Biogenic silica (biogenic Si) is a bioactive component crucial for the biogeochemical cycling of Si in terrestrial and aquatic ecosystems. Its formation and dissolution dynamics are intricately linked to carbon (C) cycling. However, knowledge about the source, composition, and factors controlling the distribution of biogenic Si in coastal wetland sedimentary environments is still limited. To address this lack of knowledge, we introduced a suite of geometric models for biogenic Si biovolume calculation and investigated biogenic Si assemblages, along with biogenic Si and biogenic Si‐occluded C contents, in sediments from representative coastal wetlands along the west coast of Bohai Bay. Our analysis showed that sedimentary biogenic Si predominantly derived from phytoliths (78.1% ± 5.8%), diatoms (18.2% ± 4.8%), and sponge spicules (3.7% ± 2.9%). Notably, phytolith assemblages were primarily composed of forms derived from the Poaceae family. The biogenic Si‐occluded C content (0.035–1.870 g kg−1) within these wetland sediments was consistent across both sites, accounting for 0.97–5.71% of the total organic C pool. Structural equation modeling indicated that total organic C, pH, and amorphous aluminum oxides either directly or indirectly influenced the biogenic Si content in coastal wetlands sediments. These results demonstrate the critical role that biogenic Si plays in the Si biogeochemical cycle and provide valuable information that advances our understanding of the biogeochemical interactions between Si and C in coastal wetland ecosystems.

Patient and caregiver involvement can enhance the uptake and impact of research, but the involvement of patients and caregivers who are underserved and marginalized is often limited. A better understanding of how to make involvement in research more broadly accessible, supportive, and inclusive for patients with chronic kidney disease (CKD) and caregivers is needed. We conducted a national workshop involving patients, caregivers, clinicians, and researchers from across Australia to identify strategies to increase the diversity of patients and caregivers involved in CKD research. Six themes were identified. Building trust and a sense of safetywas considered pivotal to establishing meaningful relationships to support knowledge exchange. Establishing community and connectednesswas expected to generate a sense of belonging to motivate involvement. Balancing stakeholder goals, expectations, and responsibilitiesinvolved demonstrating commitment and transparency by researchers. Providing adequate resources and supportincluded strategies to minimize the burden of involvement for patients and caregivers. Making research accessible and relatablewas about nurturing patient and caregiver interest by appealing to intrinsic motivators. Adapting to patient and caregiver needs and preferencesrequired tailoring the approach for individuals and the target community. Strategies and actions to support these themes may support more diverse and equitable involvement of patients and caregivers in research in CKD.

Coastal wetlands contribute to the mitigation of climate change through the sequestration of “blue carbon”. Microbial necromass, lignin, and glycoproteins (i.e., glomalin-related soil proteins (GRSP)), as important components of soil organic carbon (SOC), are sensitive to environmental change. However, their contributions to blue carbon formation and the underlying factors remain largely unresolved. To address this paucity of knowledge, we investigated their contributions to blue carbon formation along a salinity gradient in coastal marshes. Our results revealed decreasing contributions of microbial necromass and lignin to blue carbon as the salinity increased, while GRSP showed an opposite trend. Using random forest models, we showed that their contributions to SOC were dependent on microbial biomass and resource stoichiometry. In N-limited saline soils, contributions of microbial necromass to SOC decreased due to increased N-acquisition enzyme activity. Decreases in lignin contributions were linked to reduced mineral protection offered by short-range-ordered Fe (FeSRO). Partial least-squares path modeling (PLS-PM) further indicated that GRSP could increase microbial necromass and lignin formation by enhancing mineral protection. Our findings have implications for improving the accumulation of refractory and mineral-bound organic matter in coastal wetlands, considering the current scenario of heightened nutrient discharge and sea-level rise.

This paper analyzes the orientation towards sustainable investment by global sovereign wealth funds (henceforth SWFs) over the last two decades. Our data analysis reveals that over the last 5 years, there has been a noticeable uptick in investing along the Sustainable Development Goals (henceforth SDGs) by SWFs. From 2018 onwards, momentum has been building in climate and energy, especially in deal value. We also see agriculture come to the fore in 2020, and to a lesser degree, investments in education, as SWFs appear to extend their investment remits slowly but surely into other long-term investment themes. The paper also provides preliminary descriptive evidence about the drivers of sovereign sustainable investment, showing that the presence of explicit environmental, social, and governance (henceforth ESG) policies in place favor capital deployment aligned with SDGs. Finally, the paper studies the ESG performance of a sub-sample of listed firms that SWFs have invested in, finding a significant deterioration in the governance pillar. This result is broadly consistent with previous research on the agency costs of sovereign ownership. The paper concludes by making policy recommendations regarding fiduciary duty, investee corporate governance, and climate investments, which would contribute to modernizing the role of SWFs in the global economy.

Patients treated with kidney replacement therapy experience a 1.5- to 2-fold increased risk of cancer and cancer mortality compared with the general population. Whether this excess risk extends to people with earlier stage chronic kidney disease and whether reduced kidney function is causally related to cancer is unclear.

While many studies have examined the role of biochar in carbon (C) accrual in short-term scale, few have explored the decadal scale influences of biochar on non-biochar C, e.g., native soil organic C (SOC) and added substrate. To address this knowledge gap, soils were collected from decade-old biochar field trials located in the United Kingdom (Cambisol) and China (Fluvisol), with each site having had three application rates (25–30, 50–60 and 75–100 Mg ha−1) of biochar plus an unamended Control, applied once in 2009. We assessed physicochemical and microbial properties associated with sucrose (representing the rhizodeposits) mineralization and the priming effect (PE) on native SOC. Here, we showed both soils amended with biochar at the middle application rate (50 Mg ha−1biochar in Cambisol and 60 Mg ha−1biochar in Fluvisol) resulted in greater substrate mineralization. The enhanced accessibility and availability of sucrose to microorganisms, particularly fast-growing bacterial genera like Arenimonas, Spingomonas,and Paenibacillus(r-strategists belonging to the Proteobacteria and Firmicutes phyla, respectively), can be attributed to the improved physicochemical properties of the soil, including pH, porosity, and pore connectivity, as revealed by synchrotron-based micro-CT. Random forest analysis also confirmed the contribution of the microbial diversity and physical properties such as porosity on sucrose mineralization. Biochar at the middle application rate, however, resulted in the lowest PE (0.3 and 0.4 mg of CO2-C g soil−1in Cambisol and Fluvisol, respectively) after 53 days of incubation. This result might be associated with the fact that the biochar promoted large aggregates formation, which enclosed native SOC in soil macro-aggregates (2–0.25 mm). Our study revealed a diverging pattern between substrate mineralization and SOC priming linked to the biochar application rate. This suggests distinct mechanisms, biophysical and physicochemical, driving the mineralization of non-biochar carbon in a field where biochar was applied a decade before.

In this multi-center longitudinal cohort study conducted in Australia and New Zealand, we assessed the trajectories of health-related quality of life (HRQoL) in children with chronic kidney disease (CKD) over time. A total of 377 children (aged 6-18 years) with CKD stages 1-5 (pre-dialysis), dialysis, or transplant, were followed biennially for four years. Multi Attribute Utility (MAU) scores of HRQoL were measured at baseline and at two and four years using the McMaster Health Utilities Index Mark 3 tool, a generic multi-attribute, preference-based system. A multivariable linear mixed model was used to assess the trajectories of HRQoL over time in 199 children with CKD stage 1-5, 43 children receiving dialysis and 135 kidney transplant recipients. An interaction between CKD stage at baseline and follow-up time indicated that the slopes of the HRQoL scores differed between children by CKD stage at inception. Over half of the cohort on dialysis at baseline had received a kidney transplant by the end of year four and the MAU scores of these children increased by a meaningful amount averaging 0.05 (95% confidence interval 0.01 to 0.09) per year in comparison to those who were transplant recipients at baseline. The mean difference between baseline and year two MAU scores was 0.09 (95% confidence interval -0.05, 0.23), (Cohen’s d effect size 0.31). Thus, improvement in HRQoL over time of children on dialysis at baseline was likely to have been driven by their transition from dialysis to transplantation. Additionally, children with CKD stage 1-5 and transplant recipients at baseline had no changes in their disease stage or treatment modality and experienced stable HRQoL over time.

To determine the effect of biochar application (BIO) and exogenous spraying of methyl jasmonate (MJ) on some biochemical attributes of winter barley under withholding irrigation at the flowering stage (for two weeks), a two-year glasshouse study was carried out. The drought stress increased the electrolyte leakage by 51.7%, however addition of biochar into the soil and spraying methyl jasmonate decreased this parameter remarkabley. Moreover, drought stress increased the enzymatic activity of catalase in both years (1016% and 716% respectively in the first and the second year), super oxid dismutase in both years (1302% in the first year and 382% in the second year), proline content (256%), total soluble sugar content (366% in the first and 490% in the second year) and Malondyaldehyde (383% and 417% respectively in the first and the second year). However, due to application of biochar and methyl jasmonate, the adverse effects of drought stress on these characteristics were adjusted. The highest (9.51 gr/pot) and the lowest (4 gr/pot) grain yield was recorded as a result of combined application of 0.25% of biochar and 50 μmol of methyl jasmonate. Conclusively, it is deduced that, addition of biochar into the soil and spraying methy jasmonate can be effective in managing water shortage conditions if they are used in appropriate doses which were shown to be 0.25% of biochar and 50 μmol per liter of methyl jasmonate respectively, to optimize water use and minimize drought stress.

In this study, we observed the effect of a newly developed slow release brown coal-urea (BCU) fertilizer on the greenhouse gas emissions, plant growth, yield, and nitrogen use efficiency (NUE) of sweet corn grown under field conditions, as compared with conventional urea fertilizer. The soil was treated with urea and BCU as a N fertilizer at a rate of 90 or 180 kg N ha–1at sowing and incorporated. Seventeen N2O and 10 NH3gas samples were collected to determine the gaseous emissions of fertilizer-N. The addition of BCU considerably enhanced the availability and uptake of fertilizer-N to sweet corn by decreasing greenhouse gas emissions by 59% as N2O and by 29% as NH3compared to commercial urea. Slow release BCU fertilizers enhanced NUE and cob yield by 32 and 24%, respectively, over urea. Furthermore, the soil incorporation of BCU fertilizer resulted in a significant increase in the organic carbon content of soil at harvest. Therefore, the application of slow release BCU fertilizer could be a more economic, profitable, and environmentally friendly N fertilizer alternative over the use of conventional urea.

To explore the amount and type of research funded under relevant Medical Research Future Fund (MRFF) Initiatives that addressed public health and prevention from 2018 to July 2023.

Lawsuits concerning the impacts of climate change make causal claims about the effect of defendants’ greenhouse gas (GHG) emissions on plaintiffs and have proliferated around the world. Plaintiffs have sought, inter alia, compensation for climate-related losses and to compel governments to reduce their GHG emissions. So far, most of these claims have been unsuccessful. Here we assess the scientific and legal bases for establishing causation and evaluate judicial treatment of scientific evidence in 73 lawsuits. We find that the evidence submitted and referenced in these cases lags considerably behind the state of the art in climate science, impeding causation claims. We conclude that greater appreciation and exploitation of existing methodologies in attribution science could address obstacles to causation and improve the prospects of litigation as a route to compensation for losses, regulatory action and emission reductions by defendants seeking to limit legal liability.

Context Compaction removal and organic amendment application are commonly used to mitigate the compaction-induced declines in crop yield, soil carbon (C) and soil health. However, the response of microbial activities and nutrient pools to the combination of mill-mud amendments and decompaction in the soil profile are not fully understood. Aims A field trial was conducted at Burdekin, Australia, to investigate the effects of different decompaction managements on soil nutrient cycling, associated biological activities and sugarcane yield. Methods This experiment included four treatments: control (CK, without mill-mud), mill-mud shallow furrow (MS), deep trenching without mill-mud (DT) and deep trenching mill-mud application (MD). Key results The MD treatment increased concentrations of hot water extractable organic C by 30–70% and hot water extractable total nitrogen (N) by 30–90% at the application depth. Soil microbial biomass C and N were also higher in mill-mud applied layers. Mill-mud applied treatments increased plant cane yield by 7% (MS treatment) and 14% (MD treatment) compared to CK. The DT treatment also increased plant cane yield by 11% compared to CK. Conclusion The MD treatment increased the supply of organic C and nutrients to the microbial community within the entire soil profile, enhanced nutrient cycling processes, improved soil environmental conditions and soil health for sugarcane growth and thus increased sugarcane productivity. Implications Further research should focus on microbial community composition shifts to further explore the mechanisms responsible for soil microorganism regulation of nutrient cycling in sugarcane farming systems.

Health data are often plagued with missing values that can greatly reduce the sample size if only complete cases are considered for analysis. Furthermore, analyses that ignore missing data have the potential to introduce bias in the parameter estimates. Multiple imputation techniques have been developed to recover the information that would otherwise be lost when excluding observations with missing data and to help minimize bias. However, the validity of analyses using imputed data relies on the imputation model having been correctly specified. The aim of this guide is to aid the reader in the decision-making process when conducting an analysis with multiply imputed data in the context of nephrology research. We discuss (i) missing mechanism assumption, (ii) imputation method, (iii) imputation model, (iv) derived variables, (v) the number of imputed data sets, (vi) diagnostic checks, (vii) analysis and pooling of results, and (viii) reporting the results. This process is demonstrated using data from the National Health and Nutrition Examination Survey to explore the association between hypertension and kidney disease in adults from the general population. Example code is provided for SAS software and the micepackage in R.

Senescence of erythrocytes is characterized by a series of changes that precede their removal from the circulation, including loss of red cell hydration, membrane shedding, loss of deformability, phosphatidyl serine exposure, reduced membrane sialic acid content, and adhesion molecule activation. Little is known about the mechanisms that initiate these changes nor is it known whether they are interrelated. In this study, we show that Ca2+-dependent K+ efflux (the Gardos effect) drives erythrocyte senescence. We found that increased intracellular Ca2+ activates the Gardos channel, leading to shedding of glycophorin-C (GPC)–containing vesicles. This results in a loss of erythrocyte deformability but also in a marked loss of membrane sialic acid content. We found that GPC-derived sialic acid residues suppress activity of both Lutheran/basal cell adhesion molecule (Lu/BCAM) and CD44 by the formation of a complex on the erythrocyte membrane, and Gardos channel–mediated shedding of GPC results in Lu/BCAM and CD44 activation. This phenomenon was observed as erythrocytes aged and on erythrocytes that were otherwise prone to clearance from the circulation, such as sickle erythrocytes, erythrocytes stored for transfusion, or artificially dehydrated erythrocytes. These novel findings provide a unifying concept on erythrocyte senescence in health and disease through initiation of the Gardos effect.

However, the molecular mechanisms involved in sequestration of senescent erythrocytes, their recognition, and their subsequent degradation by RPMs remain unclear. In this study, we provide evidence that the splenic environment is of substantial importance in facilitating erythrocyte turnover through induction of hemolysis. Upon isolating human spleen RPMs, we noted a substantial lack of macrophages that were in the process of phagocytosing intact erythrocytes. Detailed characterization of erythrocyte and macrophage subpopulations from human spleen tissue led to the identification of erythrocytes that are devoid of hemoglobin, so-called erythrocyte ghosts. By using in vivo imaging and transfusion experiments, we further confirmed that senescent erythrocytes that are retained in the spleen are subject to hemolysis. In addition, we showed that erythrocyte adhesion molecules, which are specifically activated on aged erythrocytes, cause senescent erythrocytes to interact with extracellular matrix proteins that are exposed within the splenic architecture. Such adhesion molecule–driven retention of senescent erythrocytes under low shear conditions was found to result in steady shrinkage of the cell and ultimately resulted in hemolysis. In contrast to intact senescent erythrocytes, the remnant erythrocyte ghost shells were prone to recognition and breakdown by RPMs. These data identify hemolysis as a key event in the turnover of senescent erythrocytes, which alters our current understanding of how erythrocyte degradation is regulated.

Social functioning is a key aspect of daily life and is important to patients living with chronic kidney disease (CKD) and their caregivers. Many patients with CKD experience debilitating symptoms and treatment burden that can diminish their social functioning and thereby overall social health, which is the aspect of a person's well-being relating to their interactions and connections with others. In patients with CKD, symptoms (e.g., fatigue and pain), burden of ongoing treatments (including kidney replacement therapies), and medication side effects can impair social functioning. Having to manage responsibilities of self-management, which can include time-consuming and invasive treatments such as dialysis, can severely limit social functioning in patients with CKD. This can lead to poor social connections at many levels, including with family, friends, peers, and colleagues, and can hinder the development of new relationships. Patients with CKD with poorer social functioning have been reported to have worse quality of life and impaired mental health. Many patients with CKD rely on an informal caregiver—usually a family member or friend—to assist with management of their disease. This can place strain on the caregiver, further limiting opportunities for social connections for both the patient and caregiver. Although social functioning is critical for the overall well-being of patients with CKD, it remains underaddressed clinically, and patient-reported outcome measures (PROMs) to assess social functioning are limited. The objective of this article is to define social functioning, discuss the impacts of social functioning in patients with CKD and their caregivers, outline PROMs that have included assessment of social functioning, and discuss considerations in developing an appropriate PROM to measure social functioning in patients with CKD. This may help to inform the evaluation of interventions and care regarding social functioning within the CKD population.

We examined associations between intra-generational social mobility (reflected in life-course socioeconomic trajectories) and mortality, among older men.

The integration of legumes into coppiced tree crop systems to replace some or all of the external nitrogen (N) fertiliser requirements may be one means to lower seasonal nitrous oxide (N2O) emissions. We investigated soil N2O emissions using static chamber methodology in field trials located within two commercial tea tree (Melaleuca alternifolia) plantations (Casino and Tweed Heads) where N (116 and 132 kg N ha-1 respectively) was supplied via poultry litter application (5 t wet ha-1) or by termination of annual legumes (soybean or mung bean) grown in the inter-row. While there was no treatment effect at the Tweed Heads site, both legume treatments had significantly (P = 0.01) lower cumulative N2O emissions (0.33 and 0.30 kg N2O-N ha-1 season-1 for soybean and mung beans respectively) than the poultry litter treatment (0.66 kg N2O-N ha-1 season-1) at the Casino site. However, the amount of N added to soils in each treatment was not identical owing to an inability to accurately predict N inputs by legume crops, and thus differences could not be attributed to the N source. A third site was thus established at Leeville comparing N2O emissions from poultry litter amendment (5 t wet ha-1 contributing 161 kg N ha-1) to an inter-row faba bean crop (contributing 92 kg N ha-1) and a nil-N control. Cumulative seasonal N2O emissions were significantly (P < 0.05) lower in the faba bean treatment than the poultry litter treatment (0.08 and 0.23 kg N2O-N ha-1 season-1 respectively), but owing to different N inputs and generally low emissions, it was not possible to draw definitive conclusions on whether green manure legume crops can lower N2O emissions. Overall, soil N2O emissions in coppiced tea tree systems under current management practices were very low, offering limited potential to reduce seasonal N2O emissions through management practice change.

Neuromodulation is a promising treatment modality for tinnitus, especially in chronic and severe cases. The auditory thalamus plays a key role in the pathophysiology of tinnitus, as it integrates and processes auditory and limbic information. The effect of high frequency stimulation and low frequency stimulation of the medial geniculate bodies on tinnitus in a noise‐induced tinnitus rat model is assessed. Presence of tinnitus was verified using the gap‐induced prepulse inhibition of the acoustic startle response paradigm. Hearing thresholds were determined before and after noise trauma with auditory brainstem responses. Anxiety‐related side‐effects were evaluated in the elevated zero maze and open field. Results show tinnitus development after noise exposure and preserved hearing thresholds of the ear that was protected from noise trauma. We found that high frequency stimulation of the medial geniculate bodies suppressed tinnitus. This effect maintained directly after stimulation when the stimulator was turned off. Low frequency stimulation did not have any effects on the gap:no‐gap ratio of the acoustic startle response. High frequency stimulation of the MGB has a direct and residual suppressing effect on tinnitus in this animal model. Low frequency stimulation of the MGB did not inhibit tinnitus.

Core IdeasBiochar‐based fertilizer treatments induced significant microbial community structure shifts compared with no‐biochar and biochar alone.Biochar‐based fertilizer especially at higher biochar addition rate increased soil microbial biomass and fertility.Soil microbial species were strongly correlated with soil nutrients under biochar and biochar‐based fertilizer into karst soil. Karst ecosystems are vulnerable to degradation and constitute a significant proportion of southwestern China. Here, we explore the amendment of these soils with wood biochar in combination with N‐P‐K fertilizer and swine manure compost with the goal of improving microbial community structure and soil nutrient status of these soils. A pot experiment with six treatments, including control (CK, no amendment), swine manure compost plus N‐P‐K fertilizer (MF), wood biochar at 2% in soil (W2), wood biochar at 4% in soil (W4), wood biochar‐based fertilizer at 2% in soil (W2MF), and biochar‐based fertilizer at 4% in soil (W4MF), was set up for 20 months. The results showed that soil organic matter and soil available nutrients N, P, and K in W4MF increased by 63.95, 66.23, 284.96, and 94.56%, respectively, compared with CK. Soil microbial biomass C, N, and P increased in sequence by 60.05, 87.64, and 307.85%, respectively. Soil bacterial and fungal diversity indices (Simpson and Shannon) and richness index (Chao1) were generally higher in biochar‐based fertilizer treatments (W2MF and W4MF) than biochar alone (W2 and W4). The bacterial and fungal community structures of biochar based fertilizer treatments differed from those of biochar amendment alone or control (P< 0.05). Linear discriminant (Effect Size) analysis showed that the bacterial biomarkers of the W2MF + W4MF group were mainly the phylum Acidobacteriaand class Alphaproteobacteria, and the fungal biomarkers were the phylum Rozellomycotaand Glomeromycota. Redundancy analysis revealed strong relationships between microbial community structure and soil organic matter and soil available nutrients N, P, and K (P< 0.05). In conclusion, either sole biochar or biochar‐based fertilizer amendment improved karst soil nutrient conditions and induced microbial community structural shifts. The performance was better in biochar‐based fertilizer treatment than that in biochar addition alone.

Dairy pastures can be a major source of soil nitrous oxide (N2O) emissions due to the combination of intensive nitrogen (N) fertiliser use and high soil water content, from either rainfall and/or irrigation. Biochar application is a promising approach to lower soil greenhouse gas emissions, particularly under high soil moisture conditions where denitrification is the primary N-transformation pathway. In a replicated field trial, we evaluated the effects of two contrasting biochars derived from poultry litter and from hardwood on soil N2O emissions, soil ammonium (NH4+) and nitrate (NO3−) status, pasture productivity and herbage nutrient content. A liming treatment to mimic the liming equivalence of the poultry litter biochar was used to separate any effects observed from changes in soil pH. To further separate the effects of biochars on soil N status, N2O emissions and pasture N uptake, high and low N fertiliser doses (annual application of 672 kg N ha−1, 336 kg N ha−1) were superimposed across all of the treatments. The N fertiliser dose had no significant impact on pasture yield. Application of poultry litter biochar resulted in significant increases in pasture productivity under both high and low N inputs. This was achieved by alleviating soil P, and possibly K nutritional constraints that are typical in Australian Ferralsols. Under the high N fertiliser dose, emissions of N2O from the treatments and control were not significantly different (p> 0.05) and ranged between 1.14 and 1.78 kg N2O-N ha−1across the 11-month study. The low N dose resulted in significantly lower emissions of N2O of between 0.80 and 0.84 kg N2O-N ha−1, but biochar had no significant effect on net emissions across the season. The lack of impact of biochar on N2O emissions was attributed to the relatively dry conditions over the trial period resulting in nitrification being the most likely N-transformation pathway. During brief episodes of high soil moisture, peak emissions from the biochar plots were lower than from the control or lime treatment, but these differences did not impact on the emission budget over the 11-month sampling campaign.

In the general population, cognitive impairment is associated with increased mortality, and higher levels of education are associated with lower risks for cognitive impairment and mortality. These associations are not well studied in patients receiving long-term hemodialysis and were the focus of the current investigation.

Patient navigators enable adult patients to circumnavigate complex health systems, improving access to health care and outcomes. Here, we aimed to evaluate the effects of a patient navigation program in children with chronic kidney disease (CKD). In this multi-center, randomized controlled trial, we randomly assigned children (aged 0-16 years) with CKD stages 1-5 (including children on dialysis or with kidney transplants), from low socioeconomic status backgrounds, and/or residing in remote areas, to receive patient navigation at randomization (immediate) or at six months (waitlist). The primary outcome was self-rated health (SRH) of participating children at six months, using intention to treat analysis. Secondary outcomes included caregivers’ SRH and satisfaction with health care, children’s quality of life, hospitalizations, and missed school days. Repeated measures of the primary outcome from baseline to six months were analyzed using cumulative logit mixed effects models. Semi-structured interviews were thematically evaluated. Of 398 screened children, 162 were randomized (80 immediate and 82 waitlist); mean age (standard deviation) of 8.8 (4.8) years with 64.8% male. SRH was not significantly different between the immediate and wait-listed groups at six months. There were also no differences across all secondary outcomes between the two groups. Caregivers’ perspectives were reflected in seven themes: easing mental strain, facilitating care coordination, strengthening capacity to provide care, reinforcing care collaborations, alleviating family tensions, inability to build rapport and unnecessary support. Thus, in children with CKD, self-rated health may not improve in response to a navigator program, but caregivers gained skills related to providing and accessing care.

Cancer has been identified by kidney transplant recipients as a critically important outcome. The co-occurrence of cancer and kidney transplantation represents a complex intersection of diseases, symptoms, and competing priorities for treatments. Research that focuses on biochemical parameters and clinical events may not capture the priorities of patients. Patient-centered research can improve the relevance and efficiency of research and is particularly pertinent in the setting of cancer and kidney transplantation to facilitate shared decision-making in complex clinical situations. In addition, patient-reported outcomes can facilitate the assessment of patients’ experiences, symptom burden, treatment side effects, and quality of life. This review discusses patient-centered research in the context of kidney transplantation and cancer, including consumer involvement in research and patient-centered outcomes and their measures and inclusion in core outcome sets.

BackgroundChronic conditions are the leading cause of mortality, morbidity and disability in children. However, children and caregivers are rarely involved in identifying research priorities, which may limit the value of research in supporting patient-centred practice and policy.ObjectiveTo identify priorities of patients, caregivers and health professionals for research in childhood chronic conditions and describe the reason for their choices.SettingAn Australian paediatric hospital and health consumer organisations.MethodsRecruited participants (n=73) included patients aged 8 to 14 years with a chronic condition (n=3), parents/caregivers of children aged 0 to 18 years with a chronic condition (n=19), representatives from consumer organisations (n=13) and health professionals including clinicians, researches (n=38) identified and discussed research priorities. Transcripts were thematically analysed.ResultsSeventy-eight research questions were identified. Five themes underpinned participants’ priorities: maintaining a sense of normality (enabling participation in school, supporting social functioning, promoting understanding and acceptance), empowering self-management and partnership in care (overcoming communication barriers, gaining knowledge and skills, motivation for treatment adherence, making informed decisions, access and understanding of complementary and alternative therapies),strengthening ability to cope (learning to have a positive outlook, preparing for home care management, transitioning to adult services), broadening focus to family (supporting sibling well-being, parental resilience and financial loss, alleviating caregiver burden), and improving quality and scope of health and social care (readdressing variability and inequities, preventing disease complications and treatment side effects, identifying risk factors, improving long-term outcomes, harnessing technology, integrating multidisciplinary services).ConclusionResearch priorities identified by children, caregivers and health professionals emphasise a focus on life participation, psychosocial well-being, impact on family and quality of care. These priorities may be used by funding and policy organisations in establishing a paediatric research agenda.

ObjectiveThe aim was to compare quality of life (QoL) among children and adolescents with different stages of chronic kidney disease (CKD) and determine factors associated with changes in QoL.DesignCross-sectional.SettingThe Kids with CKD study involved five of eight paediatric nephrology units in Australia and New Zealand.PatientsThere were 375 children and adolescents (aged 6–18 years) with CKD, on dialysis or transplanted, recruited between 2013 and 2016.Main outcome measuresOverall and domain-specific QoL were measured using the Health Utilities Index 3 score, with a scale from −0.36 (worse than dead) to 1 (perfect health). QoL scores were compared between CKD stages using the Mann-Whitney U test. Factors associated with changes in QoL were assessed using multivariable linear and ordinal logistic regression.ResultsQoL for those with CKD stages 1–2 (n=106, median 0.88, IQR 0.63–0.96) was higher than those on dialysis (n=43, median 0.67, IQR 0.39–0.91, p<0.001), and similar to those with kidney transplants (n=135, median 0.83, IQR 0.59–0.97, p=0.4) or CKD stages 3–5 (n=91, 0.85, IQR 0.60–0.98). Reductions were most frequent in the domains of cognition (50%), pain (42%) and emotion (40%). The risk factors associated with decrements in overall QoL were being on dialysis (decrement of 0.13, 95% CI 0.02 to 0.25, p=0.02), lower family income (decrement of 0.10, 95% CI 0.03 to 0.15, p=0.002) and short stature (decrement of 0.09, 95% CI 0.01 to 0.16, p=0.02).ConclusionsThe overall QoL and domains such as pain and emotion are substantially worse in children on dialysis compared with earlier stage CKD and those with kidney transplants.

Introduction

Lutheran/basal cell adhesion molecule (Lu/BCAM) is a transmembrane adhesion molecule expressed by erythrocytes and endothelial cells that can interact with the extracellular matrix protein laminin-a5. In sickle cell disease, Lu/BCAM is thought to contribute to adhesion of sickle erythrocytes to the vascular wall, especially during vaso-occlusive crises. On healthy erythrocytes however, its function is unclear. Here we report that Lu/BCAM is activated during erythrocyte aging. We show that Lu/BCAM-mediated binding to laminin-a5 is restricted by interacting, in cis, with glycophorin-C–derived sialic acid residues. Following loss of sialic acid during erythrocyte aging, Lu/BCAM is released from glycophorin-C and allowed to interact with sialic acid residues on laminin-a5. Decreased glycophorin-C sialylation, as observed in individuals lacking exon 3 of glycophorin-C, the so-called Gerbich phenotype, was found to correlate with increased Lu/BCAM-dependent binding to laminin-a5. In addition, we identified the sialic acid–binding site within the third immunoglobulin-like domain within Lu/BCAM that accounts for the interaction with glycophorin-C and laminin-a5. Last, we present evidence that neuraminidase-expressing pathogens, such as Streptococcus pneumoniae, can similarly induce Lu/BCAM-mediated binding to laminin-a5, by cleaving terminal sialic acid residues from the erythrocyte membrane. These results shed new light on the mechanisms contributing to increased adhesiveness of erythrocytes at the end of their lifespan, possibly facilitating their clearance. Furthermore, this work may contribute to understanding the pathology induced by neuraminidase-positive bacteria, because they are especially harmful to patients suffering from sickle cell disease and are associated with the occurrence of vaso-occlusive crises.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency is the most common enzyme deficiency worldwide. Detection of heterozygously deficient females can be difficult as residual activity in G6PD-sufficient red blood cells (RBCs) can mask deficiency. In this study, we compared accuracy of 4 methods for detection of G6PD deficiency in females. Blood samples from females more than 3 months of age were used for spectrophotometric measurement of G6PD activity and for determination of the percentage G6PD-negative RBCs by cytofluorometry. An additional sample from females suspected to have G6PD deficiency based on the spectrophotometric G6PD activity was used for measuring chromate inhibition and sequencing of the G6PDgene. Of 165 included females, 114 were suspected to have heterozygous deficiency. From 75 females, an extra sample was obtained. In this group, mutation analysis detected 27 heterozygously deficient females. The sensitivity of spectrophotometry, cytofluorometry, and chromate inhibition was calculated to be 0.52 (confidence interval [CI]: 0.32–0.71), 0.85 (CI: 0.66–0.96), and 0.96 (CI: 0.71–1.00, respectively, and the specificity was 1.00 (CI: 0.93–1.00), 0.88 (CI: 0.75–0.95), and 0.98 (CI: 0.89–1.00), respectively. Heterozygously G6PD-deficient females with a larger percentage of G6PD-sufficient RBCs are missed by routine methods measuring total G6PD activity. However, the majority of these females can be detected with both chromate inhibition and cytofluorometry.

Bulk chemical analysis was undertaken on samples taken from 143 negative features (wells, pits, post-holes, cess pits and ditches) across the area of excavation at Silchester Hampshire in order to help us understand the disposal of waste during late Iron Age and earliest Roman occupation. Results show that it is possible to split features into waste disposal which included animal/human waste and those which probably did not. It is also possible to identify post-holes based on organic matter content. This work forms part of the larger Town Life project run by the University of Reading.

Biochar can increase the stable C content of soil. However, studies on the longer-term role of plant–soil–biochar interactions and the consequent changes to native soil organic carbon (SOC) are lacking. Periodic 13CO2pulse labelling of ryegrass was used to monitor belowground C allocation, SOC priming, and stabilization of root-derived C for a 15-month period—commencing 8.2 years after biochar (Eucalyptus saligna, 550 °C) was amended into a subtropical ferralsol. We found that field-aged biochar enhanced the belowground recovery of new root-derived C (13C) by 20%, and facilitated negative rhizosphere priming (it slowed SOC mineralization by 5.5%, that is, 46 g CO2-C m−2yr−1). Retention of root-derived 13C in the stable organo-mineral fraction (<53 μm) was also increased (6%, P < 0.05). Through synchrotron-based spectroscopic analysis of bulk soil, field-aged biochar and microaggregates (<250 μm), we demonstrate that biochar accelerates the formation of microaggregates via organo-mineral interactions, resulting in the stabilization and accumulation of SOC in a rhodic ferralsol.
