Aims The aim of this study was to determine the contemporary use of reperfusion therapy in the European Society of Cardiology (ESC) member and affiliated countries and adherence to ESC clinical practice guidelines in patients with ST-elevation myocardial infarction (STEMI). Methods and results Prospective cohort (EURObservational Research Programme STEMI Registry) of hospitalized STEMI patients with symptom onset Conclusions The use of reperfusion therapy for STEMI in the ESC member and affiliated countries was high. Primary PCI was the most frequently used treatment and associated total in-hospital mortality was below 5%. However, there was geographic variation in the use of primary PCI, which was associated with differences in in-hospital mortality.

We present XMM-Newton RGS observations of the soft X-ray spectrum of NGC 4051, and explore their implications for the inner accretion disk and ionized outflow in the active galactic nucleus. We fit the soft X-ray excess with a relativistically broadened O VIII recombination spectrum, including the entire line series and recombination continuum. This plus an underlying power law continuum provides a much better fit to the soft X-ray spectrum than a single temperature or disk blackbody plus power law. The emission line profiles, computed for a Kerr metric around a maximally rotating black hole, reveal a sharply peaked disk emissivity law and inner radius smaller than 1.7 R_G. The spectrum also includes narrow absorption and emission lines from C, N, O, Ne, and Fe in an ionized outflow. Outflow column densities are relatively low and do not create significant edges in the spectrum. The small amount of absorption bolsters confidence in the detection of relativistic emission line features. The narrow-line emitter has a large (76%) global covering fraction, leading to strong forbidden lines and filling in of the resonance absorption lines. We also find broad C VI Ly-alpha and very broad O VII emission from the broad-line region. The narrow and broad-line regions span large ranges in ionization parameter, and may arise in a disk outflow. The ionized absorber has a large ionization range which is inconsistent with pressure equilibrium in a multiphase medium. The improvement and the refinement of non-viable Rhizopus arrhizus biomass were investigated via immobilization. Immobilization was carried out by using sodium alginate/CaCl solution and formaldehyde/HCl cross-linking with dead Rhizopus arrhizus biomass and were used for the sorption of radionuclides from low level effluent wastes. The sodium alginate/CaCl immobilized biomass (ratio 1:2) showed about 86% sorption for Am activity but due to its soft nature and tendency to undergo distortion in shape, is unsuitable for practical applications. The biomass cross-linked with 15% formaldehyde/0.1 M HCl solution has a relatively high mechanical strength and rigidity. It was showing a sorption of >99% for Am activity and has the sorption capacity of ~65 mg/g for americium and uranium. ABSTRACT We examine the 3–10 keV EPIC spectra of Mrk 205 and 509 to investigate their Fe K features. The most significant feature in the spectra of both objects is an emission line at 6.4 keV. The spectra can be adequately modelled with a power law and a relatively narrow (σ < 0.2 keV) Fe Kα emission line. Better fits are obtained when an additional Gaussian emission line, relativistic accretion-disc line, or Compton reflection from cold material, is added to the spectral model. We obtain similar goodness of fit for any of these three models, but the model including Compton reflection from cold material offers the simplest, physically self-consistent solution, because it only requires one reprocessing region. Thus the Fe K spectral features in Mrk 205 and 509 do not present strong evidence for reprocessing in the inner, relativistic parts of accretion discs. [ABSTRACT FROM AUTHOR]

Abstract The X-ray-bright Seyfert 1 galaxy III Zw 2 was observed with XMM–Newton in 2000 July. Its X-ray spectrum can be described by a power law of photon index Γ= 1.7 and an extremely broad (FWHM∼ 140 000 km s[sup -1] ) Fe Kα line at 6.44 keV. The iron line has an equivalent width of ∼800 eV. To study the long-term X-ray behaviour of the source we have analysed 25 yr of data, from 1975 to 2000. There is no evidence of significant intrinsic absorption within the source or of a soft X-ray excess in the XMM or archival data. We do not detect rapid X-ray variability (a few × 10[sup 3] s ) during any of the individual observations; however, on longer time-scales (a few years) the X-ray light curve shows 10-fold flux variations. We infer a black hole mass of ∼10[sup 9] M[sub ⊙] (from Hβ FWHM) for III Zw 2 which is much higher than some previous estimates. A comparison of X-ray variability with light curves at other wavelengths over a 25-yr period reveals correlated flux variations from radio to X-ray wavelengths. We interpret the variable radio to optical emission as synchrotron radiation, self-absorbed in the radio/millimetre region, and the X-rays mainly as a result of Compton up-scattering of low-energy photons by the population of high-energy electrons that give rise to the synchrotron radiation. We present spectroscopic and high-speed photometric data of the eclipsing polar V895 Cen. We find that the eclipsed component is consistent with it being the accretion regions on the white dwarf. This is in contrast to Stobie et al. who concluded that the eclipsed component was not the white dwarf. Further, we find no evidence for an accretion disc in our data. From our Doppler tomography results, we find that the white dwarf has M≳0.7 M[sub ⊙]. Our indirect imaging of the accretion stream suggests that the stream is brightest close to the white dwarf. When we observed V895 Cen in its highest accretion state, emission was concentrated along field lines leading to the upper pole. The article presents a case report of a successful pregnancy in a 33-year-old patient with a cloacal malformation consisting multiple anomalies, including a cloaca with an extremely short vagina, which are associated with difficulty in maintaining a pregnancy. The patient was in her sixth pregnancy, and was being managed with low dose aspirin and Fragmin daily from the first trimester. At 28+4 weeks gestation, she was delivered by caesarean section.

The highly infectious disease COVID-19 caused by the Betacoronavirus SARS-CoV-2 poses a severe threat to humanity and demands the redirection of scientific efforts and criteria to organized research projects. The international COVID19-NMR consortium seeks to provide such new approaches by gathering scientific expertise worldwide. In particular, making available viral proteins and RNAs will pave the way to understanding the SARS-CoV-2 molecular components in detail. The research in COVID19-NMR and the resources provided through the consortium are fully disclosed to accelerate access and exploitation. NMR investigations of the viral molecular components are designated to provide the essential basis for further work, including macromolecular interaction studies and high-throughput drug screening. Here, we present the extensive catalog of a holistic SARS-CoV-2 protein preparation approach based on the consortium’s collective efforts. We provide protocols for the large-scale production of more than 80% of all SARS-CoV-2 proteins or essential parts of them. Several of the proteins were produced in more than one laboratory, demonstrating the high interoperability between NMR groups worldwide. For the majority of proteins, we can produce isotope-labeled samples of HSQC-grade. - Abstract
Functional studies of circular RNAs (circRNAs) began quite recently, and few data exist on their function in vivo. Here, we have generated a knockout (KO) mouse model to study circDlc1(2), a circRNA highly expressed in the prefrontal cortex and striatum. The loss of circDlc1(2) led to the upregulation of glutamatergic-response-associated genes in the striatal tissue, enhanced excitatory synaptic transmission in neuronal cultures, and hyperactivity and increased stereotypies in mice. Mechanistically, we found that circDlc1(2) physically interacts with some mRNAs, associated with glutamate receptor signaling (gluRNAs), and with miR-130b-5p, a translational regulator of these transcripts. Notably, differently from canonical microRNA (miRNA) "sponges," circDlc1(2) synergizes with miR-130b-5p to repress gluRNA expression. We found that circDlc1(2) is required to spatially control miR-130b-5p localization at synaptic regions where gluRNA is localized, indicating a different layer of regulation where circRNAs ensure robust control of gene expression via the correct subcellular compartmentalization of functionally linked interacting partners., Competing Interests: Declaration of interests The authors declare no competing interests., (Copyright © 2024 The Author(s). - Abstract
The nucleocapsid protein of severe acute respiratory syndrome coronavirus 2 encapsidates the viral genome and is essential for viral function. The central disordered domain comprises a serine-arginine-rich (SR) region that is hyperphosphorylated in infected cells. This modification regulates function, although mechanistic details remain unknown. We use nuclear magnetic resonance to follow structural changes occurring during hyperphosphorylation by serine arginine protein kinase 1, glycogen synthase kinase 3, and casein kinase 1, that abolishes interaction with RNA. When eight approximately uniformly distributed sites have been phosphorylated, the SR domain binds the same interface as single-stranded RNA, resulting in complete inhibition of RNA binding. Phosphorylation by protein kinase A does not prevent RNA binding, indicating that the pattern resulting from physiologically relevant kinases is specific for inhibition. Long-range contacts between the RNA binding, linker, and dimerization domains are abrogated, phenomena possibly related to genome packaging and unpackaging. This study provides insight into the recruitment of specific host kinases to regulate viral function.

45. Laboratory diagnosis and epidemiological features of leptospirosis cases in a tribal region of Western Maharashtra, India: A retrospective study.

- Abstract
Purpose: Palghar district, located in the coastal region of the Konkan division of Maharashtra, has a predominantly tribal population. Leptospirosis is a major neglected public health problem and is highly underreported in Palghar district. The study aimed to evaluate the seropositivity of Leptospira infection and its associated epidemiological factors in tribal areas of the Palghar district of Maharashtra., Methods: The present retrospective study included 94 samples of patients clinically suspected of leptospirosis during a period of one year (2021-2022) tested at Model Rural Health Research Unit (MRHRU) Dahanu. The serum sample testing was done for the presence of specific Leptospira IgM antibodies using the Panbio™ Leptospira IgM ELISA kit. Leptospirosis seropositivity was correlated with various epidemiological risk factors., Results: A total of 12 samples of patients tested positive for specific IgM antibodies by ELISA method, indicating an overall positivity of 12.8%. Among those who tested positive, fever (83.3%), headache (58.3%), myalgia (50%), redness of the eyes (50%), and calf tenderness (16.7%) were the common symptoms observed. Subjects with redness of the eyes were significantly associated with leptospirosis (p = 0.018). The highest positivity (50%) was reported from the Ganjad area of Dahanu taluka. Farmers and animal handlers were most affected by leptospirosis., Conclusion: The high proportion of Leptospirosis cases reflects the endemic nature of the disease in the Palghar district. This study shows seasonal trends in leptospirosis incidence over the year. The clinical presentation of leptospirosis may vary from sub-clinical to mild illness to severe and potentially fatal. Objective: Empty nose syndrome (ENS) is a relatively uncommon disease that greatly impacts the quality of life and presents diagnostic challenges. We sought to identify objective clinical findings unique to patients with ENS, and in doing so identified compensatory mucosal hypertrophy in an alternating, undulating swelling on endoscopy and coronal computerized tomography (CT) that we have termed the "Serpentine Sign." Here, we investigated whether this radiographic finding is a reliable manifestation in ENS patients., Methods: Retrospective review was undertaken to identify ENS patients with past turbinoplasty, an ENS6Q score of at least 11/30, and symptomatic improvement with the cotton placement test. Control patients without complaints of ENS symptoms (ENS6Q < 11) were identified for comparison. ENS and control patients had coronal CT imaging available to evaluate for the Serpentine Sign, as well as ENS6Q scores, and histologic analysis of nasal tissue., Results: 34 ENS and 74 control patients were evaluated for the presence of the Serpentine Sign. Of the 34 patients with ENS, 18 exhibited this radiographic finding on CT imaging (52.9%) compared to 0 of the 74 control patients (p < 0.0001). Further analysis demonstrated that ENS patients with the Serpentine Sign had lower median scores on ENS6Q than ENS patients without (17.5 vs. 22, p = 0.033). Histology revealed disorganized subepithelium rich in seromucinous glands similar to the nasal septum swell body., Conclusion: The "Serpentine Sign" is a unique presentation of hypertrophic change to the nasal septum soft tissue that is specific to ENS patients and may serve as a reliable radiographic and endoscopic finding in diagnosis., Level of Evidence: 4 Laryngoscope, 134:1089-1095, 2024., (© 2023 The American Laryngological, Rhinological and Otological Society, Inc.)

Background: Studies evaluating the safety and efficacy of drug coating balloons (DCB) for the treatment of lesions in large coronary vessel are limited., Aims: Our study aimed to evaluate the performance of a sirolimus DCB in large coronary arteries., Methods: We analyzed all the procedures included in the EASTBOURNE Registry (NCT03085823) enrolling patients with a clinical indication to percutaneous coronary intervention performed by a sirolimus DCB according to investigator judgment. In the present analysis, a cut-off of 2.75 mm was used to define large coronary arteries. Primary endpoint of the study was clinically driven target lesion revascularization (TLR) at 24 months whereas secondary endpoint included procedural success, myocardial infarction (MI), cardiac death and total mortality., Results: Among the 2123 patients and 2440 lesions enrolled in the EASTBOURNE study between 2016 and 2020, 757 patients/810 lesions fulfilled the criteria for the present analysis. Mean reference vessel diameter was 3.2 ± 0.3 mm with mean lesion length of 22 ± 7 mm. Procedural success was high (96%) and at 2-year follow up the device showed a good efficacy with a TLR rate of 9%. There were 34 deaths (4.5%), 30 MIs (4%) and 8 BARC type 3-5 bleedings (1.1%). In-stent restenosis (629 lesions) and de novo lesions (181) were associated with 11% and 4% rates of TLR at 2 years, respectively (p = 0.003)., Conclusions: Clinical performance of a sirolimus DCB in large coronary artery vessels shows promising signals at 2-year follow up, both in de novo and in-stent restenosis lesions., (© 2024 Wiley Periodicals LLC.)

- Abstract
Objective: Early identification, referrals and timely treatment is crucial to improve the outcome of Birth Defects (BDs). The role of Community Health Workers (CHWs) in India is largely fulfilled by Accredited Social Health Activists (ASHAs) who can play an important role in community-based reporting and referrals of BDs in newborn. In the study area, ASHAs have been previously trained for identification of BDs under Rashtriya Bal Swasthya Karyakram (RBSK); however, reporting through them was found to be sub-optimal., Methods: To strengthen the community based reporting of BDs through ASHAs, audio visual based training was provided to 1225 ASHAs in 6 tribal blocks of Palghar district in Maharashtra, India. The change in knowledge about BDs before and after training among 1023 participants was analyzed using paired sample t test & McNemar test., Results: Findings reveal a significant increase in the knowledge of participants with regards to prevention, risk factors and identification of BDs after intervention as compared to before intervention., Discussion: The study suggests that audio-visual based refresher training increased knowledge of ASHAs with respect to identification, prevention and risk factors of BDs which may eventually improve reporting and referrals of BDs at community level., (© 2023 Wiley Periodicals LLC.)

- Abstract
Background Objectives: Despite several adversities imposed by the COVID-19 pandemic, it was crucial to sustain research having public health relevance such as investigations around sickle cell disease (SCD). Against this background, an ongoing ICMR-multicentric study for newborn screening of SCD in the tribal population at Model Rural Health Research Unit (MRHRU-Dahanu) in Palghar District, Maharashtra constituted the current study setting. This was a descriptive study wherein, certain measures were undertaken and strategies were developed in view of the challenges in newborn screening for SCD due to the COVID-19 pandemic during December 2019-September 2021 at Sub District Hospital, MRHRU-Dahanu., Methods: During the onset of the pandemic, (December 2019-March 2020), the follow up was possible in 26.7 per cent (20/75) of the newborns. Subsequently, challenges such as travel restrictions, fear of COVID-19, shortage of staff were experienced with respect to enrolment and follow up visits., Results: After implementing certain pragmatic strategies (ASHA involvement, usage of virtual platform and flexible visits), follow up rate increased to 47.5 per cent (66/139) between July 2020-April 2021 (post first lockdown) and to 66 per cent (65/98) during the second wave (May 2021-August 2021)., Interpretation Conclusions: The study emphasizes the importance of network building, use of virtual platform and engaging health workers in tribal settings. Background: Tracheobronchial foreign body aspiration is a classic pediatric emergency, and its associated morbidity particularly depends on the anesthetic management, which differs according to the center and the practitioner., Aims: The aim of this study was to evaluate the different anesthetic practices for tracheobronchial foreign body extraction., Methods: A survey was sent via email to the member physicians of the Association des Anesthésistes Réanimateurs Pédiatriques d'Expression Française (ADARPEF). The survey included 28 questions about the organizational and anesthetic management of an evolving clinical case., Results: A total of 151 physicians responded to the survey. Only 13.2% of the respondents reported that their institution had a management protocol, and 21.7% required a computerized tomography scan before the procedure was performed for children who were asymptomatic or mildly symptomatic during the night. There were 56.3% of the respondents who reported that extraction with a rigid bronchoscope is the only procedure usually performed in their institution. Regarding rigid bronchoscopy, 47.0% used combined intravenous-inhalation anesthesia. The objective was to maintain the child on spontaneous ventilation for 63.6% of the respondents, but anesthesia management differed according to the physician's experience., Conclusions: Our study confirms the diversity of practices concerning anesthetic for tracheobronchial foreign body extraction and found reveal differences in practice according to physician experience., (© 2023 The Authors.
