Over the last 20 years, the development of meta-analysis has been aimed at obtaining objective synthesis of the available results on specific research questions. The main achievements of meta-analysis include the application of techniques to perform systematic literature searches and to obtain unbiased selection of studies, data extraction and pooled estimates of effect. This paper discusses the methodologic steps to follow when conducting a meta-analysis, with emphasis on study selections, data collection and statistical methods to combine the results from individual studies. We also present a set of guided questions as an aid to critically evaluate the conclusions of published meta-analyses. The application of meta-analytic techniques to cardiology is illustrated using a meta-analysis of the randomized controlled trials of angioplasty versus bypass surgery in the management of patients with ischemic heart disease.

Even if arterial hypertension (HT) is a process of multiorganic involvement, it is nevertheless in practice, one of the most powerful factors for the development of cardiovascular complications that increases very seriously, both cardiovascular morbidity and mortality. Paradoxically, in Spain, a significant proportion of cardiologists have neglected for many years this disease, at least, at its initial phase, acting mainly, only when the usual and severe cardiovascular complications of HT were clinically well established. Currently, this attitude is changing in such a way, that in the near future, the cardiologist's commitment to HT, will be definitely engaged in both; basic research and clinical aspects. The Spanish Hypertension Working Group, has worked out in an inquiry sent to all Spanish cardiologists, in order to know their point of view in relation to the main aspects involving HT and cardiological practice. This paper collects the results of the answers given anonymously by the doctors.

In the last few years the has been an enormous development in noninvasive testing in the field of clinical cardiology. In fact, excellent monographs on each one of these techniques have been published elsewhere, but fewer publications exist that treat the topic of their indications and use in an integrated way, except for in the most common clinical situations. In this paper, the treatment of patients who present chest pain, stable and unstable angina is discussed, including the study of postinfarction patients. Furthermore, the role of noninvasive tests in the detection of coronary heart disease in women and in patients with left bundle branch block is thoroughly analyzed; as well as their usefulness after surgical or percutaneous coronary revascularization and in patients with peripheral vascular disease.

Intracoronary stents are, without any doubt, a major breakthrough in interventional cardiology. Their widespread use has expanded to more difficult anatomical situations and the search for more suitable stents continues to grow. We review, in this paper, the technical characteristics of stents that are currently approved or in clinical investigation. We have also reviewed the role of intravascular ultrasound in the study of the anatomical characteristics of plaque, the length of the lesion, and their influence of the stent selection and the ultrasound influence on the determination of appropriate stent expansion. After reviewing the current role of the intracoronary stent, we tried to look for the most appropriate stent in three "unconventional" anatomical situations: long and bifurcated lesions, lesions containing thrombus and saphenous vein aortocoronary bypass grafts. In conclusion, the intracoronary stent plays a major role in interventional cardiology. The second and third generation stents are more suitable for "specially difficult" situations, but there are some lesions such as bifurcations where the is not yet a definitive solution.

Exercise testing still plays an important role in the management of patients with chronic ischemic heart disease, not only in the diagnosis but also in their prognostic and functional evaluation, and in the assessment of effects of the therapy. Moreover, ambulatory electrocardiography, signal averaged electrocardiography and heart rate variability provide useful information for certain groups of patients. This paper reviews the rationale, methodology and indications of the above mentioned procedures applied to chronic ischemic heart disease, with special reference to exercise testing. Recommendations for their use in clinical practice are also made.
