Background Many surgeons routinely place intraperitoneal drains after elective colorectal surgery. However, enhanced recovery after surgery guidelines recommend against their routine use owing to a lack of clear clinical benefit. This study aimed to describe international variation in intraperitoneal drain placement and the safety of this practice. Methods COMPASS (COMPlicAted intra-abdominal collectionS after colorectal Surgery) was a prospective, international, cohort study which enrolled consecutive adults undergoing elective colorectal surgery (February to March 2020). The primary outcome was the rate of intraperitoneal drain placement. Secondary outcomes included: rate and time to diagnosis of postoperative intraperitoneal collections; rate of surgical site infections (SSIs); time to discharge; and 30-day major postoperative complications (Clavien–Dindo grade at least III). After propensity score matching, multivariable logistic regression and Cox proportional hazards regression were used to estimate the independent association of the secondary outcomes with drain placement. Results Overall, 1805 patients from 22 countries were included (798 women, 44.2 per cent; median age 67.0 years). The drain insertion rate was 51.9 per cent (937 patients). After matching, drains were not associated with reduced rates (odds ratio (OR) 1.33, 95 per cent c.i. 0.79 to 2.23; P = 0.287) or earlier detection (hazard ratio (HR) 0.87, 0.33 to 2.31; P = 0.780) of collections. Although not associated with worse major postoperative complications (OR 1.09, 0.68 to 1.75; P = 0.709), drains were associated with delayed hospital discharge (HR 0.58, 0.52 to 0.66; P < 0.001) and an increased risk of SSIs (OR 2.47, 1.50 to 4.05; P < 0.001). Conclusion Intraperitoneal drain placement after elective colorectal surgery is not associated with earlier detection of postoperative collections, but prolongs hospital stay and increases SSI risk.

The applicability of molybdenum-containing catalysts based on SBA-15, which were synthesized in one stage by a co-condensation method, to the oxidation processes of various organosulfur compounds was demonstrated for the first time. The composition and structure of the obtained materials were confirmed using a set of modern physicochemical methods, such as low-temperature nitrogen adsorption/desorption, X-ray spectral fluorescence analysis, X-ray diffraction (XRD) analysis, TEM, and IR spectroscopy. The effects of the amount of the active phase and the process conditions on the oxidation of a model mixture of dibenzothiophenes were studied. It was shown that the resulting catalyst worked at least in four cycles without significant loss of activity. A sample containing 5 wt % molybdenum made it possible to achieve a dibenzothiophene conversion of 100% in 5 min under the following conditions: 0.5 wt % of a 5% Mo-SBA-15 catalyst, the molar ratio H2O2 : S = 6 : 1, and T = 80°C. 15. Hollow microneedle array fabrication using a rational design to prevent skin clogging in transdermal drug delivery.

- Abstract
Microneedle (MN) technology is promising to replace hypodermic needles for practical use and painless drug delivery. However, the complex top-down fabrication process of functional MN arrays is a bottleneck that hinders their widespread use. Here, we fabricate the tapered hollow MN array using a unique bi-level-tip by combining strain-engineering and capillary self-assembly of carbon nanotube (CNT) microstructures. Strain-engineering facilitated by the offset pattern of the catalyst enables the growth of bent, bi-level CNT microstructures while capillary self-assembly helps in constituting the tapered geometry of MNs. The bottom-up fabrication that consists of only two standard photolithography steps and CNT growth to form the scaffold of MNs followed by a polymer (polyimide) reinforcement step to impart mechanical stiffness to MNs provides scalable and fewer processing steps. The tapered shape of the MN allows an 8 times smaller force to pierce and penetrate the skin compared to the straight MN. The liquid delivery rate of the bi-level-tip MN is measured to be 26% better than the flat tip MN of the same lumen size as its geometry reduces skin clogging effect at the needle tip. In addition, cytotoxicity tests verify that the polyimide reinforced CNT-MNs are biocompatible for future in vivo applications. [ABSTRACT FROM AUTHOR]

Context. Insulin resistance has been detected in a majority of patients with polycystic ovary syndrome (PCOS). Elevated neprilysin levels are associated with insulin resistance. Objective. The present study aims to investigate plasma neprilysin and its relationship with endocrine and metabolic characteristics in patients with PCOS. Subjects and Methods. Thirty-five premenopausal PCOS patients and 35 healthy volunteers of similar age were included in the study. Demographic characteristics, biochemical and hormonal findings and also plasma neprilysin levels were determined in these patients and healthy controls. Results. In our study, HOMA-IR values were significantly higher in PCOS patients (3.3 ± 1.8) compared with the controls [(1.6 ± 1), p<0.01]. Plasma neprilysin levels were significantly higher in the PCOS group compared to the control group (1502.1 ± 1641.2 vs. 764.6 ± 562.6 pg/mL). There was no difference in plasma neprilysin levels when PCOS patients were classified as overweight-obesity (BMI≥25kg/m²) or non-obesity (BMI<25kg/m²). Conclusion. Our findings revealed significantly higher levels for plasma neprilysin and HOMA-IR values in PCOS patients when compared to controls. No significant differences were noted between obese PCOS patients and non-obese PCOS patients in terms of plasma neprilysin levels. [ABSTRACT FROM AUTHOR]

- Abstract
Background: Obesity is the main obstacle for metabolic control in patients with type 2 diabetes. Turkey has the highest prevalence of obesity and type 2 diabetes in Europe. The effect of obesity on the metabolic control, and the macro-and microvascular complications of patients are not apparent. Objectives: This nationwide survey aimed to investigate the prevalence of overweight and obesity among patients with type 2 diabetes and to search for the impact of obesity on the metabolic control of these patients. We also investigated the independent associates of obesity in patients with type 2 diabetes. Methods: We consecutively enrolled patients who were under follow-up for at least 1 year in 69 tertiary healthcare units in 37 cities. The demographic, anthropometric, and clinical data including medications were recorded. Patients were excluded if they were pregnant, younger than 18 years, had decompensated liver disease, psychiatric disorders interfering with cognition or compliance, had bariatric surgery, or were undergoing renal replacement therapy. Results: Only 10% of patients with type 2 diabetes (n = 4,648) had normal body mass indexes (BMI), while the others were affected by overweight (31%) or obesity (59%). Women had a significantly higher prevalence of obesity (53.4 vs. 40%) and severe obesity (16.6 vs. 3.3%). Significant associations were present between high BMI levels and lower education levels, intake of insulin, antihypertensives and statins, poor metabolic control, or the presence of microvascular complications. Age, gender, level of education, smoking, and physical inactivity were the independent associates of obesity in patients with type 2 diabetes. Conclusion: The TEMD Obesity Study shows that obesity is a major determinant of the poor metabolic control in patients with type 2 diabetes. These results underline the importance of prevention and management of obesity to improve health care in patients with type 2 diabetes. Also, the results point out the independent sociodemographic and clinical associates of obesity, which should be the prior targets to overcome, in the national fight with obesity. Background: Persistent hypercalcemia and hyperparathyroidism after successful kidney transplantation can be detrimental in some recipients and should be ameliorated. Objective: To point out the concerns regarding resistance to cinacalcet in kidney transplant recipients with persistent hypercalcemia. Serum creatinine, estimated glomerular filtration rate (eGFR), calcium, phosphorus, and intact parathyroid hormone (PTH) levels at the baseline and throughout the treatment, and ultrasonography and parathyroid scintigraphy findings were recorded. Results: Cinacalcet treatment was initiated after a mean±SD of 20.7±19.7 months of transplantation and maintained for 16.9±7.9 months. Serum calcium levels were significantly decreased with the cinacalcet treatment. There were no significant changes in serum creatinine, eGFR, phosphorus, and PTH levels. In all participants, serum calcium levels were increased from 9.8±0.6 to 11.1±0.6 mg/dL (p<0.001) within 1 month of cessation of cinacalcet. 7 recipients with adenoma-like hyperplastic glands underwent parathyroidectomy (PTx) due to failure with cinacalcet. Conclusion: Cinacalcet may be an appropriate treatment for a group of recipients with hypercalcemia without adenoma-like hyperplastic glands or who had a contraindication for surgery. Recipients with enlarged parathyroid gland may resist to cinacalcet-induced decrease in serum PTH, although the concomitant hypercalcemia may be corrected. [ABSTRACT FROM AUTHOR]

DNA polymerases exhibit a surprising tolerance for analogs of deoxyribonucleoside triphosphates (dNTPs), despite the enzymes' highly evolved mechanisms for the specific recognition and discrimination of native dNTPs. Here, individual DNA polymerase I Klenow fragment (KF) molecules were tethered to a single-walled carbon nanotube field-effect transistor (SWCNT-FET) to investigate accommodation of dNTP analogs with single-molecule resolution. Each base incorporation accompanied a change in current with its duration defined by τclosed. Under Vmax conditions, the average time of τclosed was similar for all analog and native dNTPs (0.2 to 0.4 ms), indicating no kinetic impact on this step due to analog structure. Accordingly, the average rates of dNTP analog incorporation were largely determined by durations with no change in current defined by τopen, which includes molecular recognition of the incoming dNTP. All α-thio-dNTPs were incorporated more slowly, at 40 to 65% of the rate for the corresponding native dNTPs. During polymerization with 6-Cl-2APTP, 2-thio-dTTP, or 2-thio-dCTP, the nanocircuit uncovered an alternative conformation represented by positive current excursions that does not occur with native dNTPs. A model consistent with these results invokes rotations by the enzyme's O-helix; this motion can test the stability of nascent base pairs using nonhydrophilic interactions and is allosterically coupled to charged residues near the site of SWCNT attachment. This model with two opposing O-helix motions differs from the previous report in which all current excursions were solely attributed to global enzyme closure and covalent-bond formation. The results suggest the enzyme applies a dynamic stability-checking mechanism for each nascent base pair.

Ionizing radiation is known to be one of the most potent cataractogenic agents. We aimed to evaluate two different radiotherapy techniques with respect to the doses received by the organs at risk (OAR) in patients with brain metastasis who undergone WBRT. Materials and Methods: Ten consecutive patients with brain metastasis were included. For each patient, two different treatment plans were created for whole brain. Helmet-field (HF) (anterior border was 2 cm posterior to lens, inferior border was the bottom of C2 vertebra) and classical technique with collimation (CT) (anterior border was defined as skin fall off, inferior border was the bottom of cranial base) were generated for all patients. Two techniques were compared with respect to the doses received by the OAR including bilateral lenses, optic nerves and eye-balls, the dose homogeneity index (DHI), and the monitor unit counts (MU) required for the treatment. Student-t test was used for statistical analysis. Results: There was no difference between two techniques in terms of both DHI (p: 0.182) and MU counts (p: 0.167). The maximum and mean doses received by the right lens, left lens and right eye-ball were significantly reduced with CT (p values for maximum doses 0.007, 0.012 and 0.010; for median doses 0.027, 0.046 and 0.002 respectively). Conclusion: CT was found to be more advantageous, with respect to the lens doses in addition the dose received by the right eye-ball during WBRT.

Objective: To evaluate dysphagia and manometric changes in the upper oesophageal sphincter in patients with unilateral vocal fold paralysis. Methods: Thirty patients with unilateral vocal fold paralysis due to vagal nerve paralysis scheduled for evaluation were enrolled in the study group; 24 healthy subjects were included in the control group. Upper oesophageal sphincter basal and residual pressure, relaxation time, and pharyngeal pressure values were evaluated by manometry. All patients completed the Turkish Eating Assessment Tool 10 questionnaire, the MD Anderson dysphagia questionnaire and the reflux symptom index form. Results: Swallowing assessment questionnaires and reflux symptom index results were significantly higher in the study group. Upper oesophageal sphincter basal and relaxation pressures were lower in the study group. Upper oesophageal sphincter relaxation time was shorter in the study group, but pressure values recorded from the pharynx were higher. Conclusion: Upper oesophageal sphincter manometric pressure was lower in patients with unilateral vocal fold paralysis. A hypotonic sphincter likely contributes to dysphagia and aspiration. Endogenous hyperinsulinemia was found in a 38-year-old female patient with recurrent neuroglycopenic symptoms (the glucose level was 25 mg/dL, insulin 43.9 µ/mL, C-peptide 5.54 ng/mL). No lesion was found on imaging tests including enhanced computed tomography (CT) methods performed with a preliminary diagnosis of insulinoma. A suspicious hyperperfusion was present in the pancreatic tail on the perfusion CT examination performed after obtaining approval. The selective arterial calcium stimulation test (SACST) result was consistent with a diffuse disease in the body and tail. The patient underwent partial (75%) pancreatectomy and is now followed up as a diabetes patient on intensive insulin treatment at the postoperative 38th month. The most common cause of endogenous hyperglycemia in hypoglycemia patients is insulinoma. The noninsulinoma pancreatogenous hypoglycemia syndrome (NIPHS) is rarely seen in the adult age group. Although invasive, SACST seems to be the most suitable test to differentiate diffuse or multiple disease from insulinoma and to guide the surgery when advanced radiological imaging methods are inadequate to detect the presence of insulinoma. Regarding perfusion CT, it would be more appropriate to wait for comparative data to be put forward in a more consistent manner. When no response can be obtained to medical treatment in NIPHS, partial/total pancreatectomy is the most appropriate treatment option as it enables recovery from the hypoglycemic episodes despite leading to a significant condition like diabetes. Kelvin probe force microscopy (KPFM) should be a key tool for characterizing the device physics of nanoscale electronics because it can directly image electrostatic potentials. In practice, though, distant connective electrodes interfere with accurate KPFM potential measurements and compromise its applicability. A parameterized KPFM technique described here determines these influences empirically during imaging, so that accurate potential profiles can be deduced from arbitrary device geometries without additional modeling. The technique is demonstrated on current-carrying single-walled carbon nanotubes (SWNTs), directly resolving average resistances per unit length of 70 kΩ/μm in semimetallic SWNTs and 200 kΩ/μm in semiconducting SWNTs. We evaluated the morbidity, complications and outcomes in 42 patients who underwent abdominal paravaginal defect repair (PDR) for vaginal wall prolapse due to a paravaginal defect and the prolapse of other pelvic organs. Out of a total of the 42 patients, 32 underwent PDR and concomitant pelvic reconstructive procedures, and 10 patients underwent PDR plus anti-incontinence surgery and concomitant pelvic reconstructive procedures. The mean patient age was 44.5 years. The mean follow-up period was 40 months. One bladder injury and two hemorrhages occurred intraoperatively. The cure rate of anterior vaginal wall prolapse was 92.9%. Of the 20 patients with urodynamic stress incontinence (USI) who underwent PDR alone, the rate of recurrence of USI was 20%; however, there was no recurrence in the 10 patients who underwent PDR plus the anti-incontinence procedures. Paravaginal repair is a safe and effective procedure for the surgical correction of a paravaginal defect, but has limited applicability in the surgical correction of USI. [ABSTRACT FROM AUTHOR]

The effect of low temperature plasma nitriding on wear resistance of Ti6Al4V alloy were investigated. There have been several studies to investigate the low temperature plasma nitriding on Ti6Al4V alloy. Plasma nitriding processes under gas mixture of N2/H2 = 3 were performed at temperature 535°C for duration of 4, 8 and 12 h. Adhesive wear tests were carried out by using a tribometer in block-on-ring configuration (ASTM G77), in sliding conditions, without lubricants and in air. Surface hardness of the plasma nitrided samples were measured by a Vickers hardness tester machine. Scanning electron microscopy studies were conducted to understand the wear mechanisms involved during the adhesive wear. Wear rate was calculated using weight loss per unit sliding distance. It was found that the wear resistance and surface hardness of the alloy improved considerably after plasma nitriding process. The wear resistance of the plasma nitrided samples were higher than of the unnitrided samples. Extension of nitriding times from 4 h to 12 h in the Ti6Al4V alloy improved remerkably the wear resistance and surface hardness. [ABSTRACT FROM AUTHOR]
