Objectives: To evaluate the clinical features of pediatric non-infectious uveitis (NIU) patients treated with adalimumab (ADA) and the efficacy of ADA in patients unresponsive to conventional immunosuppressive therapy. Materials and Methods: The records of 91 NIU patients aged ≤16 years who received ADA therapy were evaluated retrospectively. The patients’ demographic and clinical characteristics and treatment approaches were recorded. The efficacy of ADA in patients treated for at least 1 year after failure of conventional immunosuppressive treatment was evaluated by comparing the best corrected visual acuity (BCVA), severity of intraocular inflammation, uveitis flare-ups, topical and systemic corticosteroid (CS) use, and central macular thickness (CMT) values before and after ADA treatment. Results: The study included 103 eyes of 53 patients, of whom 29 (54.7%) were female. The mean age at presentation was 8.2±3.4 (range: 3-16) years. The mean follow-up period was 41.6±28.2 (range: 18-120) months. Twenty-six patients (49.0%) had anterior uveitis, 22 (41.5%) had intermediate uveitis, and 5 (9.4%) had panuveitis. The mean duration of ADA treatment was 23.0±13.7 (range: 12-60) months. Uveitis flare-ups developed in only 13 patients (24.5%) while on ADA treatment. Objectives: Behçet uveitis (BU) is a potentially blinding disorder. The main determinant of visual prognosis is early and appropriate treatment that provides rapid suppression of inflammatory attacks, control of subclinical inflammation, and prevention of new attacks. Our study aimed to determine the Turkish uveitis specialists’ approach regarding the treatment choices and management of special situations such as pregnancy, vaccination, and surgical planning in BU patients, and to increase information sharing and raise awareness of issues where knowledge is lacking. Materials and Methods: A web-based survey including 16 questions about the treatment approach in ocular involvement of Behçet’s disease was sent via e-mail to uveitis specialists in Türkiye. Based on the answers of 49 ophthalmologists who responded to the survey, we evaluated the approaches of uveitis specialists in our country to initiating treatment, selecting therapeutic agents, monitoring, switching and stopping treatment, and special situations such as surgical planning, vaccination, and pregnancy in BU patients. Results: Uveitis specialists in our country mostly act in accordance with the guidelines in the decision to start treatment, selection of therapeutic agents, and monitoring the safety of treatment in BU. However, there is a lack of information about the therapeutic approach in pregnancy and vaccination practices. It was also observed that there is no consensus on the precautions to be taken before cataract surgery. Conclusion: Our study has shown that there is a need for more detailed and widespread information sharing on treatment in preparation for ocular surgery, safety monitoring, drug use during pregnancy, and vaccination in BU patients.

Objective: Emergency department (ED) blood product requests are rising, but it is unclear if they are excessive. We aimed to examine the blood product requests and usage rates made by emergency physicians to determine whether the requests for blood products are excessive. Material and Method: This prospective, observational, and single-center study analyzed demographic data indications for blood transfusion, and transfusion rates of patients aged 18 years and older admitted to a tertiary adult emergency department in five months. Results: One thousand four hundred seventy-five blood product requests (with a mean of 6.92 units per patient) were examined. Of 63.1%, these requests were not used. The transfusion rates were 40.89 % for erythrocyte suspension, 25.61% for thrombocyte suspension, and 44.34% for fresh frozen plasma. The main indications for requesting blood products were gastrointestinal bleeding and anemia. Specifically, 30.04% of erythrocyte suspensions, 31.9% of thrombocyte suspensions, and 60.56% of fresh frozen plasma were used for patients with gastrointestinal bleeding. In trauma patients, 12.75% of requested erythrocyte suspensions, 0.083% of thrombocyte suspensions, and 13.89% of fresh frozen plasma were utilized. Conclusion: Excessive requests for blood products in the emergency department can misuse resources. The transfusion committee should develop specialized strategies and increase physician awareness to reduce unnecessary requests and optimize resource utilization.

Amaç: Bu çalışma hemodiyaliz tedavisi uygulanan bireylerin psikososyal bakım gereksinimlerini belirlemek amacıyla yapılmıştır. Bu özel popülasyonun ihtiyaçlarının değerlendirilmesi ve belirlenmesi önemli bir durumdur. Bu çalışmada hemodiyaliz hastalarının yaşadığı zorluklar genel olarak anlatılarak hemodiyaliz alan bireylerin yaşadığı sıkıntılar açık bir şekilde ortaya çıkarılmıştır. Yöntemler: Bu çalışma, betimleyici, fenomenolojik desende nitel olarak yapılmıştır. Çalışmada yarı yapılandırılmış anket formu kullanılmış ve 12 katılımcı çalışmaya dahil edilmiştir. Bulgular: Çalışmada 3 bağlam, 6 tema ve 38 alt tema ortaya çıkmıştır. Hastalığa bağlı gelişen sorunlar (komplikasyonlar ve semptomlar), bakıma yönelik ihtiyaçlar (size bakım vermesini istediğiniz sağlık çalışanı, tedavi ve bakıma yönelik gereksinimler, bakım vericinin özellikleri) hastalıkla başa çıkma mekanizmaları (destek sistemleri ve hastalıkla başa çıkma davranışları) olarak temalar belirlenmiştir. Sonuç: Çalışmada hemodiyaliz hastaları birçok psikososyal ve fiziksel sorunla karşılaştıklarını ifade etmiştir. Hemodiyaliz hastalarının psikososyal gereksinimlerinin belirlenmesi ve buna yönelik müdahalelerin uygulanması hastalığın seyrini olumlu yönde etkileyebilir. Bu psikososyal sorunların belirlenip gerekli müdahalelerin uygulanmasında en önemli rol hemşirelere düşmektedir. Hemşireler diyaliz hastalarının bakımında önemli görev ve sorumluluklara sahiptir. Etkili hemşirelik bakımı hastaların hastalıkla ilgili yan etkilerini azaltabilir ve yaşam kalitesini arttırabilir. Hasta merkezli hemşirelik bakımı hastaların diyaliz bakımının kalitesini ve hasta memnuniyetini artırabilir.

Objectives:To report the ocular findings, laboratory results, and management of patients with tubulointerstitial nephritis and uveitis syndrome (TINU), whose numbers increased during the 2019 coronavirus disease (COVID-19) pandemic.Materials and Methods:Demographic characteristics, ophthalmic examination findings, laboratory results including polymerase chain reaction (PCR) test for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), serum SARS-CoV-2 immunoglobulin G (IgG) antibody, and treatment of patients diagnosed with TINU between March 2020 and March 2022 were evaluated retrospectively.Results:The study included 19 eyes of 10 patients (6 female/4 male). The mean age was 13.5±2.4 years (range: 8-16 years). The mean follow-up duration was 13.5±6.1 months (range: 6-24 months). All patients presented with anterior uveitis. Anterior uveitis was bilateral in 9 patients (90%) and unilateral in 1 patient (10%). Posterior segment findings were normal in 8 patients (80%), and bilateral optic disc edema was observed in only 2 patients (20%). None of the patients had a previous SARS-CoV-2 infection and/or vaccination history. The SARS-CoV-2 PCR test was negative in all patients at presentation. The SARS-CoV-2 IgG antibody test was reactive in 7 patients (70%). Recurrent uveitis developed in 8 patients (80%) during follow-up. Systemic immunomodulatory therapy was required for the control of ocular inflammation in 7 patients (70%) with severe uveitis flare-ups.Conclusion:TINU is a multisystemic autoimmune disease, especially in response to environmental triggering factors such as viral infections. Although TINU is a rare disease, the number of cases increased during the COVID-19 pandemic. SARS-CoV-2 antibodies were detected at a significant rate of 70% in these patients, who did not have a history of SARS-CoV-2 infection and vaccination. Previous asymptomatic SARS-CoV-2 infection in children may be a triggering factor in the development of TINU.

Aim: Disease-related immune dysfunction and/or treatment with immunosuppressive drugs may affect the course of coronavirus disease-2019 (COVID-19) infection in rheumatic patients. The aim of this study was to evaluate the course of COVID-19 infection and predictors of COVID-19 pneumonia in patients with rheumatological disease using biological or targeted therapies. Methods: This cross-sectional study was conducted between April 2022 and July 12, 2022. Demographic and clinical parameters and COVID-19-related data in patients with and without COVID-19 pneumonia were recorded and compared. Logistic regression analyses were performed to identify the predictors of COVID-19-related pneumonia. Results: A total of 110 patients (67 with spondyloarthritis, 25 with rheumatoid arthritis, 8 with familial Mediterranean fever, 5 with Takayasu arteritis, 3 with granulomatosis with polyangiitis, and 2 with Behçet’s disease) were included in the study. The mean age of 110 rheumatic patients was 47.27±12.77 years. Their mean body mass index (BMI) was 29.59±5.59 kg/m2, and 67.3% of them were female. Twenty-one (19.1%) patients had a history of COVID-19 pneumonia. This study examines the history, language, social, and economic transformation of the Yuruks living in the Yamanlar Village located in the Karşıyaka district, where there is relatively little information in the literature despite its proximity to the Izmir city center. Information collected from the Ottoman Era cemetery adjoining the village reveals that the Yamanlar Yuruks have lived here for at least 400 years. Mount Yamanlar, where the village is located, is of great importance because of its ancient history, as it witnessed the founding of Izmir (Smyrna). The social and economic lifestyle in the village has transformed as the city expanded toward residential areas atop Mount Yamanlar. The loss of traditional livelihood practices challenged the villagers and severely influenced their customs, which might be considered valuable pieces of cultural heritage. In this study, we conducted fieldwork, during which various data collection methods and techniques were employed to describe daily life in the village. Within the scope of this study, semi-structured and unstructured interviews were conducted with the local people, and an oral historical study was carried out. With this method, we aimed to present an ethnohistorical description of the Yamanlar Village by trying to establish a balance between the analysis of both written documents and oral data to make sense of the social and economic change it has undergone and to contribute to the literature on Izmir Yuruks.

Objectives:To evaluate the demographic characteristics, clinical findings, and treatment approach of patients with sympathetic ophthalmia (SO).Materials and Methods:The records of 14 patients with SO between 2000 and 2020 were retrospectively reviewed. The patients’ Snellen best corrected visual acuity (BCVA), detailed ophthalmological examination, optical coherence tomography (OCT), enhanced depth imaging-OCT (EDI-OCT), fundus fluorescein angiography findings, and treatment approaches were recorded.Results:The study included the 14 sympathizing eyes of 14 patients with SO (7 female, 7 male). The mean age was 48.5±15.4 years (range: 28-75), and the mean follow-up duration was 55.1±48.7 months (range: 6-204). Ten patients (71%) had a history of ocular trauma and 4 (29%) had a history of ocular surgery. The time to symptom onset in the sympathizing eye after trauma or ocular surgery ranged from 15 days to 60 years. The most common posterior segment findings were optic disc edema (36%) and exudative retinal detachment (36%). In the acute period, the mean choroidal thickness value on EDI-OCT was 716.5±63.6 μm (range: 635-772) and decreased to 296±81.6 μm (range: 240-415) after treatment. Treatment with high-dose systemic corticosteroid was given to 8 patients (57%), azathioprine (AZA) to 7 (50%), AZA and cyclosporine-A combination to 7 (50%), and tumor necrosis factor-alpha inhibitors to 3 patients (21%). Recurrence was observed in 4 patients (29%) during follow-up. At last follow-up, BCVA values were better than 20/50 in 11 (79%) of the sympathizing eyes. Remission was achieved in 13 patients (93%), but 1 patient (7%) lost her vision due to acute retinal necrosis.Conclusion:SO is a bilateral inflammatory disease that presents with granulomatous panuveitis after ocular trauma or surgery. Favorable functional and anatomical results can be obtained with early diagnosis and initiation of appropriate treatment.

The aim of this study is to investigate the breeding biology of Collared pratincole, a migratory bird species in Antalya/Boğazkent (Turkey). There are a lot of habitat types in research area of 200 ha. This speciality serves great biodiversity. Field work was carried out from March to October between 2016 and 2017. 35 individuals and 16 nests were found in the first year and 25 individuals and eight nests in the next year. The egg volume has been calculated according to the formulation V=0.51 X L X W2 while egg index has been figured with regard to EI=100xW/L formulation. According to the hatchling size, the breeding success of the species was 86.7% in 2016 and 85.7% in 2017.Considering the fledgling size, the figures were 46.4% in 2016 and 66.7% in 2017 respectively. Many factors affecting the breeding success of the species were specified in the region. Hooded crows (Corvus cornix), for example, was detected as being the natural predator of the species. Furthermore, the invasion of the breeding areas by humans and human-related activities such as construction, tourism and agriculture also harmfully affects the breeding success. According to our study Antalya/Boğazkent is a very important bird area in Turkey.

The paper aims to determine the effects of some economic, psychological and sociological motivators on motivation levels of employees in furniture industry companies depending on some socio-demographic characteristics. In order to measure their motivation levels, a questionnaire was applied to 100 people working in enterprises that employ 10 or more people, which are thought to apply a motivation system in the Furniture Manufacturers Site in Ankara. The obtained data were processed using package programs for statistical analysis and the data were evaluated. According to the results: The type of production was not effective on employee motivation. While increasing occupational health and safety and giving more responsibility provided more motivation for male employees, the openness of career paths was more effective for female employees. The effect of both factors that "job assurance" and "independent work and performance feedback" effects on employee performance increased as the working time of employees in the current workplace increased.

Introduction:The aim of this study is to determinate the frequency and characteristics of pressure ulcers in critically ill children hospitalized in the pediatric intensive care units (4 centers) in Adana. In addition, it is to identify the risk factors that cause pressure ulcers and to discuss the data in the light of the literature.Methods:For this multi-center, prospective point prevalence study, pediatric intensive care units were visited and demographic data, hospitalization diagnoses, treatment and equipment applied, presence and location of pressure ulcers were recorded.Results:Pressure ulcer was detected in 22.4% (17/76) of the patients. There was no difference between patients with and without pressure ulcer in terms of age, gender, body weight, height, hemoglobin and albumin level, nutritional status, feeding route, repositioning and pressure assessment scale. The mortality score of the patients with pressure ulcer was found to be statistically significantly higher than that of those without pressure ulcer, and the Glasgow Coma score was found to be statistically significantly lower. Inotrope therapy, mechanical ventilation therapy, intubation tube and the presence of edema are the risk factors for pressure ulcer.Conclusion:Despite the precautions taken, pressure ulcers continue to be an important problem for critically ill patients. It is possible to reduce the frequency of pressure ulcers by using valid scales to identify patients at risk for pressure ulcers within the first 8 hours after hospitalization and by customizing preventive care activities according to the patient’s condition.

Aim: Esophagus is the most frequently involved gastrointestinal segment in systemic sclerosis (SS). Gastroesophageal reflux, which occurs with the motility disorder caused by esophageal involvement, has an important place in the course of the disease due to many local and systemic complications. In this study, we investigated the demographic features, endoscopic, manometric, and 24-hour pH monitoring findings of patients with SS. Materials and Methods: Twenty-six patients with SS who presented with dysphagia or heartburn complaints were identified. Patients' files, endoscopic, manometric, and 24-hour ph monitoring findings of the esophagus were examined. Results: All of the patients were symptomatic, 96.1% were women. 46.1% of them applied with the complaint of heartburn and 53.9% with the complaint of dysphagia. The frequency of esophagitis was found to be significantly higher (p = 0.005). Pathological reflux was detected in 90% of the patients who underwent 24-hour pH monitoring and it was significantly higher (p = 0.013). Conclusion: Esophageal involvement is a hallmark manifestation of SS and typically occurs secondary to heartburn and dysphagia.In our society, the incidence of GERD and GERD-related complications is high in SS patients with esophageal symptoms. It is important to refer these patients to experienced gastroenterology centers to be evaluated primarily by endoscopic and then other diagnostic methods.

In this study, Netflix’s original series Freud (Marvin Kren, 2020 – ), adapted from Sigmund Freud’sbiography (1856-1939) was analyzed according to traditional adaptation understanding based on theloyalty to the source text. Freud is an eight-part Austrian-German series in the crime, thriller, mysterygenre covering the early professional years of Freud. The story of the series is created in connectionwith Freud’s psychoanalytic theory. In adaptation, a text can remain faithful during its conversion intoa visual narrative such as a series or a film, or it can be approached through an intertextual perspective.Since Freud was faithfully adapted following psychoanalytical theory, this study preferred essentialistapproaches in engaging the subject. Even though the first season of the series narrates the developmentperiod of Freud’s work, psychoanalytic theory is handled holistically in the analysis part of the study. Asa result of the analysis, it was observed that although mystery and crime events were added to Freud’sbiography, the hierarchy was preserved between the source text and Freud series. Hence, this study aimsto contribute to the existing literature by engaging in a psychoanalytical analysis of an online series inan interdisciplinary way.

Bu çalışmada Netflix’in küresel düzeyde en fazla izlenen The Witcher (2019- ) dizisi 1990’lı yıllardan beri televizyon literatüründe sıklıkla ele alınan “nitelikli televizyon” ve “nitelikli drama” kavramları zemininde incelenmiştir. Dijital medya platformları, televizyonun evrimini derinden etkilemiş ve daha “sinematik” dramalar için fırsat sunmuştur. The Witcher’ın dijital televizyon kültürünün yönelimlerini içeren bir dizi olması örneklem olarak seçilmesinde etkili olmuştur. Çalışmanın temel amacı Netflix’te nitelik kavramının işleyişini sorgulamaktır. Makalede ilk olarak kısa bir tarihçesiyle birlikte nitelikli televizyon kavramı açıklanmış ve nitelikli dramaların karakteristik özellikleri tanıtılmıştır. Sonrasında Netflix’in kendine özgü üretim sürecinin, hikâye anlatıcılığı tarzının, niş izleyici kitlesi yakalama ve üst üste izletmeye teşvik etme eğiliminin nitelikli drama üretimiyle ilişkisi ortaya konulmuştur. Çalışmada, bahsedilen eğilimleri somutlaştırmak üzere The Witcher dizisinin ilk sezonu metin analizine tabi tutulmuştur. Dizi, nitelikli dramaların ayırıcı niteliği haline gelen anlatı karmaşıklığı, estetik değer ve özgünlük gibi kategoriler aracılığıyla değerlendirilmiştir. Robert J. Thompson, Jane Feuer, Robin Nelson, Janet McCabe ve Kim Akass gibi öncü kuramcılara yaslanılarak belirlenen bu kategoriler aracılığıyla dizinin kültürel değerini belirleyen özellikler açığa çıkarılmıştır. Bu bağlamda Netflix platformunun sağladığı olanaklarla birlikte The Witcher’ın anlatı formlarının nitelik değerlerine denk düştüğü ve dijital medya platformlarının nitelikli televizyonda önemli bir geçiş alanı olduğu savunulmuştur.

Toplumsal değişimi anlama söz konusu olduğunda, kültüre ait verileri barındırması ve toplumu yansıtması bakımından diziler önemli bir konumdadır. Televizyon dizilerinde modernlik, eski/yeni, geleneksel/modern gibi çatışmaları iç içe barındıran bakışla yansıtılırken, yeni medya platformlarının ortaya çıkışıyla dizi anlatılarında yeni üslup arayışlarına gidildiği göze çarpmaktadır. Bu yenilikten yola çıkan çalışmada, bireycilik olgusunun günümüzde nasıl görünümlerde sunulduğu araştırılacaktır. Türkiye’de yeni medya platformları için üretilen ilk içeriklerden olan Fi dizisi örneklem olarak seçilmiştir. Son dönemlerde dijital platformların kültürel anlamların dolaşımında önemli bir mecra olarak konumlanması, örneklemin seçilmesinde etkili olmuştur. Bu doğrultuda çalışmada bireyciliğin karakter betimlemelerine nasıl yansıtıldığı, bireycileşmiş kültürde yaşayan insanların duygusal durumları, bireyciliğin kutsanışı, ben-biz çatışması, bencillik, benliğin yeniden keşfi, gibi konular değerlendirilmiştir. Dizi, Geert Hofstede’nin kültürel boyutları aracılığıyla betimsel analize tabi tutulmuştur. Hofstede’nin kolektivizm-bireycilik sınıflandırması, çalışmanın savının güçlenmesinde etkili olmuştur. Sonuç olarak Fi’de neredeyse tüm karakterlerin bireycilik yaklaşımının altını çizen görünümlerde sunulduğu bulgulanmıştır.

Homemade (2020) isimli kısa film antolojisi koronavirüs pandemisi nedeniyle nere-deyse tüm dünyanın karantinada olduğu sırada Netflix tarafından yayınlanmıştır. Farklı yönetmenler tarafından çekilmiş on yedi kısa film, birbirinden bağımsız hikâyelerinden oluşmaktadır. Filmler, pandemi kriziyle birlikte yaşanan “kaygı”, “korku”, “endişe”, “yalnızlık” ve “ölüm” gibi varoluşsal temaları, sanatsal bir dille ifşa etmektedir. Bu dü-şüncelerden yola çıkan çalışmada Homemade’in, Martin Heidegger’in felsefesini bir eylem olarak açığa çıkaran anlatı yapısına sahip olduğu ileri sürülmektedir. Bu doğrultuda, Homemade örneklemi üzerinden Heidegger’in varoluşsal ontolojisinin özcü temalarının bulgulanması amaçlanmaktadır. Çalışmada, Heidegger’in ontolojisi, pandeminin oluş-turduğu kriz ortamındaki insanın duygu ve düşünce dünyasındaki değişimlerle ilişkilen-dirilmiştir. Kısa filmler, Heidegger’in varlığı anlama minvali olarak temellendirdiği Da-sein kavramı ekseninde değerlendirilmiştir. Dasein olmanın temel ereklerinden “dünya-içinde-varlık”, “kaygı-duyan-varlık” ve “ölüme-doğru-varlık” üzerinden karantina ve pandemi süreci tahlil edilmiştir. Bu bağlamda karantinanın, her günkü Dasein’ın kendi varlığının en zati imkanını açığa çıkardığı savunulmaktadır. Dasein izole bir şekilde ya-şarken, başkalarıyla-birlikte-varolduğunu bilerek kendi varoluşunu tesis etme yoluna girmiştir.

Objective: This study was conducted to examine the Nursing Care Perception and Satisfaction Levels of Surgical Patients. Materials and Methods: The sample of this descriptive and cross-sectional study consisted of 300 patients aged 18 and over, who accepted to participate in the study, who were hospitalized at a university hospital surgical clinics between 17 December 2018 and 15 March 2019. Data were collected with the “Patient Information Form” and “Newcastle Nursing Satisfaction Scale” and ”Perception of Nursing Care of Scale” created by the researchers. Percentage, mean, t test, ANOVA, Kruskal Wallis test, Mann-Whitney U test and Pearson’s correlation analysis were used to evaluate the data.This study was supported by Ege University Scientific Research Projects Coordination Unit(Project Number: TLP-2019-20440). Results: The mean satisfaction score of the patients hospitalized in surgical clinics was 79.86 ± 19.31 and the mean score of perception of nursing care was 68.03 ± 9.87. It was found that there was a statistically significant relationship between the total score average of the Newcastle nursing satisfaction scale and the total score average of the perception of nursing care scale (r =, 665; p =, 000). While there was no significant difference between gender, marital status, residence, social security, previous hospital experience and satisfaction with care (p gt; 0.05), a significant difference was found in terms of education level and chronic disease status, type of hospitalization, number of patients in the room. (p lt;0.05). Conclusion: Surgical patients were highly satisfied with nursing care, there is a significant relationship between their perception of nursing care and nursing care satisfaction levels, as the educational level of the patients increased, their perception of satisfaction decreased.

Amaç: Bu çalışma, cerrahi hastalarının hemşirelik bakımını algılayışını ve memnuniyet düzeylerini incelemek amacıyla yapıldı. Gereç, Yöntem: Tanımlayıcı ve kesitsel türdeki bu çalışmanın örneklemini, 17 Aralık 2018- 15 Mart 2019 tarihleri arasında bir üniversite hastanesinin cerrahi kliniklerinde yatan, araştırmaya katılmayı kabul eden, 18 yaş ve üzeri, iletişim kurabilen 300 hasta oluşturdu. Veriler, araştırmacılar tarafından oluşturulan “Hasta Bilgi Formu” ve “Newcastle Hemşirelik Memnuniyet Ölçeği”, “Hastaların Hemşirelik Bakımın Algılayış Ölçeği (HHBAÖ)” kullanılarak toplandı. Verilerin değerlendirilmesinde yüzdelik, ortalama, t testi, ANOVA, Karuskal Wallis testi, Mann-Whitney U testi ve Pearson’s Korelasyon analizi kullanıldı. Bu çalışma “Ege Üniversitesi Bilimsel Araştırma Projeleri (BAP) Koordinasyon Birimi”nce desteklendi. Proje Numarası: TLP-2019-20440. Bulgular: Cerrahi kliniklerde yatan hastaların; hemşirelik bakımından memnuniyet puan ortalaması 79.86±19.31 ve hemşirelik bakımını algılayışı puan ortalaması 68.03±9.87 olarak bulundu. Hastaların Newcastle hemşirelik bakımından memnuniyet ölçeği toplam puan ortalaması ile hemşirelik bakımını algılayışı ölçeği toplam puan ortalaması arasında istatistiksel olarak anlamlı bir ilişki olduğu saptandı (r= ,665; p= ,000). Cinsiyet, medeni durum, yaşadığı yer, sosyal güvence varlığı ve önceki hastane deneyimi ile bakımdan memnuniyet arasında anlamlı bir fark bulunmazken (p>0,05), eğitim düzeyi ve kronik hastalık durumu, hastaneye yatış şekli, odada bulunan hasta sayısı açısından anlamlı fark bulundu (p

Amaç: Bu çalışma, çocuk yoğun bakım ünitemizde yatarak tedavi gören zehirlenme olgularının demografik, epidemiyolojik ve klinik özelliklerini geriye dönük olarak gözden geçirmek ve literatür ile karşılaştırmak amacıyla planlanmıştır. Gereç ve Yöntem: 2015-2019 yılları arasında akut zehirlenme nedeniyle çocuk yoğun bakım ünitesine kabul edilen 581 hastanın kayıtları geriye dönük olarak incelendi.Bulgular: Çalışmaya 581 hasta dahil edildi. Hastaların yaş aralığı 3 ay-18 yaş arasında değişmekteydi ve kız/erkek oranı 1.6 idi. Zehirlenmelerin çoğunluğunun oral yolla (%97.8) gerçekleştiği, %57.1’inin kaza ile, %39.1’inin özkıyım amaçlı ve %3.8’nin ise terapötik nedenlerle zehirlendiği gözlendi. Hastaların % 80.9’u ilaç ilişkili iken, %19.1’i ilaç dışı maddelerle gerçekleşmişti. İlaç ilişkili zehirlenmelerde santral sinir sistemi ilaçları (%40.6) en sık rastlanan ajan olup ilaç dışı nedenlerden en çok fare zehiri ile zehirlenme tespit edildi. Bu çalışmamızda mortalite oranı %0.1 olarak saptandı. Sonuç: Bölgemizde çocuk yoğun bakım ünitesine yatış gerektiren en sık zehirlenme nedenin ilaç zehirlenmeleri olması, öncelikle bu konuda toplumun daha fazla bilinçlendirilmesi gerektiğini göstermektedir. Ayrıca çalışmamızın ilimiz ve çevresinde görülen çocuk yoğun bakım ünitesine yatış gerektiren çocukluk çağı zehirlenme hastalarının epidemiyolojik ve klinik özelliklerini yansıtması nedeniyle görev yapan hekimlere yol göstereceğini ve sonuçlarımızın bölgesel zehirlenme verilerinin güncellenmesine katkıda bulunacağı görüşündeyiz.

Every individual is confronted with numerous events and problems throughout his life, and they can get over most of these problems. However, some of these events affect their life significantly. Unexpected death of a loved one is such an event. Grief can be described as emotional, cognitive, and behavioral reactions given by individuals who lost their loved ones. The acute response after loss of loved one are usually not considered as abnormal because most of them are natural. The grief reactions can show a variety according to age, gender, culture and previous experience of person and because of this the reactions of people when they face to grief can show differences, it is important for healthcare professionals to consider these variety while approaching people experiencing loss. Supportive approach in grief include some techniques like psychoeducation, role modeling, encouragement, counseling and problem solving. Studies show that health professionals are lacking in information about communication with the individual who is experiencing mourning, and coping with mourning process. This review aims to emphasize the importance of the psychosocial approach in grief.

Background:Preparation depth of laminate veneers are essential in retention and structural integrity of restorations. The aim of this study was to evaluate the difference between a trained professional prosthodontist (PP) and a postgraduate prosthodontics student (PPS) in the preparation depth of laminate veneers by scanning the tooth with a 3D scanner before and after the preparation. Methods: Twenty extracted human maxillary central incisors were evaluated by a PP and PPS. A 0.5 mm depth guide bur was used in for facial reduction and a 1 mm incisal reduction was performed with a deep chamfer. The teeth were scanned with a laser scanner at an accuracy of 10 µm.The teeth were separated into 3 parts (incisal, middle and cervical) and then 5 points were randomly selected on the unprepared tooth and then lapped over when the preparation had been completed. The data of the difference was calculated in 3D scanning software Magics (Materialise NV, Belgium).Statistical analysis were performed with one-way ANOVA (p < 0.05).Results: PP results showed mean values in the Incisal of 0.328+0.045 mm, Middle of 0.375+0.097 mm and Cervical of 0.471+0.07 mm. PPS results showed mean values in the Incisal of 0.323+0.056 mm, Middle results of 0.403+0.083 mm and Cervical results of 0.462+0.075 mm.Conclusion: These results suggest that a PP or a PPS can both achieve the same precision in there preparation using a depth guide bur. Giriş:Hemşirelikeğitiminde simülasyon kullanımının amacı, klinik gerçekliği taklit ederek ve öğrenciyiaktif kılarak öğrenim sürecini kolaylaştırmaktır. Simülasyon, psikomotorbecerinin yanı sıra, kognitif becerilerin kullanılmasına ve geliştirilmesine dekatkıda bulunmaktadır. Ülkemizde Cerrahi ve İç Hastalıkları Hemşireliğialanında simülasyon kullanımının sonuçlarını inceleyen sınırlı sayıda çalışmabulunmaktadır.Amaç:Araştırma,Cerrahi ve İç Hastalıkları Hemşireliği dersi kapsamında yürütülen simülasyon uygulamalarınailişkin öğrencilerin görüşlerini ve öğrenme sürecine ilişkin memnuniyet/özgüvendüzeylerini incelemek amacıyla gerçekleştirildi.Yöntem:Retrospektifve tanımlayıcı nitelikteki bu çalışma, Ankara’da bir hemşirelik fakültesinde2018-2019 Eğitim-Öğretim yılında öğrenim gören, toplam 115 ikinci sınıföğrencisiyle yürütüldü. Öğrenciler beşer kişilik gruplara ayrıldı ve her grup10 farklı simülasyon uygulamasına katıldı. Verilerin toplanmasında SimülasyonTasarım Ölçeği ve Öğrenmede Öğrenci Memnuniyeti ve Özgüven Ölçeği kullanıldı. Bulgular:Öğrencilerinuygulanan simülasyonun tasarımı hakkındaki görüşleri incelendiğinde; SimülasyonTasarım Özelliklerinin Uygunluğu (4,53 ± 0,47) ve Simülasyon TasarımÖzelliklerinin Öğrenciye Göre Önemi’ne (4,67 ± 0,41) ilişkin toplam puanortalamalarının oldukça yüksek olduğu bulundu. Öğrencilerin simülasyonuygulamalarına ilişkin öğrenmede memnuniyet ve özgüven puan ortalamalarının dayüksek olduğu değerlendirildi. Simülasyon Tasarım Ölçeği ile Öğrenmede ÖğrenciMemnuniyeti ve Özgüven Ölçeği arasında da pozitif yönde anlamlı bir ilişkiolduğu (r=0,717, p lt;0,001; r=0,747, p lt;0,001) saptandı.Sonuç:ÖğrencilerinCerrahi ve İç Hastalıkları Hemşireliği dersi kapsamında yürütülen simülasyon uygulamalarındançok memnun olduğu ve uygulamanın bu konuya ilişkin özgüvenlerini artırdığı görüldü.Bu dersler kapsamında, özellikle klinik uygulama öncesi öğrencilerin hempsikomotor becerilerini hem de kognitif becerilerini kullanmalarına vegeliştirmelerine olanak tanıyacak, çeşitli senaryolarla zenginleştirilmişsimülasyon uygulamalarına yer verilmesi yararlı olacaktır.

Aim: Lower respiratory tract infections are an important reason for mortality and morbidity in children with congenital heart disease. This study aimed to evaluate 50 children who had congenital heart disease and were hospitalized with lower respiratory tract infection in the Ege University Faculty of Medicine Pediatrics Hospital. Materials and Methods: Fifty pediatric patients were taken into the study. Their clinical symptoms, acute phase reactants, chest X-rays, bacterial culture of transtracheal aspirate, respiratory virus panel (with multiplex polimerase chain reaction) from nasopharyngeal swab were examined. The groups were evaluated in terms of age, gender, enviromental smoke exposure, living with school-aged siblings, Respiratory Syncytial virus (RSV) prophylaxis, hospitalization time, causative pathogen, additional risk factors. Results: Of the 50 cases, 12 (24%) were cyanotic, 38 (76%) were acyanotic. There were 26 boys and 24 girls. The most common diagnosis in the acyanotic group was hemodynamically significant VSD (isolated or with other diagnoses) with 20 cases. The average age of the cyanotic group was 23.88±28.81, and the acyanotic group was 12.25±15.45 months old. Hospitalizations most frequently occured in winter. The most frequent viral agent was RSV, which was not seen in the cyanotic group. All of the RSV infected patients were under 12 months old. In 16.7% of cyanotic and 52.6% of acyanotic patients there were extra risk factors such as immune deficiency, Down syndrome, prematurity, Di George syndrome, cerebral palsy, postoperative early period. Three cases lost their lives due to severe respiratory failure. There was no statistically significant difference between the two groups when compared for demografic variables, risk factors, causative pathogens, hospitalization times. Conclusion: Lower respiratory tract infections and especifically RSV pneumonia are important causes of mortality and morbidity in patients diagnosed with congenital heart disease. To prevent risk factors, more studies must be done.

A 3-year-old girl with the diagnosis of chronic granulomatous disease (CGD) was hospitalized for bronchopneumonia and congestive heart failure. Her medical history included methylprednisolone medication for autoimmune gastric outlet obstruction. Computed tomography revealed pneumonic infiltrations and pericardial thickening. A pulsed-wave Doppler recording revealed E/A >1. During a pericardiectomy, multiple islands of thick, firm-walled, fibrinous exudate-containing, small abscess formations were observed. Histopathological evaluation of pericardial tissue revealed granulomatous inflammation. Aspergillus fumigatus was cultured from the abscess. In conclusion, development of constrictive aspergillus pericarditis should be considered in patients with CGD because immediate initiation of anti-fungal management with aggressive surgical treatment is life-saving.

Purpose: Febrile seizures (FS) are the most common type of seizures in children.The aim of this study was to evaluate the relationship between platelet indices and FS in children. Materials and Methods: This prospective study included 40 children who presented with FS and 30 controls who presented with febrile illnesses without seizures. Complete blood counts, including platelet count (PC), mean platelet volume (MPV), and platelet distribution width (PDW) from both groups within 1 hour of FS and 1 month later were obtained. Results: We found that the MPV and PDW within 1 hour of seizure in children with complex FS group was higher than simple FS group while there was no significant difference in MPV and PDW between patients in the simple and complex FS groups at 1 month. The mean PC was not significantly different between simple and complex FS groups; but, we found that the mean PC in the complex FS group was slightly lower than simple FS group.There was a moderate significant positive correlation between MPV and PDW in children with FS while there was a moderate significant negative correlation between PC and MPV, PDW for FS . Conclusion: Our findings suggest that the increasing platelet turnover in complex FS group causes a slightly decrease in the PC, an significantly increase of MPV and PDW values indicating that these parameters may play an important role in predicting the severity of FS in children at diagnosis. Bu araştırmanın amacı, lise son sınıf öğrencilerinin kariyer tercihinde etkili olan karar stratejileri, öz-yeterlilik inancı ve psikolojik danışmanlık-rehberlik hizmetleri faktörlerinin öğrencilerin mesleki olgunluk düzeyi üzerine etkisini incelemektir. İlgili faktörlerin yaş, cinsiyet, eğitim görülen alan türü gibi demografik değişkenlere göre farklılaşıp farklılaşmadıklarını belirlemek ise çalışmanın diğer amacını oluşturmaktadır. Araştırma kapsamındaki veriler, Sakarya ilindeki liselerde öğrenim gören 12. Sınıf öğrencilerine uygulanan standardize edilmiş anket ile elde edilmiştir. Elde edilen veriler SPSS kullanılarak analiz edilmiştir. Araştırma bulgularına göre değişkenlerin mesleki olgunluk düzeyine etkisine bakıldığında; mesleki olgunluk ile karar stratejilerinin kararsızlık boyutu arasında istatistiki açıdan anlamlı, orta kuvvette ve negatif yönde bir ilişki saptanmıştır. Mesleki olgunluk ile mantıklı karar stratejisi ve genel öz-yeterlilik değişkeni arasında ise istatistiki açıdan anlamlı, orta kuvvette ve pozitif yönde bir ilişki tespit edilmiştir. PDR faaliyetlerinin ve işlevselliğinin ise mesleki olgunluk düzeyi üzerinde istatistiksel olarak anlamlı bir etkiye sahip olmadığı görülmüştür.

Günümüzde hayatın her alanını sarmış olan sosyal medya platformlarının, televizyon gibi gündelik yaşamın baş aktörlerinden biriyle yolunun kesişmesi kaçınılmaz olmuştur. Televizyonun en kıymetli zamanı olan prime time diliminin en yaygın program türü televizyon dizileridir. Temel başarı kriteri en fazla seyirciyi ekran başına toplamak olan diziler, son dönemde meydana gelen gelişmelerden etkilenmiştir. Dizilerin başarısı sadece reyting ölçüm sonuçlarıyla değil sosyal medya reytingleriyle de ölçülmeye başlanılmıştır. ‘Katılımcı televizyon’ denilen sürece yeni medya olanaklarının eklenmesiyle interaktivite farklı bir boyut kazanmış, seyirci dizinin bir parçası haline gelerek aktif konuma gelmiştir. Bu çalışmada dizi reyting ölçümleriyle sosyal medya ölçümleri karşılaştırılarak, Türkiye’nin her bölgesinden İletişim Fakültesi öğrencilerinin dizi izlemeye eş zamanlı olarak Twitter kullanım alışkanlığı ölçülmüştür. Anket Araştırma Modelinin kullanıldığı araştırmada 550 kişiye anket uygulanmıştır. Çalışmada elde edilen bulgulara göre hiçbir dizinin TNS reyting ölçümlemesi, sosyal medya ölçümlemesi ve öğrencilerin izleme oranı aynı sıralamada değildir. Dizi izleyen öğrenciler arasında Twitter paylaşımlarında bulunmayan ve izledikleri diziyle ilgili yapılan paylaşımları takip etmeyenlerin çoğunlukta olması araştırmanın dikkate değer sonucudur.

Chanarin Dorfman Sendromu multisistemik genetik geçişli metabolik hastalık olup konjenital iktiyozis ve çeşitli hücrelerde lipid depolanması ile ilişkilidir. İktiyozisli hastalarda periferik kan yaymasında nötrofillerde lipid vakuollerinin (Jordan’s anomalisi) gözlenmesi tanı için diagnostiktir. 1974 yılında Dorfman tarafından bildirilen ilk vakadan bu yana, literatürde büyük çoğunluğu Orta doğu ülkelerinden yaklaşık 50 vaka bildirilmiştir. Bu yazıda 5 yaşında kreatin kinaz yüksekliği nedeniyle hastanemize başvuran, klinik ve laboratuar bulguları ile Chanarin Dorfman Sendromu tanısı konulan bir vaka sunulmuştur.

Amaç: Lenfadenopati, lenf nodunun boyut ve/veya karakterindeki anormallik olarak tanımlanır. Bu çalışmada amacımız çocukluk çağında selim ve malign nedenlere bağlı periferik lenfadenopatilerin klinik ve laboratuvar bulgularını belirlemek. Materyal ve Metod: Çalışmaya periferik lenfadenopati nedeni ile hastanemize başvuran 224 hasta alındı. Hastaların; yaşı, cinsiyeti, laboratuvar ve radyolojik bulguları, tanıları, lenfadenopatinin; süresi, lokalizasyonu, boyutu, kıvamı, yayılımı, eşlik eden lokal ve sistemik semptomları kaydedildi. Malign ve selim hastalıklara bağlı lenfadenopatilerin klinik ve laboratuvar özellikleri karşılaştırıldı. Bulgular: İki yüz yirmi dört hastanın 126’sı (%56.0) erkek, 98’i (%44.0) kız idi. Hastalar 2 gruba ayrıldı. Birinci grupta selim nedenlere bağlı lenfadenopatisi olan 186 hasta, ikinci grupta ise malign nedenlere bağlı lenfadenopatisi olan 38 hasta mevcuttu. Yüz altmış hastada(%73.2) localize lenfadenopati vardı. Selim hasta grubunda lokalize lenfadenopatinin en sık nedeni akut lenfadenitler (%34.8) iken, malign hasta grubunda en sık nedenin Hodgkin lenfoma (%4.3) olduğu tespit edildi. İki yüz yirmi dört hastanın 60’ında (%26.8) jeneralize lenfadenopati vardı. Jeneralize lenfadenopatinin selim hasta grubunda başlıca nedeni Epstein–Barr virus enfeksiyonu (%10.0) iken, malign hasta grubunda Hodgkin lenfoma (%23.3) olduğu tespit edildi. Servikal bölge lokalize ve jeneralize lenfadenopatilerde en sık tutulan bölgedir. Malign ve selim nedenlere bağlı lenfadenopailer en sık servikal bölgede görüldü. Elde ettiğimiz sonuçlara göre kronik seyirli, jeneralize lenfadenopati, supraklavikular, servikal ve inguinal yerleşimli lenfadenopati, organomegali, abdomanial lenfadenopati, hilier lenfadenopati ve anormal laboratuvar bulgularının (trombositopeni, periferik yaymada blast ) varlığında öncelikle malign nedenler düşünülmelidir. Sonuç: Enfeksiyonlar çocukluk çağında periferik lenfadenopatinin en sık nedenidir. Malignite riski yaş ve lenfadenopatinin süresi ile artmaktadır. İleri yaş, kronik lenfadenopati, jeneralize lenfadenopati, eşlik eden organomegali, anormal laboratuvar bulguların varlığında öncelikle malignite düşünülmelidir.

Purpose: To define the clinical and laboratory findings of lymphadenopathy (LAP) resulting from benign and malignant causes during childhood. Material and Methods: 224 patients who were admitted to the clinic with peripheral lymphadenopathy were evaluated. Patient age, gender, laboratory and radiologic results, final diagnoses, and duration, localization, size, consistency, spread, and accompanying local and systemic symptoms of lymphadenopathy were determined. Lymphadenopathy in patients with benign and malignant causes were compared. Results: One hundred twenty-six of 224 patients (56.0%) were male and 98 (44.0%) were female. The analysis comprised 224 patients divided into 2 groups. The first group included 186 patients with benign causes and the second group included 38 patients with malignant causes. The most frequent cause of local lymphadenopathy in the benign group was acute lymphadenitis (34.8%), the most common cause in the malignant group was Hodgkin's lymphoma (4.3%). The most significant cause of general lymphadenopathy in the benign group was EBV (10.0%), whereas Hodgkin's lymphoma (23.3%) was most influential in malignant cases The results revealed that findings, such as chronic course, general lymphadenopathy, supraclavicular and cervical location, organomegaly, abdominal lymphadenopathy, weight loss, constitutional symptoms (night sweats or fatigue, anemia, thrombocytopenia, and leucopenia) were associated with malignant diseases. Conclusion: Infections are the most common cause of lymphadenopathy among benign diseases. Malignant causes and tuberculosis should be considered in older patients, chronic lymphadenopathy, general lymphadenopathy, associated organomegaly, abnormal laboratory findings, and the presence of constitutional symptoms. Kurumsal sosyal sorumluluk (KSS) kavramı ve uygulamalarının sürdürülebilir gelişmeye olumlu yönde katkılarda bulunduğunun tartışılması örgütler nezdinde bu kavramı daha da merkezi bir konuma taşımıştır. Bu noktada örgütlerin toplum, çevre, eğitim ve sağlık gibi alanlardaki gönüllülüğe dayalı projelere daha çok yer verdiği de görülmektedir. Bu çalışmanın amacı, KSS uygulamalarının sahip oldukları sürdürülebilir gelişme bağlamı çerçevesinde örgüt toplulukları arasında hangi yönde eşbiçimleşmeye neden olacağına dair bir bakış açısı geliştirmektir. Bu araştırmada Türkiye’de faaliyet gösteren 10 holding vaka çalışması yöntemi kullanılarak incelenmiştir. Vaka çalışması kapsamında bu 10 holdingin KSS uygulamaları, KSS uygulamalarını anlattıkları web siteleri, varsa KİS (Küresel İlkeler Sözleşmesi) çerçevesinde yayınladıkları raporları ya da genel yıllık raporları nitel içerik analizi yöntemi ile analize tabi tutulmuştur. Nitel içerik analizi çerçevesinde elde edilen sonuçlar kurumsal kuram perspektifinden incelenerek vakalar arasında bir eşbiçimlilik olup olmadığı şayet varsa bu eşbiçimliliğin türünün ve yönünün ne olduğu belirlenmeye çalışılmıştır.

Chanarin Dorfman Syndrome is a multisystem inherited metabolic disorder associated with congenital ichthyosis and accumulation of lipid droplets in varios types of cells. Observation of lipid vacuoles in neutrophils (Jordan's anomaly) in peirpheral blood smears in patients with ichthyosis is diagnostic for Chanarin Dorfman Syndrome. Since the initial case was reported by Dorfman in 1974, nearly 50 cases have been reported in the literature, and the majority were from Middle East countries. In this report we presented a 5 year old patient who admitted to our hospital for creatine kinase elevation and diagnosed as Chanarin Dorfman Syndrome with clinical and laboratory findings. Kawasaki disease is a vasculitis of infant and childhood period. Diagnosis of Kawasaki disease is based on clinical criteria including cervical lymphadenopathy. Here we reported a 3.5-year-old boy presenting with Kawasaki disease mimicking bacterial lymphadenitis and fever. If a child presents with lymphadenopathy only which is unresponsive to antibiotics, Kawasaki disease should be considered.

Introduction: Febrile neutropenia, developing in hematological cancer patients, is a common complication requiring hospitalization and resulting in death. It is assumed that it is caused by infection until its reason is clarified. Hence, it requires a multidisciplinary approach and treatment as a medical emergency. For these patient groups, most of whom are at high risk, death caused by infection is attempted to be minimized by continuously updating treatment approaches. This study aimed to determine clinical characteristics, risk factors, distribution and frequency variance of isolated pathogens and impact in mortality of hematological cancer patients with febrile neutropenia prospectively. Materials and Methods: In this study, 161 febrile neutropenia episodes of 99 patients with hematological cancer monitored between January 2012 and January 2013 in the Department of Hematology in the Hospital of our Medical Faculty were evaluated. Age, gender, underlying disease, chemotherapy regimens, hematologic and radiologic findings, antibacterial and fungal therapeutic regimens were recorded. Routine microbiological blood and urine cultures were performed. When necessitated, cultures from other sterile body sites were taken. Results: Mean age of the patients in the study group was 50.7 ± 15.9. Mean duration of hospitalization was 29.7 ± 14.9 days. Meanduration of neutropenia was 14.6 ± 5.7 days. In 21% of the evaluated episodes, microbiologically defined infection was found, where clinically defined infection was determined in 40% and fever of unknown origin was determined in 39%. A total of 47 isolates were isolated in total. When distribution of all isolated microorganisms, in all cultures was evaluated, 40% were grampositives, 47% were gram-negatives and 13% were fungi. The most frequently observed microorganisms were coagulase negative staphylococci with 31% and Escherichia coli with 22%. In blood cultures, 58% were gram-positives, 32% were gramnegatives and 10% were fungi. The most frequent febrile neutropenia treatment used in our center was carbapenem monotherapy. Mortality rate was 25.4%. Neutropenia duration and stage were the most important factors increasing mortality risk. Conclusion: Febrile neutropenia is a complication which may result in mortality in patients with hematological cancer. In order to define treatment models, each center is supposed to determine its center’s microorganism spectrum and antibiotic resistance profiles periodically.

Summary Osteogenesis imperfecta is a disease that involves mesodermal tissues and characterized by decreased bone mass and increased bone fragility. Bone fractures developed due to ligamentous laxity, low energy trauma and joint deformities and contractures due to dislocations may be seen during the disease process. For that reason, it could be confused especially with rheumatoid arthritis. In this article we presented a case with osteogenesis imperfecta that misdiagnosed as rheumatoid arthritis because of joint deformities and common joint pain. Leprosy is a chronical infectious disease caused by Mycobacterium Leprae involving peripheral nerve, skin, reticuloendothelial system, eyes, testicles and other internal organs. Low bone mineral density in male patients with leprosy due to inactivity, neuropathy, musculosceletal pathology, testicular atrophy and hypogonadism is possible.
