This study explores the ways in which African Caribbean families communicate with each other and the outside world in the context of post migration. The Big and Small Story in narrative inquiry was used to make sense of the data as it pays attention to the manner and style in which this story is told, and what identities are being claimed post migration. The key findings are set out using three archetypal positions: the trickster, passer/conformer and resister/revolutionary. These were used to capture different ways people responded to power and identities claimed within their daily life. Findings in part of reflect Du Bois' concept of double consciousness to consider the way black people carry out their negotiations with power. These African Caribbean participants were having to think about what is acceptable to the power base and how can this be negotiated. This paper reports a collaborative project, developed jointly by a group of community mental health workers in association with a specialist team commissioned to initiate family work throughout a central London mental health provider. The result of this project – The ‘Mental Health Matters Workshops’ – was a series of day workshops for patients who had experienced a major mental illness, and their families, carers and mental health workers. The positive impact of the workshops on attenders is reported, as well as the development of a ‘multi-family’ and social network culture. [ABSTRACT FROM AUTHOR]

This paper is an account of the author's evolving practice in working with Bangladeshi families in the East End of London. It considers first of all the circumstances of such families: their socio-economic situation, and their beliefs about family life, mental illness and seeking help. Then it describes two case examples illustrative of those styles of working and techniques of intervention from different family therapy approaches which have proven most applicable to working with families from this community. This paper describes the integration of structural family therapy and systemic consultation into an in-patient paediatric unit. Two case examples illustrate the approach used. It is concluded that six stages are involved in the successful integration of a systemic approach: planning and convening an initial staff meeting, developing a structural hypothesis, enlisting hierarchical support in the ward system, holding the family meeting, reconvening the professional team and disengaging the family and ward system. The Family Day Unit at the Marlborough Hospital, London, carries out an intensive treatment programme for highly dysfunctional families. Up to ten families attend the Unit five days per week from nine a.m. to three p.m. They stay for a period of at least three months and not longer than fifteen months. Normally some twenty to thirty patients, adults and children, attend every day. This paper gives the reasons for setting up this Family Day Unit. This study presents the results of a randomized treatment trial of family therapy and two control treatments for 119 outpatient opiate users. All treatments were combined with a methadone reduction programme. The control treatments were: (1) a ‘standard’ treatment (supportive psychotherapy) and (2) a ‘low contact’ intervention. Treatment outcome was evaluated six and twelve months after the initial assessment. Both the family therapy and minimal intervention groups had a significantly higher number of drug–free days at six and twelve months, compared to the standard treatment, despite receiving fewer treatment sessions. Across all treatments there was evidence for a gender difference in response to therapy in users who were in a couple relationship. In couples where both partners were using drugs women did significantly better than men. Men living with a non–drug–abusing partner fared better than men living with a drug–using partner. Across the treatment groups diminution in drug use was accompanied by improvements in psychosocial functioning. Unemployment, sharing needles and injecting drugs were predictive of poor outcome.
