2. Case of Obtaining Permission for Data Collection in Turkey within the Context of Academic Freedom and Ethics

- Abstract
The main purpose of this qualitative study is to identify the lived experiences of academicians working in the field of educational administration in Turkey with regard to obtaining permission for scientific research in educational institutions and examine it within the context of academic freedom and ethics. A case-study approach was used to collect the data. The study was conducted with the participation of 41 academicians working in universities in different regions. The data were collected with e-mail interviewing and analyzed with content analysis. The results revealed that the academicians experience serious problems regarding obtaining permission and their requests for research permission are not granted for various reasons and sometimes without justification. Based on the results, it can be suggested that the participants' suggestions should be paid attention and more objective criteria should be taken into consideration.

3. The Views of Educational Constituencies about the Compulsory Religious Culture and Morality Education Course = Zorunlu Din Kültürü ve Ahlak Bilgisi Dersine Iliskin Egitimci Görüsleri

- Abstract
In this qualitative study aiming to identify the views of educational constituencies (teachers and academics) about compulsory Religious Culture and Morality Education (RCME) course. The study was designed as a case study and the study group constituted 53 participants, who are graduate teachers and postgraduate students who are studying at the Graduate School of Education. The data of the study were gathered through face to face interviews using semi-structured interview forms. The results of the study suggest that the view of the participants regarding religion education is parallel to the general discussions and critics in the society. In this sense, the study has demonstrated that while some of the participants support the current practice compulsary religion education; a great majority of the participants do not approve both the current practice and the content of the course and they suggested that RCME course should be taught electively in accordance with the demands of students or their parents and the content of the course should be enlarged in a way to include different religions, beliefs and denominations. Accordingly, RCME course should be revised in terms of both its content and status of delivery. 5. A Look at Mother Tongue Education in the Context of the Right to Education

- Abstract
In Turkey, education in state schools can be delivered in foreign languages such as German, French and English. However, mother tongue education cannot be provided in the languages of minorities or local groups other than those officially accepted as minorities (that is, according to the Treaty of Lausanne). In this regard, the primary aim of this study was to reveal the views of postgraduate students (Masters (MA)/PhD) studying at a graduate school of educational sciences on mother tongue education and the applicability of mother tongue education in Turkey. The study was conducted with a total of 46 students, 28 MA and 18 PhD students. The data were gathered through semi-structured open-ended questions in November 2017. According to the findings, most of the participants viewed mother tongue education as one of the basic human rights. Moreover, while some of the participants thought that the country was not yet ready to provide education to non-official minorities or local communities in their mother tongues, some objected to mother tongue education with the concern that it would cause division within the country.

6. Roma Children: Victims of Social Injustices in Education

- Abstract
In this qualitative study aiming to identify the opinions of postgraduate students studying in Izmir, Turkey about the schooling practices of Roma children from social justice perspective, case study design was used. The study group included 46 post graduate students. As result of the study, it was identified that most of the participants agreed that the Roma children have limited access to schooling for such reasons as economics, Roma culture and the family structure, exclusion practices towards them, lack of social state and learned desperateness and made some suggestions accordingly. It can be argued that the disadvantaged status of Roma children and the inequities they exposed to manifest themselves as economic, cultural and thus participative injustices in education. Accordingly, improving the socio-economic conditions of Roma families thus children, valuing the culture of Roma and including Roma families and children in the education system were recommended.

7. The Reproduction of Social Inequality through Education: The Case of Vocational High Schools in Turkey

- Abstract
The main purpose of this qualitative-phenomenological study is to examine vocational high schools in Turkey within the context of social inequality. Using criterion sampling technique, fourteen teachers working in vocational high schools participated in a semi-structured interview. The obtained data were analyzed through the descriptive analysis method. The findings revealed that vocational high schools which have mostly students from low socio-economic background and an inferior social status can not function effectively and fail to enable the students to have jobs in a short time. It was also found out that the division of high schools as academic and vocational was accepted by almost half of the participants as a required condition and most of the teachers associated the current problems regarding vocational high schools with students' incompetences, indifferences, problematic behaviours, etc. Thus, they were found to be inclined to ignore the inequalities arising from the nature of class differences. In this sense, it can be argued that vocational education and vocational high schools reproduce existing social inequalities with socio-economic backgrounds of the students, low social status of these schools, division of schools as vocational and academic and teachers' discourses.

10. A Critical Review of Religious Education Policies and Practices of Turkey from the Perspective of Inclusion

- Abstract
This study presents a critical review of religious education policies and practices from the perspective of inclusion. In this sense, it raises the problematics of compulsory and optional religious education courses and Imam Hatip schools, which have an important place in state-religion relationships in Turkey as an inclusive education issue. Considering the education policies and practices that emerged as a result of these policies, it can be argued that no matter how objective, democratic, pluralistic and thus inclusive the discourses the authorities and governments used were, from the very beginning, they did not manage to be independent of any ideology and included exclusive practices in differing aspects. Accordingly, what is expected from the Turkish education system is to embrace an inclusive approach that encourages the representation of all religious groups and atheists and to prioritise "students," not ideologies.

Aim: This study presents the causes of death-censored graft loss among kidney transplant recipients. Materials and Methods: Medical records of the patients, who had undergone kidney transplantation at a tertiary center between November 2010 and December 2018, were retrospectively reviewed. Death-censored graft loss was described as an irreversible graft failure signified by return to long-term dialysis (or re-transplantation). Inclusion criteria were: patients who had undergone kidney transplantation, and subsequently lost their first graft, and a follow-up of more than one year after kidney transplantation. Results: Of 269 kidney transplant recipients, 33 recipients with a mean age of 33.54 ± 15.37 years (17 male and 16 female) were included in the study. The rate of death-censored graft loss was 12.26%. Of graft failures, 3.03% occurred in the hyperacute phase, 18.18% in the acute phase, and 78.78% in the chronic phase. Chronic allograft nephropathy was the leading cause of graft failure (48.48%). Other causes were medical problems (18.18), immunological problems (18.18%) and surgical complications (15.15%). Conclusion: Identification of the true causes of graft failure described under the heading chronic allograft nephropathy is noteworthy. Comprehensive biochemical, physiological, pathological, immunological, and genetic research should be implemented to remove the obstacles in kidney transplantations. [ABSTRACT FROM AUTHOR]

14. The Principals of Primary Schools Ideas on Their School Development Strategies and Practices

- Abstract
The school improvement came into question that schools meet the educational needs has been made to make it more effective, changing in parallel globalization. The purpose of school improvement is to rise levels of student achievement and school environment to increase participation. In this context, the basic aim of this study was to determine the ideas of the principals about school improvement and what strategies they use in order to improve their schools. The research was conducted with the principals of the primary schools in Karabaglar, a town in the province of metropolitan area of Izmir. This study is a descriptive research that used qualitative data. The data were collected through semi-structured questions from 37 principals and analyzed qualitatively. Some of the results obtained in this study were as follows: According to the principals, on the top of the list of the things to do to improve schools were "improving co-operation and communication". In the second place, there were "teacher development" and "meet the need for personnel". In third place there were "improving the physical conditions" and "provision of educational technologies and increasing their use". The first thing the principals do to improve their schools was "increasing the number of the technological devices and the use of technology". On the top of the list of what the principals want to do but cannot do were "lack of financial allocations" and "lack of financial sources".

15. Prospective Teachers' Ideas about Teacher Recruitment and Their Professional Future

- Abstract
This research aimed to reveal how prospective teachers viewed their professional future and to discern how their lives would be affected in case they were not recruited as teachers. The research was conducted on senior students at Buca Faculty of Education. The sample of the research consisted of 149 senior students, including 82 girls and 67 males. The data were collected through a measurement instrument developed for this research, consisting of six open-ended questions. In data analyses, Miles' and Huberman's approach, namely "data reduction", "data display" and "conclusion verification", was made use of. Some of the results of the research could be outlined as follows: About 40% of the participants attended the programs they had chosen unwillingly, 87% would like to work as teachers, 81% thought that working on contract was not fair, 34% stated that they would not work everywhere in the country, 81% had negative ideas about their professional future, and 91% thought that their lives would be affected very badly in case they were not appointed as teachers.

Background In the general population with coronavirus disease 2019 (COVID-19), obesity is associated with an increased risk of mortality. Given the typically observed obesity paradox among patients on kidney function replacement therapy (KFRT), especially dialysis patients, we examined the association of obesity with mortality among dialysis patients or living with a kidney transplant with COVID-19. Methods Data from the European Renal Association COVID-19 Database (ERACODA) were used. KFRT patients diagnosed with COVID-19 between 1 February 2020 and 31 January 2021 were included. The association of Quetelet's body mass index (BMI) (kg/m2), divided into: Results In 3160 patients on KFRT (mean age: 65 years, male: 61%), 99 patients were lean, 1151 normal weight (reference), 1160 overweight, 525 obese I and 225 obese II/III. During follow-up of 3 months, 28, 20, 21, 23 and 27% of patients died in these categories, respectively. In the fully adjusted model, the hazard ratios (HRs) for 3-month mortality were 1.65 [95% confidence interval (CI): 1.10, 2.47], 1 (ref.), 1.07 (95% CI: 0.89, 1.28), 1.17 (95% CI: 0.93, 1.46) and 1.71 (95% CI: 1.27, 2.30), respectively. Results were similar among dialysis patients (N = 2343) and among those living with a kidney transplant (N = 817) (Pinteraction = 0.99), but differed by sex (Pinteraction = 0.019). In males, the HRs for the association of aforementioned BMI categories with 3-month mortality were 2.07 (95% CI: 1.22, 3.52), 1 (ref.), 0.97 (95% CI: 0.78. 1.21), 0.99 (95% CI: 0.74, 1.33) and 1.22 (95% CI: 0.78, 1.91), respectively, and in females corresponding HRs were 1.34 (95% CI: 0.70, 2.57), 1 (ref.), 1.31 (95% CI: 0.94, 1.85), 1.54 (95% CI: 1.05, 2.26) and 2.49 (95% CI: 1.62, 3.84), respectively. Conclusion In KFRT patients with COVID-19, on dialysis or a kidney transplant, obesity is associated with an increased risk of mortality at 3 months. This is in contrast to the obesity paradox generally observed in dialysis patients. Additional studies are required to corroborate the sex difference in the association of obesity with mortality.

In the general population with COVID-19, the male sex is an established risk factor for mortality, in part due to a more robust immune response to COVID-19 in women. Because patients on kidney function replacement therapy (KFRT) have an impaired immune response, especially kidney transplant recipients due to their use of immunosuppressants, we examined whether the male sex is still a risk factor for mortality among patients on KFRT with COVID-19. From the European Renal Association COVID-19 Database (ERACODA), we examined patients on KFRT with COVID-19 who presented between February 1st, 2020, and April 30th, 2021. 1204 kidney transplant recipients (male 62.0%, mean age 56.4 years) and 3206 dialysis patients (male 61.8%, mean age 67.7 years) were examined. Three-month mortality in kidney transplant recipients was 16.9% in males and 18.6% in females (p = 0.31) and in dialysis patients 27.1% in males and 21.9% in females (p = 0.001). The adjusted HR for the risk of 3-month mortality in males (vs females) was 0.89 (95% CI 65, 1.23, p = 0.49) in kidney transplant recipients and 1.33 (95% CI 1.13, 1.56, p = 0.001) in dialysis patients (pinteraction = 0.02). In a fully adjusted model, the aHR for the risk of 3-month mortality in kidney transplant recipients (vs. dialysis patients) was 1.39 (95% CI 1.02, 1.89, p = 0.04) in males and 2.04 (95% CI 1.40, 2.97, p interaction = 0.02). In patients on KFRT with COVID-19, the male sex is not a risk factor for mortality among kidney transplant recipients but remains a risk factor among dialysis patients. The use of immunosuppressants in kidney transplant recipients, among other factors, may have narrowed the difference in the immune response to COVID-19 between men and women, and therefore reduced the sex difference in COVID-19 mortality risk.

Medical detection dogs have potential to be used to screen asymptomatic patients in crowded areas at risk of epidemics such as the SARS-CoV-2 pandemic. However, the fact that SARS-CoV-2 detection dogs are in direct contact with infected people or materials raises important concerns due to the zoonotic potential of the virus. No study has yet recommended a safety protocol to ensure the health of SARS- CoV-2 detection dogs during training and working in public areas. This study sought to identify suitable decontamination methods to obtain nonpathogenic face mask samples while working with SARS-CoV-2 detection dogs and to investigate whether dogs were able to adapt themselves to other decontamination procedures once they were trained for a specific odor. The present study was designed as a four-phase study: (a) Method development, (b) Testing of decon- tamination methods, (c) Testing of training methodology, and (d) Real life scenario. Surgical face masks were used as scent samples. In total, 3 dogs were trained. The practical use of 3 different decontam- ination procedures (storage, heating, and UV-C light) while training SARS-CoV-2 detection dogs were tested. The dog trained for the task alerted to the samples inactivated by the storage method with a sensitivity of100 % and specificity of 98.28 %. In the last phase of this study, one dog of 2 dogs trained, alerted to the samples inactivated by the UV-C light with a sensitivity of 91.30% and specificity of 97.16% while the other dog detected the sample with a sensitivity of 96.00% and specificity of 97.65 %. Introduction: We aimed to study the characteristics of peritoneal dialysis (PD) patients with coronavirus disease-19 (COVID-19), determine the short-term mortality and other medical complications, and delineate the factors associated with COVID-19 outcome. Methods: In this multicenter national study, we included PD patients with confirmed COVID-19 from 27 centers. The baseline demographic, clinical, laboratory, and radiological data and outcomes at the end of the first month were recorded. Results: We enrolled 142 COVID-19 patients (median age: 52 years). 58.2% of patients had mild disease at diagnosis. Lung involvement was detected in 60.8% of patients. Eighty-three (58.4%) patients were hospitalized, 31 (21.8%) patients were admitted to intensive care unit and 24 needed mechanical ventilation. Fifteen (10.5%) patients were switched to hemodialysis and hemodiafiltration was performed for four (2.8%) patients. Persisting pulmonary symptoms (n = 27), lower respiratory system infection (n = 12), rehospitalization for any reason (n = 24), malnutrition (n = 6), hypervolemia (n = 13), peritonitis (n = 7), ultrafiltration failure (n = 7), and in PD modality change (n = 8) were reported in survivors. Twenty-six patients (18.31%) died in the first month of diagnosis. The non-survivor group was older, comorbidities were more prevalent. Fever, dyspnea, cough, serious-vital disease at presentation, bilateral pulmonary involvement, and pleural effusion were more frequent among non-survivors. Age (OR: 1.102; 95% CI: 1.032–1.117; p: 0.004), moderate-severe clinical disease at presentation (OR: 26.825; 95% CI: 4.578–157.172; p < 0.001), and baseline CRP (OR: 1.008; 95% CI; 1,000–1.016; p: 0.040) were associated with first-month mortality in multivariate analysis. Discussion/Conclusions: Early mortality rate and medical complications are quite high in PD patients with COVID-19. Age, clinical severity of COVID-19, and baseline CRP level are the independent parameters associated with mortality. [ABSTRACT FROM AUTHOR]

Objective: Blood culture are of vital importance in patient follow-up, as they enable the identification and production of sepsis causative microorganisms, initiate antibiotic treatment in a timely manner and reduce mortality and morbidity. In this study, it is aimed to evaluate the microorganisms grown in the automated blood culture in the microbiology laboratory of the hospital in terms of quality indicators. Methods: In this study, microorganisms grown from automated blood culture BACTEC-9120 (Becton Dickinson, USA) system from the blood culture samples sent to Duzce University Medical Microbiology Laboratory were evaluated retrospectively. For this purpose, the rejection and contamination rate of the samples for which blood culture was requested, the result of Gram staining-final identification compliance, the number of samples sent from a single bottle, and the growth times of microorganisms after incubation were determined. Results: 5037 blood culture samples were sent to the laboratory from various clinics. 1.7% of these samples were rejected as inappropriate samples. Gram stain-final identification compatibility of blood cultures was investigated and it was determined as 97.8%. The single bottle number of the samples sent was found to be 511. For the 5037 samples included in the study, growth was detected in 20.7%, of which 10.2% were considered as contaminants In our study, the average breeding time of the factors examined for breeding time was determined to be 30.29 hours. Conclusions: As conclusion, there is no gold standard to distinguish true pathogens from contaminant agents in blood cultures. WOS:000709040200011

Intravascular lymphomatosis (IVL) is a rare type of Bcell non-Hodgkin lymphoma (BCNHL), which can mimic many other diseases. Fever, neurological symptoms, and skin findings are the most frequent clinical findings. Intravascular lymphomatosis may be associated with genetic factors and infection with human immunodeficiency virus (HIV), human herpesvirus 8 (HHV-8), human T-lymphotropic virus 1 (HTLV1), Epstein-Barr virus (EBV), and hepatitis B virus (HBV). A 50-year-old man was hospitalized with recalcitrant hyponatremia of unknown cause. He had also telangiectatic, indurated, slightly erythematous plaques on his trunk for the last 10 days. His past medical history was unremarkable, although he was a carrier of hepatitis B. Multiple skin biopsies were performed and were considered to be diagnostic of IVL. The hyponatremia was unresponsive to water restriction and hypertonic solution support but it resolved with B cell directed chemotherapy. A final diagnosis was made as syndrome of inappropriate antidiuretic hormone (SIADH) in the setting of IVL. This case had a relatively early diagnosis with just 10- days of skin lesions. Intravascular lymphomatosis is a very rare disease and is usually difficult to diagnose. An even more uncommon presentation is IVL complicated by a paraneoplastic syndrome. There are prior reported cases of SIADH in the setting of IVL. However, this case underscores the importance of evaluating patients with SIADH for potential IVL. [ABSTRACT FROM AUTHOR]

End-stage kidney disease and advanced cirrhosis are sometimes seen concomitantly. There is no consensus on dialysis modality in terms of determining the optimal way of treating these patients. It has been suggested that peritoneal dialysis is a better choice for these patients, but efficacy of hemodialysis in stable cirrhotic patients has not been evaluated sufficiently. We report a case with advanced cirrhosis and end-stage kidney disease who was faced with hepatic encephalopathy episodes up on starting renal replacement therapy. The case is also interesting in that it reveals effects of hemodialysis and peritoneal dialysis on hepatic encephalopathy episodes and quality of life of the patient.

- Abstract
To investigate the causative agents involved in atypical community-acquired pneumonia (CAP) patients with lower respiratory tract infections. This prospective study (October 2014-August 2016) included 92 adult patients with pre-existing CAP. Serum samples were analyzed for IgG and IgM antibodies using the indirect immunofluorescence antibody (IFA) method. Causative agents associated with typical pneumonia were evaluated in simultaneously obtained sputum cultures. The IgM antibodies and/or the cultures of typical agents exhibited positivity for atypical CAP causative agents in 26 (28%) of the patients. Positive IgM antibodies were found for influenza B virus in five patients, parainfluenza serotype 2 virus in four, echovirus in four, parainfluenza serotype 1 virus in two, parainfluenza serotype 3 virus in two, influenza A (HlNl) virus in two, C. pneumoniae in two and H. influenzae in one. Thirteen (50%) of these patients had typical bacterial growth, while more than one agent was detected in five (20%). The rate of atypical causative agents other than L. pneumophila was quite high. Viruses were the main agents in the atypical CAP patients. WOS: 000264805800025 Corynebacterium species are an uncommon cause of breast abscess, and Corynebacterium amycolatum is rarely described in the literature. In the present case, we report recurrent breast abscess caused by C. amycolatum in a 39-year-old patient with normal immune function. Pathological diagnosis was granulomatous mastitis. The patient was treated with incision and drainage and ampicillin-sulbactam treatment. To our knowledge, this is the second report of breast abscess caused by C. amycolatum.

Background This study was conducted to evaluate the efficacy and safety of once-monthly continuous erythropoietin receptor activator (CERA) for maintenance of stable haemoglobin (Hb) levels in adult chronic renal anaemia patients on dialysis according to local clinical judgment in Turkey. Methods This was a prospective, open-label, single-arm, multi-centre study conducted in 20 centres in Turkey. After a 4-week screening period, eligible patients receiving conventional erythropoiesis-stimulating agents were converted to monthly intravenous CERA and entered a 16-week CERA dose-titration period (DTP) followed by an 8-week efficacy evaluation period (EEP) and a 4-week safety follow-up. The primary endpoint was the proportion of patients whose Hb concentration remained stable within ±1.0 g/dL of their reference Hb and within the range of 10.0–12.0 g/dL during the EEP. Results A total of 173 patients were screened, 132 entered the DTP and 84 completed the study. Thirty-nine patients [46.4% (95% confidence interval: 35.5–57.7%)] maintained stable target Hb concentrations. The mean change in time-adjusted average Hb concentration was 0.29 ± 1.08 g/dL between baseline and the EEP. The mean CERA monthly dose was 112.4 ± 76.78 µg during the EEP, and the CERA dose was adjusted in 39 patients (36.4%). Eleven patients (8.4%) reported 13 treatment-related adverse events, the most frequent adverse events being infections and infestations, gastrointestinal and vascular disorders. Conclusions Once-monthly CERA maintains stable Hb concentrations in chronic renal anaemia patients on dialysis in Turkey. The study results confirm the known efficacy and safety profile of CERA. Background: The hands of hospital personnel are considered to be important for colonization and infection of patients with Candida spp. The aim of this study was to evaluate the effectiveness of different hand disinfectants in reducing the carriage of Candida species on the hands of hospital personnel. Methods: A controlled study was conducted at Duzce University School of Medicine Hospital. Eighty hospital personnel were included in the trial. Subjects were divided into 4 groups according to hand hygiene procedures: group 1, hand rubbing with alcohol-based solution; group 2, hand washing with 4% chlorhexidine gluconate; group 3, hand washing with 7.5% povidone-iodine; group 4, hand washing with plain soap and water. The hands of all participants were tested by culture with the broth wash technique. Results: Hand carriage of Candida spp. was lower in the 4% chlorhexidine gluconate group (10.5%, p = 0.006), in the 7.5% povidone-iodine group (18.7%, p = 0.043), and in the alcohol-based hand rub group (21.1%, p = 0.048) compared to the group washing hands with plain soap and water (50%). Conclusions: The use of hand disinfectant containing antimicrobial agents is more effective than hand washing with water and soap in reducing carriage of Candida on the hands of hospital personnel. It is recommended that hospital personnel use an antimicrobial hand disinfectant in units where there is a high risk of Candida infection. [ABSTRACT FROM AUTHOR]

Aim: Celiac disease (CD) is considered to be a risk factor for chronic kidney disease (CKD) but there is no study determining the prevalence of CD, among patients with CKD. We aim to determine the prevalence of CD in patients with CKD. Materials and methods: Anti-endomysial IgA (EMA) antibody was screened in patients with CKD (glomerular filtration rate <60 mL/min). Patients who were EMA positive underwent upper gastrointestinal system endoscopy and intestinal biopsy for confirmation of definite diagnosis for CD. Results: Two hundred and ninety-two patients (161 males, mean age was 47.3 ± 16.3 years) with CKD were included. The EMA testing was positive in 10 patients (6F/4M). Of these, eight underwent upper gastrointestinal endoscopy and biopsies, two of them rejected endoscopy. Biopsy specimen of one of the patients was not appropriate for histopathological examination. Specimens of remaining cases (4F/3M) were compatible with CD on histopathological examination. The EMA-positive CKD patients were 3.42% (1/29 cases) and frequency of CD was 2.39% (1/42 cases). Frequency of CD was 3.1% in females and 1.85% in males. Female/male ratio was 1.67. We did not find statistically significant difference between two groups according to age and gender. Apparent chronic gastrointestinal symptoms such as abdominal pain, distension, constipation, dyspepsia, and diarrhea were absent in patients diagnosed with CD. Differences between some laboratory parameters (such as complete blood count, albumin, calcium, phosphate, total cholesterol, ferritin, parathormone) of CD and non-CD patients were not significant statistically. Conclusion: Our results showed increased frequency of CD among patients with CKD and screening for CD in CKD population can be helpful. [ABSTRACT FROM AUTHOR]
