With an increasing demand for gyms, especially during summer, the risk of contamination rises despite preventive care. In many sports, exercising must require attention to hygiene, mainly due to great chances of contracting diseases. Staphylococcus aureus has widely spread caused by its ability to express various virulence factors, which can cause diseases ranging from simple to most severe infections as a pathogen. This paper investigates the incidence of Staphylococcus aureus in equipment from a gym of physical activities in Alfenas-MG and identifies the appropriate level about the importance of cleaning equipment. In this study, three collections were carried out per equipment in different places, at three times; totaled 54 samples. Staphylococcus aureus was identified by sowing in Mannitol Salt Agar, medium coloration and colonial morphology, catalase, and coagulase tests. Twenty-nine samples obtained growth from the sowings, being positive for gram staining, catalase, and coagulase. In samples with micro-organisms (53.70%) and correspondents (50%), gym-goers did not clean the equipment, and the others (50%) only used the individual towel. Therefore, the measures that can decrease the incidence of micro-organisms, particularly Staphylococcus aureus, are disinfecting with 70% alcohol and disposable paper before and after manipulating equipment in a gym. Con mayor demanda, en determinada época del año; en verano, el riesgo de contaminación en los gimnasios aumenta incluso si la atención preventiva es simple. Como en muchos deportes, la práctica requiere mucha atención a la higiene, ya que las posibilidades de contraer enfermedades son grandes. El éxito de Staphylococcus aureus como patógeno se debe, en parte, a su capacidad para expresar una variedad de factores de virulencia, que pueden causar enfermedades que van desde una simple infección hasta las más graves. Con el objetivo principal de investigar el índice de Staphylococcus aureus en equipos de gimnasia de actividad física en Alfenas-MG e identificar el grado de conocimiento sobre la importancia de la limpieza de equipos, en este estudio se realizaron 3 recolecciones por equipos en diferentes ubicaciones, en 3 diferentes veces; totalizaron 54 muestras. Staphylococcus aureus se identificó mediante la siembra en Manitol Salt Agar, coloración media y morfología colonial, pruebas de catalasa y coagulasa. De las siembras, 29 muestras obtuvieron crecimiento, siendo positivas para tinción de Gram, catalasa y coagulasa. En muestras con microorganismos (53,70%) y encuestados (50%), asistentes al gimnasio, no realizan limpieza de equipos y los demás (50%) solo utilizan la toalla individual. Así, las medidas que pueden reducir la incidencia de microorganismos, especialmente Staphylococcus aureus, es el uso de alcohol al 70% y papel desechable antes y después de utilizar el equipo en el gimnasio. O sucesso de Staphylococcus aureus como um agente patogênico é, em parte, devido à sua capacidade de expressar uma variedade de fatores de virulência, podendo provocar doenças que vão desde uma infecção simples, até as mais graves. Tendo por objetivo central investigar o índice de Staphylococcus aureus em aparelhos de academia de atividades físicas de Alfenas-MG e identificar o grau de conhecimento sobre a importância da higienização dos equipamentos, no presente estudo, realizou-se 3 coletas por equipamento em diferentes locais, em 3 momentos distintos; totalizadas 54 amostras. A identificação de Staphylococcus aureus, realizou-se por semeadura em Ágar Sal Manitol, coloração do meio e morfologia colonial, provas de catalase e coagulase. Das semeaduras, 29 amostras obtiveram crescimento, sendo positivas para coloração de gram, catalase e coagulase. Nas amostras com micro-organismos (53,70%) e os entrevistados (50%), frequentadores da academia, não realizam higienização nos equipamentos e os demais (50%) só utilizam a toalha individual. Sendo assim, as medidas as quais podem diminuir a incidência de micro-organismos, em especial Staphylococcus aureus, é o uso de álcool 70% e papel descartável antes e após utilização do equipamento na academia.

Objective: This systematic review was performed to evaluate the efficacy of glass carbomer when compared with other sealant materials in preventing carious lesions in children and retention in pit and fissures. Sources of data: The paper included only randomized clinical trials that compared pit and fissure sealants with glass carbomer and other sealant materials in children’s permanent molars with at least six-monthfollow-up. A systematic search was performed in PubMed, Scopus, Web of Science, LILACS, BBO, Cochrane Library and Grey literature (December 2020/January 2021). The risk of bias tool from the Cochrane Collaboration was used for quality assessment of the studies and GRADE approach for the quality of the evidence. Meta-analysis was performed on studies from which data could be achieved. Synthesis of data: A total of 1685 papers were identified, 54 were selected for review. From these, 40 articles were excluded after the reading of the abstract and 14 articles were put aside for assessment. Eight papers were included in qualitative and quantitative synthesis. The prevalence of caries-free pit and fissures did not show differences after six (p=0.77; I2= 0%) or 12 months (p=0.60; I2= 0%) and the quality of the evidence was judged as low; after 24 months, other sealant materials performed better (p=0.30; I2=7%) and the quality as moderate. Resumo Objetivou-se descrever as principais complicações clínicas em gestantes brasileiras relatadas na literatura, identificando os fatores de exposição mais relevantes e seus desfechos na saúde materno-infantil. Foi realizada uma revisão sistemática de artigos publicados entre 2010 e 2018. Foram incluídos três revisões e 36 artigos empíricos; transtornos mentais (n=9), doenças infecciosas (n=9) e morbidade materna grave (n=8) foram as principais complicações clínicas na gravidez. Ainda, quatro artigos enfocaram a determinação das principais afecções ou causas de internação, citando-se com maiores frequências a Infecção do Trato Urinário, a anemia e as doenças hipertensivas. Idade materna nos extremos reprodutivos, menor escolaridade, vulnerabilidade socioeconômica, características raciais e uso de serviços públicos de saúde foram os fatores de exposição mais relevantes. Resultados de saúde desfavoráveis associados a complicações clínicas na gravidez foram verificados nos oito artigos com esse perfil. A frequência de gestantes com complicações clínicas potenciais de ameaça à vida é elevada no país, evidenciando a importância da cobertura da atenção básica e a implantação de intervenções para sua redução e prevenção de desfechos maternos e infantis adversos. Abstract The scope of this paper was to describe the main clinical complications among pregnant Brazilian women reported in the literature, identifying the most relevant exposure factors and their outcomes in maternal and child health. This is a systematic review of articles published between 2010 and 2018. Three reviews and 36 empirical articles were included; mental disorders (n=9), infectious diseases (n=9) and severe maternal morbidity (n=8) were the main clinical complications during pregnancy. In addition, four articles focused on determining the main conditions or causes of hospitalization, with a higher frequency of Urinary Tract Infection, anemia and hypertensive diseases. Maternal age at reproductive extremes, lower schooling, socioeconomic vulnerability, racial characteristics and the use of public health services were the most relevant exposure factors. Unfavorable health outcomes associated with clinical complications in pregnancy were verified in the eight articles with this profile. The frequency of pregnant women with potential life-threatening clinical complications is high in the country, highlighting the importance of primary care coverage and the implementation of interventions to reduce and prevent adverse maternal and child outcomes.

Resumo O objetivo deste artigo é apresentar uma análise dos resultados do Programa de Formação em Qualidade em Saúde e Segurança do Paciente (CQSSP) da Escola Nacional de Saúde Pública Sergio Arouca, da Fundação Oswaldo Cruz. Trata-se de uma abordagem quantitativa e qualitativa, transversal e descritiva, baseada no Modelo de Avaliação de Kirkpatrick. Os achados dimensionaram o êxito da proposta pedagógica do CQSSP com avaliação considerada positiva nos níveis de Reação, Aprendizagem, Comportamento e Resultados. A partir de análise da percepção de egressos, observou-se que 82% destes passaram a considerar plenamente a ocorrência de erro na prática do cuidado, 68% a incorporar os conhecimentos adquiridos à sua prática profissional, e 73% passaram a estudar e pesquisar temas relacionados à segurança do paciente. Inquérito captando o número de Núcleos de Segurança do Paciente ou instâncias equivalentes constituídos ou reformulados durante ou depois do CQSSP foi realizado. Avaliou-se, ainda, o grau de contribuição do curso na implementação dos protocolos e ações de Segurança do Paciente (SP) nos hospitais, refletindo, assim, a significativa contribuição do CQSSP para a segurança do paciente e qualidade do cuidado hospitalar. Abstract This paper aims to present an analysis of the Quality in Health and Patient Safety Training Program (CQSSP) results of the Sérgio Arouca National School of Public Health. This is a quantitative, qualitative, cross-sectional, and descriptive study based on the Kirkpatrick’s Evaluation Model. The findings provide us with a clearer picture of the success of the CQSSP pedagogical intent through a positive assessment for the Reaction, Learning, Behavioral, Results levels. From the analysis of the perception of the graduates, we observed that 82% of them started to fully consider the occurrence of an error in the provision of care, 68% incorporated the acquired knowledge into their professional practice, and 73% started to study and research themes related to Patient Safety (PS). We also conducted a survey on the number of Patient Safety Centers or equivalent bodies established or reformulated during or after the CQSSP. We also evaluated the extent of the contribution of the course in the implementation of Patient Safety protocols and actions in hospitals. The findings point to a significant contribution of the CQSSP to PS and the quality of hospital care.

Resumo O artigo tem por objetivo contribuir na identificação das condições e tendências de acesso e utilização dos serviços da atenção primária em saúde (APS) pelos idosos brasileiros nos anos de 2008, 2013 e 2019. Um estudo transversal em painéis com análise descritiva dos percentuais e intervalos de confiança das variáveis elencadas, no qual foram resgatados os dados relativos à população idosa investigada no Suplemento Saúde da Pesquisa Nacional por Amostra de Domicílios (PNAD) do ano 2008 e da Pesquisa Nacional de Saúde (PNS) 2013 e 2019. Inicialmente identificou-se que, apesar da APS ter sido o principal serviço procurado mediante a necessidade de assistência, houve uma tendência de redução para esta procura em todas as regiões brasileiras, principalmente entre as mulheres idosas, na faixa etária de 60-69 anos e na população de raça/cor branca. Verificou-se um aumento de 15,2% no quantitativo de domicílios cadastrados na Unidade de Saúde da Família; 4,5% na procura por lugar, serviço ou profissional de saúde; e de 31,4% na população atendida. Os principais fatores de não atendimento foram não conseguir vaga/senha e não ter médico atendendo. E, dentre os investigados, verificou-se uma tendência de crescimento entre os que tinham o diagnóstico de hipertensão arterial sistêmica, diabetes mellitus e multimorbidades. Abstract This paper aims to identify the conditions and trends in access and use of primary health care (PHC) services by Brazilian older adults in 2008, 2013, and 2019. We performed a cross-sectional panel study with a descriptive analysis of the percentages and confidence intervals of the variables listed, in which data on the elderly population investigated in the Health Supplement of the 2008 National Household Sample Survey (PNAD) and the 2013 and 2019 National Health Surveys (PNS) were obtained. We initially identified that, while PHC was the primary service sought for care needs, this demand tended to decline in all Brazilian regions, among older women, the 60-69 years age group, and whites. The number of households registered with the USF increased 15.2%; the search for a place, service, or health professional by 4.5%; and the population served by 31.4%. The main factors of non-attendance were not being able to get a vacancy/service ticket and doctor unavailability. A growing trend was observed among those diagnosed with Systemic Arterial Hypertension, Diabetes Mellitus, and multimorbidities among those investigated.

Neste trabalho desenvolve-se uma variação da drenagem do seio maxilar após remoção de corpo estranho pela abordagem de Caldwell-Luc. Três pacientes com dente ou resto radicular introduzidos durante exodontias no seio maxilar foram operados e ao invés de contra-abertura nasal procedeu-se a instalação de sondas para a irrigação e aspiração do seio maxilar no período pós operatório imediato. Conclui-se que: O emprego de sondas de irrigação e aspiração reflete um avanço na técnica de Caldwell-Luc; A lavagem pós-operatória do seio maxilar dispensa a realização de contra-abertura nasal e oferece conforto para o paciente no período pós-operatório; A cirurgia de Caldwell-Luc é uma técnica segura e de simples execução para a remoção de corpos estranhos do seio maxilar., In this paper, an improvement of the maxillary sinus drainage after the remotion of foreign bodies by Caldwell-Luc surgery is developed. Three patients with tooth or radicular apex introduced into the maxillary antrum during dental extraction were submitted to the surgery. In contrast to the opening to the nasal cavity, two probes were installed to perform the irrigation and aspiration of the antrum after the surgery. The conclusions are that the use of the probes is an advance in the Caldwell-Luc surgery. With the irrigation and aspiration, the opening to the nasal cavity is not necessary and the result is a comfortable recovery to the patient. The Caldwell-Luc surgery is a safe and simple procedure to remove foreign bodies from the maxillary sinus.

In the Angolan educational scenario, with the publication of the recent Presidential Decree No. 59/20 of march 3, 2020, which approves the regulation of distance and semi-face-to-face teaching modalities in the higher education subsystem, the country reinforces education with the use of digital technologies, as measures to support face-to-face education, a timely measure, particularly in a public health emergency, such as the COVID-19 pandemic. This paper aims to present teaching-learning alternatives through free online platforms Google Classroom (asynchronous tool), Meet and Zoom (synchronous tools) and their pedagogical potential for remote teaching in angolan public higher education in the period of COVID-19. This is a diagnostic, exploratory, descriptive and qualitative research, through the application of an online questionnaire to the teachers of the Polytechnic Higher Institute of South Cuanza, in order to explore their possible perceptions regarding the use of online platforms in the pandemic period. From the analysis of contents of the questionnaire, there is evidence of positive results for the use of online platforms such as Google Classroom, Meet and Zoom in the teaching-learning process during the pandemic period and that there is a lack of training for teachers on the use of digital educational technologies in this period. It is expected that this study will assist angolan university teachers who intend to alternate their classroom classes through remote education and that certain learning outcomes can be achieved during and after the COVID-19 period. Resumo: No cenário educacional angolano, com a publicação do recente Decreto Presidencial nº 59/20, de 3 de março de 2020, que aprova o regulamento das modalidades de ensino a distância e semipresencial no subsistema de ensino superior, o país reforça o ensino com o recurso às tecnologias digitais, como medidas de apoio ao ensino presencial, uma medida oportuna, particularmente em situação de emergência de saúde pública, como o da pandemia de COVID-19. Este artigo objetiva apresentar alternativas de ensino-aprendizagem através de plataformas online gratuitas Google Classroom (ferramenta assíncrona), Meet e Zoom (ferramentas síncronas) e suas potencialidades pedagógicas para o ensino remoto no ensino superior público angolano no período de COVID-19. Trata-se de uma pesquisa diagnóstica, exploratória, descritiva e qualitativa, através da aplicação de um questionário online aos professores do Instituto Superior Politécnico do Cuanza Sul, a fim de explorar suas possíveis percepções em relação ao uso de plataformas online no período de pandemia. A partir da análise de conteúdos do questionário, há evidências de resultados positivos para o uso de plataformas online como o Google Classroom, Meet e Zoom no processo de ensino-aprendizagem no período de pandemia e que existe carência de oferta de formação para os professores sobre o uso de tecnologias digitais educacionais nesse período. Espera-se com este estudo auxiliar os professores universitários angolanos que pretendem alternar suas aulas presenciais por meio do ensino remoto e que certos resultados de aprendizagem podem ser alcançados durante e após o período de COVID-19. Healthcare-Associated Infections (HAIs) are an important public health problem that impacts negatively on hospital costs and patient prognosis. Given the importance of the hospital environment in the development of HAIs, the objective was to evaluate the bacterial profile on surfaces and equipment of the Orthopedic Clinic of the Hospital Universitário do Vale do São Francisco. Methods: This is a cross-sectional, descriptive, quantitative study. Samples were collected in 13 wards, each ward with four beds and one was chosen at random, where surfaces and equipment were sampled using swabs soaked in saline and a 1cm2 filter paper mold to standardize the samples. After passing the swab, they were stored in a tube containing 5mL of BHI (Brain Heart Infusion) liquid medium. Then, samples were transported to the Clinical Analysis Laboratory/Microbiology Sector where the microbiological analyzes were performed. Results: In total, 257 bacteria were observed, of which 5.11% were possible causes of hospital infection and 79% coagulase-negative Staphylococcus. Antibiograms of these were performed and different resistance profiles were found. The bathroom doorknob, a high-touch surface, presented the greatest variety of species among the evaluated surfaces. Conclusion: Surfaces and equipment of the evaluated clinic present possible bacteria that cause hospital infection with different profiles of antimicrobial resistance, contributing to possible cross infections

Introduction: Description of the bacterial community before and after chemomechanical preparation (CP) with the removal of a smear layer (SL) in pulpectomized primary teeth has been little reported. Objective: These case reports describe the presence of total microorganisms and Enterococcus faecalis in root canals of primary incisors before and after CP with SL removal. Case Reports: Microbiological samples were collected from the root canals of three children (3.66±0.58 years old) with necrosis (n=2) and irreversible pulpal inflammation (n=1) in maxillary primary incisors. After teeth isolation with a rubber dam and antisepsis of the operative field, the sample collections were performed with sterile absorbent paper cones before and after the CP, which included irrigation with 2.5% sodium hypochlorite followed by 6% citric acid to remove the SL. The collected samples were analysed immediately at the end of the clinical procedures. The plates were incubated anaerobically for 48 hours at 37°C. The results were expressed as colony forming units (CFU)/mL. Results: Two of the three teeth showed total microorganisms before the CP. One incisor had no microorganisms in the initial collection. No CFU was counted in the samples collected after CP. Moreover, E. faecalis was not observed any time, either before or after the CP. Conclusions: E. faecalis was not detected in any sample, yet two of the three root canals had microorganisms before CP. In cases where microorganisms were initially found, 100% elimination was observed after the applied protocol. Introdução: A descrição da comunidade bacteriana antes e após o preparo químico-mecânico (PQM) com remoção da smear layer (SL) em dentes decíduos pulpectomizados tem sido pouco relatada. Objetivo: Esses relatos de casos descrevem a presença de microrganismos totais e Enterococcus faecalis em canais radiculares de incisivos decíduos antes e após PQM com remoção de SL. Relatos dos Casos: Amostras microbiológicas foram coletadas do canal radicular de três crianças (3,66 ± 0,58 anos) com necrose (n = 2) e inflamação pulpar irreversível (n= 1) em incisivos decíduos superiores. Após o isolamento dos dentes com dique de borracha e antissepsia do campo operatório, as coletas das amostras foram realizadas com cones de papel absorvente estéril antes e após o PQM, que incluiu irrigação com hipoclorito de sódio 2,5% seguido de ácido cítrico 6% para retirada do SL. As amostras coletadas foram analisadas imediatamente ao final dosprocedimentos clínicos. As placas foram incubadas em anaerobiose durante 48 horas a 37°C. Os resultados foram expressos em unidades formadoras de colônias (UFC)/mL. Resultados: Dois dos três dentes apresentaram microrganismos totais antes do PQM. Um incisivo não apresentava microrganismos na coleta inicial. Nenhuma UFC foi contada nas amostras coletadas após o PQM. Além disso, o E. faecalis não foi observado nenhum momento, nem antes, nem depois do PQM. Conclusão: Não foi detectado E. faecalis em nenhuma amostra, porém dois dos três canais radiculares apresentavam microrganismos antes do PQM. Nos casos em que foram encontrados microrganismos inicialmente, observou-se 100% de eliminação após o protocolo aplicado.

Introduction: Studies of twins are important because environmental and genetic factors seem to be related to the phenotypic alterations. Objective: This paper presents a unique case of monozygotic twins with mirror image of a retained primary central incisor. Case report: Twin male brothers, 9-years-old, presented prolonged retention of the primary central upper incisor. The over-retained teeth in one twin were a mirror image of those in the other twin. The first twin presented a prolonged retention of the tooth 51 whereas the other twin presented a prolonged retention of tooth 61. After radiographic exams the over-retained teeth were extracted. Conclusion: Twins may show similarity in pattern of dental anomalies supporting the influence of genetic factors. In identical twins the location of diagnosed anomalies can be mirror imaged. This fact should lead the professional to examine the pair of twins in order to diagnose any dental anomaly that may be present. Introdução: Estudos feitos em gêmeos são importantes porque fatores ambientais e genéticos parecem estar relacionados às alterações fenotípicas. Objetivo: Este artigo apresenta um caso raro de gêmeos monozigóticos apresentando imagem em espelho de retenção prolongada de incisivos centrais decíduos superiores homólogos. Relato do caso: Os irmãos gêmeos, com 9 anos de idade, não apresentavam história de trauma orofacial ou doença comum da infância. Após exames clínicos e radiográficos foram identificados a retenção do dente 51 no gêmeo 1 e do dente 61 no gêmeo 2. Em ambos os pacientes, os dentes 11 e o 21estavam em erupção. O tratamento proposto foi a exodontia dos dentes decíduos com anestesia local e acompanhamento. Conclusão: Gêmeos podem apresentar semelhança no padrão de anomalias dentárias devido à influência de fatores genéticos. Adicionalmente, em gêmeos monozigóticos, a localização dasanomalias diagnosticadas pode se apresentar invertidas ou imagem em espelho. Esse fato deve estimular o profissional a examinar o par de gêmeos para diagnosticar qualquer anomalia dentária que possa estar presente. O diagnóstico precoce e tratamento adequados devem ser realizados para evitar danos funcionais e estéticos em pacientes com retenções dentárias.

Acoustic Schwannoma is a benign neoplasm of the 8th cranial nerve pair, and accounts for 8-10% of all intracranialtumors. Intratumoral haemorrhage is unusual in these tumors and can lead to hydrocephalus or aggravate a preexisting condition. This paper reports a case of acoustic Schwannoma in a child with complication with intratumoral hemorrhage that lead patient to death. Female patient, 9 years old, presented anacusis on the right, sudden, progressing with vomiting, intense headache and vertigo. Magnetic resonance imaging of the skull showed expansive lesion in the cistern of the right cerebellar angle, with content inside the internalauditory canal, with compression of the adjacent brain stem. The diagnostic hypothesis was acoustic Schwannoma. It progressed with worsening of headache, initially controlled with medication and later loss of consciousness. Computed tomography of the skull revealed an expansive lesion in the cistern of the right cerebellar angle with signs of bleeding with extension to the ventricular system. External ventricular shunt was performed, but the patient evolved with several complications, septic shock and death. Intratumoral hemorrhage in the acoustic Schwannoma is a rare event, but its occurrence worsens the clinical picture and may lead to death of the patients due to intracranial hypertension.
