The aims of this study, in a sample of outpatients with COPD, were to (i) assess the prevalence of physical frailty using the Japanese version of the Cardiovascular Health Study (J-CHS) criteria and the Short Physical Performance Battery (SPPB) and the degree of agreement between the findings of the two assessments and (ii) identify factors associated with the disparity in the results obtained with these instruments.Patients and Methods: This was a multicenter cross-sectional study of individuals with stable COPD enrolled in four institutions. Frailty was assessed using the J-CHS criteria and the SPPB. Weighted Cohen’s kappa (k) statistic was performed to investigate the magnitude of agreement between the instruments. We divided participants into two groups depending on whether there was agreement or non-agreement between the results of the two frailty assessments. The two groups were then compared with respect to their clinical data.Results: A total of 103 participants (81 male) were included in the analysis. The median age and FEV1 (%predicted) were 77 years and 62%, respectively. The prevalence of frailty and pre-frail was 21% and 56% with the J-CHS criteria and 10% and 17% with the SPPB. The degree of agreement was fair (k = 0.36 [95% CI: 0.22– 0.50], P< 0.001). There were no significant differences in the clinical characteristics between the agreement group (n = 44) and the non-agreement group (n = 59).Conclusion: We showed that the degree of agreement was fair with the J-CHS criteria detecting a higher prevalence than the SPPB. Our findings suggest that the J-CHS criteria may be useful in people with COPD with the aim of providing interventions to reverse frailty in the early stages.Keywords: cardiovascular health study, Short Physical Performance Battery, chronic lung disease, physical function

This paper investigates the impact of four drivers of Governance diversity, namely gender, tenure, age, and educational attainment, on strategic direction and variation. It then incorporates corporate financial results as a moderating variable, testing how it impacts the links between board diversity and strategic variation. Strategic variation or change is assessed based on measuring deviation from past strategies. Our sample consists of 5011 firm‐year observations from 930 firms in the United States between 2010 and 2018. The findings indicate that the four drivers of Governance diversity show a positive relationship between the corporate board of directors and strategic variation/change. However, the strength of the effect depends on overall firm results/performance. [ABSTRACT FROM AUTHOR]

The common age for boys to have OSD is between 12 and 15 years and for girls, between 8 and 12 years. Radiographic studies are helpful in diagnosis and treatment of OSD.Purpose: We examine the age at onset of OSD in detail and investigate the relationship between clinical findings, radiographic bone morphology, and the severity of OSD in adolescents.Results: The average age at onset of knee pain was 12 years and 6 months – 12 years and 9 months in boys, and 12 years and 1 month in girls. Boys were significantly older than girls at onset. In addition, there were significant relationships between duration from first onset to visit to the clinic, radiographic bone stage, body morphology, and radiographic severity. The patients who delayed their visit to the clinic from the first onset of pain and who were older showed a later bone stage and more radiographic severity grade of OSD. There was significant differences concerning weight and body mass index between severity grade I and III. Conclusion: For the age at the onset of OSD, the mean age of boys was significantly older than that of girls. The patients at a later bony stage had a higher severity grade. The boys and girls with OSD who had less weight or body mass index showed less severity.Keywords: Osgood–Schlatter disease, radiographic classification, tibial tuberosity, body morphology

The stability with respect to a peeling–ballooning mode (PBM) was investigated numerically with extended MHD simulation codes in JET, JT-60U and future JT-60SA plasmas. The MINERVA-DI code was used to analyze the linear stability, including the effects of rotation and ion diamagnetic drift ( *w i), in JET-ILW and JT-60SA plasmas, and the JOREK code was used to simulate nonlinear dynamics with rotation, viscosity and resistivity in JT-60U plasmas. It was validated quantitatively that the ELM trigger condition in JET-ILW plasmas can be reasonably explained by taking into account both the rotation and *w i effects in the numerical analysis. When deuterium poloidal rotation is evaluated based on neoclassical theory, an increase in the effective charge of plasma destabilizes the PBM because of an acceleration of rotation and a decrease in *w i. The difference in the amount of ELM energy loss in JT-60U plasmas rotating in opposite directions was reproduced qualitatively with JOREK. By comparing the ELM affected areas with linear eigenfunctions, it was confirmed that the difference in the linear stability property, due not to the rotation direction but to the plasma density profile, is thought to be responsible for changing the ELM energy loss just after the ELM crash. A predictive study to determine the pedestal profiles in JT-60SA was performed by updating the EPED1 model to include the rotation and *w i effects in the PBM stability analysis. It was shown that the plasma rotation predicted with the neoclassical toroidal viscosity degrades the pedestal performance by about 10% by destabilizing the PBM, but the pressure pedestal height will be high enough to achieve the target parameters required for the ITER-like shape inductive scenario in JT-60SA. Spatial non-uniformity of the dissociative hydrogen atom (H0) production has been investigated in a large negative ion source (JAEA 10 A source) with the electron energy distribution function (EEDF) obtained by a Monte-Carlo simulation code for electron transport in 3D3V (three dimensional real and velocity) space. It has been shown that the H0 production rate becomes larger in the upper region (one side in the longitudinal direction) of the source chamber. This spatial non-uniformity of the H0 production profile is mainly explained by the non-equilibrium features of the EEDF in the upper region, i.e., the EEDF consists of thermal electron component with kinetic energy [variant_greek_epsilon] < 25 eV and fast electron component with energy [variant_greek_epsilon] > 25 eV in the upper region, while the EEDF mainly consists of only thermal electrons in the bottom region. These characteristics for the EEDF and the energy dependence of cross-sections for dissociation and dissociative ionization processes lead to the non-uniform profile of the H0 production. The above numerical results of the spatial H0 non-uniformity are validated and confirmed by comparisons with those by spectroscopic measurement. It has been clarified that the non-equilibrium (fast electron) component of the EEDF has a large contribution to the non-uniformity of the H0 production rate. [ABSTRACT FROM AUTHOR]

The production and transport processes of H
0 atoms are numerically simulated using a three-dimensional Monte Carlo transport code. The code is applied to the large JAEA 10 ampere negative ion source under the Cs-seeded condition to obtain a spatial distribution of surface-produced H- ions. In this analysis, the amount of H0 atoms produced through dissociation processes of H2 molecules is calculated from the electron temperature and density obtained by Langmuir probe measurements. The high-energy tail of electrons, which greatly affects H0 atom production, is taken into account by fitting a single-probe characteristic as a two-temperature Maxwellian distribution. In the H0 atom transport process, the energy relaxation of the H0 atoms, which affects the surface H- ion production rate, is taken into account. The result indicates that the surface H- ion production is enhanced near the high-electron-temperature region where H0 atom production is localized. [ABSTRACT FROM AUTHOR]- Published

In multi-beamlet negative ion accelerators for fusion applications, the required transverse magnetic field for the suppression of co-extracted electrons is generated by permanent magnets arranged in parallel arrays, embedded in the Extraction Grid. However, such configuration also produces an undesired "criss-cross" deflection of ion trajectories, which can downgrade the overall beam optics. This deflection is traditionally counterbalanced by electrostatic means, i.e. by introducing a small offset in the axis of the grid apertures. During the final design of MITICA, which is the prototype of the ITER Heating Neutral Beam Injector presently under construction in Padova [1], an innovative magnetic configuration called ADCM (Asymmetric Deflection Compensation Magnets) has been conceived [2] and adopted, offering several advantages in terms of operational flexibility compared to the traditional solution. In the framework of a research collaboration between RFX and QST, the new concept has been experimentally tested for the first time in 2016 by a joint RFX-QST team. A new ITER-like [3] Extraction Grid (designed and built at RFX) was mounted on the existing NITS device at the QST lab in Naka. For direct comparison, half of the Extraction Grid was provided with the new ADCM configuration, whereas the rest of the grid had standard magnets with no compensation. A target made of monoaxial carbon-carbon fibre composite and an infrared camera were used for accurate beam optics measurement. The results obtained have confirmed the effectiveness of the ADCM concept for the compensation of the criss-cross ion deflection. Some discrepancies in the scaling of ion deflection with respect to 3D simulations were also evidenced and investigated. The paper presents the main results obtained during the experimental campaign on NITS at QST, which lasted in total six weeks, together with a discussion of the first data analysis results.

- Abstract
The ITER Heating Neutral Beam Injector (HNB) is designed to deliver 16.5MW of additional power to the plasma with pulse duration of one hour. The beam acceleration is obtained by a multistage accelerator composed of five acceleration grids that are powered by the so-called Acceleration Grid Power Supply (AGPS). The AGPS feeds around 56MW at 1MV dc to the acceleration grids in quasi-steady state and is able to interrupt the power delivery in some tens of microseconds in case of grid breakdown. The operation of the AGPS will be controlled by a dedicated control system, the AGPS Local Control (AGPS-LC). The AGPS-LC will be in charge of coordinating the operation and interlocks of the AGPS, putting in place all necessary actions to protect the AGPS from internal and external faults and limit the amount of energy flowing from the power supply to the grids in case of breakdowns. The AGPS-LC will be interfaced to a central control devoted to the operation of the Neutral Beam Injector and to a central interlock system in charge of coordinating interlocks and protection functions of the overall plant. The coordination of actions and their time synchronization is of paramount importance to guarantee a reliable operation. The complexity of the system and the different operational scenarios require an in-depth analysis to identify the most suitable control and protection strategy during faults and breakdowns. Such analysis is based on the existing experience of the JT-60U negative ion HNB system and on dedicated simulations performed on a detailed model of the power supplies. This paper will present a strategy for the coordination of the AGPS integrated protection during faults and breakdowns. The operational scenario has been simulated by using software tools for circuit analysis and a proposal for the design of the AGPS-LC has been derived.

We herein report an unusual case of Brodie's abscess of the radius in a child. A 13-year-old boy presented with pain on his right distal forearm. A plain radiograph showed an 8 cm translucent lesion in the distal radius. MRI showed a penumbra sign on the T1-weighted image, hyperintensity on T2-weighted images, and ring enhancement on the contrast-enhanced T1 image.
18 F-FDG PET/CT images showed an uptake at the margin of the radius. Curettage and iliac cancellous bone grafting were undertaken for Brodie's abscess. Bacteriological examinations were found to be negative, however, the pathologic diagnosis showed chronic osteomyelitis. Eight months after surgery, the patient was asymptomatic and there was no sign of recurrence of infection. For Brodie's abscess in a child, thorough debridement is mandatory in addition to cancellous bone grafting. Brodie's abscess should be considered in the differential diagnosis of a patient who presents with forearm pain and exhibit the radiolucent osteolytic lesion on simple radiography. To realize dc 1 MV power supply system for neutral beam injector in ITER, long-pulse dc ultrahigh-voltage (HV) insulation technologies have been developed. For a dc 1 MV insulating transformer, a composite bushing has been newly developed to overcome the manufacturing difficulty of a conventional condenser bushing (CB). The composite bushing has a small-sized CB and fiber reinforced plastic tube to sustain 1 MV stably inside and outside of the bushing. The mock-up of the insulating transformer has fulfilled the ITER requirement of dc 1.2 MV insulation for 3600 s. An HV bushing to transmit multiple voltages simultaneously has also been developed through a careful experimental study on vacuum insulation characteristics with multiple gaps between large electrodes. It has been found that the voltage holding is dominated not by number of the electrodes but by a total cathode surface area of electrodes. Based on this result, the vacuum insulation inside the HV bushing with five electrostatic screens has been designed to realize a stable vacuum insulation of 1 MV. [ABSTRACT FROM AUTHOR]

Under cyclic loading, we recorded the fatigue strength of a six-strand tendon repair with different symmetry in the lengths of suture purchase in two stumps of 120 dental rolls and in 30 porcine tendons. First, the strengths of the repairs with 1, 2, 3, 4 and 5 mm asymmetry were screened using the dental rolls. The asymmetric core suture repairs were then made with a Kessler repair of equal suture purchase (10 mm) in two tendon stumps, and shifting two other Kessler repairs by 1, 3 or 5 mm, respectively, along the longitudinal axis of the tendon in relation to the first (symmetric) Kessler repair. The core repairs with 3 mm or more asymmetry in suture purchases in two tendon ends showed significantly greater fatigue strength and significantly smaller gaps compared with 1 mm asymmetry in core suture repair. Our results support that asymmetric placement of core sutures in two tendon ends favour resisting gapping at the repair site and 3 mm or more asymmetry is needed to produce such beneficial effects.
