Objectives/hypothesis: Treatment of non-squamous cell carcinoma sinonasal malignancies (NSCCSMs) typically involves surgery and radiotherapy (RT), but optimal sequencing remains controversial.Study Design: Retrospective chart review.Methods: Patients with NSCCSM treated with combined surgery and RT between 2000 and 2011 were identified. Margin control, overall survival, disease-free survival, local recurrence-free survival, and regional recurrence-free survival were compared between neoadjuvant and adjuvant RT groups.Results: Eight-four patients were included (23 neoadjuvant and 61 adjuvant RT). A higher proportion of patients receiving neoadjuvant RT achieved negative/close resection margins compared to those receiving adjuvant RT (83% vs. 41%, P = .003). Multivariable analysis also showed that neoadjuvant RT was associated with an 81% decreased odds of positive margins odds ratio: 0.19, 95% confidence interval: 0.05-0.77, P = .02).Conclusions: Neoadjuvant RT may be associated with improved margin status among patients with NSCCSM treated with surgery and RT. If patients experience equivalent outcomes with and without postoperative antibiotics, then reducing routine use could positively impact antibiotic-resistance trends and healthcare costs, while avoiding the systemic side-effects of antibiotics. There was also no significant difference in postoperative sinus infection rate between the antibiotic and placebo groups, although the rate of diarrhea was significantly higher in the antibiotic group, at 24.3% compared to 2.5% in the placebo group ( I P i = .02). Objectives/Hypothesis: A 1-day intensive course (bootcamp) was developed, to teach junior otolaryngology-head and neck surgery (OTO-HNS) residents emergency procedural skills, clinical reasoning, and communication skills. This learning paradigm utilized a number of novel task trainers, panel discussions, and emergency simulations. The study objective was to assess the educational value of this bootcamp. Study Design: Prospective cohort survey. Methods: Residents were recruited from regional teaching centers within a 2000-km radius of the simulation center. Preceptors fluent in English and in French were in attendance. Pre-bootcamp outcome measures included the Kolb Learning Style Inventory and a self-administered survey measuring confidence levels in performing specific OTO-HNS tasks. Postbootcamp outcome measures included a survey evaluating the bootcamp experience and a structured follow-up telephone interview. Results: Twenty-eight residents participated in the bootcamp from across the United States and Canada. When asked if they felt that "overall, the educational day was an effective learning process," the average score was 4.75/5.0. The vast majority of participants (92.9%) felt they would recommend the bootcamp to a future junior resident. Kolb learning styles that prefer active experimentation (acting, initiating, and deciding) were more common than those that utilize reflective observation (imagining, analyzing, reflecting), which favors a hands-on model of learning. Conclusion: This first Canadian OTO-HNS bootcamp demonstrated the feasibility and effectiveness of conducting a centralized bootcamp for regional training centers spanning multiple states/provinces and languages. It further states that the review includes one meta-analysis, two randomized controlled trials, one retrospective case-control study, and one retrospective case series.

The article presents studies which examine the use of bevacizumab to reduce epistaxis in hereditary hemorrhagic telangiectasia (HHT). A 2009 study showed that the frequency of epistaxis, impact on social life and disability were all improved in the potassium titanyl phosphate (KTP) and bevacizumab group at one month postsurgery. A 2014 placebo-controlled trial found a trend toward lessened epistaxis in bevacizumab and saline injection using a visual analogue scale.

Topical tranexamic acid is used to treat epistaxis. We reviewed the evidence for this practice, and found based on the current literature, it may be a useful adjunct in managing epistaxis., (© 2024 The Author(s).
