Myoepithelial carcinoma (MECA) is a rare type of carcinoma for which the clinicopathological features and prognostic factors have not yet been fully clarified. A retrospective study of 42 patients diagnosed with salivary gland MECA was performed, focusing on the clinicopathological features and prognostic factors. Of the 42 patients, 20 died of cancer, 20 lived without tumour, one lived with distant metastasis, and one was lost to follow-up. Overall, 69.0% had tumour recurrence, 16.7% had cervical nodal metastasis, and 21.4% had distant metastasis. The 5-year overall survival rate was 70.2%. Kaplan–Meier analysis revealed that patients with pathological positive lymph nodes (pN+), multiple recurrences of tumour, and higher histological grade had worse overall survival. Multivariate Cox analysis indicated pN+ and higher histological grade to be independent predictors of decreased survival. The 5-year overall survival rate in the pN0 group was 87.5%, while that in the pN+ group was 28.6%. In conclusion, myoepithelial carcinoma can be defined as a tumour with a high incidence of recurrence and poor prognosis, especially in pN+ patients. Background: Myoepithelioma is a rare type of salivary gland tumor that mainly occurs in the parotid glands and palate. Case presentation: A 43‐year‐old woman presented with swelling and pain in the right maxillary molar. We performed enucleation and curettage under general anesthesia. The tumor was cyst‐like. Histopathological examination revealed nests of tumor cells diffusely interspersed within fibrotic connective tissues. A few cells were positive for cytokeratin AE1/AE3, α‐SMA, GFAP, and vimentin; S‐100 protein and calponin were also positive. Final diagnosis was myoepithelioma. Tumours of salivary glands are rare and have various histo-pathological subtypes. Myoepitheliomas were first classified by Sheldon et al. and the criterion to classify or diagnose it was first defined by Barnes et al. and Sciubba and Brannon. Myoepithelioma accounts for less than 1 % of all salivary gland tumours, 40 % of these tumours occur in the parotid gland while 21 % occur in the minor salivary glands. A case of myoepithelioma of a minor salivary gland of the cheek is described, emphasizing the problems of the differential diagnosis. A 40-year-old female reported to the department with a complaint of a cheek bite on her right side for a few months. The physical examination showed a presence of lobulated whitish mucosa on the right buccal mucosa at the level of the occlusal plane, on palpation it revealed a non-painful mass approximately 1.5 cm in radius, mobile to bimanual palpation. An excisional biopsy was performed under local anaesthesia. Microscopic and immunohistochemistry confirmed the tumour to be a myoepithelioma of a minor salivary gland with the absence of definitive features of malignancy. Due to their infrequency and multiplicity of histopathology, myoepitheliomas present difficulties in diagnosis. Cellular varieties can be misdiagnosed as malignancies. A key to determining diagnostic criteria for myoepitheliomas is to study cellular morphology, cytoplasmic filament expression, and ultrastructural features of the tumour and apply this information to defining myoepitheliomas. Soft tissue myoepithelioma (STM), a rare mesenchymal neoplasm morphologically analogous to its more common salivary gland (SG) counterpart, is the subject of single case reports regarding its fine-needle aspiration (FNA) biopsy. To our knowledge, ours is the first case series of STM. A search was made of our pathology databases for cases diagnosed as STM. FNA biopsy smears and cell blocks were performed using standard techniques. Seven cases were retrieved from 4 men and 3 women (M:F = 1.3:1; age range: 25-79 years, x = 54 years). All but 1 presented as a primary neoplasm. Six aspirates were from the extremities, and 1 from the abdominal wall. Mean tumor size was 5.7 cm. Cytologic diagnosis of STM or suspicious for STM was made in 3 cases (43%). Remaining FNA diagnoses were spindle cell neoplasm/lesion (2), spindle cell sarcoma (1), and extraskeletal myxoid chondrosarcoma (1). Three cases were composed primarily or solely of uniform spindle cells, 3 primarily of uniform epithelioid cells with plasmacytoid features, and 1 case a mixture of these 2 cell types. Myxoid/chondromyxoid stroma was relatively abundant except in the single hypocellular example. Immunohistochemical (IHC) testing performed in 71% was nonspecific, but positive with S-100 in 4 of 5, EMA in 3 of 3, calponin in 2 of 2, and keratin in 1 of 3 examples. FNA biopsy smears of STM are remarkably similar cytomorphologically to their SG equivalent. However, STM can be misidentified principally as extraskeletal myxoid chondrosarcoma, thus requiring a relatively broad IHC panel for a specific diagnosis. • A series of 7 fine-needle aspiration (FNA) biopsy cases of soft tissue myoepithelioma (STM) is presented. • Correct FNA diagnosis was suggested in 43% of cases. • STM cytomorphology is composed of spindle and epithelioid cells, but 1 cell type is usually predominant. • STM FNA cytopathology imitates its salivary gland counterpart. • Extraskeletal myxoid chondrosarcoma is a close mimic of STM in FNA smears. SMARCB1/INI1-deficient soft tissue tumors with epithelioid and myxoid features are diverse and mainly include soft tissue myoepithelial tumor, extraskeletal myxoid chondrosarcoma, and the recently described myoepithelioma-like tumor of the vulvar region and myxoepithelioid tumor with chordoid features. Because of their overlapping features, the accurate diagnosis and classification of these tumors are often challenging. Herein, we report two unique cases of SMARCB1/INI1-deficient soft tissue neoplasm with epithelioid and myxoid features occurring in male paratesticular region. The first case was a 52-year-old man presented with an intermittent painful left paratesticular mass for 1 year. The second case was a 41-year-old man presented with a painless paratesticular mass on the right side for 3 months. Both patients underwent an orchiectomy. After 6 and 26 months of follow-up, both were alive with no evidence of recurrence or metastasis. In both cases, the tumor was relatively well-demarcated and showed monomorphic round to epithelioid cells arranged in a nested, trabecular, reticular, and corded pattern, setting in a myxohyalinized and vascularized matrix. The tumor cells showed relatively uniform round nuclei with vesicular chromatin and variably prominent nucleoli. No rhabdoid cells were identified. Mitoses numbered 3 and 2 per 10 high-power fields. Tumor necrosis or lymphovascular invasion was absent. Immunohistochemically, both tumors expressed epithelial membrane antigen (focal), calponin (focal), and CD99. SMARCB1/INI1 expression was deficient in both cases. In addition, case 1 diffusely expressed pan-cytokeratin, and case 2 diffusely expressed CD34 and synaptophysin. Molecular genetically, case 1 showed SMARCB1 homozygous deletion as detected by fluorescence in-situ hybridization (FISH), and case 2 demonstrated SMARCB1 copy number deletions by next-generation sequencing and SMARCB1 monoallelic deletion by FISH. Both cases lacked EWSR1 rearrangements by FISH. The overall clinicopathologic profiles of the two cases made it difficult to classify them as one of the established categories of SMARCB1/INI1-deficient mesenchymal tumors. Myoepitheliomas of the salivary glands have been described in laboratory mice, but not in rats. A 20-week-old Wistar (Han) female rat developed a white to grey firm mass at the left side of the neck. Histologically, the mass was unencapsulated and infiltrated the adjacent tissue. The tumour parenchyma was cell rich without acinar or tubular architecture. The tumour showed a palisading basal cell pattern adjacent to blood vessels. There were areas of necrosis filled with cellular debris. The tumour cells showed strong immunohistochemical labelling for pan-cytokeratin types I and II (AE1/AE3), pan-cytokeratin (cytokeratins 1, 5, 6 and 8), cytokeratin 5, cytokeratin 14, vimentin and podoplanin, and only very slight positivity for cytokeratin 8 in small areas. There was no expression of smooth muscle actin. Based on the histological appearance and the immunohistochemistry, the tumour was diagnosed as a malignant myoepithelioma of the parotid gland originating from the parotid duct. Myoepithelioma is a benign salivary gland neoplasm, locally invasive and uncommon in children. The aim of this study is to present and discuss the clinical, morphological and immunohistochemical findings of this tumor in a pediatric patient. This is a case report of a 12-year-old female patient showed a nodular lesion in the palate with osseous resorption with one year of evolution. After incisional biopsy, the histopathological examination revealed encapsulated lesion with proliferation of myoepithelial cells, including plasmacytoid, fusiform and clear cells, arranged in sheets, nests and cords. Immunohistochemical analysis revealed immunostaining for AE1/AE3, p63, cytokeratin-7, calponin, SMA and S-100. Based on these findings, diagnosis of myoepithelioma was confirmed. The lesion was completely removed through enucleation, in follow-up 2 years; there was no recurrence of the tumor. Myoepithelioma of salivary glands although rare in children may be invasive, however it is less aggressive than other tumors, a conservative treatment avoiding facial mutilation in pediatric patients is sufficient in these cases. Myoepithelioma is a rare form of salivary gland tumor composed entirely of myoepithelial cells. This tumor was formerly considered to be a subtype of pleomorphic adenoma; however, in the 1991 World Health Organization classification, it is listed as an independent entity. The most favorable site of occurrence of myoepithelioma is the parotid gland. Here, we report an extremely rare case of myoepithelioma of the upper lip. A 56-year-old woman presented with a painless mass on her upper lip. Magnetic resonance imaging revealed a 23 mm × 18 mm well-defined ovoid tumor. A benign minor salivary gland tumor was clinically suspected, and the patient underwent complete resection of the tumor under general anesthesia. The tumor was histopathologically diagnosed as a benign myoepithelioma of the minor salivary gland. Immunohistochemically, the tumor cells were positive for S-100 protein, AE1/AE3, CAM5.2, CK7, vimentin, and calponin, confirming the morphologic diagnosis of myoepithelioma. The patient's postoperative clinical course was uneventful, and satisfactory results were obtained both functionally and esthetically. Myoepithelial tumours of the salivary glands, including benign myoepithelioma and myoepithelial carcinoma, are rare tumours that account for <1% of all salivary gland tumours. We report a case of a 30-year-old male patient who presented with a complaint of a slow growing mass in the mouth for the past six months. Clinical examination revealed a single firm and tender swelling in left side of palate with an ulcerated surface. CT scan revealed a lesion infiltrating into the left side of the maxillary alveolus and extending into the maxillary sinus. Microscopically, diffuse sheets of spindle to ovoid shaped eosinophilic cells were noticed. Cellular atypia and mitotic figures were observed. Positive immunoreactivity was observed for vimentin and smooth muscle actin, along with a Ki-67 labelling index of 7%. Although the present case was diagnosed as a benign myoepithelioma, the presence of aggressive clinical and histopathological features prompted us to classify this tumour as a myoepithelioma with uncertain malignant potential. A 12-year-old female miniature dachshund was presented with a tan-white, firm mass (4 × 3 × 2 cm) occupying the left medial canthus. The mass compressed and displaced the left eye dorsally, and it was surgically removed. Microscopically, the mass was composed of interlacing bundles of spindle cells with clear cytoplasm and a small number of atypical glandular epithelial cells. Immunohistochemically, the spindle cells expressed p63, α-smooth muscle actin and calponin, and were negative for cytokeratin AE1/AE3. The glandular epithelial cells expressed cytokeratin AE1/AE3. Myoepithelioma is a very rare tumour. This tumor type has been reported in the soft tissue, ear, sinonasal cavity, breast and lung. Although rare, myoepithelioma can occur in bone. We describe the clinicopathological, immunohistochemical, and molecular features of 3 primary juxtacortical myoepithelioma/mixed tumor of bone. The patients were 2 males (13 and 23 years of age) and a 15-year-old female. The juxtacortical lesions were all located in the femur, and were surgically removed, 2 with wide margins and one with marginal margins. This latter tumor recurred locally 18 months later. The 3 patients were free of disease at 6 to 17 months follow-up. Histologically, all lesions showed a prominent multinodular architecture, and were formed by epithelioid and stellate elements, organized in solid sheets, or embedded in myxoid or chondroid matrix. Areas of osteoid formation were also observed. One tumor had the appearance of classical mixed tumor, showing aspects of duct formation and focal squamous differentiation. Immunohistochemically, all cases were positive for cytokeratins, epithelial membrane antigen, and S100 protein. The expression of other myoepithelial markers, including p63, glial fibrillary acid protein and calponin was more limited. No rearrangement of Ewing sarcoma region 1 (EWSR1) and fused in sarcoma (FUS) genes was observed by fluorescent in situ hybridization. To our knowledge, this is the first report of primary myoepitheliomas of bone arising at juxtacortical sites. These lesions must be distinguished from other benign and malignant bone and cartilage-forming surface tumors, including periosteal chondroma and chondrosarcoma, juxtacortical chondromyxoid fibroma, and periosteal and paraosteal osteosarcoma. Abstract: Myoepithelioma is a rare tumor, accounting for less than 1.5% of all salivary gland tumors. Generally, this tumor exhibits painless solid mass. We here report an extremely rare case of myoepithelioma with remarkable cystic formation in the parotid gland in an 80-year-old female. In this case, a malignant tumor with cystic formation of the parotid gland was clinically suspected following preoperative imaging examinations. Standard superficial parotidectomy was thus performed under general anesthesia. Histopathologic findings revealed that the tumor consisted of spindle cells, plasmacytoid cells, and epithelioid cells, and remarkable cystic formation was observed. Immunohistochemically, the neoplastic cells were positive for S-100, cytokeratin, vimentin, and calponin. The final histopathologic diagnosis was myoepithelioma. Myoepithelial lesions of the breast are extremely rare and can present with a diverse morphology. We report a case of malignant myoepithelioma characterized by proliferation of atypical oval to polygonal cells expressing typical myoepithelial markers. A 45-year-old lady presented with a mass in the left breast. Fine needle aspiration yielded a cellular smear with large papillae-like clusters of monomorphic cells with naked nuclei in the background. A diagnosis of sub-areolar sclerosing duct hyperplasia was made on cytology and the patient underwent excision. The surgical specimen showed a grey-white 5x3 cm mass on cut surface. Histopathology revealed mitotically active (5-6 per 10hpf) oval to polygonal cells tumor cells with clear to eosinophilic cytoplasm arranged in the form of nodules separated by dense sclerotic stroma mimicking clear cell or adenoid-cystic carcinoma. A diagnosis of malignant myoepithelioma was made as the cells were CK 14 and SMA positive, and negative for ER and PR on immunohistochemistry. We discuss the unusual morphological features of malignant myoepithelioma, cytological findings and the important differential diagnoses of malignant myoepthelial lesions. A high degree of suspicion with a keen eye for morphological details coupled with relevant immunohistochemistry will aid in arriving at the diagnosis. [ABSTRACT FROM AUTHOR]

Summary: We report 4 myoepitheial neoplasms involving the vulva (2 cases) or vagina (2 cases) in patients aged 40 to 45. Two tumors were composed entirely of ovoid or spindle-shaped cells, one entirely of epithelioid cells, and in the other, there was a mixture of spindled and epithelioid cells. Small foci of ductal differentiation with squamous metaplasia were present in one case and a minor stromal component, which varied from myxoid to hyalinized, in all cases. In all cases, the tumor cells were positive for epithelial markers (cytokeratins and/or epithelial membrane antigen) and the myoid markers α smooth muscle actin and calponin. Desmin was positive in 3 cases. S100 and p63 were positive in 1 of the 4 neoplasms. On the basis of the nuclear features and degree of mitotic activity, 2 neoplasms were classified as benign myoepitheliomas and 2 as myoepithelial carcinomas. Judging by the paucity of cases in the literature, myoepithelial neoplasms appear extremely rare in the vulvovaginal region with only 3 previous case reports of primary vulval tumors. As far as we are aware, this is the first description of a primary vaginal myoepithelial neoplasm. At these sites, myoepithelial tumors are liable to be misdiagnosed as a variety of other neoplasms because the pathologist may not think of the diagnosis. Abstract: Over the past decade, myoepithelial neoplasms have been recognized to arise primarily in skin and soft tissue. Similar to their salivary gland counterparts, owing to the plasticity of myoepithelial cells, these tumours show considerable cytological and architectural heterogeneity. Recognition of these uncommon neoplasms can be challenging, and a broad differential diagnosis may be considered. Criteria for malignancy in myoepithelial tumours of soft tissue have recently been established and are essentially cytological in nature, in contrast to the criteria used in salivary gland neoplasms. Myoepithelial carcinomas of soft tissue appear to be overrepresented in the paediatric population, in whom an aggressive clinical course is common. Whereas some cutaneous myoepitheliomas are similar to those in soft tissue, a distinctive ‘syncytial variant’ comprises at least 50% of dermal cases. This review will examine the pathological features, clinical behaviour and differential diagnosis of myoepithelial tumours of skin and soft tissue. [Copyright &y& Elsevier]

Abstract: Myoepithelial tumors of the soft tissues have only recently been described. Two cases of lower extremity malignant myoepithelial tumors are reported. One case of malignant mixed tumor overlying the gastrocnemius muscle was treated with wide local excision, but metastasized to regional lymph nodes 14 months after surgical excision. One patient with malignant myoepithelioma of the right lower leg was treated with limb amputation and is alive without disease at 46 months. A review of the literature discloses 120 additional cases of soft tissue myoepithelial tumors, 102 of which are myoepitheliomas and 18 are mixed tumors. Thirty-seven percent of the myoepitheliomas met the criteria for malignancy, and 33% of the mixed tumors were malignant. Of these, 30% had locally recurrent disease and 32% developed metastatic disease. Treatment benefit from chemotherapy and radiation therapy is unclear. [Copyright &y& Elsevier]

Summary: The present paper describes the clinical and pathologic features of a benign myoepithelioma of the parotid gland. Through 1985, only 42 other cases had been reported in the literature: three malignant and 39 benign. Fewer than 100 cases had been reported through 1993. The number of case reports of myoepitheliomas is increasing as pathologists have become more aware of their existence. This uncommon tumor is most commonly diagnosed in the parotid gland and in the minor salivary glands of the palate. The complex and varied morphologic expression of neoplastic myoepithelium have attracted numerous investigators who have presented valuable but often contradictory data. It is our opinion that these tumors are not as rare as is generally believed, but they are simply not well recognized. Myoepithelioma is a rare form of salivary gland tumor composed entirely of myoepithelial cells. It represents about 1 to 1.5% of all salivary gland tumors and is most frequently located in the parotid. The authors present a case of myoepithelioma of the gingiva. The tumor presented a focal strong positivity for cytokeratins, a diffuse positivity for S-100 protein, and a rare focal positivity for actin. No duct formation was observed. The article reports on the study that determines the appropriate management of myoepithelioma of the vulva. It shows that vulval myoepitheliomas can be low or high-grade malignancy and suggests that the grades, which determines the prognosis, should be the main factor to consider when planning the management. It concludes that the clinical behavior of the tumors of the vulva most probably follow the course of the myoepitheliomas that affect the soft tissues.

Salivary gland myoepitheliomas are rare tumours. We report the case of a 40-year-old female who presented to us with a hard palatal mass. The tumour was excised with a healthy rim of surrounding tissue. Histological and immunohistochemical analyses revealed the tumour to be a myoepithelioma of the clear cell variety. Myoepithelial tumors only recently have been recognized as occurring primarily in soft tissue, and only a few cases have been described in the literature. The occurrence of these tumors in the hand is even more limited. Summary: Pleomorphic adenoma of a buccal or molar minor salivary gland, which lies on the external aspect of buccinator, has not been reported previously. We report a case of a pleomorphic adenoma apparently arising from such a gland. Histologically there was marked cystic degeneration producing an apparently empty lumen surrounded by an encapsulated cellular mass. While the final diagnosis was pleomorphic adenoma, there were a number of features of myoepithelioma and the differences between these entities are discussed. [Copyright &y& Elsevier]

Introduction Malignant myoepethelial cells are rare cancer cells that commonly occur within the salivary glands. It rarely occurs in the vulva (1). These tumors are composed of epitheliod, plasmatoctoid, spindle-shaped, and clear cells with a characteristic of microscope and immunohistochemical appearance (2). The PubMed database was chosen for research, as it is the most widely used resource for medical literature. The following terms were used for myoepithelioma of the vulva and nine literatures were found. Case A 64-year-old woman came to our center for a second opinion after a biopsy result. She was complaining of a non-painful swelling of the left labia majora for six months. After that, she had excisional biopsy. The pathology results revealed Epitheliod Sarcoma. Therefore, the case was discussed in the Tumor Board Meeting and the decision was made for radical vulvectomy with bilateral inguinofemoral lymphadenectomy. We performed the needed surgery on the patient. The pathologist requested the original pathology specimens. The re-review of the original pathology slides showed the myoepithelioma of the vulva. The case was again discussed in the Tumor Board Meeting and recommended for an adjuvant radiotherapy. Discussion Malignant myoepithelioma of the vulva are rare tumors and need an expert pathologist. The Pathologist requested to review the first biopsy result that reported Epitheliod sarcoma and our surgery pathology results revealed myoepthelioma. Therefore, all available glass slides and pathology reports were re-reviewed. Immunohistochemically, the tumor cells were positive for these staining EMA, Actin (SMA), Pan-ck (5, 6, 8, 18), CD 34 and Calponin; whereas it was negative for GFAP and TLE 1. There were four mitosis figures per ten high-power field (HPF) and the Ki-69 index was 25%. Therefore, based on that, the final diagnosis was malignant myoepithelioma. The myoepithelioma can present as low or high-grade malignancy, and the grade is the one that determines the overall prognosis. Optimal treatment should comprise radical excision of the primary site, bilateral inguinal lymph node dissection and adjuvant radiotherapy (3). Conclusion An Expert pathologist is needed in this field due to the rarity of myoepitheloma. It would be more helpful if there was a bank for soft tissue sarcoma registration. Abstract: Myoepitheliomas of the parotid glands are very rare in the pediatric population. Only a few cases of myoepitheliomas of the salivary glands have been reported in children. They have some similar features with pleomorphic adenoma. However, they may be malignant and infiltrate locally. Hence, histopathologic examination should be considered to differentiate these tumors. The treatment of myoepitheliomas is complete removal of the tumor. We report a child with myoepithelioma of the parotid gland and review the related literature. [Copyright &y& Elsevier]
