Objectifs La radiothérapie stéréotaxique est une option curative chez les patients atteints de tumeur hépatique primitive ou secondaire. La tomographie d’émission monophotonique à la ( 99m Tc)-mébrofénine (99TcM-Spect) est utilisée avant l’opération pour estimer la fonction du foie restant après une hépatectomie. Nous présentons le cas d’un patient recevant une irradiation stéréotaxique pour un carcinome hépatocellulaire avec un plan optimisé en utilisant les données de la 99TcM-SPECT. Matériels et méthodes Une indication de radiothérapie stéréotaxique a été donnée pour un patient de 84 ans atteint d’un carcinome hépatocellulaire développé sur une cirrhose (de score CHILD B8) d’origine métabolique. Après une scanographie dédiée à la radiothérapie, le patient a bénéficié d’une 99TcM-SPECT. Les données de la 99TcM-SPECT ont été intégrées sur cette scanographie et les structures correspondant à l’activité hépatique ont été créées. Une contrainte de dose moyenne inférieure à 15 Gy a été appliquée sur les 700 cm 3 de foie les plus actifs. Trois mois après le traitement, nous avons réalisé une 99TcM-SPECT et étudié la variation de l’activité hépatique avant et après le traitement. Résultats Nous avons délivré une arcthérapie dynamique de 45 Gy en trois fractions de 15 Gy sans complication. La 99TcM-SPECT de contrôle a permis de mettre en évidence que l’activité hépatique était diminuée après la radiopthérapie stéréotaxique. Pour une dose donnée, les variations d’activité hépatique étaient plus importantes dans les régions de foie plus actives. La variation moyenne d’activité hépatique du foie le moins actif était de 46 % (16 %–60 %) tandis qu’elle était de 80 % (52 %–87 %) pour le foie le plus actif. Cette différence était statistiquement significative ( p < 0,01). Conclusion L’intégration de la 99TcM-SPECT dans la planification de radiothérapie est faisable est utile pour protéger les régions du foie qui fonctionnent le mieux. L’intérêt de protéger ces régions est d’autant plus grand que ces régions semblent être aussi les plus sensibles à l’irradiation. [ABSTRACT FROM AUTHOR]

Objectif de l’étude Les objectifs de ce travail étaient d’identifier, pour des cancers non oropharyngé localement évolués pris en charge par chimioradiothérapie, des paramètres de tomographie par émission de positons (TEP) quantitatifs prédictif de la survie sans progression et dans un second temps d’établir un score pronostique. Matériel et méthode Un total de 78 patients, avec un suivi médian de 23 mois, issus de trois centres (Rennes, Lorient et Lausanne), ont été inclus dans cette étude rétrospective. En plus de la standard uptake value maximale (SUVMax), les paramètres de TEP de volume ( metabolic tumor volume [MTV] et total lesion glycolysis [TLG]) ont été calculés en utilisant différentes valeurs de seuillage en valeur absolue (de 0 à 20) ou relative (de 0 à 100 % de la SUVMax). L’ensemble des paramètres cliniques et de TEP a été analysé et une corrélation avec la survie sans récidive a été recherchée. Un modèle pronostique de survie sans progression a été défini. Une validation interne a été réalisée par une méthode de ré-échantillonnage ( bootstrap ). Résultats En analyse multifactorielle, le MTV était le seul facteur pronostique indépendant de la survie sans progression, alors que les facteurs classiques cliniques ([stade T, volume tumoral macroscopique], stade selon l’American Joint Committee on Cancer [AJCC]) étaient peu ou pas significatifs. Après ré-échantillonnage, le c-index ajusté était de 0,69. La calibration interne a retrouvé un bon ajustement entre la survie sans progression prédite et la survie sans progression observée à 24 mois. Un score prédictif a été calculé, permettant d’identifier deux groupes à risque. La durée médiane de la survie sans progression était de 37 mois (intervalle de confiance à 95 % [IC95 %] : 36–55 mois pour le groupe de bas risque et de 8 mois [IC95 % : 5–15 mois] pour le groupe de haut risque, p < 0,01). Conclusion Le MTV est un puissant facteur pronostique de la survie sans progression chez les patients pris en charge par chimioradiothérapie. L’utilisation du MTV pourrait permettre une identification précoce des cancers de pronostic défavorable, candidat potentiel à une intensification thérapeutique. Objectif de l’étude Les objectifs de ce travail étaient d’identifier, pour des cancers du col utérin pris en charge par chimioradiothérapie, les paramètres de tomographie par émission de positons (TEP) quantitatifs prédictifs du contrôle locorégional, de la survie sans récidive et de la survie globale. Matériel et méthode Trente-sept patientes, avec un suivi médian de 52 mois, ont été incluses dans cette étude rétrospective. En plus de la standard uptake value maximale (SUVMax), les paramètres de TEP de volume ( metabolic tumour volume [MTV] et total lesion glycolysis [TLG]) ont été calculés en utilisant différentes valeurs de seuillage en valeur absolue (de 0 à 20) ou relative (de 0 à 100 % de la SUVMax). Un nouveau paramètre de distance tumorale métabolique combinant le MTV ganglionnaire avec la distance entre la tumeur primitive et chaque ganglion atteint a également été défini. L’ensemble des paramètres cliniques et de TEP a été analysé et une corrélation avec la survie globale, la survie sans récidive et le contrôle locorégional a été recherchée. Un modèle pronostique de survie globale a été défini. Une validation interne a été réalisée par validation croisée. Résultats En analyse multifactorielle la TLG de la tumeur et la distance tumorale métabolique étaient les deux facteurs pronostics indépendants prédisant la survie globale ( p < 0,01), la survie sans récidive ( p < 0,01) et le contrôle locorégional ( p = 0,046). Après validation croisée, le c-index pour la survie globale, la survie sans récidive et le contrôle locorégional était de 0,63, 0,68 et 0,66. Un score prédictif a été calculé, permettant d’identifier deux groupes à risque. Les probabilités de survie globale, de survie sans récidive et de contrôle locorégional à 3 ans étaient de 88 %, 78 % et 84 % pour le groupe à bas risque contre 45 %, 33 % et 38 % pour le groupe à haut risque ( p < 0,01). Conclusion La TLG tumorale et la distance entre la tumeur primitive et chaque ganglion atteint sont des paramètres pronostique de la survie après chimioradiothérapie pour des cancers du col utérin. Ces paramètres pourraient permettre une identification précoce des patientes atteintes de cancer de pronostic défavorable, candidates potentielles à une intensification thérapeutique. Objectif de l’étude L’objectif de cette étude était d’évaluer l’impact de la dose de radiothérapie sur la perfusion hépatique en intégrant la scintigraphie hépatique à la mébrofénine avant et aprèsradiothérapie chez des patients traités par irradiation stéréotaxique hépatique. Matériels et méthode Entre avril et septembre 2015, six patients atteints d’une néoplasie hépatique, primaire pour trois, secondaire pour trois, ont été traités par irradiation stéréotaxique, de trois fractions de 15 Gy pour quatre, cinq de 8 Gy pour un, six de 5 Gy pour un). Tous les patients ont eu une scintigraphie hépatique à la mébrofénine pour évaluer la perfusion hépatique avant et trois mois après la radiothérapie, qui ont été fusionnées avec le scanographie de planification. Pour chaque patient, l’équivalent biologique de dose à 2 Gy par fraction (EQD2) a été calculé avec un rapport alpha/bêta de 10 Gy pour la toxicité aigüe. Les courbes d’isodoses (5, 10, 20, 30, 40, 50, 60, 70, 80 et 90 Gy) ont été dessinées, puis, nous avons calculé l’activité (MBq) dans ces volumes. Résultats Une régression linéaire a montré une réduction significative de la fonction hépatique à trois mois. Cette variation était proportionnelle à l’augmentation de dose de radiothérapie ( p = 0,0009), avec une réduction de 0,78 % de la perfusion hépatique pour chaque Gy délivré. L’équation linéaire trouvée avait une valeur prédictive de la perte de fonction en fonction de la dose délivrée de 96 % (R2 = 0,9605). Conclusion Nous avons montré pour la première fois l’utilité de la scintigraphie hépatique à la mébrofénine pour l’évaluation de la variation de fonction hépatique après une radiothérapie stéréotaxique. Objectif de l’étude Nous avons évalué l’impact de la dose de radiothérapie sur la perfusion pulmonaire avec la scintigraphie pulmonaire avant et après le traitement chez des patients traités pour des cancers pulmonaires primitifs ou secondaires. Patients et méthodes Entre juin 2014 et septembre 2015, 15 patients atteints d’une néoplasie pulmonaire primaire ( n = 11) ou secondaire ( n = 4) ont été pris en charge par chimiothérapie et radiothérapie (dix séances de 3 Gy, un patient ; 13 de 3 Gy, un patient ; six de 8 Gy, un patient ; sept de 7,5 Gy, un patient ; trois de 18 Gy, un patient ; 12 de 4,5 Gy, un patient ; cinq de 11 Gy, un patient ; 30 de 2 Gy, quatre patients ; 33 de 2 Gy, deux patients ; cinq de 12 Gy, un patient ; 12 de 5 Gy, un patient). Trois patients étaient traités dans un contexte de re-irradiation. Tous ont bénéficié d’une un patient pour évaluer la perfusion pulmonaire avant et trois mois après la radiothérapie, qui a été fusionnée avec la scanographie de planification. Pour les patient traités avec un schéma hypofractionné, l’équivalent biologique de dose à 2 Gy par fraction (EQD2) a été calculé (alpha/bêta = 10 Gy pour la toxicité aiguë). Les isodoses (5, 10, 20, 30, 40, 50, 60, 70, 80 et 90 Gy) ont été dessinées, puis, nous avons calculé l’activité (en MBq) dans ces volumes. Résultats Une régression linéaire a montré une réduction significative de la fonction pulmonaire à trois mois, proportionnelle à l’augmentation de dose délivrée ( p = 0,0017), avec une réduction de 0,14 % de la perfusion pulmonaire pour chaque Gy délivré. L’équation linéaire trouvée a une valeur prédictive de la perte de fonction pulmonaire en fonction de la dose délivrée de 98 % (R 2 = 0,9807). Conclusion La scintigraphie pulmonaire est un bon outil pour l’évaluation de la variation de perfusion pulmonaire après une irradiation thoracique. Alveolar echinococcosis is a rare but severe parasitic disease and is now in Europe the parasitic infection associated with the most morbidity and mortality. Its prevalence is increasing in Switzerland in both urban and rural areas. Echinococcosis is a differential diagnosis that should be considered when facing a cystic hepatic lesion. Moreover, this parasitic infection is increasing amongst immunocompromised patients, making the diagnosis more complex, because of atypic lesions and a more rapid evolution. By integrating specialist nurses, it follows international recommendations. These specialists' nurses rephrase the information given by the doctor and ensure patients' understanding. They assess the patient's psychosocial situation and provides guidance if necessary. They support the patient in making informed choices about treatment and coping strategies. In addition to the outpatient clinics planned in accordance with the care pathway, she can be contacted between appointments to answer questions or concerns of any kind. Intraarterial procedures such as chemoembolization and radioembolization aim for the palliative treatment of advanced hepatocellular carcinoma (stage BCLC B and C with tumoral portal thrombosis). The combination of hepatic intraarterial chemotherapy and systemic chemotherapy can increase the probability of curing colorectal cancer with hepatic metastases not immediately accessible to surgical treatment or percutaneous ablation.

Although generally considered as rare, incidence of gastrointestinal neuroendocrine tumors (GI-NETs) is increasing. The general practitioner has thus to be familiar with the vast array of clinical presentations and the growing family of diagnostic tools that can be used. Symptoms can be related to their hormonal production, their local extent or a bleeding complication. The prognosis depends on the grade of tumor, its local extent at diagnosis and its localization. The diagnosis relies on radiologic, endoscopic and nuclear medicine strategies. In case of typical symptoms, a hormonal secretion should be sought. Treatment options are extensive and should be discussed in an interdisciplinary manner.

The implementation of new techniques of imaging in the daily practice of the radiation oncologist is a major advance in these last 10 years. This allows optimizing the therapeutic intervals and locoregional control of the disease while limiting side effects. Among them, positron emission tomography (PET) offers an opportunity to the clinician to obtain data relative to the tumoral biological mechanisms, while benefiting from the morphological images of the computed tomography (CT) scan. Recently hybrid PET/CT has been developed and numerous studies aimed at optimizing its use in the planning, the evaluation of the treatment response and the prognostic value. The choice of the radiotracer (according to the type of cancer and to the studied biological mechanism) and the various methods of tumoral delineation, require a regular update to optimize the practices. We propose throughout this article, an exhaustive review of the published researches (and in process of publication) until December 2011, as user guide of PET/CT in all the aspects of the modern radiotherapy (from the diagnosis to the follow-up): biopsy guiding, optimization of treatment planning and dosimetry, evaluation of tumor response and prognostic value, follow-up and early detection of recurrence versus tumoral necrosis. In a didactic purpose, each of these aspects is approached by primary tumoral location, and illustrated with representative iconographic examples. Positron emission tomography is a functional imaging technique that allows the detection of the regional metabolic rate, and is often coupled with other morphological imaging technique such as computed tomography. The rationale for its use is based on the clearly demonstrated fact that functional changes in tumor processes happen before morphological changes. Its introduction to the clinical practice added a new dimension in conventional imaging techniques. Sarcoidosis is a multi-systemic inflammatory disease of unknown etiology, histologically characterized by the presence of non caseating granulomas. The diagnostic suspicion relies on clinical, epidemiological, biological and radiological elements. It is confirmed by an evocative histology and by the exclusion of other granulomatous pathologies. The aim of this article is to expose some clinical manifestations of extrapulmonary sarcoidosis particularly the cardiac and abdominal involvements. A register was made on cases of sarcoidosis diagnosed in CHUV from 2000 to 2009. It demonstrates the rarity of the disease in the region of Lausanne and confirms the existence of purely extra-thoracic affections.

Diagnostic and treatment management of prostate cancer at its initial stage continues to raise important debates within the involved medical community. To establish a protocol for active surveillance, a validated option in specific conditions of localised prostate cancer management for eight years, is a unique opportunity to gather different specialists in this field. This paper presents this concept.

The non-invasive evaluation of myocardial ischemia is a priority in cardiology. The preferred initial non-invasive test is exercise ECG, because of its high accessibility and its low cost. Stress radionuclide myocardial perfusion imaging or stress echocardiography are now routinely performed, and new non-invasive techniques such as perfusion-MRI, dobutamine stress-MRI or 82rubidium perfusion PET have recently gained acceptance in clinical practice. In the same time, an increasing attention has been accorded to the concept of myocardial viability in the decisional processes in case of ischemic heart failure. In this indication, MRI with late enhancement after intravenous injection of gadolinium and 18F-FDG PET showed an excellent diagnostic accuracy. This article will present these new imaging modalities and their accepted indications.

Echocardiography is the preferred initial test to assess cardiac morphology and ventricular function. Cardiac MRI enables an optimal visualisation of heart muscle without contrast injection, and precise measurement of the ventricular volumes and systolic function. It is therefore an ideal test for patients with poor echocardiographic windows or for the specific evaluation of right heart chambers. Heart CT also remarkably images heart muscle and precisely measures ventricular systolic function after intravenous injection of iodinated contrast. Coronary CT may also, in selected cases, avoid the need for diagnostic coronary angiography. Although very accurate, these imaging modalities are expensive and may be contra-indicated for a particular patient. Their use in clinical practice has to follow the accepted guidelines.

Metabolic syndrome represents a grouping of risk factors closely linked to cardiovascular diseases and diabetes. At first, nuclear medicine has no direct application in cardiology at the level of primary prevention, but positron emission tomography is a non invasive imaging technique that can assess myocardial perfusion as well as the endothelium-dependent coronary vasomotion--a surrogate marker of cardiovascular event rate--thus finding an application in studying coronary physiopathology. As the prevalence of the metabolic syndrome is still unknown in Switzerland, we will estimate it from data available in the frame of a health promotion program. Based on the deleterious effect on the endothelium already observed with two components, we will estimate the number of persons at risk in Switzerland.

Giant cell arteritis (GCA) and Takayasu arteritis (TA) are both chronic granulomatous vasculitis of unknown aetiology that involve large vessels. Signs and symptoms are related to the affected vessels: the primary branches of the aorta in TA and the extra-cranial branches of the external carotid artery in GCA. TA affects women under 40 of all ethnic origins with a greater prevalence in Asia, whereas GCA affects elderly of caucasian ancestry. In both diseases the diagnosis and follow up are rendered difficult by the absence of specific marker but advances in imaging technologies such as angio-MRI and PET/CT are very promising. Corticosteroids still represent the basis of treatment, most often alone in GCA and usually in association with another immunosuppressive drug in TA.

The discovery of hematuria in a child should lead the pediatrician to a methodical evaluation of the patient based on an extensive history and comprehensive physical examination. The microscopic examination of the urine is the cornerstone in the evaluation process and may suggest the origin of the hematuria. For instance, red blood cell casts indicate glomerular lesions and rule out the need for further urological examinations. According to the type of hematuria and the associated symptoms, a complete and immediate evaluation is sometimes necessary. This article presents a decisional tree to help the pediatrician to investigate an hematuria and to refer the child to a specialist, when needed.
