabricated prototype fosters an impedance bandwidth of 66.67%, isolation >18.5 dB, an average suppression of 15 dB, an average peak gain >3.7 dB, envelope correlation coefficient < 0.015, Actual Diversity Gain (ADG) > 9.9 dB, Effective Diversity Gain (EDG) > 8.8 dB, and efficiency >88%. The proposed design is suitable for 5G sub-6 GHz applications such as n48, n77, n78, n79 bands, WiMAX, and LTE bands 42, 43, 46, 47, 48.

28. Interesting case series on distinct histopathological findings of six cases of epididymo-orchitis

Several clinicopathological illnesses impact the testis and paratesticular area, which are divided into non-neoplastic and neoplastic diseases. Despite the availability of imaging and tumor marker testing, histopathological examination is the primary method for diagnosing testicular lesions. Common indications of orchidectomy for non-neoplastic lesions are cryptorchidism, epididymitis and testicular torsion. Few non- neoplastic lesions present as mimickers of neoplasm with commonest being granulomatous orchitis and AFB positivity detected only in 2.1% of cases with bilateral presentation being rarest. We report here a series of six cases of epididymo-orchitis to discuss the histopathological spectrum with varied etiologies. Age of the patients ranged between 33-65yr old males. Amongst six cases, two of them were clinically and radiologically suspected as neoplastic lesions, which were histopathologically diagnosed as granulomatous epididymo-orchitis with one among it showing strong positivity for AFB, and two cases as testicular torsion with orchitis and one of the case was diagnosed as cryptorchid testis with foci of atypical germ cell and last case was reported as mild non-specific orchitis, for surgical castration. In this study, we also noted unilateral presentation being the commonest, specifically of Right side. Knowledge of histopathological spectrum of epididymo-orchitis is of prime importance to detect underlying specific etiology for targeted therapy and prevent undue orchidectomy.In this study, two cases which were clinically suspected as neoplastic lesions, were confirmed as granulomatous epididymo-orchitis on histopathological examination with one case showing strong AFB positivity.In this study, we noted late presentation (5th decade) of cryptorchidism with some atypical germ cells which are forerunner of malignancy.

- Abstract
Mannich bases are beta-amino ketone-carrying compounds that are the end products of the Mannich reaction. The Mannich reaction is essential in the synthesis of nitrogen-containing compounds. The Mannich reaction, a nucleophilic addition reaction that forms carbon-carbon bonds, is employed in the synthesis of numerous natural products, drugs and other compounds. There are several aminoalkyl chain bearing Mannich bases with high curative value, such as fluoxetine, atropine, ethacrynic acid, trihexyphenidyl, and many others. According to the literature, Mannich bases are highly reactive and have been demonstrated to have powerful anti-inflammatory, anticancer, antifilarial, antibacterial, antifungal, anticonvulsant, anthelmintic, antitubercular, analgesic, anti-HIV, antimalarial, antipsychotic, antiviral, antilipase activity and other properties. The primary source of, -α,β-unsaturated ketone, which can be produced by deaminating the hydrogen atom of the amine group, is responsible for the biological action of Mannich bases. However, a number of minor biological functions of Mannich bases have been reported, including their ability to control blood pressure or prevent platelet aggregation, their impact on parasites and ulcers, and their use as medications for mental health issues. Since there is a greater need for anticonvulsant pharmaceuticals, these anticonvulsant medications containing N-mannich base groups will exhibit an increase in lipophilic nature and hence have improved blood-brain barrier penetration. The overview concludes with a brief explanation of the potential of Mannich bases as inhibitors of various enzymes or ligands for various receptors.

Seed Production of rice fallow blackgram variety, ADT3 has limitations due to inherent issues of soil fertility, climate change and thermo sensitivity results in poor flowering, reduced pod setting and poor seed filling. Macro and micro nutrients application during critical stages of growth were tried to improve the seed yield and quality in rice fallow blackgram variety ADT3. The combined foliar spraying of 0.5% NPK (macro) and 0.2% chelated micro nutrients mixture (Fe 2.5%, Mn 1.0%, Zn 3.0%, Cu 1.0%, Mo 0.1% and B 0.5%), were sprayed on 15th (Vegetative phase), 30th (Reproductive phase), 45th ( Flowering and pod formation phase) days after germination improved the growth and seed parameters viz., plant height, number of branches, number of pods per plant, number of seeds per pod, filled seed % which resulted in increased seed yield in rice fallow blackgram variety, ADT3. Response of macro and micro nutrients through foliar mode revealed the lack of soil fertility resulting in poor uptake of macro and micro nutrients. Foliar application induced uniform flowering and synchronous maturity of pods with least shattering which would facilitate mechanical harvesting. Foliar application of macro and micro nutrients phenotypically altered the source-sink which greatly influenced the yield of seed, germination and seedling quality parameters.

This manuscript proposes a novel nonisolated negative output buck-boost converter topology for wide voltage conversion applications. To design this converter, a typical buck-boost converter configuration is used. In conventional buck-boost converter, the active switches designed are replaced by two switches-two inductors (2S2L) cells. The proposed converter operates in the continuous conduction mode (CCM) operation under steady-state conditions. This converter has a lower component count and low voltage stress on the switches and diodes. Moreover, the major advantage of this buck-boost converter topology is that a wide range of step-down and step-up voltage conversions can be achieved. The performance of the proposed system is designed in MATLAB/SIMULINK. A few other comparisons are also presented to demonstrate the competitiveness of the proposed buck-boost converter.

In digital manipulation, creating fake images/videos or swapping face images/videos with another person is done by using a deep learning algorithm is termed deep fake. Fake pornography is a harmful one because of the inclusion of fake content in the hoaxes, fake news, and fraud things in the financial. The Deep Learning technique is an effective tool in the detection of deep fake images or videos. With the advancement of Generative adversarial networks (GAN) in the deep learning techniques, deep fake has become an essential one in the social media platform. This may threaten the public, therefore detection of deep fake images/videos is needed. For detecting the forged images/videos, many research works have been done and those methods are inefficient in the detection of new threats or newly created forgery images or videos, and also consumption time is high. Therefore, this paper focused on the detection of different types of fake images or videos using Fuzzy Fisher face with Capsule dual graph (FFF-CDG). The data set used in this work is FFHQ, 100K-Faces DFFD, VGG-Face2, and Wild Deep fake. The accuracy for FFHQ datasets, the existing and proposed systems obtained the accuracy of 81.5%, 89.32%, 91.35%, and 95.82% respectively.

An excellent method with simple, precise was developed for Nebivolol HCl and Valsartan by using Forced degradation UPLC method. The column used was C-18 BEH _ 1.7µm x 2.1 x 50mm in ambient temperature. Flow rate was 0.8ml/min, wavelength of 278nm, mobile phase used was acetonitrile : Buffer (50:50). Buffer used 0.01 N Disodium hydrogen phosphate with pH 3.5 adjusted by OPA. Run time 4 min. The percentage purity and RT of Nebivolol HCl and Valsartan was found to be 99.72 and 99.30 and 1.193 and 1.827 respectively. The validation parameters was carried out, linearity of Nebivolol HCl was found to be (1μgm/ml to 8μgm/ml) R2= 0.998 and Valsartan was found to be (16μgm/ml to 128.8μgm/ml) R2= 0.999. Intermediate precision, Robustness, LOD LOQ was within the limit as per ICH guidelines. Recovery studies taken place in 80%, 100% and 120. Forced Degradation was carried out in three conditions acidic, basic and peroxide condition, degradation takes at basic and peroxide. As per literature review there is no method developed for Nebivolol HCl and Valsartan in Forced degradation UPLC method. So we made attempt to develop the Nebivolol HCl and Valsartan in UPLC method.

Recently, Panama wilt disease that attacks banana leaves has caused enormous economic losses to farmers. Early detection of this disease and necessary preventive measures can avoid economic damage. This paper proposes an improved method to predict Panama wilt disease based on symptoms using an agro deep learning algorithm. The proposed deep learning model for detecting Panama wilts disease is essential because it can help accurately identify infected plants in a timely manner. It can be instrumental in large-scale agricultural operations where Panama wilts disease could spread quickly and cause significant crop loss. Additionally, deep learning models can be used to monitor the effectiveness of treatments and help farmers make informed decisions about how to manage the disease best. This method is designed to predict the severity of the disease and its consequences based on the arrangement of color and shape changes in banana leaves. The present proposed method is compared with its previous methods, and it achieved 91.56% accuracy, 91.61% precision, 88.56% recall and 81.56% F1-score.

A recommendation System (RS) is an emerging technology to figure out the user’s interests and intentions. As the amount of data increases exponentially, it is hard to analyze the user intentions and trigger the recommendation accordingly. In this research work, a novel recommendation system called the Deep Knowledge Graph based Attribute Preserving Recommendation (DKG-APR) is presented to analyze massive data and provide personalized recommendations to users. The Deep Knowledge Graph for Recommendation System (DKG-RS) uses Deep Convolutional Neural Network (DCNN) and attention mechanism to explicitly model high-order connections in knowledge graphs. According to empirical findings, Knowledge Graph Attention Network (KGAT) performs better than other state-of-the-art recommendation techniques like RippleNet and Neural FM. Additional research demonstrates the effectiveness of embedding propagation for high-order relation modeling and the advantages of the attention mechanism for interpretability.The results also show that user information is crucial in the recommendation system, as seen from the optimal node-drop-out ratio of 0.2, which led to the best recall value of 0.2 for all datasets.

Mutations in Nucleophosmin 1 (NPM1) are associated with a favorable prognosis in newly diagnosed acute myeloid leukemia (AML), however, their prognostic impact in relapsed/refractory (R/R) settings are unknown. In a retrospective analysis, we identified 206 patients (12%) with mutated NPM1 (NPM1c) and compared their outcomes to 1516 patients (88%) with NPM1 wild-type (NPM1wt). NPM1c was associated with higher rates of complete remission or complete remission with incomplete count recovery compared with NPM1wt following each line of salvage therapy (first salvage, 56% vs 37%; P < .0001; second salvage, 33% vs 22%; P = .02; third salvage, 24% vs 14%; P = .02). However, NPM1 mutations had no impact on relapse-free survival (RFS) and overall survival (OS) with each salvage therapy with a median OS following salvage 1, 2 or 3 therapies in NPM1c vs NPM1wt of 7.8 vs 6.0; 5.3 vs 4.1; and 3.5 vs 3.6 months, respectively. Notably, the addition of venetoclax to salvage regimens in patients with NPM1c improved RFS and OS (median RFS, 15.8 vs 4.6 months; P = .05; median OS, 14.7 vs 5.9 months; P = .02). In conclusion, NPM1 mutational status has a minimal impact on prognosis in relapsed or refractory AML; therefore, novel treatment strategies are required to improve outcomes in this entity.

43. Impact of the COVID 19 pandemic on the mental health and quality of life among older adults in India

- Abstract
The Coronavirus disease 2019 (COVID-19) pandemic rapidly spread across continents causing widespread mortality. Older adults, especially those with underlying diseases, are more susceptible for COVID-19 infection. Due to this concern, reverse quarantine was adopted as a protective strategy. This can lead to several psychological and physical detrimental effects. Aims:1). To assess the mental health status (depression and anxiety) among elderly people in the community during the COVID-19 pandemic.2) To assess the impact of COVID-19 pandemic restrictions and mental health issues on quality of life. This is a descriptive cross-sectional study among older adults (>60 yrs) residing in urban community in Central Kerala, India. Of the 200 patients, more than half had moderate to high levels of anxiety and a poor social support. Depression was seen in only 1/4th of patients and that was significant in older women. Mean knowledge about COVID was below 80%. Social support was inversely associated with anxiety and depression. Anxiety and depression significantly affected QOL scores. This study emphasizes on the need for better mental health programmes tailored for our geriatric population. Joint multidisciplinary action plans with focus on enhancing social support can improve the quality of life of this vulnerable group.

Due to decentralized infrastructure in modern Internet-of-Things (IoT), the tasks should be shared around the edge devices via network resources and traffic prioritizations, which weaken the information interoperability. To solve this issue, a Minimized upgrading batch Virtual Machine (VM) Scheduling and Bandwidth Planning (MSBP) was adopted to reduce the amount of batches needed to complete the system-scale upgrade and allocate the bandwidth for VM migration matrices. But, the suboptimal use of VM and possible loss of tasks may provide inadequate Resource Allocation (RA). Hence, this article proposes an MSBP with the Priority-based Task Scheduling (MSBP-PTS) algorithm to allocate the tasks in a prioritized way and maximize the profit by deciding which request must handle by the edge itself or send to the cloud server. At first, every incoming request in its nearest fog server is allocated and processed by the priority scheduling unit. Few requests are reallocated to other fog servers when there is an inadequate resource accessible for providing the request within its time limit. Then, the request is sent to the cloud if the fog node doesn’t have adequate resources, which reduces the response time. However, the MSBP is the heuristics solution which is complex and does not ensure the global best solutions. Therefore, the MSBP-PTS is improved by adopting an Optimization of RA (MSBP-PTS-ORA) algorithm, which utilizes the Krill Herd (KH) optimization instead of heuristic solutions for RA. The simulation outcomes also demonstrate that the MSBP-PTS-ORA achieve a sustainable network more effectively than other traditional algorithms.

46. Prevalence and Detection of Multidrug Resistance Bacterial Strains Isolated from the Different Inanimate Surfaces of the Hospital Environment

- Abstract
Nosocomial infections or Hospital-acquired infections (HAI) influence the clinical outcomes in hospital in-patients and indicate a serious global concern in medicine. Fomites of hospital environments carry a deadly pathogen and transmit infectious diseases. The emergence of antimicrobial resistance (AMR) in the hospital environment has increased due to misuse and/or overuse of antibiotics. The present study was aimed to evidence the MDR bacterial pathogens from the fomites of the hospital environment. The study was conducted in Acute care hospital, in Cuddalore district, Tamil Nadu, India. Totally 65 samples were collected from the different areas of the hospital including Operation Theatre (OT), Orthopaedics Surgery (OS), Wound and emergency Unit (WEU), Intensive care unit (ICU), Dialysis Unit (DU), Special Ward (SW) from March 2022 to September 2022 and the samples were processed for the isolation of bacteria using Nutrient agar, Macconkey agar and Blood agar. Totally 84 isolates were identified in that 48 and 36 isolates were gram positive and gram-negative respectively. Bacillus spp, Staphylococcus aureus, Streptococcus spp, Micrococcus spp, E. coli, Salmonella spp and the proteus spp. were the common isolates of this study and most of them showed multidrug resistance. In gram-positive isolates Bacillus spp. (22%) was dominant followed by Staphylococcus aureus (21%) and in gram-negative bacterial isolates Salmonella spp. (17%) were dominant followed by E.coli (16%). Among the bacterial isolates, 18% showed multidrug resistance (MDR) followed by 3% and 2% of XDR and PDR respectively. All the studied surfaces of the hospital carried minimum bacterial contamination. So more hygienic practices and effective disinfection practices should be implemented in the studied hospital to prevent the spread of nosocomial infections.

Background. Streptococcus agalactiae apart from being a colonizer in the genital region is also associated with several other invasive infections in all age groups. With the varied distribution of serotypes across different regions of the world, universal vaccination is also unattainable. However, in India, the knowledge of group B Streptococcus (GBS) genotype distribution is deficient. Thus, this study was initiated to add data on this aspect. Methodology. A cross-sectional study was conducted using isolates of group B Streptococcus from all clinical specimens. Along with that, the clinical specimen type and the antibiotic resistance profile of the isolates were correlated with the genotypes recognized through a multiplex PCR assay. Results. Among the 86 isolates subjected to multiplex PCR for genotype identification, five genotypes were identified with genotype Ib as the predominant one (34.9%), followed by III (20.9%), II (16.3%), Ia (12.7%), and V (11.6%). Conclusion. The results demonstrated a correlation of types Ib and III with vaginal colonization and type II with urine specimens in the current study. This preliminary study exhibited the distribution of common genotypes and their antibiotic resistance profiles in various GBS isolates. However, multiple studies across the country with larger sample sizes are needed to validate these findings.

The aim of the present study was to assess the antipyretic activity of Siddha herbo-mineral formulation Surangusa Parpam at the dose level of 15mg/kg and 35mg/kg body weight, orally, in brewer yeast induced fever model Wistar rats. Fever was induced by subcutaneous injection of 10ml/kg of 20% w/v aqueous suspension of brewer’s yeast into the nape of the rat's neck. After eighteen hours feverish animals were treated with Surangusa Parpam 15mg/kg and 35mg/kg body weight, orally, and rectal temperatures were evaluated at 0, 1, 2 and 3 hours post-treatment by inserting a well-lubricated bulb of the clinical thermometer. Surangusa Parpam showed a significant decrease in the elevated body temperature of rats that remained sustained throughout the tested time points from 1 to 3 hours in the used model. 35mg/kg body weight dose level showed significant inhibition of elevated body temperature when compared with the standard control. These results indicate that the Antipyretic activity of Surangusa Parpam and in addition to its well-established anti-inflammatory activity possesses significant antihistamine activity that may be beneficial in symptomatic relief when it is used in the therapy of allergic and inflammatory disorders.

50. The Study of Filamentous Fungi in Potable Water and Its Biofilm Formation in Water Pipeline System

Water is essential for life and it is an inorganic constituent of living matter. Water pipeline systems are sighted as problematic in aquatic habitats in which multiple pathogens are occupied including fungi. They have rigid cell walls containing glucans and chitin. The bodies of fungi comprise filaments called hyphae. These hyphae are split into a mat of interwoven single cells made of mycelium. Fungi can pollute the drinking water system and are responsible for biofilm formation. Biofilms are complex polymers containing many times their dry weight in water. Moisture is essential for biofilm formation. The occurrence of biofilms affects the quality of drinking water. Hence, the present study is aimed at recovering the fungi from drinking water samples and their biofilm formation in the water pipeline system. Drinking water samples such as mineral water, tap water, and RO-purified water are collected from different places. Fungi such as Aspergillus, Penicillium and Mucor were recovered from these samples and most species belong to Aspergillus and Penicillium. Further, the biofilm formation of fungi from cast iron in the pipeline system was detected using fluorescence microscopy and fluorescent in situ hybridization analysis.
