Cette étude a sondé 46 médecins de famille québécois quant à leurs pratiques pour l'évaluation et le dépistage des conducteurs à risque afin 1) de mieux comprendre leur niveau de compétence perçu; 2) de recenser les difficultés rencontrées dans le processus de prise de décision et 3) de documenter leurs besoins et attitudes quant à une collaboration plus étroite avec les ergothérapeutes. Les participants (femmes : 84,8 %; moyenne d'expérience : 15,7 (±12,1) ans) ont répondu à un sondage en ligne de 30 questions. Les résultats de cette étude démontrent que malgré un certain confort à effectuer l'évaluation et le dépistage des conducteurs à risque, les médecins ne se considèrent pas comme les professionnels les mieux qualifiés pour ce faire. Ils reconnaissent également le rôle que jouent les ergothérapeutes dans le dépistage de cette clientèle et l'intervention auprès d'elle. Ils voient ainsi la pertinence d'avoir accès aux services de ces professionnels en soins de première ligne.

Résumé: Objectif. Une démarche de coopération Ville/Hôpital a été initiée pour améliorer la prise en charge des patients en sortie d'hospitalisation et les relations interprofessionnelles. L'objectif est de créer une dynamique et des échanges entre les pharmaciens officinaux et les acteurs hospitaliers pour améliorer la continuité de la prise en charge du patient à sa sortie et proposer des actions correctives et préventives. Méthode. Les prescriptions de sortie des patients hospitalisés sont analysées en recueillant des événements indésirables selon deux méthodes. Les dysfonctionnements rencontrés par la Pharmacie à Usage Intérieur sont collectés via le logiciel de gestion des risques de l'établissement et ceux rencontrés par les officines via les fiches progrès officinales. Résultats. Un Comité de Retour d'Expérience (CREX) entre la Pharmacie à Usage Intérieur (PUI) du Centre Hospitalier de l'Agglomération Montargoise (CHAM) et les officines de l'agglomération a été créé en 2016, avec l'appui du service qualité et gestion des risques de l'établissement. Deux CREX sont organisés par an pour analyser les événements indésirables rencontrés par la PUI et les officines. Depuis 2018, 258 événements indésirables ont été analysés. Les principales difficultés rencontrées par les officinaux sont des erreurs ou des absences de durée (21 %), de dosage (20 %) et de posologie (12 %) sur les ordonnances. Des actions de communication et de sensibilisation ont été mises en œuvre auprès des médecins hospitaliers pour arrêter les prescriptions manuscrites et pour être vigilants sur leurs ordonnances de sortie. Une communication a été engagée entre la PUI et les officines pour les informer lorsqu'un médicament est nouvellement disponible en ville ou d'une rupture de médicament. Conclusion. Ce projet s'inscrit dans une démarche globale d'amélioration des relations ville/hôpital. Objective. A seamless care approach has been initiated to improve Patients' care after hospital discharge and interprofessional relations. The aim was to create and favor exchanges between pharmacists and hospital's stakeholders, to improve the continuity of the patients' care after hospital discharge and to propose corrective and preventive actions. Methods. The patient discharge prescription and adverse events met by hospital and private pharmacies were analyzed. The collection of adverse events was carried out using two methods. The difficulties encountered by the Hospital Pharmacy arewere collected via the institution's risk management software and those encountered by the private pharmacies via the officinal progress document. Results. Experience Feedback Committee (known as Comité de Retour d'Expérience or CREX in French) between Hospital Pharmacy and Montargis community pharmacies was created in 2016, with the support of the quality and risk management department of the hospital. Two CREX a year are organized to analyze adverse events. Since 2018, 258 adverse events have been analyzed. The main difficulties are errors or absences of duration (21%) and dosage (32%). Communication and awareness-raising actions have been implemented with hospital's doctors to stop handwritten prescriptions and to be vigilant about their discharge orders. A communication was initiated between the hospital pharmacy and community pharmacies to inform them about drug shortage. Conclusion. 30 p. ill., Quand leur grossesse se déroule sans complications, les femmes enceintes peuvent choisir d’être suivies uniquement par un gynécologue, ou par un professionnel de santé de première ligne (sage-femme ou médecin généraliste) et un gynécologue. Dans ce dernier cas, il est important de fixer des règles pour identifier les situations où l'intervention du gynécologue est nécessaire pour garantir la sécurité de la mère et de l’enfant. Le Centre fédéral d’Expertise des Soins de Santé (KCE) a élaboré, en concertation avec les professionnels concernés, un document pour soutenir l'élaboration de telles « règles de référencement » pour 188 situations cliniques ou psychosociales considérées comme potentiellement à risque. Toutefois, une condition préalable à l’utilisation de telles règles est que gynécologues et sages-femmes puissent établir des liens solides de collaboration. Taking care of patients in complex situation is a challenge as well in outpatient clinics as in hospital settings. Patient complexity varies over time and according to the place of treatment, requiring multiple actors, and usual guidelines are rarely applicable. As multiple organ systems are involved and the need for a "tailored" approach, these patients are often hospitalized in general internal medicine wards. At the CHUV, the internal medicine intermediate care unit is regularly confronted to complex patient situations, for which the health system is not always prepared, thus generating frustrations. The development of improved interprofessional collaboration, through regular exchanges and weekly meetings, has improved care plans and team satisfaction.La prise en charge des patients en situation complexe est un défi tant en ambulatoire qu’à l’hôpital. La complexité d’un patient varie dans le temps et selon le lieu de prise en charge, nécessitant des intervenants multiples, avec des guidelines rarement applicables. De par leur atteinte multisystémique et le besoin d’une approche « sur-mesure », ces patients sont souvent hospitalisés dans des services de médecine interne. L’Unité des soins intermédiaires de médecine du CHUV est régulièrement confrontée à des situations complexes, face auxquelles le système de santé n’est pas toujours armé, générant des frustrations. Le développement du travail en réseau interprofessionnel au sein de l’équipe, à travers des points de discussion réguliers et un colloque hebdomadaire, a permis d’améliorer ces prises en charge et la satisfaction des équipes.

In hospitals, many different professions work together with the same goal: optimal patient care. This challenge requires effective coordination and communication as well as good knowledge of each other's work. Due to an ever-increasing administrative workload and a large patient flow, time dedicated to this interprofessional collaboration is dwindling. Some hospitals, particularly in North America, have been using lean management concepts for some years, as an organizational aid and aiming at continuous improvement. A cockpit is a tool that aims at dedicating a space and a period of time to interprofessional communication and collaboration.Au sein d’un hôpital, de nombreux corps de métiers travaillent ensemble dans un même but : la prise en charge optimale des patients. Ce défi nécessite une coordination et une communication performantes ainsi qu’une bonne connaissance du travail de chacun. En raison d’un travail administratif en constante augmentation et d’un flux de patients important, le temps dédié à cette collaboration interprofessionnelle s’amenuise. Certains hôpitaux, notamment en Amérique du Nord, utilisent depuis quelques années des concepts issus du « lean management », comme aide organisationnelle et dans une perspective d’amélioration continue. Un « cockpit » ou « salle de pilotage » est un outil qui a pour but de consacrer un espace et une période de temps à la communication et à la collaboration interprofessionnelle.

The World Health Organization (WHO) considers medication safety as one of the main areas for improvement in patient safety. Treatment adherence, as a dynamic process that evolves through time or life circumstances, is of paramount importance, since it depends on a number of factors that health care professionals need to approach comprehensively during patient follow-up. In this article, we describe the tools available to general practitioners or specialist physicians, in collaboration with pharmacists and home care nurses, to optimize medication adherence in an ambulatory setting. An interprofessional approach between providers allows adequate support to patients by empowerment, treatment adjustments and optimization, based on shared common goals.La problématique de la sécurité des médicaments est l’une des thématiques prioritaires pour l’OMS en matière d’amélioration de la sécurité des patients. L’adhésion au traitement est un processus dynamique qui est au centre de cette problématique, en raison de nombreux éléments qui doivent être pris en compte par les professionnels de la santé durant le suivi d’un patient. Cet article décrit des outils qui s’offrent au médecin généraliste ou spécialiste, en collaboration avec le pharmacien et l’infirmier exerçant au domicile du patient, pour optimiser l’adhésion médicamenteuse. Une approche interprofessionnelle entre prestataires de soins permet un soutien adéquat aux patients via l’empowerment, les adaptations du traitement et l’optimisation, sur la base des objectifs communs partagés.

Introduction: The goal of this study was to meet a small, remote emergency department's need to reflect on the minimum threshold of services to offer. The study's main objectives were to 1) provide a statistical profile of the emergency services in Fermont, Quebec, 2) assess the staff's and users' perception of the threshold of services offered and 3) propose solutions for improving care and services. Methods: This case study was conducted with a participatory approach and a mixed methodology. We compared the results from a questionnaire on the emergency services that was validated during a previous study with the results concerning the other rural emergency services in Quebec as well as with national and provincial recommendations. The questionnaire concerned users' sociodemographic characteristics, the hospital's and the emergency services' descriptors, the services available locally, and the physician and nurse staff. Interviews were also carried out with 33 people (health care professionals, policy-makers and citizens). Results: Fermont's emergency department is smaller than the average rural emergency department in Quebec. They have resources that are in some respects comparable to those of other emergency departments and in line with the recommendations; in other respects, their resources are rather limited. Respondents emphasized how important it is to take into account the environment's specific features when establishing the minimum threshold of services. The proposed solutions would promote collaboration, break down silos within professional practice and focus on training. Conclusion: Fermont's case aside, this exploratory case study highlights how important it is to adopt a pluralistic, participatory and local approach in order to support reflection on the minimum threshold of services in remote emergency departments and to improve their overall performance. 30. A cross-sectional qualitative comparison between the perceptions of physicians and pharmacists of their collaboration in hospital settings.

Objective : To identify and compare physician-pharmacist perceptions concerning mutual collaboration in hospital settings. Methods : Two similar questionnaires were emailed to all French speaking physicians and pharmacists working in Swiss hospitals where medical-directors/chief-pharmacists had approved the study. Questions were developed based on previous physician interviews. Results : 14/18 medical-directors and 16/16 chief-pharmacists gave permission for the study. 244 (11%) physicians and 115 (74%) pharmacists answered the surveys. 49% physicians estimated interacting together <5x/year and only 52% contact the pharmacy when they have drug-related questions. Pharmacists considered that there was more contact than this between themselves and physicians. 97% physicians highly value knowing their colleagues, but 39% have trouble identifying pharmacists. 85% physicians would like an increased pharmacist presence in wards. Pharmacists misperceive the importance physicians give to meeting and knowing them. 59% physicians and 87% pharmacists believe physicians need more information about pharmacists' activities in order to better collaborate. 91% physicians expressed an interest in collaborating with pharmacists and 79% agreed that their current human relations are good. 96% participants consider pharmacists' global competences to be good. Pharmacist participation during ward rounds is well perceived. Conclusion : Participation rates in the questionnaire give a snapshot of current physician-pharmacist collaboration; pharmacists being the more concerned. Results show an infrequent pharmacist-physician interaction. Physicians' knowledge about pharmacists' activities should be improved as understanding one another's professional roles leads to better collaborative relationships. Reasons why physicians do not contact the pharmacist when they have drug-related question should be sought, as poor communication and failure to seek information has been shown to have negative consequences for patients. Introduction: The healthcare law 2016-41 from 26 January 2016 prescribes, as the central element of the French healthcare system, the coordination of care. In the field of therapeutic patient education (TPE), the aim is to strengthen its continuity between the hospital and the city, by associating all complementary opportunities with the usual and majority place of education that the hospital represents. In order to explore an hypothesis of outsourcing of the hospital's ETP from the hospital to the city with the intention of developing a culture of decompartmentalization, it is necessary to ensure the quality of the relationship between the different parties concerned. Objectives: The objective of this study is to raise expectations and conditions with respect to such a hypothesis. Methods: A qualitative and inductive study, exploratory and propositional, was conducted from January to March 2017 on a sample of 26 therapeutic education actors in the city and hospital, including the patient. Results: If the city/hospital relationship is experienced as compartmentalized and competitive, the complementarity of these two systems is desired and structured in an organized way around the outsourcing of hospital expertise. Discussion: The results lead us to think that this outsourcing requires cooperation. Conclusion: Decompartmentalizing the hospital towards the city is possible and indispensable, especially in FTE. Nevertheless, it is necessary to define a framework for this collaboration between the city and the hospital, whose cooperative outsourcing could be a proposal. [ABSTRACT FROM AUTHOR]

Acute agitation is a frequent cause of emergency department's consultation. Teamwork is essential in emergency medicine, but in practice it can be polluted by communication difficulties, a lack of understanding of everyone's roles and responsibilities, and a discordant definition of operating methods and objectives. Today, there is a strong awareness of the need to train medical and healthcare teams in interprofessional collaborative practice to learn how to work as a team, reduce medical errors and improve patient safety. Simulation is a recognized and effective pedagogical modality for achieving these objectives. It is now permanently established in pre- and postgraduate medical-nursing training courses in emergency medicine.Le travail en équipe est indispensable en médecine d’urgence mais, dans la pratique, il peut être pollué par des difficultés de communication, une méconnaissance des rôles et responsabilités de chacun, et une définition discordante des modes de fonctionnement et des objectifs. Aujourd’hui, il y a une forte prise de conscience de la nécessité de former les équipes médicosoignantes à la pratique collaborative interprofessionnelle pour apprendre à travailler en équipe, réduire les erreurs médicales et améliorer la sécurité des patient-e-s. La simulation est une modalité pédagogique reconnue et efficace pour atteindre ces objectifs. Elle est désormais implantée de façon pérenne dans les cursus de formation médico-soignante pré et postgraduée en médecine d’urgence.

The Mermoz multi-professional health center is a healthcare practice, including 24 professionals, on the outskirts of a major French metropolis. This article presents the challenges faced from the idea to the implementation of an interprofessional primary care offer. The difficulty of freeing up time for the construction of the common real estate and sanitary project has been overcome thanks to powerful motivations: the desire for more interprofessionality in practices that are sometimes fragmented, the need for support in the management of complex patients, the pleasure of being a group, and the improvement of healthcare. Objectives: The objective of this work is to assess the impact of a simulation session on the ability of pharmacy and medicine students in general practice to communicate in the resolution of patient-facing situations., Methods: The evaluation of the impact of the session on the representation of the professions used a questionnaire to be completed before and after the session by the students. The evaluation of the impact of the session on the perception of communication and associated skills was based on an audio recording of the debriefings, which, after transcription and thematic analysis, was used as a preliminary analysis for the drafting of a questionnaire proposed the following year. This questionnaire focused on the issues of interprofessional communication and on the seminar process., Results: During the 2018 and 2019 seminars, 518 students attended, 39% were pharmacy students (n=201) and 61% were medical students (n=317). The majority of medical students initially responded that physician-pharmacist communication was confraternal and rare. More pharmacy students felt that the quality of the physician-pharmacist relationship was poor. However, there was a marked improvement for all students on this aspect of communication after the seminar. For several years, the profession of radiographer has been unattractive and is in search of professional recognition. Increasingly complex therapeutic and diagnostic evolutions forces professionals to develop their skills to ensure quality and safe care for all patients. The primary role of the radiographer is to support patients and to accompany them during their examination or treatment, combining caregiver and technician's roles. Transversal missions and delegation of tasks are inherent to the profession but are not widely recognized. Cooperation between radiotherapy professionals is a response to offer the therapeutic radiographer/radiation therapist (RTT) opportunities in terms of attractiveness, career prospects, and increased skills. In radiotherapy, advanced practice activities already exist in some departments but require regulatory adjustments, in particular regarding the redistribution of the roles of RTT but also the status of these professionals. The formalization of these practices can be largely inspired by the many feedbacks around the world. This article aims to reflect the evolution's perspectives in the career of an RTT and on the valorisation of this profession in the current context.

Introduction Public Service Interpreting (PSI) is gradually becoming part of Quebec society. The institutional and organizational mechanisms designed to provide a framework for this interpretative practice are not yet generalized to all public services in the province, and where they are, many challenges remain. Objective The objective of the study was to provide stakeholders working in the mental health sector with practical strategies to foster collaboration with public service interpreters. Methodology A critical reading of the literature (narrative review) was carried out with the objective of offering a new perspective on the already existing object of study. Because it offers a critical synthesis of key information in the field, this method is particularly well suited to the needs of the reader who is not up to date with advances in the field. Results This critical reading first made it possible to identify two general challenges concerning mental health practitioners called upon to collaborate with public service interpreters: the recognition of the interpreter's role and the acknowledgement of the diversity of communicative situations likely to foster their collaboration. This paper presents seven interprofessional collaboration strategies that mental health practitioners can apply to meet these challenges: 1) using vocabulary drawn from the public service interpreter Positioning and Postures Typologies; 2) obtaining information to be transmitted in the interpreting assignment; 3) allocating time for briefings and 4) for debriefings; 5) encouraging the interpreter's presentation; 6) framing small and big talk; and 7) promoting the continuity of care. Discussion The present study offers a new perspective on some of the distinctive tensions in the PSI domain. The 7 proposed collaboration strategies are a response to these and aim to improve the quality of intercultural communication and services offered to users by promoting knowledge transfer. They are specifically addressed to knowledge users, take their practices into account and are explained using non-specific terms.

Initiated in 2009 by the Hospital, Patients, Health and Territories Act, the system of cooperation protocols between healthcare professionals aims, for patients, to reduce delays in accessing certain specific types of care. Paramedical staff, particularly nurses, trained to carry out certain activities in this context, hitherto exclusively performed by doctors, are developing increased skills and original career development opportunities. Three nurses from the Verdun/Saint-Mihiel hospital centre (55), involved for several months in a cooperation protocol in the field of corneal sampling, share their experience here.

27 p. ill., La convention Protocole 3 a été lancée en 2010 pour soutenir des projets innovants de soins à domicile pour des personnes âgées fragiles nécessitant des soins complexes. Elle a subi quelques remaniements au fil du temps, mais est restée à l’état de projet pilote. L’INAMI a demandé au Centre fédéral d’Expertise des Soins de santé (KCE) d’en faire une évaluation avant de décider de son éventuelle pérennisation. Après avoir interrogé tous les acteurs impliqués ainsi que des bénéficiaires, le KCE constate que, même si les résultats sont globalement satisfaisants, les remaniements de la convention mettent en péril sa philosophie première et risquent de démotiver les soignants qui s’y impliquent. Il recommande que les soins proposés dans ce cadre soient transformés en un modèle généralisé de soins intégrés et bénéficient d’un cadre structurel stable. Cancer is a disease with multidimensional consequences accompanied by new challenges in order to guarantee optimal care involving physical, psychological and social consequences of the disease and his treatments. The objective of this study is to identify the main challenges encountered by different stakeholders around cancer, including patients, in Brussels. This qualitative study was carried out with various stakeholders in the field of oncology. The semi-structured interviews and focus groups were transcribed directly and the data collected was analysed manually. Healthcare professionals and patients face many challenges in the current context of oncology care. Communication difficulties can affect the therapeutic relationship and the sharing of complex information. The increasing complexity of therapeutic strategies complicates interprofessional collaborations and care coordination. In a context of early return home and home hospitalization in particular, continuity in care and collaboration between the different lines of care are crucial. Patients face a variety of challenges, such as managing long-term side effects and post-cancer difficulties, including redefining their social and professional identities. Finally, cancer involves financial toxicity and social inequalities in health are particularly marked. Cancer is a complex disease which management involves a growing number of caretakers. It is also frequently synonymous with disruption in the personal, social and professional life of patients. The emerging themes of this study are closely interconnected and communication seems to occupy a central place. A holistic approach to the individual and inter-professional interactions could guide future prospects for the overall management of this constantly changing field.

Allogeneic hematopoietic cell transplantation (allo-HCT), the only curative therapy for numerous hematological malignancies, carries a significant risk of morbidity and mortality. The patients and families' expectations regarding the procedure, the prognosis uncertainties, as well as the existence of potential new therapeutic possibilities, lead to frequent use of intensive care. Even though the transplant physicians are highly skilled in acute care, their knowledge of palliative approach is limited, making the use of palliative care insufficient and often late. By promoting reflection on the proportionality of care and the patients' quality of life, palliative care may contribute to the allo-HCT patients management. Nevertheless, obstacles to this approach remain. The objective of this work is to propose recommendations to promote the implementation of palliative care into transplant units.

Objectives Bipolar disorder is a chronic condition which significantly impacts the functioning and quality of life of patients with the disorder. Recognized efficacious psychological interventions, such as group psychoeducation, can help better address some of the limitations observed when pharmacotherapy is used alone in the management of bipolar disorder. However, access to these evidence-based interventions seems to be limited for most patients. Indeed, the translation of knowledge acquired through research towards actual clinical settings poses a significant challenge. Assessing the implementation of these efficacious interventions in clinical practice is thus a priority. The objective of this study is to describe the implementation of a psychoeducative intervention, the Life Goals Program (LGP), for the treatment of bipolar disorder in community mental health settings in Quebec City. Methods The LGP was implemented in three different clinical settings located in Quebec City. Fifteen healthcare service providers chosen by their respective clinical site were trained to deliver the intervention. They delivered the treatment to 73 patients with a diagnosis of bipolar disorder. Healthcare service providers filled a log book after each group session, in order to assess whether they had properly delivered the content of the program. At the end of the study, they also participated in a group interview in order to get a better understanding of their experience delivering the intervention and their appreciation of the research process. Results Following qualitative content analysis, four main categories of factors that could influence the implementation of the intervention were identified: 1) healthcare service providers' characteristics (academic training, clinical experience, personality, knowledge of the program, and dynamic between animators); 2) participants' characteristics; 3) organizational context (physical and material environment, staff stability, administrative management and research requirements); and 4) facilitation (perception of research, research team support, and facilitation tools). These categories derived from data analysis coincide with those observed in the literature. The following factors seemed to have had the most impact in the differences observed between sites in the implementation of the LGP: the support offered by the research team; staff stability; and the academic training of healthcare services providers. Conclusion Dissemination and implantation studies can not only help determine factors that are important to consider when implementing a program, but can also help improve and adapt these programs in order to increase acceptability and effectiveness in real world clinical settings.

47. Teaching family medicine residents how to answer clinical questions using QUIPs

Background: “Questions in Practice” (QUIP) rounds are used to encourage residents to quickly find, evaluate, and incorporate information into clinical practice. It is an opportunity for residents to identify a clinical question, research the answer, present the evidence, and discuss how to apply it to practice. The value of using this method to teach residents has not been evaluated. Methods: A sampling of all first and second-year family medicine residents enrolled in the Memorial University Family Medicine program were invited to participate in the survey. The survey gathered information about the residents’ current experiences with answering clinical questions, their experience during QUIP rounds, and the value of an interdisciplinary approach. Results: The response rate was 91% (42/46). Medical websites (45%) and journal article indexes (34%) were most often used. Through QUIPs, 50% of the students identified new methods to retrieve answers, 80% considered it a useful learning experience, 75% had improved confidence, and clinical knowledge improved in 97%. Conclusions: Residents are familiar with many general sources of medical information, and QUIPs helped improve confidence in their knowledge and ability to answer questions. QUIPs appear to be a useful tool for teaching information resources and how to interpret and apply evidence to clinical situations.
