Goals for health and health care are an indispensable basic requirement for a functioning health care system. The dilemma of the German health care system is that it has not been designed in a planned way, but that it has grown historically. In recent years, it has developed through the free play of forces into what it is today. The OECD characterizes the current state as follows: The costs of the German health system do not correspond to the often only average health outcomes for the population. To meet the legal requirements (especially SGB V §§ 12, 27 and 70), health care/the health system in Germany needs concrete goals. An orientation towards health care goals entails measures on all levels of health care: on the macro level (overall system/total population), on the meso level (subdivided according to regions, specific population groups, etc.) as well as on the micro level (patients and health care providers). Based on national and international experiences, this position paper of the DNVF e.V. (German Network for health services research) shows the potential of how operationalised health care targets can ensure effective, affordable and high-quality health care. The coalition agreement of the current government propagates a reorientation with patient-related health care goals. Now it is important to derive concrete and realisable goals from this declaration of intent and to involve all important groups in the process. In addition, values and ethical standards for implementation shall be agreed upon in this process. The Health Ministry (BMG) should facilitate and promote the process of societal will-building for the definition of national health care goals. This requires a clear political will. As a result, the National Health Care Goals are available at the end of the process, which are published and maintained together with evidence-based facts as well as valid and resilient data in a Manual "National Health Care Goals". The operational responsibility for implementation could lie with the newly to be founded Federal Institute of Public Health, as already announced in the agreement of the governing coalition. The DNVF is willing to actively participate in the development of health care targets.Gesundheits- und Versorgungsziele sind eine unverzichtbare Grundvoraussetzung für ein funktionierendes Gesundheitssystem. Das Dilemma des deutschen Gesundheitssystems ist, dass es nicht planvoll weiter entwickelt wurde, sondern dass es historisch gewachsen ist. In den letzten Jahren hat es sich im freien Spiel der Kräfte zu dem entwickelt, was es heute ist. Den aktuellen Zustand beschreibt die OECD so: Die Kosten des deutschen Gesundheitssystems entsprechen nicht den oft nur durchschnittlichen Gesundheitsergebnissen für die Bevölkerung. Zur Erfüllung der gesetzlichen Anforderungen (vor allem SGB V §§ 12, 27 und 70) braucht die Gesundheitsversorgung/das Gesundheitssystem in Deutschland konkrete Ziele. Eine Orientierung an Versorgungszielen zieht Maßnahmen auf allen Ebenen der Versorgung nach sich: auf der Makroebene (Gesamtsystem/gesamte Bevölkerung), auf der Mesoebene (unterteilt nach Regionen, spezifischen Bevölkerungsgruppen etc.) sowie auf der Mikroebene ( Patient:innen und Leistungserbringer) Ausgehend von nationalen und internationalen Erfahrungen zeigt das vorliegende Positionspapier des DNVF e.V. (Deutsches Netzwerk Versorgungsforschung) das Potenzial, wie operationalisierte Versorgungsziele eine effektive, finanzierbare und qualitativ hochwertige Gesundheitsversorgung gewährleisten können. Der Koalitionsvertrag der Ampelregierung propagiert eine Neuausrichtung mit Patient:innen bezogenen Versorgungszielen. Jetzt gilt es, aus dieser Absichtserklärung konkrete und umsetzbare Ziele abzuleiten und dabei alle wesentlichen Gruppen zu beteiligen. Ergänzend werden in diesem Prozess Werte und ethische Normen für die Umsetzung vereinbart. Das BMG (Bundesministerium für Gesundheit) sollte den Prozess der gesellschaftlichen Willensbildung zur Definition von Nationalen Versorgungszielen ermöglichen und fördern. Dazu bedarf es einer klaren politischen Willensbildung. Als Ergebnis liegen am Ende des Prozesses die Nationalen Versorgungsziele vor, die zusammen mit evidenzbasierten Fakten sowie mit validen und belastbaren Daten in einem Handbuch „Nationale Versorgungsziele“ veröffentlicht und gepflegt werden. Die operative Verantwortung für die Umsetzung könnte bei dem neu zu gründenden Bundesinstitut für öffentliche Gesundheit liegen, wie es bereits im Koalitionsvertrag der Ampelkoalition angekündigt wurde. Das DNVF ist bereit, an der Entwicklung von Versorgungszielen aktiv mitzuwirken.

Hundreds of thousands of individuals who experience lasting sequelae after sepsis and infections in Germany do not receive optimal care. In this White Paper we present measures for improvement, which were developed by a multidisciplinary expect panel as part of the SEPFROK project. Improved care rests on four pillars: 1. cross-sectoral assessment of sequelae and a structured discharge and transition management, 2. interdisciplinary rehabilitation and aftercare with structural support, 3. strengthening the specific health literacy of patients and families, and 4. increased research into causes, prevention and treatment of sequelae. To achieve this, appropriate cross-sectoral care structures and legal frameworks must be created.Hunderttausende Menschen mit Sepsis- und Infektionsfolgen werden derzeit in Deutschland nicht optimal versorgt. Dieses White Paper stellt Maßnahmen zur Verbesserung der Versorgung vor, die von einer multidisziplinären Expertengruppe im Rahmen des Innovationsfonds-Projektes SEPFROK erarbeitet wurden. Eine optimale Versorgung beruht auf 4 Säulen: 1. der sektorenübergreifenden Erfassung der Folgen und einem strukturierten Entlass- und Überleitungsmanagement, 2. einem gezielten Angebot von interdisziplinärer Rehabilitation- und Nachsorge mit struktureller Unterstützung, 3. der Stärkung der spezifischen Gesundheitskompetenz von Betroffenen und Angehörigen und 4. der Intensivierung der Forschung zu Ursachen, Prävention und Therapie der Folgen. Hierfür müssen entsprechende sektorenübergreifende Versorgungsstrukturen und sozialrechtliche Rahmenbedingungen geschaffen werden.

The inpatient care of children after ENT surgery is organized heterogeneously across Germany. In this statement, the working group on Pediatric ENT comments a national survey on the status quo of child care after ENT interventions in German hospitals. The statement should serve as a basis for further interdisciplinary discussion.Die stationäre Betreuung von Kindern nach Operationen im HNO-Bereich ist deutschlandweit heterogen organisiert. In dieser Stellungnahme kommentiert die Arbeitsgemeinschaft Pädiatrische HNO-Heilkunde eine nationale Umfrage zum Status quo der Kinderversorgung nach HNO-Eingriffen in deutschen Kliniken. Diese dient als Grundlage für die weitere interdisziplinäre Diskussion.

At the time of admission to an intensive or intermediate care unit, assessment of the patients' nutritional status may have both prognostic and therapeutic relevance with regard to the planning of individualized medical nutrition therapy (MNT). MNT has definitely no priority in the initial treatment of a critically ill patient, but is often also neglected during the course of the disease. Especially with prolonged length of stay, there is an increasing risk of malnutrition with considerable prognostic macro- and/or micronutrient deficit. So far, there are no structured, evidence-based recommendations for assessing nutritional status in intensive or intermediate care patients. This position paper of the Section Metabolism and Nutrition of the German Interdisciplinary Association for Intensive and Emergency Medicine (DIVI) presents consensus-based recommendations for the assessment and technical monitoring of nutritional status of patients in intensive and intermediate care units. These recommendations supplement the current S2k guideline "Clinical Nutrition in Intensive Care Medicine" of the German Society for Nutritional Medicine (DGEM) and the DIVI.Die Erhebung des Ernährungsstatus zum Zeitpunkt der Aufnahme im Intensiv- oder Intermediate Care Bereich hat sowohl prognostische als auch therapeutische Relevanz im Hinblick auf die Planung einer individualisierten medizinischen Ernährungstherapie (engl. „medical nutrition therapy“, MNT). Diese Planung wird im Rahmen der Erstversorgung eines vital bedrohlichen Krankheitsbilds nachvollziehbar nicht priorisiert, jedoch im weiteren Verlauf häufig auch oft nicht mehr angemessen durchgeführt. Vor allem bei längerer Verweildauer besteht das Risiko einer Mangelernährung mit Aufbau eines kumulativen, prognoserelevanten Makro- und/oder Mikronährstoffdefizits. Bisher gibt es für Patient*innen auf Intensiv- und Intermediate Care Einheiten keine strukturierten Empfehlungen zur Erhebung des Ernährungsstatus. Das vorliegende Positionspapier der Sektion Metabolismus und Ernährung der Deutschen Interdisziplinären Vereinigung für Intensiv- und Notfallmedizin (DIVI) beinhaltet konsensbasierte Empfehlungen zur Erfassung und zum apparativen Monitoring des Ernährungsstatus von Patient*innen auf Intensiv- und Intermediate Care Stationen. Diese Empfehlungen ergänzen die aktuelle S2k-Leitlinie „Klinische Ernährung in der Intensivmedizin“ der Deutschen Gesellschaft für Ernährungsmedizin (DGEM) und der DIVI.

The treatment of patients with advanced heart failure requires interdisciplinary care in a qualified heart failure team, especially prior to and in the follow-up of mechanical circulatory support (MCS) implantation and heart transplantation (HTx). The basic prerequisite is the early specialized evaluation of symptomatic patients even under optimized heart failure treatment. Diagnostics and treatment are initially aimed at improving the prognosis. If the prerequisites for MCS or HTx treatment are present, possible contraindications and problems have to be evaluated in order to achieve an optimal risk-benefit ratio for the abovementioned complex treatments with limited resources. The optimal treatment is still HTx if the conditions are right, so this should be sought in all potential patients. At the same time, the optimal individual transplantation window should not be missed. The provision of a MCS system is a treatment performed with very good results for patients with exclusion criteria for HTx or with hemodynamic instability that prevents a longer waiting time for a donor organ. Short-term and medium-term survival is now comparable to HTx when carefully indicated. Timely implantation before the onset of manifest end-organ damage is crucial. Optimized implementation of advanced treatment requires professional structures that enable effective interdisciplinary cooperation between different sectors of health care.Die Behandlung von Patienten mit fortgeschrittener Herzinsuffizienz erfordert eine interdisziplinäre Betreuung in einem qualifizierten Herzinsuffizienzteam, insbesondere im Vorfeld und in der Nachbetreuung der Versorgung mit einem mechanischen Kreislaufunterstützungssystem (MCS) und einer Herztransplantation (HTx). Grundvoraussetzung ist die frühzeitige spezialisierte Evaluation symptomatischer Patienten auch unter optimierter Herzinsuffizienzbehandlung. Die Diagnostik und Therapie ist zunächst auf eine Prognoseverbesserung ausgerichtet. Liegen die Voraussetzungen für eine MCS- oder HTx-Therapie vor, so sind mögliche Kontraindikationen und Probleme zu evaluieren, um eine optimale Nutzen-Risiko-Relation für die genannten aufwendigen Therapien mit begrenzten Ressourcen zu erzielen. Die optimale Therapie besteht bei entsprechenden Voraussetzungen nach wie vor in der HTx, sodass diese bei allen potenziellen Patienten angestrebt werden sollte. Dabei darf das optimale, individuelle Transplantationsfenster nicht verpasst werden. Die Versorgung mit einem MCS-System ist eine mit sehr guten Ergebnissen durchgeführte Therapie für Patienten mit Ausschlusskriterien für eine HTx oder mit einer hämodynamischen Instabilität, die eine längere Wartezeit auf ein Spenderorgan verhindert. Das kurz- und mittelfristige Überleben ist bei sorgfältiger Indikationsstellung mittlerweile vergleichbar mit dem nach HTx. Entscheidend ist die rechtzeitige Implantation vor dem Auftreten von manifesten Endorganschäden. Für die optimierte Umsetzung erweiterter Therapien bedarf es professioneller Strukturen, die eine effektive interdisziplinäre Kooperation unterschiedlicher Sektoren der Gesundheitsversorgung ermöglichen.

Today, patients with chronic wounds are treated in many different fields of medicine. Despite this great interdisciplinary and interprofessional importance, there is still a lack of uniformly accepted definitions and classifications. Therefore, a group of experts from the professional society Initiative Chronische Wunden (ICW) e. V. translated and adapted the classification of chronic wounds into healable, maintenance and nonhealable wounds on the basis of the internationally published literature into German. This classification results in the aim of curative, limited respectively non-curative or palliative wound care, which are very important for everyday clinical practice. It thus becomes clear that complete wound closure is not always the central intention of wound treatment. For many patients with chronic wounds, other aspects such as the best possible quality of life and the promotion of health-related self-management as well as the avoidance of complications are important for treatment concepts. These therapy intentions should be differentiated and individually discussed with patients in order to facilitate shared decision making.Patienten mit chronischen Wunden werden heute in vielen unterschiedlichen Bereichen der Medizin behandelt. Trotz dieser großen interdisziplinären und interprofessionellen Bedeutung, fehlt es weiterhin an einheitlich akzeptierten Definitionen und Einteilungen. Eine Expertengruppe der Fachgesellschaft Initiative Chronische Wunden (ICW) e. V. hat daher auf der Basis international publizierter Literatur eine Einteilung chronischer Wunden in heilende, schwer heilende und nicht heilbare Wunden vorgenommen. Hieraus ergeben sich für den klinischen Alltag sehr wichtigen übergeordneten Ziele einer kurativen, bedingt bzw. nicht kurativen oder palliativen Wundversorgung. Es wird somit deutlich, dass der vollständige Wundverschluss nicht immer das zentral wichtige Ziel der Wundbehandlung ist. Bei vielen Patienten mit chronischen Wunden stehen daher andere Aspekte wie beispielsweise die bestmögliche Lebensqualität und die Förderung des gesundheitsbezogenen Selbstmanagements sowie die Vermeidung von Komplikationen im Vordergrund der Behandlungskonzepte. Diese Therapieziele sollten möglichst frühzeitig gemeinsam mit den Patienten differenziert und individuell festgelegt werden.

The nomenclature used today in wound treatment varies widely across different disciplines and professions. Therefore, it is a mission of the professional association Initiative Chronische Wunde (ICW) e. V. to exactly and comprehensibly describe terms that were previously unclear. Therefore, the experts of the ICW defined in a consensus procedure debridement of chronic wounds as the removal of adherent, dead tissue, scabs or foreign bodies from wounds. There are various therapy options for this, which can be differentiated into autolytic, biosurgical, mechanical, osmotic, proteolytic/enzymatic and technical debridement. In the case of surgical debridement, a distinction is also made between sharp debridements that can usually be performed on an outpatient basis, such as minor surgical procedures, and surgical debridements with adequate anaesthesia in an operating theatre. Wound irrigation is defined by the ICW as the removal of non-adherent components on wounds with sterile solutions. Debridement and/or wound irrigation are often the first step in phase-appropriate modern wound treatment. Several methods are suitable for use in a combined or successive therapy. When deciding which therapeutic option to use, a number of individually different factors should be taken into account, depending on the patients to be treated but also on the therapists. The final individual decision for a method should be made together with the patient in each case and then adequately documented.Die heute in der Wundbehandlung verwendete Nomenklatur ist interdisziplinär und interprofessionell sehr unterschiedlich. Daher ist es ein Anliegen der Fachgesellschaft Initiative Chronische Wunde (ICW) e. V. bislang unklare Begriffe eindeutig und nachvollziehbar zu beschreiben. Von den Experten der ICW wurde daher in einem Konsensusverfahren als Débridement chronischer Wunden die Entfernung von anhaftendem, abgestorbenem Gewebe, Krusten oder Fremdkörpern aus Wunden bezeichnet. Hierfür gibt es verschiedene Therapieoptionen, die als autolytisches, biochirurgisches, mechanisches, osmotisches, proteolytisches/enzymatisches und technisches Débridement unterschieden werden können. Bei dem chirurgischen Débridement wird zudem zwischen meist ambulant durchführbaren scharfen Débridements wie beispielsweise kleineren chirurgischen Eingriffen und chirurgischen Débridements mit adäquater Anästhesie in einem Operationssaal differenziert. Als Wundspülung wird von der ICW die Entfernung von nicht haftenden Bestandteilen auf Wunden mit sterilen Lösungen bezeichnet.Débridement und/oder Wundspülung sind oft der erste Schritt einer phasengerechten modernen Wundbehandlung. Mehrere Methoden eignen sich für die Anwendung einer kombinierten oder sukzessiven Therapie. Bei der Entscheidung, welche therapeutische Option hierbei zum Einsatz kommt, sollte eine Vielzahl individuell unterschiedlicher Faktoren in Abhängigkeit von den zu behandelnden Patienten, aber auch von den Therapeuten berücksichtigt werden. Die letztendliche individuelle Entscheidung für eine Methode sollte jeweils gemeinsam mit den Patienten getroffen und anschließend adäquat dokumentiert werden.

Irrespective of the great impact stroke exerts on the society as a whole and far-reaching advances in acute treatment and rehabilitation of stroke, so far outpatient services for post-stroke care have not been established on a national level in Germany.Against the background of this contemporary lack of care, in May 2020 the German Stroke Society (DSG) established the stroke aftercare commission. This position paper discusses structural models of future services addressing outpatient post-stroke care.The specialized care by a neurologist should be central to a multidisciplinary, interprofessional and transsectoral treatment. Structural concepts of post-stroke care must take regional differences but also effective strategies for quality control into account. Certification processes and appropriate financing of follow-up registries at state and federal levels may pave the way for improvement over the medium term. Structured outpatient post-stroke care services should be open to all subgroups of stroke patients. Additionally, innovative technologies can make an important contribution to post-stroke care; however, the implementation of specialized services demands adequate funding as well as separate financial incentives for the providers. The solution must carefully balance the advantages and disadvantages of the specific care and financing models. Currently the discussion of new models of post-stroke care is gaining new momentum, which opens up perspectives for the advancement of the otherwise still insufficient contemporary care structures.HINTERGRUND: Ungeachtet der großen gesamtgesellschaftlichen Bedeutung des Schlaganfalls sowie der Fortschritte in der Akutversorgung und Rehabilitation konnten bisher keine flächendeckenden Versorgungsstrukturen zur strukturierten ambulanten Nachsorge in Deutschland etabliert werden.Vor dem Hintergrund der bestehenden Versorgungslücken wurde im Mai 2020 die Kommission Schlaganfallnachsorge der Deutschen Schlaganfall-Gesellschaft (DSG) gegründet. Das Positionspapier diskutiert strukturelle Konzepte für zukünftige Versorgungsformen der Schlaganfallnachsorge.Eine neurologische Betreuung sollte zentraler Bestandteil einer multidisziplinären, interprofessionellen und sektorenübergreifenden Behandlungsplanung sein. Strukturelle Konzepte zur Schlaganfallnachsorge müssen sowohl regionale Strukturunterschiede als auch den Aspekt der Qualitätssicherung berücksichtigen. Zertifizierungsprozesse und die angemessene Finanzierung von Nachsorgeregistern auf Landes- und Bundesebene können den Weg hierhin mittelfristig ebnen. Das Angebot zur strukturierten Schlaganfallnachsorge sollte allen Subgruppen von Schlaganfallpatienten offenstehen. Auch innovative Technologien können einen wichtigen Beitrag zur Schlaganfallnachsorge leisten. Die Einführung und Umsetzung einer strukturierten Schlaganfallnachsorge bedarf in jedem Fall einer ausreichenden Finanzierung und eigener finanzieller Anreize für die Leistungsträger. Die Vor- und Nachteile der spezifischen Versorgungs- und Finanzierungsmodelle müssen dabei kritisch gegeneinander abgewogen werden. Die Diskussion neuer Versorgungsformen zur Schlaganfallnachsorge erfährt gegenwärtig ein neues Momentum und eröffnet Perspektiven für eine Verbesserung der aktuell noch unzureichenden Versorgungslösung.

Use of Electronic Cigarettes (e-cigs) and e-Shishas by Children and Adolescents: Evidence Paper of the Joint Addiction Commission of the German Societies and Professional Associations of Child and Adolescent Psychiatry and Psychotherapy

The non-physician employees in telemedicine centers (TMC) play a decisive role in the care and treatment of patients with heart failure. For this reason, a holistic profile consisting of professional and methodological as well as social and personal competences is necessary, which should be built up or promoted in corresponding training concepts. This position paper underlines the urgency of appropriate and standardized further training of non-physician employees for quality assurance in TMCs and summarizes the requirements for the additional qualification of a telemedical assistant across the board.Das nichtärztliche Personal in Telemedizinzentren (TMZ) nimmt in der Betreuung und Behandlung von Patienten mit Herzinsuffizienz eine entscheidende Rolle ein. Aus diesem Grund ist ein ganzheitliches Anforderungsprofil bestehend aus Fach- und Methodenkompetenz sowie sozialer und persönlicher Kompetenz gefordert, welches in entsprechenden Schulungskonzepten aufgebaut bzw. gefördert werden sollte. Das vorliegende Positionspapier betont die Dringlichkeit einer angemessenen und standardisierten Fortbildung des nichtärztlichen Personals zur Qualitätssicherung in TMZ und fasst die Anforderungen an die Zusatzqualifikation einer Telemedizin-Assistenz übergreifend zusammen.

Research in mental health services in Germany is of increasing relevance. To this end, the recently founded "Mental Health Working Group" as part of the German Network of Health Services Research (DNVF) has written a discussion paper presenting key specifics, challenges, and goals of health service research in the field of mental health. Some research-relevant specifics in the area of mental health care, pragmatic challenges of research organization, ethical problems, and particular research topics in this field are presented and discussed critically.Versorgungsforschung im Feld der seelischen Gesundheit ist von zunehmender Bedeutung. Angesichts dieser Entwicklung hat die im Oktober 2017 gegründete DNVF Fachgruppe „Seelische Gesundheit“ ein Diskussionspapier verfasst, welches zentrale Besonderheiten, Herausforderungen und Ziele der Versorgungsforschung im Bereich der seelischen Gesundheit zusammenfasst. Im Einzelnen werden forschungsrelevante Besonderheiten des Versorgungsfeldes, pragmatische Probleme bei der Forschungsorganisation, ethische Herausforderungen und für dieses Feld besondere Themenstellungen vorgestellt und diskutiert.

Long-term management after stroke has not been standardized in contrast to acute and rehabilitative stroke care. The fragmented outpatient sector of healthcare allows a high degree of flexibility but also variability in the quality of care provided. The commission on long-term stroke care of the German Stroke Society developed a concept on how to provide standardized multiprofessional long-term stroke care with a focus on patient-centered comprehensive care. We propose a diagnostic work-up in stages, with an initial patient-reported screening, followed by an adapted in-depth assessment of affected domains. This includes internistic domains (management of risk factors) and also genuine neurological domains (spasticity, cognitive deficits etc.) that must be considered. This information is then merged with patient expectations and prioritization to a standardized treatment plan. Special challenges for the implementation of such a comprehensive care system are the intersectoral and intrasectoral communication between healthcare providers, patients and relatives, the need to create a compensation system for providers and the establishment of appropriate quality management services. Digital health applications are helpful tools to provide aspects of diagnostics, treatment and communication in long-term stroke care.Die Schlaganfallnachsorge ist im Gegensatz zur akuten und rehabilitativen Versorgung des Schlaganfalls wenig standardisiert. Der fragmentierte ambulante Sektor erlaubt hierbei ein hohes Maß an Flexibilität, leidet aber folglich an variabler Qualität der Nachsorge. Die Kommission Nachsorge der Deutschen Schlaganfall-Gesellschaft formuliert in diesem Positionspapier ein inhaltliches Konzept, um eine strukturierte Nachsorge mit multiprofessionellem Ansatz zu entwickeln. Diese soll im Sinne einer „Comprehensive-care“-Versorgung und patientenzentriert erfolgen. Dazu schlagen wir ein diagnostisches Stufenkonzept mit Screening und ggf. weitergehender Untersuchung vor, das in Absprache mit den Betroffenen zu einem standardisierten Therapieplan führt, der im Langzeitverlauf entsprechend angepasst werden muss. Inhaltlich sind sowohl internistische Domänen (Management von Risikofaktoren) als auch genuin neurologische Domänen (Spastik, kognitive Defizite etc.) zu berücksichtigen. Besondere Herausforderungen an dieses Konzept sind die sektorenübergreifende (inter- und intrasektorale) Kommunikation zwischen den Akteuren im Gesundheitswesen untereinander sowie mit den Patienten und Angehörigen, die Notwendigkeit zur Schaffung eines Vergütungsmodells für eine solche Nachsorge und letztlich die Etablierung eines entsprechenden Qualitätsmanagements. Digitale Lösungen erachten wir als hilfreiche Werkzeuge für Aspekte der Diagnose, Therapie und Kommunikation in der Schlaganfallnachsorge.

Erysipelas is a bacterial soft tissue infection caused by β-haemolytic streptococci that spreads proximally along the lymphatic system of the skin. The entry sites of the pathogens can be minor injuries or chronic wounds. The diagnosis of erysipelas is made clinically by the spreading eythema and overheating of the skin, the reduced general condition with fever and chills as well as by means of serological inflammation parameters and must be distinguished from numerous differential diagnoses.Systemic therapy is carried out with penicillin usually. In local therapy, the value of measures such as compression therapy or cooling is currently still controversial. Long-term therapy of the lymphoedema and the consistent avoidance and treatment of entry sites are essential, especially for the prevention of recurrence.Das Erysipel ist eine bakterielle Weichgewebeinfektion, die durch β-hämolysierende Streptokokken verursacht wird und sich entlang der Lymphspalten der Haut nach proximal ausbreitet. Die Eintrittspforten der Erreger können kleinere Verletzungen oder auch chronische Wunden sein. Die Diagnose Erysipel wird klinisch durch die sich ausbreitende Rötung und Überwärmung der Haut, den reduzierten Allgemeinzustand mit Fieber und Schüttelfrost sowie mittels serologischer Entzündungsparameter gestellt und muss von zahlreichen Differenzialdiagnosen abgegrenzt werden. Die systemische Therapie erfolgt meist mit Penicillin. In der Lokaltherapie ist der Stellenwert der Maßnahmen wie Kompressionstherapie oder Kühlung derzeit noch umstritten. Insbesondere für die Rezidivprophylaxe sind die langfristige Therapie des Lymphödems sowie die konsequente Vermeidung und Therapie von Eintrittspforten essenziell.

The logistical and infectious peculiarities and requirements challenge the intensive care treatment teams aiming at a successful liberation of patients from long-term mechanical ventilation. Especially in the pandemic, it is therefore important to use all potentials for weaning and decannulation, respectively, in patients with prolonged weaning.Weaning centers represent units of intensive medical care with a particular specialization in prolonged weaning. They are an integral part of a continuous care concept for these patients. A systematic weaning concept in the pandemic includes structural, personnel, equipment, infectiological and hygienic issues. In addition to the S2k guideline "Prolonged weaning" this position paper hightlights a new classification in prolonged weaning and organizational structures required in the future for the challenging pandemic situation. Category A patients with high weaning potential require a structured respiratory weaning in specialized weaning units, so as to get the greatest possible chance to realize successful weaning. Patients in category B with low or currently nonexistent weaning potential should receive a weaning attempt after an intermediate phase of further stabilization in an out-of-hospital ventilator unit. Category C patients with no weaning potential require a permanent out-of-hospital care, alternatively finishing mechanical ventilation with palliative support.Finally, under perspective in the position paper the following conceivable networks and registers in the future are presented: 1. locally organized regional networks of certified weaning centers, 2. a central, nationwide register of weaning capacities accordingly the already existing DIVI register and 3. registration of patients in difficult or prolonged weaning.Die logistischen und infektiologischen Besonderheiten und Erfordernisse in der Pandemie stellen die intensivmedizinischen Behandlungsteams vor außerordentliche Herausforderungen, die eine erfolgreiche Liberation von Patienten im prolongierten Weaning erschweren können. Gerade in der Pandemie gilt es daher, alle Potenziale zur Beatmungsentwöhnung und Dekanülierung von Patienten im prolongierten Weaning oder in der Langzeitbeatmung auszuschöpfen. Weaning-Zentren stellen Einheiten der intensivmedizinischen Betreuung mit besonderer Spezialisierung auf prolongiertes Weaning dar und sind integraler Bestandteil eines kontinuierlichen Versorgungskonzeptes dieser Patienten. Ein systematisches Weaning-Konzept in der Pandemie setzt strukturelle, personelle, apparative, infektiologische und hygienische Aspekte voraus. Im Mittelpunkt dieses Positionspapiers stehen als Ergänzung zur S2k-Leitlinie „Prolongiertes Weaning“ eine neue Klassifikation im prolongierten Weaning und zukünftig erforderliche Organisationsstrukturen für die Pandemie-Situation. Patienten der Kategorie A mit hohem Weaning-Potenzial bedürfen der strukturierten Respiratorentwöhnung in spezialisierten Weaning-Einheiten, um so die größtmögliche Chance auf erfolgreiches Weaning zu realisieren. Patienten der Kategorie B mit geringem oder aktuell nicht vorhandenem Weaning-Potenzial sollten verzögert nach einer Zwischenphase der weiteren Stabilsierung in einer außerklinischen Beatmungseinrichtung einen Weaning-Versuch erhalten. Patienten der Kategorie C ohne Weaning-Potenzial benötigen eine dauerhafte außerklinische Versorgung, alternativ eine palliativmedizinisch begleitete Beendigung der Beatmung. Die sich aus den 3 Kategorien ergebenden Aufgaben für die Weaning-Zentren werden im Positionspapier vertieft.Schließlich werden unter perspektivischer Betrachtung im Positionspapier folgende zukünftig denkbare Netzwerke und Register benannt: 1. lokal organisierte regionale Netzwerke von zertifizierten Weaning-Zentren, 2. ein zentrales, bundesweites Register der Weaning-Kapazitäten entsprechend dem DIVI-Register und 3. eine Registrierung der Patienten im schwierigen oder prolongierten Weaning.

Tobacco smoking is associated with severe health risks. In 2020, the WHO estimated that 8 million people have died due to smoking. Furthermore, smoking tobacco is a well-known risk factor for various infectious pulmonary diseases. The question raised, whether smoking is facilitating SARS-CoV-2-infections and increases adverse outcomes of COVID-19. To answer these questions a narrative review was conducted, finally including 7 systematic reviews with meta-analyses published in January and February 2021. Tobacco smoking was associated with an increased COVID-19 disease severity (odds ratio range of active vs. never smokers 1.55-2.19 and former vs. never smokers 1.20-2.48) and an increased COVID-19 in-hospital mortality (odds ratio range of active vs. never smokers 1.35-1.51 and former vs. never smokers 1.26-2.58). Beside immediate pulmonary toxic effects through active smoking, the cumulative livelong tobacco exposition and subsequent tobacco-associated diseases seem to predominantly predict adverse outcomes in patients with COVID-19. Data regarding an increased risk of infection among smokers is conflicting. However, a large observational study from England with 2.4 million persons reported an association between tobacco smoking and typical symptoms of COVID-19. For e-cigarettes and vaping less data exist, but experimental and first clinical investigations also suggest an increased risk for adverse outcomes for their use and SARS-CoV-2 infections. Especially during the current SARS-CoV-2 pandemic with limited therapeutic options it is particularly important to advise smokers of their increased risks for unfavourable COVID-19 outcomes. Evidence based support for smoking cessation should be offered. In Germany, the existing and well-established methods to support tobacco cessation need to be reimbursed by statutory health insurances.Tabakrauchen geht mit einem erheblichen Gesundheitsrisiko einher. Die Mortalität durch Tabak-assoziierte Erkrankungen wird durch die WHO für das Jahr 2020 auf über 8 Millionen Menschen weltweit geschätzt. Tabakrauchen ist auch ein seit langem gut belegter Risikofaktor für unterschiedliche pulmonale Infektionserkrankungen. Somit stellt sich die Frage, ob Rauchen das Auftreten und schwere Verläufe einer SARS-CoV-2-Infektion begünstigt.Um diese Frage zu beantworten, haben wir einen narrativen Review durchgeführt. Insbesondere haben wir systematisch nach Metaanalysen zum Thema gesucht, die im Jahr 2021 publiziert wurden. Sieben Meta-Analysen wurden identifiziert. Tabakrauchen war dabei mit einem erhöhten Risiko schwerer Krankheitsverläufe (Bereich des Odds Ratios/ORs von aktiven Rauchern vs. Nierauchern 1,55–2,19 und von ehemaligen Rauchern vs. Nierauchern 1,20–2,48) und einer erhöhten Krankenhaussterblichkeit (Bereich der ORs von aktiven Rauchern vs. Nierauchern 1,35–1,51 und ehemaligen Rauchern vs. Nierauchern 1,26–2,58) an COVID-19 assoziiert. Dabei sind offenbar neben einer direkten pulmonalen Schädigung durch das Rauchen v. a. Tabak-assoziierte Begleiterkrankungen und damit die kumulative Tabakexposition für schwere Verläufe verantwortlich. Für das Infektionsrisiko ist die Datenlage nicht eindeutig, auch wenn eine britische Studie mit über 2,4 Millionen Personen eine Assoziation von Tabakrauchen und COVID-19-typischen Symptomen beschreibt. Für die E-Zigarette und Tabakerhitzer stehen weniger Daten zur Verfügung. Laborexperimentelle und erste klinische Daten legen aber auch für diese Nikotinprodukte einen ungünstigen Einfluss auf SARS-CoV-2-Infektionen nahe.Gerade während der SARS-CoV-2-Pandemie mit stark begrenzten therapeutischen Möglichkeiten für COVID-19 ist es wichtig, aktive Raucher auf die Gefahren des Konsums hinzuweisen und eine Entwöhnungsbehandlung zu ermöglichen, die auf einer breiten Evidenz und Erfahrung fußt. In Deutschland gibt es dabei erheblichen Nachholbedarf. Erforderlich ist daher eine Regelung, die die Kostenübernahme für wissenschaftlich gesicherte Methoden der Tabakentwöhnung durch die gesetzliche Krankenversicherung ermöglicht.

Radiofrequency denervation has been established for many years as an important minimally invasive procedure for the treatment of chronic pain conditions. Positive experiences of many users for various indications are contrasted by a nonuniform evidence. With meticulous patient selection and correct assessment of the indications a longer term reduction of pain, a reduced need for analgesics and an improvement in the quality of life can be achieved. The aim of this interdisciplinary position paper is to present the value of radiofrequency denervation in the treatment of chronic pain. The summarized recommendations of the expert group are based on the available evidence and on the clinical experiences of Austrian centers that frequently implement the procedure. The position paper contains recommendations on patient selection and proven indications. We discribe safety aspects, complications, side effects and contraindications.In der Therapie chronischer Schmerzzustände ist die Radiofrequenzdenervierung seit Jahren als wichtiges minimal-invasives Verfahren etabliert. Positiven Erfahrungen zahlreicher Anwenderinnen und Anwender in unterschiedlichen Indikationen steht eine nicht einheitliche Evidenzlage gegenüber. Bei sorgfältiger Patientenselektion und korrekter Indikationsstellung können damit eine längerfristige Schmerzreduktion, ein reduzierter Analgetikabedarf und eine Verbesserung der Lebensqualität erreicht werden. Ziel dieses interdisziplinären Positionspapiers ist es, den Stellenwert der Radiofrequenzdenervierung in der Therapie chronischer Schmerzen darzustellen. Die zusammenfassenden Empfehlungen der Expertengruppe basieren auf verfügbarer Evidenz ebenso wie auf den klinischen Erfahrungen österreichischer Zentren, die das Verfahren häufig einsetzen. Das Positionspapier enthält Empfehlungen zur Patientenselektion und zu bewährten Indikationen. Sicherheitsaspekte, Komplikationen, Nebenwirkungen und Kontraindikationen werden beschrieben.

This consensus paper summarizes the expert consensus and recommendations of the working group "Heart and Kidney" of the German Cardiac Society (DGK) and the German Society of Nephrology (DGfN) on contrast medium-induced acute kidney injury. Potentially nephrotoxic contrast agents containing iodine are frequently used in interventional medicine and for computer tomography diagnostics. Acute kidney injury occurs in approximately 8-17% of patients exposed to contrast media. The risk factors and underlying pathophysiology are discussed and recommendations for the prophylaxis and treatment of contrast medium-induced acute nephropathy are presented.Dieses Konsensuspapier ist eine Stellungnahme der Arbeitsgemeinschaft „Herz – Niere“ der Deutschen Gesellschaft für Kardiologie – Herz- und Kreislaufforschung e. V. und der Deutschen Gesellschaft für Nephrologie e. V. zum Thema kontrastmittelinduzierte akute Nierenschädigung. Potenziell nierenschädigendes jodhaltiges Kontrastmittel wird in der interventionellen Medizin und der Computertomographiediagnostik häufig eingesetzt. Eine akute Nierenschädigung tritt bei etwa 8–17 % der Patienten auf. Es werden Risikofaktoren und die zugrunde liegende Pathogenese diskutiert und Empfehlungen zur Prophylaxe und Therapie kontrastmittelinduzierter akuter Nierenschädigungen vorgestellt.

In Germany every second offence occasioning bodily harm and every fourth sexual offence occur under the influence of alcohol or drugs. Untreated substance use disorders are a risk factor for further offences. The §64 of the German Penal Code (StGB) regulates the commitment of offenders to a Forensic addiction treatment unit as a result of an addiction to excessive consummation of intoxicating substances. The previous regulation has come under criticism because too many patients are committed, the sentence is often given to the wrong persons, the treatment is often terminated without success and the sentence consumes to many resources. From a psychiatric perspective the reform of commitment to a Forensic addiction treatment unit (§64 StGB) must fulfil the principles of medical ethics, in particular the respect for the autonomy of the patient, the principle of distributional justice and the code of medical professional ethics. The commitment according to §64 must be restricted to the treatment of people with a clinically relevant substance use disorder. From a psychiatric perspective, decisive for the prospect of success are the willingness to be treated and self-determination for admission to the clinic. In order to release the treatment from the extrinsic influences of the enforcement law, on admission to treatment a sufficient amount of the sentence should have already been served that the commitment only serves the purpose of the treatment and resocialization, so that the risk of further substance-related offences is reduced. The legal term "Entziehungsanstalt" should be replaced by "Forensic Clinic for Dependancy Diseases".In Deutschland geschehen jedes zweite Körperverletzungsdelikt und etwa jedes vierte Sexualdelikt unter Alkohol- oder Drogeneinfluss. Unbehandelte Substanzkonsumstörungen sind ein Risikofaktor für weitere Straftaten. § 64 StGB regelt die Unterbringung in einer Entziehungsanstalt für Straftäter in Folge eines Hangs zur Einnahme berauschender Substanzen im Übermaß. Die bisherige Regelung steht in der Kritik, weil zu viele Patienten zugewiesen würden, das Angebot oftmals die falschen Personen bekämen, die Behandlung häufig nicht erfolgreich beendet wird und das Angebot zu viele Ressourcen verbrauche. Eine Reform der Unterbringung in einer Entziehungsanstalt (§ 64 StGB) muss aus psychiatrischer Sicht den medizinethischen Prinzipien, insbesondere dem Respekt vor der Autonomie des Patienten, dem Prinzip der Verteilungsgerechtigkeit und der ärztlichen Berufsordnung gerecht werden. Die Unterbringung nach § 64 StGB muss auf die Behandlung von Menschen mit klinisch relevanten Substanzkonsumstörungen beschränkt werden. Entscheidend für die Erfolgsaussicht einer psychiatrischen Suchtbehandlung sind die Behandlungsbereitschaft und Selbstbestimmung bei Aufnahme in die Klinik. Um die Therapie von behandlungsfremden Einflüssen des Strafvollstreckungsrechts zu entlasten, sollte bei Antritt der Behandlung bereits so viel Strafe verbüßt sein, dass die Unterbringung nur noch dem Behandlungs- und Resozialisierungszweck und dem Ziel dient, das Risiko weiterer substanzmittelbedingter Straftaten zu senken. Der Begriff „Entziehungsanstalt“ sollte ersetzt werden durch die angemessenere Bezeichnung „Forensische Klinik für Abhängigkeitserkrankungen“.

EINFüHRUNG: Um die Reservoirfunktion des Magens zu ersetzen und ein Postgastrektomiesyndrom zu vermeiden, wird nach einer Gastrektomie nicht selten ein jejunaler Pouch als Teil der Roux-en-Y-Ösophagojejunostomie zur anatomischen Resektion im oberen Gastrointestinaltrakt gebildet. Eine Abflussstenose mit Pouchdilatation, die mit konsekutiven abdominellen Beschwerden einhergeht, ist eine mögliche Spätkomplikation. Eine konservative Therapie bleibt oft ohne dauerhaften Erfolg, was meist eine operative Sanierung erforderlich macht. Eine endosonografisch geführte Jejunojejunostomie mit „Lumen-apposing Metal Stent“ (LAMS) ist dahingehend eine neue vielversprechende Therapieoption. FALLPRäSENTATION: Bei einer 75-jährigen Patientin mit jejunalem Pouch nach vorheriger Gastrektomie wegen Magenkarzinoms (pT1b N1 [1/40] M0 L1 V1 G2 R0) traten seit Jahren wiederholt Beschwerden wie Erbrechen, postprandiale Bauchschmerzen und Krämpfe bei einer Abflussstörung des jejunalen Pouches mit einer konsekutiven Pouchdilatation auf. Konservative Therapie und Platzierung eines intestinalen Stents im stenotischen Abgang des abführenden Jejunalschenkels blieben ohne Erfolg. Es wurde eine endosonografisch geführte (EUS-geführte) Jejunojejunostomie mittels Hot-AXIOS-Stent-Platzierung (Boston Scientific, Ratingen, Deutschland) vom Pouchlumen aus zu einem aboralen Segment der hochgezogenen Jejunalschlinge angelegt. Seitdem (Follow-up: 12 Monate) war die Patientin dauerhaft beschwerdefrei mit konsekutiver Gewichtszunahme.Die Jejunojejunostomie mit EUS-geführter Einlage eines „LAMS“ bietet eine Alternative zu einer operativen Sanierung bei Stenose und Passageproblemen des jejunalen Pouches. Sie ist mit einer weit geringeren Invasivität, zügigeren Rekonvaleszenz und damit entscheidend verbesserter Lebensqualität verbunden.In order to adequately replace the reservoir of the stomach and to avoid a post-gastrectomy syndrome, the anatomy of the upper gastrointestinal (GI) tract is reconstructed after gastrectomy – creating a jejunal pouch as part of Roux-en-Y oesophagojejunostomy. Stenosis of the efferent loop with pouch dilatation associated with subsequent abdominal discomfort and complaints can be considered a possible late complication. A conservative therapeutic approach is often unsuccessful and subsequent surgical intervention is necessary. Creation of an endosonography-guided jejunojejunostomy with a “lumen-apposing metal stent” (LAMS) is a novel and promising therapeutic option.In a 75-years old woman with jejunal pouch after previous gastrectomy due to gastric cancer (pT1b N1 [1/40] M0 L1 V1 G2 R0), there had been recurrent complaints for years, such as vomiting, abdominal discomfort and postprandial pain as well as spasms because of disturbed GI passage via the efferent loop with consecutive pouch dilatation. Conservative measures and placement of an intestinal stent within the stenotic segment of the efferent jejunal loop did not result in permanent improvement. Therefore, endosonography-guided (EUS-guided) jejunojejunostomy was performed with a Hot-AXIOS stent (Boston Scientific, Ratingen, Germany) from the endoluminal site of the pouch to an aboral segment of the jejunal loop. At the 12-month follow-up, patient was permanently asymptomatic with resulting increase in body weight.Jejunojejunostomy with EUS-guided implantation of an “LAMS” can be considered an alternative approach to surgery to solve problems with a strictured jejunal pouch due to disturbed GI passage. This is associated with far less invasiveness and traumatisation, prompt convalescence and, thus, substantially improved quality of life.

The National Action Plan for People with Rare Diseases contains 52 concrete actions, including in the fields of care, research, diagnosis, and information management. With the aim of improving the quality and interoperability of national registries in the long term, action 28 proposed the establishment of a "Rare Diseases Registry" strategy group. The strategy group began its work in 2016. In this report, the group takes into account developments at the national and international level in order to develop recommendations for national initiatives.In addition to this, the group reports on consent and implementation as well as on the adaptation of a minimal dataset for use in rare disease registries and mapping the used data elements and schemata in a metadata repository. This position paper was created by the strategy group together with additional authors. The paper reached a consensus within the strategy group and can be seen as a concept paper of the Rare Diseases Registry strategy group.Der Nationale Aktionsplan für Menschen mit Seltenen Erkrankungen (SE) enthält 52 konkrete Maßnahmen, u. a. in den Handlungsfeldern Versorgung, Forschung, Diagnose und Informationsmanagement. Mit dem Ziel, langfristig die Qualität und Interoperabilität von nationalen Registern zu erhöhen, sieht Maßnahmenvorschlag 28 die Etablierung einer Strategiegruppe „Register für Seltene Erkrankungen“ vor. Diese Strategiegruppe hat 2016 ihre Arbeit aufgenommen. Sie berichtet hier über Entwicklungen auf nationaler und internationaler Ebene, um Empfehlungen für nationale Initiativen daraus abzuleiten.Zusätzlich werden die Konsentierung und Implementierung sowie mit der Zeit ggf. die Anpassung eines Minimaldatensatzes zur Verwendung in Registern für Seltene Erkrankungen erläutert. Zusätzlich werden die verwendeten Datenelemente bzw. -schemata in einem sog. Metadata Repository abgebildet. Dieses Positionspapier wurde durch die Strategiegruppe sowie weitere Autoren erarbeitet und innerhalb der Gruppe konsentiert. Es wird als Konzeptpapier zum Aufbau und Betrieb von Registern der Strategiegruppe „Register“ veröffentlicht.

Digitalization in the healthcare system is a great challenge for rheumatology as for other medical disciplines. The German Society for Rheumatology (DGRh) wants to actively participate in this process and benefit from it. By founding the commission on digital rheumatology, the DGRh has created a committee that deals with the associated tasks, advises the DGRh on questions and positions associated with digital health. For the DGRh, this affects the most diverse areas of digitalization in medicine and rheumatology. This position paper presents the topics and developments currently handled by the commission and the tasks identified.

Media-associated disorders in childhood and adolescence: Evidence paper of the joint addiction commision of the German societies and professional associations of child and adolescent psychiatry and psychotherapy

The effects of mental diseases on the employment and working situation can be substantial. They are one of the main reasons for inability to work and reduced earning capacity. Against this background the question arises about suitable occupational reintegration measures for people with severe mental illnesses. In recent years, the principle of supported employment has been internationally shown to be increasingly more successful. In this context mentally ill people are primarily placed at a position of the first employment market and supported on-site by a job coach. This concept is inclusive, individual and evidence based. Despite proven effectiveness, it has so far been insufficiently implemented in German-speaking regions. In the future it will be a matter of considering the individual needs for assistance of mentally ill people more intensively than previously and to respond with functional and in a best-case scenario, multiprofessional and flexible offers.Die Auswirkungen psychischer Erkrankungen auf die Arbeits- und Erwerbssituation können erheblich sein. Sie sind eine der Hauptursachen für Arbeitsunfähigkeit sowie Erwerbsminderung. Vor diesem Hintergrund stellt sich die Frage nach geeigneten beruflichen Wiedereingliederungsmaßnahmen für Menschen mit schweren psychischen Erkrankungen, wobei sich in den letzten Jahren international das Prinzip des Supported Employment als zunehmend erfolgreich erwiesen hat. Hierbei werden psychisch erkrankte Menschen mit beruflichem Rehabilitationsbedarf primär auf einem Arbeitsplatz des ersten Arbeitsmarktes platziert und vor Ort durch einen Jobcoach unterstützt. Dieses Konzept ist inklusiv, individuell, nachhaltig und evidenzbasiert. Trotz erwiesener Wirksamkeit ist es im deutschsprachigen Raum bislang noch wenig umgesetzt. Es wird zukünftig darauf ankommen, stärker als bislang den individuellen Hilfebedarf psychisch erkrankter Menschen aufzunehmen und mit funktional, im besten Fall multiprofessionell und mobil aufgestellten Angeboten zu begegnen.

Increased awareness of the rising antimicrobial resistance problem and impending loss of suitable treatment options for infectious diseases have changed patient care. Antimicrobial/antibiotic stewardship (ABS) activities aiming to optimize antimicrobial treatment were specified in international (WHO, ECDC) and national programmes and evidence-based practice guidelines. In Germany, ABS in hospitals is enshrined in the Infection Protection Act §23 and in a national guideline. The position paper presents the goals and tasks of ABS as well as the necessary organisational and staff requirements.Qualified training and education, mandates and support from hospital directors, and the provision of sufficient resources are essential prerequisites for the successful work of the ABS team. The ABS team should work hospital-wide across clinical services. Their main tasks are developing and implementing an ABS programme tailored to local needs and the conditions of the hospital, taking into account anti-infective drug prescribing, the resistance situation and case mix. Their tasks also include drafting and implementing in-house recommendations for diagnosis, prophylaxis and treatment of important infectious diseases. The ABS team should be interdisciplinary and include specially qualified doctors and pharmacists. Doctors commissioned for ABS should support this team and take the department-specific concerns into account. The document specifies a minimum ABS staff of one full-time equivalent (FTE) per 500 beds. Depending on the case mix and specialties, additional staff may be required. It proposes that there should be 0.1 FTE doctors commissioned for ABS per 100 beds.Gesteigertes Bewusstsein für die Problematik zunehmender Antibiotikaresistenzen und drohender Verlust geeigneter Therapieoptionen für Infektionen haben die Rahmenbedingungen in der Patientenversorgung verändert. In internationalen (WHO, ECDC) und nationalen Programmen und evidenzbasierten Leitlinien wurden Maßnahmen und Instrumente von Antibiotic Stewardship (ABS) konkretisiert. In Deutschland ist ABS im Krankenhaus im Infektionsschutzgesetz §23 verankert und Empfehlungen dazu in einer S3-Leitlinie. Das vorliegende Positionspapier stellt Ziele von ABS sowie erforderliche strukturelle und personelle Voraussetzungen für Krankenhäuser dar.Für die erfolgreiche Arbeit des ABS-Teams sind die Sicherung einer qualifizierten Fort- und Weiterbildung, die Mandatierung, die Unterstützung und die Bereitstellung ausreichender Ressourcen durch die Krankenhausleitung unabdingbar. Das ABS-Team soll krankenhausweit und abteilungsübergreifend arbeiten. Aufgaben sind Ausarbeitung und Umsetzung eines auf die lokalen Bedürfnisse und Möglichkeiten des Krankenhauses zugeschnittenen ABS-Programms unter Berücksichtigung von Antiinfektivaeinsatz, Resistenzlage und Patientenschwerpunkten. Dazu kommen Erstellung und Implementierung hausinterner Empfehlungen zur Diagnostik, Prophylaxe und Therapie wichtiger Infektionen. Das ABS-Team soll interdisziplinär arbeiten und als Teammitglieder speziell qualifizierte Ärzte und Apotheker umfassen. ABS-beauftragte Ärzte unterstützen das ABS-Team und berücksichtigen abteilungsspezifische Belange. Es wird von einer Mindestpersonalstärke für das ABS-Team von 1 Vollzeitäquivalent (VZÄ) pro 500 Betten ausgegangen. Je nach Krankenhausschwerpunkten und Anforderungen wird ein Zusatzbedarf gesehen. Die Wochenarbeitszeitstunden für einen ABS-beauftragten Arzt einer mittelgroßen Abteilung wurden auf 0,1 VZÄ pro 100 Betten geschätzt.

Spinal muscular atrophy (SMA) is a severe, life-limiting neurodegenerative disease. A disease-modifying and approved therapy with nusinersen has been available in Germany since July 2017. Gene therapies offer another promising treatment option through a once in a lifetime administration. In May 2019 a gene replacement therapy for the treatment of SMA was approved for the first time by the U.S. Food and Drug Administration (FDA). An application for approval in Europe has been submitted and is currently pending.This consensus paper was compiled at the invitation of the German Society for Muscular Diseases (DGM) with the participation of all potential German neuromuscular treatment centers, the German section of the Society for Pediatric Neurology (GNP) and with the involvement of the medical scientific advisory board of the DGM. The aim was to define and establish the necessary prerequisites for a safe and successful application of the new gene replacement therapy in clinical practice.Gene replacement therapy with onasemnogene abeparvovec has the potential to significantly influence the course of SMA. Long-term data on sustainability of effects and possible adverse effects of gene replacement therapy are not yet available. The application of this innovative therapy must be carried out in specialized and appropriately qualified treatment centers under strict safety conditions. This article makes suggestions for the necessary framework conditions and gives recommendations for a systematic pretreatment and posttreatment assessment schedule under gene therapy. The effectiveness and safety of the therapy should be systematically documented in an industry-independent and disease-specific register.

Even in the era of modern guidelines, the treatment of rheumatic diseases is only as good as the framework of rheumatological care within which the treatment is carried out. The access to high-quality medical treatment for all patients is therefore essentially decisive for the prognosis of the patients. This article describes the current state of outpatient treatment in rheumatology and demonstrates which quality projects, such as treatment contracts, outpatient specialized medical treatment (ASV), digitalization and training as specialized rheumatological assistant (RFA), have been created in order to ensure the treatment of our patients. Furthermore, standards are defined that can guarantee a contemporary and guideline-conform treatment in outpatient rheumatological units. As an example it is an affirmation of the Professional Association of German Rheumatologists (BDRh) for ensuring optimal care for all rheumatology patients through early or emergency rheumatology clinics, treat to target, appropriate delegation of medical duties and diversification of treatment, thus an assurance of the quality and comprehensive treatment in rheumatology. The important topic of safeguarding the next generation of rheumatologists, which is indispensable for this, is also discussed.Auch im Zeitalter moderner Leitlinien ist die Behandlung rheumatischer Erkrankungen nur so gut wie die rheumatologische Versorgung, in deren Rahmen die Behandlung erfolgt. Der Zugang zu einer qualitativ hochwertigen Medizin für alle Patienten entscheidet daher wesentlich über die Prognose der Patienten. Die vorliegende Arbeit beschreibt den aktuellen Stand der ambulanten Versorgung in der Rheumatologie und zeigt auf, welche Qualitätsprojekte, wie z. B. Versorgungsverträge, ambulante spezialärztliche Versorgung (ASV), Digitalisierung, Ausbildung zur rheumatologischen Fachassistentin (RFA) u. a., geschaffen wurden, um die Versorgung unserer Patienten zu sichern. Darüber hinaus definiert sie Standards, welche in ambulanten rheumatologischen Einheiten eine zeitgemäße und leitliniengerechte Behandlung sicherstellen können. Als Leitbild ist sie ein Bekenntnis des BDRh zur Versorgungssicherheit aller rheumatologischer Patienten durch Früh- bzw. Notfallsprechstunden, Treat-to-Target, angemessener Delegation ärztlicher Leistungen und Verbreiterung der Versorgung, also einer Sicherung der Qualität und umfassenden Versorgung in der Rheumatologie. Auch auf das wichtige Thema der Sicherung des rheumatologischen Nachwuchses, welches dafür unabdingbar ist, wird eingegangen.

Die Nierenersatztherapie ist neben der Beatmung eines der wichtigsten und am häufigsten angewendeten Organersatzverfahren in der täglichen Praxis in der Intensivmedizin. Im Gegensatz zur Beatmungstherapie sind Qualitätsstandards für die Nierenersatztherapie weniger gut definiert und bekannt. In diesem Positionspapier der Deutschen Interdisziplinären Vereinigung für Intensivmedizin (DIVI) beschreiben wir Qualitätsstandards zur Nierenersatztherapie mit dem Ziel die Behandlungsqualität der Patienten mit einem schweren akuten Nierenversagen zu verbessern.

Sarcoidosis is a multisystemic granulomatous disorder which affects the respiratory system in the majority of the cases. Symptomatic cardiac manifestations are found in less than 10 % of the affected cohorts and show a large heterogeneity based on the ethnic background. Cardiac sarcoidosis is not only found in patients with rhythmogenic heart disease, such as atrial and ventricular fibrillation but also in all phenotypes of cardiomyopathy. The overall morbidity and mortality caused by cardiac sarcoidosis in Germany remains unclear and large prospective international observational studies.underline the importance of this disease entity. This consensus paper on diagnostic and therapeutic algorithms for cardiac sarcoidosis is based on a current literature search and forms an expert opinion statement under the auspices of the German Respiratory Society and the German Cardiac Society. The rationale of this statement is to provide algorithms to facilitate clinical decision-making based on the individual case situation.Während 90 % aller Sarkoidosepatienten eine parenchymatöse Beteiligung der Lunge aufweisen, ist die kardiale Sarkoidose (in weniger als 10 % der Fälle klinisch manifest) selten. Generell können alle Strukturen des Herzens von der Granulombildung betroffen sein. Meist jedoch sind das Myokard des linken Ventrikels sowie das Reizleitungssystem beeinträchtigt. Klinisch manifestiert sich die kardiale Sarkoidose als dilatative Kardiomyopathie oder in Form von Herzrhythmusstörungen wie Vorhofflimmern und/oder ventrikulären Tachykardien. Das Spektrum reicht von der benignen, subklinischen Manifestation als Zufallsbefund bis zur lebensbedrohlichen Komplikation, wie akuelle Kohortenstudien zeigen. Diagnostische Schritte und therapeutische Empfehlungen zur kardialen Sarkoidose sollten daher einem einheitlichen Standard unterliegen. Die vorliegende Arbeit ist die Expertenstellungnahme unter dem Schirm der Deutschen Gesellschaft für Pneumologie und Beatmungsmedizin (DGP) und der Deutschen Gesellschaft für Kardiologie – Herz- und Kreislaufforschung (DGK). Die nachstehenden Empfehlungen ersetzen jedoch nicht die ärztliche Begutachtung des individuellen Patienten und Anpassung der Diagnostik und Therapie an dessen spezifische Situation.

In Germany, there are currently many voices calling for a reform of hospital planning and reimbursement to correct some aberrations of the last decades and to enable the system to cope with future challenges. Some recent political decisions to change the structures of emergency medical services as well as the introduction of mandatory nurse-to-patient ratios and the exclusion of the cost for nursing from the case-based hospital reimbursement represent first steps of a reform, which also affects intensive care and emergency medicine. In this discussion paper a group of intensivists, emergency physicians, medical controllers, and representatives of nurses suggest more far-reaching changes, which can be summarized in 5 points: (1) General hospitals with intensive care units (ICU) and emergency departments (ED) which are part of the emergency medical system should be considered as an element of public service and be planned accordingly. (2) The planning of the intensive care infrastructure should be based on the three levels of emergency medical services to identify hospitals that are system relevant and to define appropriate criteria for structure and quality measures. (3) Hospital reimbursement should consist of a base amount (covering costs for hospital staff, infrastructure plus investments) and case-based fees (covering material costs). (4) To determine the requirements for nurses, physicians, and other medical staff, adequate tools for ICU and ED should be applied. (5) For these purposes as well as for quality management and optimal medical care, hospitals should be provided with a substantially improved IT-infrastructure.

This short review article aims to explain the changes in the treatment strategies of interventional care designed to treat secondary mitral valve regurgitation with edge-to-edge repair in comparison to the position paper published in 2013 by the German Society of Cardiology and the German Society of Thoracic and Cardiovascular Surgery. To this end the current data situation with respect to the European Society of Cardiology (ESC) guidelines from 2017, the intraprocedural assessment of mitral valve regurgitation and new technical developments are discussed.

The position paper of the working group 33/Telemonitoring in the German Society for Cardiology e. V. (DGK) discusses the importance of digital solutions in the German health care system and highlights the application possibilities and potentials of telemonitoring in the treatment of patients with cardiac diseases. In addition to telemonitoring of acute ischaemic diseases, acute coronary syndrome and acute cardiac arrhythmias, telemonitoring of chronic cardiac diseases is discussed. Chronic diseases, such as chronic heart failure, are age-associated and present society with the great challenge of providing high-quality, yet cost-efficient care to an increasing number of patients in the future. Telemonitoring offers an opportunity to meet this challenge. However, the introduction of telemonitoring and the associated changes for patients, doctors and other service providers must be accompanied by measures to ensure the acceptance of telemonitoring and the secure handling of sensitive data as well as the quality of telemonitoring services.

The proportion of elderly, frail, and multimorbid people has increased dramatically in recent decades resulting from demographic changes and will further increase, which will impact acute medical care. Prospective, randomized studies on geriatric intensive care are still lacking. There are also no international or national recommendations regarding the management of critically ill elderly patients. Based on an expert opinion, this consensus paper provides 16 statements that should be considered when dealing with geriatric critical care patients.

Difficulties in falling asleep and maintaining sleep, nonrestorative sleep and decreased daytime wakefulness represent very common but relatively unspecific health complaints. Around 100 specific sleep-related disorders will be classified in their own major chap. 7 (sleep wake disorders) for the first time in the upcoming 11th version of the International Classification of Diseases (ICD 11). With respect to the disciplines of psychiatry and psychotherapy there is a bidirectional relationship between mental health and sleep wake disorders. Sleep wake disorders can be an independent risk factor for the onset of a mental disorder and have a negative influence on the course of the disease. In addition, sleep wake disorders can also precede a mental disease as an early symptom and therefore be an important indication for early recognition. Many sleep wake disorders can be diagnosed based on the anamnesis and routine clinical investigations. In special cases, examination in a specialized sleep laboratory and treatment in a sleep medicine center following a staged care approach can be mandatory. Polysomnography represents the gold standard for the differential diagnostics; however, there is no legal foundation in the field of neuropsychiatric disorders for remuneration in the German healthcare system. This review summarizes the current guidelines with respect to the criteria for an investigation in a sleep laboratory from the perspective of the disciplines of psychiatry and psychotherapy. From this the requirements for guideline-conform diagnostics and treatment are derived.

Currently two intravitreally applied corticosteroids (dexamethasone and fluocinolone) are licensed in Germany for treatment of diabetic macular oedema (DME). The use of DEX implant for DME in daily clinical practice has not been defined in detail. Following a Delphi panel survey, a group of retina experts set out to come up with a consensus for use of the DEX implant in DME.International and national treatment recommendations were identified from the literature. A steering group generated a catalogue of 72 statements on the aetiology and pathogenesis of DME, therapy with DEX implant, use of DEX implant in patients previously treated with VEGF-inhibitors, use of DEX implant in combination therapy, safety of DME therapies as well as patients' burden of treatment. Twenty-two ophthalmologists from private practice and 6 hospital ophthalmologists participated in the Delphi panel via Survey Monkey. Consensus was reached if at least 75% of participants agreed or disagreed with a statement. Statements for which consensus was not reached were discussed once more during the expert consensus meeting and a vote was taken. Based on these results a treatment algorithm for foveal DME was proposed.If a patient does not show sufficient response after 3 - 6 months of anti-VEGF treatment (visual acuity gain of 5 ETDRS letters or reduction of central retinal thickness ≤ 20%), a switch to DEX implant should take place. DEX implant is also suitable in eyes with longer presentation of DME, showing e.g. massive lipid exudates. DEX implant is suitable as first-line therapy especially in pseudophakic patients, patients unwilling or able to comply with tight anti-VEGF injection intervals or patients with known vascular diseases. With fixed control visits every 4 - 8 weeks, use of DEX implant is flexible and individual. Decision parameters for repeated use should be visual acuity, retinal thickness and intraocular pressure. Treatment of both eyes on the same day should not take place.The algorithm presented reflects survey as well as expert discussion results and may differ from recommendations issued by the German professional society. The consensus recommendations for the treatment of DME generated during the survey and meeting of retina experts are intended to guide use of DEX implant in daily practice.In Deutschland sind 2 intravitreal anzuwendende Kortikosteroide (Dexamethason und Fluocinolon) für die Behandlung des diabetischen Makulaödems (DMÖ) zugelassen. Die Definition des Konsenses zu einer Aussage galt als erfüllt, wenn mehr als 75% der Befragten einer Aussage zustimmten bzw. nicht zustimmten. Über nicht konsensfähige Aussagen wurde nach erneuter Diskussion in der Konsensrunde nochmals abgestimmt. In der Folge sollte auch ein Behandlungsschema für DMÖ mit fovealer Beteiligung vorgeschlagen werden.Sofern ein Patient unter VEGF-Hemmern unzureichend anspricht (Visusgewinn 5 ETDRS-Buchstaben oder Reduktion der zentralen Netzhautdicke ≤ 20%), sollte nach 3 – 6 Monaten auf das DEX-Implantat umgestellt werden. DEX-Implantat ist ebenfalls geeignet für Augen mit länger bestehendem DMÖ, bei denen z. B. massive Lipidexsudate vorhanden sind. DEX-Implantat eignet sich als Ersttherapie insbesondere bei Pseudophaken, Patienten, die nicht willens oder in der Lage sind, enge Injektionsintervalle unter Anti-VEGF-Therapie einzuhalten oder für Patienten mit vaskulären Vorerkrankungen. Bei festgelegten Kontrollintervallen von 4 – 8 Wochen kann die Anwendung des DEX-Implantats flexibel und individuell erfolgen. Als Entscheidungsparameter gelten hierbei neben Visus und Netzhautdicke auch der Augeninnendruck. Eine Behandlung von beiden Augen am selben Tag sollte nicht stattfinden.Der hier vorgestellte Algorithmus entspricht den Ergebnissen des Delphi-Prozesses und weicht u. U. von den Empfehlungen der Fachgesellschaft ab. Die im Rahmen der Befragung und Expertenrunde erarbeiteten Konsensempfehlungen für die Therapie von DMÖ können für den Einsatz des DEX-Implantats in der täglichen Praxis dienen.

Since March 2017, the prescription of medical cannabis at the expense of the statutory health insurance is possible after approval by the respective medical services. Chronic pain is the most common indication, as health claims data and the accompanying survey show. From the point of view of the law, a prescription is indicated in cases of serious illness, missing or not indicated established therapeutic approaches and a not entirely remote prospect of improvement of the illness or its symptoms. This describes a broader indication spectrum than can currently be based on randomised controlled clinical trials. There is weak evidence of low efficacy for neuropathic pain. For pain related to spasticity and cancer-related pain there is evidence of improvements in quality of life, but effects on pain are of little relevance. For all other indications, only an individual therapeutic trial can be justified based on the available external evidence. However, this usually corresponds to the demand of "a not entirely remote prospect" of a noticeably positive effect of medical cannabis. It is also problematic that almost no long-term studies for the application and efficacy of flowers and extracts are available.Current knowledge on the use of cannabis-based drugs and, more clearly, medical cannabis for chronic pain is insufficient. The increase in the number of countries with marketing authorisations or exemptions for medicinal cannabis or cannabis-based drugs for chronic pain will also pave the way for larger empirical and population-based studies that will further improve the evidence base of research and clinical use.

Incontinence-associated dermatitis (IAD) describes damage to the skin caused by repeated and prolonged contact with urine and/or feces. Patients of all ages can be affected; the diagnosis is preferably used in adults and adolescents. The predilection sites are perineal, perianal, the inner thighs and the convex areas of the buttocks. Increased moisture on the skin surface, increased pH, occlusion, feces digestive enzymes and repeated skin cleansing procedures are pathophysiologically relevant factors. Typical clinical signs of IAD are erythema, erosions, excoriations and pain. Diagnosis is often difficult because there are many relevant differential diagnoses, some of which may occur together with an IAD. In particular, pressure ulcer, contact dermatitis and intertrigo need to be differentiated and treated. Effective strategies of prevention and therapy of IAD are continence management, the use of efficient, absorbent products as well as consistent skin protection and care. IAD is a skin disease that is often not or incorrectly diagnosed. It should be treated as early as possible, together with the causative factors such a fecal and/or urinary incontinence, as otherwise it can lead to an unnecessarily long duration and severity of the symptoms.

Nowadays, childhood cancer patients survive much more often than they did 40 years ago. Therefore, the cure rates rise over 80%. Approximately 33,000 cured childhood cancer patients are documented by the German Childhood Cancer Registry (GCCR) for long-term follow-up in Germany. But does that mean, they are healthy, too? When compared to the normal population, it can be seen that morbidity and mortality are significantly higher among former childhood cancer patients. In two out of three survivors, the cancer and its treatments can lead to treatment-related late complications 30 years later; in about one-third, these late complications are classified as severe. A structured long-term follow-up is needed to detect and to treat new diseases early. The purpose of this position paper is to update the first position paper from 2007. Current developments are described, how appropriate aftercare structures and aftercare facilities should be planned, taking into account existing structures and increasing needs.Krebskranke Kinder und Jugendliche überleben heute dank Heilungsraten von über 80% viel häufiger ihre Krebserkrankung als vor 40 Jahren. Etwa 33 000 geheilte ehemalige krebskranke Kinder und Jugendliche sind in der Langzeitbeobachtung über das Deutsche Kinderkrebsregister (DKKR) bundesweit erfasst. Heißt geheilt aber auch gesund? Beim Vergleich mit der Normalbevölkerung ist festzustellen, dass die Morbidität und Mortalität bei den ehemaligen Patienten deutlich erhöht ist. Die Krebserkrankungen und deren Behandlungen können bei zwei von drei Überlebenden 30 Jahre später zu therapiebedürftigen Spätfolgen führen, bei etwa einem Drittel werden sie als schwer eingestuft. Mittels einer strukturierten Nachbeobachtung können diese frühzeitig erkannt und bei Bedarf behandelt werden. Ziel dieses Positionspapiers ist eine Aktualisierung des ersten Positionspapiers aus dem Jahre 2007. Hierbei werden die aktuellen Entwicklungen beschrieben und dargestellt, wie geeignete Nachsorgestrukturen und Nachsorgeeinrichtungen – unter Berücksichtigung der bestehenden Strukturen und dem steigenden Bedarf – geplant werden sollten.

The joint position paper of the working community "Cardiovascular Nuclear Medicine" of the German Society of Nuclear Medicine (DGN) and the working group "Nuclear Cardiology Diagnostics" of the German Cardiac Society (DKG) updates the former 2009 paper. It is the purpose of this paper to provide an overview about the application fields, the state-of-the-art and the current value of nuclear cardiology imaging. The topics covered are chronic coronary artery disease, including viability imaging, furthermore cardiomyopathies, infective endocarditis, cardiac sarcoidosis and amyloidosis.

Geriatric syndromes are the pathognomonic columns of geriatric medicine. In contrast to many syndromes in younger people, in geriatric patients, the chief complaint does not typically represent the specific pathological condition underlying the change in health status. Geriatric syndromes are usually highly prevalent, multicausal and share a number of common risk factors. In recent years, scientific controversy over anemia in the aged has revealed a high prevalence in geriatric patients, which prompted the "working group on anemia" to publish its first position paper at a European level. The development of anemia is multicausal and the causes of the various forms of anemia range from iron deficiency, malnutrition, chronic inflammation, hormonal dysregulation, functional organ disorders, impaired synthesis to malignancies. The corresponding pathomechanisms are closely associated with the development of other geriatric syndromes such as gait disorders, sarcopenia, frailty, and falls. Against this backdrop, the "working group on anemia" of the German Geriatric Society has devised a second position paper:"Multicausality and the significant association between anemia and assessment-based quantifiable impairments suggest the consideration of anemia in the aged to be a geriatric syndrome."

Injuries, especially traumatic brain injury, or specific illnesses and their respective sequelae can result in the demise of the patients afflicted despite all efforts of modern intensive care medicine. If in principle organ donation is an option after a patient's death, intensive therapeutic measures are regularly required in order to maintain the homeostasis of the organs. These measures, however, cannot benefit the patient afflicted anymore-which in turn might lead to an ethical conflict between dignified palliative care for him/her and expanded intensive treatment to facilitate organ donation for others, especially if the patient has opted for the limitation of life-sustaining therapies in an advance directive.The Ethics Section and the Organ Donation and Transplantation Section of the German Interdisciplinary Association of Critical Care and Emergency Medicine (DIVI) have convened several meetings and a telephone conference and have arrived at a decision-making aid as to the extent of treatment for potential organ donors. This instrument focusses first on the assessment of five individual dimensions regarding organ donation, namely the certitude of a complete and irreversible loss of all brain function, the patient's wishes as to organ donation, his or her wishes as to limiting life-sustaining therapies, the intensity of expanded intensive treatment for organ protection and the odds of its successful attainment. Then, the combination of the individual assessments, as graphically shown in a {Netzdiagramm}, will allow for a judgement as to whether a continuation or possibly an expansion of intensive care measures is ethically justified, questionable or even inappropriate.The aid described can help mitigate ethical conflicts as to the extent of intensive care treatment for moribund patients, when organ donation is a medically sound option. The regulations for ability to drive with cerebrovascular diseases in the German Driving License Regulations (Fahrerlaubnisverordnung, FeV) and German Guidelines for the Evaluation of Driving Ability of the Federal Highway Research Institute (BASt) are not up to date with the current medical knowledge and are not consistent with comparable regulations regarding cardiovascular diseases. This is particularly true for the assessment of future risks for a sudden loss of control during driving. The present position paper of six medical and neuropsychological societies in Germany presents the current conditions for the assessment of driving ability of patients a cerebrovascular diesease and recommends an estimation of the ability to drive founded on the current state of scientific knowledge. It addresses the following: 1. Physical and mental functional limitations and the possibilities for compensation, which if necessary enable a fitness to drive under conditions or within limits, including the importance of behavioral or personality changes and cognitive deficiencies that interfere with safety. 2. The potential danger due to a sudden loss of control as a result of a transient ischemic attack (TIA) new stroke event, or another cardiovascular event while driving. A summary in the form of a table provides physicians and expert assessors with assistance for the most important cerebrovascular diseases.

The AWMF and its medical societies perceive an increasing dominance of economic targets in the hospital health care sector, leading to impairment of patient care. While resource use in health care should be appropriate, efficient and fairly allocated, "economization" creates a burdensome situation for physicians, nurses and other health care professionals.The AMWF and the medical societies studied causes and developed measures for a scientific, patient-centred and resource-conscious medical care. Disincentives due to the remuneration system, number and equipment of hospitals resp. specialist departments and their basic funding need to be overcome. Proposed actions relate to the patient-doctor-level, the management level of hospitals and the level of planning and financing hospitals including compensation of hospital care. To place patients and their health in the forefront again, joint efforts of all stakeholders in health care are needed.Die AWMF und ihre Fachgesellschaften nehmen eine zunehmende Dominanz betriebswirtschaftlicher Ziele im stationären Gesundheitssektor wahr, die sich negativ auf die Patientenversorgung auswirken. Während eine angemessene, effiziente und gerechte Mittelverwendung geboten ist, belastet „Ökonomisierung“ Ärzte und Pflegende sowie die weiteren Gesundheitsberufe.Die AWMF hat zusammen mit Fachgesellschaftsvertretern Ursachen analysiert und Maßnahmen für eine wissenschaftliche, patientenzentrierte und ressourcenbewusste Medizin erarbeitet. Es bestehen Fehlanreize durch Vergütung, Zahl und Ausstattung von Krankenhäusern bzw. Fachabteilungen und deren Grundfinanzierung. Maßnahmen werden für die Patient–Arzt-Ebene, das Krankenhausmanagement, die Planung sowie Finanzierung von Krankenhäusern einschließlich der Vergütung stationärer Leistungen vorgeschlagen. Dabei bedarf es der gemeinsamen Anstrengung aller Akteure, um den Patienten und seine Gesundheit wieder in den Mittelpunkt zu stellen.

Recent epidemiological investigations have shown that approximately 2-3% of all Austrians suffer from diabetes with renal involvement, i. e. 250,000 people in Austria are affected. The risk of occurrence and progression of this disease can be ameliorated by life style interventions as well as optimization of blood pressure, blood glucose levels and special drug classes. The present article represents the joint recommendations of the Austrian Diabetes Association and the Austrian Society for Nephrology for the diagnostics and treatment strategies of diabetic kidney disease.

Demographic changes in Germany means that nursing and medical care is becoming increasingly complex. This is especially true for intensive care. Despite the closure of hospitals in Germany, the number of beds in intensive care units rose between 2002 from 23,113 to 26,162 in 2010. At the same time the number of patients treated in intensive care units increased by 148,989 to 2,049,888. The increasing complexity requires specific education for nurses in intensive care based on a model of advanced nursing practice (ANP). Nursing experts who fulfill an advanced practise nursing are competent to make decisions autonomously and accountably within complex care situations. This includes decisions such as artificial ventilation, weaning, nutrition management and the management of delirium, wounds and pain. Nursing experts are responsible for specific clearly defined areas of care and initiate nurse-led services and practice. Scientific and research based evidence are transferred directly into health care practice and can be implemented directly. The DGF demands a scientific and with it academic development of critical care as advanced nursing practice based on a master level qualification. This also includes the request for expansion of competencies and roles and the assignment of decision making authority within complex health care situations and an orientation on the Competencies for European Critical Care Nurses of the European Federation of Critical Care Nursing Associations (EfCCNa).
