Acquired Neurogenic Communication Disorders: An Integrated Clinical Approach provides an overview of acquired neurogenic communication and swallowing disorders for undergraduate courses in communication sciences and disorders programs. Compared to other books on this subject, this text is organized by anatomical systems and locations, not by disorder. The authors aim to teach students about the cognitive, speech, language, and swallowing disorders that occur with damage to neurological systems in a manner that breaks down silos that artificially separate disorders that routinely co-occur. This approach reflects the reality that most individuals with acquired brain injuries have multiple cognitive, communication, and swallowing disorders.

Many aspects of drug safety have become an outstanding and even persistent issue and may occur during the process of both drug discovery and development. Until 15 years ago, drug discovery and evaluation was primarily a sequential process starting with the selection of the most pharmacologically active compound from a series of newly synthesized small molecule chemical series by means of distinctive pharmacological assays. Safety aspects were addressed by evaluation of the selected compound at high doses in a series of specific studies directed at indications other than the intended indication of the new compound. These tests are then followed by pharmacokinetic studies, which are primarily conducted to confirm whether the selected compound possesses a suitable half-life for sufficient exposure and efficacy and, whether it has the desired properties specificity to the intended route of administration. Safety aspects relied predominantly on the conduct of single and repeat toxicologydose studies, which inform changes in organ structure rather than organ function. Both toxicological and pharmacokinetic studies are adapted to the progress of studies in clinical pharmacology and clinical trials. The new edition of this well and broadly accepted reference work contains several innovative and distinguished chapters. This'sequential'strategy has been abandoned with this new version of the book for several reasons: - Of the possible multitude of negative effects that novel drugs may impart on organ function, e.g. ventricular tachy-arrhythmia, many are detected too late in non-clinical studies to inform clinicians. On the other hand, negative findings in chronic toxicity studies in animals may turn out to be irrelevant for human beings. - New scientific approaches, e.g. high-throughput screening, human pluripotent stem cells, transgenic animals, knock-out animals, in silico models, pharmaco-genomics and pharmaco-proteomics, as well as Artificial Intelligence (AI) methods offered new possibilities. - There are several examples, that show that the'druggability'of compounds was considerably underestimated when the probability of success of a new project was assessed. The success rate in the pharmaceutical industry and the introduction of new chemical entities to the market per year dropped dramatically, whereas the development time for a new compound increased, sometimes exceeding the patent protection. Research and development scientists, involving the following changes, therefore adopted a change of strategy: - Parallel instead of sequential involvement of the various disciplines (multidimensional compound optimization). - The term'Safety Pharmacology'was coined. The International Conference on Harmonization (ICH) founded a Safety Pharmacology Working Group and the Safety Pharmacology Society (SPS) was launched. The discipline provided for evaluation, development and validation of a multitude of safety tests outlined in the'Core Battery of Studies'. - Characterizing the exposure profile of a drug by conducting pharmacokinetic studies that evaluates the absorption, distribution, metabolism and excretion should to be investigated at an early stage of development as results contribute to the selection of a compound for further development. Advancements in Toxicology were achieved by the introduction of new methods, e.g., in silico methods, genetic toxicology, computational toxicology and AI. The book is a landmark in the continuously changing world of drug research and developments. Die erste Revision der S3-Leitlinie zur Behandlung von Zwangsstörungen bei Erwachsenen wurde wieder unter Beratung und Moderation durch die Arbeitsgemeinschaft der wissenschaftlichen medizinischen Fachgesellschaft (AWMF) von einem großen Gremium an wissenschaftlichen Experten und Vertretern von Fachverbänden erstellt. Die Empfehlungen der revidierten Fassung basieren auf einer Sichtung der Evidenz der verfügbaren randomisierten klinischen Studien zu Zwangsstörungen sowie der Meta-Analysen dazu, einer Synthese der Empfehlungen anderer Leitlinien sowie einem Expertenkonsens bei den Fragen, zu denen es keine Evidenz aus Studien gibt. Aufgrund der großen Datenbasis sowie der breiten klinischen Erfahrungen von Experten auf dem Gebiet können in dieser Leitlinie konkrete und fundierte Empfehlungen für die Behandlung der Zwangsstörungen gegeben werden. Änderungen der ersten Revision der Leitlinie im Vergleich zur Ursprungsversion aus dem Jahre 2013 betreffen insbesondere die weitere Stärkung des Stellenwertes der kognitiven Verhaltenstherapie mit Exposition, die nach neueren Meta-Analysen das wirksamste Therapieverfahren und daher Therapie der ersten Wahl ist. Neu ist die Empfehlung für hochfrequente Expositionsübungen, z.B. im Blockformat, die, wo die Voraussetzungen vorliegen, genutzt werden sollten. Darüber hinaus wurden Empfehlungen zur Pharmakotherapie bei Therapieresistenz wesentlich überarbeitet, weil es in diesem Bereich in den vergangenen 10 Jahren viele neue Studien gab. Weitere neue Empfehlungen betreffen spezielle Behandlungsaspekte, den Umgang mit Komorbidität und die Versorgung, die in der ursprünglichen Leitlinie nicht enthalten waren.

Psychiatric symptoms (or mood/emotional disturbances) are diverse in patients with neurological diseases, that include depression, anxiety, emotional incontinence, anger, fatigue, and apathy. These symptoms are common; more than 1/3 of the patients suffer from these symptoms. Unfortunately, they have been neglected because 1) unlike other neurological symptoms such as motor dysfunction, speech disturbances or visual field defect, these symptoms are not visible and difficult to be noticed unless they are specifically examined by a physician who is properly educated on this problem 2) they are often not regarded as neurological symptoms either by the patients or their caregivers 3) they are relatively poorly studied by both neurologists and psychiatrists, and accordingly, are unfamiliar to the physicians. Especially, although `depression'is well known to physicians, other symptoms such as emotional incontinence, anger, fatigue or apathy are not appropriately assessed by physicians, and frequently misdiagnosed as depression. Moreover, there are difficulties in diagnosing depression in patients neurological diseases. For example, the individual items included in depression diagnosis such as sleep disturbances, appetite loss, or fatigue can result from neurological diseases or comorbid physical conditions in patients with neurological diseases. Therefore, a diagnosis of `depression'should be made cautiously. Finally, it should be understood that the importance of different mood/ emotional syndromes differ among various neurological diseases; for instance, although emotional incontinence and anger occur in as many as 20-30% of stroke patients, apathy and fatigue are more important symptoms than these for patients with neuro-degenerative disease.In clinical practice, recognizing these symptoms is important because they negatively affect the patients'quality of life, impair the functional recovery, increase the mortality, and increase the caregiver burden. Nevertheless, unlike other neurological symptoms such as motor/sensory dysfunction, and speech disturbances, they are relatively well managed mostly by pharmacological therapy such as selective serotonin re-uptake inhibitors (SSRI).The phenomenology of these neuropsychiatric symptoms and related factors, pathophysiological mechanism, and treatment strategies should be properly educated for neurologists, psychiatrists and other physicians. The book is written by experts in this field to disseminate important knowledges to neurologists, psychiatrists, psychologists and other physicians and will eventually benefit patients with neurological diseases.

This is an original book on the philosophy of medicine. It considers philosophy of medicine as a subdiscipline of philosophy of science. This volume is grounded on an epistemological bottom-up account that arises from the clinical situation, the epidemiologic, and the resulting public health account. It is not a review of the literature, and it is not intended to frame the debates, or to analyze and compare the various number of viewpoints.Medicine is the human activity, which begins by a linguistic act that identifies the negative norms of health: it begins with a first distinction that splits biological processes into three conventional parts, normal, abnormal and pathologic. Neither of them is a natural kind. Being abnormal is intrinsically bad and admits of degrees, while being pathologic is dichotomous. Being normal is factitious and counterfactual much the same as frictionless planes in physics. Leaving apart the ethical aspects, this book endeavors to uncover the implicit conceptual network, the chief junctures of medicine, should they be found, and their articulations with clinical and community medicine. It results that medicine is pervaded with dichotomous concepts such as scientific vs pragmatic discourse, function and malfunction, abnormal and pathologic, needs and wants, causation and explanation, clinical vs community-oriented care, physical vs psychiatric diseases, mental illness vs deviancy, and so on. Medical thinking has two dimensions intrinsically interweaved, namely a constant amalgam and admixture of biological and normative aspects, so that this essential hybrid nature of the grammar of medicine endorses opposite approaches, naturalistic or normativist, biological or value-laden, realist or instrumental, reductionist or holistic, phenomenological or analytic.

The Predictive Processing Theory of Mind is a recent theory developed by philosophers, cognitive scientists, and neuroscientists about the nature and function of the brain and its role in creating the conscious mind that we humans, and perhaps some non-human animals, have. Huntington's disease (HD) is a fatal, inherited, neurodegenerative disorder, characterized by chorea, motor instabilities, psychiatric manifestations and cognitive decline. Early genetic testing provides an opportunity for clinical interventions aimed at delaying onset and/or slowing progression of disease; however, current treatments for HD are limited, with only two FDA-approved drugs available to manage chorea. Encouragingly, however, several disease-modifying treatment approaches are in the therapeutic pipeline, with more than 200 clinical studies, and many more preclinical studies, in the works. Robust and reliable biomarkers are needed to predict disease onset, monitor disease progression and assess treatment responses.
