Introduction. Amidst a steady increase in hyperplastic processes in uterus and mammary glands, numerous studies have shown that women with dyshormonal pathology of mammary glands had a range of gynecological diseases: external or internal endometriosis, hyperplastic processes of the endometrium and myometrium, polyps, polycystic ovary syndrome (PCOS), etc.Objective. To assess the mammary gland condition in premenopausal women with endometrial hyperplasia during indole-3-car-binol (I3C) therapy.Materials and methods. A total of 42 premenopausal patients aged 45-52 years (48.54 ± 1.83) were enrolled in an open-label prospective study. All patients received I3C 400 mg a day for 12 months. The examination included clinical laboratory, ultrasound imaging and mammographic examination of the mammary glands, cytologic examination of a nipple discharge. The examinations were carried out at the baseline, 6 and 12 months after starting treatment. The study findings were evaluated using descriptive statistics. In the review article the basic problems of children’s health of young age are considered deficiency of vitamins and minerals at breastfeeding, complexity of skin care, allergies, acute respiratory viral infections. Attention is paid to hypovitaminosis and nipple cracks in mothers during lactation, as well as to the necessity of vitamin-mineral complexes subsidizing and mammary gland skin care during this period. Proper care for a child of early age is a guarantee of his health and harmonious development. Paediatricians should promote maternal literacy in modern methods of child care, disease prevention and treatment.

Breast tissue has a high concentration of cytosolic tissue receptors to steroid compounds, so like other organs of women’s reproductive system it is susceptible to 15 hormones. Estrogen concentration in the breast connective tissue in 2–20 times higher than in blood plasma. Decisive role in the emergence of breast pathology belongs not to the absolute concentrations of hormones in blood plasma, but condition of sex steroids receptors in breast tissue. Mastodynia is a reflection of functional changes and response of breast tissue to hormonal imbalance. Proliferative mastopathy without atypia and with atypia significantly increase the risk of breast cancer. The purpose of this work is to determine the optimal markers for clarifying the degree of proliferative activity in benign breast lesions, which can be used in surgical treatment planning.Materials and methods. The immunocytochemical results of tumor proliferation marker (Ki-67), cancer-embryonic antigen (CEA), matrix metalloproteinase-9 (MMP-9) and vascular endothelial growth factor (VEGF) expression were studied in biopsy samples collected from 108 patients with benign breast lesions and 49 patients with breast cancer.Results. It was found that the proliferative processes rising was accompanied by an increase in the number of patients with expression of Ki-67 to 28.0 %, CEA to 44.0 %, VEGF to 80.0 %, and a decrease in the number of patients with MMP-9 expression from 75.0 % to 36.4 %. Among patients with breast cancer, cells expressing Ki-67 were observed in 36.8 %, MMP-9 in 20.7 %, CEA in 36.4 %, VEGF in 48.3 %. The results indicate the presence of a neoplastic potential in benign lesions and its enhancement with an increase in proliferative properties.Conclusions. This diagnostic method can be used to determine the malignant potential in benign breast lesions and for therapeutic decision making., Цель работы – определить оптимальные маркеры для уточнения степени пролиферации в доброкачественных образованиях молочной железы, что может быть использовано при планировании оперативного лечения.Материалы и методы. Изучены результаты исследования иммуноцитохимическим методом в биопсийном материале экспрессии онкомаркеров пролиферации (Кi-67), раково-эмбрионального антигена (РЭА), металлопротеиназы (ММП-9) и фактора роста эндотелия сосудов (VEGF) у 108 пациентов с доброкачественными заболеваниями молочной железы и 49 больных раком молочной железы.Результаты. Установлено, что по мере нарастания пролиферативных процессов возрастало количество больных с проявлением экспрессии Кi-67 до 28,0 %, РЭА до 44,0 %, VEGF до 80,0 %, а ММП-9 уменьшался с 75,0 % до 36,4 %. Среди больных раком молочной железы экспрессия клеток Кi-67 отмечена у 36,8 %, ММП-9 – у 20,7 %, РЭА – у 36,4 %, VEGF – у 48,3 %. Результаты свидетельствуют о наличии у доброкачественных образований неопластического потенциала и повышении его с увеличением пролиферативных свойств.Выводы. Вивчили результати дослідження імуноцитохімічним методом у біопсійному матеріалі експресії онкомаркерів проліферації (Кi-67), раково-ембріонального антигена (РЕА), металопротеїнази (ММП-9) і фактора росту ендотелію судин (VEGF) у 108 пацієнтів із доброякісними захворюваннями молочної залози та 49 хворих на рак молочної залози.Результати. Встановили, що разом із наростанням проліферативних процесів збільшувалася кількість хворих з проявом експресії Кi-67 до 28,0 %, РЕА до 44,0 %, VEGF до 80,0 %, а ММП-9 зменшувалася з 75,0 % до 36,4 %. Серед хворих на рак молочної залози експресію клітин Кi-67 спостерігали у 36,8 %, ММП-9 – у 20,7 %, РЕА – у 36,4 %, VEGF – у 48,3 %. Результати свідчать про наявність у доброякісних утвореннях неопластичного потенціалу та його підвищення зі збільшенням проліферативних властивостей.Висновки. Метод дослідження, який описали, можна використовувати для визначення злоякісного потенціалу в доброякісних утвореннях молочної залози і під час вибору методу лікування.

Objective. Investigation of quality of life (QL) in patients, suffering inoperable forms (locally-spread) of mammary gland cancer (MGC), before and after conduction of neoadjuvant courses of systemic polychemotherapy (SPCHTH) and selective intraarterial polychemotherapy (SIAPCHTH) in combination with SPCHTH. Маterials and methods. Complex treatment was conducted in 154 patients, suffering locally-spread MGC T4A-DN0-3M0 in a period 2000 -2017 yrs, on base of Donetsk Regional Antitumoral Centre and The University Clinic of Odessa National Medical University. All the patients were divided into two groups. Tо Group I (control) were included 65 patients, suffering inoperable forms of a locally-spread MGC, to whom as a neoadjuvant course a SPCHTH was conducted; and to the Group II (investigating) - 89 patients, suffering inoperable forms of locally-spread MGC, to whom a SIAPCHTH was conducted as a neoadjuvant course. Results. Through the period of treatment a wave-like dynamics of the quality of life index was observed, which indicated a definite advantage of SIAPCHTH over SPCHTH. A limited statistically trustworthy difference in favor of regional polychemotherapy was formed between the Groups of patients in accordance to the quality of life index. Conclusion. Investigation of quality of life constitutes a secure, informative and economic method of estimation of a patient state of health on the groupings and the individuals levels. In oncological investigations the QL assessment constitutes an important criterion of the treatment efficacy estimation and has a prognostic significance. Estimation of quality of life in clinical investigations improves the investigation quality itself.

The delayed reconstruction of the mammary gland after radical mastectomies is a surgical method for the rehabilitation of patients. Breast reconstruction using an expander is used in women with a small breast size, as well as in patients who refuse to use the patchwork methods for restoring the mammary gland. The article describes expansions, which were applied from the moment of their creation and those that are currently in use. Indications and contraindications to the delayed reconstruction of the mammary gland with the use of an expander are determined.The article presents the mammary gland marking and stages of delayed reconstructions performed in the department of oncology and reconstructive-plastic surgery of the breast and skin of the P. Hertsen MORI. In the case of a small size of the breast and the consent of patients for the use of implants, this reconstruction is the main surgical intervention. Rehabilitation of patients after radical mastectomies is a series of surgical interventions using expander techniques. At the 1st stage – the use of the expander, at the 2nd stage – the replacement of the expander for the implant, at the third stage – tatooing of areola and / or reconstruction of the nipple-areolar complex from its own tissues.Due to consistent surgical interventions, as well as the correction of the second breast, it is possible to achieve symmetry of the mammary glands, and, of course, to recreate the removed mammary gland.

Background. The last decades are devoted to the search for various factors leading to the development of benign breast diseases, which are the risk of breast cancer. In gynecological diseases, the frequency of benign processes in the mammary glands increases to 76-97.8 %. Aims: this research is devoted to the study of the prevalence and structure of breast pathology in women with chronic endometritis and reproductive disorders. Materials and methods. The study included patients with reproductive disorders: 50 patients with histological verification of the diagnosis of chronic endometritis and 50 patients without chronic endometritis. Hormonal examination was carried out using radioimmunoassay and immunoenzymatic methods. Methods of mathematical statistics implemented Statistica 6.1 (USA) were used parametric criteria: Student's t-test and Fisher's F-criterion, the nature of the distribution was estimated using the methods of Kolmogorov - Smirnov with a significance level smaller and equal to 5 % (p < 0.05). To confirm the fact that the observed differences between the products of A x D and B x C are not random, we used the х2 criterion. Results and conclusions. In women with chronic endometritis, the pathology of the mammary glands is characterized by the presence of ectasia of the ducts of the mammary gland; clinically, it is manifested by lactorrhea. Benign dysplasia of mammary glands is accompanied by the presence of hyperestrogenia, hypoprogesteronemia, which is typical for ovarian dysfunction. It is possible to use a pathogenetically reasonable combination correction: anti-inflammatory therapy and the use of gestagens in women with reproductive disorders and chronic endometritis.

В статье приводятся результаты хирургического лечения 88 пациенток с асимметрией молочных желез разной степени выраженности: c истинными 53, смешанными 28, ложными 7. Выбор хирургической тактики основывался на оценке вариантов асимметричности молочных желез и определении ориентированной молочной железы. В данной статье проведена сравнительная характеристика методов лечения мастита коров с применением антибиотиков и препаратов, содержащих витамины, ферменты, гормоны в комплексе с нетрадиционной медициной. Основной целью научно-исследовательской работы является сравнение методов лечения мастита коров антибиотикосодержащими препаратами и препаратами содержащими витамины, гормоны, ферменты в комплексе с нетрадиционной терапией. Выявить наилучший метод, который бы не уступал по лечебной и профилактической эффективности имеющимся противомаститным препаратам. В качестве нетрадиционной медицины был использован метод АПИ терапии, который разработан на кафедре анатомии и физиологии животных института биотехнологии и ветеринарной медицины. Для чего были сформированных группы животных, каждая из которых получала лечение соответствующим препаратом. В результате оценки состояния животных в условиях ФГУП ПЗ «Тополя» Тюменской области было установлено, что продолжительность антибиотикотерапии по срокам лечения, короче, но имеет ряд недостатков, основной из которых является ограничение на продукты животноводства. Производственные опыты проводили в ФГУП ПЗ «Тополя» Тюменского района Тюменской области в период 2014–2017 гг. Хозяйство специализируется на молочном скотоводстве. Порода животных черно-пестрая. Содержание животных, пастбищно-стойловое. Диагноз ставили на основании анамнеза, клинической картины и исследования секрета вымени. При исследовании молочной железы обращали внимание на величину и консистенцию отдельных четвертей, их болезненность, состояние сосков, местную температуру, состояние кожи вымени и надвымянных лимфатических узлов, характер секрета. Диагностику мастита у коров проводили отбором проб молока для визуальной оценки и постановки экспресс – реакции с применением Кенотеста и молочной (тестовой) пластины. Для изучения терапевтической эффективности различных методов лечения коров с клиническими формами мастита по принципу аналогов были сформированы 2 группы животных по 15 голов в каждой. В качестве лечения для первой группы были выбраны различные препараты, в основе которых лежат антибиотики. Для наиболее полной оценки действия используемых препаратов данная группа была разделена на 3 подгруппы, каждая из которых получала лечение соответствующими препаратами. Вторая группа так же была разделена на 3 подгруппы, лечение проводилось с помощью специфических препаратов, в состав которых входят витамины, гормоны, ферменты, кроме того использовался метод АПИ терапии по разработанной нами схеме. На пораженную долю, в область биологически активных точек, подсаживалась пчела, для пчелоужаления. Количество пчел увеличивалось на одну каждый последующий день лечения. Анализируя статистические данные по результатам исследования коров на маститы в Тюменском районе Тюменской области за 2016 год в приближенных к г. Тюмени хозяйствах было установлено, что больных коров с субклинической формой мастита на 90% больше, чем с клинической формой. Результаты всех исследований были оформлены в таблицы 1–4. На основе проведенных исследований рекомендуется метод лечения мастита коров «Мастизим + АПИ терапия». Применение препаратов на основе антибиотиков имеет место быть только при лечении гнойно – катарального и геморрагического мастита, а также при хроническом течении болезни., A comparative description of methods for treating mastitis in cows with the use of antibiotics and preparations containing vitamins, enzymes, hormones in combination with alternative medicine has been carried out. As an alternative medicine was used the method of APN therapy, which was developed at the Department of Anatomy and Animal Physiology in the Institute of Biotechnology and Veterinary Medicine. There were formed groups of animals, each of which received treatment with an appropriate preparation. As a result of assessing the treatment of animals in the animal plant “Topol”, Tyumen region, it was found that the duration of antibiotic therapy is shorter, but it has several disadvantages, the main of which is the restriction on livestock products. Based on our research, we recommend the method of treating mastitis in cows with “Mastizim + APN therapy”. Целью научного исследования явилось проведение сравнительной гигиенической оценки уровня канцерогенного риска здоровью населения, проживающего на территориях с различным уровнем заболеваемости раком молочной железы. В работе проанализированы многолетние данные о структуре заболеваемости, многолетняя динамика онкологической заболеваемости, концентрации канцерогенных веществ в атмосферном воздухе и питьевой воде по данным регионального информационного фонда социальногигиенического мониторинга, Территориального органа Федеральной службы государственной статистики. Установлены территории наблюдения, с самым высоким уровнем заболеваемости злокачественными новообразованиями молочной железы, а также территория сравнения, где уровень заболеваемости в 1,7 раз ниже. Рассчитанные концентрации веществ в атмосферном воздухе и питьевой воде соответствуют гигиеническим требованиям. Оценка канцерогенного риска показала, что на территории наблюдения суммарный канцерогенный риск находится на неприемлемом уровне как от веществ, содержащихся в питьевой воде, так и веществ, содержащихся в атмосферном воздухе, при этом суммарный канцерогенный риск на территории наблюдения выше в 10 раз. Корреляционной анализ установил достоверную связь слабой силы с канцерогенными веществами, содержащимися в питьевой воде и атмосферном воздухе: хром, мышьяк, кадмий, которые могут выступать в роли индикативного показателя по установлению причинноследственной связи с уровнем онкологической заболеваемости молочной железы. Исследование подтверждает необходимость разработки плана профилактических мероприятий по снижению уровня заболеваемости раком молочной железы с учетом существующей канцерогенной нагрузки от факторов окружающей среды., The purpose of the research was to conduct a comparative hygienic assessment of the level of carcinogenic risk to the health of the population living in areas with different levels of the incidence of breast cancer. The paper analyzes longterm data on the structure of morbidity, the longterm dynamics of cancer incidence, the concentration of carcinogenic substances in the atmospheric air and drinking water according to the regional information fund of social and hygienic monitoring, the territorial body of the Federal State Statistics Service. Areas of observation have been established, with the highest incidence rate of malignant tumors of the mammary gland, as well as the territory of comparison, where the incidence rate is 1.7 times lower. The calculated concentrations of substances in the air and drinking water meet hygienic requirements. The assessment of carcinogenic risk showed that in the observation area, the total carcinogenic risk is at an unacceptable level both from substances contained in drinking water and substances contained in the atmospheric air, while the total carcinogenic risk in the observation area is 10 times higher. Correlation analysis established a significant correlation of weak force with carcinogenic substances contained in drinking water and atmospheric air: chromium, arsenic, cadmium, which can act as an indicative indicator for establishing a causal relationship with the incidence of breast cancer. The study confirms the need to develop a plan of preventive measures to reduce the incidence of breast cancer, taking into account the existing carcinogenic load from environmental factors., №4(172) (2019)

Introduction. Proliferative benign dysplasia of mammary glands (MG) is one of pretumoral processes. Surgical intervention constitutes the main procedure for the benign dysplasia of MG treatment. Miniinvasive surgical technologies are in-demand. Маterials and methods. Three-year experience of miniinvasive surgical treatment for the MG ductal papillomatosis was analyzed. Comparative analysis for the proliferation and dysplasia degree, as well as for the alpha-receptors expression (EsRα), was conducted. Results. Efficacy of the proposed method for the MG ductal papillomatosis treatment was established. Dependence between degree of the MG ductal proliferation and the EsRα expression was proved. Discussion. Universal method of surgical treatment for the MG ductal papillomatosis is lacking now. The modern immunohistochemical methods application and the intervention procedures individualization promote the treatment results improvement. Conclusion. Miniinvasive methods of surgical treatment guarantee a radical treatment of ductal papillomatosis of MG as well as esthetic satisfaction of women-patients and a physician. Results of treatment of 74 patients, suffering mammary gland cancer, after a one-side radical mastectomy were adduced. In 38 women-patients (the comparison group) the intervention was performed in accordance to classic procedure, in 36 (the main group) – a zigzag-like access was applied. In women-patients of the main group the postoperative period course is more favorable, than in the comparison group: the last drain was removed by 5.9 days earlier, duration of the parenteral analgetic therapy - by 5.3 days esser, the partial insufficiency of sutures -by 10.4% lesser, and the seroma occurrence - by 7.6% lesser; the operative wound suppuration was absent.

Опыт проведен методом латинского квадрата на трех группах коз по три головы в каждой, находившихся к началу опыта на 2-м месяце лактации. Продолжительность каждого из периодов опыта составляла три недели. В исходном периоде козы получали основной рацион (ОР) с рекомендуемым уровнем доступного протеина (ДП) и обменной энергии. В опытных периодах к основному рациону добавляли соевый шрот в количестве 150 г, соответствующем 10-му приросту доступного сырого протеина, а также 90 г ацетата натрия 3-х водного и 36 г пропионата натрия на голову в сутки, что соответствовало 3 от обменной энергии рациона. В конце каждого опытного периода брали пробы молока и крови из яремной и молочной вен перед утренним кормлением и через 3 и 7 ч после кормления. Плазмоток через вымя определяли расчетным путем по соотношению выхода -аминоазота с белком молока и артериовенозной разности (АВР) концентрации аминоазота по молочной железе. Активность транспорта аминокислот в секреторные клетки ( Т , л/час), оценивалась в единицах клиренса по формуле: TQE /(1- E ), , где Q - объёмная скорость кровотока (плазмотока) через молочную железу, мл/час и E - эффективность извлечения из крови E 100( c - c )/ c c и c - концентрация субстрата в плазме крови яремной и молочной вен соответственно. При скармливании добавок СШ, США и СШП молочная продуктивность повысилась соответственно на 8,2, 14,7 (P0.05), 13,3 (P0.05) и выход белка с молоком соответственно на 9,5, 18,9 (P0.05), 16,1 (P0.05) по сравнению с ОР. Добавка ацетата или пропионата к высокопротеиновому рациону снижала уровень мочевины в молоке (P0.05), что свидетельствует о более эффективном использовании аминокислот на продуктивные цели. Включение в рацион протеиновой добавки увеличило плазмоток в молочной железе. Скармливание протеиновой добавки совместно с ацетатом или пропионатом натрия оказало дополнительное стимулирующее влияние на плазмоток. Активность транспорта аминокислот в секреторные клетки при включении в рацион кормовых добавок была выше, чем на основном рационе. Заключили, что выявленный продуктивный эффект был в существенной мере связан с положительными сдвигами в показателях органного кровотока и формировании субстратного баланса в молочной железе., The experiment was carried out using the Latin square method on three groups of goats, 3 goats each, which were at the beginning of experiment at the 2nd month of lactation. The duration of each periods of experience was three weeks. In the initial period, goats were fed the basic ration (BR) with the recommended level of available protein (DP) and metabolizable energy. In the experimental periods, soybean meal (SBM) was added to the basic ration at an amount of 150 g, corresponding to a 10 increase in available crude protein, and also 90 g of sodium acetate (SA) and 36 g of sodium propionate (SP) per goat per day, which corresponded to 3 of the diet metabolizable energy. At the end of each experimental period, samples of milk and blood were taken from the jugular and milk veins before the morning feeding and 3 and 7 hours after the feeding. Plasma volume flow through the udder was determined by calculating the ratio of the -amino nitrogen output with milk and arteriovenous difference (ABD) concentration of amino nitrogen in the mammary gland. The activity of transport of amino acids to secretory cells ( T , l/h), was estimated in units of clearance ( T Q E /(1- E ), where Q is plasma flow rate, ml/min, and E is efficiency of extraction from the blood, E 100( c - c )/ c c and c are substrate concentration in the blood plasma of the jugular and dairy veins, respectively. When feeding supplements SBM, SBMSA and SBMSP, milk productivity increased by 8.2, 14.7 (P0.05), 13.3 (P0.05) and the yield of milk protein by 9, 5, 18.9 (P0.05), 16.1 (P0.05) respectively compared to BR. The addition of acetate or propionate salts to a high-protein diet reduced the level of urea in milk (P0.05), which indicates a more efficient use of amino acids for productive purposes. The inclusion of protein supplements in the diet increased plasma volume flow rate in the mammary gland. The feeding of protein supplements together with sodium acetate or sodium propionate had an additional stimulating effect on the plasma flow. When included of feed additives in the diet, the activity of amino acids transport to secretory cells was higher compared to BR. Concluded that the identified productive effect was significantly associated with positive changes in the indicators of organ blood flow and the formation of substrate balance in the mammary gland., №4 (2018)

Работа посвящена решению актуальной научно-технической задачи – повышению качества визуализации анатомических и патологических структур на слабоконтрастных полутоновых изображениях молочной железы за счет разработки новых методов и подсистемы визуализации маммографических снимков, что позволит обеспечить поддержку врача-рентгенолога при принятии решений в медицинской практике. Анализ существующих систем обработки и анализа маммографических изображений выявил их недостатки, что позволило сформулировать цель и задачи исследования. На основании аналитического анализа методов, используемых при цифровой обработке изображений, разработаны математическая модель изображения анатомических и патологических структур молочной железы на цифровой маммограмме, которая основана на декомпозиции исходного изображения на составляющие, и на ее основе декомпозиционный метод повышения качества визуализации цифровых рентгенологических снимков молочной железы. Выполнена оптимизация параметров метода повышения качества визуализации маммографических снимков IMRI-MAM. Сформулированы требования к функциональным возможностям системы поддержки принятия решений в маммологии и представлена ее структурная схема. Разработано программное обеспечение для визуализации маммографических изображений. С использованием реляционной системы управления базы данных SQLite выполнено проектирование базы данных для хранения маммографической информации. Решена проблема подключения разработанного программного модуля IMRI-MAM к отечественному цифровому рентгенологическому маммографическому комплексу SYMA с использованием библиотеки динамической компоновки DLL. Выполнена комплексная проверка разработанной подсистемы на реальных медицинских данных, которая подтвердила эффективность ее работы. This work is dedicated to solving important scientific and technical tasks to improve the quality of visualization of anatomical and pathological structures on low-contrast gray-scale images of mammary glands (MG) using new method and a subsystem for visualizing mammographic images. A mathematical model of the MG image in a mammogram based on the original image decomposition into components was developed. The model further served as a foundation for the development of a decomposition method to improve the image quality of the MG. Next, the parameters of the method for improving the quality of IMRI-MAM mammogram visualization were optimized. As a result of this work, a new method of multi-criteria image quality assessment method, based on subjective and objective mammographic image characteristics, was developed. The results of processing digital mammograms with the IMRI-MAM method are presented. The structure of the decision making support system to be used in mammalogy is presented. A software to be used to visualize mammographic images was developed. The subsystems were verified on real medical data.

A review of the literature on diagnostic systems for the detection of breast cancer by optical tomography was presented. The actuality of the method of transmission of optical tomography and its advantages over existing methods of medical diagnosis of cancer have been substanti- ated. We have analyzed tomographic systems used for the diagnosis of breast cancer. The basic advantages and disadvantages of tomograph- ic systems using various types of radiation have been indicated. The results of review can be used in the development of technique for optical transmission tomography.

Исследована активность катепсина Н в дольково-инфильтративной опухоли и в пограничной с нею ткани молочной железы женщин. В опухоли молочной железы установлено повышение содержания белка, по сравнению с неизмененной тканью молочной железы. Прогрессия опухоли сопровождается снижением активности катепсина Н в опухолевой ткани. Максимальная его активность установлена на І–ІІ стадии развития опухоли., Досліджено активність катепсину Н в дольково-протоковій пухлині та в суміжній пухлині тканині молочної залози жінок. В пухлині молочної залози встановлено підвищення вмісту білка, порівняно з незміненою тканиною молочної залози. Прогресія пухлини супроводжується зниженням активності катепсину Н в пухлинній тканині. Максимальна його активність встановлена на І–ІІ стадії розвитку пухлини., Breast cancer takes the first place in the structure of oncologic morbidity of female population of the planet. Potential targets in elaboration of drugs are cysteine proteinases taking part in many processes, associated with cancer progression: Hyperproliferation of tumor cells, apoptosis, tumor-induced angiogenesis, invasion and matastasis.Cathepsin H cysteine proteinase possesses both proteinase and exopeptidase activity. High cathepsin H activity has been determined in the following tumors: glioblastoma, anaplastic astrocytoma, colorectal carcinoma, in head and neck tumors in blood plasma of patients with lung cancer, melanoma.In this connection the aim of the work consisted in analysis of cathepsin H activity in the tissue of invasive lobular form of breast cancer in order to elaborate additional biochemical test for presence of a neoplasm.Samples of mammary gland without neoplasms (circumferential tissue) and samples with invasive lobular form of breast cancer at I-II stages of cancer development were analysed. The patients were informed and gave the written agreement for the use of the anatomical material for biochemical research. In the supernatant the content of protein was determined by biuret method and cathepsin Н activity was determined by the modified Bradshaw method. Cathepsin activity was expressed in mmole of leucine, formed in the process of oxytocin hydrolysis, per a mg of protein per a min at 37 °С.It was found that in case of invasive lobular form of BC, the content of protein in tumour tissue at all stages of its development was 1.6-2.6 higher than the content of protein in the circumferential tissue. It was shown that cathepsin Н activity changed depending on the stage of invasive lobular BC development. The maximal relative and specific activity of cathepsin Н in tumour tissue was determined at I-II stage of tumor development. В последние годы отмечен возрастающий интерес к дисгормональным заболеваниям молочных желез, в первую очередь, диффузной фиброзно-кистозной болезни (ФКБ), частота которой составляет 36-50% в общей популяции, а среди женщин репродуктивного возраста, страдающих различными гинекологическими заболеваниями, достигает 67-95%. По данным некоторых авторов риск развития рака молочной железы (РМЖ) при непролиферативной форме ФКБ превышает популяционный в 1,27 раза, при умеренной пролиферации — в 1,88 раза, а при атипической пролиферации, относимой рядом авторов к предраковому состоянию, — в 4,24 раза. Прослеживается тенденция роста дисгормональных заболеваний молочных желез, которые диагностируются у каждой 4-й женщины в возрасте до 30 лет. Особую актуальность эта проблема приобретает в связи с тем, что пролиферативные формы мастопатии рассматриваются как маркеры риска развития рака молочных желез (РМЖ), который в 3-5 раз чаще возникает при диффузных изменениях паренхимы молочных желез и в 30-40 раз при наличии в ней узловых образований. Внедрение в практику онкологии реконструктивно-пластических операций на молочной железе (МЖ) привело к необходимости разработки методов динамического наблюдения за этими больными после лечения. Предложена методика выполнения маммографии после реконструктивно-пластических операций и операций с использованием силиконовых эндопротезов. Для послеоперационного обследования молочной железы разработаны критерии использования различной мощности маммографического аппарата при разных видах реконструктивных операций. Динамическое рентгеномаммографическое исследование проводили 167 оперированным пациенткам на протяжении 8 летПредложенная методика позволяет достоверно оценивать результаты реконструктивно-пластических операций и прогнозировать появление возможных осложнений., Introduction of reconstructive-plastic operationts in practice of breast cancer surgical treatment have led to the necessity of dynamic monitoring methods development in patients after such treatment. We have proposed technique of mammography after reconstructive-plastic operations and operations with the use of silicone implants. Вопрос своевременного выявления заболеваний молочной железы сохраняет значение в течение многих десятилетий. В связи с этим поиск методов исследования, не оказывающих повреждающего действия на организм молодых женщин, привел к разработке метода микроволновой радиотермометрии (РТМ), основанного на измерении термоасимметрии поверхностных и глубоких отделов молочной железы. Для определения диагностических границ метода были обследованы 619 женщин в возрасте от 30 до 65 лет и проведено сравнение метода РТМ с онкоэпидемиологическим тестированием (методом прогноза) и ультразвуковым исследованием молочной железы, получившим распространение в клинической практике.Выявление термоасимметрии Th 5 при РМЖ у женщин молодого возраста позволяет использовать этот бездозовый метод для скрининга группы риска.Сравнение информативности РТМ и дуплексного сканирования показало преимущества РТМ в выявлении начальных проявлений инфильтративного роста и патологической пролиферации, повышая возможность распознавания зоны риска в 16% при отсутствии кровотока при дуплексном сканировании и в 29% при наличии единичного питающего сосуда., The question about revealing of breast diseases keeps value within many decades. In this connection search of investigation methods which have no damaging action on an organism of young women, has led to development of a radiothermometry microwave method (RTM), based on temperature asymmetry of superficial and deep areas of breast. For definition of diagnostic borders of this method were investigated 619 women aged 30 till 65 years were investigated, including comparison of RTM method with oncoepidemiological testing (a forecast method) and ultrasound (US) diagnostics, known to be wide spread in clinical practice.Revealing Th 5 temperature asymmetry at young women with breast cancer allows to use this method for screening in risk group.
