Abstract

International audience; IntroductionANCA vasculitis may involve the skin and develop slowly without specific histology, and without autoantibodies.Case reportWe report a 50-year-old woman who experienced bilateral mastectomy because of ulcero-necrotic, non-specific inflammatory cutaneous lesions of the breasts. First considered by others as a malinger patient, she developed oto-neurological lesions leading to the diagnosis of Wegener's granulomatosis. Five years later, specific antibodies of the disease were present.ConclusionCutaneous involvement by ANCA vasculitis can be isolated for a long time. Physicians must have a high degree of suspicion to avoid diagnostic delay of ANCA vasculitis.; IntroductionUne granulomatose avec polyangéite peut débuter par une atteinte cutanée isolée et torpide. L’analyse histologique peut alors être totalement aspécifique et les anticorps anti-cytoplasme des polynucléaires absents.ObservationNous rapportons l’observation d’une patiente de 50 ans qui avait subit une mastectomie bilatérale, devant des lésions ulcéro-nécrotiques sans diagnostic. Longtemps considérée par d’autres comme pathomimique, elle développait par la suite une atteinte otoneurologique spécifique de granulomatose avec polyangéite. Les auto-anticorps, quant à eux, ne se positivaient que cinq ans plus tard.ConclusionUne granulomatose avec polyangéite peut parfois avoir une évolution très torpide et le diagnostic peut en être largement retardé. Il ne faut pas conclure trop vite à une pathomimie devant une lésion cutanée qui ne cicatrise pas.

Abstract

Sicca syndrome after Stevens-Johnson syndrome is classically described. However, to our knowledge, authentic Sjögren's syndrome following epidermal necrolysis has not been described previously.A 36-year-old woman with Steven-Johnson syndrome developed transient hepatitis and a persistent sicca syndrome. Fourteen years later an authentic Sjögren's syndrome was identified with presence of antinuclear and anti-SSA antibodies and lymphocytes infiltration of the accessory salivary glands, i.e. grade IV disease in the Chisholm classification.The initial mucosal destruction observed in our patients may have laid the ground for Sjögren's syndrome via abnormal antigen presentation in a predisposed dysimmune context.

Abstract

Primary periodic palsy is a group of muscular diseases transmitted by autosomal dominant inheritance. The characteristic features are flaccid muscular deficiency, abolition of reflexes, and dyskalemia.A 36-year-old woman presented a 5-year history of acute episodes of myalgia, muscle contracture, and muscle paralysis involving first the hand, then all four limbs and the face, sparing the respiratory muscles. The symptoms totally regressed spontaneously within one hour or in a shorter time if the patient ingested sugar. Demonstration of hyperkalemia during an acute episode led to the diagnosis of transfer hyperkalemia typical of Gamstorp's disease. Several members of the family presented identical symptoms.Despite progress in our understanding of this disease, diagnosis is often made late. The diagnostic strategy is however quite simple: serum potassium during an acute episode and provocation test with fasting and rest.

Abstract

Résumé Introduction Le syndrome de l’homme raide est une pathologie neurologique rare associant une rigidité fluctuante du tronc et des membres et des spasmes musculaires douloureux. Les variants cliniques incluent le syndrome des membres raides où la rigidité ne touche qu’un des membres et l’encéphalomyélite progressive avec rigidité et myoclonies ( progressive encephalomyelitis with rigidity and myoclonus [PERM]), caractérisée par l’association d’une raideur et de spasmes myocloniques avec des signes focaux. Observation Nous rapportons un cas de PERM, chez une patiente âgée de 73 ans, initialement explorée pour une altération de l’état général assortie de douleurs abdominales. Conclusion De par ses présentations cliniques diverses et parfois non neurologiques, le diagnostic de PERM est souvent fait tardivement. La recherche d’anticorps antineuropile dans le sérum et le LCR, et l’aspect de spasmes à l’électromyogramme sans atteinte myogène caractérisent cette pathologie neurologique atypique. Introduction Stiff-person syndrome is rare neurological disease, associating trunk rigidity and painful muscular spasms. A clinical variant of stiff person syndrome is the progressive encephalomyelitis with rigidity and myoclonus (PERM), which includes neurological cognitive disturbances. Case report We report a 73-year-old woman initially addressed for abdominal pain, anorexia and severe weight-loss, for whom diagnosis of PERM was made. Conclusion Because of its various clinical presentations, sometimes without evidence for neurological disease, the diagnosis of PERM is delayed. The presence of antineuropile antibodies associated with muscular spasms at electromyogram are strong evidence for this diagnosis. [ABSTRACT FROM AUTHOR]

Abstract

Abstract: Links between Takayasu''s arteritis (TA) and tuberculosis are discussed in the literature. We report the case of a Caucasian woman who was first seen for a regressive fever, associated with a normal clinical and chest and abdominal CT-scan examination. A minor granulomatous hepatitis was documented. She had no symptoms for the following four years. A second episode of persisting fever led to the diagnosis of simultaneous occurrence of diffuse TA and severe disseminated tuberculosis. Both affections were treated and the patient was still in good health after three years of follow-up. Simultaneous occurrence of both diseases in our observation supports evidence for a relationship between those two granulomatous diseases. [Copyright &y& Elsevier]

Abstract

Introduction. – Ictero-hemorrhagic leptospirosis is an endemic disease in France. Weil''s disease, a form of leptospirosis, is well known. Fort Bragg fever is characterized by a constant pretibial papular lesion. First described in the USA, this non icteric form of leptospirosis is usually benign. We report the first French case of a mixed form of leptospirosis.Exegesis. – A 52-year-old man living in South East France suffered from fever and myalgias associated with a pretibial papular lesion. A severe icterus appeared and permitted a diagnosis of leptospirosis.Conclusion. – Our case recalls the clinical presentation of Fort Bragg fever, which is recognized through its inflammatory pretibial lesion associated or not with icterus.
