Green Architecture (GA) began to appear -as an expression - in the seventies of the last century. Recently, Arab world begun to interest in GA. GA evaluation systems have emerged as a basic requirement to determine the quality of these buildings and their commitment to the principles of green architecture. Although there are many GA evaluation systems in many countries, Yemen delayed in applying the principles of GA, and its modern buildings moved away from applying the internationally recognized GA principles (GAP), although some of these principles appeared throughout history in Yemeni traditional architecture.Applying the principles of GA always faces different circumstances that may affect it positively or negatively, and they differ from one society to another. In Yemen, the applicability of all the principles of GA in buildings has not been tested. Thus, this paper seeks answer a central question that asks about the applicability of GAP in Yemen. The research aims to find an answer to the central question by examining the applicability of GAP in the Yemeni built environment. A questionnaire is conducted for architects and specialists in Yemeni Architecture to examine the applicability of GAP in Yemen in light of its circumstances. The study ends with results of the questionnaire, analysis and discussion of the applicability of green architecture principles in Yemen and the degree of their importance in terms of application. The research concluded that there is a set of principles that are applicable in Yemen which are: Respect for site; Adapting with climate; Energy efficiency; Material and resource efficiency; Water efficiency; and Indoor environment quality, with a set other GAP that specialists demonstrate that they are not applicable in Yemen. The study also recommend to apply the approved GAP in Yemen in relevant contexts.

Objectif : évaluer l’impact de la lombalgie sur les activités de la vie quotidienne et sur la sexualité des patients. Matériels et méthodes : Etude cas-témoins allant du 04 décembre 2021 au 15 mai 2022 au service de rhumatologie du CHU Ignace Deen (Conakry). Les patients souffrant de lombalgie ont été inclus. Les témoins appariés selon l’âge et le sexe souffraient d’affections rhumatologiques sans lombalgie. Nous n’avons pas inclus les patients ayant, ou ceux dont le/la partenaire avait une pathologie pouvant affecter les activités de la vie quotidienne et la sexualité. La qualité de vie a été évaluée par Owestry disability index. La sexualité par Femal sexual function index et International index of erectil function. Résultats : Nous avons colligé 638 patients (318 cas et 320 témoins). L’âge moyen des patients était de 54,8 ±13,1 ans avec prédominance féminine (61,3%). Dans les activités de la vie quotidienne, une incapacité sévère était retrouvée chez 133 (41,8%) patients lombalgiques. Les domaines les plus fréquemment affectés étaient le soulèvement d’objets lourd et la position debout prolongées. Les patients lombalgiques avaient 6 fois plus de risque de dysfonction sexuelle (p=0,0006). Elles étaient plus affectées que les témoins pour l’excitation sexuelle, la satisfaction sexuelle et la douleur. Chez les hommes lombalgiques, 61,8 % avaient une dysfonction sexuelle affectant surtout l’érection (IIEF=1,7 ± 0,9). Conclusion : La lombalgie affecte négativement les activités de la vie quotidienne et la sexualité des patients.

Indoor thermal and visual comfort are core issues in designing sustainable buildings. Natural daylight and ventilation are proved to improve the occupants’ performance and productivity levels. The buildings sector in Egypt and specifically the residential sector is responsible for 42 % of the total energy consumption. Utilizing natural daylight in buildings plays a great role in reducing the energy used for artificial lighting and especially in a country like Egypt with high solar exposure rates.This research paper proposes an evaluation of solar tubes configurations in a space located in Cairo, Egypt (semi desert climatic zone). This evaluation is conducted using a simulation study that has been held to determine the optimum size of the solar tunnel as well as the optimum placement of them, and to optimize their performance in the case study investigated space. Twenty-four configurations of solar tunnels have been studied and twelve combinations of skylight have undergone evaluation. The analytical study provides the optimum size of the light tubes that is able to provide the highest level of daylight factor in the case study space.The selected space is located in Cairo, Egypt in a building. The coordinates are 30.0444° N, 31.2357° E and 23m elevation above sea level. The size of the room is 4.20 m length, 3.60 m width and 3.00 m height, oriented to the north, the main feature of the room is that it has no windows in order to accurately analyze the effect of the light tubes.The building envelope of the selected case study is as follows, the wall finishing is plaster and flooring tiles both have the following characteristics 0.90% reflectance, 0.03% rugiosity and 0.0% specularity. The reference plane where the light properties have been calculated is a wooden table centered in the room, and has the following characteristics, 0.66% reflectance, 0.02% rugosity, 0.05% specularity

AbstractThe world is constantly moving towards environmental-friendliness, and there are plenty of modern concepts that attempt to link architecture and the environment, in previous as well as current times. This is done in order to connect a heritage that’s full of spirit, with a modernity that’s heavily influenced by material inclinations. This is to be accomplished through the use of engineering as well as technical capabilities that prepare and adapt the environment to make it suitable for living and that allow it to maintain its quality. Furthermore, “sustainability” has become part of the mainstream discourse. It refers to “developmental strategies that take into account the requirements and aspirations of current and future generations at once, and is simultaneously aimed at providing a balance of interests that serves everyone in the fields of economy, society and environment.” This is in addition to the emergence of the ecological infrastructure concept, which is concerned with the natural and material environmental surroundings. This concept also recommends the use of environment-friendly material, calling towards the preservation of the natural resource base. Sustainable development has adopted a set of strategies, including the following: optimally orienting the building, exploiting natural energies and taking into account planning systems, building details… etc.” In addition to that, it has adopting strategies to improve the quality of life for the tenants in matters of public health. Sustainable design is also regarded as an essential step to achieve economic growth while protecting natural resources and raw material, in order to achieve sustainable design, and principles of sustainability. .In Egypt, we find that people of the early Egyptian civilizations used local materials such as mud bricks, papyrus and wood in their private architectural systems (such as the workers’ quarters); whereas they used natural stones and mountain carvings for their sacred architectural systems (such as temples). For this reason, those buildings have maintained their interior design and furniture up to this day.This necessitated the study of some modern design models that were designed with a sustainable mind-set, and that used modern technological methods to benefit from the concepts and principles of sustainability to the largest extent possible, with the end goal being the creation of a design that respects natural factors and creates a harmony with them.

La mise en œuvre de l’arsenal normatif et institutionnel qui encadre la gouvernance des données de santé découle de la volonté de l’Etat du Mali à protéger la vie privée des individus contre les dérives inhérentes à la collecte et au traitement de leurs données de santé. Toutefois, en dépit de cette volonté manifeste, la problématique de la sécurisation des données de santé demeure en partie non résolue. A travers cet article, il sera démontré que malgré son encadrement strict, le traitement des données de santé est confronté à d’énormes difficultés parmi lesquelles l’inadaptation du cadre législatif face à l’utilisation de nouveaux outils de collecte et de traitement ainsi que la diversification et la multiplication des finalités, d’où la nécessité de son adaptation à l’évolution des technologies de l’information et de la communication. Heat exchangers have an important role in everyday life. A heat exchanger is a device used as a means of energy transformation. The development of heat exchangers undergoes a form transformation that aims to increase efficiency through its work function. Double-pipe heat exchanger with a corrugated tube type is a simple and often applied method used in heat transfer. The use of a concave shape on the corrugated tube aims to increase the convection heat transfer area. The purpose of this research is to determine the effect of using concave shapes on corrugated tubes on the effectiveness of double-pipe heat exchangers. The object of this research is a double pipe heat exchanger with a concave corrugated tube and then varied with triangular, trapezoidal, and square concave shapes. Data collection of experimental results on double pipe heat exchanger using pressure gauge, flowmeter, and thermocontrol as research instruments. The research data were analyzed descriptively calculated theoretically and then converted into graphs and tables. The results achieved to date are the addition of concave shape variations on corrugate tubes can increase the value of thermal conductivity and other characteristics, in the form of a concave triangle with the effectiveness value increased by 11,63%.

The research deals with the use of some contemporary glass sculptors of reflective glass (the mirror) to form their sculptural compositions, taking advantage of the properties of light and its reflections on the sculptural glass figure, which worked on changing the sculptor's thought and creating a kind of interaction between scenes and the sculptural work. Outdoor Spaces are considered important elements in cities as they provide a space for activities which enhance the livability and vitality of the city (Liang Chen, Edward NG, 2012). Well- designed outdoor spaces encourage more people to use them which will be beneficial from different aspects; environmental, economic, social and physical aspects.Urban outdoor spaces are considered significant elements in university campuses; which makes the thermal comfort in these spaces very important to be achieved. Outdoor microclimate is an important issue affecting the quality of outdoor spaces. Outdoor spaces have very rich microclimatic characteristics that affect users’ thermal comfort and thermal comfort perception. Users’ needs to reach thermal comfort in outdoor spaces vary according to gender, ages and other factors. The paper studies users’ approach of the thermal environment in outdoor spaces in university campuses. The case study was chosen, ten different spots in the British University in Egypt where the measurements and survey were conducted. The purpose was to measure the level of users’ perception and preference of the surrounding thermal environment. The devices used for the measurements were Anemometer, humidity meter, thermometer, Globe thermometer and CM6B Pyranometer. The data from measurements and surveys were gathered in order to determine the neutral physiologically equivalent temperature (PET) which was determined by analyzing the relationship between thermal sensation votes (TSV) and PET. The findings showed that respondents in BUE campus were sensitive to the variations of the PET. It is due to, every 3.9 oC of PET, mean thermal sensations changed one unit in BUE campus, Cairo, Egypt.

This study discusses the importance of using multisensory architecture tools to reach the best performance in designing learning spaces for different levels of visually impaired children. It discusses the effect of designing by our five senses in achieving the purpose of our design. As it’s known that Architecture is one of the main basics of environmental components. This study mainly aims to shed light on the idea that the Architecture can’t be considered as a visual tool but it is a multisensory tool that could be used to feel and understand the environment. So, it was important to discuss the architecture tools that could be felt by our all senses. Besides discussing the idea, that in the case of losing the visual sense, the building should have the same ability to be understood by other senses. By missing these tools, the pupils with visual impairment disability cannot be able to use their right to live normally. The study analysis a successful case study of designing a classroom for the blind in Pattaya city in Thailand. This project has won a prize in the 2019 Inside World Festival of Interiors in Amsterdam. The analysis of this project appeared the architecture tools that used to make this classroom a multisensory space that could be felt by students 'different senses. Besides highlighting the basic senses that should be used to design a space for visually impaired users. The study concludes that by applying the Multisensory Design tools, visually impaired users could feel space by Touching, Hearing, smelling or even feel space by their thermoreceptors. A building that created with the multisensory can achieve great success as an architecture product or a human product.

The research highlights the Design Standards of the architectural Surfaces using New Cladding materials. The research aims to achieve the functional benefit besides achieving the aesthetic aspect through the deep study of raw materials and smart materials, the knowledge of the technology of each application, and how they interact with the environment. And to achieve the concept of sustainability through the optimal employment of these materials, and also the innovative criteria for the design of architectural surfaces and aesthetic values and technology for the use of raw materials in the design of architectural surfaces, The research assumes that enhancing the architectural understanding of the designer through his deep and complete understanding of all modern techniques and materials enables him to develop basic design criteria to achieve the best interaction of architectural surfaces and environmental space around them and achieve the concept of sustainability. There is many evidences of the impact of the design process in general and the thought of the design of architectural surfaces, especially that this impact was directly motivated by the designer to go to this technology to know its benefits, Or indirectly as a response to the impact of modern technologies on the development of life, society, interior design, and architecture in general, such as Thermobimetals, Alucobond, Coagulated Glass and Aeragel Glazing, etc. The research finds that the importance of the interaction between technology and design leads to the creativity in architecture, where the smart materials add aesthetic value of the building and give a distinctive texture of the outer shell, finally the research recommendation is designers should be continuously updated with all brand-new in design, materials, techniques and modern technology.

Language is one of the most important parts of cultures around the world. Even if today advanced technology has brought people closer than never before, connections between different people and their cultures have continued to grow as well. This extraordinary growth in communication between people who do not speak the same language requires the intervention of interpreters to help bridge the linguistic gap. Many cultures across the globe speak different languages which result in so many communication barriers. Community interpreting is a new field of endeavour in Ghana as over the years researchers have not really given it the attention it deserves. However, with the influx of migrants into Ghana in recent years, there is the need to give it a lot more attention. Beyond the technical or strictly professional issues, it seems interesting to us to address the human component involved in the interpreting process. For this purpose, community interpreting is the best field to explore the feelings and emotions that, in this case, burst in and influence the course of the communication. In this type of intercultural communication it is important to explore the language models, policies and practices in that, without these policies we can never bridge the linguistic gap in a multilingual country such as Ghana. This article seeks therefore to establish the current state of community interpreting in Ghana, with a particular emphasis on its historical context, professionalization and interpreter training.

Life in a limited-space dwelling is one of the most important problems facing residents due to the failure to meet their changing living needs. Which requires re-thinking in a functional way to provide the largest number of human needs without the need to change housing and move from one housing unit to another larger. Thus, it is possible to overcome the problem of owning large and expensive spaces in order to satisfy the wishes of the user and meet his changing needs. Hence, the responsibility of the interior designer comes towards finding appropriate design solutions to solve this problem in a way that is consistent with the user culture in light of modern technology for furniture design.Through the technological developments in the design of residential buildings using artificial intelligence technology (computer home, digital home, interactive home ... etc.). Consequently, the traditional concept of furniture units has changed from just inanimate objects that humans use to units that are able to sense, identify and interact with users to meet their needs and desires.It was therefore necessary to resort to the idea of intelligent furniture that integrates with everything that surrounds it and interacts with it in response to changing human needs within the void using intelligent systems that have helped to time and effort to perform the required tasks in a given time.It is worth mentioning that the change in the function of the furniture unit was not only a functional change but was followed by a change in shape to suit this advanced technology, the reductive trend became the dominant in furniture design, where the designs with geometric shapes became easy to configure and realize, far from Jostling decorative elements, the clean lines are suitable for the computer technology used in the furniture unit, as well as with the executing materials of wood, glass and metal that have made an unprecedented breakthrough in the age of technology.

The current study aims to identify the diversity of digital processing of Islamic motifs and some of the folk symbols, as a source of enriching the decorative painting through remote learning, via social sites as e-mail, Whatsapp, or Facebook using Photoshop as a technical processing program by which plastic solutions can be developed for students' work that had been produced in specific sizes and manifolds. This study has used the descriptive-analytical and quasi-experimental methods of research with the intention of producing artwork represented in the decorative paintings through remote learning. . The teaching treatment of synchronous and asynchronous through e learning across specific educational platforms at Minia University (http://courses.minia.edu.eg). As well as, communicating with the sample students through social media platforms such as, e-mail, Facebook, and WhatsApp. . Remote learning has proved its success in this technical experiment, and it has also proved its worth as a replacement for the suspension of on-campus learning due to COVID-19 that has put a halt in how the entire world functions. This study has also reached several results, and the most prominent is that most of our sample students have produced decorative paintings with creative and plastic solutions. The results also showed the importance and the effectiveness of remote learning on the practical teachings and applied courses. Finally, these findings were discussed and as a result, a set of recommendations have been suggested. The most prominent of which is, paying attention to the importance of the quality of the infrastructure and the training program that enhance and update our curricula constantly, promote and strengthen the idea of remote learning amongst higher education students in order to raise their training competence. Last but not least, is to take an interest in training university professors on such methods of education to contribute to the distinguished applications inside and outside the university campus.

It saves money and increases the profit margin. The cost of recycled materials is not the cost of raw materials extracted from nature, which is why employers sometimes rely on recycling, and lower costs for manufacturers increase the exports of countries abroad, Their revenues. Reduces the problems of accumulation of waste and the diseases it brings to different people, through the accumulation of pathogens above the waste, and also rid the environment of non-degradable waste such as plastic materials that may cause many problems for the different environments in which they exist

The human right to housing has been recognized since 1948, when the Universal Declaration of Human Rights was promulgated, by which housing was established as an essential component of the right to adequate standards of living, accompanied by basic human needs such as food, clothing, health care and social services .The dwelling always meets the primitive and spontaneous needs of the human being and was linked to the human being and its presence in the urban association with the psychological needs of social, economic, service and aesthetic . The individual's access to basic human needs in the dwelling is a major and important objective in improving the standard of living of individuals, and thus their lives, health and well-being and thus their productivity and contribution to the overall development of the state . Whereas the provision of adequate housing is necessary to achieve community stability and provide comfort and safety to its users . From this standpoint, many studies seek to develop the right design vision for the house in a way that seeks to improve the lifestyle and user's interest in providing the required needs commensurate with the available material possibilities .Taking into account these needs in the housing is a design necessity, which may vary from one housing to another depending on the nature of each community and each family, and neglect may negatively affect living in the housing and user behavior as well as physical and psychological health .Hence the problem of research, which is the omission of the role of humanitarian needs in the interior design of some housing. Therefore, this research aims to define standards and foundations that can be used in the interior design processes of the house and the design of furniture units.The research gets its importance from Shed light on the importance of meeting the humanitarian needs in the house through the role of interior designer.

Objectif : cet article vise à étudier les facteurs managériaux et économiques qui impactent la performance opérationnelle des banques de l’UEMOA. Méthode : l’approche méthodologique porte sur un panel dynamique avec la méthode des moments généralisés (GMM). Les données proviennent de la base de données de la BCEAO et portent sur un échantillon de 83 banques de l’UEMOA durant la période de 2012 à 2018, soit 581 observations. Résultats : notre étude montre que pour ce qui sont des facteurs managériaux, la taille et le poids du risque de crédit impactent négativement la performance opérationnelle des banques de l’UEMOA au moment où la surface financière et l’âge de la banque impactent positivement cette performance. Pour ce qui sont des facteurs économiques, on constate que le taux de croissance du PIB et le rendement de l’indice BRVMC impactent positivement le rendement des actifs de ces banques; alors que l’impact du taux d’inflation est négatif. Originalité / pertinence : ce travail tire son origanité du fait que nous avons analysé l’influence à la fois des facteurs manageriaux et macro-économiques sur la performance operationnelle des banques des huits pays de l’UEMOA avec un modèle panel dynamique. Ce travail pourrait être utile dans la prise de décision et la mise en place de stratégie pour les dirigeants des banques, les gestionnaires de portefeuilles, les investisseurs ainsi que la BCEAO.

In our study, we can confirm that teaching/learning oral French as a foreign language is not an easy task, but it is, however, important in the training of learners. The spoken word represents a daily language practice, but it is poorly considered in the educational process. Considered as elusive, ephemeral, it benefits of a poor representation in relation to the written language practice, to which it is constantly compared and for which it is the most often used only as support. For many language teachers, the spoken language remains a conflictual, unclear and poorly defined practice in the language class, being reduced to an uncertain practice. The purpose of this study is to highlight the importance of the spoken word and to show that the acquisition of oral communication skills can constitute, independently of the written word, an objective in its own during a foreign language class. It is also a question of proposing a methodology for oral teaching / learning and its evaluation that takes into account its variations and specificities.

Researchers have agreed upon the importance of social media in regard to establishing athlete brands. In this sense, Twitter is a very important example of social media which represents fans’ perspectives about their favorite (or least favorite) athletes and provides solid fan feedback, which can be helpful in the context of image branding for an athlete. The current study has employed a qualitative content analysis of Twitter users during Pakistan Super League (PSL). The tweets were collected during the 2017 edition of PSL. Data containing #HBLPSL and #PSL was collected and tweets with names of foreign athletes were selected from more than 38,000 tweets. Findings from this study suggested that fans’ tweets about athletes could be separated into two major categories: 1) fan related motives and 2) athlete’ related motives. Fan related motives included sharing: information, visuals, predictions and feedback regarding certain athletes. Athlete’ related motives included: athlete performance, style of celebration and athlete comments. Results of the current study have highlighted the motives that persuaded fans to tweet about a certain athlete during the PSL. Resumen: Los investigadores han estado de acuerdo en la importancia de las redes sociales en la búsqueda de establecer marcas de atletas. En este sentido, Twitter es un tipo muy importante de medios de comunicación social que representa las perspectivas de los aficionados sobre sus atletas favoritos (o no) y proporciona información sólida de los aficionados, que puede ser útil en el contexto de la imagen de marca para un atleta. El presente estudio utiliza un análisis cualitativo del contenido de los usuarios de Twitter durante el PSL. Las colecciones de tweets comenzaron el primer día del PSL 2017 (9 de febrero de 2017) y concluyeron el último día del estudio (5 de marzo de 2017). Se recopilaron datos que contenían #HBLPSL y #PSL y se seleccionaron tweets con nombres de atletas extranjeros de entre más de 38. 000 tweets. Los resultados de este estudio sugirieron que los tweets de los aficionados sobre los atletas, podrían separarse en dos categorías principales: 1) motivos relacionados con los aficionados, y 2) motivos relacionados con los atletas. Los motivos relacionados con los aficionados eran compartir información, imágenes, predicciones y comentarios sobre ciertos atletas. Los motivos relacionados con los atletas fueron el rendimiento de los atletas, el estilo de celebración de los atletas y los comentarios de los atletas. Los resultados del estudio actual han resaltado los motivos que persuadieron a los aficionados a utilizer twittear sobre un determinado atleta durante la PSL.

The ruling regime in this type of distribution gives individuals to a certain extent the right to exploit the wealth that has not been affected by productive activity, which earns them the right to enter into the process of productive activity to be entitled to a return of productive wealth. It is a rule that we need to know how far it extends in parallel with the distribution of land ownership. Land is the most important factor of production because of its natural ability to participate in various social activities and its ability to maintain various characteristics in different times. In this research we tried to reach the philosophy of Islamic jurisprudence regarding the distribution of land ownership, while trying to explore the multiple views of the various Islamic jurists.

L’étude a porté sur la caractérisation du peuplement ligneux de deux systèmes d’utilisation des terres (la forêt qui est une mise en défens et les champs) dans le terroir de Ngouye au Sénégal. Elle a été effectuée dans une zone agropastorale qui accueille annuellement plusieurs familles de transhumants. Au moyen d’un échantillonnage par relevés de la végétation ligneuse et d’enquêtes auprès des populations nous avons inventorié 28 espèces appartenant à 25 genres et 15 familles botaniques. Les Fabaceae (sous-familles : Mimosoideae, Caesalpinioideae, Faboideae) ont été les plus représentées, suivies des Combretaceae. Les paramètres de l’étude ont révélé qu’il existait des différences nettes entre les deux systèmes, ce qui montrait leur hétérogénéité, avec par ailleurs une grande stabilité de peuplement en forêt. Le peuplement était généralement très jeune, avec un grand pourcentage d’individus dans la première catégorie de diamètre et de hauteur. Les champs avaient la plus forte proportion d’individus adultes mais le taux de régénération dans cette zone était inférieur à celui de la forêt. La richesse spécifique des champs était bien supérieure à celle de la forêt. Sur le plan pastoral les ligneux intervenaient dans l’alimentation du bétail.

INTRODUCTION: La polyarthrite rhumatoïde (PR) altère de façon considérable la qualité de vie des patients. L´objectif de notre étude était d´étudier le lien entre l´activité de la maladie et la qualité de vie de patients guinéens et camerounais atteints de polyarthrite rhumatoïde. METHODES: étude pilote transversale multicentrique (Hôpital national Ignace Dean de Conakry en Guinée et Hôpital de district d´Efoulan Yaoundé au Cameroun) de 15 mois (1er octobre 2016 au 30 janvier 2018). Le diagnostic de PR était basé sur les critères de l´ACR/EULAR (American college of rheumatology, European League against Rheumatism). Le questionnaire EMIR (échelle de mesure de l´impact de la polyarthrite rhumatoïde) et le score de Steinbrocker ont été utilisés pour évaluer la qualité de vie. RESULTATS: cinquante-deux patients dont 82% de femmes ont été colligés. Le score EMIR total était de 5,06±0,50 considéré comme une qualité de vie relativement altérée. L´altération de la qualité de vie était plus marquée sur les composants psychiques (6,78±0,99) et douleur (5,37±0,99). Le composant travail était le moins affecté (4,03±0,98). Le DAS28 était significativement lié aux composants psychique (p=0,036 ; R=0,29), douleur (p=0,076 ; R=0,25), physique (p=0,0029 ; R=0,41 ) et à la qualité de vie globale (EMIR total) (p=0,027 ; R=0,31). CONCLUSION: l´impact le plus significatif de la PR sur la qualité de vie est corrélé à la douleur (EVA-douleur) et à l´activité de la maladie (DAS 28). Les résultats de cette étude pilote devront être confirmés par une étude d´une plus grande ampleur.

Yellow fever appeared in Senegal during the Atlantic slave trade. The epidemic episodes of this pathology were both a revealer of social processes and a driving force for the transformation of colonial society. From the point of view of the history of science and medical ideas, yellow fever sheds light on the social body in its various interactions and makes more visible the essential joints of the group, the lines of force and tensions that run through it. The present study focuses on a long-term analysis of the medical discourse and practices around yellow fever with regard to, among other things, the domestication of Western biomedical science outside its zone of elaboration, as well as the implementation of a particular form of medical practice as an expression of domination. The epidemics of 1878-1881, 1900 and 1927 were important moments for the evolution of ideas about disease, while at the same time they induced ruptures and transformations in social groups and geographical space (architecture). They show how scientific views and theories condition and determine health policies in the colonies. L´éviscération transanale de l´intestin grêle par empâlement est exceptionnelle chez l´enfant. Nous rapportons l´observation d´un adolescent de 11 ans reçu à 2 heures d´une issue des anses grêles à travers l´anus suite à une chute avec réception sur un morceau de bois pointu. L´examen clinique avait objectivé un bon état général, une issue, par l´anus, d´environ 25 cm d´intestin grêle viable et un abdomen sensible dans son ensemble. Le bilan biologique préopératoire était normal et aucune imagerie n´était réalisée. Après une réanimation, l´exploration chirurgicale avait mis en évidence un liquide séro-hématique (300 ml) et une issue d´environ 60 cm d´anse grêle inflammatoire à travers une brèche d´environ 5 cm de la paroi antérieure du rectum. Une réduction des anses grêles éviscérées par traction douce, une réparation de la paroi rectale par des points séparés, une toilette et un drainage ont été réalisés. Le patient était sous antibiotique à large spectre. Les suites opératoires étaient simples avec une reprise du transit à J2 post-opératoire. La sortie a été autorisée à J7 post opératoire. Après un recul d´un mois, la patiente a été revue en consultation et l´examen clinique était normal.

Les infections nosocomiales constituent un problème majeur de santé publique dans le monde. La co-infection VIH-Tuberculose pulmonaire augmente la fréquence des infections nosocomiales, du fait de l´immunodépression et des hospitalisations itératives. Nous présentons quatre (04) patients âgés de 28, 36, 42 et 52 ans, co-infectés de VIH et tuberculose, (Tuberculose multifocale chez 2, miliaire tuberculeuse, tuberculose bactériologiquement confirmée), tous ayant un taux CD4 100 cellules/mm3. Ils étaient à la phase intensive du traitement antituberculeux et sous antirétroviral (ARV). Ils avaient été admis au Service de Maladies Infectieuses du CHU du Point G pour toux productive, et/ou hyperthermie après des séjours antérieurs de plus de 48heures en milieux hospitaliers. L´anamnèse avait révélé que l´un n´avait pas été observant au traitement antituberculeux du fait des effets indésirables classés mineurs. Il n´y avait pas eu d´amélioration clinique chez les 3 autres malgré une observance optimale aux différents traitements. L´examen cytobactériologique des expectorations et/ou du liquide de tubage gastrique, les hémocultures, prélèvements spécifiques avaient permis d´isoler Klebsiella pneumoniae multirésistante. Sous antibiothérapie spécifique, l´évolution clinique de ces patients avait été favorable. Les infections nosocomiales peuvent faire penser à tort à une mauvaise réponse thérapeutique lors d´un traitement antituberculeux. L´examen cytobactériologique des liquides biologiques doit être systématique chez les patients co-infectés de VIH-tuberculose. Notamment chez ceux ayant eu des séjours hospitaliers d´au moins 48 heures, et chez qui les signes pulmonaires et/ou la fièvre persistent en dépit d´une bonne observance aux traitements.

CLes affections cardiaques dans leur ensemble sont la cause la plus fréquente de morbidité et de mortalité chez les patients insuffisants rénaux chroniques traités par dialyse. Les principales anomalies morphologiques des cardiomyopathies peuvent s'accompagner d'une altération de la fonction systolique ou diastolique avec comme conséquence clinique une insuffisance cardiaque, un trouble du rythme, voire la mort subite. L´objectif de cette étude était de déterminer la fréquence de la dysfonction diastolique chez les patients hémodialysés chroniques. Il s´agissait d´une étude transversale de type descriptif d´une durée de 4 mois allant du 1er Mai au 31 Août 2017. Cette étude a été réalisée au centre national d´hémodialyse de Donka (CNHD). Etaient inclus dans cette étude tout patient hémodialysé chronique ayant bénéficié d´une échographie Doppler cardiaque ayant mis en évidence une dysfonction diastolique du VG. La dysfonction diastolique du VG était retenue chez tous patient ayant un rapport E/A1, un rapport E/E´10 et une FEVG50% évaluée par la méthode Simpson biplan. Selon la classification de Redfield. Sur 87 patients ayant bénéficié de l´échographie Doppler cardiaque, 32 avaient une dysfonction diastolique soit 36,78%. L´âge moyen de nos patients était de 50,62±13,06 ans avec des extrêmes de 18 et 70 ans. La tranche d´âge la plus représentée était celle de 58-67 ans. La dysfonction diastolique du ventricule gauche était modérée chez 17 patients (53,13%), et moyenne chez 15 patients (46,87%). Il ressort clairement de cette étude que les néphropathies glomérulaires et interstitielles sont les plus fréquemment rencontrées et responsables d´une dysfonction diastolique de modérée à moyenne du VG. La maladie de Takayasu (MT) est une artériopathie inflammatoire chronique touchant l´aorte, ses principales branches et les artères pulmonaires. Son appellation tient de l´ophtalmologiste japonais MikitoTakayasu qui publia en 1908 la première description de la maladie. Il s´agissait d´un patient de 78ans admis pour douleur abdominale, douleur du membre inférieur droit à la marche, insomnie. Evoluant depuis 1an sans antécédent de maladie cardio vasculaire connu. A l´examen physique: le rythme cardiaque régulier à 87 battement par minute sans bruits pathologique surajoutés avec une absence de pouls pédieux droit, tension artérielle à 120/78 mmhg , poumons libres, abdomen souple avec une masse battante dans la fosse iliaque droite dont l´auscultation met en évidence un souffle continu. Le reste de l´examen est sans particularité. L´Angio scanner abdominal confirmait un aspect de dissection aortique étendue sur l´ensemble de l´aorte abdominale avec opacification synchrone des deux chenaux, un anévrisme thrombosé des artères iliaques primitives mesurant 48mm x100mm à droite et 38mm x 90mm à gauche, absence de fissuration visible. Nous rapportons le cas d´une dissection de l´aorte abdominale associée à un anévrisme thrombosé des artères iliaques primitives révélant une maladie de Takayashu au service de cardiologie de l´hôpital national Ignace Deen. La fréquence de la dissection de l´aorte abdominale au cours de la maladie de Takayasu est rare. Elle est plus souvent diagnostiquée dans sa phase occlusive. Le pronostic dépend des complications évolutives.

La déhiscence post traumatique constitue une affection rare et qui, souvent, assombri le pronostic visuel des patients greffés. Elle peut survenir après toute kératoplastie transfixiante même pour des traumatismes de faible énergie.

Le tétanos demeure un problème majeur de santé publique en Afrique Subsaharienne. La forme localisée semble rare, contrairement à la forme généralisée suffisamment décrite. Nous rapportons un cas de tétanos localisé sans porte d´entrée apparente, pris en charge dans un service des Maladies Infectieuses à Bamako. Il s´est agi d´une infirmière à la retraite de 59 ans qui n´avait pas fait de rappel de vaccination antitétanique depuis 10 ans, correspondant à la période de son dernier accouchement. Elle avait été référée dans un tableau clinique associant une dysphagie et impossibilité d´ouvrir la cavité buccale. L´histoire de la maladie avait révélé un long itinéraire thérapeutique marqué par de nombreuses consultations spécialisées sans amélioration aucune. Le diagnostic de tétanos localisé sans porte d´entrée apparente avait été retenu après avoir éliminé toute autre affection locale. L´évolution était déjà favorable dix jours après une prise en charge adéquate. Sous diagnostiqué ou méconnu du personnel de santé, le tétanos localisé peut mimer d´autres affections retardant le diagnostic et la prise en charge. La sensibilisation ciblée de la population doit être renforcée en vue du respect scrupuleux du calendrier des rappels vaccinaux.

One of the complications of Etonogestrel implant (Implanon NXT®) is it usually migration 2 cm away from its insertion point. The case presented here is of a rarity. Indeed, the rod has migrated up to 17 cm from the insertion point and was localized in the left axillary region. After unsuccessful extraction of the Etonogestrel implant in the first attempt, its extraction was possible and completed without complications with the second attempt.

In our study, we can confirm that the teaching/learning of oral French as a foreign language is not an easy task, however important in the training of learners. The spoken word is a main daily language practice, but it is poorly considered and does little real education. Considered as elusive, ephemeral, it benefits from a negative representation in relation to written language practice, to which it is constantly compared and to which it is most often used as support. For many language teachers, teaching / learning of the spoken language remains a conflictual, unclear and poorly defined practice in the language class, reducing itself to global and sometimes uncertain practices. The purpose of this study is to highlight the notion of the spoken word and to show that the acquisition of oral communication skills can constitute, independently of the written word, an objective in its own right in a foreign language class. It is also a question of proposing a methodology for oral teaching / learning and its evaluation which takes into account its variations and specificities.

This research study aims, on the one hand, to reveal the fundamental role of writing and, on the other hand, to better understand the real needs of learners towards it.To do this, we opted for a representative field. Our field of study is made up of learners of different profiles and, in other words more specific, of varied fields of study. This makes our research more credible and more qualitative. Starting from this principle, we have reformulated open questions such as: What is the importance of writing in the teaching / learning of a foreign language ?, What type of writing do you practice the most ?, How to develop the competence of writing ?, etc.Analysis of the data collected enabled us to identify some relevant didactic proposals. Also, the reformulation of the questionnaire helped us considerably to better identify the strengths or weaknesses of the learners and, at the same time, to find reliable solutions for these challenges

Objectif : Etudier les aspects épidémiologiques, cliniques et thérapeutiques des gangrènes de Fournier dans le service d'urologie du CHU du Point G à Bamako (Mali).Patients et Méthodes : Nous avons effectué une étude rétrospective transversale et descriptive de tous les patients admis pour Gangrène de Fournier et pris en charge dans le service d'urologie du C.H.U de Point G sur une période de quatre ans allant du 1er Janvier 2009 au 31 Décembre 2012. Les paramètres étudiés étaient : l'âge, le sexe, le motif de consultation, les facteurs de risque, l'étendue des lésions, les méthodes thérapeutiques utilisées et le résultat des traitements. Résultats : La fréquence de la gangrène de Fournier représentait 1,2 % des pathologies urologiques enregistrées. La moyenne d'âge était de 49 ans avec des extrêmes de 22 et 76 ans. Une cause urinaire était notée dans 26% des cas. Les germes rencontrés étaient Escherichia coli dans 26% des cas suivi de Klebsiella pneumonie et Staphylococcus aureus. La tuméfaction des organes génitaux externes a été l'expression clinique la plus fréquente dans 56 % des cas. Les lésions avaient une étendue périnéo-scrotale dans 65 % des cas. Les patients étaient traités par une triple antibiothérapie dans 87% des cas suivie d'une excision des tissus nécrotiques avec mise à plat et drainage urinaire. Conclusion: La gangrène de Fournier est une affection rare mais très grave. Une prise en charge médico-chirurgicale intensive et précoce améliore le pronostic de façon spectaculaire.

L'insuffisance cardiaque (IC) est une affection chronique, complexe et grave, dont la symptomatologie est susceptible d'affecter fortement la qualité de vie des patients atteints avec une létalité élevée. Elle constitue un problème important de santé publique. Elle représente la première cause d'hospitalisation des plus de 60-65 ans. Il s'agissait d'une étude rétrospective descriptive transversale faite au service de cardiologie de l'hôpital national Ignace Deen CHU de Conakry. Les données ont été recueillies à partir de dossiers de patients hospitalisés pour insuffisance cardiaque pendant la période allant du 01 Janvier 2018 au 30 Juin 2019. La mortalité de l'IC a été déterminée à partir des dossiers de tous les patients décédés admis pour insuffisance cardiaque durant période d'étude. Cette étude nous permis de connaitre la mortalité liée à l'insuffisance cardiaque qui était de 9.6% au service de cardiologie de l'hôpital national Ignace Deen. Ce taux était inférieur aux résultats de certains travaux africains notamment au Gabon avec 10.3% à Lomé avec 11.86% et au Nigéria avec 10.1%. Cependant notre résultat est superposable à celui de Yaoundé qui était de 9.03%. Il reste supérieur à celui du Maroc avec 6.1%. La mortalité de l'insuffisance cardiaque reste non négligeable. Celle ci devrait être dépistée aussi précocement que possible et prise en charge correctement.

Introduction: les conséquences de la malnutrition aiguë sévère s’expriment en termes de santé et de survie de la personne affectée, mais aussi en termes de développement intellectuel de l’individu, sa productivité et globalement l’économie nationale. Sa prise en charge nécessite d’énormes moyens financiers. L’objectif de la présente étude est d’évaluer la performance de la prise en charge versus coût du traitement des enfants malnutris aigus sévères.Méthodes: c’est une étude rétrospective réalisée de janvier à décembre 2014 auprès de 199 enfants de 0 à 59 mois admis au centre de récupération et d’éducation nutritionnelle de Kaya, Burkina Faso. Le coût de la prise en charge, la durée de séjour, le gain de poids journalier et la vitesse de récupération ont été analysées selon les méthodes de calcul standards. Le test de Mann-Whitney et de Kruskall-Wallis ont été utilisés pour comparer les médianes au seuil de 0,05.Résultats: comme présagé, les enfants de 6 à 23 mois étaient les plus affectée (51,8 %) et les infections respiratoires aiguës étaient les pathologies les plus associées (57,9 %). La durée médiane de séjour était de 9,0 (7,0 - 13,0) jours, la vitesse de récupération médiane de 100,0 (65,8 - 143,3) g/j et le gain pondéral médian journalier de 18,1 (11,6 - 27,7) g/kg/j. Le coût moyen de traitement d’un enfant malnutri est estimé à 15715,3 FCFA (25,2 USD).Conclusion: le coût de traitement est difficilement supportable par les parents des enfants malnutris d’où la nécessité de l’intervention du gouvernement et ses partenaires au développement.

Le rétrécissement urétral est une pathologie dont l'étiologie et la prise en charge varient selon le contexte. L'objectif de l'étude était de procéder à une analyse des aspects épidémiologiques, étiologique et thérapeutique du rétrécissement urétral dans notre contexte. Il s'agit d'une étude transversale longitudinale portant sur les cas de rétrécissement urétral acquis admis dans notre service entre mars 2014-février 2016. Age moyen: 24,5ans (10 et 81 ans). Le diagnostic fut confirmé par l'Uretro-Cystographie Rétrograde et Mictionnelle (UCRM). La longueur moyenne du rétrécissement était de 2,28cm (0,5-5cm). Les modalités thérapeutiques adoptées: la Résection Anastomose Termino-Terminale (RAT-T); la Dilatation Urétrale Rétrograde Itérative (DURI) etc. L'évaluation du résultat 6-15 mois postopératoire; satisfaisant en l'absence de récidive, RPM ≤30cc et un jet urinaire fort et mauvais en cas de récidive de dysurie ou RPM ≥100cc. Le RU occupe 7,14% de nos activités urologiques. La plus part de nos patients sont des cultivateurs provenant de la zone rurale. Les antécédents d'urétrite répétée ont été le plus souvent évoqués par nos patients et 78,57% d'entre eux étaient des hommes mariés dont 91% polygames. Les patients ont consulté surtout pour dysurie soit 50% de l'effectif et 50,01% de nos patients ce sont présentés avec une infection urinaire secondaire dont le germe prédominant est Escherichia coli. L'étiologie infectieuse a été la plus dominante soit 56,52%. La portion la plus atteinte a été l'urètre bulbaire soit 45,60% des cas. La RAT-T a été la technique la plus utilisée soit 39,13%. Globalement nous avons eu 85,65% de bon résultat et 13,04% d'échec; les taux de succès les plus élevés ont été obtenus avec la résection anastomotique soit 94,44% respectivement. Le rétrécissement urétral reste une pathologie fréquente chez les jeunes. L'étiologie infectieuse demeure la plus dominante dans notre contexte. L'accent doit être mis sur la prévention et la Prise en Charge (PEC) efficace et efficiente des Infections Sexuellement Transmissible (IST).

L'objectif de cette étude était d'analyser les aspects épidémiocliniques, thérapeutiques et anatomopathologiques de la fistule urogénitale (FUG). L'étude rétrospective porte sur les cas de FUG admis dans le Service de Chirurgie Générale et Gynéco-obstétrique entre le 1er janvier 2014 et le 30 décembre 2015 y compris les 5 premières campagnes du projet fistule-Mali. La FUG occupe 19,53% de nos activités urologiques. L'âge moyen au 1er mariage: 16,57 ans. La majorité (96,70%) de nos patientes était analphabètes et non salariées provenant à 85,36% des communes rurales. La stagnation du travail était l'étiologie dominante avec 91,50% de mort-né. Les patientes étaient primipares à 43,33% et, parmi elles 53,60% n'ont effectué aucune consultation prénatale. Le taux de divorce lié à la maladie était estimé à 7,30%. La plupart des patientes ont bénéficié d'une fistulorraphie simple dont 121 par voie basse, 26 par voies hautes et 3 par la voie mixte. Les résultats ont été satisfaisants chez 65,33% et mauvais chez 34,66% des patientes. Les fistules type I et type V ont montré les plus forts taux de succès comparés aux fistules types IV. La fistule urogénitale demeure un réel problème de santé publique. Le traitement est surtout chirurgical et son pronostic est compromis par l'étroitesse du champ, la complexité des lésions et l'état du tissu environnant. L'accent doit être mis sur la promotion socioéconomique des filles et l'accessibilité aux soins obstétricaux d'urgence. La recherche et les échanges doivent continuer afin de faciliter la mise au point d'une classification standard.

In 2017, the Crown Prince of Saudi Arabia announced the new mega-city NEOM. It further explores how city branding strategies position NEOM and the Kingdom regionally and globally and reaffirm the Crown Prince’s power. It also questions the conceptual nature of the branded object itself, as branding documents show that NEOM alternates between being a city, a start-up, a country, and the Crown Prince’s legacy.

L’objectif de cette étude était de participer à la sauvegarde et à l’amélioration des performances du zébu Azawak au Mali en proposant une estimation des performances de croissance et de reproduction de cette race bovine élevée au nord du pays. Le suivi a concerné 1129 animaux dans quatre communes du cercle de Ménaka, entre novembre 2009 et février 2010. A 30 jours, les mâles pesaient en moyenne 39 ± 10 kg et les femelles 32 ± 4 kg. Aucune différence significative (p > 0,05) n’a été enregistrée entre les poids adultes des taureaux (370 ± 88 kg) et des vaches (327 ± 46 kg). L’âge au premier vêlage a été de 50 ± 11 mois avec un intervalle entre vêlages de 16 ± 4 mois. Les corrélations du poids avec le périmètre thoracique (r = 0,95) et la hauteur au garrot (r = 0,94) ont été les plus élevées. Des équations barymétriques ont été établies pour chaque catégorie animale (veau/velle, génisse/taurillon, vache/taureau) au moyen de régressions polynomiales du poids (y) sur le périmètre thoracique (x). Celles-ci pourront servir à établir une table de conversion du périmètre thoracique en poids vif ou un ruban barymétrique à l’attention des agents de l’élevage, afin de faciliter un suivi zootechnique et sanitaire des animaux sur le terrain.

Le but de notre étude était d'évaluer les résultats du traitement chirurgicale du plastron appendiculaire après appendicectomie différée ou immédiate. Il s'agit d'une étude rétrospective de type descriptif allant de Janvier 2000 au 31 Décembre 2007 portant sur 27 patients reçus et traités pour plastron appendiculaire. Le diagnostic était clinique par la présence d'une masse à la fosse iliaque droite, échographique, ou per opératoire. Tous les patients opérés en urgence ont été classés dans le groupe I et ceux opérés à froid, groupe II. Nous avons noté 18 hommes et 9 femmes avec un sex-ratio homme /femme = 3. L'âge moyen de nos patients était de 33 ans avec des extrêmes de 19 et 57 ans. Les signes cliniques étaient dominés par la douleur à la fosse iliaque droite et la fièvre respectivement 25 (92,6%) et 15 (55,6%) des cas. Dans le groupe I, l'appendicectomie n'a pu être réalisée pour 7 patients (n = 15) dû aux difficultés opératoires. Dans les autres cas, l'appendicectomie était possible au prix de l'élargissement de l'incision de Mac Burney et une durée de séjour plus longue. Le groupe II a concerné 12 patients (n = 12) avec 9 abords laparoscopiques et dans 3 cas, la voie de Mac Burney a été empruntée. Trois cas d'adhérences péritonéales ont été notés lors de la coelioscopie. L'appendicectomie à froid du plastron appendiculaire est un procédé sûr et efficace. Elle permet d'éviter les cicatrices inesthétiques et les fistules digestives iatrogènes. L'appendicectomie immédiate doit être abandonnée chez les patients porteurs de plastron appendiculaire car elle engendre beaucoup de morbidité.
