Einleitung: Die Atopische Dermatitis (AD) ist eine chronisch verlaufende Hauterkrankung, welche die Gesundheit und Lebensqualität von Patient*innen stark beeinträchtigt. Trotz leitliniengerechter Behandlung lassen sich AD Symptome nicht immer ausreichend lindern, daher wenden AD-Patient*innen zusätzlich komplementärmedizinische Verfahren wie beispielsweise Akupunktur an. Ziel dieser Studie war es, die Wirksamkeit von Akupunktur bei AD-Patient*innen zu untersuchen und Daten als Vorbereitung für größere Folgestudien zu gewinnen. Methodik: In dieser monozentrischen, randomisierten, kontrollierten, explorativen Studie erhielt die Interventionsgruppe (IG) acht Akupunkturbehandlungen innerhalb eines 12-wöchigen Interventionszeitraumes. Die Kontrollgruppe (KG) erhielt in den ersten 12 Wochen keine studienspezifische Intervention. Diese Studie ist ein Teil einer dreiarmigen Studie, in der auch die Wirksamkeit der Osteopathie untersucht wurde. Der Vergleich Osteopathie vs. KG ist Teil einer weiteren Doktorarbeit. Folgende Zielparameter wurden zu den Zeitpunkten 6, 12 und 24 Wochen erhoben: Mittlerer subjektiv empfundener Juckreiz auf einer Visuellen Analog Skala (VAS Juckreiz, 0 mm = kein Juckreiz, 100 mm = maximaler Juckreiz); Schwere der AD nach Score of Atopic Dermatitis (SCORAD, 0 Punkte = leicht betroffen, 103 Punkte = schwer betroffen) Index, spezifische gesundheitsbezogene Lebensqualität anhand des Dermatology Life Quality Index (DLQI, 0 = keine Beeinträchtigung, 30 = maximale Beeinträchtigung). Ergebnisse: In die IG wurden 39 Patient*innen (Durchschnittsalter 31,4 ± 10,9 Jahre; 71,8% weiblich) und in die KG 42 Patient*innen (31,1 ± 10,8 Jahre; 73,8% weiblich) randomisiert. Die Mittelwerte (MW) der VAS Juckreiz waren nach Abschluss der Interventionsphase zur Woche 12 in der IG mit 28,0 mm (Standardabweichung (SD) ± 23,8 mm) deutlich niedriger als in der KG mit 42,2 mm (SD ± 22,6 mm). Die MW des SCORAD-Index stellten sich zur Woche 12 mit 21,3 Punkten (SD ± 13,8 Punkte) in der IG und 25,8 Punkten (SD ± 14,7 Punkte) in der KG ohne deutlichen Unterschied dar. Der DLQI zeigte im Intergruppenvergleich zur Woche 12 leicht bessere (niedrigere) MW in der IG mit 5,0 Punkten (SD ± 3,3 Punkte) als in der KG mit 6,7 Punkten (SD ± 9,4 Punkte). Schlussfolgerung: Die Studienergebnisse dieser explorativen Studie zeigten Hinweise auf eine Wirksamkeit der Akupunktur hinsichtlich einer Linderung des Juckreizes. Dagegen fand sich bezüglich der Schwere der AD sowie der krankheitsspezifischen Lebensqualität keine bzw. nur geringe Hinweise auf eine Wirksamkeit. Insbesondere die Wirksamkeit der Akupunktur auf den Juckreiz bei AD sollten in qualitativ hochwertigen, multizentrischen, kontrollierten, konfirmatorischen randomisierten Studien überprüft werden, Daten aus dieser Studie können für eine solide Fallzahlschätzung eingesetzt werden., Introduction: Atopic dermatitis (AD) is a common, often chronic skin disease that severely affects patients' health and quality of life. Despite guideline-based treatment, AD symptoms are not always sufficiently alleviated, which is why AD patients also use complementary medicine methods such as acupuncture. The aim of this study was to investigate the effectiveness of acupuncture in AD patients and to obtain data in preparation for larger follow-up studies. Methods: In this monocentric, randomised, controlled, exploratory study, the intervention group (IG) received eight acupuncture treatments during a 12-week intervention period. The control group (KG) received no study-specific intervention during the first 12 weeks. This study is part of a three-arm study that also investigated the effectiveness of osteopathy. The comparison of osteopathy vs. KG is part of another doctoral thesis. The following target parameters were collected at the time points of 6, 12 and 24 weeks: Mean subjectively felt itch on a Visual Analogue Scale (VAS itch, 0 mm = no itch, 100 mm = maximum itch); severity of AD according to Score of Atopic Dermatitis (SCORAD, 0 points = mildly affected, 103 points = severely affected) index, specific health-related quality of life using the Dermatology Life Quality Index (DLQI, 0 = no impairment, 30 = maximum impairment). Results: 39 patients (mean age 31.4 ± 10.9 years; 71.8% female) were randomised to IG and 42 patients (31.1 ± 10.8 years; 73.8% female) to KG. The mean values (MW) of the VAS itch at the end of the intervention phase at week 12 were significantly lower in the IG with 28.0 mm (standard deviation (SD) ± 23.8 mm) than in the KG with 42.2 mm (SD ± 22.6 mm). The MW of the SCORAD index at week 12 was 21.3 points (SD ± 13.8 points) in the IG and 25.8 points (SD ± 14.7 points) in the KG, with no clear difference between them. The DLQI showed slightly better (lower) MW in the IG with 5.0 points (SD ± 3.3 points) than in the KG with 6.7 points (SD ± 9.4 points) in the intergroup comparison at week 12. Conclusion: The study results of this explorative study showed clear indications of an effectiveness of acupuncture with regard to relief of itching. In contrast, there was little or no evidence of effectiveness with regard to the rated severity of AD and disease-specific quality of life. In particular, the effectiveness of acupuncture on itch in AD should be tested in high-quality, multicenter, controlled, confirmatory randomised trials. Data from this trial can be used for case number estimation in follow-up studies.

KurzfassungHintergrund: Studierende des Studiengangs Gesundheits- und Krankenpflege sind durch eine Berufsausbildung, die mit großer Verantwortung einhergeht, einem erhöhten Stresslevel ausgesetzt und leiden zudem häufig an Angstzuständen sowie Depressionen. Die regelmäßige Konfrontation mit Stress wirkt sich auf die psychische und physische Gesundheit aus und kann darüber hinaus einer effektiven Patientenversorgung entgegenwirken. Unterschiedliche Studien der letzten Jahre zielen darauf ab, Achtsamkeit und Meditation in Form der Interventionen „MBSR“ und „MM“ bei dieser Studierendengruppe einzusetzen und veranschaulichen eine effektive Reduktion von Stress, Angst sowie Depression und ermöglichen durch eine erhöhte Aufmerksamkeitskapazität mehr Patientensicherheit.Forschungsfrage: „Welche Auswirkung hat der Einsatz von Achtsamkeit und Meditation auf psychische Belastungen bei Studierenden des Studiengangs Gesundheits- und Krankenpflege?“Methode: Als Leitfaden des methodischen Vorgehens konnte eine Empfehlung von Nordhausen und Hirt (2020) herangezogen werden. Anhand des PIKO-Schemas wurden Suchkomponenten des Suchstrangs festgelegt, sowie die Forschungsfrage der vorliegenden Arbeit formuliert. Die Studienselektion erfolgte anhand eines Titel- und Abstract Screenings unter Berücksichtigung gewählter Ein- und Ausschlusskriterien, einem Entfernen von Duplikaten, einem Volltextscreening und letztlich einer Bewertung inkludierter Studien. Neben einer elektronischen Datenbanksuche sowie einer Handsuche, wurden auch Bücher und Internetquellen zur Bearbeitung des Hintergrundes herangezogen.Ergebnisse: Die sechs inkludierten Studien befassen sich mit den zwei benannten Interventionen in Bezug auf Achtsamkeit und Meditation. Es konnten signifikante Ergebnisse zur Linderung von Stress und Angst sowie Depression festgestellt werden.Schlussfolgerung: Die Ergebnisse dieser Literaturarbeit belegen, dass der Einsatz von Achtsamkeit und Meditation eine Möglichkeit darstellt, um psychische Belastungen von Studierenden des Studiengangs Gesundheits- und Krankenpflege effektiv zu reduzieren. Regular confrontation with stress impacts mental and physical health and may also prohibit effective patient care. Different studies in recent years aim to use mindfulness and meditation in the form of the interventions "MBSR" and "MM" with this group of students and illustrate effective reduction of stress, anxiety as well as depression and enable more patient safety through increased attentional capacity.Aim: What are the effects of using mindfulness and meditation on psychological bur-dens in nursing students?Methods: A recommendation by Nordhausen and Hirt (2020) was used as a guide for the methodological procedure. Using the PIKO scheme, search components of the search strand were determined and the research question of the present work was created. The study selection was based on a title and abstract screening in consideration of inclusion and exclusion criteria, a removal of duplicates, a full text screening and finally an assessment of included studies. In addition to an electronic database search as well as a manual search, books and internet sources were consulted to process the background.Results: The six included studies address the two previous named interventions related to mindfulness and meditation. Significant results were found for alleviating stress and anxiety as well as depression. Conclusion: Results of this literature review prove that the use of mindfulness and meditation is a way to effectively reduce psychological stress symptoms among students in the nursing curriculum. Ziel. Zu evaluieren, ob die Ergebnisse aktueller randomisierter klinischer Studien (RCT) zur rheumatoiden Arthritis (RA) und Arthrose auf die allgemeine Patientenpopulation bezüglich Alter und Geschlecht generalisierbar sind. Methoden. Dieses systematische Review mit Meta-Analyse wurde zu Beginn im internationalen Protokollregister für systematische Reviews, PROSPERO, registriert (Identifizierungscode CRD42018085409). Die medizinische Datenbank MEDLINE wurde systematisch nach RCT (Jahre 2016 und 2017) und populationsbasierten Studien (PBS; Jahre 2013 bis 2017) durchsucht. Zusätzlich wurde eine Handsuche nach relevanten Artikeln durchgeführt. Der Anteil älterer Menschen (definiert als ≥65 Jahre alt) wurde unter Annahme einer trunkierten Normalverteilung geschätzt, falls er nicht im Forschungsartikel berichtet wurde. Das Risiko für Bias der eingeschlossenen PBS wurde durch die Joanna Briggs Institute Critical Appraisal Checklist for Prevalence Studies beurteilt. Mithilfe von random-effects Meta-Analysen wurden der durchschnittliche Anteil älterer Menschen an den Studienpopulationen, das Durchschnittsalter, dessen Standardabweichung (SA) und der durchschnittliche Anteil an Frauen geschätzt – stratifiziert nach Krankheit (RA und Arthrose) und Studientyp (RCT und PBS). Diese stratifizierten Schätzungen wurden anschließend verglichen und mittels zwei-Stichproben Z-Tests auf signifikante Unterschiede geprüft. Zusätzlich wurde evaluiert, ob der Anteil älterer Menschen mit der Art der Studienfinanzierung und der Art der Intervention zusammenhängt. Ergebnisse. Es wurden 265 RCT und 53 PBS mit entsprechend 51.240 bzw. 523.630 Teilnehmern eingeschlossen. Sowohl RCT zur RA als auch zur Arthrose schlossen signifikant weniger ältere Menschen ein als PBS: Die Proportionen unterschieden sich um –0.18 (95% Konfidenzintervall –0.22 bis –0.13) bei RA und um –0.20 (–0.30 bis –0.09) bei Arthrose. Dementsprechend berichteten RCT auch niedrigere Durchschnittsalter: RA –5.2 Jahre (–6.8 bis –3.5); Arthrose –4.7 Jahre (–7.5 bis –2.0); und schmalere SA: RA –1.9 Jahre (–2.6 bis –1.3); Arthrose –2.7 Jahre (–4.2 bis –1.2); (alle Vergleiche: p ≤ 0.001). Der Anteil weiblicher Teilnehmer in RCT unterschied sich bei beiden Krankheiten nicht signifikant von den jeweiligen PBS. Die Art der Studienfinanzierung übte keinen Einfluss auf das Defizit an älteren Menschen in RCT aus, die Art der Intervention jedoch bei Studien zur RA (p = 0.02), nicht aber bei Studien zur Arthrose (p = 0.60). Fazit. Im Gegensatz zu Frauen sind ältere Menschen in klinischen Studien zur RA und Arthrose unterrepräsentiert. Aktuelle Evidenz ist daher nur begrenzt auf diese wachsende Patientengruppe generalisierbar. To assess whether the results of current trials in rheumatoid arthritis (RA) and osteoarthritis (OA) are generalizable to the general patient population with regard to age and sex. Methods. This systematic review and meta-analysis was preregistered with the international protocol registry for systematic reviews, PROSPERO (identifier CRD42018085409). MEDLINE was systematically searched for randomized controlled trials (RCT) on any intervention (years 2016 and 2017) and for population-based studies (PBS; years 2013 to 2017) in both RA and OA, complemented by a hand search. If not reported in the original research manuscript, the proportion of elderly people (defined as being ≥65 years old) was estimated from an assumed truncated normal distribution. The risk of bias of included PBS was evaluated with the Joanna Briggs Institute Critical Appraisal Checklist for Prevalence Studies. Random effects meta-analyses estimated the pooled proportion of elderly people, the mean age, its standard deviation (SD), and the proportion of women stratified by disease (RA and OA) and study type (RCT and PBS). These stratified estimates were subsequently compared by two-sample Z-tests. Additionally, the influence of the type of funding and intervention on the proportion of elderly people was assessed. Results. The search yielded 265 RCT and 53 PBS comprising 51,240 and 523,630 participants, respectively. In both diseases, RCT included significantly fewer elderly people than PBS: Proportions differed by –0.18 (95% confidence interval –0.22 to –0.13) in RA and –0.20 (–0.30 to –0.09) in OA. Correspondingly, RCT had lower mean ages: RA –5.2 years (–6.8 to –3.5); OA –4.7 years (–7.5 to –2.0); and smaller SD: RA –1.9 years (–2.6 to –1.3); OA –2.7 years (–4.2 to –1.2); (all comparisons: p ≤ 0.001). Proportions of women did not differ significantly between RCT and PBS in both RA and OA. Funding source did not influence the deficit of elderly people in RCT, however, the type of intervention did in RA (p = 0.02) but not in OA (p = 0.60) trials. Conclusion. While adequate proportions of women are included, the elderly are underrepresented in RA and OA trials. Current evidence is thus only limitedly generalizable to this growing subgroup of patients. It is important to study age as a determinant for outcome and to improve the inclusion of elderly people in RA and OA clinical trials.

Daten gelten gemeinhin als essenzieller Baustein für die digitale Transformation. Anzahl und Diversität der Datenquellen steigen sukzessive und somit auch das Datenvolumen. Dieser Trend gilt auch für den Gesundheitsbereich. Es entstehen laufend neue Daten, die für die Gesundheitsversorgung zunehmend eine Rolle spielen bzw. spielen könnten. Eine spezielle Kategorie von Gesundheitsdaten stellen die so genannten Real-World Data (RWD) dar, routinemäßig gesammelte Daten zum Gesundheitszustand einer Person bzw. zur Dokumentation von Gesundheitsleistungen. Vorliegende Forschungsarbeit befasst sich mit einer Bestandsaufnahme der Erwartungshal-tung an den Nutzen der Verwendung von RWD in der klinischen Forschung seitens beteiligter Akteurinnen und Akteure. Welches Verständnis haben diese vom Begriff und von der Bedeu-tung von RWD für die klinische Forschung? Welche Erwartungen/Befürchtungen knüpfen sich daran? Die Thematik wird aus einer kritischen Public-Health-Perspektive behandelt: Das bedeutet die Entscheidung der Verwendung oder Nicht-Verwendung von RWD in Klinischen Studien (KS) ist an das Ziel zu koppeln, allen Patientinnen und Patienten die bestmögliche Behandlung und Medikation dauerhaft zur Verfügung zu stellen. Den theoretischen Hintergrund bilden sozialwissenschaftliche Diskussionen der Arten, Quellen und Komplexität von Gesundheitsdaten sowie Hintergrundwissen zur Praxis klinischer Forschung. Auf dieser Basis werden Einsatzmöglichkeiten von RWD in der klinischen Forschung in Theorie und Praxis untersucht. Für die Forschungsarbeit wurden qualitative Forschungsmethoden ausgewählt und folgende Erhebungsmethoden eingesetzt: Eine Desk Research-basierte Aufarbeitung der Fachpublikationen zur Thematik RWD in KS zeigt Use Cases, Herausforderungen sowie Positionen relevanter Stakeholder/-innen auf. Parallel dazu wurden halbstrukturierte Interviews mit ausgewählten Akteurinnen und Akteuren aus dem österreichischen Gesundheitswesen geführt und inhaltsanalytisch nach Mayring (2015) ausgewertet. Die Auswertungen zeigen, dass ein Einbeziehen von Daten, die nicht unter den Laborbedingungen klassischer KS erhoben, sondern der Alltagsroutine der Gesundheitsversorgung oder dem Alltag einzelner Personen entnommen werden, eine Art Paradigmenwechsel in der klinischen Forschung bedeutet. Der bisherige Goldstandard randomisierter kontrollierter Studien steht zur Diskussion. Dementsprechend unterschiedlich gestalten sich die Erwartungshaltungen. An die tendenziell eher positive Grundhaltung an einen Nutzen knüpfen sich viele Fragestellungen bezüglich Datenzugang und Schnittstellen, einheitlich geregelter Data Governance sowie qualitätsgesicherter Erhebung und Dokumentation von RWD. Viele Vorleistungen technischer, methodischer und prozesshafter Natur werden als nötig erachtet. Wer diese konkret in Angriff nehmen soll, bleibt offen. Ein sehr hoher Wert wird der Einbeziehung von patientenseitig zur Verfügung gestellten Daten beigemessen, da diese konkret widerspiegeln, wie Behandlungen im Alltag empfunden werden. Nutzung und damit Aufwertung dieser RWD führe darüber hinaus zu einer Stärkung von Patientinnen/Patienten. Diese erste Erhebung von Erwartungshaltungen an den Nutzen von RWD in KS gibt eine Standortbestimmung in dem derzeit laufenden Fachdiskurs wieder. The number and diversity of data sources are continuously increasing, and so is the volume of data. This trend also applies to the healthcare sector. New data is constantly being created that plays or could play a role in healthcare. A special category of health data is Real-world data (RWD), routinely collected data on the health status of a person and or the delivery of healthcare routinely collected from a variety of sources. The research project investigates expectations of the use of RWD in clinical trials on the part of the actors involved. What is their understanding of the concept and of the importance of RWD for clinical research? What are their expectations? The topic is considered from a critical public health perspective: the value of using RWD in clinical trials must be measured against the overarching goal of providing all patients with the best possible treatment and medication. The theoretical background is provided by social science discussions of the types, sources, and complexity of health data, as well as background knowledge of clinical research practice. Possible applications of RWD in clinical research are examined in theory and practice. A desk research-based review of publications on the topic of RWD in clinical trials identifies use cases, challenges and positions of relevant stakeholders. Moreover semi-structured in-terviews were conducted with selected stakeholders in Austria and the outcomes subject to qualitative content analysis according to Mayring (2015). The results show that including data taken from everyday healthcare routines instead of the laboratory conditions of classical clinical trials, represents a paradigm shift in clinical research. The gold standard of randomized controlled trials is at stake, therefore expectations vary accordingly. A basically positive attitude towards the benefits of using RWD in clinical trials is linked to many questions regarding data access and interoperability, standardized data governance, and quality-assured collection and documentation of data. Many technical, methodological and process-related preliminary steps seem necessary to gain benefit from RWD. The question remains who will tackle these in concrete terms. Expectations are particularly high with regard to patient-supplied data, since these reflect how treatments are experienced in everyday life, and thus the valorisation of RWD also leads to an empowerment of patients. Background The number of total knee replacements (TKRs) and total hip replacements (THRs) has been increasing noticeably in high-income countries, such as Germany. In particular, the number of revisions is expected to rise because of higher life expectancy and procedures performed on younger patients, impacting the budgets of health-care systems. Quality transparency is the basis of holistic patient pathway optimization. Nevertheless, a nation-wide cross-sectoral assessment of quality from a patient perspective does not yet exist. Several studies have shown that the use of patient-reported outcome measures (PROMs) is effective for measuring quality and monitoring post-treatment recovery. For the first time in Germany, we test whether early detection of critical recovery paths using PROMs after TKR/THR improves the quality of care in a cost-effective way and can be recommended for implementation into standard care. Methods/design The study is a two-arm multi-center patient-level randomized controlled trial. Patients from nine hospitals are included in the study. Patient-centered questionnaires are employed to regularly measure digitized PROMs of TKR/THR patients from the time of hospital admission until 12 months post-discharge. An expert consortium has defined PROM alert thresholds at 1, 3, and 6 months to signal critical recovery paths after TKR/THR. An algorithm alerts study assistants if patients are not recovering in line with expected recovery paths. The study assistants contact patients and their physicians to investigate and, if needed, adjust the post-treatment protocol. When sickness funds’ claims data are added, the cost-effectiveness of the intervention can be analyzed. Discussion The study is expected to deliver an important contribution to test PROMs as an intervention tool and examine the determinants of high-quality endoprosthetic care. Depending on a positive and cost-effective impact, the goal is to transfer the study design into standard care. During the trial design phase, several insights have been discovered, and there were opportunities for efficient digital monitoring limited by existing legacy care models. Digitalization in hospital processes and the implementation of digital tools still represent challenges for hospital personnel and patients. Furthermore, data privacy regulations and the separation between the in- and outpatient sector are roadblocks to effectively monitor and assess quality along the full patient pathway. IntroductionCongenital heart disease (CHD) is a major cause of infant mortality. Many infants with CHD require corrective surgery with most operations requiring cardiopulmonary bypass (CPB). CPB triggers a systemic inflammatory response which is associated with low cardiac output syndrome (LCOS), postoperative morbidity and mortality. Delivery of nitric oxide (NO) into CPB circuits can provide myocardial protection and reduce bypass-induced inflammation, leading to less LCOS and improved recovery. We hypothesised that using NO during CPB increases ventilator-free days (VFD) (the number of days patients spend alive and free from invasive mechanical ventilation up until day 28) compared with standard care. Background: Individual placement and support (IPS) has proven to be effective for vocational outcomes in people with mental illness. The original concept of IPS requires temporally unlimited provision of support. Using limited placement budgets and investigating factors that predict their effectiveness may inform decisions about resource allocation. Methods: A range of patient characteristics were tested as predictors of employment outcomes in participants who attended six outpatient psychiatric clinics in Switzerland between June 2010 and May 2011. Overall, 116 patients with the full spectrum of psychiatric conditions were randomly assigned and started an IPS intervention, which was provided by three different placement budgets. Support lasted 2 years for those who found a job, and outcomes were repeatedly assessed over 3 years. The intervention ended for those who failed to find competitive employment by the time their placement budget had run out. Results: Of the 15 variables tested, only Global Assessment of Functioning (GAF) and Clinical Global Impression (CGI) scores were predictors for obtaining work (for ≥1 day) and for maintaining it over a longer period (>3 months). Higher GAF and lower CGI scores increased the odds of obtaining employment and keeping it for at least 3 months. Functional role impairment, quality of life, self-esteem, or education level did not predict employment. Conclusion: Our data suggest that, if time-restricted budgets are offered to a wide range of patients, such as those included in this study, better functioning and lower symptom severity at baseline are predictive of better employment outcomes (finding and maintaining work) on the first (competitive) labor market in Switzerland. Zusammenfassung: Chronische Erschöpfungszustände ohne bzw. ohne adäquate somatische Ursache sind für Betroffene sowie für Behandler ein Problem, da mit dem Fehlen einer eindeutigen nosologischen Zuordnung und eines eindeutigen Ätiologiemodells auch die Behandlungsoptionen nicht immer direkt auf der Hand liegen bzw. Gegenstand von Kontroversen sind (s.Gaab, in diesem Heft). Zur Behandlung des chronischen Erschöpfungssyndroms ("chronic fatigue syndrome", CFS) werden verschiedene psychotherapeutische Ansätze neben graduierter Aktivierung vorgeschlagen. Ziel des vorliegenden Beitrags ist es, den aktuellen Stand der Evidenzbasierung von Psychotherapie bei CFS auf Basis vorliegender Metaanalysen und neuerer randomisierter kontrollierter Studien darzustellen. Robuste Evidenz zeigt sich für die kognitive Verhaltenstherapie (KVT) mit im Durchschnitt moderater Wirksamkeit im Hinblick auf die Schwere der Erschöpfungssymptomatik und die körperliche Funktionseinschränkung nach Therapieabschluss sowie in kurz- bis mittelfristigen Katamnesen. Klinisch relevante Verbesserungen sind bei 40-50% der so behandelten Personen zu beobachten. Andere Psychotherapieverfahren wurden bislang selten evaluiert - vielversprechende Ergebnisse weisen jedoch Einzelstudien zu Gesprächspsychotherapie und Psychoedukation auf. Insgesamt besteht die Notwendigkeit, in der Therapie Konzepte zur Rückfallprophylaxe stärker zu berücksichtigen und die Nachhaltigkeit der therapeutisch erzielten Verbesserungen empirisch abzusichern

Introduction: \ud Etelcalcetide is a novel intravenous calcimimetic for the treatment of secondary hyperparathyroidism (SHPT) in haemodialysis patients. The clinical efficacy and safety of etelcalcetide (in addition to phosphate binders and vitamin D and/or analogues [PB/VD]) was evaluated in three phase III studies, including two placebo-controlled trials and a head-to-head study versus the oral calcimimetic cinacalcet.\ud \ud Objective: \ud The objective of this study was to develop a decision-analytic model for economic evaluation of etelcalcetide compared with cinacalcet.\ud \ud Methods: \ud We developed a life-time Markov model including potential treatment effects on mortality, cardiovascular events, fractures, and subjects’ persistence. Long-term efficacy of etelcalcetide was extrapolated from the reduction in parathyroid hormone (PTH) in the phase III trials and the available data from the outcomes study in cinacalcet (EVOLVE trial). Etelcalcetide was compared with cinacalcet, both in addition to PB/VD. We applied unit costs averaged from five European countries and a range of potential etelcalcetide pricing options assuming parity price to weekly use of cinacalcet and varying it by a 15 or 30% increase.\ud \ud Results: \ud Compared with cinacalcet, the incremental cost-effectiveness ratio of etelcalcetide was €1,355 per QALY, €24,521 per QALY, and €47,687 per QALY for the three prices explored. The results were robust across the probabilistic and deterministic sensitivity analyses.\ud \ud Conclusions: \ud Our modelling approach enabled cost-utility assessment of the novel therapy for SHPT based on the observed and extrapolated data. This model can be used for local adaptations in the context of reimbursement assessment.

Background Clinical efficacy of pollen allergen immunotherapy has been broadly documented in randomized controlled trials. The underlying clinical endpoints are analysed in seasonal time periods pre-defined on the basis of the background pollen concentration. However, any validated or generally accepted definition from academia or regulatory authorities for this relevant pollen-exposure intensity or period of time (season) is currently not available. Therefore, this Task Force initiative of the European Academy of Allergy and Clinical Immunology (EAACI) aimed to propose definitions based on expert-consensus. Methods A Task Force of the Immunotherapy and Aerobiology and Pollution Interest Groups of the EAACI reviewed the literature on pollen-exposure in the context of defining relevant time intervals for evaluation of efficacy in allergen immunotherapy trials. Underlying principles in measuring pollen exposure and associated methodological problems and limitations were considered in order to achieve a consensus. Results The Task Force achieved a comprehensive position in defining pollen exposure times for different pollen types. Definitions are presented for ‘pollen season’, ‘high pollen season’ (or ‘Peak pollen period’) and ‘high pollen days’. Conclusion This EAACI position paper provides definitions of pollen exposures for different pollen types for use in Allergen Immunotherapy trials. Hintergrund: Die Hühnereiallergie gehört zu den häufigsten Nahrungsmittelallergien im Kindesalter. Da es keine kausalen Therapiemöglichkeiten gibt, haben Präventions-maßnahmen einen hohen Stellenwert. Ergebnisse aus Beobachtungsstudien gaben Hinweise darauf, dass die frühe Einführung von potenten Nahrungsmittel-allergenen vor der Entwicklung von Nahrungsmittelallergien schützen könnte. Es war Ziel der randomisierten, placebokontrollierten Interventionsstudie zu untersuchen, ob die frühe Einführung von Hühnerei zwischen dem 4.-6. Lebensmonat in der Allgemeinbevölkerung ein sicherer und effektiver Weg zur Vorbeugung von Hühnereiallergien sein kann. Methoden: Die Kinder wurden bei Geburt rekrutiert und vor Beginn der Intervention auf eine vorbestehende Sensibilisierung auf Hühnerei getestet. Nicht sensibilisierte Säuglinge (Hu ̈hnerei-spezifische IgE-Antikörper im Serum < 0.35 kUA/l) wurden einer Verum- oder Placebo-Gruppe randomisiert zugeordnet und erhielten mit Beginn der Beikost ab dem 4.-6. Lebensmonat bis zum 12. Lebensmonat dreimal pro Woche pasteurisiertes Hühnereiweißpulver (Verum) oder Reispulver (Placebo) unter gleichzeitiger Hühnerei-freier Ernährung. Primärer Endpunkt war die Häufigkeit von Hühnerei-Sensibilisierungen im Alter von 12 Lebensmonaten. Die Häufigkeit von Hühnerei-Allergien wurde mit Hilfe der Durchführung von Nahrungsmittelprovokationen bestimmt. Die Studie wurde im Deutschen Register Klinischer Studien registriert (DRKS00005668). Ergebnisse: 406 Kinder wurden im Alter von 4-6 Lebensmonaten vor Beginn der Intervention auf eine vorbestehende Hühnereisensibilisierung gescreent. Bei 23/406 (5.7%) konnten bereits Hühnerei-spezifische Serum-IgE-Antikörper ≥ 0.35 kUA/l nachgewiesen werden. 17 der sensibilisierten Kinder erhielten eine doppelblinde, Placebo- kontrollierte Nahrungsmittelprovokation; hierbei wurden 16 Kinder als Hu ̈hnerei-allergisch getestet und 11 Kinder zeigten eine anaphylaktische Reaktion. Von den nicht-sensibilisierten Kindern (383/406) wurden 184 Kinder in die Verum- und 199 Kinder in die Placebo-Gruppe randomisiert. Im Alter von 12 Lebensmonaten waren 5.6% der Kinder in der Verum-Gruppe und 2.6% der Kinder in der Placebo-Gruppe Hühnerei-sensibilisiert (primärer Endpunkt; RR, 2.20; 95% KI, 0.68 bis 7.14; p=0.24). 2.1% in der Verum-Gruppe wurden als allergisch getestet im Vergleich zu 0.6% in der Placebo-Gruppe (RR, 3.30; 95% KI, 0.35 bis 31.32; p=0.35). Schlussfolgerung: Die frühe Einführung von pasteurisiertem Hühnerei ab dem 4.-6. Lebensmonat erwies sich in unserer Studie als nicht erfolgreich für die Prävention von Hühnerei-allergien. Since there is no causal treatment, prevention strategies are sought keenly. Studies suggested that the early introduction of allergenic foods might reduce the risk of food allergy. The aim of this randomized placebo-controlled intervention study was to investigate whether the introduction of hen’s egg at 4-6 months of age can be a safe and effective measure to reduce hen’s egg allergy in a general population. Methods: All participating children were recruited at birth and screened for preexisting hen’s egg-specific serum IgE prior to intervention. Only children who were not sensitized against hen’s egg were randomized to receive either verum (pasteurized egg white powder) or placebo (rice powder) added to the first weaning food three times a week under concurrent egg-free diet from age 4-6 until 12 months. The primary outcome was sensitization to hen’s egg (hen’s egg-specific serum IgE ≥ 0.35 kUA/l) by age 12 months. Hen’s egg allergy (secondary outcome) was confirmed by food challenge tests. This study was registered in the German Clinical-Trials-Registry, number DRKS00005668. Results: Among 406 screened infants, 23 (5.7%) had hen’s egg-specific IgE ≥ 0.35 kUA/l prior to randomization. 17/23 underwent subsequent double-blind, placebo-controlled food challenges and 16 were confirmed as hen’s egg allergic including 11 with anaphylactic reactions. Of the 383 non-sensitized infants (56.7% male), 184 were randomized to verum and 199 to placebo. At 12 months of age, 5.6% of the children in the verum group were hen’s egg-sensitized versus 2.6% in the placebo group (primary outcome; RR, 2.20; 95% CI, 0.68 to 7.14; p=0.24), and 2.1% were confirmed to be hen’s egg allergic versus 0.6% in the placebo group, (RR, 3.30; 95% CI, 0.35 to 31.32; p=0.35). Conclusion: We found no evidence that the consumption of hen’s egg starting at 4-6 months of age prevents hen’s egg sensitization or allergy. In contrast, it might result in frequent allergic reactions in the community considering that many 4-6 months old infants were already allergic to hen’s egg.

Objective: To evaluate the acceptability and efficacy of adding cognitive behaviour therapy to the medical care of patients presenting with the chronic fatigue syndrome. Design: Randomised controlled trial with final assessment at 12 months. Setting: An infectious diseases outpatient clinic. Subjects: 60 consecutively referred patients meeting consensus criteria for the chronic fatigue syndrome. Interventions: Medical care comprised assessment, advice, and follow up in general practice. Patients who received cognitive behaviour therapy were offered 16 individual weekly sessions in addition to their medical care. Main outcome measures: The proportions of patients (a) who achieved normal daily functioning (Karnofsky score 80 or more) and (b) who achieved a clinically significant improvement in functioning (change in Karnofsky score 10 points or more) by 12 months after randomisation. Results: Only two eligible patients refused to participate. All randomised patients completed treatment. An intention to treat analysis showed that 73% (22/30) of recipients of cognitive behaviour therapy achieved a satisfactory outcome as compared with 27% (8/30) of patients who were given only medical care (difference 47 percentage points; 95% confidence interval 24 to 69). Similar differences were observed in subsidiary outcome measures. The improvement in disability among patients given cognitive behaviour therapy continued after completion of therapy. Illness beliefs and coping behaviour previously associated with a poor outcome changed more with cognitive behaviour therapy than with medical care alone. Conclusion: Adding cognitive behaviour therapy to the medical care of patients with the chronic fatigue syndrome is acceptable to patients and leads to a sustained reduction in functional impairment. Contains fulltext : 152564.pdf (Publisher’s version ) (Open Access) OBJECTIVE: To compare the effectiveness of pain exposure physical therapy (PEPT) with conventional treatment in patients with complex regional pain syndrome type 1 (CRPS-1) in a randomised controlled trial with a blinded assessor. SETTING: The study was conducted at a level 1 trauma centre in the Netherlands. PARTICIPANTS: 56 adult patients with CRPS-1 participated. Three patients were lost to follow-up. INTERVENTIONS: Patients received either PEPT in a maximum of five treatment sessions, or conventional treatment following the Dutch multidisciplinary guideline. MEASUREMENTS: Outcomes were assessed at baseline and at 3, 6 and 9 months after randomisation. The primary outcome measure was the Impairment level Sum Score-Restricted Version (ISS-RV), consisting of visual analogue scale for pain (VAS-pain), McGill Pain Questionnaire, active range of motion (AROM) and skin temperature. Secondary outcome measures included Pain Disability Index (PDI); muscle strength; Short Form 36 (SF-36); disability of arm, shoulder and hand; Lower Limb Tasks Questionnaire (LLTQ); 10 m walk test; timed up-and-go test (TUG) and EuroQol-5D. RESULTS: The intention-to-treat analysis showed a clinically relevant decrease in ISS-RV (6.7 points for PEPT and 6.2 points for conventional treatment), but the between-group difference was not significant (0.96, 95% CI -1.56 to 3.48). Participants allocated to PEPT experienced a greater improvement in AROM (between-group difference 0.51, 95% CI 0.07 to 0.94; p=0.02). The per protocol analysis showed larger and significant between-group effects on ISS-RV, VAS-pain, AROM, PDI, SF-36, LLTQ and TUG. CONCLUSIONS: We cannot conclude that PEPT is superior to conventional treatment for patients with CRPS-1. Further high-quality research on the effects of PEPT is warranted given the potential effects as indicated by the per protocol analysis. TRIAL REGISTRATION NUMBERS: NCT00817128 and NTR 2090.

To pilot a substantive randomized control trial comparing post-operative external ear canal pack with no ear pack after middle ear surgery, 32 adults undergoing primary posterior bony canal wall preserving middle ear surgery were randomized to have either a bismuth iodoform paraffin paste pack or no ear pack post-operatively. Outcome measures included clinician- and patient-recorded visual analogue scale scores for ear signs and symptoms at 3 weeks and 3 months and audiometric findings at 3 months post-operatively. There was no statistically significant inter-group difference in 3-week clinician and patient cumulative scores for ear signs and symptoms. There was also no significant difference in graft take rate, appearance of ear canals and audiometric results in either group at 3 months. No difference in ear symptoms, clinician findings or hearing was demonstrated between patients with a post-operative pack compared to those without a pack following middle ear surgery in this pilot study.

Die vorliegende Arbeit untersuchte – nach Wissen der Autorin zum ersten Mal – im Rahmen eines kontrollierten, randomisierten Studiendesigns die Wirksamkeit von Biofeedbacktherapie im Vergleich zu graduiertem Bewegungstraining in der Behandlung von erwachsenen Patientinnen mit Chronischem Erschöpfungssyndrom (CFS; ICD-10 F48.0). CFS als Erkrankung mit den Hauptsymptomen von Müdigkeit und verstärktem Erschöpfungserleben sowie zusätzlichen Beschwerden wie Muskel- und Kopfschmerzen oder Konzentrationsschwierigkeiten geht einher mit einem starken Leidensdruck der Betroffenen und deutlichen Einschränkungen in der Alltagsbewältigung. CFS lässt sich der übergeordneten Störungsgruppe der funktionellen somatischen Syndrome (FSS) zuordnen. Hinsichtlich der Ätiologie werden multifaktorielle und bio-psycho-soziale Störungsmodelle herangezogen. Etablierte und evidenzbasierte Behandlungsverfahren des CFS stellen kognitiv-behaviorale und aktivitätsfördernde Interventionen dar. Biofeedback als Verfahren, das den verhaltenstherapeutisch orientierten Verfahren zugeordnet werden kann, intendiert neben der konkreten Beeinflussung einzelner Übungsparameter ein verbessertes Verständnis für psychosomatische Zusammenhänge sowie eine Stärkung von Selbstwirksamkeits- und Kontrollerleben. Es bietet zusätzlich die Möglichkeit der Korrektur dysfunktionaler, krankheitsaufrechterhaltender Überzeugungen. Für den Einsatz von Biofeedback im Rahmen von FSS liegen zahlreiche Wirksamkeitsnachweise vor. Zur Biofeedbackbehandlung von CFS wurde im Rahmen der vorliegenden Arbeit ein Manual entwickelt, wobei die respiratorische Sinusarrhythmie als Trainingsparameter gewählt wurde. Die Überprüfung der Wirksamkeit der Biofeedbackbehandlung erfolgte in den Bereichen ´subjektives Beschwerdeerleben` und ´körperliche Leistungsfähigkeit` in Bezugnahme auf ein graduiertes Bewegungstraining als etabliertem Kontrollverfahren. Die Behandlungsdauer in beiden Gruppen erstreckte sich über acht Sitzungen über einen Behandlungszeitraum von drei Monaten. Die Datenerhebung erfolgte zu drei Messzeitpunkten, d. h. vor und nach Abschluss der Intervention sowie zu einer 5-Monats-Katamnese. Die Ergebnisse basieren auf den Daten von 24 Probandinnen von denen 13 am Biofeedback- und 11 am graduierten Bewegungsprogramm am Universitätsklinikum Tübingen teilnahmen. 4 Probandinnen beendeten das Bewegungsprogramm vorzeitig und wurden nicht in die Auswertung aufgenommen. Die Auswertung der Daten erfolgte mittels non-parametrischer Verfahren. Zur Beurteilung der klinischen Signifikanz wurden Standardeffektstärken berechnet. Zwischen den Behandlungsgruppen zeigten sich keine signifikanten Unterschiede in grundlegenden demographischen Variablen. Während beider Interventionen zeigte sich eine substantielle Reduktion des Erschöpfungs- und Beschwerdeerlebens. Die Biofeedbacktherapie zeichnete sich insbesondere durch Einflussnahme auf das psychische Wohlbefinden einschließlich der Depressivität und der psychophysiologischen Regulation aus. Die Bewegungstherapie zeichnete sich dahingegen insbesondere durch Einflussnahme auf das körperliche Wohlbefinden einschließlich körperlicher Schmerzen und auf die körperliche Aktivität bzw. Leistungsfähigkeit aus. Es zeigten sich im Behandlungsverlauf keine maßgeblichen Veränderungen der Kontrollüberzeugungen in Bezug auf das eigene gesundheitliche Befinden. Bei der Interpretation und Diskussion der Ergebnisse werden die Reduktion von Schon- und Vermeidungsverhalten, die Zunahme an Selbstwirksamkeitserleben, das verbesserte Verständnis in Bezug auf psychophysiologische Zusammenhänge, die Korrektur dysfunktionaler Annahmen in Bezug auf Erschöpfung und Bewegung sowie eine positive Behandlungserwartung als mögliche Wirkfaktoren erörtert. Biofeedback als Behandlungsverfahren zeichnet sich durch eine spezifische Anwendbarkeit hinsichtlich dieser Wirkfaktoren aus. Limitationen der Studie sind in Zusammenhang mit der kleinen Stichprobengröße zu sehen. Als ermutigend hinsichtlich zukünftiger Interventionen im Bereich von CFS und FSS erweist sich die hohe Akzeptanz des Biofeedbackverfahrens.

Das Ziel dieser klinischen, doppelt-verblindeten, randomisierten Studie war es den Nutzen eines arbiträren Gesichtsbogens bei der Modellmontage zu evaluieren bei vorausgegangener Änderung der vertikalen Relation. Dabei sollte der Einfluss des Gesichtsbogens auf die Kontaktpunktsituation sowie auf das Ausmaß der Nachsorge untersucht werden. Die anfängliche Probandenanzahl betrug 32 (unbezahnt, mindestens eine der Totalprothesen war nicht älter als 4 Wochen), aus denen zwei Gruppen mit jeweils 16 Probanden randomisiert gebildet wurden. In der Gesichtsbogengruppe wurde ein arbiträrer Ohr-Gesichtsbogen (PROTAR 7 digma, KaVo, Biberach/Riß, Deutschland) angewandt, in der Kontrollgruppe stattdessen die mittelwertige Modellmontage durchgeführt. Für die Modellmontage wurde jeweils ein semi-adjustierbarer Artikulator verwendet. Die Änderung der vertikalen Relation wurde durch eine sekundären Remontage erzielt. Die Okklusionskontakte beider Gruppen wurden nach dem Absenken im Labor gleichmäßig eingeschliffen (mindestens ein Kontaktpunkt je Seitenzahn in der Statik, keine Frontzahnkontakte). Die Kontaktpunkte wurden anschließend direkt und 84 Tage nach der sekundären Remontage am Patienten registriert. Zudem wurden jegliche Druckstellen dokumentiert und das Ausmaß der Nachsorge in beiden Gruppen untersucht. Ferner wurden alle Prothesen vor dem Einschleifen im Labor dupliert, sodass für jeden Probanden zusätzlich beide Montagemethoden durchgeführt werden konnten, da gruppenunabhängig bei jedem Probanden die Gesichtsbogenregistrierung durchgeführt wurde. Für jedes Modellpaar wurde nach dem Absenken ein Registrat erstellt, das mit Hilfe der „Greifswald Digital Analyzing System“ Software (GEDAS 2 Reader) ausgewertet und auf signifikante Unterschiede in der Anzahl der Kontaktpunkte untersucht wurde. Die Anzahl der klinischen Kontaktpunkte je Front- und Seitenzahn sowie die Zähne in Kontakt waren in beiden Gruppen zu keinem Zeitpunkt signifikant verschieden (p>0,05). Die Auswertung der Modelldaten bezüglich der Seitenzahnkontakte ergab für beide Gruppen keine signifikanten Unterschiede (p>0,05). Hinsichtlich der Frontzahnkontakte und der Anzahl aller Zähne in Kontakt waren die Werte in der Gesichtsbogengruppe signifikant höher (Anzahl Frontzahnkontakte: p=0,005; F-Test, Anzahl Frontzähne in Kontakt: p=0,007; F-Test, Anzahl aller Zähne in Kontakt: p=0,027; F-Test). Zudem ergab sich eine geringe Korrelation zwischen dem Ausmaß der Sperrung und der Kontaktsituation. Das Ausmaß der Nachsorge war in beiden Gruppen ähnlich. Der in dieser Studie verwendete Ohr-Gesichtsbogen zur Übertragung von Totalprothesen in den Artikulator nach vorausgegangener Änderung der vertikalen Dimension kann der mittelwertigen Modellmontage aufgrund der vorliegenden Ergebnisse als gleichwertig eingestuft werden und zeigt bezüglich der untersuchten Parameter keinerlei Vorteile. Im Bereich der Totalprothetik kann die Modellmontage nach Mittelwerten bei vorausgegangener Änderung der vertikalen Dimension empfohlen werden., The quality effect of a face-bow on the static occlusion in connection with an alteration of the vertical dimension. A randomized clinical study. It was the purpose of this clinical, double-blinded, randomized study to evaluate the benefits of an arbitrary face-bow in the mounting procedure of casts in connection with an alteration of the vertical dimension. The influence of the face-bow concerning the occlusal contact situation as well as the extent of the follow-up had to be examined. The initial number of patients (edentulous, complete dentures) was 32, randomized into two groups with 16 patients. In the face-bow group an arbitrary earpiece face-bow (PROTAR 7 digma, KaVo, Biberach / Riss, Germany) has been used. In the control group the casts were mounted arbitrarily. The alteration of the vertical relationship was achieved by a remount procedure. After lowering the cast the occlusal contacts in both groups were grinded uniformly (no occlusal contacts per front tooth). The number of occlusal contacts was evaluated directly after the remounting procedure and 84 days after. Sore spots on the oral mucosa were documented as well. All complete dentures had been duplicated in the laboratory before the occlusal contacts were grinded. In that way the model casts for one patient could be mounted in both ways (the face-bow transfer was used initially for all subjects regardless of the group membership). After lowering the casts a silicon interocclusal record was made for each pair of casts which was evaluated using the "Greifswald Digital Analyzing System" software (Reader GEDAS 2). The number of clinical occlusal contacts on each anterior and posterior teeth and the number of teeth in contact in both groups were not significantly different at any time (p> 0.05). The evaluation of the model data for the contacts of the posterior teeth revealed no significant differences (p> 0.05). The number of anterior tooth contacts (p = 0.005), the number of anterior teeth in contact (p = 0.007) and the total number of teeth in contact (p=0.027) were significantly higher in the face-bow group. The extent of follow-up was similar in both groups. Based on the present results the earpiece face-bow used in this study for the transfer of complete dentures in the articulator after a previous change in the vertical dimension can be considered to be equivalent to the arbitrarily mounting method and achieved no advantages over the control group.

Einleitung: Zur Therapie akuter Infekte der oberen Atemwege wird traditionell häufig die lokale Anwendung von Wärme eingesetzt (Inhalation heißen Dampfes, Trinken heißer Flüssigkeiten). Die Wirksamkeit lokaler Erwärmung wurde in nur wenigen Studien untersucht, die widersprüchliche Ergebnisse zeigten. Fragestellung: Das Ziel der vorliegenden Studie war es zu untersuchen, ob die Inhalation 90°C heißer, trockener Luft in einer Sauna bei Patienten mit einem beginnenden akuten Infekt der oberen Atemwege die Symptome stärker reduziert als die Inhalation raumtemperierter Luft. Methodik: Eine randomisierte, einfachblinde, kontrollierte Studie mit einer Interventionsdauer von 3 Tagen und einer Follow-up-Dauer von 4 Tagen wurde durchgeführt. Patienten mit Symptomen eines akuten Infekts der oberen Atemwege (mindestens 2 von 10 vordefinierten Erkältungssymptomen seit 24 Stunden oder weniger) wurden zufällig der Saunaluft-Gruppe oder der Raumluft-Gruppe zugeteilt. Die Intervention bestand für beide Gruppen aus jeweils einem 3-minütigen Trockensaunaaufenthalt an 3 aufeinander folgenden Tagen. Isoliert mit einem Wintermantel inhalierten die Patienten der Saunaluft-Gruppe 90°C heiße, trockene Saunaluft, während die Patienten der Raumluft-Gruppe normaltemperierte Raumluft (24°C) inhalierten. Diese spezielle Art der Saunaanwendung hat mit normalem Saunabaden wenig gemeinsam, da es in dieser Studie primär um die Inhalation großer Mengen heißer Luft und nicht um die Induktion von Schwitzen ging. Der Schweregrad der Symptome des akuten Infekts der oberen Atemwege wurde mittels eines Symptomscores erhoben. Der Hauptzielparameter war die Fläche unter der Kurve (Zusammenfassung der einzelnen Symptomscores an den Tagen 2, 3, 5 und 7). Sekundäre Zielparameter waren unter anderem der Symptomscore zu den einzelnen Erhebungszeitpunkten, das Allgemeine Krankheitsgefühl, der Medikamentengebrauch gegen den akuten Infekt der oberen Atemwege und die Inzidenz Unerwünschter Ereignisse. Ergebnisse: 157 Patienten (Alter, Mittelwert (MW) ± Standardabweichung 32,0 ± 10,2 Jahre; 59,2% Frauen) wurden in die Studie eingeschlossen (Saunaluft- Gruppe n = 80, Raumluft-Gruppe n = 77). Für den Hauptzielparameter, die Fläche unter der Kurve der Symptomscores, ergab sich zwischen der Saunaluft-Gruppe (MW ± Standardfehler 31,2 ± 1,8 [95% Konfidenzintervall (KI) 27,6 bis 34,8]) und der Raumluft-Gruppe (35,1 ± 2,3 [30,5 bis 39,7]) kein statistisch signifikanter Unterschied (Gruppendifferenz -3,9 [95% KI -9,7 bis 1,9], p = 0,189, Cohens d = 0,13). Bei den sekundären Zielparametern zeigten sich die einzigen statistisch signifikanten Unterschiede zwischen den Gruppen für den Symptomscore am Tag 2 (p = 0,037), den Medikamentengebrauch gegen den akuten Infekt der oberen Atemwege am Tag 1 (p = 0,011) und die Wirksamkeitsbeurteilung der erhaltenen Intervention am Tag 7 (p = 0,026). Schlussfolgerung: Die Inhalation heißer, trockener Luft in einer Sauna hatte keinen signifikanten Einfluss auf den Schweregrad der Symptome eines akuten Infekts der oberen Atemwege., Introduction: Traditionally, in the treatment of common colds a local application of heat is often used (inhaling hot steam, drinking hot fluids). The efficacy of local heating was evaluated just in a few trials which have had conflicting results. Objective: The aim of the present study was to evaluate whether inhaling 90°C hot dry air in a sauna reduces common cold symptoms greater than inhaling air at room temperature in patients with a newly acquired common cold. Methods: A randomised single blind controlled trial with a treatment duration of 3 days and a follow-up duration of 4 days was conducted. Patients with symptoms of a common cold (at least 2 out of 10 predefined common cold symptoms for 24 hours or less) were randomly assigned to an intervention group or a control group. The intervention consisted for both groups of a 3-minute stay in a dry-heat sauna on 3 consecutive days. Insulated with a winter coat the patients in the intervention group inhaled 90°C hot dry air, whereas the patients in the control group inhaled air at room temperature (24°C). This particular kind of using a sauna has little in common with regular saunavisits, because in this study the focus was on inhaling hot air and not on sweating. The severity of the common cold symptoms was measured with a symptom severity score. The primary outcome measure was the area under the curve which summarised symptom severity on days 2, 3, 5 and 7. Secondary outcome measure were among others the symptom severity scores for individual days, the general ill feeling, the use of medication for the common cold and the incidence of adverse events. Results: 157 patients (age, mean ± standard deviation 32.0 ± 10.2 years, 59.2% women) were included into the trial (intervention group n = 80, control group n = 77). For the primary outcome measure the area under the curve for symptom severity no significant difference between the intervention group (mean ± standard error 31.2 ± 1.8 [95% confidence interval (CI) 27.6 to 34.8]) and the control group (35.1 ± 2.3 [30.5 to 39.7]) was observed (group difference -3.9 [95% CI -9.7 to 1.9], p = 0.189, Cohens d = 0.13). For the secondary outcome measures symptome severity score on day 2 (p = 0.037), medication use for the common cold on day 1 (p = 0.011) and patients’ ratings of the effectiveness of the therapy on day 7 (p = 0.026) significant differences between the groups were found. Conclusion: Inhaling hot dry air in a sauna had no significant impact on the symptom severity of a common cold.

Systemic steroids are widely used worldwide as a standard of care for primary therapy of idiopathic sudden sensorineural hearing loss (ISSHL). The German ISSHL guideline recommends high-dose steroids for primary therapy of ISSHL, without evidence from randomized controlled trials (RCTs). The rationale for the treatment of ISSHL using high dose steroids is only based on retrospective cohort studies.This article describes the planning and initiation of a multicenter, national, randomized, controlled clinical trial entitled Efficacy and safety of high dose glucocorticosteroid treatment for idiopathic sudden sensorineural hearing loss - a three-armed, randomized, triple-blind, multicenter trial (HODOKORT). This clinical trial aims to compare standard dose with two types of high-dose steroids for primary systemic therapy with respect to their efficacy in improving hearing, and thus communication ability, in patients with idiopathic sudden sensorineural hearing loss.This study is funded by the "Clinical Trials with High Patient Relevance" research program in the health research framework of the German Federal Ministry of Education and Research. It is one of two studies by the German Study Center of Clinical Trials of the German Society of Otorhinolaryngology, Head and Neck Surgery (DSZ-HNO). Planning and initiation was done in cooperation with the DSZ-HNO, the Coordination Center of Clinical Trials of the Martin-Luther-University Halle-Wittenberg, and the Study Center of the University Hospital Freiburg.

Kindheitsbelastungen haben weitreichende negative Konsequenzen und wurden mit Depressionen und Diabetes mellitus in Zusammenhang gebracht. In einer randomisierten, kontrollierten, multizentrischen Studie wurden Diabetespatienten (N = 251) mit unzureichender Stoffwechseleinstellung und Major Depression 12 Wochen mit Sertralin oder einer diabetesspezifischen Kognitiven Verhaltenstherapie behandelt. Therapieresponder wurden in die einjährige Langzeitphase integriert. Die Therapie fokussierte auf die Stoffwechsellage und die Depression. Die Stichprobe wurde auf Kindheitsbelastungen hin untersucht. Hauptergebnisse: Die Prävalenz wurde erhoben: Emotionaler Missbrauch (N = 74; 32 %), Körperlicher Missbrauch (N = 49; 21,3 %), Sexueller Missbrauch (N = 36; 15,5 %), Körperliche Vernachlässigung (N = 63; 27,2 %). Emotionale Vernachlässigung wurde am häufigsten berichtet (N = 91; 39,4 %). Kindheitsbelastungen waren unter anderem mit makrovaskulären Komplikationen, erhöhtem hs-CRP und Einbußen psychischer Gesundheit und Lebensqualität assoziiert. Zur Baseline lag bei Körperlicher Vernachlässigung (F = 5,003; p = 0,026), Körperlichem Missbrauch (F = 3,996; p = 0,047), extremem Emotionalem Missbrauch (F = 6,697; p = 0,011) und extremer Körperlicher Vernachlässigung (F = 10,002; p = 0,002) eine größere Depressionsschwere vor. Der Therapieerfolg wurde durch Kindheitsbelastungen nicht beeinträchtigt. Wirksamkeitsunterschiede der Interventionen waren nicht nachweisbar. Beurteilung: Kindheitsbelastungen sind bei depressiven Diabetikern hoch prävalent. Die Betroffenen sind depressiver und weisen möglicherweise ein deutlicher ausgeprägtes kardiovaskuläres Risikoprofil auf. Eine Indikation für personalisierte Therapiekonzepte in der Behandlung depressiver Diabetespatienten in Abhängigkeit von Kindheitsbelastungen stellt sich für die untersuchten Therapieziele nicht dar.

Hintergrund: Schröpfen existiert in vielen traditionellen Medizinsystemen weltweit; heute wird es im Rahmen der Komplementärmedizin eingesetzt. Ziel dieser Pilotstudie war die Untersuchung der Wirksamkeit des pulsierenden Schröpfens bei Gonarthrose. Methodik: In einer prospektiven, randomisierten, klinischen Pilotstudie wurde die Wirksamkeit des pulsierenden unblutigen Schröpfens verglichen mit einer Wartelistenkontrolle. Die Behandlung bestand aus 8 Sitzungen in 4 Wochen; 8 Wochen danach erfolgte eine Nachkontrolle. Das betroffene Knie und korrespondierende lumbo-sakrale Gebiete wurden geschröpft, Paracetamol war als Bedarfsmedikation erlaubt. Aufgenommen wurden Patienten mit radiologisch nachgewiesener Gonarthrose (Kellgren Lawrence II – IV) und einem Schmerzscore von ≥ 40 mm auf der VAS (0 – 100 mm). Die wichtigsten Outcome-Parameter nach 4 und 12 Wochen waren der Western Ontario and McMaster Universities Osteoarthritis (WOMAC) Score, Schmerzintensität gemessen mit einer VAS (0 – 100 mm), gesundheitsbezogene Lebensqualität (SF 36) sowie die Globalbeurteilung des Behandlungsergebnisses. Die Analyse der Ergebnisse erfolgte mittels einer Kovarianzanalyse (ANCOVA) adjustiert auf die Baseline Werte. Die Paracetamol-Einnahme wurde während der ersten 4 Wochen dokumentiert. Ergebnisse: Vierzig Patientinnen und Patienten wurden in Gruppen für Schröpfen (n = 21; 5 männlich; Alter 68 ± 7,2 [MW ± SA] Jahre) und für keine Intervention (n = 19; 8 männlich; 69 ± 6,8 Jahre) randomisiert. Es fanden sich keine signifikanten Gruppenunterschiede zu Baseline. Nach 4 Wochen lag der WOMAC Gesamtscore in der Schröpfgruppe signifikant niedriger als in der Kontrollgruppe (MW 27,7 [95% Konfidenzintervall 22,1; 33,3] vs. 42,2; [36.3; 48,1]; p = 0,001). Nach 12 Wochen war der Unterschied des WOMAC- Gesamtscores weiterhin signifikant (31,0 [24,9; 37,2] vs. 40,8 [34,4; 47,3]; p = 0,032). Der VAS Schmerzwert zeigte nach 4 Wochen in der Schröpfgruppe eine signifikante Besserung (38,4 [95 % KI: 30,5; 46,2] vs. 55,0 [46,8; 63,2]; p = 0,005). Die Schröpfgruppe zeigte signifikant bessere Ergebnisse als die Kontrollgruppe in der physischen Summenskala des SF 36 nach 4 Wochen (36,0 [95 % KI: 33,5; 38,6] vs. 31,9 [29,2; 34,6]; p = 0,030) und nach 12 Wochen (36,3 [95 % KI: 32,9; 39,8] vs. 30,2 [26,6; 33,9]; p = 0,019), aber nicht in der psychischen Summenskala. Insgesamt benötigte die Schröpfgruppe etwas weniger Paracetamol-Tabletten (9,1 ± 20,0 vs. 11,5 ± 15,9). Es wurden keine schwerwiegenden unerwünschten Ereignisse beobachtet. Mehr qualitativ-hochwertige Studien sollten durchgeführt werden um die klinische Relevanz dieses Verfahrens zu evaluieren., Background: Cupping exists in several traditional medical systems worldwide and remains a practice in the field of complementary medicine. The aim of this pilot study was to investigate the effectiveness of cupping in knee osteoarthritis. Methods: In a prospective, randomized clinical pilot study, dry pulsating cupping was compared to a waiting list control. Treatment consisted of 8 sessions within 4 weeks, with a 8 weeks follow-up. The affected knee and a corresponding lumbo-sacral area were cupped. Included were patients with radiologically confirmed knee osteoarthritis (Kellgren Lawrence degree II – IV) and a pain score of ≥ 40 mm on a VAS (0 - 100 mm). Outcome parameters included the Western Ontario and McMaster Universities Osteoarthritis (WOMAC) score, pain intensity on a VAS (0 to 100 mm), and health related quality of life (SF 36) after 4 and 12 weeks. Results were calculated with analysis of covariance (ANCOVA) adjusted for baseline differences. The use of paracetamol was documented during the 4 intervention weeks. Results: Forty patients were randomized into a cupping group (n = 21; 5 male; age 68 ± 7.2 [mean ± SD] years) and a waiting list group (n = 19; 8 male; 69 ± 6.8 years). No significant baseline differences were found. After 4 weeks, the WOMAC score was significantly lower in the cupping group than in the control group (mean 27.7 [95% confidence interval 22.1; 33.3] vs. 42.2; [36.3; 48.1]; p = 0.001). After 12 weeks the differences of the WOMAC score (31.0 [24.9; 37.2] vs. 40.8 [34.4; 47.3]; p = 0.032) were still significant in favour for cupping. Pain intensity (VAS) improved significantly after 4 weeks in the cupping group (38.4 [95 % KI: 30.5; 46.2] vs. 55.0 [46.8; 63.2]; p = 0.005). The cupping group showed significantly better outcomes than the control group in the SF‑36 physical component scale after 4 weeks (36.0 [95 % KI: 33.5; 38.6] vs. 31.9 [29.2; 34.6]; p = 0.030) and after 12 weeks (36.3 [95 % KI: 32.9; 39.8] vs. 30.2 [26.6; 33.9]; p = 0.019) but not for the mental component scale. Overall, the cupping group required slightly fewer paracetamol tablets (9.1 ± 20.0 vs. 11.5 ± 15.9). No severe adverse events were observed. Conclusions: In this exploratory study on symptoms of knee OA, pulsating dry cupping was more effective compared to waiting list control. More high quality research is needed to evaluate the clinical relevance of this treatment.

Background The immediate instillation of mitomycin after transurethral resection of bladder tumor (TURBT) is widely used and recommended in the guidelines. Recently it was shown that pre-TURBT intravesical electromotive drug administration (EMDA) of mitomycin reduces the recurrence rate of non-muscle invasive bladder cancer. Our aim was to describe the pharmacokinetics and patient safety after post-TURBT EMDA. Methods We performed a single centre study with 25 patients diagnosed with non-muscle invasive bladder cancer. All patients underwent complete resection of all visible tumors and post-TURBT intravesical electromotive drug administration (EMDA) of mitomycin (40 mg) for 30 min. Blood samples were taken before starting the electrical current and 15, 30, 60, and 120 min after starting the procedure for quantification of mitomycin serum levels. Results In 24 patients, the measured serum level of mitomycin was below the detection threshold of 50 ng/ml. In one patient serum level was elevated 15 min (155 ng/ml) and 30 min (65 ng/ml) after intravesical instillation. Nine patients reported a slight tingling sensation in the bladder during mitomycin administration. Discreet pressure in the suprapubic area was reported by one patient. One patient had a first degree skin burn at the site of one skin electrode. Conclusion Postoperative EMDA with mitomycin is a safe procedure. The measured mitomycin serum levels were below toxic concentrations. These findings encourage the initiation of large randomized controlled trials with postoperative EMDA-assisted instillation of mitomycin to test its influence on the recurrence rate of non-muscle invasive bladder cancer.

Objectives This study aims to evaluate feasibility, face validity, influence on technical factors and subjective sense of utility of patient-specific rehearsal (PsR) prior to endovascular aortic aneurysm repair (EVAR). Design A prospective, multicentre pilot study. Methods Patients suitable for EVAR were enrolled and a three-dimensional (3D) model of the patient's anatomy was generated. Less than 24 h prior to the real case, rehearsals were conducted in the laboratory or clinical angiosuite. Technical metrics were recorded during both procedures. A subjective questionnaire was used to evaluate realism, technical and human factor aspects (scale 1–5). Results Ten patients were enrolled. In one case, the treatment plan was altered based on PsR. In 7/9 patients, the rehearsal significantly altered the optimal C-arm position for the proximal landing zone and an identical fluoroscopy angle was chosen in the real procedure. All team members found the rehearsal useful for selecting the optimal fluoroscopy angle (median 4). The realism of the EVAR procedure simulation was rated highly (median 4). All team members found the PsR useful to prepare the individual team members and the entire team (median 4). Conclusions PsR for EVAR permits creation of realistic case studies. Subjective evaluation indicates that it may influence optimal C-arm angles and be valuable to prepare the entire team. A randomised controlled trial (RCT) is planned to evaluate how this technology may influence technical and team performance, ultimately leading to improved patient safety.

Hintergrund Bei etwa 60.000 jahrlich in Deutschland durchgefuhrten koronaren Bypassoperationen (CABG) weisen 6–8 % der betroffenen Patienten eine hochgradige Karotisstenose auf, was mit einem erhohten perioperativen Risiko fur Schlaganfall und kardiovaskulare Komplikationen assoziiert ist. Auch wenn die Koronaroperation mit kombinierter Karotisendarterektomie (CEA) in vielen deutschen Zentren derzeit die bevorzugte Behandlungsoption darstellt, existiert kein einheitlicher Behandlungsstandard.

We thank the authors Garbe, Suissa, and Vorlat for their interest in our study,1 and we would like to reply to the different issues raised in their letters. Dr Garbe outlined that a previous large randomized trial involving 20 536 patients with coronary artery disease, peripheral artery disease, or diabetes mellitus failed to demonstrate an improved cancer-free survival in patients who received 40 mg simvastatin and had an 11-year follow-up.2,3 However, there remain several methodological issues with this study. The mean follow-up in the original randomized trial was 5.3 years,3 and only inclusion of a nonrandomized post-trial observational time yielded a follow-up of 11 years. [english] Bone graft substitutes are increasingly being used as supplements to standard care or as alternative to bone grafts in the treatment of traumatic fractures.The efficacy and cost-effectiveness of bone graft substitutes for the treatment of traumatic fractures as well as the ethical, social and legal implications of their use are the main research questions addressed.A systematic literature search was conducted in electronic medical databases (MEDLINE, EMBASE etc.) in December 2009. Randomised controlled trials (RCT), where applicable also containing relevant health economic evaluations and publications addressing the ethical, social and legal aspects of using bone graft substitutes for fracture treatment were included in the analysis. After assessment of study quality the information synthesis of the medical data was performed using metaanalysis, the synthesis of the health economic data was performed descriptively. 14 RCT were included in the medical analysis, and two in the heath economic evaluation. No relevant publications on the ethical, social and legal implications of the bone graft substitute use were found. In the RCT on fracture treatment with bone morphogenetic protein-2 (BMP-2) versus standard care without bone grafting (RCT with an elevd high risk of bias) there was a significant difference in favour of BMP-2 for several outcome measures. The RCT of calcium phosphate (CaP) cement and bone marrow-based composite materials versus autogenous bone grafts (RCT with a high risk of bias) revealed significant differences in favour of bone graft substitutes for some outcome measures. Regarding the other bone graft substitutes, almost all comparisons demonstrated no significant difference.The use of BMP-2 in addition to standard care without bone grafting led in the study to increased treatment costs considering all patients with traumatic open fractures. However, cost savings through the additional use of BMP-2 were calculated in a patient subgroup with high-grade open fractures (Gustilo-Anderson grade IIIB). Cost-effectiveness for BMP-2 versus standard care with autologous bone grafts as well as for other bone graft substitutes in fracture treatment has not been determined yet. Although there were some significant differences in favour of BMP-2, due to the overall poor quality of the studies the evidence can only be interpreted as suggestive for efficacy. In the case of CaP cements and bone marrow-based bone substitute materials, the evidence is only weakly suggestive for efficacy. From an overall economic perspective, the transferability of the results of the health economic evaluations to the current situation in Germany is limited.The current evidence is insufficient to evaluate entirely the use of different bone graft substitutes for fracture treatment. From a medical point of view, BMP-2 is a viable alternative for treatment of open fractures of the tibia, especially in cases where bone grafting is not possible. Autologous bone grafting is preferable comparing to the use of OP-1. Possible advantages of CaP cements and composites containing bone marrow over autogenous bone grafting should be taken into account in clinical decision making. The use of the hydroxyapatite material and allograft bone chips compared to autologous bone grafts cannot be recommended. From a health economic perspective, the use of BMP-2 in addition to standard care without bone grafting is recommended as cost-saving in patients with high-grade open fractures (Gustilo-Anderson grade IIIB). Based on the current evidence no further recommendations can be made regarding the use of bone graft substitutes for the treatment of fractures. Beim Vergleich Bone morphogenetic protein (BMP)-2 gegenüber Standardtherapie ohne Knochentransplantat bei Frakturen ergeben sich in einem RCT (mit erhöhtem Verzerrungspotenzial) signifikante Unterschiede zugunsten BMP-2 bezüglich mehrerer Endpunkte. Bei Calciumphosphat (CaP)-Zementen und knochenmarkhaltigen Kompositmaterialien zeigen sich beim Vergleich gegenüber Knochentransplantaten in den RCT (alle mit hohem Verzerrungspotenzial) signifikante Unterschiede zugunsten der Knochenersatzmaterialien bezüglich einzelner Endpunkte. Bei allen anderen Materialien finden sich in fast allen Vergleichen keine signifikanten Unterschiede.Die BMP-2-Anwendung zusätzlich zur Standardbehandlung ohne Knochentransplantation führt bei Betrachtung aller Patienten mit traumatischen offenen Frakturen zur Kostenerhöhung. Einsparungen durch zusätzlichen BMP-2-Einsatz lassen sich allerdings bei der Patientensubgruppe mit hochgradig offenen Frakturen (Gustilo-Anderson-Grad-IIIB) errechnen. Die Kostenwirksamkeit beim BMP-2-Einsatz vs. Standardbehandlung mit Knochentransplantation sowie beim Einsatz anderer Knochenersatzmaterialien ist bisher nicht ermittelt. Aufgrund der schlechten Studienqualität kann trotz einiger signifikanter Unterschiede zugunsten BMP-2 nur von einigen starken Hinweisen, bei CaP-Zementen und knochenmarkhaltigen Knochenersatzmaterialien nur von wenigen schwachen Hinweisen auf eine Wirksamkeit ausgegangen werden. Die Ergebnisübertragbarkeit aus den bewerteten gesundheitsökonomischen Analysen auf die heutige Situation in Deutschland mit gesamtwirtschaftlicher Perspektive ist eingeschränkt.Die Evidenzlage zum Einsatz von Knochenersatzmaterialien ist unbefriedigend. Aus medizinischer Sicht ist der Einsatz von BMP-2 bei Patienten mit offenen Tibiafrakturen eine Therapieoption, insbesondere wenn keine Knochentransplantation infrage kommt. Dem Einsatz von Osteogenic protein (OP)-1 ist die Knochentransplantation vorzuziehen. Mögliche Vorteile beim Einsatz von CaP-Zementen und knochenmarkhaltigen Kompositmaterialien gegenüber einer Knochentransplantation sollen in die klinische Entscheidungsfindung einbezogen werden. Der Einsatz des Hydroxylapatitmaterials und der Allograft-Knochenchips kann gegenüber einer Knochentransplantation nicht empfohlen werden.Aus gesundheitsökonomischer Sicht ist der Einsatz von BMP-2 zusätzlich zur Standardbehandlung ohne Knochentransplantation nur bei Patienten mit hochgradig offenen Frakturen (Gustilo-Anderson-Grad-IIIB) als kostensparend zu empfehlen. Es können keine weiteren Empfehlungen zum Einsatz von Knochenersatzmaterialien bei Patienten mit Frakturen oder Nonunions gemacht werden. Zur Vermeidung von juristischen Implikationen soll der Einsatz von Knochenersatzmaterialien außerhalb der zugelassenen Indikationen vermieden werden.

[english] Menopause is described as the transition from the reproductive phase of a women to the non reproductive. Changes in hormone levels might lead to complaints and health consequences especially during peri- and postmenopause. Hormone therapy has a potential damaging health risk profile and is recommended for temporal limited therapy for acute vasomotor symptoms only.The present HTA-report aims to assess the effectiveness and the cost-effectiveness of alternative treatment methods for women with postmenopausal symptoms in Germany regarding patient relevant endpoints (reduction of symptoms and frequency of adverse events and improvement of quality of life).A systematic literature search was carried out in 33 relevant databases in September 2010. Citations were selected according to pre-defined criteria and were extracted and evaluated.In the systematic research 22 studies are identified for the effectiveness evaluation, 22 primary studies and one review.High doses of isolated genistein reduce the frequency/intensity of hot flashes while low doses of genistein show no significant effect. Intake of isoflavone extract such as genistein, daidzein, glycitein in various combinations does not have an effect on improvement of cognitive function or vaginal dryness. The effect of black cohosh and hop extract for menopausal complaints cannot be determined since results are heterogenous. The combination of isoflavone, black cohosh, monk’s pepper, valerian and vitamin E has a positive effect on menopause symptoms. Ginkgo biloba shows no significant effect on menopause symptoms and cognitive improvement beside mental flexibility. Insbesondere in der Peri- und Postmenopause können Beschwerden aufgrund des sich veränderten Hormonspiegels auftreten. Aktuell wird die Hormontherapie wegen der mit dieser verbundenen Risiken nur noch zur kurzfristigen Behandlung akuter vasomotorischer Symptome empfohlen. Von den betroffenen Frauen werden alternative und komplementäre Heilmethoden eingesetzt.Im vorliegenden Health Technology Assessment (HTA) sollen die Effektivität und Kosteneffektivität alternativer Behandlungsmethoden von postmenopausalen Wechseljahresbeschwerden in Deutschland hinsichtlich der patientenrelevanten Endpunkte Verringerung der Symptomatik, Häufigkeit unerwünschter Ereignisse sowie Verbesserung der Lebensqualität bewertet werden. In 33 relevanten Datenbanken wird im September 2010 eine systematische Literatursuche durchgeführt. Gefundene und relevante Literaturstellen werden gemäß vorab definierter Kriterien selektiert. Akupunktur hat einen signifikanten Einfluss auf Hitzewallungen, insbesondere auf deren Schweregrad.Aufgrund der qualitativen Mängel (Concealment, Randomisierung, Fallzahlplanung) der eingeschlossenen Studien und der begrenzten Anzahl an Studien kann keine abschließende zusammenfassende Bewertung zur Wirksamkeit alternativer Heilmethoden gegeben werden. Die Verallgemeinerbarkeit des vorliegenden HTA wird durch die Beschränkung auf postmenopausale Frauen reduziert.

Hintergrund: Manuelle Medizin (MM) hat einen hohen Stellenwert in der ambulanten Versorgung von Beschwerden des Bewegungsapparats. Trotz Vorliegen zahlreicher randomisiert-kontrollierter Studien und Meta-Analysen im Bereich der MM ist die Bewertung aufgrund verschiedener Schulen und Techniken teilweise schwierig. Ziel der vorliegenden Studie war es, bei der großen Beliebtheit dieser Therapieform die Motivation und Erfahrungen von Ärzten bezüglich der Anwendung von MM in der täglichen Praxis zu ergründen. Methoden: Im Rahmen eines qualitativen Forschungsansatzes wurden 21 semi-strukturierte, telefonische Einzelinterviews mit Ärzten mit der Zusatzqualifikation für MM durchgeführt. Die Rekrutierung erfolgte über die Deutsche Gesellschaft für Manuelle Medizin sowie über einen Verteiler von Ärzten, die an Forschungsfragen zu MM interessiert sind. Die Interviews wurden aufgezeichnet, transkribiert und, unterstützt durch das Computerprogramm Atlas.ti, inhaltsanalytisch ausgewertet. Ergebnisse: Als Hauptmotivation für die Anwendung von MM wurde eine Stärkung der Arzt-Patienten-Beziehung durch einen unmittelbaren Zugang zum Patienten mit schnellen Behandlungserfolgen geschildert. Weitere beschriebene Aspekte sind die Einfachheit der Methode, die Unabhängigkeit von räumlichen oder apparativen Gegebenheiten und die damit verbundenen niedrigen Kosten. Positive, als direkt die Zufriedenheit mit MM beeinflussend beschriebene Faktoren sind die Möglichkeit, eigene Fähigkeiten zu nutzen, den Patienten zu einem gesunden Lebensstil zu motivieren und die Dankbarkeit des Patienten. Als Aspekte, welche die Zufriedenheit negativ beeinflussen, wurden die schlechte Vergütung der Therapie, die unklare Evidenzlage, die Anwendung bei oft selbstlimitierenden Krankheiten und das Risiko schwerwiegender Nebenwirkungen sowie die Gefahr einer iatrogenen Fixierung genannt. Schlussfolgerungen: Die Ergebnisse dieser Studie zeigen eine große Bandbreite von zumeist positiven Sichtweisen und Erfahrungen von Ärzten bei der Anwendung von MM. Dem Wunsch nach dem unmittelbar heilenden Handeln scheint durch MM begegnet zu werden. Die MM scheint als willkommene Abwechslung zur alltäglichen Berufsausübung zu dienen, die als distanziert wahrgenommen wird.

Craniomandibuläre Dysfunktionen (CMD) sind durch Schmerzen und Funktionseinschränkungen von Kiefermuskulatur und -gelenken charakterisiert. Sie sind mit einer Prävalenz von 5 bis 10% ein recht häufiges Phänomen. Bei der Ätiologie werden strukturelle (Okklusion), psychologische (Somatisierung, Depression) und neurophysiologische (muskuläre Hyperaktivität) Faktoren diskutiert, wobei bislang kein gesichertes Störungsmodell existiert. Obwohl sich psychologische Behandlungsansätze, insbesondere Biofeedback und kognitive Verhaltenstherapie, als wirksam erwiesen haben, kommen in der Praxis hauptsächlich zahnmedizinische Behandlungen zum Einsatz. Es liegen jedoch kaum Vergleichsstudien zur Effektivität der beiden Behandlungsansätze vor. Die vorliegende Dissertation verfolgte zwei Ziele. Erstens wurde ein acht Sitzungen umfassendes Biofeedback-gestütztes kognitiv-behaviorales Behandlungskonzept für chronische CMD entwickelt (Studie I) und im Vergleich zur zahnmedizinischen Aufbissschienen-Behandlung im Rahmen einer randomisiert-kontrollierten Therapiestudie an 58 Patienten evaluiert (Studie II). Die Ergebnisse belegen eine vergleichbare Effektivität beider Ansätze im Hinblick auf die Reduktion von Schmerzen und Beeinträchtigung. Die psychologische Behandlung erwies sich hinsichtlich der Verbesserung des psychosozialen Funktionsniveaus, der Schmerzbewältigung und der Akzeptanz als leicht überlegen. Entgegen psychophysiologischer Annahmen waren die Verbesserungen von einer Veränderung der nächtlichen Kiefermuskelaktivität unabhängig. Zweitens wurden Unterschiede in Psychopathologie und nächtlicher Kiefermuskelaktivität an Patienten mit CMD, Personen mit schmerzfreiem Bruxismus sowie gesunden Personen untersucht (Studie III). Die Ergebnisse sprechen für ein deutlich erhöhtes Maß an Psychopathologie bei Patienten mit CMD. Es zeigten sich bei den CMD-Patienten zudem positive Zusammenhänge zwischen nächtlicher Kiefermuskelaktivität, somatoformen Symptomen und CMD-Symptomen, was auf die Bedeutung des psychophysiologischen Erregungsniveaus in der Ätiologie von CMD hinweist. Insgesamt belegt die vorliegende Arbeit die gute Wirksamkeit und Akzeptanz Biofeedback-gestützter kognitiver Verhaltenstherapie im Vergleich zur zahnmedizinischen Behandlung und liefert so einen wichtigen Beitrag zur Verbesserung der Behandlungsmöglichkeiten. Die Befunde tragen darüber hinaus zum besseren Verständnis psychophysiologischer Zusammenhänge bei, wobei die Bedeutung der nächtlichen Kieferaktivität in der Ätiologie und Behandlung von CMD kritisch reflektiert werden muss., Temporomandibular disorders (TMD) are characterized by pain or dysfunction in the temporomandibular muscles and joint. TMD are quite common affecting 5 to 10% of the adult population. Biopsychosocial models of TMD consider structural (occlusion), psychological (somatization, depression), and neurophysiological (muscular hyperactivity) factors. However, the etiology of TMD is far from being completely understood. Despite evidence for the effectiveness of cognitive-behavioral treatment, dental treatment approaches remain far more common in the clinical management of TMD. Few studies, however, have evaluated comparative effectiveness. The present dissertation pursued the following two aims: First, a short biofeedback-based cognitive-behavioral treatment was developed (study I). The approach was evaluated compared to dental treatment with occlusal relaxation splint in a randomized controlled trial of 58 patients (study II). The results showed a comparable effectiveness in terms of pain and disability reduction, however, cognitive-behavioral treatment was slightly superior in improving psychosocial functioning, pain coping, and showed higher treatment satisfaction. Improvements were independent of changes in nocturnal masseter muscle activity. Second, associations of nocturnal masseter muscle activity, TMD symptoms, psychopathology, and somatization were investigated in patients with TMD, subjects with pain free bruxism and healthy controls (study 3). The results showed elevated levels of psychopathology in patients with TMD. Though nocturnal masseter muscle activity was unrelated to pain intensity in TMD, it was positively related to TMD symptoms and somatization, which emphasizes the importance of psychophysiological arousal in the etiology of TMD. In conclusion, the present dissertation supports the importance and feasibility of biofeedback-based cognitive-behavioral treatment in comparison to dental treatment and contributes to improving the clinical management of TMD. Furthermore, the findings contribute to a better understanding of psychophysiological associations within TMD. The role of muscular hyperactivity in the etiology and treatment of TMD needs to be critically reflected.

Scientific background Stable Angina Pectoris (AP) is a main syndrome of chronic coronary artery disease (CAD), a disease with enormous epidemiological and health economic relevance. Medical therapy and percutaneous coronary interventions (PCI) are the most important methods used in the treatment of chronic CAD. Research questions The evaluation addresses questions on medical efficacy, incremental cost-effectiveness as well as ethic, social and legal aspects in the use of PCI in CAD patients in comparison to optimal medical therapy alone. Methods A systematic literature search was conducted in June 2010 in the electronic databases (MEDLINE, EMBASE etc.) and was completed by a hand search. The medical analysis was initially based on systematic reviews of randomized controlled trials (RCT) and was followed by the evaluation of RCT with use of current optimal medical therapy. The results of the RCT were combined using meta-analysis. The strength and the applicability of the determined evidence were appraised. The health economic analysis was initially focused on the published studies. Additionally, a health economic modelling was performed with clinical assumptions derived from the conducted meta-analysis and economic assumptions derived from the German Diagnosis Related Groups 2011. Results Seven systematic reviews (applicability of the evidence low) and three RCT with use of optimal medical therapy (applicability of the evidence for the endpoints AP and revascularisations moderate, for further endpoints high) were included in the medical analysis. The results from RCT are used as a base of the evaluation. The routine use of the PCI reduces the proportion of patients with AP attacks in the follow-up after one and after three years in comparison with optimal medical therapy alone (evidence strength moderate); however, this effect was not demonstrated in the follow-up after five years (evidence strength low). The difference in effect in the follow-up after four to five years was not found for the further investigated clinical endpoints: death, cardiac death, myocardial infarction and stroke (evidence strength high) as well as for severe heart failure (evidence strength moderate). Two studies were included in the health economic analysis. The costs estimations from these studies are not directly transferable to the corresponding costs in Germany. The average difference in the total costs for PCI in comparison with optimal medical therapy alone, which was calculated in the modelling, was found to be 4,217 Euro per patient. The incremental cost-effectiveness ratio per life-year of a patient with avoided AP attacks was estimated to be 24,805 Euro (evidence strength moderate). No publication was identified concerning ethical, social or legal aspects. Discussion Important methodical problems of the studies are a lack of blinding of the patients and incomplete data for several endpoints in the follow-up. The determined incremental cost-effectiveness ratio per life-year of a patient with avoided AP attacks was appraised not to be cost-effective. Conclusions From a medical point of view the routine use of PCI in addition to the optimal medicinal therapy in patients with stable AP can be recommended for the reduction of the proportion of patients with AP attacks after one year and after three years (recommendation degree weak). Otherwise, PCI is to be performed in patients with refractory or progressing AP despite of optimal medical therapy use; in this case PCI is expected to be applied in 27% to 30% of patients in five years. From the health economic view the routine use of PCI in addition to an optimal medical therapy in patients with stable AP cannot be recommended. Background Despite many activities to prevent risky alcohol consumption among adolescents and young adults there is an increase of alcohol intoxications in the group of ten to twenty year old juveniles. Objectives This report gives an overview about the recent literature as well as the German federal prevention system regarding activities concerning behavioral and policy prevention of risky alcohol consumption among children, adolescents and young adults. Furthermore, effective components of prevention activities are identified and the efficiency and efficacy of ongoing prevention programs is evaluated. Methods A systematic literature review is done in 34 databases using Bool’sche combinations of the key words alcohol, prevention, treatment, children, adolescents and young adults. Results 401 studies were found and 59 studies were selected for the health technology assessment (HTA). Most of the studies are done in USA, nine in Germany. A family strengthening program, personalized computer based intervention at schools, colleges and universities, brief motivational interventions and policy elements like increase of prices and taxes proved effective. Discussion Among the 59 studies there are three meta-analyses, 15 reviews, 17 randomized controlled trials (RCT) and 18 cohort studies. Despite the overall high quality of the study design, many of them have methodological weaknesses (missing randomization, missing or too short follow-ups, not clearly defined measurement parameters). The transferability of US-results to the German context is problematic. Only a few prevention activities reach a sustainable reduction of frequency and/or amount of alcohol consumption. Conclusion The HTA-report shows the need to develop specific and target group focused prevention activities for the German situation. Essential for that is the definition of target goals (reduction of consumption, change of behaviour) as well as the definition and empirical validation of risky alcohol consumption. The efficacy of prevention activities should be proven before they are launched. Background The HTA-report (HTA = Health Technology Assessment) deals with the primary prevention of cardiovascular diseases and diabetes mellitus type 2. In 2009 approximately 356,000 people died in Germany due to cardiovascular diseases. According to estimations about 6.3 million people are suffering from diabetes mellitus type 2. The interventions that are subsidized by the public health insurance are mainly focused on sufficient physical activities, healthy nutrition, stress management and the reduction of the consumption of addictive drugs and luxury food. Objectives Which lifestyle-related measures and/or programmes for primary prevention of cardiovascular diseases and of the metabolic syndrome are effective? To what extent will the health status be improved by these offers? To what extent will existing health resources and skills be strengthened by these offers? Are there any differences regarding the effectiveness among the interventions with respect to different settings or subgroups? Which lifestyle-related interventions and/or programmes for primary prevention of cardiovascular diseases and of the metabolic syndrome are sustainable and cost-effective? Which outcome parameters are in the view of the contributors decisive for the evaluation of the effectiveness? In the view of the contributor are there different values between the outcome parameters? In the view of the payers and other actors are there different values between the outcome parameters? Which ethical and juridical factors have to be considered? Which social and/or socio-economic parameters influence the use of the services and effectiveness? Methods A systematic literature research is done in 35 databases. For the period 2005 to 2010, reviews, epidemiological and clinical studies as well as economical evaluations which deal with primary prevention programmes regarding cardiovascular diseases or the metabolic syndrome are included. Results 44 publications meet the inclusion criteria. These studies confirm the effectiveness of the primary prevention programmes. Physical activity programs seem to have a stronger effect than nutrition programmes. Psychological programmes prove as well effectiveness, if they include cognitive behaviour therapy. The identified economical studies indicate that programmes for cardiovascular prevention can be conducted cost-effectively. Interventions that focus on the general population turn out to be particularly cost-effective and sustainable. Discussion There is a wide range of primary preventive effective lifestyle-related interventions with high evidence. The outcomes and results are consistent with the recommendations of the two identified evidence-based guidelines regarding the recommendations on lifestyle and healthy nutrition. Furthermore, the cost-effectiveness of primary prevention services is proven. With regard to the economical studies it is however worth noting that this result is based on very few publications. The transferability has to be critically assessed as the studies mainly originate from the American health system. Conclusion On the whole a comprehensive setting approach with educative, somatic, psychosocial and activity therapeutic components is recommended. The sustainability of a prevention intervention must be ensured from programme to programme. Long-term studies are necessary to make valid statements regarding the sustainable effectiveness: There is an essential deficit in the current practiced evaluation of the use of primary prevention services provided by the health insurance – mainly regarding the comprehensive setting approach – regarding the evidence-based evaluation of the prescribed preventive interventions. With regard to the ethical, social and economical evaluation the research situation is deficient. Although echinocandins such as micafungin and caspofungin have been shown to be effective and tolerated in the treatment of candidal infections there has been no appraisal of the cost-effectiveness of both echinocandins in Germany. This study compares the cost-effectiveness of micafungin and caspofungin in the treatment of invasive candida infections in Germany. To this end, a health economic decision model, based on a phase-III double-blind randomised controlled trial and German hospitalisation and primary medication costs, was used. The effectiveness outcome is defined as patients who are successfully treated and alive at the end of the study period and cost-effectiveness is measured as total costs per patient related to effectiveness. To test for robustness, a two-way sensitivity analysis and a probabilistic sensitivity analysis (PSA) were performed and a cost-effectiveness acceptability curve (CEAC) was calculated. The two-way sensitivity analysis renders micafungin more cost-effective than caspofungin in the global population sample in 19 out of 20 scenarios (20 out of 20 scenarios in the European subsample). Average costs in the PSA were €38,793 for micafungin and €40,072 for caspofungin, while effectiveness was 59.61% and 57.52%, respectively. The average cost-effectiveness is €65,271 for micafungin and €69,835 for caspofungin. For the European subsample, the calculated average costs in the PSA are €40,025 for micafungin and €40,082 for caspofungin, while the effectiveness is 63.29% and 53.69%. Micafungin shows an average cost-effectiveness of €64,412, compared to €76,791 for caspofungin. The CEAC shows that the probability for micafungin being more cost-effective than caspofungin ranges between 51.6% (no willingness to pay for additional costs per successfully treated patient) and 67.7% (infinitely high willingness to pay) in the total sample and between 48.8% and 79.5% in the European subsample. According to this study both lower costs and higher effectiveness show micafungin to be more cost-effective than caspofungin for the treatment of systemic candida infections in Germany.

Die vorliegende Studie hatte zum Ziel zu untersuchen, ob Qigong effektiver bei der Reduktion der mittleren Schmerzen bei Patienten mit chronischen HWS- Schmerzen als keine Therapie ist. Darüber hinaus sollte überprüft werden, ob Qigong einem physiotherapeutischen Nackenübungsprogramm nicht unterlegen ist. Hierzu wurde eine multizentrische, randomisierte, kontrollierte Studie mit drei Armen durchgeführt. Eingeschlossen wurden Patienten, deren unspezifische Nackenschmerzen zwischen sechs Monaten und fünf Jahren bestanden und deren mittlere Intensität bezogen auf die letzten sieben Tage mit mindestens 40 mm auf der Visuellen Analogskala angegeben wurde. Die Patienten der Interventionsgruppen erhielten in den ersten drei Monaten wöchentlich eine Therapieeinheit, in den darauffolgenden drei Monaten nur zweiwöchentlich. Die Teilnehmer der Wartegruppe erhielten während der sechs Monate keine Therapie. Nach Ablauf der Studie konnten die Wartegruppen-Teilnehmer zwischen Qigong und einem physiotherapeutischen Nackenübungsprogramm wählen, wobei das Ergebnis nicht erfasst wurde. Das auf Patienten mit chronischen Nackenschmerzen zugeschnittene Übungsprogramm wurde von hochqualifizierten Therapeuten entwickelt. Mit „Neiyanggong“ wurde eine spezielle Form des Qigongs ausgewählt, die den Schwerpunkt auf stille und bewegte Formen sowie auf spezielle Atemtechniken legte. Als Hauptzielparameter wurde die Differenz der Schmerzintensität zwischen der Qigong- und Wartegruppe anhand der Visuellen Analogskala nach sechs Monaten erhoben. Als sekundärer Zielparameter wurden auch Daten zu Nackenschmerz und Funktionseinschränkung mit der Neck pain and Disability Scale (NPAD), zur Selbswirksamkeitserwartung (SWE), Lebensqualität (SF-36), Schlafqualität und -zufriedenheit, die Übehäufigkeit, die Glaubwürdigkeit und Zufriedenheit mit der erhaltenen Therapie sowie unerwünschte Therapiewirkungen erhoben. Die Patienten füllten vor der Randomisierung (Baseline), nach drei und sechs Monaten standardisierte Fragebögen aus. Als statistische Auswertung wurde eine Kovarianzanalyse (ANCOVA) mit multipler Imputation fehlender Werte und Adjustierung auf Baselinewerte und Erwartungshaltung sowie eine hierarchische Testprozedur durchgeführt. Insgesamt konnten 123 Patienten in die ITT-Analyse eingeschlossen werden. Das Durchschnittsalter lag bei 46 Jahren, 88% der Teilnehmer waren Frauen. Durchschnittlich litten die Patienten 3,2 Jahre an chronischen Nackenschmerzen. Für den primären Zielparameter konnte für den Vergleich der Qigong- mit der Wartegruppe nach sechs Monaten eine signifikante Schmerzreduktion gezeigt werden. Der Vergleich von Qigong- und Nackenübungsgruppe nach sechs Monaten zeigte auch eine größere Schmerzreduktion in der Qigonggruppe als in der Nackenübungsgruppe. Die Nichtunterlegenheit für Qigong konnte aber nicht statistisch gesichert werden. Die Ergebnisse für die sekundären Zielparameter (NPAD und SF-36) waren ähnlich. Nur für den Parameter Allgemeine Selbstwirksamkeitserwartung (SWE) konnte in allen Gruppen zu allen Messzeitpunkten keine Veränderung nachgewiesen werden. In zukünftigen Untersuchungen sollte dieser durch ein sensitiveres Messinstrument ersetzt werden. Zusammenfassend kann festgehalten werden, dass eine sechsmonatige Intervention mit Qigong beziehungsweise Nackenübungen im Vergleich zu keiner Therapie effektiv zur Behandlung von chronischen Nackenschmerzen bezogen auf Schmerzreduktion, Funktionseinschränkung und Lebensqualität ist. Die Nichtunterlegenheit von Qigong gegenüber Nackenübungen konnte jedoch nicht statistisch gesichert werden. Zukünftige Studien sollten sich daher dem weiteren Vergleich von Qigong mit Nackenübungen widmen. Eine höhere Fallzahl scheint hierfür erforderlich., Study Design: Randomized controlled trial Objective: To evaluate whether qigong is more effective than no treatment and not inferior to exercise therapy. Summary of Background Data: Life time prevalence of chronic neck pain is close to 50%. Qigong is often used by patients, although the evidence is still unclear. Methods: Patients (age 20-60 years) with chronic neck pain (VAS ≥ 40 mm) were randomized to 1) qigong or 2) exercise therapy (18 sessions over six months) or 3) waiting list (no treatment). At baseline and after three and six months, patients completed standardized questionnaires assessing neck pain (VAS), neck pain and disability (NPAD), and quality of life (SF-36). The primary endpoint was average pain in the last seven days on VAS at six month follow-up. Statistical analysis included GEE models adjusted for baseline values and patient expectation. Results: A total of 123 patients (age 46 ± 11 years, 88% women) suffering from chronic neck pain for 3.2 (SD ± 1.6) years were included. After six months a significant difference was seen between the qigong and waiting list control groups (VAS mean difference: -14 mm [95% CI, -23.1;-5.4], P= 0.002). Mean improvements in the exercise group were comparable to those in the qigong group (difference between groups -0.7 mm [CI: -9.1; 7.7]) with trend for non-inferiority (p=0.092). NPAD and SF-36 results also yielded superiority of qigong over no treatment and similar results in the qigong and exercise therapy groups. Conclusion: Qigong was more effective than no treatment in patients with chronic neck pain and showed trend for non-inferiority compared to exercise therapy. Further studies with larger sample sizes should clarify the value of qigong compared to other therapeutic strategies.

Background With the advent of remote monitoring, current models of implantable cardioverter defibrillators (ICDs) have the possibility of sending automatic alert messages that allow early diagnosis of events such as lung fluid overload, atrial fibrillation and device integrity issues. Timely treatment of these events has the potential to improve patient outcome, but this has not as yet been proven. Methods The MORE-CARE study is a multicenter randomized controlled trial evaluating the efficacy of advanced device diagnostics and remote monitoring in improving the outcome of patients with biventricular ICDs. Up to 1720 patients with a standard indication for a biventricular ICD will be randomized to standard in-office follow-up, or to a remote monitoring strategy using the CareLink network and involving automatic alerts for lung fluid overload, atrial fibrillation, and device integrity issues. The first phase aims at evaluating the delay between an alert event, and clinical action to the event. The second phase of the study will evaluate whether the remote monitoring strategy results in a significant reduction of a combined end point of total mortality or cardiovascular and device-related hospitalization. The duration of the study will be event-driven due to its sequential design. Conclusion MORE-CARE will evaluate the efficacy of remote monitoring for improving patient outcome in patients implanted with a biventricular ICD.

Health political background The comparison of the effectiveness of health technologies is not only laid down in German law (Social Code Book V, § 139 and § 35b) but also constitutes a central element of clinical guidelines and decision making in health care. Tools supporting decision making (e. g. Health Technology Assessments (HTA)) are therefore in need of a valid methodological repertoire for these comparisons. Scientific background Randomised controlled head-to-head trials which directly compare the effects of different therapies are considered the gold standard methodological approach for the comparison of the efficacy of interventions. Because this type of trial is rarely found, comparisons of efficacy often need to rely on indirect comparisons whose validity is being controversially debated. Research questions Research questions for the current assessment are: Which (statistical) methods for indirect comparisons of therapeutic interventions do exist, how often are they applied and how valid are their results in comparison to the results of head-to-head trials? Methods In a systematic literature research all medical databases of the German Institute of Medical Documentation and Information (DIMDI) are searched for methodological papers as well as applications of indirect comparisons in systematic reviews. Results of the literature analysis are summarized qualitatively for the characterisation of methods and quantitatively for the frequency of their application. The validity of the results from indirect comparisons is checked by comparing them to the results from the gold standard - a direct comparison. Data sets from systematic reviews which use both direct and indirect comparisons are tested for consistency by of the z-statistic. Results 29 methodological papers and 106 applications of indirect methods in systematic reviews are being analysed. Four methods for indirect comparisons can be identified: Unadjusted indirect comparisons include, independent of any comparator, all randomised controlled trials (RCT) that provide a study arm with the intervention of interest. Adjusted indirect comparisons and metaregression analyses include only those studies that provide one study arm with the intervention of interest and another study arm with a common comparator . While the aforementioned methods use conventional metaanalytical techniques, Mixed treatment comparisons (MTC) use Bayesian statistics. They are able to analyse a complex network of RCT with multiple comparators simultaneously. During the period from 1999 to 2008 adjusted indirect comparisons are the most commonly used method for indirect comparisons. Since 2006 an increase in the application of the more methodologically challenging MTC is being observed. For the validity check 248 data sets, which include results of a direct and an indirect comparison, are available. The share of statistically significant discrepant results is greatest in the unadjusted indirect comparisons (25,5% [95% CI: 13,1%; 38%]), followed by metaregression analyses (16,7% [95% CI: -13,2%; 46,5%]), adjusted indirect comparisons (12,1% [95% CI: 6,1%; 18%]) and MTC (1,8% [95% CI: -1,7%; 5,2%]). Discrepant results are mainly detected if the basic assumption for an indirect comparison - between-study homogeneity - does not hold. However a systematic over- or underestimation of the results of direct comparisons by any of the indirectly comparing methods was not observed in this sample. Discussion The selection of an appropriate method for an indirect comparison has to account for its validity, the number of interventions to be compared and the quality as well as the quantity of available studies. Unadjusted indirect comparisons provide, contrasted with the results of direct comparisons, a low validity. Adjusted indirect comparisons and MTC may, under certain circumstances, give results which are consistent with the results of direct comparisons. The limited number of available reviews utilizing metaregression analyses for indirect comparisons currently prohibits empirical evaluation of this methodology. Conclusions/Recommendations Given the main prerequisite - a pool of homogenous and high-quality RCT - the results of head-to-head trials may be pre-estimated by an adjusted indirect comparison or a MTC. In the context of HTA and guideline development they are valuable tools if there is a lack of a direct comparison of the interventions of interest. Gesundheitspolitischer Hintergrund Die vergleichende Nutzenbewertung von gesundheitsrelevanten Technologien ist nicht nur gesetzlich festgeschrieben (§ 139 und § 35b, SGB V: SGB = Sozialgesetzbuch), sondern ist auch ein zentrales Element von klinischen Leitlinien bzw. Entscheidungssituationen. Entscheidungsunterstützungsinstrumente wie Health Technology Assessments (HTA) sollten daher über ein valides methodisches Instrumentarium verfügen. Wissenschaftlicher Hintergrund Randomisierte kontrollierte Head-to-head-Studien, die Therapien direkt miteinander vergleichen, gelten als Goldstandard für den Wirksamkeitsvergleich. Da Studien dieses Typs nur begrenzt zur Verfügung stehen, sind Wirksamkeitsvergleiche auf indirekt vergleichende Methoden angewiesen, deren Validität allerdings noch kontrovers diskutiert wird. Fragestellung Fragestellungen für das vorliegende Assessment sind: Welche (statistischen) Methoden zur Durchführung indirekter Vergleiche therapeutischer Interventionen gibt es, wie häufig werden sie eingesetzt und wie ist ihre Validität im Vergleich zu den Ergebnissen direkter Vergleiche zu beurteilen? Methodik In einer systematischen Literaturrecherche werden die medizinnahen Datenbanken des Deutschen Instituts für medizinische Dokumentation und Information (DIMDI) nach Methodenpapieren und Anwendungen von indirekten Vergleichen in systematischen Reviews durchsucht. Die Literaturauswertung erfolgt qualitativ beschreibend (Methoden) und quantitativ für die Häufigkeit ihres Einsatzes. Eine Validitätsprüfung der Methoden für indirekte Therapievergleiche ist über den Vergleich ihrer Ergebnisse mit dem Goldstandard - den Ergebnissen von Head-to-head-Studien - möglich. In systematischen Reviews, in denen Therapieverfahren sowohl direkt als auch indirekt verglichen werden, werden diese Ergebnisse mithilfe der z-Statistik auf Übereinstimmung geprüft. Ergebnisse 29 Methodenpapiere und 106 Methodenanwendungen werden ausgewertet. Aus diesen lassen sich vier Methoden für indirekte Vergleiche identifizieren: Nicht-adjustierte indirekte Vergleiche schließen, unabhängig vom Komparator, alle randomisierten kontrollierten Studien (RCT), die einen Studienarm mit einer der interessierenden Therapieoptionen enthalten, ein. Adjustierte indirekte Vergleiche und Metaregressionen greifen nur auf Studien zurück, die einen Arm mit einer Therapieoption von Interesse und einen Arm mit einem gemeinsamen Komparator aufweisen. Während die genannten Verfahren konventionelle Metaanalysetechniken einsetzen, können die mit Bayes'schen Methoden arbeitenden Mixed treatment comparisons (MTC), ein komplexes Netzwerk aus RCT mit multiplen Komparatoren simultan analysieren. Im Zeitraum von 1999 bis 2008 werden die adjustierten indirekten Vergleiche am häufigsten angewendet. Seit 2006 ist auch ein deutlicher Anstieg der Verwendung des methodisch anspruchsvolleren MTC zu verzeichnen. Für die Validitätsprüfung stehen 248 Datensätze mit Ergebnisgegenüberstellungen aus direktem und indirektem Vergleich zur Verfügung. Der Anteil diskrepanter Ergebnisse mit statistischer Signifikanz war am größten bei den nicht-adjustierten indirekten Vergleichen (25,5% [95%-KI: 13,1; 38]), gefolgt von dem der Metaregressionen (16,7% [95%-KI: -13,2; 46,5]), der adjustierten indirekten Vergleiche (12,1% [95%-KI: 6,1; 18]) und des MTC (1,8% [95%-KI: -1,7; 5,2]). Diskrepante Ergebnisse werden vor allem dann beobachtet, wenn die Voraussetzung für die Durchführung eines indirekten Vergleichs - eine homogene Studienlage - nicht gegeben ist. Eine systematische Über- oder Unterschätzung der Ergebnisse direkter Vergleiche durch den indirekten Vergleich zeigt sich in dieser Stichprobe bei keinem der genannten Verfahren. Diskussion Die Auswahl einer geeigneten Methode für einen indirekten Vergleich hat sich an deren Validität, der Anzahl der zu vergleichenden Therapieoptionen sowie an der Qualität und Quantität der verfügbaren Studien zu orientieren. Nicht-adjustierte indirekte Vergleiche weisen in Gegenüberstellung zu direkten Vergleichen eine geringe Validität auf. Adjustierte indirekte Vergleiche und MTC können dagegen unter bestimmten Voraussetzungen Ergebnisse liefern, die in den meisten Fällen denen direkter Vergleiche entsprechen. Die Validität von indirekten Vergleichen mittels Metaregression kann auf Basis der wenigen, bisher verfügbaren Anwendungsbeispiele noch nicht beurteilt werden. Schlussfolgerung/Empfehlung Bei Einhaltung der zentralen Voraussetzung - Anwendung an einem Pool homogener, qualitativ hochwertiger RCT - lassen sich die Ergebnisse von qualitativ hochwertigen Head-to-head-Studien durch den Einsatz adjustierter indirekter Vergleiche und MTC abschätzen. Im Kontext von HTA und Leitlinienerstellung stellen sie somit wertvolle Hilfsinstrumente dar, wenn direkte Evidenz für einen Wirksamkeitsvergleich von Therapieverfahren nicht zur Verfügung steht.

BACKGROUND Many patients with chronic hepatitis C have not responded to therapy with pegylated interferon plus ribavirin. OBJECTIVE To evaluate use of peginterferon-alpha2a plus ribavirin to re-treat nonresponders to peginterferon-alpha2b plus ribavirin. DESIGN Randomized, parallel-group trial conducted between September 2003 and February 2007. Patients and researchers were not blinded to intervention assignment. Random assignment was centralized, computer-generated, and stratified by geographic region, hepatitis C virus (HCV) genotype, and histologic diagnosis. SETTING 106 international centers. PATIENTS 950 nonresponders to 12 or more weeks of therapy with peginterferon-alpha2b plus ribavirin. INTERVENTION Peginterferon-alpha2a, 360 microg/wk, for 12 weeks, then 180 microg/wk to complete 72 weeks (group A) or 48 weeks (group B), or peginterferon-alpha2a, 180 microg/wk for 72 weeks (group C) or 48 weeks (group D). Background: The optimal choice of technique for the surgical treatment of pancreatic head lesions in chronic pancreatitis (CP) is still under debate. This systematic review and meta-analysis aims to compare the effectiveness and safety of duodenum-preserving pancreatic head resection (DPPHR) versus pancreatoduodenectomy (PD) by means of parameters of mortality and morbidity as well as functional outcomes and quality of life. Methods: Systematic literature search (Medline, Embase, Biosis, The Cochrane Library and Science Citation Index). Quality assessment and data extraction of identified and included randomized controlled trials (RCTs) comparing DPPHR and PD. Meta-analysis of predefined endpoints (pain relief (primary endpoint), blood loss, operation time, hospital stay, occupational rehabilitation, quality of life etc.) using a random effects-model (Review Manager Software package 4.2.1). Results: 1284 citations were checked for eligibility and 5 RCTs were finally included. Critical appraisal revealed heterogeneous methodological quality of included trials. While postoperative pain relief as well as overall mortality and morbidity showed no significant results, intraoperative blood replacement and hospital stay were significantly reduced in the DPPHR group. Postoperative weight gain, occupational rehabilitation and quality of life were also significantly improved in the DPPHR group. Discussion: This systematic review and meta-analysis suggests that PD has no advantage in terms of effectiveness or safety when compared to DPPHR. Given significant superiority of DPPHR regarding several perioperative outcome parameters as well as improved occupational rehabilitation and quality of life, surgical treatment of chronic pancreatitis has to be re-evaluated in pragmatic multiinstitutional randomized trials to support evidence-based surgical decisions.

Background: Fast track programs — multimodal therapy strategies — have been introduced in many surgical fields to minimize postoperative morbidity and mortality. Morbidity after elective open infrarenal aneurysm repair is as high as 30 %, mortality ranges up to 10 %. In terms of open infrarenal aneurysm repair no randomized controlled trials exist to introduce and evaluate such patient care programs. Methods: In a prospective, randomized controlled pilot study a »traditional« and »fast track« treatment regimen in patients undergoing open infrarenal aneurysm repair was compared. Main differences between the two groups consisted in preoperative bowel washout (none vs. 3L cleaning solution) and analgesia (patient controlled analgesia vs. patient controlled epidural analgesia: PCA vs. PCEA). Study endpoints were morbidity and mortality, need for postoperative mechanical ventilation and length of stay (LOS) on intensive care unit (ICU). Results: 82 patients underwent randomization 79 were analyzed of per protocol. Both study groups were similar in terms of age, sex, ASA score and aneurysm diameter. In both groups trans- and retroperitoneal approach to the infrarenal aorta was performed. The need for assisted postoperative ventilation was significantly higher in the traditional group (33.3 % vs. 5.4 %; p = 0.011). Median LOS on ICU was shorter in the fast track group: 41 h vs. 20 h. Gastrointestinal, renal, cardiac or infection associated complications were significantly lower in the fast track group: 16.2 % vs. 35.7 %; (p = 0.045); mortality in both groups was 0 %. Conclusion: We introduced and evaluated an optimized patient care program for patients undergoing open infrarenal aortic aneurysm repair. In terms of postoperative mechanical ventilation and morbidity we found a significant advantage for »fast track« patients. Using this optimized patient care management morbidity of open infrarenal aortic aneurysm repair could be significantly decreased as compared to a traditional treatment regimen.

Even though the primary success rate of Punction of the Vena Subclavia (PVS) for an implantation of a Totally Implantable Access Port (TIAP) is described as significantly higher than the surgical technique of classical Venae Sectio (cVS) the latter has the advantage of not causing pneumothorax. Therefore, the surgical technique must be improved. We conducted a randomized controlled trial to evaluate the primary success rate of the cVS in comparison to a modified Venae Sectio (cVS + Seldinger technique). 206 patients were screened between March 2006 and January 2007. 164 patients could be randomized and the primary success rate was determined. There was no significant difference between the primary success rate of cVS (81 %) and mVS (84 %) employing the Chi2 test. But performing either cVS or mVS lead to a overall success rate of 94 % of the two surgical techniques. Only 6 % of the patients needed to receive a PSV for implantation of a TIAP.

Dargestellt werden die Ergebnisse aus drei feldexperimentellen Studien zu den kurzfristigen Effekten von sozialem Stress auf die Konzentrationsleistungen von Kindern. An den Experimenten waren insgesamt 250 Kindergartenkinder beteiligt, deren kurzfristige Konzentrationsleistungen wiederholt mit der Kaseler-Konzentrations-Aufgabe für 3- bis 8-Jährige (KKA) erfasst wurden. Ergänzend wurden die Kinder direkt nach der Konzentrationstestung zu subjektiv erlebten Störungen und ihrer aktuellen Müdigkeit befragt. Nach dem Zufall wurden 90 Kindergartenkinder in Studie I einer Standard-Einzeltestung, einer triadischen Testsituation mit einem anderen Kind als Beobachter und einer individuellen Testsituation in der lauten Kindergartengruppe zugewiesen. In Studie II wurden 40 Kindergartenkinder individuell wiederholt in einer ruhigen Standard-Testsituation, unter Lärmbedingungen durch außerhalb des Testraums frei spielende Kinder und unter Geräuschbedingungen durch lautes Singen anderer Kinder in ihren Gruppenräumen getestet. An Studie III waren 120 Kindergartenkinder beteiligt, die kurz nach der Kindergarten-Ankunft, nach dem gemeinsamen Frühstück sowie vor und nach dem gemeinsamen Mittagessen untersucht wurden. Die Ergebnisse weisen auf sehr starke negative Effekte von direktem sozialen Stress (Studie I) auf die Konzentrationsleistungen. Mit Ausnahme mittlerer Stressreaktionen bei der Testung nach dem gemeinsamen Mittagessen waren für die typischen Kindergarten-Geräuschbedingungen in Studie III und auch in Studie II keine negativen Einflüsse auf die Konzentration zu beobachten. Damit weisen die Befunde bei Kindergartenkindern auf solide Habituationen an den in Kindergärten üblichen sozialen Stress und auf starke spezifische Stressreaktionen (Konzentrationsabfälle) in sozialen Situationen mit Beobachtern und lauten Gruppenbedingungen. Die subjektiven Belastungseinschätzungen weichen deutlich von den objektiven Testbefunden ab, was die Fähigkeiten von Kindergartenkindern in Frage stellt, sprachlich valide über ihre Stressreaktionen zu berichten. In all 250 kindergarten-children were tested repeatedly with the Kaseler-Konzentrations-Aufgabe (KKA; Kasel-Concentration-Task for Children Aged 3-8 Years; test-duration: 6-7 minutes). In addition, after each KKA-test the children were asked about their subjective feelings of tiredness and of distractions during the test. In Study I 90 kindergarten-children were randomized to a standard individual test-setting, a triadic test-setting with another child observing the test child, and an individual test-setting within the noisy kindergarten group. Study II refers to 40 kindergartenchildren tested individually and repeatedly in a silent standard setting, in a noisy setting with other children playing loudly outside, and in a noisy setting with other children singing very loudly in their group-rooms outside the testing-room. 120 kindergarten-children took part in Study III, in which test time and social setting (shortly after arrival in kindergarten, after joint breakfast, before and after joint lunch) were varied. In Study I results point at large negative effects of social stress on concentration performance. No effects of typical kindergarten noises on concentration were observed in Study II and Study III, with the exception of stress reactions on concentration in the after-joint-lunch kindergarten setting. Results point at good habituations to common social stress in kindergarten-children as well as specific strong stress reactions on concentration in social settings, including observers and noisy group-settings. Subjective ratings of distractions during testing and of tiredness after testing are not correlated with cognitive stress reactions, putting the competence of kindergarten-children to report verbally solid about their stress reactions in question. Background Attempts to improve the tolerability of combined oral contraceptives (COCs) have included the substitution of ethinylestradiol (EE) with 17β-estradiol (E2). However, this has proved unsatisfactory, specifically in terms of cycle control. To improve upon the poor cycle control seen previously, E2 [in the form of estradiol valerate (E2V); 1 mg of E2V contains 0.76 mg of E2] was combined with dienogest (DNG) in a novel four-phasic regimen. In the current studies, the ovulation-inhibition potency of four variations of this regimen was assessed. Study Design Two randomized, open-label, Phase II studies were performed. The first study compared two regimens (Regimens 1A and 2A) with similar dosages of DNG but different lengths of application. Having established in Study 1 that the length of application of Regimen 2A was most suitable, but that the dosages of DNG were too low for effective ovulation inhibition, a second study, which compared two regimens (Regimens 2B and 2C) with similar lengths of application but with increased dosages of DNG, was undertaken. The primary efficacy variable in both studies was the proportion of women with a Hoogland score of 5 or 6 during Cycle 2. Results The full analysis set comprised 192 and 203 women in Studies 1 and 2, respectively. In Study 1, 10 women (10.9%) in Regimen 1A and 6 women (6.4%) in Regimen 2A had a Hoogland score of 5 or 6. In Study 2, three women (3.1%) in Regimen 2B and one woman (1.0%) in Regimen 2C had a Hoogland score of 5 or 6. There were no safety concerns with any of the regimens. Conclusion The results of these studies identified a four-phasic COC preparation comprising E2V/DNG that provides efficient ovulation inhibition. It is expected that this regimen will lead to an innovative COC containing E2 instead of EE.

Background: It is still under debate if the use of somatostatin and its analogues reduce the incidence of complications in pancreatic surgery. The objective of this systematic review and meta-analysis is to quantitatively assess the efficacy of the perioperative use of somatostatin in pancreatic surgery. Methods: A systematic literature search was performed. Randomized controlled trials (RCTs) comparing the perioperative treatment with or without use of somatostatin were eligible. Main outcome measures were overall postoperative morbidity, pancreatic associated complications, pancreatic fistula, mortality, and the length of hospital stay. Further analyses were performed for pre-specified subgroups. Summarized treatment effect was computed in a meta-analysis using the random effects model. Results: A total of 16 RCTs referring to the use of somatostatin or its analogues in pancreatic surgery including overall 2153 patients met the inclusion criteria. Meta-analysis of somatostatin versus control revealed a significant reduction of overall postoperative morbidity for the treatment group. Accordingly, pancreatic associated complications and the rate of pancreatic fistula were reduced significantly in the somatostatin group. There were no differences in mortality and hospital stay. Similar results were determined in subgroup analyses of patients with pancreatic cancer or chronic pancreatitis, pancreaticoduodenectomy or distal pancreatectomy, and application of somatostatin s. c. or i. v. Conclusion: According to the results no beneficial effect of the use of somatostatin in pancreatic resections regarding lower mortality rate or shortened hospital stay could be demonstrated. However, the use of somatostatin significantly reduced the rate of overall and pancreas specific postoperative complications, in particular also the occurrence of pancreatic fistula.
