The chemical composition of vine-shoots is characterised by oenological compounds with high added value, which could contribute to the sensory profile of wines. In this study, toasted vine-shoots from two varieties were used in two different formats, granules and chips, and added to wine at different winemaking stages. The results show that oenological additives in the form of vine-shoots can modulate the chemical composition of wines, and therefore their quality, by enhancing their varietal and woody aromas; thus connecting viticulture and oenology and creating a new concept: circular viticulture.

Nowadays, agronomic practices are evolving towards a “Sustainable Agriculture” using environmentally friendly systems. Modern viticulture aims to reduce inputs without reducing grape yield and wine quality. As a result of pruning, vine-shoots are generated as the waste of the vineyard. However their aqueous extracts have shown to have significant contents of bioactive compounds and, therefore, biostimulant activity. The foliar applications of vine-shoot extracts onto white grapevine variety caused a reduction in the sugar content, further an effect on the grape reaction product (trans-GRP) content was observed.

The aim of this study was to clarify the clinical course of large endolymphatic duct and sac syndrome (LEDS). Although LEDS is the most common form of radiologically detectable inner ear malformation associated with sensorineural hearing loss (SNHL), it is relatively unknown in the German-speaking countries. We hoped to derive useful pointers for clinical care.We evaluated the clinical audiometric records and imaging findings of 169 patients with clinically suspected inner ear malformation seen between 1994 and 2003. Following identification of all LEDS cases, we serially investigated the anamnestic and functional data with regard to severity and course.Among 169 patients, 17 (median age 12 years; 12 females) showed LEDS. A total of 28 ears were affected. Clinically, most cases were of prelingual or perilingual onset and displayed steady or fluctuatingly progressive severe SNHL with emphasis on the high frequencies or deafness. Episodes of sudden hearing loss were relatively frequent. No correlation could be demonstrated between the severity of morphological changes and the degree of SNHL.Progression of SNHL and episodes of sudden hearing loss were seen mainly in late childhood or adolescence. This should be taken into account when informing and advising patients and planning therapy.

Large endolymphatic duct and sac (LEDS) syndrome is the most common radiologically detectable form of inner ear malformation (IEM) associated with sensorineural hearing loss (SNHL). Nevertheless, it is relatively unknown in the German-speaking countries.Among all patients referred from ear, nose, and throat specialists to the radiology department with clinically suspected IEM between 1994 and 2003, we identified the cases of LEDS. The retrospective study included clinical records, high-resolution computed tomography, and magnetic resonance imaging.Of 169 patients, 17 (median age 12 years; 12 females) showed LEDS. A total of 28 ears were affected. Ten patients (6%; 15 ears) had isolated LEDS, while seven patients showed additional IEM (4%; 13 ears). The most frequent of these was dysplastic vestibule (13/13 ears), followed by Mondini deformity (10/13 ears) and dilated semicircular canals (7/13 ears). Three of 13 ears revealed severe IEM of the cochlea, vestibule, and semicircular canals. No correlation could be demonstrated between the severity of morphological changes and the degree of SNHL.LEDS may be an underestimated cause of SNHL. Imaging is necessary for a confident diagnosis.

Hintergrund: Das LEDS stellt die häufigste radiologisch nachweisbare Innenohrfehlbildung (IO-FB) im Zusammenhang mit einer sensorineuralen Schwerhörigkeit (SNHL) dar. Dennoch ist das LEDS im deutschsprachigen Raum weniger bekannt. Es war das Ziel, Daten zur Prävalenz, klinischen Manifestation und den Befunden der Schnittbildgebung in der eigenen Patientenpopulation zu präsentieren. Methoden: Bei 169 Patienten (P), die wegen Fehlbildungsverdacht eine Schnittbildgebung erhielten, identifizierten wir alle radiologisch nachweisbaren LEDS-Fälle. Die retrospektive Analyse umfasste die klinisch-audiologischen Daten sowie die im Zeitraum zwischen 1994 and 2003 durchgeführten HR-CT- und MRT-Untersuchungen. Ergebnisse: Bei 17 P (10%, medianes Alter: 12 Jahre, 12 weiblich) bzw. 28 Ohren (O) zeigte sich ein LEDS. Bei 10 P (6%; 15 O) bestand ein isoliertes LEDS, 7 P zeigten weitere IO-FB. Zum Zeitpunkt der Bildgebung ergab sich eine SNHL bei 9 O (acht O mit zusätzlicher Schallleitungskomponente), eine Taubheit bei 17 O und eine Normakusis bei zwei O. Von 26 O mit LEDS (2 O wegen unsicherem Verlauf ausgeschlossen) zeigten 17 O eine progrediente (PR) oder fluktuierend progrediente (FPR) SNHL (10 -71%- von 14 O mit isoliertem LEDS), bei 9 O verlief die SNHL konstant (vier O mit isoliertem LEDS). 7 von 11 P mit PR- oder FPR-SNHL (auf wenigstens einem O) erlitten teilweise rezidivierende Hörstürze (11 von 17 O; 7 O mit isoliertem LEDS). Hörsturz-Trigger fanden sich bei 6 P. Eine Korrelation zwischen morphologischem Defektausmaß und dem Grad der SNHL war nicht nachweisbar. Fazit: Das LEDS kann eine möglicherweise bisher zu wenig beachtete Ursache einer PR- oder FPR-SNHL und rezidivierender Hörstürze bei Kindern und jungen Erwachsenen sein. Die zeitnahe Bildgebung spielt eine wichtige Rolle bei der Diagnosesicherung.

Der erweiterte Ductus und Saccus endolympathicus (LEDS) stellt die häufigste radiologisch nachweisbare Fehlbildung am Innenohr dar. Dessen ungeachtet wird dieser Befund im klinischen Alltag unterschätzt. Ziel der vorliegenden Arbeit war es, die Prävalenz der Fehlbildung am eigenen Patientengut festzustellen und diese Patienten sowohl klinisch als auch radiologisch detailliert zu analysieren. Aus einem retrospektiven Kollektiv von 169 unter Missbildungsverdacht am Innenohr radiologisch untersuchten Patienten identifizierten wir 17 Fälle mit einem LEDS im Zeitraum von 1994-2002. Bei 10 Patienten lag der LEDS als alleinige Fehlbildung vor, bei 7 Fällen wurden assoziierte Anomalien am Innenohr beobachtet. Die Analyse der audiometrischen Daten bei 28 betroffenen Ohren ergab eine hochgradige Schwerhörigkeit in 22 Fällen, bei 5 Ohren eine Taubheit. 1 Ohr war klinisch hinsichtlich der Innenohrfunktion unauffällig. Bei 67 % der Ohren war im Verlauf ein Progress des Hörverlustes zu verzeichnen. Bei 11 Patienten traten zum Teil rezidivierende Hörstürze auf, die bei 5 Patienten durch Trigger ausgelöst wurden. Eine Korrelation der Ausprägung des radiologischen Befundes mit dem Grad des Hörverlustes war nicht zu beobachten. Auf Grundlage der Ergebnisse dieser Studie konnten wir ein typisches Erkrankungsbild einer schweren sensorineuralen Schwerhörigkeit mit einem hohen Prozentsatz an progredienten Verläufen bei Kindern und jungen Erwachsenen bei radiologisch nachgewiesenen LEDS definieren. Eine frühzeitige radiologische Diagnose bildet als die Grundlage zur Aufklärung dieser Patienten über das Krankheitsbild insbesondere hinsichtlich eines möglichen Progresses und zur Beratung bezüglich der Adaptation ihrer Lebensführung., The large endolymphatic duct and sac (LEDS) is the most common radiological finding in patients with suspected inner ear malformation. Nevertheless LEDS is relatively unknown among general radiologists. The aims of this study were to evaluate the incidence of LEDS in the own patient population and to present our experiences regarding imaging findings, clinical presentation and follow up. Between 1994 and 2002 we retrospectively identified among 169 patients in whom an inner ear malformation was clinically suspected 17 patients with LEDS. In 10 patients no other abnormalities were detected, seven patients showed additional inner ear abnormalities. Audiometric data revealed sensorineural hearing loss in 22 ears, deafness in 5 ears and normal hearing in 1 case of 28 ears. In 67% the hearing loss was downward-fluctuating progressive. Twelve patients had partly repeated sudden hearing losses. A trigger for worsening of hearing was found in five patients. A correlation between the severity of morphological changes on imaging and the degree of hearing disturbances could not be detected. Our observations indicate that LEDS might be the cause of progressive hearing loss and repeated acute hearing losses in children and young adults. Even if there is no causal therapy an early diagnosis is needed to explain the patient the cause of his hearing loss, the prognosis and rules of behaviour which can delay possibly a progression.

A 47-year-old patient suffered from right-sided facial pain in the chin and lower cheek, hyperpathia, and difficulties in swallowing. The diagnosis of Eagle's syndrome was based on digital palpation of the bilaterally elongated styloid process and radiography. Eagle's syndrome should be considered in patients with difficulty swallowing, masticatory pain, globus sensation, and neuropathic pharyngeal or facial pain. If conservative therapy fails, selected patients may benefit from surgical excision of elongated styloid processes if pain relief by local anesthesia is proven.

Beim erweiterten Aquaeductus vestibuli handelt es sich morphologisch um eine Erweiterung des Ductus und Saccus endolymphaticus. Typisch für dieses Krankheitsbild ist die Innenohrschwerhörigkeit, die sich schubweise, oft nach Bagatelltraumen, verschlechtert. Eine intracranielle Drucksteigerung wird als Ursache angesehen, die sich auf den Endolymphraum überträgt. Es werden 6 Fälle eines erweiterten Aquaeductus vestibuli aus der eigenen Klinik demonstriert. Hierbei sind bei einigen Patienten aus der Anamnese keine eindeutigen Hinweise auf die Fehlbildung zu erkennen. Bei zwei Patientinnen könnte das Auftreten der Hörstörung unter der Entbindung auf die Diagnose hinweisen. Es werden auch anhand der Literatur die möglichen Ursachen sowie eventuelle Therapiemöglichkeiten diskutiert.

This is a prospective analysis of the value of MRI in suspected inner ear malformations.In 50 patients (43 children and young adults, 7 adults) with suspected inner ear malformation MRI (1.5 T) was performed. In addition, 42 of these patients underwent CT. For the analysis of the inner ear structures, the constructive interference in steady state (CISS) sequence with 0.7 mm slice thickness was used. Functional tests revealed a sensorineural hearing loss or deafness in 82 temporal bones (TB) and a combined hearing loss in 4 TB. The hearing loss was unilateral in 14 patients. MRI and CT findings were compared.Imaging findings were normal in 58 TB. The pathological findings included inner ear malformations (35 TB), inflammatory changes (4 TB), partial obliteration of labyrinth (2 TB) and congenital aural atresia (1 TB). An isolated absence of the cochlear nerve (1 TB) could only be found by MRI. In the remaining cases, an inner ear malformation was diagnosed by MRI and CT with the same confidence but MRI was superior in displaying the fine details.MRI will become the method of choice in the diagnosis of inner ear malformations.
