The establishment of the newborn's intestinal microbiota is a crucial process that occurs during birth and breastfeeding, playing a fundamental role in the development of the immune system and the health of the baby throughout its life. Newborn foals are very susceptible to potentially fatal diseases. Therefore, it is of great importance to identify variables that allow to predict the survival of these patients, and promptly start the appropriate treatment for each case. A retrospective study was carried out at the Equine Foal Care Perinatology Center, located in the municipality of Cajicá, Cundinamarca. The medical records of 106 foals admitted to hospitalization between September of 2019 and December of 2020 were reviewed, the necessary data was collect, and analyzed, in order to evaluate the predictive value of the clinical examination against the survival of hospitalized neonatal foals, and to identify which of these variables function as early markers of survival. Different statistical tests were used, such as the Kolmogorov Smirnov test, the Mann-Whitney test, the binary logistic regression, the chi-square tests, the ROC curves and the odds ratio (OR). The variables most significantly related to survival were heart rate (p = 0.019), rectal temperature (p = 0.002), lukewarm ears and limbs (p = 0.013) and concordant full strong pulse (p = 0.002). Risk factors associated with foal nonsurvival were temperature < 38 °C, heart rate < 100 bpm, cold ears and extremities, and weak pulse. [ABSTRACT FROM AUTHOR]

Newborn foals are susceptible to life-threatening diseases. Therefore, it is important to identify markers that make it possible to predict the survival of these patients, and to initiate the appropriate treatment for each case. This study was conducte at the Foal Care Equine Perinatology Center located in Cajicá, Cundinamarca. The medical records of 106 foals admitted to hospitalization between September 2019 and December 2020 were reviewed and analyzed to assess the predictive value of blood gas and electrolyte parameters against the survival of hospitalized neonatal foals. Different statistical tests were used, such as the Kolmogorov Smirnov test, Mann-Whitney U test, binary logistic regression, chi-square tests, odds ratio, and receiver operator characteristic (ROC) curve analysis. It was observed that the most common acid-base disorder upon admission to hospitalization was metabolic acidosis (37/106), and the most frequent electrolyte imbalance was hypocalcemia (89/106). The variable most significantly related to survival was pH > 7.35 (p = 0.007; OR = 0.288; CI = 95%, 0.117-0.706). The risk factors associated with non-survival of the foals were pH < 7.35, lactate > 4.5, and glucose < 119.5 mg/dl. [ABSTRACT FROM AUTHOR]

BACKGRAUND: Enterovirus infections in neonates can present as very severe manifestations with high mortality, such as hepatitis with coagulopathy, meningoencephalitis, or myocarditis. CASE PRESENTATION: We present the case of a 4-day-old neonate who presented a severe enterovirus infection. The mother had presented peripartum fever. He initially developed hepatitis with coagulopathy and thrombocytopenia that required several transfusions of blood products. After resolution, myocarditis developed that required inotropic support. The evolution was favourable. CONCLUSION: This clinical case suggests that those neonates with severe early enterovirus infection may develop subsequent myocarditis and should therefore be monitored after resolution of the infection. Introduction: Congenital lung tumors are rare pathologies, likewise when surgical intervention is required, the anesthesiologist faces a challenge in the anesthetic management of these patients. Presentation of the case: The following article describes the case of a 26-day-old newborn child scheduled for left posterolateral thoracotomy plus lower left lobectomy and central venous catheter placement under combined anesthesia (balanced general anesthesia plus caudal block with bupivacaine and morphine). Conclusions: Anesthetic considerations for surgical procedures in neonates and/or pediatric patients become a challenge for the anesthesiologist, due to the immaturity of some of the systems, thus considering that the ideal anesthetic technique for the proper management of in neonatal patients, the combined technique continues, with epidural blocks for the management of peri- and postoperative pain. The anesthesiologist must keep in mind this technique as well as the events and/or side effects that may derive from it, thus establishing the pertinent treatment in a timely manner. Migration to our country has led epidemiologic changes in Hospital de Linares, which motivated this study. The objective is to know the epidemiologic changes in the Maternity Ward en Neonate Unit of a High Complexity. We analyzed the admission of the Haitian women and their offspring throughout 2016 to 2018. We used descriptive stratigraphy and were compared using chi-squared test. Out of 6911 women that gave birth, we found 81 Haitian women. Out of 1499 neonates that were hospitalized, 31 were of Haitian background. We saw an important increase in the number of Haitian women giving birth even though the general birth rate was decreasing. Subsequently there was an increase in the hospitalization of neonates born to Haitian mothers, even though the general neonate hospitalization rate was decreasing and in a higher percentage when compared to the neonates born to women from other nations. The main diagnosis were severe SGA (25 %), Hypoglycemia and Hyperbilirubin. We observed a higher number of total hospitalized period in newborns of Haitian background compared to other nationalities. It's interesting the increase in Haitian background neonates, the higher percentage of severe SGA and the longer stay due to the language and cultural barrier that influences on the mothers following the medical indications. The neonatal stage comprises from the first day of birth until day 28 of life. It is characterized by being a stage of anatomical, physiological and metabolic immaturity. Due to these characteristics of neonates, anesthetic drugs have an important impact on their respiratory and cardiovascular physiology, and their immaturity on pharmacokinetics. Laryngomalacia is one of the main causes of stridor in this stage, which depending on its severity could require surgical treatment, such as an epiglotopexy. This procedure requires potent analgesia and at the same time the possibility of maintaining spontaneous ventilation for its appropriate evaluation. This can be achieved with the use of remifentanyl. We present the clinical case of a term neonate of 22 days of age who we administered Remifentanyl as the basis of sedoanalgesia for the successful completion of an epiglotopexy. Its analgesic potency, ability to maintain ventilatory automatism and predictable pharmacokinetics in the neonate, makes it an ideal agent to provide adecuate sedoanalgesia for different procedures with an easy titration profile and rapid recovery in the neonate. Transfusion of erythrocyte concentrate (EC) in critically ill neonates may be justified in the intention of increasing the arterial oxygen content (CaO2), it is possible that the serious condition modifies its effectiveness. Objective: To compare the change in post-transfusion CaO2 in neonates without sepsis, with sepsis and septic shock. Material and methods: 110 neonates in their first transfusion: 14 with septic shock, 38 with sepsis and 58 without sepsis. CaO2 was compared before and after the transfusion. With data obtained by arterial blood gas, CaO2 was established with the formula: (Hb * 1.34 * SatO2) + (pO2 * 0.003)]. Results: Post-transfusion hemoglobin levels increased, but differed between groups, being lower in septic shock: without sepsis 13.9 ± 1.2 g / dL, sepsis 14.1 ± 1.6 and septic shock 12.5 ± 2.4. The same was observed with CaO2 mL/dL: 18.2 ± 2.02, 17.3 ± 2.8 and 15.3 ± 3.6, respectively. Conclusion: Apparently, hemodynamic instability is related to a minor increase in CaO2 after EC transfusion. Further studies are required to verify these findings. [ABSTRACT FROM AUTHOR]

Introducción: la ruptura intraperitoneal del uraco resulta en el acumulo de orina en la cavidad peritoneal‚ lo que a su vez genera desordenes hidroelectrolíticos y ácido base que requieren ser tratados previo a la corrección quirúrgica de la ruptura. Anamnesis: se describe el caso de un potro macho criollo colombiano de 15 días de edad previamente evaluado por el clínico remitente desde el oriente antioqueño debido a la presencia de distensión abdominal‚ depresión‚ postración y goteo de orina por las estructuras umbilicales. Adicional a lo anterior‚ se reportan valores alterados de urea (182 mg/dl)‚ BUN (85,54 mg/dl)‚ creatinina sérica (4,42 mg/dl) y creatinina peritoneal (8,67 mg/dl). Hallazgos clínicos: durante la evaluación en el hospital se observaron signos clínicos como depresión‚ taquicardia‚ deshidratación calculada del 8 %‚ distensión abdominal marcada y distensión marcada de tres articulaciones (tarsos y carpo del miembro anterior derecho). Fueron contemplados como diagnósticos diferenciales septicemia neonatal‚ poliartritis séptica y uroperitoneo. Ayudas diagnosticas: ultrasonografía abdominal donde se confirmó la presencia de abundante líquido. El hemograma reveló disminución del hematocrito (19 %)‚ leucocitosis leve con neutrofilia absoluta y relativa. Se realizó medición de analitos con el equipo EPOC presentando las siguientes alteraciones en la gasometría: acidemia (pH 7,23)‚ déficit de base (-7,2 mmol/L)‚ disminución del bicarbonato (20,3 mmol/L). los resultados de la bioquímica fueron hiponatremia (115 mmol/L)‚ hipercalemia (5,5 mmol/L)‚ hipercalcemia (1,44 mmol/L)‚ hipocloremia (83 mmol/L)‚ déficit de base (-6,7 mmol/L)‚ los metabolitos reflejan hiperglicemia (224 mg/ dl)‚ hiperlactatemia (6,33 mmol/L) y azotemia (creatinina 3,91 mg/dl). Introduction: Introduction: breast milk is the first fundamental food of newborns and it provides all the sources of energy, nutrients and the immunological protection they need during their first months of life. Unfortunately, there are specific circumstances that imply that the mother cannot feed her baby correctly, since the mother's nutritional needs differ to a certain extent during different periods of life. Especially in breastfeeding, since nutritional needs are increased, due to the loss of nutrients, first by colostrum and then by breast milk. Objective: to demonstrate the influence of the diet of Mexican women on the nutritional quality and the presence of beneficial microorganisms in human milk. Methods: seventy descriptive surveys of nutrition and sampling of milk to women in nursing state were carried out. The milks were subjected to various bromatological and microbiological analyzes to evaluate their nutritional quality and possible probiotic activity. Results: it was shown that the mother's food intake influences the nutritional quality of the milk. Likewise, it affects the development and growth of lactic acid bacteria. Several strains were isolated and identified in human milk of the genus Lactobacillus, as well as pathogenic bacteria such as Lodderomyces elongisporus among others. Conclusions: the mothers' nutrition is directly reflected in the nutritional quality of the milk. It was observed that the amount of essential nutrients of milk such as carbohydrates, lipids and proteins vary according to the diet and life rhythm of the mothers, as well as the significant decrease of lactic bacteria with probiotic potential. The most common cause of lower urinary tract obstruction in males is posterior ureteral flaps; its incidence is between 1-in 5,000 to 8,000 male births. This malformation may be detected from the second trimester of pregnancy and the prognosis depends on the severity of the obstruction and the renal failure. Case report: This was a male neonate, product of the sixth gestation with adequate prenatale care, at the sixteenth gestational age megacystis was detected in a prenatal ultrasound. On the 33rd gestational week, posterior ureteral flaps were diagnosed. He was born prematurely at 34 weeks of gestation with respiratory distress and was admitted to the NICU. Since he was admitted to NICU, hyperkalemia and elevated creatinine levels were determined, which gradually increased. Antihyperkalemic treatment was started without needed dyalsis. Cystostomy was done and he was discharged at 40 days of postnatal life with oral sodium bicarbonate, furosemide and weekly erythropoietin. Conclusions: Patients with posterior flaps have a fetal mortality of 30 to 50%. The available literature either national or foreign about the neonate morbidity and mortality was reviewed and it was decided to make reference to important aspects related with the topic, among which there were figured, besides a historical review: classification and register of the death causes (distress syndrome, hyaline membrane disease, infections, bronchopulmonary dysplasia and congenital malformations), delivery type (complications and prematurity), as well as low birth weight. Of the obtained information it was inferred that the neonates birth with ponderal inadequacy for the gestational age, still constitutes a severe health situation in today's world, mainly in the third world countries, where enough resources are nor disposable to offer an appropriate health care to the population. Intestinal arteriovenous malformations (AVM) are high flow vascular anomalies arising from an embryogenesis error, due to direct communication ofvein and an artery, without capillary bed in between them. There are few reports of intestinal AVM in neonates. Most of them are isolated and genetic defects have not been found. Definitive diagnosis is established through histopathologic studies. We present a newborn, offspring of a primigest mother, with adequate prenatal control, with pregnancy ultrasounds showing no evidence of malformations. Cesarean delivery was performed due to HELLP syndrome at 32 weeks ofgestation, APGAR at birth was 8/9, and Silverman Andersen score 0/2/3, weight at birth was 1300 grams and length of 37 cm. The newborn was admitted to NICU due to respiratory distress syndrome and prematurity (gestational age of 32 weeks), no apparent malformations were reported. Treatment and hospital management was done following clinical practice guidelines, according to his diagnoses at admission. During his tenth day of life, he developed sudden abdominal distension, along with clinical and radiographic data suggestive of intestinal perforation. Immediately treatment was started with intravenous solutions, vasopressors, metabolic acidosis correction and penrose drain abdominal decompression. Sudden cardiac arrest occurred and in spite of cardiovascularsupport, the patient failed to respond. The autopsy reported an AVMwith a length of 4cm, located in mesocolon and outer external colon wall of the transverse colon with perforation. Acute generalized peritonitis was found without data suggesting necrotizing enterocolitis. The presence of intestinal arteriovenous malformations in neonates is an infrequent diagnosis and cause of death. It is fundamental to continue the practice of autopsies in hospitals. In the case of neonates with multiple pathologies for which required hospital stay and in prolonged fasting for indefinite periods, is essential for the mother support from nurse with experience on breastfeeding, because the first days are still critical for establishing a nice fall of human milk and the maintenance of satisfactory levels of production without receiving the stimulus the suction the baby, depend mainly of the use of a good extractor of human milk; taking in to account its ease of use and effectiveness inside the hospital and in turn to achieve to strengthen the bond between mother and child. This paper reports on the implementation of techniques of human milk with extractor connected to the vaccum, is done at Children's Hospital of Ciudad Juarez, Chihuahua, which poses no any economic costs for the institution; besides there is a written authorization of the 24 breastfeeding mothers who participated in the use this extraction techniques of human milk, photos and videos recordings. Objective: To validate the Preterm Oral Feeding Readiness Assessment Scale, Preterm Oral Feeding Readiness Assessment Scale (POFRAS), by expert judgment. Material y methods: Sixteen speech-language experts participated. Results: The content validity ratio (CVR) was used to determine the validation of the POFRAS; 13 of the 18 items had a CVR higher than 0.5, indicating that the degree of agreement between the judges is valid. The items of global posture and maintenance of the alert state obtained a total agreement of the 16 judges; on the contrary, the rest of the items did not reach the minimum. Introduction: Congenital heart disease is the most common major malformation; its incidence is close to 1%. Cardiac auscultation may be limited in the neonate and oxygen saturation helps detect complex congenital heart disease. Objective: To determine the sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of murmur and cyanosis for the detection of neonatal congenital heart disease. Material and methods: In a maternalinfant hospital, we studied patients who had persistent low saturations or murmurs within the first 30 days of life; a control group was also included. Transthoracic echocardiography was performed in both groups. Those neonates with patent foramen ovale as the only finding and those with a physiological ductus arteriosus were excluded. Results: We evaluated 92 patients with cyanosis and/or murmur, and 126 controls, detecting 56 with a cardiac defect. Murmur for noncomplex heart disease detection: sensitivity 88%, specificity 91%, PPV 72%, NPV 97%. Cyanosis for detection of complex congenital heart diseases: Sensitivity 95%, specificity 72%, PPV 45%, NPV 99.2%. Cyanosis with murmur: sensitivity 63%, specificity 95%, PPV 92%, NPV 75%. Conclusions: Murmur is the cardinal sign for detection of non-critical heart disease. Cyanosis was the most important sign for detection of complex congenital heart disease, increasing its PPV from 45 to 92% when associated with murmur. [ABSTRACT FROM AUTHOR]

Objective: This research aimed to observe relationships and find groups of neonates show a similar behavior with respect to the variables of physical growth, morbidity and intervention and to model the evolution of the weight of neonates with low birth weight over time. Materials and Methods: A sample of 159 medical records of premature neonates born at the Valle University Hospital (HUV) from February 2002 to November 2010 and who have been admitted to the HUV high-risk and Kangaroo Program was analyzed. A correspondence and a cluster analysis were carried out, and secondly, a random coefficient model was fitted. Results: Two well-defined groups of low-weight neonates were found: those with extremely low weight and those with low weight and very low birth weight. The curves estimated with the model fitted show that neonates who received prenatal control have a relatively higher weight, with slight variations when compared with curves depicting average weight. Conclusion: Having a model to estimate the evolution curve of the weight of each neonate with low birth weight in the HUV, it is of great advantage for pediatricians because it allows us to observe that it is so variable it against the curve of the mean weight expected. Neonatal liver abscess associated with umbilical vein catheter (UVC) is rare and difficult clinical diagnosis so that imaging studies are essential. We report the case of a premature newborn of 35 weeks gestation with respiratory distress, installing a umbilical vein catheter . Subsequently develops tachypnea, abdominal distension, pain and progressive increase in PCR. Abdominal ultrasonography and tomography revealed solitary hepatic abscess, treated with meropenem and vancomycin for 21 days with favorable evolution. [ABSTRACT FROM AUTHOR]

A descriptive serial cases study of 57 neonates who died in "Dr. Antonio Penados del Barrio", San Benito Petén Hospital, in Guatemala was carried out from January to June, 2011, with the objective of characterizing neonatal deaths occurring according to clinical and epidemiological variables. The information was obtained from the medical records of neonates and from data of the prenatal care to the mother. In the series 85,9% of mothers were identified with risks, among which hypertension, asthma, diabetes mellitus, urinary sepsis and absence of prenatal control had the higher frequencies. Mortality prevailed in the first 24 hours after birth and it was linked to factors such as prematurity, hyaline membrane disease, meconium aspiration syndrome and neonatal sepsis. Background Ventilation associated pneumonia is among principal causes of nosocomial infections. It impairs local defense mechanisms; increases cost and hospital stay. The aim of this research was identified risk factors to develop ventilation associated pneumonia. Patients and methods It included a cohort of 101 neonates undergoing ventilation in two years at a public hospital pediatric specialty of Mexico City. Demographic data were recorded, ventilation and evolution. Associated pneumonia was seen after 48 h of ventilation, with clinical and radiological criteria. Results 17% had ventilator associated pneumonia. It was more frequent with less gestational age, number reintubations, duration of ventilation, supplementary oxygen days and length of hospital stay (All with p < 0.01). Risk was increased in male (OR 7.1, IC 95% 1.5 - 33) and very low birth weight (OR 3.5; IC 95% 1 - 12). In newborns with pneumonia also increased the risk of bronchopulmonary dysplasia (OR 11.3; IC95% 3.5 - 36) and death (OR 5.4; IC 95% 1.6 - 18). Conclusions Infant newborns under mechanical ventilation and prematurity, very low birth weight, male with many reintubations and different tubes are in high risk of ventilator associated pneumonia, bronchopulmonary dysplasia and death. Objective: To determine the efficacy of the women with breastfeeding their children under 48 hours and build the demographic profile and obstetric mothers. Material and Methods: This descriptive, transversal study was conducted with 100 women in "Hospital de Apoyo Maria Auxiliadora" public hospital in Lima Perú, from November 9th to 14th, 2010. Demographic and obstetric data were collected from mother's medical history and then the BSES-SF form was applied. Results: 72 (72%) mothers showed high self-efficacy to breastfeed. We found a statistically significant association between age, marital status, parity, prenatal checkups and breastfeeding self-efficacy scores (p=<0.05). Conclusions: The levels reached in the BSES-SF for the study population were high. Being under 20, unmarried, primiparous and fewer prenatal care, increase the risk for breastfeeding is not successful. [ABSTRACT FROM AUTHOR]

Objective: To evaluate clinical features and develop a scale, that allows predict a serious bacterial infection in febrile infants without apparent focus. Patients and methods: Cross-sectional study with 269 infants. The data were analyzed statistically evaluating partnerships and using regression for the multivariate analyis. It set a level of significance with p<0.05. The models were constructed validated. Results: 34.20% had severe bacterial infection. The models built for urinary tract infection and other severe bacterial infection were good predictors. The performance of the new scale was higher than that young infant observation scale (YIOS). The urinary tract infection (51.2%), sepsis (14.1%), and sepsis associated with meningitis (11.95%) were the most frequent presentation. Escherichia coli and Streptococcus group B were the main agents associated. Conclusions: These new scales are useful for treating febrile infants, surpassing the young infant observation scale.
