53. Anterior Segment Findings in Women with Polycystic Ovary Syndrome

Objectives: This study aimed to investigate the anterior segment in women with polycystic ovary syndrome (PCOS) and to compare them with those of healthy reproductive-age female volunteers. Materials and Methods: The study included 50 right eyes of 50 women with PCOS (group 1) and 50 right eyes of 50 healthy women (group 2). Intraocular pressure, Schirmer’s test, tear film break-up time and central corneal thickness were evaluated in all subjects. Correlations between serum hormone (estradiol and testosterone) levels and observed findings were also investigated. Results: Mean central corneal thickness values were significantly higher in the PCOS group (p=0.001). The mean intraocular pressures values were similar between the two groups (p=0.560). Schirmer’s test results and tear film break-up time values were significantly lower in the PCOS group (p=0.001 and p=0.001 respectively). Serum estradiol levels were moderately positively correlated with mean central corneal thickness (r=0.552), weakly positively correlated with intraocular pressure (r=0.351) and weakly negatively correlated with tear film break-up time (r=-0.393). Serum free testosterone levels were weakly correlated with intraocular pressure (r=0.342) and central corneal thickness (r=0.303), and showed weak negative correlations with Schirmer’s test results (r=-0.562) and tear film break-up time (r=-0.502). Conclusion: PCOS leads to physiological and structural changes in the eye. Dry eye symptoms were more severe and central corneal thickness measurements were greater in patients with PCOS. Those are correlated serum testosterone and estradiol levels.

Objectives:To evaluate sarcoidosis-induced tear film changes using subjective and objective diagnostic tests, particularly conjunctival impression cytology (IC), and to compare the results with healthy individuals.Materials and Methods:This study evaluated clinical data collected between January 2019 and January 2021 from 57 right eyes of 57 sarcoidosis patients without ocular involvement (Group 1) and 33 right eyes of 33 healthy individuals with similar demographic characteristics (Group 2). The Schirmer I test, tear break-up time (TBUT), fluorescein staining, and conjunctival IC were all performed as part of the conjunctival and corneal examinations following a thorough ophthalmological examination. The Ocular Surface Disease Index (OSDI) was used to assess subjective ocular symptoms.Results:The mean ages in Groups 1 and 2 were 49.26±3.18 and 51.91±2.89 years, respectively (p=0.720). 56. The Effect of Mask Use on the Ocular Surface During the COVID-19 Pandemic

Objectives:The new coronavirus disease 2019 (COVID-19) pandemic emerged in Wuhan, China in October 2019 and spread rapidly all over the world, making extended mask use an inescapable rule of daily life. Literature data indicate that the use of face masks increases the symptoms of dry eye in addition to preventing the spread of COVID-19. The aim of our study was to evaluate the relationship between the clinical signs and symptoms of dry eye and the duration of mask use in healthy individuals using regular face masks.Materials and Methods:Thirty-five patients aged 20-60 years with no additional ophthalmologic pathology were included in the study. Participants were stratified by duration of face mask use: ≤6 hours/day (group 1) and >6 hours/day (group 2). The patients were assessed with the ocular surface disease index (OSDI) questionnaire, fluorescein ocular surface staining, and tear break-up time (TBUT) to evaluate the effect of extended mask use on the ocular surface.Results:A total of 62 eyes of 35 patients, 20 women (57.1%) and 15 men (42.9%), were included in the study. The two mask use duration groups had similar OSDI values (p=0.618). When the ocular surface staining pattern was examined according to the Oxford scale, 50% (10/20) of the eyes in group 1 were assessed as stage 1 and the other 10 eyes as stage 0. In group 2, 47.6% (20/42) of the eyes were assessed as grade 1, 11.9% (5/42) as grade 2, and 4.7% (2/42) as grade 3.Conclusion:Face mask use was shown to cause decreased TBUT and ocular surface staining even in healthy individuals. Further studies are needed to investigate changes in tear film after extended daily mask use.

Objective: The aim of this study was to assess the effect of the carbomer-based lipid-containing (CBLC) gel on tear break-up time (TBUT), Schirmer's score, and ocular surface disease index (OSDI) through a one-masked clinical trial in patients with dry eye syndrome. Material and Methods: A 4-week and retrospective study was conducted. A total of 60 eyes of 30 patients with mild to moderate dry eye syndrome were included and treated with the CBLC gel two times a day within twelve hours interval. The primary end point was global assessment of study treatment by the patients before treatment and at the end of a 4-week treatment. All patients met the diagnostic criteria of impaired tear function and ocular surface abnormalities. Outcomes measured at baseline and at the end of the 4-week treatment included Schirmer's test, TBUT, and OSDI values. Results: The mean age of 30 patients was 53.79±12.73 years. At baseline, the mean for TBUT was 5.03±1.04 second and 9.81±4.95 mm for Schirmer's test. The TBUT score increased to a mean of 8.12±1.61 second and the Schirmer's test score increased to a mean of 13.15±5.08 mm after the 4-week treatment. Subjective patient assessment was better after the treatment, the mean for the OSDI score was 50.59%±13.29% at baseline and 36.86% ±11.93% after the 4-week treatment. These changes for TBUT, Schirmer's test, and OSDI scores were statistically significant (p<0.05). Conclusion: CBLC gel is an effective treatment option for the patients who have suffered from mild to moderate dry eye syndrome. [ABSTRACT FROM AUTHOR]

58. The Effects of Face Mask Usage on Ocular Structures in Children During the COVID-19 Pandemic

Aim:To determine the effects of face mask usage on ocular structures in children during the coronavirus disease-2019 (COVID-19) pandemic.Materials and Methods:Forty-two children’s ocular data from the pandemic period were compared with the same children’s ocular data from the pre-pandemic period.Results:Their mean age was 14.6±2.1 (9-18) years initially. The tear film break-up time values of the children with a mask in the pandemic period were significantly shorter than those of the children without a mask in the pre-pandemic period (9.35±1.40 vs. 12.10±1.05 seconds, p=0.033). There was no staining in any case in the pre-pandemic period, while minimal punctate epithelial corneal staining pattern was determined in 4 (9.5%) children in the pandemic period. The Schirmer test 1 value detected in the pandemic period was similar to the value detected in the pre-pandemic period (11.14±2.07 vs. 12.03±1.01 mm, p=0.127). Additionally, there were no significant changes in visual acuities, central corneal thicknesses, anterior chamber depths, lens thicknesses, and axial lengths between the pre-pandemic and pandemic periods (p>0.05).Conclusion:To the best of our knowledge, this is the first study to date to evaluate the effects of face mask usage on ocular structures in just children. The use of face masks may cause increased tear evaporation and dry eye in pediatric cases. Educating children about the correct use of face masks can be important. Thus, possible ocular surface changes which may occur relating to masks can be prevented, and children may be more willing to use masks. Increased mask compliance may also indirectly help protect children from the virus.

60. Sistemik İzotretinoin Tedavisine Bağlı Oküler Yüzey ve Gözyaşı Film Tabakası Değişiklikleri.

- Abstract
Objective: To investigate dry eye symptoms, tear film abnormalities, ocular surface changes and periods of their development in patients treated with systemic isotretinoin for acne vulgaris. Material and Methods: Forty patients given systemic isotretinoin for nodulocyctic acne were included in the study. Patients were evaluated before the initiation of treatment, monthly during the treatment and one month after termination of the treatment. Ocular Surface Disease Index (OSDI) Questionnarie for subjective dry eye complaints, tear function tests including Schirmer tests with and without anesthesia and tear break up time (TBUT) test, corneal and conjunctival stainings to detect tear film abnormalities were evaluated. Results: At first month of the treatment mean OSDI score increased from 7.74±6.01 to 25.98±10.60 (p<0.01), mean Schirmer score without anesthesia decreased from 21.18±5.75 to 14.75±6.50 (p<0.01), mean Schirmer score with anesthesia decreased from 13.78±4.67 to 8.63±4.20 (p<0.01) and mean TBUT score decreased from 12.25±3.05 to 7.98±3.71 (p<0.01) compared to the pretreatment values. These findings continued throughout the treatment. Twenty two (55%) patients developed conjunctival hyperemia, 22(55%) patients developed lid margin telengiectasia and 10(25%) patients developed corneal punctate epitheliopathy one month after therapy. These symptoms and signs were seen to continue decreaseingly at one month control of termination of the therapy. Conclusion: Isotretinoin is a retinoic acid derivative used frequently in dermatology clinics. It has side effects in many systems as well as the eye. In our study dry eye complaints, ocular surface changes and tear function abnormalities commonly started within the first month. One month after termination of the treatment tear funtions did not not reach the pretreatment levels. We advise that patients given systemic isotretinoin should be consulted to an ophthalmologist. They should use artificial tears If dry eye complaints start and tear functions tests are abnormal until they return to normal levels. [ABSTRACT FROM AUTHOR]

62. Effects of Upper Eyelid Surgery on the Ocular Surface and Corneal Topography

Objectives:To evaluate the effect of upper eyelid surgery on ocular surface and corneal topography.Materials and Methods:Patients who underwent upper eyelid blepharoplasty and/or blepharoptosis repair were evaluated prospectively. Tear film break-up time (TBUT), Schirmer tests, corneal staining pattern, Ocular Surface Disease Index questionnaire, corneal topography, and autorefractor parameters were measured preoperatively and at 1 day, 1 week, 1 month, 3 months, and 6 months postoperatively.Results:Thirty-two eyes of 20 patients (9 male, 11 female) were included in the study. The mean age was 44.8±18.9 years (range: 8-74). Patients were divided into the following 3 groups according to the type of surgery performed: upper eyelid blepharoplasty (group 1), upper eyelid blepharoplasty and levator advancement ptosis surgery (group 2), and levator advancement ptosis surgery (group 3). There was a significant decrease in Schirmer test results at 6 months in groups 1 and 2 but no change in group 3. TBUT values were decreased at 1 week in group 3 (p=0.028) and returned to baseline at 1 month. Corneal punctate staining was detected at 1 day and 1 week in all groups. On corneal topography, group 3 showed a significant change in K2 values (0.3 diopters) at 1 month (p=0.006). There was no statistically significant change in autorefractor measurements postoperatively compared to preoperative values (p>0.05).Conclusion:Depending on the type of surgical procedure performed, blepharoptosis repair and upper eyelid blepharoplasty can lead to dry eye of varying severity that may persist at postoperative 6 months.

64. The Effect of Artificial Tears on Corneal Higher Order Aberrations in Dry Eye Patients.

- Abstract
Purpose: To assess the effects of artificial tears on corneal higher order aberrations in dry eye patients. Materials and Methods: 30 right eyes of 30 newly diagnosed dry eye patients were evaluated in this prospective study. After routine ophthalmological examination, Schirmer test and tear break-up time (TBUT) test were performed in all patients. Anterior corneal aberrations were derived from conversion of the corneal elevation profile into corneal wavefront data with 6.0 mm pupil diameter using Zernike polynomails by corneal topography before and 5 minutes after instillation of artificial tear (Eyestil®). Corneal optical aberrations were compared before and after instillation of eyedrop. Results: The study included 17 women and 13 men; the average age of the patients was 44.36±13.22 years. Mean TBUT was 4.78±2.78 seconds and mean Schirmer value was 3.58±2.45 mm/5 minutes. After instillation of artificial tear, significant reductions in corneal total aberration from 1.120±0.35 μm to 0.960±0.34 μm, higher order aberration from 0.674±0.26 μm to 0.464±0.18 μm, coma-like aberration from 0.283±0.10 μm to 0.238±0.09 μm, and spherical-like aberration from 0.254±0.11 μm to 0.221±0.08 μm were detected (all, p<0.001). After eyedrop instillation, statistically significant increment was observed in Strehl ratio (p<0.001). Conclusion: As well as reducing the dry eye symptoms, artificial tears also cause increment in optical quality of the eye. Benefits of artificial tears on visual quality can be evaluated objectively via corneal wavefront aberrations. [ABSTRACT FROM AUTHOR]

65. Ocular Findings of Pediatric Dry Eye Related to Graft-Versus-Host Disease

Objectives:To evaluate the frequency and findings of dry eye associated with ocular graft-versus-host disease (GVHD) in pediatric hematopoietic stem cell transplantation (HSCT) patients.Materials and Methods:Retrospectively the records of pediatric patients with ocular GVHD were evaluated and ophthalmologic examination findings as well as Schirmer test results, tear film break-up time, and corneal staining grades were recorded. In severe dry eye patients topical cyclosporine-A was prescribed and the results were evaluated.Results:GVHD was detected in 51 (23.4%) of 218 HSCT patients, 4 of whom died during follow-up. Thirty (63.8%) of the remaining 47 patients had chronic ocular GVHD and 4 patients with severe dry eye were treated with topical cyclosporine-A with a median follow-up of 12.1 months. Severe dry eye symptoms and findings significantly improved in 2 patients. However, 1 patient had to stop treatment due to side effects.Conclusion:In children, chronic ocular GVHD is a common finding of GVHD after HSCT. Therefore, these patients should be examined periodically for dry eye.

Objectives:We aimed to evaluate the efficacy of periocular intense pulsed light (IPL) therapy in the treatment of moderate to severe acute blepharitis or blepharoconjunctivitis.Materials and Methods:This was a retrospective study performed in one institution. Eleven patients who received bilateral periocular IPL therapy using an IPL device (E>Eye, ESwin, Paris, France) were retrospectively evaluated. The following findings obtained at baseline and 10 weeks after the treatment were recorded: slit-lamp examinations; symptom scores of the Compression of the Eyelid (COTE) grading system and Ocular Surface Disease Index (OSDI); ocular surface staining with Oxford grading scale (OXFORD) scores; lipid layer thickness (LLT); and non-invasive tear meniscus test (TMH), non-invasive break up time measurement (NIBUT), and meibography performed by using I.C.P. Objectives:To evaluate aqueous and lipid tear film parameters and the meibomian glands (MGs) with non-contact meibography in patients with vitiligo.Materials and Methods:This case-control study was conducted in the right (OD) and left (OS) eyes of 43 patients with vitiligo and 43 controls in Birjand, Iran. In addition to demographic information and skin disease characteristics, the Ocular Surface Disease Index (OSDI) questionnaire was completed for each patient, followed by eye examinations including slit lamp examination, Schirmer test, strip meniscometry (SMTube), and tear break-up time (TBUT) measurement. The MGs were also imaged using a non-contact meibography system (SBM System, Italy). The data were analyzed using SPSS version 22.0 with a significant level of less than 0.05.Results:Patients had higher OSDI score than controls but it was not significant (10.90±13.03 vs. 5.57±6.85; p=0.07). 68. Evaluation of the Correlation Between Tear Meniscus Parameters and Conventional Dry Eye Tests.

- Abstract
Purpose: To evaluate the correlation between clinical tests, patient symptoms and spectral optical coherence tomography (OCT; RTVue, Optovue)-derived lower tear meniscus (TM) parameters and to verify sensitivity and specificity of TM parameters in the diagnosis of dry eye disease. Material and Method: 38 eyes of 38 patients (22 dry eye patients, 16 healthy subjects) were examined in this prospective study. After routine ophthalmologic examination, anterior segment OCT-derived TM height (TMH), TM depth (TMD), and TM area (TMA) were measured, and tear break-up time (TBUT) and Schirmer test with anesthesia were assessed in all patients. For evaluation of symptoms, the participants completed ocular surface disease index (OSDI) questionnaire. Correlation between tests was assessed using Pearson's correlation coefficient (r). Results: There was a significant positive correlation between Schirmer test results and TMH, TMA and TMD (r=0.79, 0.58, 0.58, respectively). TBUT was positively correlated with TMH, however, it was not correlated with TMA and TMD (r=0.63, 0.14 and 0.10, respectively). There was no significant correlation between OSDI score and 3 parameters of TM. TM measurements were significantly lower in dry eyes than in controls. Sensitivity and specificity for dry eye diagnosis were 81.5% and 86.8% for TMH, 78.9% and 76.3% for TMA, and 76.3%, and 52.6% for TMD, respectively. Discussion: OCT-derived TM parameters were correlated with Schirmer test, but there was no correlation between TM parameters and patient symptoms. 70. Evaluating of Tear Meniscus Parameters with Optical Coherent Tomography in Dry-Eye Patients.

Purpose: To evaluate the changes in meniscus parameters with optical coherence tomography (OCT) after treatment and consider the correlation between the OCT parameters and dry-eye tests. Material and Method: Thirty-two dry-eye patients and 30 healthy individuals were included. Visual acuities, biomicroscopic evaluation, Schirmer-I test, tear-break-up time (T-BUT), and lower-tear meniscus parameters measured with OCT were evaluated. Ocular surface disease index (OSDI) was performed. The measurements were repeated on the 10th day and 1st month of the treatment. Lower tear meniscus height (LTMH), depth (LTMD), area (LTMA) and α-angle were measured. The tear meniscus parameters of the dry-eye group were compared with the control group before treatment, and the correlation between the dry-eye tests and OCT measurements were evaluated. The change in the results of the dry-eye tests and OCT measurements with treatment were assessed. Results: There was statistically no significant difference between the age and gender of the patients in groups. Before treatment the mean Schirmer-I test, TBUT and OSDI scores were different. The LTMH and LTMA were higher in the control group. In the dry-eye group before treatment, there was a negative correlation between the OSDI score and OCT parameters, however, no meaningful correlation was observed between any other test and OCT parameters. Although there were no change in LTMD and α-angle with treatment, the 1st month values of LTMH and LTMA were significantly higher from the pre-treatment and 10th day values. Discussion: When diagnosing dry-eye, TBUT and OSDI scores were found to be more effective compared to Schirmer-I test for the diagnosis of dry eye. Although not adequate by itself, tear meniscus parameters measured with OCT, LTMH and LTMA, may be helpful for evaluating the efficacy of treatment, and the correlation of these two parameters with the OSDI score may increase the objectivity while questioning the symptoms [ABSTRACT FROM AUTHOR]

71. Dry Eye Disease after Cataract Surgery: Study of its Determinants and Risk Factors

Objectives:To study the incidence of dry eye and its determinants in patients undergoing cataract surgery.Materials and Methods:One hundred twenty patients with senile cataract underwent Schirmer’s test, tear break-up time (TBUT) test, lissamine green staining of the cornea and conjunctiva, and Ocular Surface Disease Index (OSDI) for evaluation of dry eye preoperatively and again at first and second follow-up examinations at 1 week and 1 month after cataract surgery.Results:Mean age of the patients was 59.25+9.77 years and 73 (60.8%) were men. None of the patients had dry eye at the time of enrollment as per the criteria of our study. Postoperatively, Schirmer’s test values ranged from 12-35 mm and 8-24 mm at first and second follow-ups, respectively. Mean TBUT was 13.16±2.45 and 9.64±2.20 seconds, while lissamine green staining score was 3 in 67 (55.8%) and 1 in 67 (55.8%) subjects at first and second follow-up, respectively. OSDI values ranged from 1-30 and 10-33 with a mean of 25.97±5.34 and 11.96±7.47 respectively at first and second follow-up. Psoriasis, keskin sınırlı eritemli plak veya papüller üzerinde yerleşmiş, sedefî-beyaz skuamlarla karakterize, epidermal proliferasyon artışı ile seyreden, tırnakları ve eklemleri de tutabilen, kronik, inflamatuvar bir deri hastalığıdır. Lezyonlar sıklıkla saçlı deri, diz, dirsek, sakral bölge ve eklemlerin ekstansör yüzlerinde simetrik olarak yerleşmektedir. Psoriasisin bir deri hastalığından fazlası olup aynı zamanda diğer sistemleri de etkileyebildiği bilinmektedir. Blefarit, inferior punktat korneal opasite, konjonktivit, göz kuruluğu, punktat epitelyopati, superfisyal punktat keratit, iritis, kronik iridosiklit, üveit, pigment dispersiyonu, trikiyasis, semblefaron, katarakt, episklerit ve periferal korneal erime sendromu psoriasiste bildirilmiş göz bulgularındandır. Bu çalışmada psoriasisli hastalarda göz bulgularının değerlendirilmesi ve bu bulguların yaş, cinsiyet, hastalık süresi, hastalık şiddeti, saçlı deri, eklem ve tırnak tutulumu ile ilişkisinin araştırılması amaçlanmıştır. Çalışmamıza klinik ve histopatolojik olarak psoriasis tanısı almış 18 yaş üstü 77 hasta ve 75 sağlıklı kontrol grubu dâhil edildi. Hastaların yaş, meslek, sigara kullanımı ve süresi, aile öyküsü, hastalık süresi gibi bilgileri sorgulandı. Tırnak, saç ve eklem tutulumu değerlendirildi. PASI skoru hesaplandı. PASI skoru 1-10 olanlar ''hafif'', 10 un üzeri olanlar ''şiddetli'' psoriasis olarak değerlendirildi. Tüm olgulara biyomikroskopik muayenesi, fundus bakısı, gözyaşı kırılma zamanı, Schirmer testi, görme keskinliği, göz içi basıncı, göz hareketleri değerlendirmesi yapıldı. Hastaların speküler mikroskopi ve optik koherens tomografisi (OKT) çekildi. Korneal menisküs parametreleri hesaplandı. Kuru göz semptomları açısından OSDI skoru hesaplandı. Çalışmamızda tırnak ve saç tutulumu olan hastalarda olmayanlara göre PASI skoru anlamlı derecede yüksek bulundu. Yapılan göz muayenesinde hastaların %5,19 unda blefarit, %2,59 unda nükleer katarakt ve %1,29 unda pterjium izlendi. Hastaların yapılan göz dibi muayenesinde %2,59 hastada retinal epitel pigment değişikliği ve %1,29 unda asteroid hyelozis izlendi. BUT testi sonuçlarına göre psoriasisli grupta %79,2 hastanın ve kontrol grubunda %2,6 kişinin gözyaşı kırılma zamanının 10 saniyenin altında olduğu görüldü. Blefarit kadın hastalarda daha sık izlenirken göz kuruluğu erkeklerde sıktı. Speküler mikroskopide CV (değişkenlik katsayısı) ve HEX (hegzagonal hücre oranı) istatistiksel olarak anlamlı izlendi. ÖS-OKT (ön segment optik koherens tomografi) de santral, temporal, superior ve inferior korneal epitel kalınlıkları istatistiksel olarak anlamlı saptandı. Gözyaşı menisküs bulguları değerlendirildiğinde psoriasisli grupta gözyaşı menisküs alanının anlamlı olduğu izlendi. Psoriasisli tüm hastalar, takip ziyaretleri sırasında kuru göz, blefarit, konjonktivit ve üveit gibi göz hastalıkları açısından düzenli olarak taranmalıdır. Hastalar rutin muayene sırasında gözlerde rahatsızlık (ağrı / tahriş / yabancı cisim hissi / kuru göz / fotofobi), kirpiklerin dökülmesi, şişmiş / ağrılı göz kapakları, gözlerde kızarıklık, periokuler veya kapak lezyonları ve görmede değişiklikler açısından sorgulanmalı; gereklilik halinde göz hastalıklarına yönlendirilmelidir. Psoriasis is a chronic, inflammatory skin disease; characterized by white squam located on sharp erythematous plaques or papules with increased epidermal proliferation and also involving the nails and joints. The lesions are often located symmetrically on the extensor surfaces of the knee, elbow, sacral region, scalp and joints. It is known that psoriasis is more than a skin disease and can also affect other systems. Blepharitis, inferior punctate corneal opacity, conjunctivitis, dry eye, punctate epitheliopathy, superficial punctate keratitis, iritis, chronic iridocyclitis, uveitis, pigment dispersion, trichiasis, symlepharone, cataract, episcleritis, and peripheral corneal melt syndrome are eye findings reported in psoriasis. In this study, we aimed to investigate eye findings and examination results in psoriatic patients and their relationship with age, sex, disease duration, disease severity, scalp, joint and nail involvement. Seventy-seven patients over 18 years of age diagnosed as psoriasis clinically and histopathologically and 75 healthy control groups were included in the study. Information about age, occupation, smoking habit and duration, family history, duration of illness was questioned. Nail, hair and joint involvement were evaluated. PASI score was calculated. Those with a PASI score of 1-10 were considered as 'mild' and those with a score of more than 10 were considered as 'severe' 'psoriasis. Biomicroscopic examination, fundus examination, tear break-up time, Schirmer test, visual acuity, intraocular pressure and eye movements were evaluated in all cases. Specular microscopy and optical coherence tomography (OCT) were performed. Corneal meniscus parameters were calculated. OSDI score was calculated for dry eye symptoms. In our study, the PASI score was significantly higher in patients with nail and hair involvement than those without. Blepharitis was observed in 5.19%, nuclear cataract was 2,59% and pterygium was found in 1,29% of the patients. Retinal epithelial pigment changes were observed in 2,59% of the patients and asteroid hyelosis was found in 1,29% of the patients. According to the results of the BUT test, 79,2% of the patients with psoriasis and 2,6% of the control group had a tear break-up time of less than 10 s. Blepharitis was more common in female patients, whereas dry eye was common in men. In specular microscopy, CV (coefficient of variability) and HEX (hexagonal cell ratio) were statistically significant. Central, temporal, superior and inferior corneal epithelial thicknesses were found to be statistically significant in AS-OCT (anterior segment optical coherence tomography). When tear meniscus findings were evaluated, it was observed that tear meniscus area was significant in psoriasis group. All patients with psoriasis should be screened regularly for eye diseases such as dry eye, blepharitis, conjunctivitis and uveitis during follow-up visits. Patients should be questioned for routine discomfort (pain / irritation / foreign body sensation / dry eye / photophobia), loss of eyelash, swollen / painful eyelids, redness of the eyes, periocular or eyelid lesions and visual changes; if necessary, it should be recommended to see an opthalmologist. 140

Amaç: Kliniğimizde Konjonktivaşalazis tanısı alan olguların, klinik semptom ve bulgularının literatür eşliğinde gözden geçirilmesi amaçlandı.Gereç ve Yöntem : Bu çalışma, Mart 2019 ve Haziran 2019 tarihleri arasında Meram Tıp Fakültesi Hastanesi Göz Hastalıkları Polikliniği'ne başvuran ve Konjonktivaşalazis tanısı almış olan hastalarda prospektif olarak yapılmıştır. Kşa tanısı alan 18 yaş ve üzerindeki 57 hastanın 57 gözü çalışmaya dahil edildi. Muayene başında hastalara anket çalışması ile ilgili bilgi verildi ve katılmak istediklerine dair onamları alındı. Hastaların rutin oftalmolojik muayeneleri yapıldı. Hastaların refraksiyon değerleri aksiyel uzunluk ölçümleri kaydedildi. Ön segment muayenesinde break up time (BUT), korneal boyanma, konjonktivaşalazisin lokalizasyonu, pingekula veya pterjium varlığı değerlendirildi. Herhangi bir kapak hastalığı olup olmadığı not edildi. Her hastanın gözyaşı miktarı ölçümü hem Schimer kağıdı ile hem de Strip meniskometri kağıdı ile yapıldı. Konjonktivaşalazis, LIPCOF sınıflamasına göre derecelendirildi. Verilerin analizi IBM SPSS Statistics 17.0 (IBM Corporation, Armonk, NY, USA) paket programında yapıldı ve p0,05). Schimer düzeyi arttıkça Strip meniskometri düzeyi de istatistiksel anlamlı olarak artmaktaydı (r=0,302 ve p=0,022). Buna karşın Schimer ile OSDI arasında istatistiksel olarak anlamlı korelasyon saptanmadı (p=0,616). Sonuç: Bu tez çalışmasında kliniğimizde konjonktivaşalazis tanısı alan hastaların ayrıntılı anamlezlerini, semptomlarını ve klinik muayene bilgilerini sunarak literatur eşliğinde sonuçları değerlendirdik. Konjonktivaşalazis hastalarının gerçek yaşam verileri sunuldu. Ancak bu konun aydınlatılmasında daha fazla hasta sayısı ve kontrol grubu ile yapılacak prospektif çalışmalara ihtiyaç vardır. Purpose: The aim of this study was to review the clinical signs and symptoms of patients with conjunctivalchalasis in our clinic in the light of the literature.Materials and Methods: This study was performed prospectively in patients with conjunctivalchalasis who were admitted to Meram Faculty of Medicine Ophthalmology Department between March 2019 and June 2019. 57 eyes of 57 patients aged over 18 years were included in the study. Before ophthalmologic examination, the patients informed consent was obtained. Axial length measurements of the patients were recorded. In the anterior segment examination, break up time (BUT), corneal staining, localization of conjunctivalchalasis, presence of pingecula or pterygium were evaluated. The tear amount of each patient was measured with schimer paper and strip menisometry paper. Conjunctivalchalasis was graded according to the LIPCOF classification. Data were analyzed using IBM SPSS Statistics 17.0 (IBM Corporation, Armonk, NY, USA) and p 0.05). We found that, as Schimer level increased, Strip meniscometry level also increased statistically significant. It was not correlation between Schimer level and OSDI scores statistically significant (p = 0.616).Conclusions : In this study, we presented sign and symptoms and clinical examination of patients diagnosed with conjunctivalchalasis in our clinic and evaluated the results in the light of literature. Real-life data of conjunctivalchalasis patients are presented. However, more number of patients and prospective studies with the control group are needed to clarify this issue. 54

74. Meibomian bez disfonksiyonuna bağlı kuru göz şikayeti olan hastalarda meibomiografinin kuru göz parametreleriyle ilişkisi

AMAÇ: Kuru göz şikayeti olan hastalarda meibomiografinin kuru göz parametreleriyle ilişkisini araştırmak.GEREÇ ve YÖNTEM: Balıkesir Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalı Polikliniğine Mayıs 2018- Temmuz 2018 tarihleri arasında başvuran 60 hastanın 120 gözü çalışmaya alındı. Tüm hastalara; EİDGK ölçümü, biomikroskopik ve oftalmoskopik muayene yapılarak meibomian bez disfonkiyonu olan ve olmayan iki grup oluşturulmuştur. Grup 1 (n=30) meibomian bez disfonksiyonu olmaksızın, grup 2 (n=30) meibomian bez disfonksiyonuna bağlı kuru göz şikayeti olan katılımcılar olarak tanımlanmıştır. Sonrasında tüm hastalara sırasıyla oküler yüzey hastalığı indeksi (Ocular Surface Disease Index OSDI) anketi, gözyaşı ozmolaritesi ölçümü, Scheimplug topografi ile meibomiografi, Optik Koherans Tomografi (OKT) ile alt kapak gözyaşı menisküs yüksekliği (GMY) ölçümü, gözyaşı kırılma zamanı (GKZ) ve Schirmer testi yapıldı. Hastaların sağ ve sol gözler meibomiografik görüntüleri meiboskala metoduyla göre 0'dan 4'e kadar 5 evreye derecelendirildi. Subgrup analizlerinde meibomian bez disfonksiyonu olan ve olmayan hastalara meibomian bez kaybı dikkate alınarak hastalık şiddetine göre düşük ve yüksek dereceli olarak gruplandırıldı. Meibomioskalaya göre evre 2 altı olanlar düşük dereceli, evre 2 ve üstü olanlar yüksek dereceli olarak gruplandırıldı. Düşük ve yüksek dereceli gruplar arasında da tüm kuru göz parametreleri değerlendirilmiştir.BULGULAR: Çalışmaya toplamda 25'i kadın (%41.7), 35'i erkek (%58.3) toplam 60 kuru göz hastası alınmıştır. Hastaların yaşları 19-75 arasında değişmekte olup, yaş ortalaması 41.97 ±14.36 yıl olduğu görülmüştür.Çalışmaya alınan hastaların OSDI skorlarının ortalaması 39.66 ±17.02 bulunmuştur. Çalışmamıza dahil edilen sağ gözlerin gözyaşı osmolarite ortalaması 309.08±11.90 mosml, gözyaşı kırılma zamanı ortalaması 8.97±5.62 sn, gözyaşı menisküs yükseklik ortalaması 184.60 ± 40.67 mikron, Schirmer testi ortalaması 8.68 ± 7.65 mm bulunmuştur. Çalışmamıza dahil edilen sol gözlerin invaziv gözyaşı kırılma zamanı ortalaması 9.43±6.25 sn, gözyaşı menisküs yükseklik ortalaması 181.77 ±44.10 mikron, Schirmer testi ortalaması 8.45 ± 7.68 mm bulunmuştur. Çalışmaya katılan meibomian bez disfonksiyonu olmaksızın kuru göz şikayeti olan ve meibomian bez disfonksiyonuna bağlı kuru göz şikayeti olan gruplar arasında yaş, OSDI, gözyaşı menisküs yüksekliği, osmolarite, gözyaşı kırılma zamanı, Schirmer testi ortalamaları ve dağılımları arasında istatistiksel olarak anlamlı farklılık yoktu. ( p > 0.05 )Meibomian bez disfonksiyonu olmayan grupta subgrup analizinde sol ve sağ gözyaşı menisküs yüksekliği parametresinde anlamlı istatistiksel farklılık bulunmuştur. Meibomian bez disfonksiyonu olmayan hastalarda sol göz yaşı menisküs yüksekliği ortalaması düşük dereceli grupta 184.40 mikron, yüksek dereceli grupta 144.80 mikron (p = 0.04); sağ gözyaşı menisküs yüksekliği ortalaması düşük dereceli grupta 190.58 mikron, yüksek dereceli grupta 143.00 mikron olarak bulunmuştur.(p = 0.001) Sağ gözyaşı kırılma zamanı ortalaması düşük dereceli grupta 9.13 sn; yüksek dereceli grupta 6.67 sn olarak bulunmuştur. (p = 0.041)SONUÇ: Meibomian bez disfonksiyonu olan ve olmayan grup arasında yaş, OSDI, gözyaşı menisküs yüksekliği, osmolarite, Schirmer testi ortalamalarında anlamlı fark bulunmadı. Subgrup analizlerinde hastalık şiddeti yüksek olan grupta tüm parametreler kuru göz lehine saptanmasına rağmen istatistiksel olarak anlamlı fark sadece meibomian bez disfonksiyonu olmayan hastalarda sol ve sağ gözyaşı menisküs yüksekliği ve sağ gözyaşı kırılma zamanı parametrelerinde saptanmıştır. OBJECTIVE: To investigate the relationship of meibomiography with dry eye parameters in patients with dry eye symptoms.MATERIAL and METHODS: 120 eyes of 60 patients who applied to Balıkesir University Faculty of Medicine Department of Ophthalmology between May 2018 and July 2018 were included in the study. All patients have been divided into two groups as with (Group 1, n: 30)/without (Group 2, n:30)meibomian gland dysfunction and have been examined that include best-corrected visual acuity (BCVA), slit-lamp biomicroscopic and ophthalmoscopic examination. After that, Ocular Surface Disease Index(OSDI) questionnaire, tear-osmolarity measurement, meiomiography with Scheimplug topography, optical coherence tomography (OCT) and lower tear meniscus height(TMH) measurement, break-up time (BUT) and Schirmer test has been performed respectively for all participants. Meibomiographic images of right and left eyes of the patients were graded from 0 to 4 according to meiboscale method in 5 stages. In subgroup analysis, patients with/without meibomian gland dysfunction were taken into consideration and grouped as low or high grade dry eye according to severity of disease. Patients who have been graded under stage 2 due to meiboscale has been defined as low grade and graded as 2 or more has been defined as high grade dry eye disease. Dry eye parameters have been compared in low and high grade groups.RESULTS: A total of 60 dry eye patients were included in this study. Of the patients, 25 (41.7%) were female and 35 (58.3%) as male. The mean age was 41.97 ± 14.36 years (ranged from 19 to 75 years). In right eyes; the mean of tear osmolarity of the right eyes was 309.08 ± 11.90 mosml and tear break-up time was 8.97 ± 5.62 sec and the mean tear meniscus height was 184.60 ± 40.67 microns and the mean Schirmer test was 8.68 ± 7.65 mm. In left eyes; the mean break-up time was 9.43 ± 6.25 sec, the mean tear meniscus height was 181.77 ±44.10 microns and Schirmer test mean was 8.45 ± 7.68 mm. There was no statistically significant difference between age, OSDI, tear meniscus height, osmolarity, tear break-up time, Schirmer test averages and distribution between the groups with dry eye complaint due to meibomian gland dysfunction without meibomian gland dysfunction.(p>0.05) A significant statistical difference was found in the left and right tear meniscus height parameters. In subgroup analysis, there was statistically significant difference between right and left tear meniscus height parameter in dry eye patients without meibomian gland dysfunction. In dry eye patients without meibomian gland dysfunction, the mean height of meniscus in the left eye was 184.40 microns in low-grade group and 144.80 microns in high-grade group (p = 0.04). In dry eye patients without meibomian gland dysfunction, the mean height of meniscus in the right eye was 190.58 microns in low-grade group and 143.00 microns in high-grade group (p = 0.001) The mean tear break-up time was 9.13 sec in low-grade group; 6.67 sec in high grade group.(p=0.041) DISCUSSION: There was no significant difference in age, OSDI, tear meniscus height, tear osmolarity and Schirmer test in dry eye patients with or without meibomian gland dysfunction. In subgroup analysis, there was statistically significant difference in both right and left tear meniscus height and right break-up time, although all dry eye parameters were found in favor of dry eye in the group with high disease severity. 68

Objectives: To compare the effectiveness of electrocoagulation and conventional medical drops for treatment of conjunctivochalasis using anterior segment-optical coherence tomography (AS-OCT). Materials and Methods: Forty eyes of 20 patients with bilateral conjunctivochalasis were included in this prospective study. Twenty eyes of 10 patients were assigned to Group 1 and underwent electrocoagulation. The other 20 eyes of 10 patients were assigned to Group 2 and received conventional medical treatment consisting of non-steroidal antiinflammatory drop (topical 0.5% ketorolac tromethamine) 4 times a day and artificial tears (0.15% sodium hyaluronate) 6 times a day for 4 weeks. Before and 4 weeks after treatment, all patients were evaluated by slit-lamp biomicroscopy, tear film break-up time (TBUT) test, and ocular surface disease index (OSDI) questionnaire. Tear meniscus height (TMH), tear meniscus area (TMA), and conjunctivochalasis area (CCA) were measured with AS-OCT. Results: In Group 1, posttreatment values of TMH, TMA, and TBUT were significantly higher (p0.05 for all values). 77. Evaluation of Ocular Surface Disease in Glaucoma Patients Using Topical Antiglaucomatous Agents

Aim: Evaluation of ocular surface disease (OSD) in glaucoma patients using topical antiglaucomatous agentsPatients and Method: 159 eyes of 83 patients at the age 18 years or over, who had any types of different glaucoma and had been using antiglaucomatous agents for at least 6 months were included in the study. The Ocular Surface Disease Index (OSDI) questionnaire was filled by asking patients individually. Afterwards, in order to examine ocular surface clinically, patients underwent the Schirmer test(under anesthesia) evaluation, tear break-up time(TBUT) and corneal staining. Results: A total of 159 eyes from 51 male and 32 female patients were included in the study. The average age of men was 64,51 ± 9,07 and the average age of women was 54,32 ± 11,44. Mean duration of medication use of the patients was 6.35 ± 5.63 years. 36.5% of the patients had single medication, 32.1% had two medications, 13.8% had three medications and 17.6% had four medications. Of the patients whose dry eye symptoms were assessed using the OSDI questionnaire, 3.8% were normal, 15.7% were mild, 22.6% were moderate and 57.9% were severe dry eye. The mean OSDI score of all patients was 46.80 ± 19.44. The Schirmer test was found at less than 5 mm in 34.1% of eyes. In terms of TBUT, 52.3% of the eyes were evaluated as mild, 29.5% as moderate, 8.5% as severe. When evaluated for corneal staining, normal staining was observed in 13.2%, slight staining in 39.6%, moderate staining in 23.3% and severe staining in 23.9%. Conclusion: In diagnosing OCD, it would also be beneficial to use a test that includes not only the OSDI questionnaire but also tests showing the quality and stability of the tears.

Konjonktivaşalazisin Gözyaşı MMP-9 ve Oküler Yüzey Üzerine EtkisiAmaç: Grade 2 veya 3 konjonktivaşalazisli hastalarda farklı tedavi yöntemlerinin gözyaşı MMP-9 ve oküler yüzey üzerine etkisini değerlendirmek.Yöntem: Bu prospektif çalışmaya Yokoi evrelemesine göre grade 2 veya 3 semptomatik konjonktivaşalazisi bulunan 66 hasta alındı. Bireyler aldıkları tedavi yöntemine göre üç gruba ayrıldılar. Birinci gruptaki 22 hastaya prezervansız suni gözyaşı damla (Tears Naturale Free, Alcon), ikinci gruptaki 22 hastaya prezervansız suni gözyaşına ilaveten antiinflamatuar ajan (Acular LS, Allergan), üçüncü gruptaki 22 hastaya konjonktiva elektrokoterizasyonu tedavisi verildi. Bireylerin tedavi öncesi, tedavinin birinci ve üçüncü ayında gözyaşı ozmolaritesi, gözyaşı MMP-9 seviyesi (İnflammaDry test), Schirmer testi, gözyaşı kırılma zamanı (TBUT), oküler yüzey hastalık indeksi (OSDI) anketi, korneal floresein boyanma (OXFORD) evrelemesi ve sağ göz konjonktivasından impresyon sitolojisi için örnek alındı.Bulgular: Tedavi öncesi gruplar arasında yaş, cinsiyet, gözyaşı fonksiyon testleri; gözyaşı ozmolaritesi, Schirmer testi, gözyaşı kırılma zamanı (TBUT), oküler yüzey hastalık indeksi (OSDI) anketi, korneal floresein boyanma (OXFORD) evrelemesi açısından fark yok idi (hepsi, p>0,05). Tedavi öncesi gruplar arasında MMP-9 seviyesi açısından fark bulunmazken (p>0,05) impresyon sitolojisi ile değerlendirilen Nelson evreleri gruplar arasında farklı bulundu (p0,05). Birinci grupta üç aylık tedavi sonucunda OSDI skoru ve korneal floresein boyanmada anlamlı azalma, Schirmer testinde anlamlı artış vardı (p0,05). Üç aylık tedavi sonucunda birinci grupta ve ikinci grupta MMP-9 seviyesi anlamlı değişim göstermezken üçüncü grupta MMP-9 pozitifliğiistatistiksel olarak anlamlı düşüş gösterdi (p0,05).Sonuç: Konjonktivaşalazise bağlı kuru göz bulguları ve oküler yüzey inflmasyonu görülmektedir. The effect of Conjunctivochlasis on Tear MMP-9 Level and Ocular SurfacePurpose: To assess the effect of different treatment methods on tear MMP-9 level and ocular surface in patients with grade 2 or 3 conjunctivochalasis.Methods: 66 patients with grade 2 or 3 symptomatic conjunctivochlasis according to Yokoi grading method included in this study. Individuals were divided into three groups according to the treatment method. 22 patients in group 1 were treated with preservative-free artificial tear drops (Tears Naturale Free, Alcon), 22 patients in group 2 were treated with preservative-free artificial tear drops and ophthalmic anti-inflammatory agent (Acular LS, Allergan), and 22 patients in group 3 were treated with conjunctival electrocauterization method. Tear osmolarity measurement, tear MMP-9 level (inflammaDry test), Schirmer test, fluorescein break-up time (BUT), ocular surface disease index questionnaire (OSDI), corneal fluorescein staining (OXFORD) and conjunctival impression cytology procedure (right eye) were carried out before treatment and at the first and third month of the treatment.Results: Before treatment there was no difference between the groups according to age, sex, and tear function tests including tear osmolarity test, Schirmer test, tear break-up time test (TBUT), OSDI questionnaire, corneal fluorescein staining (OXFORD) (all, p>0.05). While there was no difference in MMP-9 level before the treatment (p>0.05), the difference in Nelson's grading system of conjunctival impression cytology procedure was significant between the groups (p0.05). There was a significant reduction in OSDI score and corneal fluorescein staining and significant increment in Schirmer test results of the first group at the third month of the treatment (all, p< 0.05). The TBUT and Schirmer test results were significantly increased whereas OSDI score was significantly decreased in the second group. (all, p0.05). After three month treatment no significant difference was occurred in the levels of MMP-9 in the first and second group, but there was a significant decrease in the level of MMP-9 positive patients in the third group (p0.05).Conclusion: Dry eye signs and ocular surface inflammation caused by conjunctivochalasis can be seen. Objectives: To evaluate ocular surface health in obstructive sleep apnea syndrome (OSAS) and to investigate the tendency of these patients toward dry eyes. Materials and Methods: Fifty patients who underwent polysomnography and were diagnosed with OSAS and 50 normal control subjects were compared with respect to ocular surface disease index (OSDI), Schirmer I test and tear film break-up time (TBUT) values. Amaç: Bu çalışma keratokonus tanısı ile sert gaz geçirgen kontakt lens (SGGKL) kullanan hastaların, gözyaşı ve oküler yüzey değişikliklerini; SGGKL kullanmayan keratokonus hastaları ve sağlıklı popülasyon ile karşılaştırarak keratokonus seyrinde SGGKL'nin rolünü değerlendirmek amacıyla yapılan interdisipliner, klinik bir çalışmadır.Gereç ve Yöntemler: SGGKL kullanan 24 keratokonus hastası (grup 1); lenssiz izlem yapılan 22 keratokonus hastası (grup 2), oküler ve sistemik hastalığı olmayan 21 sağlıklı birey (grup 3) çalışma kapsamına alındı. Gözlerindeki subjektif yakınmalar oküler yüzey hastalıkları indeksi (OSDI) anketiyle sorgulandı. Düzeltilmiş en iyi görme keskinliği kontrolü, biyomikroskopik ön segment ve fundus muayeneleri tamamlandı. Pentacam - Scheimflug görüntüleme sistemiyle kornea topografileri incelendi. Schirmer testi, gözyaşı kırılma zamanı (GKZ), florosein boya şeritleriyle kornea ve konjonktiva boyanması ve bu boyanma şeklinin Oxford kornea boyanma skalasına göre derecelendirilmesi ve konjonktiva impresyon sitolojisini içeren detaylı muayeneleri yapıldı. Bulgular: Schirmer testi, GKZ, gözyaşı ozmolarite sonuçları grup 1'de sırasıyla 11.01±7.08mm, 5.68±2.66sn, 290.29±8.72mOsm/L,; grup 2'de 11.1±6.53mm, 8.14±6.41 sn, 293.12±9.97mOsm/L; grup 3'te ise 14.64±6.41mm, 13.68±7.93 sn, 293.03±9.06 mOsm/L olarak bulunmuştur. Oxford ve OSDI skorları ise grup 1'de 0.82±0.72, 17.54±12.16; grup 2'de 0.57±0.74, 21.26±15.99 ve grup 3'te 0.08±0.28, 7.02±6.92'dir. Sonuç: Grup 1 ve 2 arasında gözyaşı film tabakasının değerlendirilmesinde kullanılan Schirmer testinde anlamlı fark yokken (p=0.953) , GKZ değerlendirmesinde ise anlamlı fark bulunmuş (p=0.042) gözyaşı ozmolaritesinde ise anlamlılık izlenmemiştir (p=0.214). Oxford ve OSDI skorlamalarında iki grup arası anlamlı fark bulunamamıştır (p=0.152 ve p=0.271). Glob konjonktivasına ait Saini impresyon sitolojisi sonuçlarında gruplar arasında anlamlı fark bulunamamıştır. Purpose: This interdisciplinary, clinical study investigates changes in tear and ocular surface of patients with keratoconus using rigid gas permeable contact lenses (RGPCL) and compares them to keratoconus patients who ado not wear lenses and a control group with healthy subjects.Material-Method: 24 keratoconus patients using RGPCL (Group 1) 22 patients who do not wear lenses (Group 2) and 21 healthy subjects (Group 3) are included in the study. Subjective complaints about the subjects' eyes have been investigated using the ocular-surface disease index (OSDI). After the control of best-corrected visual acuity, anterior chamber and fundus examinations are performed. All subjects' anterior chambers are scanned using the Pentacam-Scheimflug scanning system. Schirmer-I test, tear break-up time test, fluorescein staining and Oxford scheme for grading ocular surface staining have been applied to all the subjects.Findings: Schirmer test, tear break-up time test and tear osmolarity values in group 1 are 11.1±6.53mm, 5.68±2.66sn, 290.29±8.72mOsm/L, respectively. They are 11.1±6.53mm, 8.14±6.41 sn, 293.12±9.97 mOsm/L, in group 2 and 14.64±6.41mm, 13.68±7.93 sn, 293.03±9.06 mOsm/L, in group 3. Oxford and OSDI scores are 0.82±0.72, 17.54±12.16 in group 1; 0.57±0.74, 21.26±15.99 in group 2 and 0.08±0.28, 7.02±6.92 in group 3. Conclusion: Schirmer test didn't reveal any significant differences (p=0.953) between group 1 and 2 but tear break-up time test did (p=0.042). Tear osmolarity between group 1 and 2 is also not significantly different (p=0.214). Neither do Oxford and OSDI scores between the two groups indicate significant differences (p=0.152, p=0.271). Conjunctiva Saini impression cytology analysis didn't indicate any significant differences. 75

Bu amaçla çalışmaya en az 6 ay süreyle silikon hidrojel kontakt lens kullanan ve meibomius bez disfonksiyonu olan 20 hasta, en az 6 ay süreyle silikon hidrojel kontakt lens kullanan ve meibomius bez disfonksiyonu olmayan 20 hasta, meibomius bez disfonksiyonu olan ve kontakt lens kullanmayan 20 hasta ve oküler ve sistemik hastalığı olmayan 20 sağlıklı birey olmak üzere toplam dört grupta gözyaşı kırılma zamanı, oküler yüzey boyanması, Schirmer testi, OSDI (Ocular Surface Disease Index) anketi ile gözyaşı fonksiyon değerlendirmesi yapılmış ve hastaların gözyaşında IL-1RA, IL-1β, IL-2, IL-2R, IL-4, IL-5, IL-6, IL-7, IL-8, IL-10, IL-12, IL-13, IL-15, IL-17, IFN-α, IFN-γ, TNF-α, GM-CSF, IP-10, MIG, RANTES, eotaksin, MIP-1α, MIP-1β, MCP-1 seviyeleri luminex ile çalışılmıştır. Kontakt lens kullanan ve meibomius bez disfonksiyonu olan grupta kontrol grubuna göre gözyaşı kırılma zamanı daha düşük (p=0.048), OSDI skorları (p=0.001) ve oküler yüzey boyanması (p=0.032) daha yüksek bulunmuştur. In this study we investigated tear function tests; tear break-up time, ocular surface staining, Schirmer test, OSDI (Ocular Surface Disease Index) questionaire and tear IL-1RA, IL-1β, IL-2, IL-2R, IL-4, IL-5, IL-6, IL-7, IL-8, IL-10, IL-12, IL-13, IL-15, IL-17, IFN-α, IFN-γ, TNF-α, GM-CSF, IP-10, MIG, RANTES, Eotaxin, MIP-1α, MIP-1β, MCP-1 levels with luminex in 20 patients who have used silicone hydrogel contact lenses for at least 6 months coexisting meibomian gland dysfunction (group 1), 20 patients who have used for at least 6 months silicone hydrogel contact lenses without meibomian gland dysfunction (group 2), 20 patients who had meibomian gland dysfunction and who were not using contact lenses (group 3) and 20 healthy individuals without any known systemic and ocular diseases and who were not using contact lenses (group 4). In group 1 tear break-up time was lower (p=0.048); ocular surface staining (p=0.032) and OSDI scores (p=0.001) were higher than the control group but tear cytokine levels were similar in all groups. Dünya popülasyonunun %1-3'ünü etkileyen psoriasis eklemlerin yanı sıra mukozaları da tutabilir. Coğrafik dil, psoriasisin oral mukoza bulgusu olabilir ve göz kapakları, kornea, uvea, retina ve gözyaşı bezleri gibi göz bölgeleri hastalık seyri sırasında tutulabilir.Çalışmamızda; erişkin psoriasis hastalarında göz bulgularını değerlendirmeyi ve saptanan göz bulgularının cinsiyet, hastalık şiddeti ve oral mukoza, tırnak, eklem tutulumu ve de baş-boyun bölgesindeki psoriatik plaklar ile ilişkisini araştırmayı amaçladık. Çalışmaya klinik bulgularla ve/veya histopatolojik olarak psoriasis tanısı almış 48 hasta ile yaş ve cinsiyet olarak uyumlu 48 sağlıklı kontrol olgusu dahil edildi. Olguların psoriasis tipi, hastalık süresi, tırnak, eklem ve oral mukoza tutulumları saptandı. Hastalık şiddeti, Psoriasis Alan Şiddet İndeksi (PAŞİ) ve Dermatoloji Yaşam Kalite İndeksi (DYKİ) ölçekleri kullanılarak ölçüldü. PAŞİ ve DYKİ skorları 10'un altında olanlar `hafif`, 10 ve 10'un üzerinde olanlar `şiddetli` psoriasis olarak değerlendirildi. Tüm olgulara biyomikroskop ve fundus bakısı ardından gözyaşı kırılma zamanı (BUT) ve Schirmer-1 testleri yapıldı. Schirmer-1 testi 10 mm'nin altında ve BUT testi 10 saniyenin altında olan hastalara `göz kuruluğu` olarak tanı kondu.Psoriasisli hastaların %2,1'inde konjonktivit, %6,3'ünde punktat epitelyopati, %2,1'inde nükleer katarakt, %2,1'inde korneal opasite, %29,2'sinde blefarit, %18,8'inde sağ göz kuruluğu, %14,6'sında sol göz kuruluğu vardı. Hastaların %62,5'inde azalmış göz yaşı kırılma zamanı saptandı. Kontrol grubu ile karşılaştırıldığında; blefarit, sağ göz kuruluğu ve göz yaşı kalitesinde bozulma hasta grubunda istatistiksel olarak daha yüksek bulundu. Blefarit; kadın hastalarda ve PAŞİ ve DYKİ skoru 10'un üzerinde olan hastalarda daha sıktı.Psoriasisin deri ve eklem tutulumunun yanı sıra gözleri de tutabileceği akılda bulundurulmalıdır. Çalışmamızın sonuçlarına göre; blefarit, göz kuruluğu ve gözyaşı kalitesinde bozulma psoriasis hastalarında daha sık saptanmıştır. Blefarit, özellikle `şiddetli` psoriasis hastalarında daha çok görülmüştür. Sonuç olarak; özellikle şiddetli psoriasis hastalarının göz hekimine yönlendirilmesinin uygun olduğunu düşünmekteyiz. Psoriasis is a chronic disease characterized by erythematous, sharply-circumscribed plaques and papules covered with silvery white scales which tend to be especially localized to scalp and extensor surfaces. Psoriasis, which influences 1-3% of the population, may have impact on joints as well as mucosa. Geographic tongue may be an oral mucosal manifestation of psoriasis and any part of the eye; such as eyelids, conjunctiva, cornea, uvea, retina and tear glands may be involved in the disease process.In our study; we aimed to search ocular symptoms in adult psoriatic patients and its relationship with gender, severity of disease, and oral mucosa, nail, joint involvement and also with existence of psoriatic plaques in head–and-neck region.Fourty eight patients who are clinically and/or histopathologically diagnosed with psoriasis and sex-age matched 48 healthy individuals were included in the study. Their clinical type of psoriasis, duration of disease, nail, joint and oral mucosal involvement are recorded. Severity of the disease was measured with Psoriasis Area Severity Index (PASI) and Dermatology Life Quality Index (DLQI). Patients with PASI and DLQI scores under 10 are considered as `mild`, whereas scores 10 and over are considered `severe` psoriasis. All cases had biomicroscopic and fundus examinations followed by tear break-up time (BUT) test and Schirmer-I tests. Patients with Schirmer test below 10 mm and Break-up time test under 10 second are diagnosed as `dry eye`.In psoriasis patients; 2.1% had conjunctivitis, 6.3% had punctate epitheliopathy, 2.1% had nuclear cataract, 2.1% had corneal opacity, 29.1% had blepharitis, 18.8% had right eye dryness, 14.6% had left eye dryness. Decreased break-up time was recorded in 62.5% of the patients. When compared with control group; blepharitis, right eye dryness and decreased quality of tear film was found statistically higher in patient group. Blepharitis was more frequent in women and in patients with PASI and DLQI scores over 10.Besides joint and skin involvement, it must be kept in mind that psoriasis can affect the eyes. According to our study; blepharitis, eye dryness and reduced quality of tear film are detected more frequently in psoriasis patients. Blepharitis was more commonly seen especially in `severe` psoriasis patients. As a result, especially severe psoriasis patients have to be referred to a ophthalmologist. 74

Objectives: The effects of various hormonal replacement treatments (HRT) on tear functions in postmenopausal women were evaluated. Material and Methods: The study group consisted of 84 postmenopausal women, 26 patients using conjugated estrogen (Group1), 15 patients using conjugated estrogen + medroxyprogesterone acetate (Group 2), 19 patients using Tibolone (Group 3) and 24 patients as control group (Group 4). Schirmer test was performed and tear break up time (BUT) was measured. The mean of right and left eye measurements were used in the study. Groups that receiving HRT were compared with each other and with control group. Results: Mean age of 84 postmenopausal women was 51.97±5.70 years. Mean value of right and left eye Schirmer test was 18.57±3.98 mm in group 1, 20.80±2.84 mm in group 2, 21.00±5.05 mm in group 3, and 13.75±3.49 mm in group 4. Mean tear BUT was 12.05±3.45 sec. in group 1, 12.90±2.13 sec. in group 2, 14.00±2.77 sec. in Group 3, 8.10±3.21 sec. in Group 4. Schirmer test results and tear break up time of all groups that receive HRT were statistically higher than the control group. There was no statistical significant difference between the HRT receiving groups. 84. Evaluation of Ocular Surface Changes Using Impression Cytology in Acne Vulgaris Patients

Purpose: Acne vulgaris is a disease with no known ocular involvement and is most commonly seen in the younger population. The aim of this study was to examine the ocular surfaces of acne vulgaris patients with no ocular signs and symptoms by impression cytology. Methods: The study groups included 74 eyes of 37 acne vulgaris patients and 28 eyes of 14 healthy control subjects from the same age group. Schirmer I test, the tear break-up time test and the conjunctival impression cytology were performed in each patient. Results: We observed no difference between the tear break-up time and the Schirmer's test values between the two groups. However, there was a statistically significant difference between the two groups in terms of impression cytology results (p=0.0001). Conclusion: It was determined that impression cytology properties were altered in patients with acne vulgaris although there was no ocular involvement. Consequently, we suggest that acne vulgaris patients require more detailed ophthalmologic evaluation

85. Clinical Findings, Follow-up and Treatment Results in Patients with Ocular Rosacea

Objectives: To report the clinical features, treatment options and complications in patients with ocular rosacea. Materials and Methods: The records of 48 eyes of 24 patients with ocular rosacea were retrospectively reviewed. Patients’ ocular signs and symptoms were scored between 1 and 4 points according to disease severity; tear film break-up time (BUT) and Schirmer’s test results were recorded before and after the treatment. Preservative-free artificial tears, topical antibiotic eye drops/ointments, short-term topical corticosteroids, topical 0.05% cyclosporine and oral doxycycline treatment were applied as a standard therapy to all patients. Additional treatments were given as needed. Complications were recorded. Results: Twenty-four patients with a mean age of 48.5±35.4 (32-54) years were followed for a mean 15±9.4 (8-36) months. Ocular findings included meibomitis in 100% of cases, anterior blepharitis in 83% (40 eyes), punctate keratopathy in 67% (32 eyes), chalazia in 50% (24 eyes), corneal neovascularization in 50% (24 eyes) and subepithelial infiltrates in 16.6% (8 eyes). 86. Effect of Systemic Infliximab Therapy in Patients with Sjögren’s Syndrome

Objectives: To investigate the effect of systemic infliximab therapy on tear function tests and the ocular surface in patients with Sjögren’s syndrome secondary to various autoimmune diseases. Materials and Methods: This prospective study included 22 eyes of 22 patients with Sjögren’s syndrome who began treatment with systemic infliximab. Tear film break-up time (TBUT), anesthetized Schirmer’s 1 test, fluorescein staining test, and Ocular Surface Disease Index (OSDI) scores were recorded before treatment and in the 3rd and 6th months of treatment. Results: In the 3rd month of infliximab therapy, no significant changes were observed in Schirmer’s values, TBUT, fluorescein staining, or OSDI scores (p=0.260, p=0.357, p=0.190 and p=0.07, respectively). In the 6th month of infliximab therapy, no significant changes were observed in TBUT, fluorescein staining, Schirmer’s value or OSDI scores (p=0.510, p=0.320, p=0.220 and p=0.344, respectively). Conclusion: Infliximab therapy, which is commonly used in systemic autoimmune diseases such as rheumatoid arthritis, Crohn’s disease, ulcerative colitis, and ankylosing spondylitis, did not show a positive effect on ocular surface and tear function tests. Keratokonus etiyolojisi ve progresyonunda gözyaşı ozmolaritesinin etkisi Amaç: Keratokonus hastalarında gözyaşı fonksiyon testleri ve gözyaşı ozmolaritesinin keratokonus etiyolojisi ve progresyonundaki etkisinin araştırılması.Gereç ve Yöntem: Ondokuz Mayıs Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalı?nda Eylül 2011- Şubat 2012 tarihleri arasında keratokonus tanısı konulan 28 hastanın 50 gözü ve refraksiyon kusuru dışında herhangi bir göz hastalığı bulunmayan 20 bireyin 40 gözü prospektif olarak incelendi. Her iki gruba rutin oftalmolojik muayeneyi takiben korneanın keratometrik ölçümleri, gözyaşı kırılma zamanı (GKZ), Schirmer testi, TearLab Ozmolarite Sistemi ile gözyaşı ozmolaritesi ölçümleri yapıldı. Sistemik hastalıkları ve ilaç kullanım öyküleri sorgulandı. Her iki grup ve keratokonus hasta grupları (hafif, orta, ileri evre) arasında elde edilen değerler istatistiksel olarak karşılaştırılarak değerlendirildi.Bulgular: Yaş ve cinsiyet açısından iki grup arasında istatistiksel açıdan anlamlı bir farklılık yoktu. Ozmolarite ölçüm ortanca değerleri keratokonus grubunda 300,9 (279,0-350,0) mOsm/L, kontrol grubunda 298 (277,0-321,0) mOsm/L olarak tespit edildi (p=0,108). Schirmer testi ölçüm ortanca değerleri keratokonus grubunda 23,0 (6,0-35,0) mm, kontrol grubunda 25,0 (9,0-35,0) mm (p=0,793) idi. Gözyaşı kırılma zamanı ölçüm ortanca değerleri keratokonus grubunda 11,0 (5,0-18,0) sn, kontrol grubunda 12,5 (6,0-15,0) sn idi (p=0,793). İki grup arasında bu üç parametre için istatistiksel olarak anlamlı fark yoktu.Keratokonus hasta grupları ortalama korneal keratometri ölçümlerine göre hafif (grup 1), orta (grup 2), ileri (grup 3) evre olarak sınıflandırıldı. Ozmolarite ölçüm ortanca değerleri grup 1, 2 ve 3?de sırasıyla 297,5 (284,0-316,0) mOsm/L, 300,0 (279,0-329,0) mOsm/L, 305,0 (287,0-350,0) mOsm/L idi. Kruskal-Wallis testi analizi ile ozmolarite değerleri, keratokonus grupları arasında istatistiksel olarak anlamlı derecede farklı bulundu (p=0,014). The Effect of Tear Osmolarity on the Etiology and Progression of Keratoconus Purpose: To investigate the effects of tear functions tests and tear osmolarity on the etiology and progression of keratoconus in patients with keratoconus. Materials and Methods: Fifty eyes of 28 patients diagnosed with keratoconus at Ondokuz Mayıs University, Ophthalmology Department between September 2011 and February 2012 and forty eyes of 20 individuals without any other eye disease except for refraction error were examined prospectively.Following by routine ophthalmological examination, keratometry measurement, tear film break up time (BUT), Schirmer?s test and tear osmolarity measurement using the TearLab Osmolarity System were performed to the both groups. Systemic diseases and drug therapies were recorded. The results obtained from both groups and keratoconus groups were compared and statistically evaluated. Results: No statistically significant difference was observed between two groups with respect to age and gender. The mean osmolarity values in keratoconus and control group were 300.9 (279.0- 350.0) mOsm/L and 298.0 (277.0-321.0) mOsm/L respectively (p=0.108). The mean Schirmer?s test values were 23.0 (6.0 -35.0) mm and 25.0 (9.0-35.0) mm in keratoconus and control group respectively (p=0.793). The mean tear break up time values were 11.0 (5.0-18.0) in keratoconus group and 12.5 (6.0 -15.0) in control group (p=0.793). No statistically significant difference was observed between these two groups with respect to these three parameters.In accordance with the mean keratometry measurements, keratoconus patient groups were classified as mild (group 1), moderate (group 2) and severe (group 3). The mean osmolariy values were 297.5 (284.0-316.0) mOsm/L, 300.0 (279.0-329.0) mOsm/L and 305.0 (287.0-350.0) mOsm/L in group 1, 2 and 3, respectively. The mean Schirmer?s test values were 25.0 (9.0-35.0) mm, 25.0 (6.0-35.0) mm and 19.5 (6.0-35.0) mm in group 1, 2 and 3 respectively. The mean tear break up time measurement values of group 1, 2 and 3 were 12.0 (6.0-15.0) sec, 13.0 (6.0-18.0) sec and 10.0 (5.0 -15.0) sec respectively. Kruskal-Wallis test analysis revealed statistically significant difference between keratoconus groups with respect to osmolarity values (p=0.014). Schirmer?s test and tear break up time (BUT) measurement values were not statistically significant (Schirmer?s test p=0.308; BUT test, p=0.133). Bonferroni corrected Mann-Whitney U-test revealed statistically significant difference between group 1 and group 3 with respect to osmolarity values (p=0.008).Tear osmolarity, tear film break up time and Schirmer?s test results of mild, moderate and severe keratoconus and control groups were compared. The difference between groups was not statistically significant (tear osmolarity measurement p=0.080; Schirmer?s test p=0.502; BUT test p=0.200).Conclusion: The increase in tear osmolarity values in keratoconus patients correlated with the increase in keratoconus degree. Hyperosmolar tears may be an influential factor in the etiology and progression of keratoconus. 71

Gözyaşı üretiminin yetersizliği ya da buharlaşmanın fazla oluşu nedeniyle ortaya çıkan kuru göz hastalığı, son yıllarda görülme sıklığı ve yaşam kalitesine olan olumsuz etkisi bakımından giderek artan öneme sahip olmaktadır. Kuru gözün tedavisinde hala geleneksel tedavi biçimi olan suni gözyaşı kullanımı yaygın olmakla birlikte tüm dünyada hastalığın inflamatuar bir hadisenin neticesinde ortaya çıktığı kabul görmeye başlamakta ve buna yönelik tedavi modelleri üzerine çalışmalar hız kazanmaktadır.Kuru göz hastalığında, tek başına suni gözyaşı tedavisi ile siklosporin A ve suni gözyaşı kombinasyonu tedavisinin, kuru gözün klinik bulgularına ve goblet hücre yoğunluğuna etkilerinin karşılaştırılmasını amaçlayan bu prospektif, randomize, kontrollü çalışmaya, anestezisiz Schirmer testi değeri 10 mm'nin altında, gözyaşı kırılma zamanı 10 sn'nin altında ölçülen 42 hastanın 42 gözü dahil edilmiştir. Rastgele iki gruba ayrılan hastaların(22 müdahale ve 20 kontrol hastası), tedaviye başlamadan önce Schirmer test skoru, gözyaşı kırılma zamanı, Oxford şemasından yararlanılarak derecelendirilen lissamin yeşiliyle konjonktival boyanma ve floreseinle korneal boyanma ile impresyon sitolojisi yöntemiyle bakılan goblet hücre yoğunluğu değerleri kaydedilmiştir. Kontrol grubu hastalarına 4 ay boyunca 4×1 suni gözyaşı tedavisi, müdahale grubu hastalarına ise 2×1 %0,05'lik siklosporin A ile 4×1 suni gözyaşı kombinasyonu tedavisi başlanmıştır. Dört aylık tedavi sonunda kaydedilen Schirmer test skoru, gözyaşı kırılma zamanı, lissamin yeşiliyle konjonktival boyanma derecesi, floreseinle korneal boyanma derecesi ve impresyon sitolojisi yöntemiyle bakılan goblet hücre yoğunluğu değerleri, tedavi öncesi değerlerle karşılaştırılmıştır.Kombinasyon tedavisi alan grubun Schirmer test skoru, gözyaşı kırılma zamanı, lissamin yeşiliyle konjonktival boyanma derecesi, floreseinle korneal boyanma derecesi ve impresyon sitolojisi yöntemiyle bakılan goblet hücre yoğunluğu değerlerinde başlangıca göre istatistiksel olarak anlamlı iyileşme görülürken, tek başına suni gözyaşı tedavisi alan hasta grubunun da bu değerlerinde iyileşme olduğu ancak bu iyileşmenin istatistiksel olarak anlamlı olmadığı gözlemlenmiştir.Siklosporin A ve suni gözyaşı kombinasyonu tedavisi goblet hücre yoğunluğunda belirgin artış sağlamakta, kuru göz klinik bulgularını iyileştirerek oküler yüzey sağlığının olumlu yönde etkilemektedir. Dry eye disease which is resulted by aqueous tear deficiency or increased evaporation of tear, has been becoming an area of interest in recent years because of its increasing incidence and negative effects on life quality. Even though the use of artificial tears, which is the traditional treatment of dry eye, is still common; lately this disease is thought to be a result of an inflammatory process and so new treatment modalities are getting developed according to this mechanism.In our study, it was aimed to compare the effects of artificial tears and the combination of artificial tears and cyclosporine A treatment on the clinicial signs and goblet cell density in dry eye disease. Forty-two eyes of 42 dry eye patients having Shirmer test scores below 10mm/5min without anesthesia and tear break-up time below 10 sec. were enrolled in this prospective randomized, controlled study. The patients were randomly divided into two groups (22 patients as study group, 20 patients as control group) and fluorescein tear break up time (BUT), Schirmer test scores, corneal fluorescein staining and conjunctival lissamine green staining according to Oxford scheme and goblet cell density derived by impression cytology were recorded before treatment for each group. The control group had artificial tears 4×1 for 4 months and the study group had 0.05%cyclosporine A treatment 2×1 and artificial tears 4×1 for 4 months. Tear break up time (BUT), Schirmer test scores, corneal fluorescein staining and conjunctival lissamine green staining according to Oxford scheme and goblet cell density derived by impression cytology after treatment of 4 months were compared to the pretreatment values.Break up time, Schirmer scores, corneal fluorescein staining, conjunctival lissamine green staining and goblet cell density statistically significantly improved in the combination group while the test scores in the artifical tears group were also improved but the change was not statistically significant.Combination of artificial tears and cyclosporine A treatment significantly increases goblet cell density and improves ocular surface health by decreasing the signs of dry eye. 59

Korneal duyarlılık, gözyası kırılma zamanı ve Schirmer Itesti preoperatif olarak ve postoperatif 1. hafta, 1., 3. ve 6. aylarda incelendi.Subjektif kuru göz semptomları OSDI skoru ile korneal duyarlılık ise Cochet-Bonnet esteziyometresi ile degerlendirildi.BULGULAR: LASIK sonrası korneal duyarlılık ablasyon zonunda cerrahi sonrası ilk 3ay boyunca azaldı ve preoperatif degerlerine ancak 6 ay sonra ulastı. Bununlabirlikte LASEK sonrası korneal duyarlılık ilk ayda iyilesti. Amaç: LASEK’in gözyaşı fonksiyonlarında neden olduğu değişikliklerin gözyaşı sekresyonu ve stabilitesi ölçümleri ile değerlendirilmesi. Gereç ve Yöntem: Düşük ve orta dereceli miyopinin düzeltilmesi amacıyla LASEK uygulanan 20 hastanın 36 gözünde Schirmer 1 testi, bazal gözyaşı sekresyonu ve gözyaşı kırılma zamanı ölçümleri preoperarif, postoperatif 1. hafta, 1. ay, 3. ay ve 6. ayda gerçekleştirilerek sonuçları değerlendirildi. Bulgular: Schirmer test değerinde postoperatif tüm ölçümlerde preoperatif 25 9.3 mm olan ortalama değere göre anlamlı bir değişiklik gözlenmedi (p0.05). Bazal gözyaşı sekresyonu ve gözyaşı kırılma zamanında postoperatif 1. haftada (p0.01) ve 1. ayda (p0.05) istatistiksel olarak anlamlı düzeyde düşüş izlenirken 3. ayda preoperatif değerlere ulaşarak 6. ayda stabil seyrettiği saptandı. Sonuç: LASEK sonrası gözyaşı fonksiyonlarında geçici ve kısa süreli bozulma meydana gelmektedir. Bu çalışmada psöriazis ve akne vulgaris olgularında oküler yüzey değişiklikleri ve gözyaşı fonksiyonu impresyon sitolojisi, gözyaşı kırılma zamanı (GKZ), Bazal Schirmer testi (BST) ve mukus fern testi (MFT) ile değerlendirildi.Psöriazisli 32 (64 göz) olgu, akne vulgarisli 31 (62 göz) olgu ve 35 kontrol (70 göz) olgu olmak üzere çalışmaya toplam 98 (196 göz) olgu alındı. Tüm olgulara GKZ, BST ve MFT uygulandı. Temporal interpalpebral, üst ve alt nazal konjoktivadan olmak üzere 3 ayrı bölgeden impresyon sitolojisi örnekleri alınıp değerlendirildi.Gözyaşı kırılma zamanı psoriazis ve akne vulgarisli olgularda kontrol grubuna göre istatistiksel olarak anlamlı şekilde düşük bulundu (p0.05). Her iki grubun MFT paterni kontrol grubuna göre anlamlı şekilde yüksekti (p0.05). Kontrol grubunda MFT sonuçları normal olmasına karşın impresyon sitolojisi sonuçlarında goblet hücre yoğunluğu düşük olarak saptandı.Sonuç olarak psoriazis ve akne vulgaris olgularının gözyaşı fonksiyon testleri, mukus fern testi ve impresyon sitolojisi sonuçları gözyaşı tabasının etkilendiğini göstermektedir. In this study ocular surface changes and functions are evaluated by using impression cytology, tear break-up time (BUT), Basal Schirmer test (BST) and mucous ferning test (MFT).32 patient (64 eyes) with psoriasis, 31 patients (62 eyes) with acne vulgaris and 35 control patients (70 eyes) were included in this study, a total of 98 patients (196 eyes). BUT, BST and MFT is applied to each group. Impression cytology samples derived from 3 different place, temporal interpalpebral, upper and lower conjunctivas are evaluated.Tear break-up time is statistically shorter in acne vulgaris and psoriasis group comparing with the control group (p0.05). MFT patterns of each group is statistically higher comparing with the control group (p0.05). Although MFT results are normal in the control group, goblet cell density is found to be lower in impression cytology results.Consequently, results of the tear function test, impression cytology and mucous ferning test showed that tear layer is affected in patient with psoriasis and acne vulgaris. ÖZET. Amaç: Sistemik izotretinoinin uzun dönem kullanımının gözyaşı fonksiyon testlerine etkilerini araştırmak Gereç ve Yöntem: Akne vulgaris tanısıyla sistemik izotretinoin başlanan 46 olgu çalışmaya dahil edildi. Kontrol grubu oküler yüzeyi ve gözyaşı fonksiyonlarını etkileyen sistemik ve oküler bir hastalığı olmayan 15-35 yaşları arasındaki 31 kişiden oluşturuldu. Olgular başlangıçta, tedavinin 1., 6. aylarında ve tedavi bitiminden 1 ay sonra Schirmer I testi, gözyaşı kırılma zamanı, fundus muayenesi, korneanın flöresein ile boyanması, renkli görmeleri ve görme keskinlikleri yönünden prospektif olarak değerlendirildi. Sübjektif şikayetlerinin varlığı ile karanlık adaptasyon bozukluğu ve gece görmelerinde bozulma olup olmadığı sorgulandı. Schirmer I testinin 10 milimetre' nin, gözyaşı kırılma zamanının 10 saniye'nin altındaki değerleri anormal kabul edildi. İstatistik için Wilcoxon Signed Ranks, Ki-kare ve McNemar testleri kullanıldı. p0.05). Başlangıçta Schirmer I testi ortalaması 18.28±5.98 mm, gözyaşı kırılma zamanı ortalaması 16.00±4.43 sn, anormal Schirmer I testi sonucu %4.30, anormal gözyaşı kırılma zamanı sonucu %6.50, sübjektif yakınma oranı %15.20 iken; tedavinin 1. ayında Schirmer I testi ortalaması 14.28±5.49 mm (p0.05) olarak bulundu. Tedavinin 1. ayında anormal Schirmer I testi sonucu %19.56 (p0.05), tedavinin 1. ayında anormal gözyaşı kırılma zamanı sonucu %32.60 (p0.05), tedavinin 1. ayında sübjektif yakınma oranı %37.00 (p0.05) olarak bulunmuştur. Sonuç: Sistemik izotretinoin tedavisi süresince gözyaşı fonksiyon testlerinde anlamlı değişiklikler ve göz kuruluğu semptom ve bulgularında artış ortaya çıktı. Bu bulguların tümü tedavinin kesilmesinden bir ay sonra kayboldu. Bu olgularda yapay gözyaşı preparation kullanılması uygundur. Material and metod: Forty six patient with acne vulgaris treated by systemic isotretinoin were included in our study. The control group is composed of 31 subjects who between 15-35 years old without any systemic and ocular diseases affecting ocular surface and tear film functions. The patients were examined prospectively for Schirmer's I test, tear break-up time, fundus examination, corneal flouresceine staining, color vision and visual acuity at the beginning, 1st and 6th months of treatment and one month after cessation of the treatment. The patients were asked about any subjective complaint, dark adaptation and dark vision alterations. If Schirmer's I test is 0.05). At the 1st month of treatment, abnormal Schirmer's I test rate is %19.56 (p0.05). At the 1st month abnormal break-up time rate was %32.60 (p0.05). At the 1st month of the treatment, subjective complaint rate was %37.00 (p0.05). Conclusion: During systemic isotretinoin treatment, although tear function tests were significantly impaired, dry eye sign and symptoms were disappeared at the first month after cessation of treatment. Göz yaşı fonksiyonları break-up time (BUT) ve bazal Schirmer testi (BST) ile değerlendirildi. Bulgular: PS ve kontrol grubunun ortalama BUT değerleri sırasıyla 6.18±3.87 sn ve, 12.02±2.98 sn, ortalama BST değerleri 12.91±5.86 mm ve 16.47±6.36 mm olarak tespit edildi ve aradaki fark istatistiksel olarak anlamlı bulundu (p, Purpose: To evaluate tear film function of the patients who have pseudoexfoliation (PEX) syndrome. Material and Method: 55 eyes of 33 patients with PEX syndrome, and 62 eyes of 31 patients as controls were taken for this study. Their tear film functions were evaluated by break-up time (BUT) and baseline Schirmer tests (BST). Results: The mean BUT of the patient group was 6.18±3.87 sec and the control group's was 12.02±2.98 sec. The mean BST score of PEX group was 12.91±5.86 mm and the control group's was 16.47±6.36 mm. There were statistically significant differences between groups for BUT and BST. Conclusion: BST and BUT values of the patients with PEX syndrome were lower. PEX syndrome was found to be associated with impaired tear film functions.

94. Evaluation of the Ocular Surface Parameters in Pseudoexfoliation Syndrome and Conjunctivochalasis

Pur po se: To evaluate the ocular surface parameters in pseudoexfoliative (PEX) patients with conjunctivochalasis (CCh). Ma te ri al and Met hod: Fifty-three eyes of 53 patients with PEX syndrome and 107 eyes of 107 subjects without PEX were enrolled in this study. All subjects were categorized into two groups according to the presence or absence of CCh. The ocular surface characteristics were evaluated with standard tear break-up time testing (BUT), lissamine green (LG) staining of the conjunctiva and cornea, and Schirmer I test with topical anesthesia. Patients were also assessed with the Ocular Surface Disease Index (OSDI) questionnaire with respect to the severity of their subjective symptoms. The Mann-Whitney U-test, student’s t-test, and logistic regression analysis were used for statistical evaluations. Re sults: Among the 160 subjects, 30 (56.6%) in the PEX group and 38 (35.5%) in the control group had CCh (p=0.018). Logistic regression adjusted to gender and age showed that eyes with PEX were 2.3 times (odds ratio, 95% CI: 1.1 to 4.6) more likely to develop CCh than eyes without PEX (p=0.015). There were significant differences in the BUT scores, Schirmer values, LG staining scores and the OSDI scores of patients who had PEX and those without PEX. Dis cus si on: The frequency of CCh is significantly higher in patients with PEX syndrome. Both PEX and CCh affect the ocular surface test results. Pur po se: To evaluate the effect of autologous serum application on epithelization in the treatment of ocular surface disorders in hard cases and its safety. Ma te ri al and Met hod: Patients with serious ocular surface disorders, unresponsive to conventional treatment were recruited. Clinical features of retrospective cohort of patients who were prescribed serum drops are presented. From July 2007 to January 2010, 31 eyes of 21 patients, who were given autologous serum eye drops, were included into the study. Clinical examination included epithelial changes, rose bengal/lissamine green staining, fluorescein staining, and tear film break-up time. A history of systemic disease was recorded together with systemic medications used. A complete ocular history was also obtained. Re sults: Autologous serum was used in 7 patients with delayed epithelization after penetrating keratoplasty, in 4 patients with epithelial disturbances secondary to keratitis, in 2 patients with alkali burns, in 3 patients with Stevens-Johnson syndrome, in 1 patient with ligneous conjunctivitis, in 1 patient with epidermolysis bullosa, in 1 patient with corneal burn with hot water, and in 2 patients with Sjogren syndrome. The female:male ratio was 13:8. The mean age was 36.23±24.80 standard deviation (range: 7 months-87 years). No significant sight-threatening complication has been observed with the use of serum drops. Dis cus si on: Autologous serum application is safe and efficient additional therapy in the treatment of serious ocular surface problems in difficult cases. (Turk J Ophthalmol 2012; 42: 336-41)

6= ÖZET Rozasea, sıklıkla yüze yerleşen, flushing atakları, eritemli zeminde telenjiyektaziler, papüller ve papülopüstüllerle karakterize bir deri hastalığıdır. Rozaseada saptanan oküler yakınma ve bulguların insidansı değişik serilerde farklı oranlarda verilmiştir. Oküler rozasea yaygın bir hastalık olmasına karşın belirti ve bulguların özgün olmayışı nedeniyle gözden kaçabilmektedir. Hafif şiddette blefaritten, keratit ve korneal perforasyona kadar ilerleyebilen klinik bulgularla karakterize olup, sıklıkla kuru göz tablosuda eşlik etmektedir. Bu çalışmada 41 rozasealı olguda oküler yakınma ve bulgular değerlendirildi. Gözyaşı kırılma zamanı ölçüldü, Schirmer testi ve gözyaşı kristal izasyon analizi yapıldı ve epile edilen kirpiklerde Demodex folliculorum arandı. Sonuçlar 30 kontrol olgusuyla karşılaştırıldı. Olguların tümünde rozasea ile ilgili olabilecek en az bir oküler yakınma veya bulgu olduğu gözlendi. Ortalama GKZ hastalarda 8.40 saniye kontrollerde ise 11.76 saniye olarak saptandı. Fark istatistiksel olarak anlamlı bulundu. Schirmer testi rozasealı olgularda ortalama 10.02 mm, kontrollerde ise ortalama 12,41 mm olarak bulundu ve aradaki fark istatistiksel olarak anlamlı idi. Gözyaşı kristal izasyon testinde olgularda %39.2 oranında en çok Tip 2, kontrollerde %43.3 oranında en fazla Tip 1 ile uyumlu sonuçlar elde edildi. Fakat sonuçlar arasında istatistiksel olarak anlamlı bir fark yoktu. Demodex folliculorum, olgularda %51.2 kontrollerde ise %23.3 oranında pozitif bulundu. Aradaki fark istatistiksel olarak anlamlı bulundu. Çalışmada elde edilen bulgular rozasealı hastalarda göz tutulumuna yüksek oranda rastlandığını göstermektedir. Bu hastalarda yüksek oranda Demodex folliculorum saptanması nedeniyle, bu parazitin oküler tutulumun patogenezinde rolü olabileceğini düşündük. 37 7. SUMMARY OCULAR FINDINGS OF ROSACEA AND IT'S RELATIONSHIP WITH DEMODEX FOLLICULORUM Rosacea is a dermatose occuring in the central region of the face characterized by episodic flushing, erythema, telengiectasia, papules and papulopustules. The incidence of ocular signs and symptoms in rosacea were detected to be variable in different series. Although ocular rosacea is a very common disease, it is dismissed because of nonspesific signs and symptoms. İt is characterized with a spectrum from mild blepharitis, keratitis to corneal perforation and frequently symptoms of dry eye may accompany. In this study, ocular signs and symptoms in 41 cases with rosacea were evaluated. Tear break-up time were measured, Schirmer test and tear crystal ization analysis were performed, and the epilated eyelids were examined for the detection of Demodex folliculorum. The results were compared with those of 30 healthy controls. All cases showed signs and symptoms of ocular disease. The mean tear break-up time of the cases and controls were 8.40 and 11.76 seconds, respectively. This difference between those values was found to be statistically significant. While the Schirmer test showed an average of 10.02 mm of wetting in cases with rosacea, the controls had an average of 12.41 mm of wetting and the difference was also statistically significant. The results of crystalization test, revealed that 39.2% of the cases had type 2, and 43.3% of the control group had type 1 tear crystallization pattern, the difference was statistically nonsignificant. Demodex folliculorum was detected in 51.2% of the cases in rosacea group and in 23.3% of the control group. This difference was found to be statistically significant. Results of the present study showed that ocular involvement in rosacea is high. Because Demodex folliculorum is isolated in a high percentage of these cases, we thought that this parasite might play a role in the pathogenesis of ocular involvement. 546

An AHC outbreak was seen in Istanbul during August and September 2010 and lasted for four weeks. All the patients were seen at the first days of their disease period and none of them had received any treatment before. Conjunctival swab specimens were taken from the patients at their first visit. Tear film tests including Schirmer test, tear meniscus height measurement and tear break-up time (TBUT) were done in all patients. Re sults: The mean age of the patients was 27.8 years (range: 7-68 years). Forty patients were male (53%) and 36 patients were female (47%). In bilateral conjunctivitis cases, the mean Schirmer test result was 23.7±4.7 mm, mean TBUT was 15.1±2.4 seconds and the mean tear meniscus height was 0.37±0.06 mm. In unilateral conjunctivitis cases, the mean Schirmer test result was 24.4±3.6 mm, mean TBUT was 15.1±2.3 seconds and the mean tear meniscus height was 0.38±0.07 mm in the diseased eyes. Purpose: To evaluate the dry eye in patients with Corona virus 19 disease (COVID-19).Materials and methods: In this prospective study 54 eyes of 27 patients who had COVID-19 within last 6months and 60 eyes of 30 age and sex matched healthy controls were included. After the eye examination the tear film break-up time (BUT) and Schirmer 1 test (without anesthesia) was performed in all subjects.Results: The mean age was 30.1±7.7 in study group and 29.6±6.2 in controls. The mean time after recoveryof COVID-19 was 11,6±7,2 weeks (min 2-max 24). The mean BUT value was 11.46±3.96 seconds in the studygrup and 11.70±3.52 seconds in controls and the difference between two groups was not statically significant (p>0.05). The mean Schirmer 1 test value was 18.75±7.82 mm in the study group and 17.31±6.23 mm in controls and the difference between two groups was not statically significant (p>0.05). There was no ocular findings other than epiphora and hyperaemia (11%) and periocular pain (7%) in COVID-19 group.Conclusions: There was no difference in dry eye tests between COVID-19 patients and healthy subjects.COVID-19 may not cause dry eye in patients., Amaç: Koronavirüs 19 (COVID-19) geçiren hastalarda kuru göz varlığının araştırılması.Gereç ve yöntem: Çalışma prospektif olarak planlandı. Süleyman Demirel Üniversitesi Tıp Fakültesi hastanesine rutin göz muayenesi için başvuran ve son 6 ay içerisinde COVID-19 geçirmiş 27 hastanın 54 gözü ile yaş ve cinsiyet bakımından uyumlu 30 sağlıklı bireyin 60 gözü çalışmaya dâhil edildi. Tüm olgulara gözyaşı kırılma zamanı (BUT) ve Schirmer 1 (anestezisiz) testi uyguladı.Bulgular: Ortalama yaş hasta grubunda 30,1±7,7, kontrol grubunda 29,6±6,2 idi. COVID-19 geçirilmesiüzerinden geçen zaman ortalama 11,6±7,2 hafta (min 2-maks 24) idi. BUT değeri hasta grubunda ortalama11,46±3,96, kontrol grubunda ise 11,70±3,52 saniye idi ve iki grup arasında istatistiksel olarak anlamlı fark yoktu (p>0,05). Schirmer testi hasta grubunda ortalama 18,75±7,82, kontrol grubunda ise 17,31±6,23 mm idi ve iki grup arasında istatistiksel olarak anlamlı fark yoktu (p>0,05). Covid grubunda rastlanan oküler bulgular gözde sulanma ve kızarıklık (%11) ve peribulber ağrı (%7) idi.Sonuç: COVID-19 geçirmiş bireylerin gözyaşı testlerinin normal bireylerden farklı olmadığı görüldü. COVID-19’un kuru göze neden olmadığı sonucuna varıldı.

7. ÖZET Rozase sıklikla yüze yerleşen, flushing atakları, eritemli zeminde telenjiektaziler, papüller ve papülopüstüllerle karakterize bir deri hastalığıdır. Hastalığa oküler yakınma ve bulgular da eşlik edebilmektedir. Bu çalışmada 50 rozaseli hastada demografik ve klinik özellikler ve hastalığın şiddetini etkileyen faktörler araştırıldı. Ek olarak bu hastaların 32'sinde oküler yakınma ve bulgular değerlendirildi, gözyaşı kırılma zamanı ölçüldü, Schirmer testi uygulandı ve rose bengal boyaması yapıldı. Sonuçlar 15 kontrol olgusuyla karşılaştınldj. Rozasenin kadınlarda daha sık görülen ve her iki cinste de benzer şiddette seyreden bir dermatoz olduğu gözlendi. Hastalığın genellikle simetrik yerleştiği ve en sık tutulum yerinin yanaklar olduğu saptandı. Hastaların %50'den fazlası güneş, stres ve yüze sabun kullanımıyla yakınmalarında artma tanımladılar. Hastaların tümünde rozase ile ilgili olabilecek en az bir oküler yakınma veya bulgu olduğu gözlendi. En sık. görülen oküler yakınmalar kaşıntı, yanma, fatofobi, en sık oküler bulgular ise blefarit, meibomit ve telenjiektazi olarak saptandı. Hastalığın evresiyle oküler bulgu sayıları arasında pozitif korelasyon olduğu gözlendi. Ancak hastalığın evresi, papül ve püstül sayıları ile gözyaşı fonksiyon testleri arasında korelasyon saptanmadı. Flushing olup olmamasıyla oküler yakınma ve bulgu sayılan ve göz yaşı fonksiyon testleri arasında da ilişki saptanmadı. Ortalama gözyaşı kırılma zamanı hastalarda 10,00 saniye, kontrollerde ise 13,27 saniye olarak saptandı. Fark istatistiksel olarak anlamlı bulundu. Hastalarda ortalama 11,56 mm, kontrollerde ise 14,27 mm olarak saptanan Schirmer testi sonuçları hasta grubunda kontrol grubundan anlamlı derecede daha düşük bulundu. Rose bengal boyanma skorları hasta grubunda kontrollerden anlamlı derecede daha yüksekti. Sonuç olarak, güneş, stres, sabun kullanımı gibi hastalar tarafından hastalık şiddetini arttırdıkları bildirilen unsurların olabildiğince ortadan kaldırılması, hastalığın kontrol altına alınması için gerekli önlemlerden biri olmalıdır. Rozasede tüm hastalarda değişen şiddetlerde olmakla birlikte oküler tutulumunun hastalığa eşlik etmesi nedeniyle, ciddi oküler komplikasyonların önlenmesi için hastaların oftalmolojik açıdan da izlemi önemlidir. 56 8. ROSACEA: FACTORS AFFECTING THE COURSE AND OCULAR INVOLVEMENT Rosacea is a dermatose occuring in the central region of the face characterized by episodic flushing, erythema, telengiectasia, papules and papulopustules. The incidence of ocular signs and symptoms in rosacea were detected to be variable in different series. In this study demographic, clinical features and provoking factors in 50 patients with rosacea were investigated. In addition the ocular signs and symptoms were evaluated in 32 of them. The rosacea patients underwent testing of tear break-up time, Schirmer testing and rose Bengal staining and the results were compared with that of 15 controls. Rosacea was detected to be more common in women, and the severity of the disease were not significantly different regarding gender. The lesions were found to be symmetrically distrubuted, the cheek being the most affected region. More than 50% of the patients reported an increase in symptoms with sun exposure, stress and soaps. All patients showed signs and symptoms of ocular disease. The most frequent ocular symptoms were itching, burning and photosensitivity, and the most frequent ocular signs were blepharitis, meibomitis, and telengiectasia. The stage of cutaneous rosacea showed a positive correlation with the number of ocular signs. The difference between flushing and the number of ocular signs and symptoms were not statistically significant. The mean tear break-up time of the patients and the controls were 10,00 and 13,27 seconds respectively. This difference was found to be statistically significant The Schirmer test showed an average of 11,56mm of wetting with rosacea, while the controls had an average of 14,27mm of wetting and the difference was statistically significant Rose Bengal staining scores were higher in rosacea patients than controls. In conclusion, all sources of local irritation such as sun exposure, stress and soaps should be avoided in rosacea. Thus, protection against sunlight and avoidance of stress requires special emphasis. The ocular involvement should be always kept in mind for early diagnosis with basic tests and appropriate treatment will prevent the development of severe ocular complications. 57 67

ÖZET Bu çalışmada uzun süreli topikal antiglokomatöz ilaç kullanan hastalarda gözyaşı fonksiyon testlerinin özellikleri ve impresyon sitolojisi tekniği ile konjonktiva sitolojisi incelenmiştir. En az 1 yıl timolol ve/veya pilokarpin kullanan toplam 32 hastanın 54 gözü gruplara ayrılarak Schirmer, gözyaşı kırılma zamanı (GKZ) testi ve konjonktiva sitolojik baskısı uygulandı. Sitolojik örneklerde goblet hücre sayımı yapıldı ve epitel hücre morfolojisi derecelendirildi. Elde edilen veriler kontrol grubu ile karşılaştırıldığında uzun süre pilokarpin kullanımının gözyaşı fonksiyon testleri üzerine, goblet hücre yoğunluğuna ve epitel morfolojisine herhangi bir etkisi saptanmazken, timolol ve kombine ilaç kullanımının gözyaşı fonksiyon testlerinde ve goblet hücre yoğunluğunda önemli derecede azalmaya ve skuamöz metaplaziye neden olduğu görüldü. SUMMARY In this study the effects of long-term topically antiglaucoma medications on the lacrymal function tests and cytology of conjunctiva were evaluated. The technique of conjunctival impression cytology was used to evaluate the conjunctival changes. Total 54 eyes of the 32 patients were divided to three groups and Schirmer's test and tear film break up time (BUT) were performed.
