Unselected type I diabetics were hospitalized for group training and treatment programme during which blood-sugar was self-monitored with the Haemo-Glukotest 20-800; quality of self-monitoring improved appropriately. The patients had taken part in either the standard training programme (67 patients) or in the "intensive" training programme (101 patients). At the beginning of the training week, after the first theoretical and practical instructions, the quality of blood-sugar self-monitoring was equally poor among patients of both programmes. At the end of the "standard" training week, self-monitoring results were still unsatisfactory, though slightly better than at the onset. Because of these poor results, the training programme was intensified. Optimal quality of self-monitoring was then achieved at the end of the training period. In 69 others, also unselected, type I diabetics the quality of self-monitoring was determined 14 months after their participation in the training programme. Using the Haemo-Glukotest 20-800 (57 patients), self-monitoring gave excellent results; using a blood-glucose measuring instrument (12 patients) it was slightly worse. Assuming intensive training, even unselected type I diabetics can obtain accurate blood-sugar measurements using test-strips.

Plasma glucagon levels were measured in 129 grossly obese patients with 77 +/- 29% excess weight according to Broca. A significantly raised basal glucagon level (179 +/- 7 pg/ml) was found when compared with normal weight controls (109 +/- 7 pg/ml). Weight reduction of 9.1 +/- 3.8 kg with reduction of glucose-stimulated hyperinsulinaemia from 97 +/- 12 microU/ml to 62 +/- 6 microU/ml had no influence on hyperglucagonaemia. A subgroup of obese patients with hypertriglyceridaemia (270 +/- 24 mg/dl) and glucose intolerance had particularly high plasma glucagon levels (198 +/- 12 pg/ml). It seems that these patients have a resistance to glucagon in addition to a marked insulin resistance.

In a study extending over five years 35 of 70 excessively overweight patients with subclinical diabetes mellitus who had not significantly changed their weight, demonstrated a further deterioration in glucose tolerance, and manifest diabetes occurred in ten. In the remaining 35 patients who had achieved 20% weight reduction from the initial level glucose tolerance had become normal. The two groups were similar as to age, initial weight, and original degree of glucose intolerance. There was a significant correlation between loss of weight and increased glucose tolerance. No such correlation occurred with subgroups of patients whose initial overweight was more than +100 relative percentage and who had been over 50 years old at the start of the study.

A long-term study of 63 extremely obese persons with disturbed glucose tolerance showed a further deterioration of glucose tolerance over 10 years. In 9 out of 23 patients manifest diabetes mellitus developed despite partial weight reduction (greater than or equal to 10% of the initial weight excess). Out of 14 patients who did not lose weight during this time even 11 developed manifestations of diabetes. All patients who were in different weight groups at the 5- and 10-year follow-up showed a deterioration in glucose tolerance after 10 years. In the individual groups there were only minimal differences in other characteristics apart from the different weight behaviour.

Objective data on duration of hospital stay and of incapacity to work were collected for 137 type I diabetics who had taken part in a structured diabetes treatment and training scheme. Before the training period the average hospital stay had been 0.76 per patient per year, duration per stay 21 days, number of episodes of work incapacity 1.6 per patient per year, duration of incapacity 23.6 days. For a subgroup of patients (62) the influence of the training programme could be demonstrated: hospital stay fell to 0.49 per patient per year, duration per stay to 12.0 days, number of incapacity episodes to 1.4 per patient per year, and the days lost from work to 12.1 per patient per year.

The dietary habits and metabolic status of 14 selected ambulant type-1 diabetics, who were previously treated with conventional insulin therapy, were investigated over a period of 2 years under intensified insulin injection therapy. In this, multiple doses of regular insulin in combination with intermediate-acting insulin were injected daily and the regular metabolic controls as well as adjustment of the insulin dosage were undertaken by the patients themselves. Whereas conventional insulin therapy made exclusive use of intermediate-acting insulin, 41% of the daily dose in the intensified insulin injection therapy was regular insulin. The total daily insulin dose of 51 +/- 12 U/d was therefore not significantly different from that in the conventional insulin therapy (49 +/- 10 U/d). During the intensified insulin injection therapy the usual diabetic diet was relaxed in several aspects: neither the daily intake of fat, protein and calories, nor substitution in the diet using exchange lists was specified. Despite this liberalization HbA1c values decreased significantly from 9.3 +/- 1.2% to 7.8 +/- 0.7% (P less than 0.05) and blood lipids remained in the normal range. The results of this retrospective investigation lead to the conclusion that despite some measure of diet liberalization, selected, trained, type-1 diabetics of normal body weight and without primary dyslipoproteinaemia can attain good metabolic control under intensified insulin injection therapy.

Diabetes-specific hand alterations (cheirarthropathy) were observed in 44 out of a total of 215 children and adolescents with insulin-dependent diabetes. In 28 of them limitations of movement in the proximal or (and) distal interphalangeal joints of one finger, and in the remaining 16 patients changes of at least two fingers were seen. Hand and shoulder joints or the vertebral column were not affected. In a control group of 110 probands without metabolic disorder only five showed cheirarthropathy. The frequency of diabetes-specific alterations of the hand increased with the age of the patients, the duration of diabetes and insulin requirements. In particular, the quality of metabolic control is of paramount importance for the as yet unclear pathogenesis of cheirarthropathy.

Analysis of 31 686 patient records for obesity among hospitalised patients in the Second Medical Clinic of Düsseldorf University between 1952 and 1972 demonstrated a steep rise to a plateau of 24 +/- 2% in 1956-1972. Adding a corresponding analysis from the Bonn Medical University Clinic some 66 000 hospitalised patients could be included for a period of 40 years. Socio-economic conditions in the war and post-war period produced a marked fall in obesity to a minimum of 2% in 1946. The mean incidence before 1935 was 10%, by 1958 it had increased two-and-a-half fold. It is thought that this had been due not to a corresponding higher calorie intake by the population but rather a general reduction in physical activity. The increase in obesity among the population, reflected in these hospital statistics, is likely to lead to a parallel increase in cardiovascular risk factors and with it the ever-rising curve of death from cardiovascular disease in the Federal Republic of Germany.

The effect of human NPH insulin (Protaphan HM) and of human ultralente insulin (Ultratard HM) on the profile of blood-glucose concentration was compared, using the glucose clamp technique, in normal subjects after subcutaneous injection of 12 U each. After Protaphan, insulin concentration rose by 11.0 +/- 1.6 microU/ml; maximal glucose infusion rate (GIR) after five hours was 4.7 +/- 0.5 mg/kg.min. But after the same dosage of Ultratard, insulin concentration rose by only 4.7 +/- 1.0 microU/ml (P less than 0.02) on a maximal GIR after ten hours of 3.2 +/- 0.5 mg/kg.min (no significant difference). Ultratard had up to 70% of the maximal Protaphan effect, the infused amount of glucose, up to maximal effect at 9.5 h (72 +/- 9 g), being significantly lower (P less than 0.02) than after Protaphan (130 +/- 18 g). After, respectively, 16 (Protaphan) and 20 (Ultratard) hours, basal insulin levels were again reached. GIR 19 h after Protaphan injection was 1.3 +/- 0.4 mg/kg.min, while 24 h after Ultratard it was 1.5 +/- 0.2 mg/kg.min, which was not significantly different from the basal rate. The difference between Protaphan and Ultratard lies in the marked initial effect of NPH insulin, up to 10 h after injection. In the later phase of the effect profile both insulins were similar at the chosen dose of 12 U.

The long-term results of total fasting during hospitalisation were evaluated an average of 21 months after the end of the hospital stay in 42 patients treated for obesity between 1971 and 1975. Further weight reduction occurred in 14, while the remainder had regained some weight. Nine of the latter had, in fact, surpassed their prehospitalisation weight. Neither age not sex, social status, time interval since fasting had any influence on the long-term results of total fasting. It is concluded that, in view of the poor correlation between cost and long-term result of such total-fasting regimen, it should be undertaken in this form only exceptionally.
