INTRODUCTION: Penetrating keratoplasty (PK) is one of the most common surgical procedure in corneal transplantation worldwide. Graft failure and rejection risk progressively increases with the increasing number of quadrants with corneal neovascularization (CNV). Optical coherence tomography angiography (OCTA) is a noninvasive imaging technique that has been widely used to visualize vascular abnormalities in the retina and some studies have shown its potential use in the anterior segment (AS) of the eye. The purpose of this study was to investigate the potential of OCTA technology to image and describe quantitatively CNV in eyes submitted to PK. MATERIAL AND METHODS: A cross-sectional study was performed, including 20 eyes from 18 patients submitted to PK at least 5 years before and with no history of graft rejection. All eyes underwent anterior segment slit-lamp photography (SLP) and OCTA with en face, b-scans and c-scans imaging. The vessel density (VD) was analyzed in the inferior, nasal and temporal corneal margin in all patients. The measurements were calculated after binarization with ImageJsoftware, using OCTA scans with 6 × 6 mm in a depth of 800 μm. RESULTS: The mean age was 59 years-old and most patients were submitted to PK due to corneal leucoma, followed by keratoconus, and a few had Fuchs endothelial corneal dystrophy and bullous keratopathy. The mean total VD was 50.16% and it was higher in the temporal quadrant and lower in the inferior one. However, there were no statistically significant differences between the 3 analyzed areas (p=0.801) or between each area and the other two and there was nocorrelation between the areas. OCTA was able to identify abnormal vessels when SLP apparently showed no abnormal vessels; OCTA was able to distinguish between larger and smaller vessels; OCTA scans allowed the investigation of several corneal planes. CONCLUSION: OCTA can become a new method for monitoring corneal diseases. It may allow the qualitative and quantitative follow-up of patients submitted to PK over time and to detect the appearance of CNV earlier than through SLP. OCTA applied to the anterior segment has promising and valuable features., INTRODUÇÃO: A queratoplastia penetrante (QP) é um dos procedimentos cirúrgicos mais realizados na transplantação corneana mundialmente. A falência do enxerto e o risco de rejeição aumentam progressivamente com o aumento do número de quadrantes com neovascularização corneana (NVC). A angiografia por tomografia de coerência ótica (OCTA) é um exame não invasivo que tem sido largamente utilizado na visualização de anomalias vasculares da retina e algunsestudos demonstraram a sua potencial utilização no segmento anterior do olho. O objetivo deste estudo foi investigar o potencial da tecnologia da OCTA na aquisição de imagens e descrição quantitativa da NVC em olhos submetidos a QP. MATERIAL E MÉTODOS: Estudo transversal, com 20 olhos de 18 doentes submetidos a QP pelo menos 5 anos antes e sem história de rejeição do transplante. A todos os olhos foi realizada uma fotografia do segmento anterior na lâmpada de fenda (LF) e uma aquisição de imagens en face, b-scans e c-scans da OCTA. A densidade vascular (DV) foi analisada na margem corneana inferior, nasal e temporal em todos os doentes. A medição da DV foi obtida após criação de imagens binárias com o software ImageJ, usando imagens da OCTA com 6 × 6 mm numa profundidade de 800 μm. RESULTADOS: A idade média dos doentes foi de 59 anos e a maioria foi submetido a QP por leucoma corneano, seguido de queratocone. A DV total média foi de 50,16% e foi mais alta no quadrante temporal e mais baixa no quadrante inferior. No entanto, não se verificou uma diferença estatisticamente significativa entre as 3 áreas analisadas (p=0,801) ou entre cada área e as duas restantes e não se verificou uma correlação entre as áreas. A OCTA foi capaz de identificar vasos anormais quando as fotografias na LF não os identificavam; a OCTA foi capaz de distinguir entre vasos de grande e pequeno calibre; a OCTA permitiu a investigação em diferentes planos corneanos. CONCLUSÃO: A OCTA poderá tornar-se um novo método para a monitorização de doenças corneanas e permitir o seguimento qualitativo e quantitativo de pacientes submetidos a QP, com a deteção precoce de NVC.

INTRODUCTION : When the ophthalmologist evaluates a patient in the emergency department with a periorbital edema, the first approach should be directed to the distinction between a periorbital/preseptal and an orbital/postseptal cellulitis. The objective examination presents valuable sign that can help in this differentiation. The main causes of cellulitis are bacterial infections, but noninfectious causes can also lead to it. The main purpose of this work is to present 4 clinical cases of patients with the same clinical complaint but whose diagnosis, treatment and follow-up were substantially different. CASE REPORTS: Two of the clinical cases presented patients diagnosed with a preseptal cellulitis, the first caused by an odontogenic abscess and the other caused by a Kerion celsi (a fungal infection). The last two cases presented patients diagnosed with a postseptal cellulitis caused by a frontoethmoidal mucopyocele and a squamous cell carcinoma of the maxillary sinus. The four patients had different clinical presentation and the medical and/or surgical approach varied among them. DISCUSSION: Clinical presentation of a preseptal and a postseptal cellulitis varies and these series of cases demonstrate these semiological differences. With them, we demonstrate the variability of diseases that may be associated and present with a periorbital edema. We also enhance the importance of a correct etiologic diagnosis to a correct therapeutic approach and of a multidisciplinary approach of these patients. It is crucial for the ophthalmologist to be aware of all signs and symptoms that the patient presents with, through a complete clinical history and a detailed objective evaluation, allowing the identification of the potentially severe cases, in which an orbital invasion might exist, leading to a correct orientation and proper treatment., INTRODUÇÃO: Quando o oftalmologista avalia um doente no Serviço de Urgência com edema periorbitário, a primeira abordagem deve ser direcionada para a distinção entre celulite periorbital/pré-septal e celulite orbitária/pós-septal. O exame objetivo apresenta achados valiosos que podem auxiliar nessa diferenciação. As principais causas da celulite são infecções bacterianas, mas causas não infecciosas também podem levar a ela. O principal objetivo deste trabalho é apresentar 4 casos clínicos de doentes com a mesma queixa clínica mas cujo diagnóstico, tratamento e seguimento foram substancialmente diferentes. CASOS CLÍNICOS: Dois dos casos clínicos apresentavam doentes diagnosticados com celulite pré-septal, o primeiro causado por um abscesso odontogénico e o outro causado por um Kerion celsi (uma infecção fúngica). Os dois últimos casos apresentavam pacientes com diagnóstico de celulite pós-septal causada por mucopiocele frontoetmoidal e carcinoma espinocelular de seio maxilar. Os quatro pacientes tiveram apresentação clínica diferente e a abordagem médica e/ou cirúrgica variou entre eles. CONCLUSÃO: A apresentação clínica de uma celulite pré-septal e pós-septal varia e esta série de casos demonstram essas diferenças semiológicas. Demonstramos a variabilidade de doenças que podem estar associadas e apresentar edema periorbitário. Salientamos ainda a importância de um correto diagnóstico etiológico para uma correta abordagem terapêutica e de uma abordagem multidisciplinar destes doentes. É fundamental que o oftalmologista esteja atento a todos os sinais e sintomas que o doente apresenta, através de uma história clínica completa e de uma avaliação objetiva detalhada, permitindo identificar os casos potencialmente graves, nos quais pode haver invasão orbitária, levando a uma orientação correta e um tratamento adequado.

The m.3243A>G variant in the MTL1 gene is a DNA point mutation which has been shown to cause a variety of diseases including retinal abnormalities. We report on the ophthalmologic phenotype and systemic findings in a new patient with this mutation, presenting with sensorineural hearing loss, diabetes and macular dystrophy. We characterized the retinal phenotype with a multimodal approach and throughout the follow-up period we observed sustained centripetal progression of the areas of chorioretinal atrophy. Given the increasing incidence of obesity, bariatric procedures are gaining attention during the past decades, being one of the most effective methods of long-term weight loss. Nutritional optic neuropathy (NON) following bariatric surgeries is a rare but devastating complication caused by a complete lack or inadequate supply of nutrients for the normal function of nerve fibers. We report a case of bilateral anterior NON after a Single Anastomosis Duodenoileal Bypass with Sleeve Gastrectomy, and present a review of the literature about neuro-ophthalmic complications associated with these procedures. Bariatric surgery may constitute an established risk factor for NON and both patient and physician have to be aware of the importance of closely monitoring vitamin depletions and neurologic and visual symptoms. Ocular surface complications secondary to continuous positive airway pressure (CPAP) machines have been previously reported such as ocular irritation, dry eye disease, lid laxity, and floppy eyelid syndrome. No severe ocular complications have been specified in the literature. We report a serious ocular complication in a patient under helmet CPAP (hCPAP). A 68-year-old male with COVID-19 pneumonia required ventilatory support with hCPAP. Following 5 days, the patient started complaining of ocular discomfort, purulent discharge in his left eye, and ocular dryness upon awakening. Ophthalmological examination showed no light perception in the left eye and slit-lamp biomicroscopy revealed atalamia, loss of corneal transparency with superior infiltrates, and a total epithelial defect with iris herniation and a central perforation. Considering the patient's progressive worsening clinical condition, with a painful blind eye, left evisceration was performed. It is important to be aware of possible ophthalmological complications related to ventilation since prompt diagnosis and treatment can avoid poor visual outcomes. We present the case report of a 6-year-old girl with anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis associated with bilateral optic neuritis. She initially presented with disturbing behaviors and gait impairments and was hospitalized in the context of meningoencephalitis, without agent isolation and with inconclusive laboratory tests. After discharge, she noted a loss of vision with impaired color sensation and returned to the emergency department. Her best-corrected visual acuity was 20/400 in both eyes, and fundoscopy revealed bilateral optic disc edema. She was hospitalized once again. Anti-NMDAR antibodies were detected in serum and cerebrospinal fluid. The patient was successfully treated with a pulse of methylprednisolone, followed by a tapering regimen of prednisolone. She recovered to 20/20 vision in the right eye and left eye. Our report highlights additional aspects of anti-NMDAR encephalitis in children and suggests that the disease could present atypical visual manifestations, which requires multidisciplinary approaches. 11. Choroidal neovascularization after blunt ocular trauma: OCT Angiography better than fluorescein angiography for diagnosis and treatment follow-up.

A 43-year-old man presented with a decrease of vision in the left eye after blunt ocular trauma. Visual acuity was of 20/20 in the right eye and 20/100 in the left eye. Fundus examination revealed a choroid rupture and submacular hemorrhage. Three months after the trauma, it was evident the development of choroidal neovascularization (CNV). A serie of intravitreal injections of ranibizumab were initiated. The optical coherence tomography angiography (OCTA) demonstrated to be better than fluorescein angiography (FA) to show evidence of CNV, and was essential to follow up the progression and guide therapeutic decisions. Decision for treatment based on OCTA findings lead to an excellent visual acuity outcome with visual acuity of 20/25 seven months after the trauma. [ABSTRACT FROM AUTHOR]

Introduction: Idiopatic Macular Telangiectasia (IMT) are a rare group of various entities presenting with incompetence, ectasia and/or irregular dilations of the juxtafoveolar capillaries of one or both eyes. Case Report: A 62 year old female with Glaucoma was sent to the Retina Department with a suspected diagnosis of retinal vein occlusion on the left eye (LE). Her best corrected visual acuity (BCVA) was 20/20 in the right eye (RE) and 20/25 in the LE. Slit lamp examination was normal; fundoscopy showed a cup-to-disc relation of 0.6 in the RE with normal macula and a cup-todisc relation of 0.7 in the LE with multiple perifoveal microaneurysms, inferior temporal hard exudates and edema comproved with macular optical coherence tomography; fluorescein angiography revealed an area of hyperfluorescence with juxtafoveolar telangiectatic vessels located inferiorly and temporally to the fovea, with progressive filling and late active leakage in the LE. The diagnosis of IMT type 1 was made and she was treated with a loading dose of intravitreal injections of ranibizumab followed by a fourth injection because of persistent macular edema. One month after, her BCVA was 20/20 in the LE with improvement of macular edema and macular árgon laser photocoagulation was performed. Three months after the laser treatment her LE BCVA was 20/20 with fundoscopy showing small macular hemorrhages and microaneurysms. Conclusion: Macular edema and exudation are the main cause of visual loss in these patients. This case suggests that ranibizumab and laser photocoagulation can be effective in the treatment of type 1 IMT. [ABSTRACT FROM AUTHOR]

14. Dry Eye Disease Symptoms Among Glaucoma Patients at a Portuguese Hospital.

Introduction: Glaucoma-related dry eye disease (DED) is often underestimated, but it is an important comorbidity affecting 40% to 59% of glaucoma patients. It may be an exacerbation of a pre-existing condition or a novel disease starting after the initiation of topical medication. The cumulative effect of medication, preservatives and excipients leads to an alteration in tear film composition and ocular surface stability. The main purpose of this investigation was to study a group of Portuguese glaucoma patients regarding the presence of DED symptoms and correlate the severity of the symptoms with the usage of different types of glaucoma topical medications., Materials and Methods: This is a cross-sectional observational study of patients diagnosed with primary and secondary open-angle glaucoma. The questionnaire Standardized Patient Evaluation of Eye Dryness (SPEED) translated to Portuguese (SPEED-Vp) was taken by patients followed in the Glaucoma Department of Unidade Local de Saúde Entre Douro e Vouga, Santa Maria da Feira, Portugal. Data was collected regarding their age, gender, type of topical medication in use as well as frequency and duration of usage. A statistical analysis was performed., Results: A total of 75 patients answered the SPEED-Vp questionnaire. The mean age was 72 ± 7 years old. Fifty-two percent (n=39) were male, and 48% (n=36) were female patients. About 49.33% (n=37) had been on intraocular pressure (IOP)-lowering eyedrops for more than five years. About 61.43% (n=43) of patients used IOP-lowering eyedrops with preservatives. Most of the patients used prostaglandin analogs (75.71%, n=53) and beta-blockers (72.86%, n=51). SPEED score average was 2.75. About 25.33% (n=19) had no DED symptoms, 58.67% (n=44) had mild symptoms, 8% (n=6) had moderate symptoms and 8% (n=6) had severe symptoms. No statistically significant correlation was found between SPEED score and age, gender, number of eyedrop containers, number of active principles, application frequency, presence of preservatives, number of eyedrop containers with preservatives, duration of eyedrops usage or any of the medication groups., Conclusion: Although a high percentage of patients were on eyedrops with preservatives, this low rate of symptoms might be because patients tended to devalue these symptoms; were already on treatment with artificial tears; or have an underestimation of the sensation of dry eye due to decreased neuronal corneal nerve responses and density. These results were surprisingly positive. - Abstract
Purpose: To assess the prevalence of amblyopia in a population of adolescents screened for amblyogenic risk factors at preschool age., Methods: Data were retrospectively collected from the preschool screening for amblyogenic risk factors routinely performed in the authors' hospital. A stratified random sampling was used. A school from the region was randomly selected and then two grades were randomly selected. All classes from these grades were evaluated and only children who were previously screened for amblyogenic risk factors were included. Ophthalmological examination included best visual acuity (distance and near, Early Treatment of Diabetic Retinopathy Study scale and Jaeger eye chart) and stereopsis (Randot Stereo Test; Stereo Optical Company, Inc). Sample size was estimated as more than 283 participants. Pertinent data were extracted for analysis., Results: A total of 520 children were recruited, and 299 met the inclusion criteria. Fifteen percent of children (n = 46) had results at the screening that prompted a further ophthalmological evaluation and 9% of children (n = 26) had meaningful refractive errors or strabismus. Overall amblyopia prevalence was 1.00%. One of the 3 children who developed amblyopia had microstrabismus, and the remaining 2 children had a previous positive screening result but missed the follow-up evaluations. At the follow-up evaluation, 79.3% (n = 237) of children were not wearing glasses., Conclusions: A structured screening may allow the early detection of amblyogenic factors and prevent further vision deterioration in children, thus improving their long-term quality of life.
