Abstract Background The aim of this study was to ascertain the prevalence of burnout in Kazakhstan firefighters with regard to position and to identify predictors of faster burnout in order to plan future preventive strategies. Methods Data on demographics, lifestyle, fatigue (Fatigue Severity Scale (FSS)), SF-8 health-related quality of life (HRQL) and Maslach Burnout Inventory (MBI) emotional exhaustion (EX), cynicism (CY) and professional efficacy (PE) were obtained from 604 (94% men, median age 27 (interquartile range (IQR) 12) years) firefighters from all 18 fire departments of the city of Almaty. Associations between predictors and burnout EX, CY and PE dimensions were tested using multivariate logistic regression analyses. Results Burnout scores were low in this sample, including EX (0.6; IQR 1.55), CY (1.2; IQR 1.8) and PE (4.8; IQR 2.4). This study aims to understand subaltern cosmopolitanism and governance dynamics in low-income neighbourhoods in Colombo, Sri Lanka. Subaltern cosmopolitanism is a relatively new underrepresentation of cosmopolitanism that draws attention to the life of various marginal groups in society. Low-income neighbourhoods in Colombo (known locally as watta) are key areas that bring together diverse ethnic, religious, and linguistic groups, especially when contrasted to the main societies in Sri Lanka. Despite the fact that economically and socially disadvantaged group of residents illustrates fundamental characteristics of cosmopolitanism resulting from daily interactions with difference, these elements have not been adequately investigated within the cosmopolitanism framework. Ethnography was the key approach used in this study. Participant observation, in-depth interviews and archival records were used to investigate and understand the present and past life of watta residents. To analyse the collected data, narrative analysis and thick description, were used. This study found that the watta culture is distinct in that it allows different groups to engage in everyday life. Religion is an essential element of watta residents through which they have interacted with different groups of people for a long time. However, a small group is involved in governance of the watta. This requires outsiders' involvement and brings in party politics, welfare assistance, and development projects which have exacerbated ethnic and religious differences among the residents of the watta. Hence, now the watta is changing like the majority society, the division in ethnicity and religion arises in the community. This thesis concludes that subaltern cosmopolitanism exists in the low-income neighbourhoods in Colombo, but it has had a significant impact due to the influence of internal and external stakeholders.

17. A critical evaluation on the innovative performance of smartphone industry : a case study on Micromax Informatics in India

There is evidence which suggest that smartphone industry is an active platform experiencing steady growth and demand. The industry is contented with tough competition from different Smartphone companies and thus innovations are considered as buzz word in the industry. Smartphone manufacturers are very keen on assessing the upcoming trends in the market and thereby establishing the updates in the technology in to their existing and new products. The main purpose of the current research study was therefore to evaluate the innovation performances of Indian Smartphone sector using the case of Micromax Informatics as an example. This research sought to understand the impact of innovation performance of Micromax from the perspective of the various stakeholders. This included surveys and interviews with the aim of gaining first hand in-depth information about the Smartphone product. This research led DBA project also sought to identify the challenges, constraints and lessons learned on innovation performance from the experiences of diverse stakeholders in India smartphone industry’s in the context of developing nations. The sample size considered for the interview and survey was 10 managers and 100 customers respectively. By conducting survey with the customers, the researcher gathered a wealth of evidence about the performances of Micromax products. The user’s opinion about the Micromax smartphones was identified by conducting survey with the customers and further interview were held with managers regarding the performances of their product in line with the trends in the industry. The results suggest that, despite the interest and enthusiasm of Micromax for introducing Smartphones, the views of different authors are much more mixed regarding the performance of Micromax which is further exacerbated by the challenges coming from the competitors. Furthermore, there was evidence of the lack of innovative ideas and products of Micromax including not introducing the 4G when in line with the competitors. The main challenges observed when introducing Smartphones were a lack of detailed analysis of competitors and the rival.

19. Can we collect health-related quality of life information from anticoagulated atrial fibrillation participants who have recently experienced a bleed? An observational feasibility study in primary and secondary care in Wales and through a UK online forum

Objective To evaluate the feasibility of recruiting participants diagnosed with atrial fibrillation (AF) taking oral anticoagulation therapies (OATs) and recently experiencing a bleed to collect health-related quality of life (HRQoL) information.Design Observational feasibility study. The study aimed to determine the feasibility of recruiting participants with minor and major bleeds, the most appropriate route for recruitment and the appropriateness of the patient-reported outcome measures (PROMs) selected for collecting HRQoL information in AF patients, and the preferred format of the surveys.Setting Primary care, secondary care and via an online patient forum.Participants The study population was adult patients (≥18) with AF taking OATs who had experienced a recent major or minor bleed within the last 4 weeks.Primary and secondary outcome measures Primary outcomes – PROMs: EuroQol 5 Dimensions-5 Levels, Perception of Anticoagulant Treatment Questionnaire, part 2 only (part 2), atrial fibrillation effect on quality of life. Secondary outcomes – Location of bleed, bleed severity, current treatment, patient perceptions of HRQoL in relation to bleeding events.Results We received initial expressions of interest from 103 participants. We subsequently recruited 32 participants to the study—14 from primary care and 18 through the AF forum. No participants were recruited through secondary care. Despite 32 participants consenting, only 26 initial surveys were completed. We received follow-up surveys from 11 participants (8 primary care and 3 AF forum). COVID-19 had a major impact on the study.Conclusions Primary care was the most successful route for recruitment. Most participants recruited to the study experienced a minor bleed. Further ways to recruit in secondary care should be explored, especially to capture more serious bleeds.Trial registration number The study is registered in the Clinicaltrials.gov database, NCT04921176.

Introduction Medical patients, admitted acutely to hospital, are at risk of venous thromboembolism (VTE). Clinical guidelines advise thromboprophylaxis prophylaxis for those at high risk of VTE. VTE is a common sequela of cancer, but guidelines take little consideration of cancer as an independent risk factor and their utility in palliative care patients is unclear. The hospice inpatient deep vein thrombosis (DVT) detection study (HIDDen) reported a 28% prevalence of asymptomatic iliofemoral DVT in hospice patients of poor performance status (PS) and prognosis, calling into question the utility of thromboprophylaxis in the palliative care setting. However, the majority of cancer inpatients receiving palliative care are admitted to hospital through the acute medical setting, yet their risk factors for VTE may differ from those admitted to hospices.Objective To better understand the prevalence and behaviours of VTE in patients with cancer receiving palliative care who are admitted as an acute medical emergency.Design Multicentre, observational cohort study.Setting Secondary care acute hospitals in South Wales, UK.Patients We plan to recruit 232 patients≥18 years old with a diagnosis of incurable cancer, and/or receiving palliative or best supportive care who are admitted acutely to hospital. Patients will be followed up for a maximum of 6 months following registration.Primary outcome Presence of lower extremity DVT.Secondary outcomes Symptom burden attributed to DVT or pulmonary embolism, patient PS, patient demographics and development of new VTE within 90 days of registration.Analysis The study statistical analysis plan will document analysis, methodology and procedures.Ethics and dissemination Ethical approval was obtained from the Wales Research Ethics Committee, reference 22/WA/0037 (IRAS 306352)—the main trial results will be analysed as soon as practically possible and the publication shared with investigators and on sponsor website; applications to access trial data will be subject to sponsor review process.

Background: Mechanical augmentation upon implantation is essential for the long‐term success of tissue‐engineered intervertebral discs (TE‐IVDs). Previous studies utilized stiffer materials to fabricate TE‐IVD support structures. However, these materials undergo various failure modes in the mechanically challenging IVD microenvironment. FlexiFil (FPLA) is an elastomeric 3D printing filament that is amenable to the fabrication of support structures. However, no present study has evaluated the efficacy of a flexible support material to preserve disc height and support the formation of hydrated tissues in a large animal model. Methods: We leveraged results from our previously developed FE model of the minipig spine to design and test TE‐IVD support cages comprised of FPLA and PLA. Specifically, we performed indentation to assess implant mechanical response and scanning electron microscopy to visualize microscale damage. We then implanted FPLA and PLA support cages for 6 weeks in the minipig cervical spine and monitored disc height via weekly x‐rays. TE‐IVDs cultured in FPLA were also implanted for 6 weeks with weekly x‐rays and terminal T2 MRIs to quantify tissue hydration at study endpoint. Results: Results demonstrated that FPLA cages withstood nearly twice the deformation of PLA without detrimental changes in mechanical performance and minimal damage. In vivo, FPLA cages and stably implanted TE‐IVDs restored native disc height and supported the formation of hydrated tissues in the minipig spine. Displaced TE‐IVDs yielded disc heights that were superior to PLA or discectomy‐treated levels. Conclusions: FPLA holds great promise as a flexible and bioresorbable material for enhancing the long‐term success of TE‐IVD implants. [ABSTRACT FROM AUTHOR]

Aims: Monitoring the performance of health care systems is a vital requirement for improving the quality of health care services. This review research was conducted on the results of recent studies in the field of organizational excellence in Iranian hospitals that were conducted with the EFQM model. Materials and Methods: The current research was conducted in 2022 to review the articles published in Persian and English journals available in SID, Google Scholar, ProQuest, PubMed, Elsevier, ISI, Scopus, IranMedex, Science Direct, Spring, and CIVILICA related to the last 15 years. Articles were searched using the keywords of organizational excellence, performance assessment or evaluation, EFQM, hospital, and health system or their Persian equivalent. Information sources were analyzed based on qualitative analysis and meta-analysis using "Metafor version 3.8-1" and "Meta version 6.1-0" packages in the R programming language. FINDINGS: In general, in the investigated period, 14 studies dealt with organizational excellence in hospitals, one of which was devoted to the qualitative study of organizational excellence and 13 to the quantitative evaluation of organizational excellence. Out of 13 studies, two studies were only limited to the evaluation of enablers. The articles' specifications and the quantitative results of organizational excellence components were presented (2006 to 2021) in Tables 2 and 3. Examining the time trend of studies shows that organizational excellence has not been a topic of interest to researchers in recent years, and no growing trend was observed in relation to the number of articles. Figure 2 is the forest diagram for the meta-analysis results of organizational excellence values on 13 studies. The overall estimate of the organizational excellence index for the mentioned studies was equal to 533 in the 95% confidence interval (464-602). In addition, the weight of each of the mentioned studies in the overall estimate of the analysis was reported as a percentage, and the two studies by Malekzadeh et al. [19] and Motaghi et al. [20] had the highest weight in the final result of this analysis with weights of 12.48 and 11.84, respectively (the large and bold square shape inside the confidence interval lines indicates the greater weight of that study). The index of organizational excellence in these studies varied between 367 and 700, and the range of changes based on random effects in the meta-analysis was estimated at 63%, indicating relatively significant changes among studies. Mental health policies and plans (MHPPs) are important policy instruments and powerful tools to facilitate development of mental health systems and services across the world. We aimed to map and analyse methods and tools used to assess the extent, process and impact of implementing MHPPs. We systematically searched peer-reviewed and grey literature across seven scientific databases. We extracted and analysed the data on a) the characteristics of included studies (e.g., policy areas, region of origin, income setting) and b) the methodology and evaluation tools applied to assess the extent and process of implementation. We included 48 studies in the analyses. Twenty-six of these studies employed only qualitative methods (e.g., semi-structured interviews, focus group discussions, desk review, stakeholder consultations); 12 studies used quantitative methods (e.g., trend analysis, survey) and 10 used mixed-methods approaches. Generally, methods and tools used for assessment were described poorly with less than half of the studies providing partial or full details about them. Only three studies provided assessment of full policies. There is a lack of rigorous research to assess implementation MHPPs. Assessments of the implementation of entire MHPPs are almost non-existent. Strategies to assess the implementation of MHPPs should be an integral part of MHPPs.

ABSTRACT RT-PCR tests based on RNA extraction from nasopharyngeal swabs (NPS) are promoted as the “gold standard” for SARS-CoV-2 detection. However, the use of saliva samples offers noninvasive self-collection more suitable for high-throughput testing. This study evaluated performance of the TaqPath COVID-19 Fast PCR Combo kit 2.0 assay for detection of SARS-CoV-2 in raw saliva relative to a lab-developed direct RT-PCR test (SalivaDirect-based PCR, SDB-PCR) and an RT-PCR test based on RNA extraction from NPS. Saliva and NPS samples were collected from symptomatic and asymptomatic individuals (N = 615). Saliva samples were tested for SARS-CoV-2 using the TaqPath COVID-19 Fast PCR Combo kit 2.0 and the SDB-PCR, while NPS samples were tested by RT-PCR in RNA extracts according to the Irish national testing system. TaqPath COVID-19 Fast PCR Combo kit 2.0 detected SARS-CoV-2 in 52 saliva samples, of which 51 were also positive with the SDB-PCR. Introduction Neutralising antibodies (NAbs) have been shown to be correlative of immune protection against SARS-CoV-2. We report the protocol for a national longitudinal study to assess and compare the level of NAbs generated in response to COVID-19 vaccines in Brunei Darussalam in adults 2–6 weeks post primary series (BBIBP-CorV, AZD1222, or mRNA-1273 vaccines) and their subsequent follow-up after administration of a third (booster-1) dose (BBIBP-CorV, mRNA-1273, or BNT162b2).Methods and analysis Participant data will be extracted and processed from the national electronic health record system (Bru-HIMS) and the national mobile health application (BruHealth) into a research data platform. Eligible adults who have received their primary or booster vaccine will be invited using a stratified random sampling strategy based on age, gender and vaccine type (baseline target population, n=3000; 2–6 weeks post last dose). Blood serum will be isolated, and NAb levels assessed using the cPass surrogate virus neutralisation test. Baseline participants will then be screened for eligibility for subsequent longitudinal analysis. Those who have received a third dose will be followed up at 1, 3, 6, 9 and up to 12 months. NAb levels will be evaluated across the participant population according to vaccine platform/booster type, time since the last dose and correlated with demographic data. The study period is from December 2021 to January 2023 and aims to evaluate how NAb levels wane following a third vaccine dose across different vaccine platforms and determine the impact and rate of breakthrough infections.Ethics and dissemination This study has been approved by the Medical and Ethical Research Committee of Ministry of Health, Brunei Darussalam. Individual NAb test results will be shared with each participant by text message. The findings from this study will help policy-makers in Brunei develop future vaccination strategies and establish regulations across multiple agencies.

The purpose of the study described in this paper is aimed at identifying the environmental situation of the agroecosystem and studying the impact of resource-saving technology methods for the cultivation of oilseed rapeseed, ensuring sustainability and productivity of the agroecosystem during environmental monitoring. The object of the study was spring rapeseed of the Maily variety. Experimental studies were carried out using generally accepted classical techniques, such as experiment and observation, in the form of a multifactorial field experiment. A two-factor field experiment was laid out by the split plots method and based on the methodology for conducting field experiments with oilseeds. The results of the study were obtained using the methods of a soil-protective agriculture system based on the principles of soil protection technology. The studied minimal tillage provides a reduction in the amount of mechanical impact on the soil, restoration and preservation of soil fertility, improvement of moisture supply by leaving the plant stubble and scattering chopped straw after harvesting the previous crop, and replacing deep moldboard tillage with shallow flat-cut tillage. Based on the obtained results of the study, it was proved that the replacement of energy-intensive moldboard tillage with shallow flat-cut tillage (mini-till), ensuring the rational use of soil resources and resource-energy conservation, was feasible. The reconstructed parameters of agrophysical factors of soil fertility were determined, such as density, aggregate composition, and the structure and water resistance of soil aggregates, which ensured the sustainability of the environmental situation of the agroecosystem. It has been established that soil protection methods of resource-saving rapeseed cultivation technology mitigate the risk and optimize the conditions for the growth and development of crops, where productivity increases to 21.3%, and also provide 29.3 thousand tenges/ha of additional income from each hectare with an increase in the profitability of rapeseed cultivation to 97% and 108% in the conditions of South-East Kazakhstan.
