OBJETIVO: Mostrar la prevalencia de la diabetes mellitus (DM) y los factores asociados con esta enfermedad en adultos a partir de datos de la Encuesta Nacional de Salud 2000 (ENSA 2000). MATERIAL Y MÉTODOS: Se realizó una entrevista, entre noviembre de 1999 y junio de 2000, a 45 294 sujetos de 20 años de edad y mayores. Se obtuvieron valores de glucosa capilar, peso, talla y tensión arterial. Para el análisis estadístico se utilizaron los factores de expansión individuales, y para el cálculo de las varianzas se tomó en consideración el diseño complejo de la encuesta mediante SUDAAN 7.5.6. RESULTADOS: La prevalencia nacional de DM en adultos de 20 y más años de edad fue de 7.5% (IC95% 7.1-7.9). En las mujeres fue de 7.8% y en los hombres de 7.2%. La prevalencia aumentó en relación directa con la edad, 2.3% antes de los 40 años y 21.2% después de los 60. En la población urbana la prevalencia fue de 8.1% y en la rural de 6.5%. La enfermedad fue más frecuente en la región norte del país (8.4%) y en el área metropolitana de la Ciudad de México (8.1%). Asimismo, fue más frecuente en la población con menor escolaridad (9.9%), menor ingreso (8.1%), hipertensión arterial (13.7%), hipercolesterolemia (23.3%), microalbuminuria (15.5%) y enfermedad renal (12.3%). Mediante modelos de regresión logística multivariada, estratificados por sexo, se identificaron como variables asociadas con la presencia de DM la edad, la baja escolaridad, el antecedente familiar de DM y la coexistencia de hipertensión arterial, enfermedad renal o hipercolesterolemia en ambos sexos. La obesidad abdominal se relacionó con DM sólo en las mujeres; la residencia en una zona urbana se vinculó con DM exclusivamente en los hombres. Las acciones más fuertes se observaron con la edad, el antecedente familiar de DM y la microalbuminuria. CONCLUSIONES: Los datos de la Encuesta Nacional de Salud 2000 son prueba del grave problema de salud que representa la DM para México. Su efecto se magnifica al afectar con mayor frecuencia a grupos de población cuyos factores sociales o económicos limitan su acceso al tratamiento. Los datos informados son útiles para la institución de programas de escrutinio y prevención. Los resultados confirman que la diabetes debe ser motivo de investigación en familiares de primer grado e individuos que tengan uno o más de los componentes del síndrome metabólico (hipertensión, dislipidemia, obesidad y microalbuminuria).OBJECTIVE: To show the prevalence of diabetes mellitus (DM) and its associated factors in adults, using data derived from the 2000 National Health Survey (NHS). MATERIAL AND METHODS: The 2000 NHS was conducted between November 1999 and June 2000. An adult questionnaire was administered to 45 294 subjects 20 years of age and older. Capillary glucose levels, weight, height and blood pressure were obtained. Individual weighted factors were considered in the statistical analysis, as was the survey's complex sampling design to obtain variances using SUDAAN 7.5.6. RESULTS: The national prevalence of DM in adults ages 20 years and older was 7.5% (95% CI: 7.1-7.9). The prevalence was 7.8% in women and 7.2% in men. It was higher according to age: 2.3% in adults 40 years or younger and 21.2% in those older than 60 years of age. In the urban population, prevalence was 8.1% and in the rural population it was 6.5%. The disease was more frequent in the northern region of the country (8.4%) and in the Mexico City metropolitan area (8.1%). DM was more frequent in the population with the least amount of schooling (9.9%), the lowest income (8.1%), high blood pressure (13.7%), hypercholesterolemia (23.3%) microalbuminuria (15.5%) and renal disease (12.3%). Using multivariate logistic regression, stratified by gender, variables associated with DM were identified: age, little schooling, family history of DM, high blood pressure, renal disease or hypercholesterolemia in both genders. Abdominal obesity was associated with DM in women only and living in an urban area was associated with DM only in men. The highest associations were observed with age, family history of DM, and microalbuminuria. CONCLUSION: Results from the 2000 NHS show that DM represents a serious public health problem in Mexico. The impact increases when DM affects populations with social or economic factors that limit access to treatment. Data reported in this paper are useful to the implementation of screening and prevention programs. These results confirm that DM should be screened in first-degree family members and everyone who has one or more metabolic syndrome components (high blood pressure, dyslipidemia, obesity and microalbuminuria).

This paper reviews data published between 1990 and 2006 regarding smoking prevalence as well as individual and contextual influences on the smoking behavior among Romanian young adults. Highlights include a consideration of multiple levels of influence, from intra-individual factors, such as demographic and cognitive factors, to social influences, such as families and peers, to the more macro, societal/cultural levels of influence, including advertising and tobacco-related policies The source of data is represented by articles and short information published in journals or in electronic format, legislation, statistics and are illustrated with pictures. Based on these data, recommendations for future smoking prevention and reduction actions for Romanian youth are taken.Se revisan datos publicados entre 1990 y 2006 sobre prevalencia de tabaquismo, así como influencias individuales y contextuales respecto al hábito de fumar entre adultos jóvenes rumanos. Los hallazgos incluyen una consideración de múltiples niveles de influencia, desde factores intraindividuales (por ejemplo, demográficos y cognitivos), influencias sociales (como familia y pares), hasta niveles de influencia macro sociales/culturales (publicidad y políticas relacionadas con el tabaco, entre otros). La fuente de datos incluye artículos y notas breves publicadas en revistas científicas o en formato electrónico, leyes, estadísticas, y se ilustran con imágenes. Con base en estos datos, se establecen recomendaciones para acciones futuras de prevención y reducción del consumo de tabaco en jóvenes rumanos.

OBJECTIVE: To review epidemiological evidence on the association between smoking and tuberculosis. METHODS: Reviewed articles were identified by searching Pubmed for the terms "smoking" or "tobacco" and "tuberculosis". Additional articles were obtained from the bibliographies of identified papers. RESULTS: Thirty-four studies were reviewed: five investigate the association between smoking and mortality from tuberculosis, 13 investigate the association between smoking and development of tuberculosis, eigth investigate the association between smoking and infection with Mycobacterium tuberculosis, and nine estimate the impact of smoking on characteristics of tuberculosis and disease outcomes. CONCLUSIONS: Taken together, evidence suggests that smoking (both current and former) is associated with: risk of being infected with Mycobacterium tuberculosis, risk of developing tuberculosis, development of more severe forms of tuberculosis, and risk of dying of tuberculosis. In many cases, there is a strong dose-response relationship -both in terms of quantity and duration of smoking. These relationships are not explained away by controlling for potentially confounding variables such as age, gender, alcohol consumption, and HIV status.OBJETIVO: Revisar evidencia epidemiológica relativa a la asociación entre el tabaquismo y la tuberculosis. MATERIALES Y MÉTODOS: Se identificaron artículos de revisión mediante la búsqueda en Pubmed de los términos "tabaquismo", "tabaco" y "tuberculosis". Se obtuvieron artículos adicionales de las bibliografías de los trabajos identificados. RESULTADOS: Se revisaron 34 estudios: cinco investigan la asociación entre tabaquismo y mortalidad a partir de la tuberculosis; 13, la asociación entre tabaquismo y el desarrollo de tuberculosis; ocho, la asociación entre tabaquismo y la infección con Mycobacterium tuberculosis; y nueve estiman el impacto del tabaquismo en las características de la tuberculosis y las enfermedades resultantes. CONCLUSIONES: En conjunto, la evidencia sugiere que el tabaquismo (tanto en la actualidad como en sus inicios) se asocia con lo siguiente: el riesgo a infectarse con Mycobacterium tuberculosis, el riesgo de desarrollar tuberculosis, de formas más severas de la misma, y con el riesgo de morir a causa de ella. En muchos casos, hay una fuerte relación dosis-respuesta, tanto en términos de cantidad como de duración del tabaquismo. Estas relaciones incluso se explican mediante el control de variables potencialmente confusoras como la edad, el género, el consumo de alcohol y la enfermedad del VIH.

La violencia doméstica masculina contra la pareja heterosexual (VDMCPH) es un tema que se ha investigado de manera sistemática en México en los últimos 15 años. El propósito de este trabajo es reflexionar sobre los aportes hechos sobre el tema en el país. La discusión está centrada en tres grandes ámbitos temáticos: la magnitud del fenómeno considerando sus implicaciones teóricas, metodológicas y técnicas; la relación de la violencia con diversos factores; y las consecuencias del ejercicio de la VDMCPH en la propia mujer. La exposición está guiada por los aportes hechos en estudios cuantitativos y complementada con la visión cualitativa.Violence against women by partners (VAWP) has been systematically studied in Mexico in the past 15 years. The aim of this paper is to reflect on the contributions on this topic in Mexico. The discussion focuses on the magnitude of this phenomenon, stressing its theoretical, methodological, and technical implications; the relationship between violence and a variety of factors; and the outcomes of VAWP on women themselves. The manuscript features the main contributions from quantitative and qualitative studies.

OBJETIVO: Comprender cómo la construcción social y la expresión de la masculinidad entre los adolescentes y varones jóvenes se articulan con los riesgos para la salud sexual y reproductiva. MATERIAL Y MÉTODOS: El estudio se realizó en una zona "marginal" y otra "popular" de la Ciudad de México. Se llevaron a cabo 18 grupos focales y 18 entrevistas individuales a una muestra propositiva de varones de tres grupos de edad: 10 a 14, 15 a 19 y 20 a 24. El análisis de la información se hizo de forma inductiva, mediante una codificación abierta de las transcripciones verbales, a partir de la cual se generaron conceptos más abstractos y se buscaron las relaciones entre ellos. RESULTADOS: Este trabajo se enfoca en las maneras de entender la masculinidad, tal y como fueron expresadas por los jóvenes. Nuestra investigación arroja alguna luz sobre lo que los adolescentes dicen y hacen para "ser hombres". Sus ideas y prácticas llevan a varias formas de masculinidad mediante las cuales se reconstruyen comportamientos arriesgados, estilos de hablar y de vestir, formas de cortejo y de noviazgo específicas. Estos y sus significados son articulados con los riesgos en el campo de la salud sexual y reproductiva de los jóvenes y de sus parejas. CONCLUSIONES: El modelo tradicional de masculinidad que predomina en ambos sectores sociales tiene como resultado, por una parte, que haya poca comunicación sobre la sexualidad en las relaciones de pareja y que, consecuentemente, la utilización de medidas de protección se dé en muy raras ocasiones, exponiendo a los jóvenes a ITS y al riesgo de transformarse en padres sin ser aún adultos. Por otra parte, las condiciones económicas en que viven dificultan que lleven a la práctica elementos centrales de su propio concepto de masculinidad, como el ser trabajador, proveedor, responsable, lo que pudiera traducirse en frustración, agresividad y en violencia intrafamiliar.OBJECTIVE: To understand how the social construction and expression of masculinity among male adolescents and young adults are related to sexual health and reproductive risks. MATERIAL AND METHODS: The study was carried out in an underserved and a popular area of Mexico City. Eighteen focal groups and 18 individual interviews were applied to a sample of males from three age groups: 10 to 14, 15 to 19, and 20 to 24 years. Data analysis was performed inductively through open codification of the verbal transcriptions. These were used to create more abstract concepts and search for relationships among them. RESULTS: This paper centers on interpretations of masculinity as expressed by the youngsters themselves. Our research sheds light on what youngsters do and say to "be men". Their ideas and practices produce different forms of masculinity through which risk behavior is manifested, as well as specific speech and dressing styles, and ways of courting and "going steady". These behaviors and their meanings are associated with sexual and reproductive health risk taking. CONCLUSIONS: The traditional model of masculinity characteristic of both social settings involves poor communication about sexuality in couple relationships, which results in infrequent protection and exposure to Sexually Transmitted Infections and unwanted pregnancies. On the other hand, their economic living conditions prevent them from fully practicing central elements of their own concept of masculinity, such as being a hard worker, a provider, and a responsible person. These unmet needs may cause frustration, aggression, and domestic violence.

OBJETIVO: Describir el consumo de drogas y los factores de riesgo en adolescentes de 12 a 17 años. MATERIAL Y MÉTODOS: Los datos provienen de la Encuesta Nacional de Adicciones 1998, realizada en hogares ubicados en zonas urbanas de la República Mexicana. El diseño de la muestra fue probabilístico, estratificado y por conglomerados en varias etapas de muestreo, donde la última unidad de selección fue un individuo en la vivienda. Los resultados se analizaron obteniendo los intervalos de confianza de las prevalencias del consumo de drogas, y se realizó un análisis de regresión logística para evaluar los factores asociados con el consumo de drogas. RESULTADOS: El 3.57% de los varones y 0.6% de las mujeres habían usado una o más drogas excluyendo al tabaco y al alcohol; 2.14 de hombres y 0.45% de mujeres lo habían hecho en los doce meses previos al estudio, y 1.4 de los hombres y 0.3% de las mujeres en los 30 días anteriores a la encuesta. La mariguana es la droga más usada (2.4 y 0.45%), seguida por los inhalables (1.08 y 0.20%) y la cocaína (0.99 y 0.22%) por hombres y mujeres, respectivamente. El riesgo de usar drogas se asoció con ser hombre, no estudiar, considerar fácil conseguir drogas, no ver mal el uso de drogas por parte de los amigos, que éstos las usaran, usarlas por parte de la familia y estar deprimido. CONCLUSIONES: El entorno que rodea a nuestros jóvenes de 12 a 17 años indica que el consumo de drogas está cada vez más presente. Los índices de consumo se han incrementado, especialmente en la región norte del país y en las grandes metrópolis (Tijuana, Ciudad de México y Guadalajara, principalmente). Estos datos refuerzan la necesidad de desarrollar campañas que permitan detectar a los menores que tienen problemas emocionales y darles la atención apropiada para evitar que consuman drogas como un mecanismo para enfrentar problemas de esta naturaleza. Asimismo, es necesario identificar a los menores con mayor probabilidad de desarrollar dependencia, quienes requieren de intervenciones preventivas más intensas.OBJECTIVE: The aim of this paper is to describe drug and associated factors use among adolescents (12 to 17 years of age). MATERIAL AND METHODS: Data come from the recent Encuesta Nacional de Adicciones, 1998 (National Survey on Addictions) undertaken in urban areas of Mexico. A probabilistic, multi-stage, stratified cluster sampling design was used to select the study population. The sampling unit was the individual in each household. Statistical analysis consisted of the estimation of prevalences of drug use, with 95% confidence intervals. Associations were analyzed using logistic regressions models. RESULTS: Excluding tobacco and alcohol, 3.57% of males and 0.6% of females have used one or more drugs; 2.14% and 0.45% used them in the 12 months previous to the survey, and 1.4% and 0.3% in the previous 30 days, respectively. Marihuana was the drug more often used by males (2.4%) and females (0.45%), followed by inhaled solvents (1.08% and 0.20%), and cocaine (0.99% and 0.22%), respectively. The risk of using illicit drugs was associated to being male, having dropped out from school, perceiving availability, drug use by family and friends, social tolerance among friends, and symptoms of depression. CONCLUSIONS: Adolescents between 12 to 17 years of age are exposed to increasing drug use. The rates of drug use have risen, especially in the northern region of Mexico and in the big urban areas (Tijuana, Mexico City and Guadalajara). These findings emphasize the need to develop campaigns to detect emotional problems. Preventive measures should provide appropriate care to prevent drug use as a mechanism to cope with such emotional conflicts. Also, more intense preventive interventions should be targeted to adolescents with a high probability of developing drug abuse.

OBJECTIVE: To compare patterns of sexual behavior among bisexual, heterosexual and homosexual men. MATERIAL AND METHODS:A household probability survey was carried out in Mexico City in 1992-1993 using the national health surveys sampling frame. Information from 8 068 men was obtained; however, the main analysis of this paper refers only to men sexually active in the previous 5 years. RESULTS: Bisexuals reported more prevalent anal intercourse with women (16% vs. 3%, p=0.01), and more sexual encounters with female sex workers than exclusive heterosexuals (10% vs. 4%; p=0.04). Bisexuals used condoms more often with sex workers than did heterosexuals (p=0.01). Most of the bisexuals (79%) did not engage in anal receptive or insertive intercourse with males in the previous year, practicing instead oral insertive sex or only masturbation; 35% of homosexuals did not report practicing anal sex. Bisexuals who engaged in anal intercourse had less anal receptive behavior than homosexuals (13% vs. 60%, pOBJETIVO: Comparar los patrones de comportamiento sexual entre hombres bisexuales, heterosexuales y homosexuales. MATERIAL Y MÉTODOS: Se llevó a cabo una encuesta probabilística en hogares de la Ciudad de México en 1992-1993, utilizando el marco muestral de las Encuestas Nacionales de Salud; se obtuvo información de 8 068 hombres entre 15 y 60 años de edad. El análisis principal de este trabajo se centra en hombres sexualmente activos en los cinco años previos a la encuesta. RESULTADOS: Los hombres bisexuales notificaron con mayor frecuencia relaciones sexuales anales con mujeres (16% vs. 3%, p=0.01), y mayor frecuencia de relaciones sexuales con trabajadoras sexuales que los heterosexuales exclusivos (10% vs. 4%, p=0.04). Los bisexuales usaron condones más frecuentemente con trabajadoras sexuales que los heterosexuales (p=0.01). La mayoría de los bisexuales (79%) no mantuvieron relaciones sexuales anales (receptivas o insertivas) con otros hombres en el año previo al estudio; en su lugar, llevaron a cabo sexo oral insertivo o sólo masturbación; 35% de los homosexuales exclusivos no reportó llevar a cabo ninguna práctica anal durante sus relaciones sexuales. El presente trabajo muestra las generalidades del trastorno disocial, así como las principales medidas para su reconocimiento y tratamiento. Aborda las generalidades y principios básicos de la terapia multisistémica enfocada para este trastorno; dicha terapia se basa en intervenciones biológicas, psicológicas y sociales, y considera el ámbito ecológico de las manifestaciones de la conducta de adolescentes. Se muestran algunos resultados en la aplicación de la terapia y se discuten sus ventajas y usos potenciales.This paper reviews the main features of conduct disorder (CD), as well as the principles for diagnosis and multisystemic treatment (MST). MST includes biological, psychological and social interventions, and considers the ecological environment of conduct manifestations. Some outcomes of MST delivery are discussed, along with its advantages and potential applications.

Objetivo. Describir el comportamiento de la demanda de tabaco en México para cuatro momentos en el tiempo: 1992, 1994, 1996 y 1998, y adicionalmente estimar una función de demanda de cigarrillos. Material y métodos. Se realizó un estudio transversal con análisis longitudinal, tomando como fuentes de información la Encuesta Nacional de Ingreso y Gasto de los Hogares (ENIGH) y la lista de precios de los cigarrillos de la Procuraduría Federal del Consumidor (Profeco). El gasto, el ingreso y los precios se deflactaron a precios de 1994. Se estratificó a la población en quintiles de ingreso real, y solamente se tomaron en cuenta las áreas rurales y urbanas. Se construyeron indicadores para el consumo diario de cigarrillos y cajetillas y se calcularon los precios por cajetilla. Se calculó la prevalencia ajustada. Mediante análisis de regresión logísticos y lineales se estableció la inferencia estadística. Se estimó una función de demanda de cigarrillos mediante una regresión lineal multivariada, para encontrar los determinantes socioeconómicos del consumo de cigarrillos. Resultados. La prevalencia ajustada de los hogares que reportaron algún gasto en tabaco disminuyó de 22.4 a 9.9% entre 1992 y 1998. Los hogares asignaron poco más de 4% de su ingreso a la adquisición de tabaco. Se observó un gradiente entre nivel de ingreso y gasto en cigarrillos. El quintil I (el más pobre) destinó una mayor proporción de su ingreso que los quintiles superiores. El promedio de cigarrillos consumidos diariamente pasó de 7.5 a 9.8 entre 1992 y 1998. Se estimó que más de 90% de los "hogares fumadores" consumió hasta una cajetilla diaria. La demanda estimada de cigarrillos sin filtro aumentó entre 1992 a 1998, pasando de 0.4 a 4.8%, registrándose así el mayor incremento en 1996. Finalmente, se encontró que, tanto los precios como el ingreso, fueron los determinantes más importantes del gasto en tabaco. Conclusiones. Una política de precios podría favorecer la reducción de la demanda y, por ende, mejorar las expectativas de salud de la población.Objective. This paper aims at describing the behavior of tobacco's demand in Mexico across four one-year periods: 1992, 1994, 1996, and 1998, as well as to estimate a cigarette demand function. Material and Methods. A cross-sectional study with longitudinal analysis was conducted. Information sources were the Encuesta Nacional de Ingreso y Gasto de los Hogares (ENIGH) (National Survey of Household Income and Spending) (NHSIS) and the tobacco pack prices reported by the Procuraduría Federal del Consumidor (Profeco) (Federal Office of Consumer's Protection) (FOCP). Spending, income, and prices were deflated to 1994 prices; the population was stratified into quintiles of real income, by rural and urban areas. Indicators of daily consumption of cigarettes and packs were constructed and prices per pack calculated. Adjusted prevalence figures were estimated. Logistic and linear regression models were used for statistical inference; a cigarette demand function was estimated using multivariate logistic regression, to find socioeconomic determinants of cigarette consumption. Results. The adjusted prevalence of household tobacco spending fell from 22.4 to 9.9% between 1992 and 1998. Households allocated more than 4% of their income to tobacco consumption. A trend between income level and cigarette spending was observed, with the first quintile (the poorest population) allocated a greater share of their income than higher quintiles. The average daily consumption of cigarettes increased from 7.5 to 9.8 between 1992 and 1998. It was estimated that 90% of "smoker homes" consumed up to one package per day. The proportion of non-filter cigarettes increased from 0.4 to 4.8% between 1992-1998, with a bigger increase in 1996. Finally, it was found that the most important determinants of spending were prices and income. Conclusions. Policies focusing on tobacco prices would help to reduce tobacco consumption and improve the health of the Mexican population.

El consumo de tabaco es uno de los retos más importantes de la Región de las Américas. Lo anterior no sólo por el gran número de muertes que se atribuyen a su consumo, muchas de las cuales son prematuras, sino por el elevado costo económico y social que la atención médica de las enfermedades causadas por el tabaco y las discapacidades que ellas generan, le imponen a los sistemas de salud y a la sociedad en general. En el panorama epidemiológico de la Región se distinguen los países del Cono Sur, como los de más altos valores de consumo, seguidos por los de la Región Andina y México, siendo las naciones centroamericanas y caribeñas las de menores cifras de prevalencia de fumadores. Sólo Estados Unidos de América y Canadá han logrado disminuir la epidemia de tabaquismo, y en el resto de los países se observa una estabilización e, incluso, una tendencia al incremento. La reducción de la edad de inicio del consumo y el aumento en la proporción de mujeres fumadoras también son rasgos actuales del área. En este artículo se revisan las medidas de control del tabaquismo presentes en las legislaciones de los países de América Latina y se analizan las siguientes características: es una región con productores de tabaco de nivel mundial, con estructura de población joven, y débiles y escasos controles. Se impone como necesidad urgente el establecimiento de medidas económicas, poblacionales, legislativas y de apoyo a la investigación en este tema, que reviertan gradualmente el escenario actual del consumo de tabaco. Partiendo de lo analizado anteriormente, se proponen elementos para un programa de acción.Tobacco consumption is one of the most important public health challenges faced in the Americas. This is not only due to the great number of deaths attributable to smoking, many of which are premature, but also to the high economic and social costs of medical care and the burden of disease and disability imposed by tobacco consumption on health systems and on the population. In the regional epidemiologic situation, South American countries are characterized by the highest consumption rates, followed by the Andean region and Mexico; Central American and Caribbean countries have the lowest smoking prevalences. Only the United States and Canada have been able to hold back the smoking epidemic; the rest of the hemisphere shows stable or increasing smoking rates. In the region, age of smoking initiation has decreased and the number of women who smoke has increased. This article reviews the current tobacco control measures in Latin American legislations and analyzes selected regional characteristics such as the structure of young populations, control measures that are weak or scarce, and the world production of tobacco. There is a compelling need to establish economic, population-based, and legislative procedures leading to a gradual reduction of the current tobacco consumption rates. This paper advances a comprehensive action plan against tobacco consumption.

Concerns that chemical exposures in the environment have been detrimental to male sexual development and fertility have been heightened by reports of declining sperm counts over the past 50 years. Marked geographic variation has been found in semen quality and in the incidence of testicular cancer and certain urogenital defects. Debate continues over the existence, magnitude and significance of these trends, and how best to evaluate the hypothesis that in utero and childhood exposures to estrogenic compounds may be to blame. Epidemiologic methods for assessing the impact of hazardous substances on male reproductive health have been developed mainly in the area of occupational medicine, and this paper will review the currently recommended methods. These include questionnaires to determine reproductive history and sexual function; reproductive hormone profiles; and semen analyses such as sperm concentration, motility, and morphology. New research tools that show significant promise from the fields of clinical reproductive medicine and reproductive toxicology are discussed as possible additions to epidemiologic studies, including assays of sperm function and genetic integrity, and biomarkers of DNA damage. For population-based studies involving occupational groups or communities with environmental exposures, issues related to the cost, validity, precision and utility of these methods must be carefully considered.Varios artículos publicados informan acerca de una declinación en la concentración de espermatozoides durante los últimos 50 años; lo anterior ha motivado una preocupación creciente en el sentido de que las exposiciones ambientales a diversos químicos actúen en detrimento del desarrollo sexual y de la fertilidad masculina. Se ha observado una marcada variación geográfica en la calidad del semen y en la incidencia del cáncer testicular y diversas malformaciones urogenitales. Persiste un debate acerca de la existencia, magnitud y significado de estos fenómenos y también acerca de la mejor forma para evaluar la hipótesis de que sus causas son las exposiciones in utero a compuestos estrogénicos. En este trabajo se revisan los métodos epidemiológicos recomendables para evaluar el impacto de sustancias peligrosas sobre la salud reproductiva masculina, varios de los cuales provienen del área de la medicina ocupacional. Se incluyen los cuestionarios para determinar la historia reproductiva y la función sexual; los perfiles de hormonas reproductivas, y los análisis de semen para medir concentración, morfología y motilidad. También se discute la posibilidad de utilizar para la investigación una serie de nuevas herramientas que provienen de la medicina y de la toxicología reproductivas, entre ellas los ensayos de la función espermática y la integridad genética y los biomarcadores de daño al DNA. Se deben tomar cuidadosamente en consideración los aspectos relativos a la utilidad, validez, precisión y costos de estos métodos para la realización de estudios poblacionales, ya sea que involucren grupos ocupacionalmente expuestos y/o comunidades con exposiciones ambientales.

Concerns that chemical exposures in the environment have been detrimental to male sexual development and fertility have been heightened by reports of declining sperm counts over the past 50 years. Marked geographic variation has been found in semen quality and in the incidence of testicular cancer and certain urogenital defects. Debate continues over the existence, magnitude and significance of these trends, and how best to evaluate the hypothesis that in utero and childhood exposures to estrogenic compounds may be to blame. AIDS and human rights are closely related issues. This paper describes the relationship between AIDS and human rights, the impact and consequences of discrimination and the importance of the defense of human rights as a cornerstone strategy in AIDS prevention. Some general ethical aspects are addressed and two dilemmas which have been raised by the epidemic are discussed: the apparent conflict between individual and community rights and the reactions of intolerance and repression from those who claim that only through coercive strategies will the epidemic be brought under control. Specific problems in Mexico are described based on data collected at CONASIDA's Human Rights Department between 1992 and 1994. Finally some conclusions are stated emphasizing that, in the AIDS epidemic, the defense of human rights is the cornerstone of any public health strategy.

Objective: To evaluate the effect of the combination of Metarhizium anisopliae and Gliocladium virens, both with Aqua Reslin Super, on the oviposition, hatching and emergence of Aedes aegypti., Materials and Methods: Evaluations were carried out to determine the effect of treatments impregnated on filter paper and exposed within plastic containers on the oviposition, hatching and emergency of Aedes aegypti., Results: The results indicated that the fungus and insecticide combinations did not affect the oviposition behavior, but if the hatching of the eggs and the adult's emergency., Conclusions: With the results it can be concluded that the combination of fungi + insecticide can be a good option to be applied in oviposition sites with a view to the development of a lethal ovitrap., Competing Interests: Declaration of conflict of interests. This paper discusses the situation of healthcare in Mesoamerica before and immediately after 1519. In the first 50 years after the Conquest, the Spaniards made extensive use of Nahuatl medicine. Objective: To describe the methodological design of the National Health and Nutrition Survey in localities with less than 100 000 inhabitants (Ensanut 100k)., Materials and Methods: The Ensanut 100k is a probabilistic survey that over-represents households with less economic capabilities. This paper describes the scope of the survey, sampling procedures, measurement and inference and logistics organization., Results: 10 461 home interviews and 26 161 individual interviews were obtained. The household response rate was 89 and 92% from individuals., Conclusions: The probabilistic design of the Ensanut 100k allows to make valid statistical inferences about parameters of interest for public health in localities with less than 100 000 inhabitants, localities where 52% of the population lives according to the 2010 census. This paper describes the creation of the legal framework and the origin, growth and consolidation of the institutions and interventions (initiatives, programs and policies) that nourished public health in Mexico in the past century. In the Mexican health scenario, the hepatitis C virus epidemic is present, along with its comorbidities and premature mortality. Acting immediately will allow its containment in short term with the proper implementation of the current available tools for prevention, diagnosis and highly effective pharmacological therapies. The Mexican Ministry of Health requested the National Institute of Public Health to constitute a group of independent, free of conflict-of-interest academic experts on front-of-pack labelling (FOP). This group was instructed to created a positioning paper to contribute to the development of a FOP system for industrialized products that offers useful information for purchase decision making. This position paper uses the best available scientific evidence, and recommendations from experts of international organizations. The FOP proposal focuses on the contents of energy, nutrients, ingredients and components that if consumed in excess on the diet, can be harmful to people's health, such as added sugars, sodium, total fat, saturated fat and energy. The academic expert group recommends the implementation of a FOP that provides an easy way to quickly assess the quality of a product. Objective: To determine the impact of Seguro Popular (SPS) on catastrophic and impoverishing household expenditures and on the financial protection of the Mexican health system., Material and Methods: The propensity score matching (PSM) method was applied to the population affiliated to SPS to determine the program's attributable effect on health expenditure. This analysis uses the National Household Income and Expenditure Survey (ENIGH) during 2004-2012, conducted by Mexico's National Institute of Statistics andGeography (INEGI)., Results: It was found that SPS has a significant effect on reducing the likelihood that households will incur impoverishing expenditures. A negative effect on catastrophic expenditures was also found, but it was not statistically significant., Conclusions: This paper shows the effect that SPS, in particular health insurance, has as an instrument of financial protection. Objective: People with intellectual developmental disorders (IDD) have worse health statuses in comparison with general population. The objective of this paper is to compare access and hospital morbimortality in people with IDD and general population., Material and Methods: We conducted a retrospective cross-sectional analytical study and analyzed data on admissions and discharges between IDD patients and the rest of them, in Ciudad Real, España., Results: Out of 51 325 hospital admissions, 441 (0.9%) belonged to the group of persons with IDD. The IDD group had fewer programmed hospitalization than the general population and fewer surgical interventions. They presented more admissions for mental disorders and respiratory system diseases., Conclusions: The data presented confirm TDI population have different patterns of disease. Furthermore, this study reveal potential difficulties in access to health care in this population.

Recent literature indicates that people with Disorders of Intellectual Development (DID) experience health disparities in the pathologies that they present, and a worst access to health care. However, current evidence-based knowledge is still sparse outside the Anglo-Saxon countries. The POMONA-I and POMONA-II European projects aimed to collect information on the health status of people with DID in Europe. The POMONA-ESP project in Spain is meant to collect health information in a wide and representative sample of persons with DID. Also, there are studies that claim for the need of specialized services for people with DID at the public health system. There are also studies about the current state of the education and training about DID for students within the health sector. In this paper we review the latest evidences about the health of the persons with DID and we present the main research activities and care initiatives about this issue.

The decreasing breastfeeding rate in México is of public health concern. In this paper we discus an innovative regulatory approach -Performance Based Regulation- and its application to improve breastfeeding rates. This approach, forces industry to take responsibility for the lack of breastfeeding and its consequences. Failure to comply with this targets results in financial penalties. Applying performance based regulation as a strategy to improve breastfeeding is feasible because: the breastmilk substitutes market is an oligopoly, hence it is easy to identify the contribution of each market participant; the regulation's target population is clearly defined; it has a clear regulatory standard which can be easily evaluated, and sanctions to infringement can be defined under objective parameters., Recommendations: modify public policy, celebrate concertation agreements with the industry, create persuasive sanctions, strengthen enforcement activities and coordinate every action with the International Code of Marketing of Breast-milk Substitutes.

The right to health and its operational form, as an organized social response to health: the right to health protection, are the mainstay for the global push towards universal health coverage. The path to achieve this goal is particular to each country and relates to the baseline and specific context in relation to what is feasible. In practical terms, universal coverage involves the correlation between demand and supply of services (promotion, prevention, and care), expressed by the ability for each individual to make use of services when these are required. In those terms universal coverage is then effective access. The objective of the paper is to explore the conceptualization of effective access to health services and propose a definition that allows its operationalization thereof. This definition considers key elements of supply and demand of services, including the availability of resources and adequate provision (quality), as well as barriers to use them.

Listeriosis is caused by Listeria monocytogenes, an important food-borne disease due to its clinical forms, high mortality rate, and the economic impact in both clinical and food production industries. In Mexico, the lack of epidemiological surveillance systems leads to the need of accurate data on the incidence of listeriosis and its association with food-borne disease. In this paper, we present data about the presence of this bacterium in food, reports related to clinical cases of listeriosis, and information of diseases in which L. monocytogenes may be involved. However, in most of these cases the etiology was not established. Given this, there's a need to inform and warn the appropriate entities, to define strategies for the mandatory search of L. monocytogenes through the whole food production chain and clinical suspects, for the epidemiological importance and control of listeriosis in Mexico.

Chikungunya fever (CHIK) is a viral disease transmitted to human beings by the same vector as dengue -the Aedes mosquito. Besides fever and severe pain in the joints, it produces other symptoms such as myalgias, headache, nausea, fatigue and exanthema. There is no specific treatment for it; the therapeutic management of patients focuses on symptom relief. Historically, outbreaks of large proportions have been reported; even since 2010 it was considered to be a potential emerging epidemic. In 2013 it was introduced into the islands of the Caribbean, and it has recently been reported in the American continent. This paper describes the first confirmed case of chikungunya in Mexico -in the municipality of Tlajomulco de Zúñiga, Jalisco, in May, 2014-, which was imported from the Caribbean island of Antigua and Barbuda by a 39 year-old woman.

Objective: . This paper aims at assessing the contribution of Chile's Health Care Integrating Councils (CIRA, Spanish acronym) to strengthening governance in health., Materials and Methods: A literature review on the official documents related to the process of creation and development of CIRA was carried out; an ad hoc questionnaire was applied to all 29 health services of the country; finally, 35 semi-structure in-depth interviews were carried on a sample of six CIRA., Results: The CIRAs have become a tool for functional integration and a valuable space for dialogue, cooperation and learning for all of the actors of the Chilean public health network., Conclusions: In this study, we conclude that there is room for improvements of CIRA's role regarding governance of the health care network as long as CIRA is authorized to deal with strategic topics, such as investment in infrastructure, technology and human resources, and budgeting.

In this paper we discuss the recent efforts to modernize the School of Public Health of Mexico (ESPM). In the first part we analyze the conditions in which the ESPM operated at the beginning of the 1980s. In part two we describe the changes introduced in the ESPM between 1983 and 1986, up until its incorporation into the National Institute of Public Health (INSP). In part three we discuss the conceptual renovation which built the platform for the new academic programs introduced by the School between 1987 and 1995. In the fourth part we discuss the construction of the new facilities of INSP and the process of moving to the city of Cuernavaca. The main message of this paper is that the transformations witnessed by the ESPM in the final decades of the past century turned it into a true academic institution which uses research as the motor for training of human resources in public health.

Objective: This paper presents the design and application in Cuba of a social construct to identify the social dimensions associated with discrimination attitudes towards people with positive diagnosis to HIV., Materials and Methods: A combination of qualitative and quantitative methodology was used for the measurement and validation of a construct, and also the K-mean procedure and factor analysis techniques. All these techniques were applied to "Survey on HIV prevention indicators" 2005, 2006 and 2009., Results: A tool for the identification of the most frequent discriminatory attitudes in a specific social context was validated. This instrument allows clustering the population into different typologies, by level of discrimination, and also, to distinguish some social dimensions of this phenomenon., Conclusions: The results allow distinguishing frequent discriminatory attitudes in any population, focusing the actions in the direction of gradually reducing them, and to evaluate the temporal behavior of this phenomenon.

Unlike many other Latin American countries Mexico has no coherent and explicit national pharmaceutical policy (NPP). Other national challenges are: high out-of-pocket expenditure on medicines despite the implementation of universal health care coverage through Seguro Popular, high prices of medicines in the private sector in comparison to other countries when adjusted for income level, and the lack of clear strategies to improve safety and efficiency in the use of medicines, in particular interventions aimed at private physicians, pharmacies and consumers. The aim of this paper is to describe the challenges and opportunities to (1) consolidate the processes of formulating, implementing and evaluating NPP, and (2) define the policy content with regard to access and use of medicines.

Objective: This paper focuses on public and private healthcare utilization among dependents living in Mexico of Mexican migrants in California, analyzing the link between remittances and enrollment in Seguro Popular, a social health insurance plan., Materials and Methods: We surveyed 1353 migrants who visited the Mexican consulate of Los Angeles in 2010., Results: 53.9% sent remittances; 72.2% of households receiving remittances used a share of remittances for health care and 74.4% of them were covered by Seguro Popular. The annual median with private health care expenditure was USD 825, compared to USD 293 for public providers. The main predictors remittances utilization for healthcare were having a sick dependent, purchase of prescription drugs, experiencing problems paying for health care and time of U.S. residence., Conclusions: Seguro Popular increases healthcare utilization with public providers, which provides an opportunity to reallocate the use of migrant's remittances for health purposes.

This paper describes a plan for the attention of addictions from a public health perspective; it defines strengths of this perspective and its limitations to face the special challenges that the problem represents for social policies; adopts a wider perspective that includes the integration of health and social issues in measuring policy results having as a main aim the benefit of the person and of the communities with a perspective of the populations well being. It describes the challenge the country faces as producer, route of traffic and rates of consumption, analyzes briefly the evidence of public policies and makes a series of recommendations.

The reforms made in recent years to the Mexican Health System have reduced inequities in the health care of the population, but have been insufficient to solve all the problems of the MHS. In order to make the right to health protection established in the Constitution a reality for every citizen, Mexico must warrant effective universal access to health services. This paper outlines a long-term reform for the consolidation of a health system that is akin to international standards and which may establish the structural conditions to reduce coverage inequity. This reform is based on a "structured pluralism" intended to avoid both a monopoly exercised within the public sector and fragmentation in the private sector, and to prevent falling into the extremes of authoritarian procedures or an absence of regulation. This involves the replacement of the present vertical integration and segregation of social groups by a horizontal organization with separation of duties. This also entails legal and fiscal reforms, the reinforcement of the MHS, the reorganization of health institutions, and the formulation of regulatory, technical and financial instruments to operationalize the proposed scheme with the objective of rendering the human right to health fully effective for the Mexican people.

Depression is an important public health problem. It is the fourth cause of disease in the world in terms of lost years of healthy life. In Mexico, it ranks first in terms disability for women and ninth for men. There is a high comorbidity between depression and other mental disorders such as anxiety and substance abuse, as well as other serious and chronic physical conditions (e.g. diabetes, and heart disease). Despite the impact of depressive disorders in the quality of life of the population, there is a large proportion of people who don't get treatment, delaying seeking help and thus don't receive adequate assistance. The aim of this paper is to present an analysis of depression status in the Mexican population from a public health perspective; it includes prevalence and associated factors, gaps in care, characteristics of the use of services and treatments available. The paper concludes with a presentation of the implications for research and mental health policy in Mexico.

Objective: To determine the prevalence of congenital cytomegalovirus infection in newborn infants included in the neonatal screening program coordinated by the State Health Services in San Luis Potosí., Material and Methods: We evaluated the presence of cytomegalovirus in blood samples stored in filter paper., Results: Cytomegalovirus was detected in 10 (0.68%) of the 1,457 samples included in the study. There were no differences in the characteristics of infants with congenital infection compared to those without infection., Conclusions: It is necessary to increase awareness of health professionals regarding the prevalence and impact of congenital cytomegalovirus infection.

Objective: This paper critically examines recent work on qualitative health research (QHR) in Mexico., Material and Methods: A review was conducted of 128 articles published between 2000 and 2009. A literature search was done drawing together a verse of indexes, search engine, data bases and citations, as well as interviewing researchers. A frequency and content analysis was carried out., Results: QHR has expanded significantly, both nationally and internationally, and papers published in public health and social medicine journals. However, several factors impede its consolidation: the institutional, territorial and thematic concentration of studies, the dependency on grounded theory, the selective nature of the research topics and populations; and its marginal scientific impact., Conclusions: QHR has grown in México, but its consolidation faces serious challenges. Several proposals are discussed that could strengthen the field.

The discovery of genes BRCA1 and BRCA2 has led to the introduction of genetic tests more complex every time for the evaluation of the hereditary cancer risk, among others. In the present paper we explore the criteria to decide when to run the testing for the genes, as well as the implications for the treatment of patients who are identified with them.

This paper describes the health conditions in Nicaragua and discusses the characteristics of its national health system including its structure and coverage, its financial sources its physical, material and human resources the stewardship functions developed by the Ministry of Health the participation of citizens in the operation and evaluation of the system and the level of satisfaction of health care users. It also discusses the most recent policy innovations, including the new General Health Law, the decentralization of the regulation of health facilities and the design and implementation of a new health care model known as Family and Community Health Model.

This paper describes the health conditions in Guatemala and, in more detail, the characteristics of the Guatemalan health system, including its structure en coverage, its financial sources, the stewardship functions developed by the Ministry of Health, as well as the generation of health information and the development of research activities. It also discusses the recent efforts to extend coverage of essential health services, mostly to poor rural areas.The most recent innovations also discussed in this paper include the Program for the Expansion of Coverage of Essential Services, the Program to Expand Access to Essential Drugs and the agreements between the Ministry of Health and several non-governmental organizations to provide essential services in rural settings.

This paper describes the Mexican health system. In part one, the health conditions of the Mexican population are discussed, with emphasis in those emerging diseases that are now the main causes of death, both in men and women: diabetes, ischaemic heart disease, cerebrovascular diseases and cancer. Part two is devoted to the description of the basic structure of the system: its main institutions, the population coverage, the health benefits of those affiliated to the different heath institutions, its financial sources, the levels of financial protection in health, the availability of physical, material and human resources for health, and the stewardship functions displayed by the Ministry of Health and other actors. This part also discusses the role of citizens in the monitorization and evaluation of the health system, as well as the levels of satisfaction with the rendered health services. In part three the most recent innovations and its impact on the performance of the health system are discussed. Salient among them are the System of Social Protection in Health and the Popular Health Insurance. The paper concludes with a brief analysis of the short- and middle-term challenges faced by the Mexican health system.

This paper describes the health conditions in Peru and, with greater detail, the Peruvian health system, including its structure and coverage, its financial sources, its physical, material and human resources, and its stewardship functions. It also discusses the activities developed in the information and research areas, as well as the participation of citizens in the operation and evaluation of the health system. The article concludes with a discussion of the most recent innovations, including the Comprehensive Health Insurance, the Health Care Enterprises system, the decentralization process and the Local Committees for Health Administration. The main challenge confronted by the Peruvian health system is the extension of coverage to more than I0% of the population presently lacking access to basic health care.

This paper describes the Chilean health system, including its structure, financing, beneficiaries, and its physical, material and human resources. This system has two sectors, public and private. The public sector comprises all the organisms that constitute the National System of Health Services, which covers 70% of the population, including the rural and urban poor, the low middle-class, the retired, and the self-employed professionals and technicians.The private sector covers 17.5% of the population, mostly the upper middle-class and the high-income population. A small proportion of the population uses private health services and pays for them out-of-pocket. Around l0% of the population is covered by other public agencies, basically the Health Services for the Armed Forces. The system was recently reformed with the establishment of a Universal System of Explicit Entitlements, which operates through a Universal Plan of Explicit Entitlements (AUGE), which guarantees timely access to treatment for 56 health problems, including cancer in children, breast cancer, ischaemic heart disease, HIV/AIDS and diabetes.

This paper describes the Bolivian health system, including its structure and organization, its financing sources, its health expenditure, its physical, material and humans resources, its stewardship activities and the its health research institutions. It also discusses the most recent policy innovations developed in Bolivia: the Maternal and Child Universal Insurance, the Program for the Extension of Coverage to Rural Areas, the Family, Community and Inter-Cultural Health Model and the cash-transfer program Juana Azurduy intended to strengthen maternal and child care.

This paper describes the health system of Argentina.This system has three sectors: public, social security and private.The public sector includes the national and provincial ministries as well as the network of public hospitals and primary health care units which provide care to the poor and uninsured population. This sector is financed with taxes and payments made by social security beneficiaries that use public health care facilities. The social security sector or Obras Sociales (OS) covers all workers of the formal economy and their families. Most OS operate through contracts with private providers and are financed with payroll contributions of employers and employees. Finally, the private sector includes all those private providers offering services to individuals, OS beneficiaries and all those with private health insurance.This sector also includes private insurance agencies called Prepaid Medicine Enterprises, financed mostly through premiums paid by families and/or employers.This paper also discusses some of the recent innovations implemented in Argentina, including the program Remediar.

This paper describes the Venezuelan health system, including its structure and coverage, financial sources, human and material resources and its stewardship functions. This system comprises a public and a private sector. The public sector includes the Ministry of Popular Power for Health (MS) and several social security institutions, salient among them the Venezuelan Institute for Social Security (IVSS). The MH is financed with federal, state and county contributions. The IVSS is financed with employer, employee and government contributions. These two agencies provide services in their own facilities. The private sector includes providers offering services on an out-of-pocket basis and private insurance companies. The Venezuelan health system is undergoing a process of reform since the adoption of the 1999 Constitution which calls for the establishment of a national public health system. The reform process is now headed by the Barrio Adentro program.

This paper describes the Brazilian health system, which includes a public sector covering almost 75% of the population and an expanding private sector offering health services to the rest of the population. The public sector is organized around the Sistema Único de Saúde (SUS) and it is financed with general taxes and social contributions collected by the three levels of government (federal, state and municipal). SUS provides health care through a decentralized network of clinics, hospitals and other establishments, as well as through contracts with private providers. SUS is also responsible for the coordination of the public sector. The private sector includes a system of insurance schemes known as Supplementary Health which is financed by employers and/or households: group medicine (companies and households), medical cooperatives, the so called Self-Administered Plans (companies) and individual insurance plans.The private sector also includes clinics, hospitals and laboratories offering services on out-of-pocket basis mostly used by the high-income population. This paper also describes the resources of the system, the stewardship activities developed by the Ministry of Health and other actors, and the most recent policy innovations implemented in Brazil, including the programs saúde da Familia and Mais Saúde.

This paper describes the health conditions in Ecuador and, in more detail, the characteristics of the Ecuadorian health system, including its structure and coverage, its financial sources, the physical, material and human resources available, and the stewardship activities developed by the Ministry of Public Health. It also describes the structure and content of its health information system, and the participation of citizens in the operation and evaluation of the health system. The paper ends with a discussion of the most recent policy innovations implemented in the Ecuadorian system, including the incorporation of a chapter on health into the new Constitution which recognizes the protection of health as a human right, and the construction of the Comprehensive Public Health Network.

Since the Salud Mesoamerica 2015 initiative (SM-2015) aim is to improve health and nutrition conditions of those most vulnerable in Mesoamerica, the goal of the evaluation is to generate evidence of the joint effectiveness of a package of interventions designed to improve the health conditions. We propose a mix design for the evaluation, which will allow to know the magnitude of changes attributable to the interventions, as well as the meanings of these changes for the target population, taking into account the specificities of each country. The main axis of this design is a locality panel where information about individuals, households, and health facilities (first and second level) will also be collected. The evaluation design described in this paper was developed between June and December, 2009, and it was integrated during workshops in Cuernavaca (Mexico), Managua (Nicaragua), and San Jose (Costa Rica). The proposed design will allow to generate evidence about the joint effectiveness of the package of interventions proposed for the SM-2015. The success of this design rests on the political commitment of countries and donors.

This paper describes the health conditions in Cuba and the general characteristics of the Cuban health system, including its structure and coverage, its financial sources, its health expenditure, its physical, material and human resources, and its stewardship functions. It also discusses the increasing importance of its research institutions and the role played by its users in the operation and evaluation of the system. Salient among the social actors involved in the health sector are the Cuban Women Federation and the Committees for the Defense of the Revolution. The paper concludes with the discussion of the most recent innovations implemented in the Cuban health system, including the cardiology networks, the Miracle Mission (Misión Milagro) and the Battle of Ideas (Batalla de Ideas).

This paper describes the health conditions in El Salvador and the main característics of the Salvadoran health system, including its structure and coverage, its financial sources, the physical, material and human resources available, the stewardship functions developed by the Ministry of Public Health, and the participation of health care users in the evaluation of the system. It also discusses the most recent policy innovations including the approval of the Law for the Creation of the National Health System, which intends to expand coverage, reduce health inequalities and improve the coordination of public health institutions.
