- Abstract
Transplantation is the most efficient alternative to treat terminal functional kidney failure. To synthesize the literature on the topic, this systematic review sought to retrieve evidence that empirically analyzed the frequency of kidney transplantation in Brazil, relating it to the COVID-19 pandemic. The PUBMED, LILACS and SCIELO databases were consulted using the descriptors: "Transplantation", "Kidney", "COVID-19", and "Brazil". Thus, 146 studies were retrieved and only six were included in the review. The review showed that the COVID-19 pandemic had a negative impact on the number of kidney transplants, increased the transplant waiting list, reduced the number of donors, and there were reports of infections and deaths among patients undergoing kidney treatment and transplant recipients during the pandemic. These findings emphasize the need for more research and attention in the area of kidney transplantation and mitigate the effect of health crises similares to COVID-19. [ABSTRACT FROM AUTHOR]

Pregnant women considered at risk for coronavirus disease (COVID-19), the virus can cause some pregnancy complications such as miscarriages, intrauterine growth restrictions and even lead to a premature birth. The objective of this study was to characterize the newborns of mothers with a hypothesis or confirmed case of COVID 19 who were admitted to a neonatal isolation unit. The research was carried out through the analysis of the records of neonates, of both genders, admitted to the COVID unit between 2020 and 2021, regardless of their length of stay in the unit and their clinical situation. Most mothers were from Fortaleza, gestational age ranged between 28 and 41 weeks, the presence of neonatal infection was not relevant compared to maternal swab and cesarean delivery was the most practiced. Regarding the possible consequences generated for the newborn due to maternal COVID 19, we can conclude that COVID 19 does not have relevant repercussions for the newborns in the sample, as well as we cannot identify the presence of vertical transmission from mother to baby within the sample. Covid-19 is an infectious disease that is part of a group of viruses responsible for causing severe acute respiratory syndromes, the Sars-Cov-2. To describe the persistence of symptoms in patients diagnosed with Sars-Cov-2 infection, the term Long-Covid has been used. Respiratory physiotherapy plays an important role in the face of persistent symptoms of Covid-19 in order to restore functionality impaired or lost by the pathology. The objective of the current research is to collect data from medical records and assemble a profile of patients with Long-Covid who sought physical therapy treatment. This is a research analysis of medical records with a diagnosis of Long-Covid who sought the sector to perform treatment between January/2021 and May/2022. A total of 55 medical records were collected and 11 were excluded for not meeting the inclusion criteria, including 20 males with an average age of 57 years and 24 females with an average age of 56 years. The main symptoms found in the research were tiredness, dyspnea and muscle pain. A population profile was found where the greatest demand for physiotherapeutic treatment was female, hypertensive individuals, who use continuous medication, and do not practice physical activities. Even most of the study population underwent hospitalizations, either in the ward or ICU, whose most persistent symptoms were fatigue, dyspnea and muscle pain, with mean blood pressure below expectations, a 6-minute walk test below ideal and average vital signs within the normal range. Noninvasive mechanical ventilation (NIV) may reduce postoperative complications of obesity--associated restrictive pulmonary syndrome. We evaluated the effects of NIV use after extubation in patients undergoing bariatric surgery on acute changes in lung function. A randomized clinical trial was performed with patients in the immediate postoperative period of bariatric surgery. The intervention group used NIV for 1 hour after arrival at the post-anesthesia recovery room, the control group received standard care. Pulmonary function assessment was performed preoperatively, immediately after surgery and 1 hour after arrival in the recovery room. Statistical analysis was performed using the Generalized Estimation Equations test and Bonferroni's post-hoc test. The significance level adopted was 0.05. A total of 46 individuals were evaluated, of which 31 were able to perform spirometry in the pre and immediate postoperative period. The sample consisted of 25 women, mean age of 42.55 ± 10.39 years and mean Body Mass Index of 50.82 ± 10.20. Preoperative spirometry showed that most patients had mild restrictive disorders (46.7% versus 43.8%) or no respiratory disorder (40% versus 31.3%). In the post-extubation evaluation, most patients had severe restrictive disorder (66.7% versus 53.8%), after one hour in the recovery room, 64.3% versus 60% of patients had severe restriction and 21.4% versus 20% severe obstruction. It was concluded that the use of prophylactic NIV did not improve pulmonary function in the immediate postoperative period. [ABSTRACT FROM AUTHOR]

Objective: To analyze the presence of acute kidney injury (AKI) in hospitalized older patients with COVID-19 and its association with hospital mortality. Methods: This was a retrospective observational case-control study of patients over 60 years of age hospitalized from April 01, 2020, to April 30, 2021, at a ward or intensive care unit (ICU) dedicated to COVID-19. The severity of AKI was stratified according to the Kidney Disease: Improving Global Outcomes (KDIGO) criteria. Binary logistic regression was used to determine the association of mortality in models with and without adjustment for previous comorbidities as risk factors. Results: Of 897 patients aged 19–107 years hospitalized with COVID-19, 398 aged ≥ 60 years were included. AKI was observed in 220 patients (55.27%), with stages 1, 2, and 3 in 25.87%, 5.52%, and 23.86%, respectively. Dialysis was required in 73 patients with AKI (33.18%), most of them were at stage 3 (91.89%). Fifty-four patients on dialysis died (73.97%). Increased AKI severity was associated with mortality even after removing the influence of associated risk factors (odds ratios 1.78, 2.35, and 3.51 at stages 1, 2, and 3, respectively). Conclusion: AKI was highly common in patients aged 60 years or older hospitalized with COVID-19, and its severity showed a progressive association with hospital mortality.

This document highlights that the Peruvian State took advantage of the pandemic as an opportunity to infringe upon the rights of indigenous peoples by accelerating the prior consultation processes in the mining sector, thus violating rights recognized in its own legislation and the jurisprudence of the Inter-American Court. The analysis of consultation processes (2020-2022) evidenced (I) lack of accessibility, as the State used legal Spanish in all process documents, despite the fact that in 77.7% of the communities, over 80% of the population had an indigenous language as their mother tongue; (II) lack of good faith, as 88% of the cases had stages shortened and the intercultural dialogue stage excluded; and (III) the failure to aim at reaching an agreement, using less time and recording only a matrix of agreements from one community.

ABSTRACT Objective: To assess whether cardiovascular risk factors (CVRF) are correlated with the severity of COVID-19 in hospitalized patients in our population. Material and Methods: Retrospective cohort study of patients with COVID-19 admitted between the months of April and December 2020 in a tertiary center in southern Spain. The association between CVRF and the severity of SARS-COV2 was assessed. Severe COVID was considered to be one who was admitted to the Intensive Care Unit (ICU) or who died. Results: 608 patients admitted with a diagnosis of COVID-19 pneumonia were analyzed, the median age was 73 years (IQR: 64-81) years. The patients were predominantly male (55.8%). Hypertension was the most common CVRF (83.7%), followed by dyslipidemia (50.2%), diabetes mellitus (43.1%), obesity (32.9%), and smoking (15.6%). During the stay, a total of 25.8% of the patients had an unfavorable prognosis: 22.9% died and 7.9% required admission in the ICU. In the multivariate analysis, significant independent variables were found for suffering from severe COVID: male sex (OR: 1,697; IC 95%: 1,138-2,532), age (OR: 1.697; CI 95%: 1,138-2,532), and obesity (OR: 1,634; 95% CI: 1,370-1,947). Conclusions: There is a high prevalence of CVRF in patients hospitalized for COVID 19 in our population. Obesity, along with older age and male gender, are associated with greater severity and hospital mortality in patients admitted with SARS-CoV-2

Introduction and Objectives: Digital health (DH) is a broad concept, bringing together technology and healthcare, that is playing an increasingly important role in the daily routine of healthcare professionals (HCPs) and promises to contribute to the prevention and treatment of cardiovascular disease. There are no solid data on the position of Portuguese HCPs toward the implementation of DH in cardiovascular medicine. This national cross-sectional study aims to provide a snapshot of DH implementation in Portuguese cardiovascular HCP routines and to identify both expectations and barriers to its adoption. Methods: An 18-question survey was created specifically for this study and distributed to 1174 individuals on the Portuguese Society of Cardiology mailing list. Results: We collected 117 valid responses (response rate 10%). Almost all participants had smartphones and laptops, and two-thirds had tablets. Electronic medical information systems were the most used DH tool (84% of respondents) and were considered the most important for improving cardiovascular care. Implantable technologies (sensors and devices), telemedicine and social media were used by more than two out of three respondents and considered «very important» or «extremely important» by most of them. Most participants showed positive expectations regarding the impact of DH in cardiovascular medicine: 78% agreed that DH could improve health outcomes, 64% that it promotes health literacy and 63% that it could decrease healthcare costs. The top-rated barriers were patients’ inability to use smartphones, limited access to electronic devices, and lack of legal regulation of DH. Conclusion: Most Portuguese cardiovascular HCPs had at least three electronic devices (mainly smartphones, laptops and tablets) and showed positive expectations regarding DH's current and future impact on cardiovascular medicine. Patient DH literacy, technology adoption, and DH regulation were identified as the most important barriers to increasing the adoption of DH tools in cardiovascular medicine.

Objective. To assess changes in reproductive, maternal, newborn, child, and adolescent health (RMNCAH) in Haiti from August 2018 to September 2021, before and during the COVID-19 pandemic. Methods. A retrospective study using surveillance data from the Haitian Unique Health Information System, examining two periods: pre- and peri-COVID-19 pandemic. Health indicators at the national level in the two periods were compared using two-sample t-tests for proportions, and average absolute monthly changes were calculated using variance-weighted regression. Results. There was a statistically significant decline in the proportion of most of the indicators assessed from the pre- to the peri-COVID-19 pandemic period. However, the most affected indicators were the proportions of pregnant women with four antenatal care visits, with five antenatal care visits or more, and those who received a second dose of tetanus vaccine, which decreased by over 4 percentage points during the two periods. Likewise, the proportions of children who received diphtheria, tetanus, and pertussis (DTaP), BCG, polio, pentavalent, and rotavirus vaccines also all declined by over 8 percentage points. In contrast, pneumococcal conjugate vaccine increased by over 4 percentage points. A statistically significant decrease was also observed in the average absolute monthly changes of several reproductive and child health indicators assessed. Conclusions. The COVID-19 pandemic may have contributed to the decline observed in several RMNCAH indicators in Haiti. However, the role played by the sociopolitical crisis and control exercised by armed groups over the population in the last three years cannot be ruled out.

Between 2020 and 2023, a period of time assailed by the COVID-19 pandemic, society – as a worldwide social system –experienced a novel kind of communication. The tendency for this communication to exponentially expand is due to the global numbers of people dead, dying or infected. Pandemic communication has arisen in the present to perpetuate itself in all ambiences for every social system around the globe. The health system, in turn, basing itself on the health/disease code, has reacted according to its own logic in the coordination of the actions of doctors. However, the true/false nature of communication arising from questions related to COVID-19 has taken a long time to be tested by the science system. Vaccines and possible treatments for the disease have created issues for political, economic, moral, religious and media matters. The continually expanding trend in pandemic communication has proven that it can hinder the functional differentiation of social systems. Legal communication has not been able to avoid all of the controversy. In the specific case of Law, the problem entails both the judicialization of health and the reverse (where Health becomes Law). Based on the Social Systems Theory Applied to Law, the paper argues that the maintenance of the functional differentiation of the Law system becomes necessary so as for Health not to become Law (and vice versa).
