Objective: Sexually Transmitted Infections (STI), as their name suggests, are infections caused by more than thirty different bacteria, viruses, and parasites and are transmitted through sexual contact, including vaginal, anal, or oral intercourse, although some sexually transmitted infections can also transmitted from mother to child during pregnancy, childbirth and lactation. According to World Health Organization, one of the main populations at risk for acquiring these diseases are sex workers and their clients. Due to the high prevalence, the characteristics and behaviors that favor the development of these diseases are analyzed. The main objective of this paper was to analyze the risk factors in the acquisition of sexually transmitted diseases in the population of sex workers., Methods: A literature search was conducted in Scopus, Pubmed, CINAHL, Medline, LILACS and IBECS, of observational, longitudinal and mixed methods studies conducted in sex workers and published between January 2011 and March 2021, with a final sample of fourty-three articles. The mixed methods assessment tool (MMAT) was used to assess the methodological quality of the papers., Results: After analyzing the literature, eleven groups of risk factors related to the acquisition of sexually transmitted diseases were obtained, these being economic factors, factors related to sexual practices, drug use, sex work, education, health problems, country of origin and mobility, partner, age, violence and other factors not belonging to the previous categories., Conclusions: Sex workers have numerous characteristics and behaviors that make them vulnerable to sexually transmitted infections. Of note are years of prostitution, drug use, number of partners and condom use, which are key factors for future prevention and intervention strategies, as well as research.

Objective: The COVID-19 pandemic indirectly affected other communicable diseases, such as human immunodeficiency virus (HIV) infection. The aim of this paper was to evaluate the impact of the COVID-19 pandemic on the epidemiological surveillance of HIV through epidemiological indicators., Methods: Data collected in the New HIV Diagnosis Information System (SINIVIH, acronym in Spanish) in the period 2013-2021 was analyzed. The epidemiological indicators analyzed were: 1) Reporting delay of new diagnoses; 2) Under diagnosis of cases, calculated per month of diagnosis and for each year of diagnosis; 3) Late diagnosis, cases diagnosed with less than 350 CD4 cell/mm 3 ., Results: Regarding the reporting delay of new diagnoses, in 2022 2,770 diagnoses were reported in 2020, 43.8% more than those reported in 2021 for that year. The cases diagnosed per month between 2016 and 2019 followed a constant trend. In 2020, a significant decrease in diagnoses was observed between March and May that was not recovered after correcting for reporting delay. When comparing the rates of cases diagnosed in 2013 with the successive years, a continuous decrease was interrupted in the last year of the study period. The percentage of cases with late diagnosis had remained stable in the study period (between 46% and 50%), and a decrease in this percentage was observed after correcting for reporting delay., Conclusions: The epidemiological surveillance of HIV suffered a significant reporting delay. A decrease in cases diagnosed is confirmed, partly due to lockdown and lack of access to the health system. Changes on late diagnosis is not observed.

Hegemonic masculinity has distanced heterosexually active men from the responses to the HIV epidemic. However, with the combined prevention paradigm, post-exposure prophylaxis offers new ways to manage the risks of HIV infection, while at the same time bringing about new challenges. The aim of this paper is to discuss - through the lens of gender and masculinities - how heterosexually active men perceive their HIV risk and how they manage prevention methods and strategies, including post-exposure prophylaxis. Employing qualitative methods, 16 heterosexually active men in five health services from different Brazilian cities were interviewed. The analysis indicates that condom use - the main prevention method - was conditioned by subjective and contextual factors, and its failure stands out as the primary reason for seeking out post-exposure prophylaxis. We argue that ideals of hegemonic masculinity permeate perceptions and risk management discourses in the current context of the HIV/AIDS epidemic.

In 2013, the Joint United Nations Programme on HIV/AIDS (UNAIDS) proposed, in terms of public health, to end the HIV epidemic by 2030 by achieving the so-called 90-90-90 targets by 2020. This goal will only be possible if coverage in the HIV care continuum indicators is increased. With the methodology proposed by PAHO/WHO, this paper estimates the indicators of the continuum of care in Peru for 2014, consisting of diagnostic coverage, antiretroviral treatment (ART), and viral suppression in people living with HIV (PLHIV). Of the 72,000 PLHIV estimated in Peru for 2014, 64% were diagnosed, 56% and 55% were linked and retained in the system, respectively; 46% were receiving ART, and 36% had viral suppression. The main gaps identified were: 19,917 PLHIV did not know their diagnosis, 25,078 had no access to treatment, and 25,633 did not achieve viral suppression at study closing. This is the first estimation exercise of the continuum of care indicators in Peru. These indicators will serve as a baseline for the monitoring progress toward the 90- 90-90 Targets. It is also the starting point for analysis and research on the determinants of each gap and new proposals by authorities and actors involved in the fight against HIV in Peru.

Objective To understand the impact that healthcare professionals have on the perception of health promotion and disease prevention campaigns of individuals with HIV in Medellin, Colombia in 2012., Materials and Methods: The research was conducted using a qualitative approach, based on the Strauss and Corbin´s Grounded Theory; thirteen people were interviewed during two sequential steps. The interviews were transcribed and analyzed in accordance with the theory mentioned above, by performing an open and axial coding on the data found. Results A total of 1 324 codes were generated in the open coding process (creating six descriptive categories) and axial coding process (identifying four analytical categories; one of them is developed as a result in this paper). The most relevant findings included the importance given by the participants to the role of health professionals when supporting HIV-positive patients in the acquisition of healthy lifestyles to properly manage their condition. Also, the role that those people who already contracted the virus have for the promotion of sexual behaviors to minimize the risk of infection is stated. Conclusion The doctor-patient relationship poses great challenges in terms of health services management for the different actors of the system. Likewise, these challenges must be taken into account when creating future public policies.

This consensus document has been prepared by a panel of experts appointed by GeSIDA. This paper reviews the recommendations on the most important non-AIDS defining malignancies that can affect patients living with AIDS. Lung cancer, hepatocellular carcinoma, anal carcinoma and other less frequent malignancies such as breast, prostate, vagina or colon cancers are reviewed. The aim of the recommendations is to make clinicians who attend to this patients aware of how to prevent, diagnose and treat this diseases. The recommendations for the use of antiretroviral therapy when the patient develops a malignancy are also presented. Incentives to provide universal access to antiretroviral therapy in order to control the HIV/AIDS epidemic also encouraged the diversification of HIV testing strategies, as demonstrated by the simultaneous existence of Voluntary Counseling and Testing (VCT) and Provider-Initiated HIV Testing and Counseling (PITC). This paper analyzes the concepts, principles and implementation of the VCT and PITC models regarding counseling, confidentiality and informed consent in Brazil and other countries, based on a literature review of works in the Lilacs, Medline, Sociological Abstracts and Cochrane databases published between 2000 and 2013. According to the literature, PITC increases rates of testing in comparison with VCT, but reduces sexual and reproductive rights and the autonomy of users. These findings suggest technical challenges and ethical tensions between the paradigm of exceptionalism and the normalization of HIV testing. The necessity to reconcile increased access to HIV tests with the local capacity to offer comprehensive care for people living with HIV/AIDS is highlighted. It is recommended that interdisciplinary studies about the social effects of VCT and PITC be amplified.

Introduction: Caring for people with HIV is an exercise of reflection and self-analysis in relation to our own beliefs about it. Along with the personal and social values, future nursing professional' beliefs have an important paper in the field of stigmatized diseases such as HIV infection, and may have an impact on nursing care. The aim of this study is to deeply know the meaning of the possibility of living with HIV among nursing students, bringing us a closer understanding of the subjective beliefs that give meaning to the act of caring., Methodology: Qualitative study with a phenomenological approach. Data collection was done through photographs and reflective stories., Results: Living with HIV would change their lives, associating it to illness and fear of social rejection. Planning the future appears difficult, generating kind of uncertainty. However, some light of hope and social change is also observed., Conclusion: The possibility of acquiring HIV is lived as a negative experience associated with old fashioned experiences. It is necessary to facilitate forms of expression to visualize the internal processes of the opinions and invite discussion and permeation to other perspectives that may have an impact, direct or indirect, in nursing care.

Objective: The main objective in the management of HIV-infected pregnant women is prevention of mother-to-child transmission; therefore, it is essential to provide universal antiretroviral treatment, regardless of CD4 count. All pregnant women must receive adequate information and undergo HIV serology testing at the first visit., Methods: We assembled a panel of experts appointed by the Secretariat of the National AIDS Plan (SPNS) and the other participating Scientific Societies, which included internal medicine physicians with expertise in the field of HIV infection, gynecologists, pediatricians and psychologists. Four panel members acted as coordinators. Scientific information was reviewed in publications and conference reports up to November 2012. In keeping with the criteria of the Infectious Diseases Society of America, 2levels of evidence were applied to support the proposed recommendations: the strength of the recommendation according to expert opinion (A, B, C), and the level of empirical evidence (I, II, III). This approach has already been used in previous documents from SPNS., Results and Conclusions: The aim of this paper was to review current scientific knowledge, and, accordingly, develop a set of recommendations regarding antiretroviral therapy (ART), regarding the health of the mother, and from the perspective of minimizing mother-to-child transmission (MTCT), also taking into account the rest of the health care of pregnant women with HIV infection. Objective: This paper presents the design and application in Cuba of a social construct to identify the social dimensions associated with discrimination attitudes towards people with positive diagnosis to HIV., Materials and Methods: A combination of qualitative and quantitative methodology was used for the measurement and validation of a construct, and also the K-mean procedure and factor analysis techniques. All these techniques were applied to "Survey on HIV prevention indicators" 2005, 2006 and 2009., Results: A tool for the identification of the most frequent discriminatory attitudes in a specific social context was validated. This instrument allows clustering the population into different typologies, by level of discrimination, and also, to distinguish some social dimensions of this phenomenon., Conclusions: The results allow distinguishing frequent discriminatory attitudes in any population, focusing the actions in the direction of gradually reducing them, and to evaluate the temporal behavior of this phenomenon.

This paper discusses the methodological problems in the scientific research on HIV/AIDS in Bolivia, both in the areas of epidemiology and social sciences. Studies associated with this research served as the basis for the implementation of health programs run by The Global Fund, The Pan-American Health Organization, International Cooperation, Non-Governmental Organizations and the Bolivian Ministry of Health and Sports. An analysis of the methodological contradictions and weaknesses was made by reviewing the bibliography of the studies and by conducting qualitative methodological research, that was focused on the quality of health care available to people living with HIV/AIDS in public hospitals and health centers, and looked at how programs targeted at this sector of the population are designed and delivered. In this manner, it was possible to observe the shortcomings of the methodological design in the epidemiological and social science studies which serve as the basis for the implementation of these health programs.

Background: Mother-to-child transmission (MTCT) during pregnancy, delivery or breastfeeding, is the main route of HIV infection in children. Strategies aimed at promoting the health of HIV infected pregnant women and MTCT prevention have reduced transmission to below 2%. This paper presents the clinical and epidemiological features of a cohort from Madrid and compares foreign-born with Spanish-born women., Method: Retrospective, observational and descriptive study on HIV infected pregnant women from South Madrid (n=70) and their offspring (n=78) who were born during the study period from August 1992 to January 2010., Results: Most pregnant women were infected by heterosexual transmission (51%). Most Spanish-born women (66%) were diagnosed before pregnancy (81%), while foreign-born women (34%) were diagnosed during pregnancy (70%). Foreign-born women had less obstetric check-ups (67%) than Spanish-born women (97%). The MTCT rate was 1.3% during the last ten years., Conclusions: Heterosexual transmission is the primary mode of acquisition of HIV infection both for Spanish-born and foreign-born pregnant women. However, the HIV infection was diagnosed earlier in Spanish-born women. There were no differences in the effectiveness of antiretroviral therapy as a preventive measure against MTCT when it is started at an early stage., (Copyright © 2011 Elsevier España, S.L. As access to antiretroviral treatment increases in the developing countries, efforts towards making it easier and less costly to collect, store, and deliver the biological samples to reference laboratories, where the serological and genetic diagnosis techniques are performed, have become a high priority. Blood sampling on filter papers is an inexpensive and practical alternative to plasma for antiretroviral treatment monitoring in countries with limited resources and no access to cold chains or refrigeration. The main clinical applications and uses of blood-sampling onto filter papers (dried blood spots [DBS]) are reviewed, focusing on how these can be applied in monitoring HIV infection, particularly for use in National Health Programs in developing countries, or in resource-limited settings. A review is presented of studies that have used the DBS technique for quantifying viral load, analysis of antiretroviral drug-resistance mutations, early infant diagnosis, adult serological diagnosis, detection of viral p24 antigen, and molecular epidemiology of HIV-1, in different geographical locations. Those variables that could affect the use of DBS, particularly in the HIV field, as well as explaining how these procedures can be optimised to increase their sensitivity are also reviewed. In recent years, a significant number of physicians want to spend part of their medical training in health facilities in developing countries. In this setting, clinical skills are extremely important due to the limited available diagnostic resources. Bacterial diseases are common, but bacterial cultures are rarely accessible. In Africa, tuberculosis affects over 200 cases per 100,000 persons, and more than 22 million people live with HIV infection; both diseases are a serious public health problem. Malnutrition is endemic in many countries in Africa and is compounded by the continuous humanitarian and food crisis. The introduction of highly active antiretroviral therapy (HAART) has resulted in a reduction of opportunistic infections associated with cellular and humoral immunosuppression. However, what is still unclear is the impact of HAART on the development of other diseases not associated with AIDS, such as lung cancer and COPD. Background: Therapeutic advance achieved over the last fifteen years in addressing HIV and AIDS correlate with changes in the subjective experiences of persons with HIV and in the sociocultural imaginary of HIV infection. This paper reports findings on two areas: the current typology of HIV patients in the Spanish case and the evolution of the perception of HIV among PLWHA., Methods: Qualitative study in eight Spanish cities (October 2010-March 2011). We conducted 9 focus groups and 30 in-depth interviews with PLWHA of different ages and length of treatment, all of them under medical care. Interviews and focus group were transcribed and narrative and discursive data were subjected to sociological analysis., Results: We suggest a classification of 7 different collectives of persons with HIV in the Spanish context, which reflect different historical periods in the epidemiology of the infection as well as the evolution of treatments and of the experiences of those infected. In recent times there have been significant changes in the social system of images associated with HIV, which point to an increasingly debilitated image of chronicity and to a decline in the perception of its danger to health., Conclusions: But this new imaginary does not distribute equally among all persons with HIV. On the contrary, there is a certain polarization of images, essentially between the two sub-groups most epidemiologically relevant among newly diagnosed HIV patients in Spain: younger men who have sex with men (<30-35 years) and immigrants.

Tuberculosis (TB) and the acquired immune deficiency syndrome (AIDS) are closely related diseases because TB is considered one of the opportunistic diseases for AIDS. When investigating the resurgence of TB it is very important epidemiologically to take into account the influence of AIDS prevalence on TB. This paper develops a procedure establishing HIV prevalence in TB for people in Armenia, Quindío, by using a mathematical model. This was done by building two deterministic models and relating them through a linear transfer function model. To parameters involved in these models had to be estimated to relate them; a technique was then built from unbiased estimators of minimum variance and secondary information for the parameters of each disease, taking into account the town's Secretariat of Health's secondary information. The model for Armenia led to concluding that tuberculosis resulted from people who had suffered from tuberculosis 4 periods (trimesters) ago and those sick from AIDS 2 periods ago, including the present. The procedure described in this work was seen to be applicable to the pathologies to which it is related.

Objectives: To compare variables associated with the risk of HIV among men ≤ 25 years old who have sex with men (YMSM) and those >25 years old, and to determine the variables associated with unprotected anal intercourse (UAI) in YMSM with casual sexual partners., Method: For 1 month, 485 participants ≤ 25 years old and 1,470 aged >25 years old completed a pen-and-paper survey carried out in gay venues (GV) or via the Internet in Madrid, Barcelona, Bilbao and San Sebastián (Spain). The survey investigated sexual behaviors in the past 3 months according to whether the men met sexual partners in GV (SPGV) or on the Internet (SPI)., Results: No significant differences were found between YMSM and those >25 years old with respect to UAI with SPGV (23% vs. 30%, p>0.05) and SPI (29% vs. 30%, p>0.05), condom use in the last anal intercourse and having had a sexually-transmitted infection (STI) in the last year (19% vs. 23%, p>0.05). However, respondents>25 years old had a higher average number of SPGV (9.2 vs. 7, p<0.05) and SPI (8.9 vs. 6.4, p<0.05), were more likely to have used drugs while having UAI (68% vs. 55%, p<0.05, with SPI, and 73% vs. 55%, p<0.05, with SPGV) and had a higher prevalence of HIV (11% vs. 4%, p<0.05). When exploring other variables, we found that YMSM more frequently had receptive UAI with SPI (73% vs. 57%, p<0.05), had less time to be in a stable relationship, and their sexual partner more frequently convinced them to have UAI (8% vs. 0%, p<0.05). In multivariate analysis, UAI in YMSM with casual sexual partners was not associated with any of the variables., Conclusions: YMSM do not constitute a group at increased risk of HIV/STI infection compared with respondents >25 years old. Introduction: Rhodococcus equi is recognized as an emerging pathogen that causes important morbidity and mortality among immunocompromised patients., Objective: to confirm the presence of R. equi in pleural fluid through the restriction fragment length polymorphism technique., Methods: the pleural fluid sample from one AIDS patient with respiratory symptoms was used. Microbiologic culture, staining tests, phenotypic and biochemical tests and restriction fragment length polymorphism technique for the diagnosis of microorganism were performed., Results: the staining technique along with the phenotypic and biochemical tests provided the presumptive diagnosis of R. equi infection, which was further confirmed by the molecular techniques., Conclusions: this paper reported the molecular detection of R. equi from one HIV/aids patient for the first time in Cuba. The results suggested that the molecular biology techniques could be used in the diagnosis and identification of R. equi.

The aim of this paper is to present the most relevant musculoskeletal disorders, their diagnosis and treatment to the scientific community and the professionals in charge of patients with human immunodeficiency virus (HIV) infection. These recommendations have been agreed by a panel of experts from the National AIDS Plan (PNS) and the AIDS Study Group (GESIDA). The group have reviewed the efficacy and safety results of clinical trials, cohort studies and pharmacokinetic studies published in biomedical journals (PubMed and Embase), or presented at conferences. Three levels of evidence have been defined according to the sources of data: level A, randomized controlled trials; level B, cohort or case-control; and level C, descriptive studies and expert opinion. Based on this evidence, the authors have decided to recommend, consider or not recommend for each situation. The decrease in bone mineral density is common in HIV-infected patients on antiretroviral treatment, especially during the first year (from 2 to 4%), with a subsequent partial recovery. A dual energy x-ray absorptiometry (DEXA) scan is recommended in patients over 50 years old with HIV infection, history of bone fractures and/or risk factors. Treatment with bisphosphonates has been shown to be effective. Osteoarticular infections are more frequent than in the non-infected population. The level of immunosuppression, risk practice, and antiretroviral treatment should be considered for a proper diagnosis and therapeutic approach. Laboratory and imaging procedures recommended for the study of musculoskeletal processes in HIV infected subjects are the same as in the general population. Osteonecrosis and decreased bone mineral density are the most frequent alterations in children. An early diagnosis of bone disorders is needed. Objective: To analyze the evolution of published scientific articles on HIV/AIDS in Peru., Methods: A bibliometric analysis of papers on HIV/AIDS published in journals indexed in MEDLINE, SciELO and LILACS until October 2010. We selected research papers fully developed in Peru, and multicenter studies with participating Peruvian sites., Results: We identified 257 publications on HIV/AIDS, showing an increase since 2003. The average publication delay was 2.8±1.8 years. Only 94 (36.6 %) articles were published in Spanish. The most studied areas were epidemiology (36.6 %) and clinical topics (35.8 %). The cross-sectional design was the most frequent (56.8 %) followed by case series. According to the WHO classification, studies to learn more about the disease and risk factors predominated (85.6 %) and according to the intervention areas, 46.7 % focused on diagnosis and treatment. Most studies were conducted in Lima (65.9 %). 48.2 % of studies focused on people living with HIV/AIDS. Finally, Revista Peruana de Medicina Experimental y Salud Pública was the journal that published most articles on HIV/AIDS (9.7 %)., Conclusions: We found a growth in scientific production on HIV/AIDS in Peru; however, we believe that the research undertaken was not based on an agreed national agenda or national research priorities, which might have limitted its dissemination and application.

Unlabelled: Antiretroviral therapy has marked a change in the course of HIV infection, posing new challenges to improve quality of life in pediatric patients., Objective: The goal of this paper was to identify psychosocial aspects of HIV-infected children and adolescents, and to evaluate their neuropsychological development and psychiatric co-morbidity., Patients and Methods: A descriptive study was conducted, evaluating the neurocognitive performance of patients attending at the Pediatric Department of Instituto de Medicina Tropical Pedro Kourí of La Habana, Cuba, between February and July of 2008. For that purpose, interviews to the patients and their relatives were done with the application of appropriate tests for each age., Results: Twenty-three patients ages 3-17 years (57% female) were studied. Sixteen (69.5%) were infected through vertical transmission, and 7 (30.5%) through other routes. School children infected through vertical transmission showed the most symptoms. Hyperactivity, anxiety, and learning disorders were the most frequently observed symptoms as well as hyperkinetic disorders, temporary tic disorders and mental retardation., Conclusion: This study shows the neuropsychological characterization of HIV-positive children and adolescents, considering the route of acquisition of the infection and the stage of the disease. It also describes psychiatric comorbidity of HIV-infected children.

Introduction: HIV-1 proviral DNA amplification is the preferential method for HIV diagnosis infection in infants and it has been used in Cuba since 1992. AMPLICOR HIV-1 DNA kit is an in vitro qualitative assay for the detection of HIV-1 proviral DNA in the whole blood., Objective: this paper showed the results of the use of this kit for the first time in Cuba for the perinatal diagnosis of HIV-1 infection., Methods: three hundred forty six whole blood samples from children of HIV seropositive women were analyzed by the AMPLICOR HIV-1 DNA kit in the period 2005-2007., Results: among the tested samples, six were positive, and 340 negative., Conclusions: the assay was reproducible under the Cuban conditions and the achieved results made the diagnosis and follow up of children of HIV-1 seropositive mothers possible.

This paper provides an overview of the main statistical methods used in the analysis of data from cohort studies and presents a detailed description of the way these methods are applied in HIV/AIDS research. First, we describe methods for analyzing events in person-time data. Second, we present survival methods for the analysis of time-to-event data. In this context, we illustrate the application of these methods to describe the AIDS incubation period and survival from HIV seroconversion, to determine the factors and biological markers associated with disease progression, and to evaluate effectiveness of therapy. In this paper, we provide a definition of cohort studies and reviews the main types of cohort studies used in the context of HIV infection. We discuss how the main sources of selection biases in cohort studies are those derived from the lack of observation of the event that determines the origin and/or of the event of interest due to losses to follow-up or development of a competing event, and how this bias must be appropriately taken into account following specific epidemiological methods. Although cohort studies play an essential role in the study of HIV infection, they are logistically complex and require considerable resources. AIDS patients and HIV carriers who are aware of their condition are under multiple kinds of stress with adverse effects on their emotional state and personal and social activity. This paper reports the psychometric properties of the DAS (Death Anxiety Scale) using the Spanish version in the clinical setting. The sample is made up of 148 HIV/AIDS patients (109 men and 39 women). The internal consistency of the scale was .72 and its test-retest reliability was .70. Principal components analysis extracted five factors that jointly accounted for 56.5% of the total variance. As, on the whole, these results are very similar to those reported by other authors, it is concluded that the Spanish DAS is a valid instrument for the assessment of death anxiety in Spanish HIV/AIDS patients.

Cytomegalovirus infection is an important cause of morbidity in immunosupressed patients with Human Immunodeficiency Virus (HIV). In this paper we present a 43 years old man with renal failure under hemodialysis, several blood transfusions because of anemia and three months of disease characterized by epigastric pain, specially at nights, ameliorated with antacid drugs. Other symptoms were early satisfy, vomits and weigh loss (18Kg). At clinical exam, the patient was pallid, presented adenopathies at cervical and inguinal regions and had a pain at epigastric region in profound touch palpation. The most important exams were HB: 10mg/dl, CMV: 83.5, leukocytes 7000, lymphocytes: 1715, erythrocyte sedimentation rate 49mm/h, the venon test (-), and Giardia lamblia trophozoites in stools. The studies demonstrated the patient was seropositive for HIV and the tests for IgG CMV and IgG Herpes virus resulted seropositives too. At endoscopy the esophagus mucosa was covered by a white plaque which suggests candida infection. In the stomach, over the body gastric, we found a big and deep ulcerated lesion (45 x 41mm), with defined rims and white fund. Biopsy from the edges of the gastric ulcer had the characteristic CMV intranuclear and intracytoplasmic inclusions; we confirmed the diagnosis by immunohystochemistry. The patient receives ganciclovir an then HAART and is getting well.

Background and Objective: To asses the human immunodeficiency virus (HIV) seroprevalence evolution of newborns' mothers., Population and Method: Unlinked anonymous study of HIV antibodies in blood spots for congenital metabolic disorders detection in newborns, from 1996 to 2005; in Baleares, Canarias, Castilla-La Mancha, Castilla y León, Galicia, Melilla and Murcia Regions, including Valencia from 2003 on. HIV-antibodies screening was performed through ELISA and results were confirmed using immunoblot., Results: Global prevalence rates were stable through the period in the 7 initially enrolled regions, near to 1 per thousand; however, during the first 5-year period the prevalence tended to increase (p < 0.05), while a reverse trend was observed in the following 5 years (p < 0.05). Baleares and Valencia showed the highest rates, although Baleares showed a declining trend. Canarias is the only region that displays an increasing trend., Conclusions: HIV prevalence in newborns' mothers remained steady during the 10-year period, but after a phase of significant increase the tendency has reverted. Data presented in this paper show the importance of monitoring HIV prevalence among women who give birth, and emphasise the need of improving antenatal prophylaxis programmes to prevent mother-to-child transmission.

Objective: To determine the HIV seroprevalence in Mexican blood donors and the residual risk of transfusion., Material and Methods: The seroprevalence was determined in a sample of first-time blood donors at one hospital center in Mexico City, from 2003 to 2007. To estimate the seroprevalence reported in Mexican blood donors, we reviewed recent papers. To determine the positive likelihood ratio (LR+) of the EIA test the specificity was calculated against the western-blot result. To infer the residual risk, the incident infections were assumed to be 1.8 times more frequent for first-time donors., Results: We analyzed 29,318 donors; 66 were reactive to HIV by EIA (225/100,000; 95% Confidence Interval: 171 to 279/100,000), but western-blot confirmed only 5 (prevalence = 17/100,000 donors; 95% CI: 2 to 32/100,000). The maximal residual risk was inferred to be 6.2 per million, or about 6.8 per year. The LR+ of the EIA test was calculated to be 476. The Bayesian analysis estimated that the disease is present in only 7.5% donors with a reactive EIA. Published reports in medical literature do not inform confirmatory tests for Mexican donors., Discussion: The residual risk for HIV had been calculated to be about 100 per million of blood donors. Although we inferred that the risk had been overestimated by not performing confirmatory tests, the results are a call for action as the risk is still several times higher than the one reported in industrialized countries.

This paper describes the increasing trends and changing patterns of human immunodeficiency virus (HIV) infection in children occurring since 1982-83 when the first cases were reported worldwide and from 1987-89 when the first cases were detected in Chile. Advances and obstacles dealing with pediatric HIV in Chile are discussed. The most relevant advance is the outstanding reduction in vertical transmission of HIV associated with the implementation of preventive strategies. Risk of vertical transmission has decreased from 35%o in absence of preventive strategies to 2%o with prevention that include maternal and newborn antiretroviral therapy, elective caesarean section and replacement of breast feeding by artificial milk. The main obstacle to further reduce cases has been the delay in implementation of universal screening for pregnant women. This delay has resulted in that currently, the great majority of pediatric HIV cases are detected among symptomatic children. We propose a guideline to detect and appropriately derive pregnant women infected with HIV and children exposed or infected with the virus in order to further reduce the number of children with AIDS in Chile.

Objectives: Standard methods to evaluate population effectiveness of treatments in observational studies have important limitations to appropriately adjust for time-dependent confounders. In this paper, we describe a recently developed methodological approach, marginal structural models (MSM), and use it to estimate the effectiveness of highly active antiretroviral therapy (HAART) on AIDS or death incidence., Subjects and Methods: We analyzed all subjects followed after 1997 as part of the GEMES project (comprised by several cohorts of HIV seroconverters in Spain) and who had not used HAART before the start of follow-up. To estimate the effect of HAART on AIDS or death incidence, we estimated the parameters of a marginal structural Cox model by fitting an inverse probability weighted logistic regression model. The estimation of the weights was based on CD4 count, time since seroconversion, sex, age, transmission category and previous treatment., Results: 917 eligible subjects were followed for an average of 3.4 years and we observed 139 events. 42.1% of the participants received HAART during the study. The estimated rate ratio was 1.01 (95% confidence interval [CI], 0.68-1.49) using a Cox model without covariates and 0.90 (95% CI, 0.61-1.32) using a Cox model with time-dependent covariates. The causal rate ratio estimated for MSM was 0.74, (95% CI, 0.49-1.12)., Conclusions: The beneficial effect of HAART estimated by the MSM, but largely missed by conventional methods, is consistent with the findings of previous randomized studies. The MSM appropriately adjusted for the time-dependent covariate CD4 count, which is both a time-varying confounder and is affected by prior treatment.

Background and Objective: The main strategies to avoid the vertical transmission of human immunodeficiency virus (HIV-1) is antiretroviral treatment during pregnancy, delivery and to the newborn. Nevertheless, there are scant data about safety of antiretroviral drugs and follow-up studies with a large number of pregnant women are needed. The aims of our study are: to assess toxicity associated with antiretroviral treatment, to determine vertical transmission, and to define epidemiological trends within this population. In this paper we present maternal characteristics of the cohort., Patients and Method: Observational, multicentric, collaborative study about an HIV-1 infected pregnant women cohort and their offspring (666 and 686 cases, respectively) who were born during the study period (from January or May 2000 to 31st december 2003)., Results: Most pregnant women were infected by heterosexual transmission (54%). 71% women were included in stage A of the Centers for Disease Control classification of HIV infection and 74% women have received high activity antiretroviral treatment during pregnancy. Prevalence of adverse effects treatment-related was 7%, mainly anaemia related to treatment with zidovudine. We found 6 cases of vertical transmission among 686 newborns (0.8%; 95% confidence interval, 0.3-1.8)., Conclusions: Most patients received highly active antiretroviral treatment, with a good tolerance and a low prevalence of adverse effects related to the treatment. Vertical transmission rate is very low, not higher than 1%. Apart from this, is necessary to pay attention to other phenomena like hepatitis C virus coinfection, other potentially serious adverse effects like hepatotoxicity and pregnancy-related pathologies more frequent in this population, like prematurity and gestational diabetes.

Objective: To present a pharmaceutical care program for pediatric patients receiving antiretroviral therapy., Method: In order to establish the pharmaceutical care procedure, papers published up to 2004 on the pharmaceutical care provided to patients receiving antiretroviral therapy were reviewed through a search in Medline and the journal Farmacia Hospitalaria. In addition, bibliographic references that can be systematically used to analyze the pharmacotherapy of each patient have been selected., Results: The pharmaceutical care procedure is divided in three stages (data collection, analysis of the pharmacotherapeutic profile and resolution of the drug-related problems identified) that take place through a semi-structured type of interview. In order to systematize the role of the pharmacist, a table with information on antiretroviral drugs used in Pediatrics was created, as well as an information three-page leaflet and a data collection form., Conclusions: The program includes the goals of the pharmaceutical care process as defined in the recommendations of GESIDA-SEFH-National AIDS Plan 2004 and systematizes the proposed intervention strategies, in an attempt to provide the patient and the caregiver with the information required for an optimum management, in the most comprehensive way and tailored to their individual characteristics.

Objective: This paper first presents the trend of HIV/AIDS expenditures in Mexico between 1997 and 2002. It then compares the allocation and magnitude of such expenditures within the international context and, in particular, how other countries in the region are allocating HIV/AIDS resources., Material and Methods: Since 1997, Mexico has developed and been utilizing a method for systematically and comprehensively analyze national resource and expenditure flows for HIV/AIDS prevention and care; an exhaustive study known as the National AIDS Account. Results. During this five year period, a systematic increase in real expenditures occurred (143% increase in total), most of which was allocated to the purchase of drugs for antiretroviral treatment. Nevertheless, compared with other countries in the region, Mexican expenditures for HIV/AIDS are less biased in their allocation between prevention and treatment., Discussion: Mexico is committed to ensure universal coverage of AIDS treatment while maintaining and even increasing prevention efforts. These activities demand increasing amounts of resources. It is, therefore, important to track how efficiently such resources are being used in order to improve allocation and targeting of health resources.

In this paper the authors update on the deletereous or beneficial roles of human and animal secretory phospholipases A2 (sPLA2). Although human sPLA2-IIA (inflammatory) was initially thought as a foe because its pathogenic implication in sepsis, multiorganic failure or other related syndromes, recent data indicates its role in in the antiinfectious host resistance. Thus, sPLA2-IIA exhibits potent bactericidal activities against gram-negative and gram-positive (in this case, together with other endogenous inflammatory factors) bacteria. Surprisingly, human sPLA-IIA does not show in vitro anti-human immunodeficiency virus (HIV) activity, whilst several sPLA2-IA isolated from bee and serpent venons do it: this is the case for crotoxin, a sPLA2-IA isolated from the venon of Crotalus durissus terrificus (sPLA2-Cdt). The mechanism for the in vitro anti-HIV activity of sPLA2-Cdt (inhibition of Gag p24) appears to be related to the ability of the drug to desestabilize ancorage (heparans) and fusion (cholesterol) receptors on HIV target cells.

Background: In cohort or longitudinal studies, subjects are recruited some time after the beginning of the problem, as in HIV infection. The aim of this paper is to show several imputation techniques of the beginning of follow up and evaluate its use in the framework of a study of HIV progression., Methods: Three subcohorts of HIV+ subjects recruited in Valencia and Castellón CIPS up to 1996 are available. Seroconversion date was estimated for 244 Seroincidents, 887 seroprevalents with CD4 measurements and 337 without CD4 measurements. For seroincidents midpoint between last HIV- and first HIV+ visits was considered. For prevalent with CD4 serocon version date was imputed from 5 random samples of a progression model of infection to a CD4 level. For prevalent without CD4 seroconversion date was imputed from 5 random samples of HIV incidence density obtained from the other subcohorts. The imputation was repeated 500 times, assigning the seroconversion date as the median of imputations and obtaining confidence limits from 5 and 95 percentiles. Imputation validity was tested comparing time to AIDS and death for each one of the 3 groups., Results: 443 and 405 deaths were observed among the 1468 subjects. Median of seroconversion was January 1993 for incidents, January 1991 for prevalents with CD4 and November 1988 for prevalents without CD4. The latest group showed a worse survival and AIDS free time compared to the other two cohorts., Conclusions: The imputation tools used showed their usefulness to reduce the survival bias in observational studies. Their generalization depends on the viability of incident cohorts, the availability of a progression marker or a origin on time.

Overview: After some time under treatment, HIV+ patients have a virologic failure rate of 50%, being development of resistance to therapy responsible for up to 80% of the virologic failure. In addition, resistance rates in naive patients is around 10% in developed countries. Inherent characteristics of HIV (replication cycle, viral subtype), of patients (therapy compliance, intra-/interindividual variability, genetic polymorphisms), and of therapy (genetic barrier to drug resistance, inhibitory ratio, drug interactions) are the factors involved in the development of resistance, and their interpretation requires to be studied. Resistance identification will be carried out using genotypical and/or phenotypical methods, and their adequacy has been validated by various expert panels on resistance. The role of the pharmacokinetic and pharmacodynamic monitoring of antiretroviral therapy is also crucial within the field of resistance, and concerns us directly as pharmacists. Finally, understanding the resistance patterns of currently available or experimental antiretroviral drug families will provide the necessary tools to prevent and/or manage their development., Objectives: To know and understand the mechanisms and patterns of resistance for each antiretroviral family. To identify factors involved in the development of resistance to ART, and to interpret various resistance tests., Search Strategy: Studies were identified using Medline, the Cochrane database of systemic reviews, abstracts from international meetings on AIDS, Conference on Retroviruses and Opportunistic Infections, international meetings on resistance to antiretrovirals, and product monographs from January 1999 to February 2004., Selection Criteria: To be eligible, studies had to describe viral genome mutations responsible for resistance or hypersusceptibility to ART in relation to precipitating factors. Papers describing resistance identification techniques were also selected., Data Collection and Analysis: In all, 1,083 full articles and 64 abstracts and communications at international meetings were retrieved, of which 74 articles and 20 abstracts met the inclusion criteria for our review., Primary Results: Of the 94 reports selected, 86 discussed factors involved in the development of resistance and resistance test interpretation. The remaining 8 reports focused on resistance patterns to the various antiretroviral drug families. Every article described the enzymatic mechanisms induced by mutations responsible for resistance or hypersusceptibility to each antiretroviral family, the classification and nomenclature for each mutation, and the influence of each mutation on the success or failure of patient treatment. REVIEWER S CONCLUSIONS: Knowledge of the mechanisms and patterns of resistance to each antiretroviral family will allow us to overall understand the evolution and outcome of treatment for any given patient. Only thus shall we be able to play an integral role in the therapy of patients.

Introduction of new antiretroviral agents and development of new prophylaxis schedules against opportunistic microorganisms have allowed increase in survival as well as better quality of life in HIV-infected patients. These new treatment schedules have changed the epidemiology of opportunistic infections that previous to use of highly active antiretroviral therapy (HAART), formerly occurred with high frequency in HIV-infected children. Specifically, pneumonia due to Pneumocystis carinii formerly occurred in 12 to 80% of these patients and was associated with high mortality. Currently, with use of combined antiretroviral therapy (ART) and prophylactic treatments important reduction of PCP has been observed. However, despite these benefits ART is not yet available for many patients from several developing countries who are at risk for opportunistic infections, mainly due to Pneumocystis carinii, which can affect life expectancy. Therefore, the purpose of this paper was a review of epidemiologic, clinical, and therapeutic characteristics of P. carinii pneumonia in HIV-infected children.

Objective: To develop a neuropsychological test battery in Spanish for the cognitive evaluation of HIV 1 infected patients., Development: Departing from the suggestions presented by the work group of the National Institute of Mental Health (USA), a neuropsychological assessment battery was developed. It was named HUMANS (HIV/University of Miami Annotated Neuropsychological test battery in Spanish). This battery includes the following domains: 1) attention and speed of processing information, 2) memory, 3) executive function, 4) language, 5) visuospacial/visuoconstructive abilities, and 6) motor abilities. Administration takes about 3 4 hours. The English parallel version of this battery has been successfully used in English for over a decade with HIV 1 infected patients. In the paper the development and adaptation to Spanish language of the HUMANS neuropsychology section is presented., Conclusions: HUMANS neuropsychological test battery fulfill the recommendations presented by the work group of the National Institute of Mental Health for evaluating HIV 1 infected patients. Studies regarding validity and reliability are still required.

Objective: To investigate the seroprevalence of viral markers for the human immunodeficiency virus (HIV), and the hepatitis B (HBV) and C (HCV) viruses, in blood donors at the General Hospital No. 2 Family Medicine Unit, of the Mexican Social Security Institute in Irapuato, Mexico., Material and Methods: A cross-sectional descriptive study. Data was recorded on blood bank forms, and risk factors and illnesses were studied in the 7,056 blood donors at the General Hospital No. 2 Family Medicine Unit, of the Mexican Social Security Institute in Irapuato, Guanajuato, Mexico, over a period of two years (from July 1998 to June 2000). A sample of 4,010 donors was obtained, each of whom underwent serological tests for HBV, HCV and HIV, serotypes 1 and 2, using an enzymatic immunoassay of third generation in serum or human plasma; seroprevalence rate of seropositive donors was calculated and stratified by age and sex., Results: The combined seroprevalence for HBV, HCV and HIV was 2.5% (101); HCV was 1.14% (46), HBV, 1.12% (45), and HIV, 0.24% (10). In males, HBV was 1.04% (33), HCV 1.07% (34), and HIV, 0.28% (9). In females, HBV was 1.42% (12), HCV was 1.42% (12), and HIV was 0.11% (1). Seropositive males had a 2.4 higher rate as compared to females., Conclusions: The seroprevalence of viral markers was greater than that reported in previous studies carried out in Mexico, which suggests that sexual transmission was the principal mechanism of infection; this reflects poor health education and the need to carefully select potential donors. This paper presents some thoughts on the problems endured by women facing HIV/AIDS. World and nationwide statistics show an upward trend of the epidemic among women, calling for the need to address this problem. A case is made for the way in which gender structure and relations facilitate the vulnerability of women to this disease, increasing their risk of infection related with specific factors like migration, employment unsafe conditions, and domestic violence, among others. Emphasis is made on the main obstacles faced by women to flag their concerns and needs in society, as well as on their lack of empowerment to cope effectively with their problems. Some initiatives advanced by the organized civil society and governmental strategies are presented, including accomplishments, deficiencies, needs, and challenges. Finally, conclusions and recommendations are given, highlighting the need to establish and evaluate preventative programs and policies from a gender and human rights perspective, to promote a greater awareness and improved responses to the problems imposed on women by the HIV/AIDS epidemic.

Background: To analyze the research output of Spanish authors on human immunodeficiency virus (HIV) infection/AIDS., Methods: Papers published over 1991-1999 which were recorder in the MedLine database (CD-ROM) were selected by using the key words Spain or Espana in the address of author field and HIV, AIDS, VIH or sida in any field. Papers were analyzed according to publication journal specialty of authors, subjecte matter of the paper, institution, center of origin and geographical distribution., Results: A total of 2,065 papers were studied. Their number doubled, from 148 in 1991 to 295 in 1999 (r = 0.917; p < 0.0001). The number of papers published in foreign journals trebled, from 51 to 182 (r = 0.933; p < 0.001). The journal with the highest number o papers published was Medicina Clínica: 241 (11.7%). Although the main topic was the clinical manifestations of HIV infection (64.6%), the proportion of papers on this subject decreased from 71.4% in 1991-93 to 58.3% in 1997-99 (p < 0.001). On the other hand, the percentage of articles on antiretroviral therapy increased from 3.6% in 1991-93 to 12.4% in 1997-99 (p < 0.001). The greatest number of contributions came from hospitals (79.8% of papers), followed by universities (9.5%). Centers with the highest output were the Hospital Clínic (Barcelona) (5.7%) and the Hospital Carlos III (Madrid) (5.5%). The Autonomous Communities of Madrid (33.1%) and Catalonia (25.3%) concentrated more than half of the total output, followed by Andalusia (11.5%) and the Valencian Community (8.1%)., Conclusions: Spanish output on HIV infection/AIDS research grew during the 1991-1999 period. There was a remarkable increase in the papers published in foreign journals. Moreover, topics changed according to the epidemiological changes observed in HIV infection during the past decade.

Objective: To identify the factors associated with the acceptance of tubal ligation after childbirth among HIV-infected patients., Material and Methods: A case-control study was conducted from March 1988 to February 1999, at Mexico's National Institute of Perinatology (INPer), in 72 HIV-positive pregnant women. Cases were 49 women who accepted postpartum tubal ligation after childbirth, and controls were 23 women who refused this birth control method. Data collected for each patient were demographic characteristics, sexual and reproductive history, and HIV status. Statistical analysis consisted of descriptive measures, Chi 2 or Fisher's exact test for categorical variables, and Student's t test for continuous variables. Odds ratios (OR) with 95% CI were used to compare groups and potential confounders were assessed by stratified analysis with the Mantel-Haenszel method., Results: The patients' mean age was 25.5 +/- 5.5 years. The median gestation period was 27 weeks (range 7 to 40 weeks); 16 women (22.2%) had no prenatal care visits at INPer. The median time of HIV positivist awareness was 9 months (range 1 to 108). Variables associated with acceptance of tubal ligation were: having a prior childbirth (OR 11.1, 95% CI 3.4 to 36), pregnancy care from 1995 onward (OR 4.7, 95% CI 1.7 to 13.3), and having given birth to an HIV-infected child (OR 4.6, 95% CI 1.05 to 23.1). Stratified analysis showed no modification of the strength of association of these variables with acceptance of tubal ligation., Conclusions: A prior childbirth was the most important predictor of tubal ligation acceptance. The emergence of acquired immunodeficiency syndrome has changed the natural history of tuberculosis which has now become the second most common infection associated to human immunodeficiency virus infection. It is only rarely that a tuberculous infection has an urogenital location, and extrapulmonary locations are generally related to severe immunosuppression. This paper presents one case of tuberculous orchitis that presented as the clinical onset of acquired immunodeficiency syndrome. Discussion of the clinical evolution and the therapeutic approach that consisted in orchiectomy associated to treatment with tuberculostatics.

On April 7, 1999, the Monographic Meeting on HIV Infection and Tuberculosis was held at the Secretariat of the National AIDS Plan for the purpose of setting out certain clinical and health care policy recommendations concerning the control of tuberculosis (TB) with regard to the HIV infection epidemic, those in attendance being listed in Appendix 1. This meeting was organized into a number of presentations of papers (Appendix 2) grouped into four subject areas, which were followed by the pertinent debates. These four areas were as follows: 1. Epidemiology of the Dual HIV-TB Infection. 2. Treatments to Combat TB. 3. TB Prevention. 4. Health Care/TB Control Programs. The papers presented regarding a review of current literature focused on the search for scientific evidence with regard to treating and preventing TB among HIV-positive patients. The rest of the papers presented were: 1) regarding epidemiological topics 2) regarding results of different TB control programs and 3) regarding the analysis of the international and national recommendations concerning TB treatments and prevent among HIV-positive patients. Following the meeting, this report has been prepared as a summary thereof and was revised by all those who were in attendance at the meeting, the goal of which is that of setting out some recommendations for health care control and clinical handling of the dual Mycobacterium tuberculosis and HIV infection. This report provides no systematic or sufficiently detailed review of the different topics analyzed, therefore opting with regard to the bibliography to recommend a number of brief articles and reference books as a general source for further consultation. In this report, the term of "dual" TB-HIV infection is used generally and, in others the more specific "dual TB-AIDS disease", depending upon the presence of infection or disease caused by M. Tuberculosis and also AIDS, therefore, in the latter case.

In the last 20 years, research on the determinants of the HIV epidemic among drug users has focused mainly on studies of risk behaviour for drug injection. Studies involving human behaviour present special methodological problems. This paper presents a review of 1) the most important features that make this field different from the study of blood-borne diseases in other populations, and 2) the basic variables used in epidemiology to analyse injecting risk behaviour. Alternatives to improve research planning and results are proposed based on collaborating with other disciplines and improving the methodological resources of epidemiology.

The cytopathic effect of HIV on CD4 T cells, as well as the active autoimmune mechanism occurring during infection, have been documented. Of the cytokines involved in the pathogenesis of AIDS, the main one produced by the monocyte-macrophage series is tumor necrosis factor alfa (TNF alpha). This cytokine induces antigens such as proteinase 3 (Pr 3) or mieloperoxidase (MPO). Anti-neutrophil cytoplasmic antibodies (ANCA) are directed against this type of PMN antigens. In the present paper, the role of anti-neutrophil cytoplasmic antibodies (ANCA) in HIV infected patients as responsible for autoimmune phenomena in relation to opportunistic infections, was studied. A total of 88 serum samples belonging to 49 asymptomatic and 39 symptomatic HIV infected patients were tested for ANCA by an indirect immunofluorescence (IIF) test over a neutrophil substrate. ANCA were detected in 53.8% of symptomatic patients as compared to 4.1% in asymptomatic cases (p < 0.0001). A 95.9% correlation was observed between ANCA-positive samples and pulmonary infection (n = 20). In those ANCA (+) samples 95.9% correlation was found with pulmonary infection (n = 20). Pulmonary infection due to Mycobacterium tuberculosis is a frequent finding in HIV infected patients from Northeastern Argentina. When the presence of ANCA in TB(+) HIV(+) and TB(+) HIV(-) patients was studied, it was seen that positive-ANCA significantly correlated with the first group (p < 0.001). The presence of ANCA was not related to viral infections, toxoplasmosis, neurological features of AIDS, vasculitis or malignant diseases. ANCA during pulmonary infection, mostly caused by TB, as well as PMN infiltration in pulmonary parenchyma, and the deregulated immune reaction elicited by HIV, may contribute to the onset of autoimmune phenomena. The presence of human T lymphocytes reactive to heat stress proteins (Hsp), an important target of immune response against certain intracellular auto-antigens such as MPO from PMN, added to the mechanism of molecular mimicry, could explain the association of ANCA and TB in patients with severe alterations of their immune response. According to these results, the limited presence of ANCA in asymptomatic patients HIV(+) and in HIV(-) with pulmonar TB, could indicate that the virus may not be responsible for the induction of these antibodies.

Renal involvement in AIDS may be specific or unspecific. Unspecific lesions, the most common, are usually an acute tubular necrosis produced by hemodynamic, infectious or electrolytic alterations that lead to an acute renal failure or drug nephrotoxicity. Specific lesions are segmental and focal hyalinosis, immune complex glomerulonephritis and thrombotic microangiopathy. Focal and segmental hyalinosis is observed almost exclusively in black people and produces a rapidly progressive renal failure. Lesions are a consequence of HIV stimulation of TGF beta in mesangial cells. Immune complex glomerulonephritis, formed by HIV antigens and anti HIV antibodies, is observed in white and black people. The glomerular lesion in this condition is less severe than in the former. Thrombotic microangiopathy could be a consequence of a pathogenic effect of the virus over glomerular capillaries and arterioles. It is clinically expressed as a hemolytic uremic syndrome. This paper reports briefly the renal pathological study of 46 patients infected with HIV-1, seen at the Nephrology Service of the Bichat Hospital in Paris.

Syphilis and gonorrhea are two sexually transmitted diseases (STD) which still persist in spite of the existence of effective treatment. In this paper the incidence of these two diseases was evaluated between 1985-1994, a decade during which the problem of HIV infection arose. Every patient who attended the STD Program at the Hospital and was suspected of having syphilis and/or gonorrhea was clinically evaluated and was subjected to the following tests: a) search for Neisseria gonorrhoeae (Ng) and penicinilase producers (PPNG) identification, b) search for Treponema pallidum (Tp) by darkfield microscopy, c) serological studies for syphilis. From 1985 to 1991 the average of positive cases for Ng was 14.8% while from 1992 to 1994 it was 4.0% (p < 0.0001) showing a dramatic decrease between 1991 and 1992. PPNG increased from 8.7% in 1985 to 44.6% in 1988 and then slowly decreased down to 26.9% in 1994. From 1985 to 1990 the average of positive darkfield examinations was 20.7% while from 1991 to 1994 it was 12.2% (p < 0.0001). Between 1985 to 1988 syphilis serological studies showed 45.7% positive cases dropping to 34.3% between 1989 and 1994 (p < 0.0001). HIV infection appearance had a wide repercussion throughout the world generating behavioral and sexual habit changes which have probably helped to prevent these two STD. Gonorrhea incidence was also influenced by the appearance of new effective treatments, even against PPNG, such as fluorated quinolones and third generation cephalosporines. The population studied belongs to the most crowded area in Argentina, so that these observations should not be extrapolated to the rest of the country.
