Abstract

Pelvic and acetabular fractures are challenging injuries to treat. This review evaluates three-dimensional intraoperative imaging and navigation-guided percutaneous SI, trans-iliac, trans-sacral, and acetabular screw placement versus conventional methods performed with C-arm imaging. A systematic search of MEDLINE, Embase, and Cochrane was performed. Two reviewers independently extracted data into a collaborative data form designed a priori and piloted prior to its use. Descriptive statistics are presented where applicable. Summary statistics analysis was presented based on the fracture type. Continuous data such as fluoroscopic and operative time were compared with unpaired Student t -test and pooled data of revision rate and complications were compared with chi-square analysis with an alpha set at 0.05. The rate of complications using conventional fluoroscopy was 11.3% (26/230) compared to three-dimensional navigation (6.7% (40/597), X 2 (DF: 1, N = 827) = 4.79, p =.028.) which translated to a higher rate of revision surgeries in the conventional fluoroscopy group (10.9% vs. 0.8%) X 2 (DF: 1, N = 827) = 47.8, p ≤.001. Average fluoroscopic time was lower for studies using three-dimensional navigation (28.8 ± 14.3 s, n = 71) compared to conventional fluoroscopy (57.8 ± 4.2 s, n = 38, p ≤.001). Abstract

Chlorhexidine skin antisepsis is frequently recommended for most surgical procedures; however, it is unclear if these recommendations should apply to surgery involving traumatic contaminated wounds where povidone-iodine has previously been preferred. We aimed to compare the effect of aqueous 10% povidone-iodine versus aqueous 4% chlorhexidine gluconate on the risk of surgical site infection in patients who required surgery for an open fracture.

3. Randomized Controlled Trial of Sensor-Guided Knee Balancing Compared to Standard Balancing Technique in Total Knee Arthroplasty.

Abstract

Background: Despite advances in total knee arthroplasty (TKA) technology, up to 1 in 5 patients remain dissatisfied. This study sought to evaluate if sensor-guided knee balancing improves postoperative clinical outcomes and patient satisfaction compared to a conventional gap balancing technique.Methods: We undertook a prospective double-blind randomized controlled trial of patients presenting for elective primary TKA to determine a difference in TKA soft tissue balance between a standard gap balancing (tensiometer) approach compared to augmenting the balance using a sensor-guided device. The sensor-guided experimental group had adjustments made to achieve a balanced knee to within 15 pounds of intercompartmental pressure difference. Secondary outcomes included differences in clinical outcome scores at 6 months and 1 year postoperative, including the Oxford Knee Score and Knee Society Score and patient satisfaction.Results: The sample comprised of 152 patients, 76 controls and 76 experimental sensor-guided cases. Within the control group, 36% (27/76) of knees were unbalanced based on an average coronal plane intercompartmental difference >15 pounds, compared to only 5.3% (4/76) within the experimental group (P < .0001). There were no significant differences in 1-year postoperative flexion, Knee Society Score, or Oxford scores. Overall, TKA patient satisfaction at 1 year was comparable, with 81% of controls and experimental cases reporting they were very satisfied (P = .992).Conclusion: Despite the use of the sensor-guided knee balancer device to provide additional quantitative feedback in the evaluation of the soft tissue envelope during TKA, we were unable to demonstrate improved clinical outcomes or patient satisfaction compared to our conventional gap balancing technique. 4. Three-dimensional printing in orthopaedic surgery: a scoping review

Abstract

Three-dimensional printing (3DP) has become more frequently used in surgical specialties in recent years. These uses include pre-operative planning, patient-specific instrumentation (PSI), and patient-specific implant production.The purpose of this review was to understand the current uses of 3DP in orthopaedic surgery, the geographical and temporal trends of its use, and its impact on peri-operative outcomesOne-hundred and eight studies (N= 2328) were included, published between 2012 and 2018, with over half based in China.The most commonly used material was titanium.Three-dimensional printing was most commonly reported in trauma (N= 41) and oncology (N= 22). Pre-operative planning was the most common use of 3DP (N= 63), followed by final implants (N= 32) and PSI (N= 22).Take-home message: Overall, 3DP is becoming more common in orthopaedic surgery, with wide range of uses, particularly in complex cases. 3DP may also confer some important peri-operative benefits.Cite this article: EFORT Open Rev2020;5:430-441. 5. Anterior Capsule Reconstruction of the Native Hip: A Technique Guide.

Abstract

This is a technique guide for open anterior capsule reconstruction in unstable native hips due to Ehlers–Danlos syndrome. Ehlers–Danlos syndrome is a connective tissue disease that predisposes an individual to hypermobility of their soft tissues and recurrent subluxations and dislocations of their joints. This may lead to constant instability, guarding of the hip, and subsequent arthritic changes and poor quality of life. We demonstrate anterior capsule reconstruction of the hip using an Achilles tendon allograft performed through a direct anterior approach to the hip (Heuter's approach). This technique guide outlines in detail the surgical steps required to accomplish this reconstructive procedure. Furthermore, a narrated surgical video is provided that clearly depicts the anticipated result of the increased constrained provided to the hip from the anterior capsule reconstruction using an Achilles tendon allograft. 8. Continuous Infusion of Bupivacaine Following Total Knee Arthroplasty: A Randomized Control Trial Pilot Study.

Abstract

Abstract: An RCT pilot-study was conducted to assess efficacy of a 48-h continuous local infiltration of intra-articular bupivacaine (0.5% at 2 cc/h) versus placebo (0.5% saline at 2 cc/h) in decreasing PCA morphine consumption following TKA. Secondary outcomes included 48-h VAS pain, opioid side effects, length of stay, and knee function scores up to 1-year postoperatively. Of 67 randomized patients, 49 completed the trial including 24 bupivacaine, and 25 placebo patients. Mean 48-h PCA morphine consumption did not differ significantly between treatment (39 mg ± 27.1) and placebo groups (53 mg ± 30.4) (P = .137). The intervention did not improve pain scores, or any other outcome studied. Given study results we would conclude that analgesia outcomes with a multimodal analgesia regimen are not significantly improved by adding 48 h of 0.5% bupivacaine infiltration at 2 cc/h. The marketing of public goods offered by the Government has not been an area of focus, possibly due to the assumption that the client would seek out information on the service. In the current climate of information bombardment, Government may be required to engage in aggressive marketing of its initiatives so that benefits are realised by individuals and the country as a whole. This is especially the case when informing on benefits and costs of liberalized access to Barbados and its partner nations created by economic arrangements such as the WTO, CSME, EPA and others. This paper proposes that the deficiency in knowledge on the CARICOM Single Market and Economy (CSME) can be successfully addressed through the application of creative and sound marketing strategies. Marketing Communications Theory can be applied to ensure that the messages to the various stakeholders of the CSME would significantly raise awareness, elicit changes in attitude, and improve the perception and reputation of the CSME. Abstract

At the initiation of the COVID-19 pandemic, restrictions forced researchers to decide whether to continue their ongoing clinical trials. The PREPARE (Pragmatic Randomized Trial Evaluating Pre-Operative Alcohol Skin Solutions in Fractured Extremities) trial is a pragmatic cluster-randomized crossover trial in patients with open and closed fractures. PREPARE was enrolling over 200 participants per month at the initiation of the pandemic. We aim to describe how the COVID-19 research restrictions affected participant enrollment.

To systematically review and compare nonoperative and operative treatments for the management of periprosthetic distal femur fractures adjacent to total knee arthroplasties. Specific operative interventions compared included locked plating, retrograde intramedullary nailing (RIMN), and conventional (nonlocked) plating. Where possible, data were pooled to arrive at summary estimates of treatment effect odds ratios (ORs) with associated 95 confidence intervals (CIs).

However, this population’s tendency toward perfectionism can make it difficult to get them out of their comfort zones, a necessary part of stuttering therapy. Procedures such as modeling, setting attainable goals, and accelerating the instruction can help them experience success and transfer skills to environments outside the clinic room.

16. The Child Who Stutters: Guidelines for the Educator

Abstract

Suttering is a speech fluency disorder affecting 5% of children at some point during language development (Guitar, 1998), usually beginning between the ages of two and five (Andrews et al., 1983). As a result, it is a condition confronted by prekindergarten, kindergarten, and early elementary teachers, some of whom are unsure how to help (Williams, 1998). For example, wondering whether to call on the child who stutters and how to handle the teasing he* receives are but two areas of uncertainty for the teacher. No easy answers exist, in part because stuttering is such a highly individualized disorder (Bloodstein, 1981; Starkweather, 1987). Still, there is general advice that can help both teachers and children who stutter.

17. Haploid and Diploid Callus from Cotton Anthers1

Abstract

We have initiated and subcultured large quantities of callus tissue from the anthers of two species of cotton, Gossypium barbadense L. and G. hirsutum L. A liquid medium was developed and cell suspension cultures were generated from anther‐derived callus from G. hirsutum. Cytological examination was made only on anther callus from G. hirsutumand the preponderance of the tissue was diploid (2n=4x=52). Liquid suspension cultures from anther callus were also found to be primarily diploid. However, 13 cultures that grew more than four times faster than 527 diploid cultures had 83% haploid cells. Therefore, haploid tissue was produced in 2 to 3% of the anthers, and it grew obscurely within the diploid callus. The haploid tissue was identified and selected by its rapid growth rate in liquid culture.

18. Causal Attribution and Affective Response as Mediated by Task Performance and Self-Acceptance

Abstract

Predictions derived from cognitive consistency theories, self-esteem theories, and ego-serving-bias theory concerning how students would make attributional and affective responses to their academic performance were investigated. 202 university students completed a measure of self-acceptance of their college ability and made attributional and affective responses to an hypothetical examination performance. Analyses showed that students receiving positive feedback perceived greater internal causality and responded with greater positive affect than students receiving negative feedback. Self-acceptance did not moderate the attributions or affective reactions. The results supported the ego-serving-bias theory and provided partial support for self-esteem theory. Findings did not support predictions from cognitive-consistency theory.

20. Ambient noise interferes with auscultatory blood pressure measurement during exercise

Abstract

This study was designed to investigate whether the acoustical characteristics of the Korotkoff sounds (K-sounds) were altered during exercise and/or masked by the ambient noise. After signing informed consent, 11 subjects (8 females, 3 males; 27 ± 2 yr; 166.2 ± 3.2 cm; 62± 5 kg; means ± SD) underwent a cycle ergometer exercise test that increased in workload by 30 W every 3 min until volitional fatigue. Heart rate, auscultatory systolic (SBP) and diastolic blood pressure (DBP), and oxygen consumption were monitored 1 and 2 min into each work stage. The auscultatory K-sounds were recorded with a microphone mounted in a stethoscope tube for later frequency (Hz) and sound pressure level (dB SPL) analysis. Frequency and SPL of ambient noise (99 ± 13 Hz and 64 ± 1 db at maximum, respectively) increased during the exercise test to magnitudes similar to the SBP and DBP K-sounds (166 Hz, 66 db; and 128 Hz, 69 db, respectively). Additionally, the ambient noise was responsible for a significant damping of the frequency and SPL of the measured blood pressure K-sounds and a rise in the measured frequency of the SBP K-sounds. Furthermore, we observed “inaudible” K-sounds at lower frequencies than adjoining audible K-sounds (100 Hz vs 126 Hz), supporting the known under-estimation of SBP by auscultation. The increase in ambient noise during exercise testing dampens and may mask the auscultatory K-sounds, thus making detection of the proper K-sounds during exercise difficult at best. Furthermore, the presence of inaudible K-sounds may further explain the published discrepancies between auscultatory and intraarterial blood pressure measurements during exercise.

21. 2001: A space odyssey: A warning before its time

Abstract

This essay examines the visual text of Stanley Kubrick's 2001: A Space Odyssey. Since previous commentaries lacked a rhetorical framework sufficiently complex to explain the nuances of 2001, this essay develops a formal model from a subset of Kenneth Burke's perspective on rhetoric, and uses the model to integrate the four basic symbolic elements of the film into a coherent message. It is argued that 2001 is essentially a “religious” film, a cinematic warning against our dependence upon the machines and tools we have created. The film's climactic sequence is explained as the visualization of ultimate transcendence.

23. A study of the bonding forces between the epithelial cells surrounding the resin canals of slash pine (Pinus elliottii Engelm.) holocellulose

Abstract

Summary Although the bonds between the cellular components of pine epithelia (isolated from holocellulose) can be strengthened by the addition of multivalent ions such as those of calcium, iron, uranium, etc., it was observed that these bonds are only weakened and not destroyed by the removal of the ions from the tissues. Reagents, such as KOH and K2SO4, which do not yield soluble calcium salts, do not cause a discernible weakening of the bonds between the cellulose components of epithelia pretreated with Ca ion. Washing the weakened epithelia with water causes them to swell. The separation of epithelial cells occurs only after the application of mechanical forces to the tissues. Electron microscopic examination of the swollen and unswollen tissues shows that fibrils and lamellae extend between the individual cells which must be destroyed by mechanical action before cell separation can occur. The swelling of the tissues by washing after removal of multivalent ions was attributed to the expansion of electrical double layers and to the increasing osmotic pressure from the increasing Donnan potential, while the difference in swelling as a result of HCl treatment vs. KCl and K4Fe(CN)6 was attributed to differences in charge development and to hydrogen bonding between acidic components. The restrengthening of the tissue as a result of the readdition of multivalent ion was attributed to the collapse of the electrical double layers and the reduction of the Donnan potential, with the re-forming of bonds between the components of the epithelia. Evidence suggesting that multivalent ions actually participate in bonding was obtained and although the nature of the bonding cannot be determined, both salt formation and complex formation between the ions and the components of the tissues is suggested.

Abstract

Cluster randomized crossover trials are often faced with a dilemma when selecting an optimal model of consent, as the traditional model of obtaining informed consent from participant's before initiating any trial related activities may not be suitable. We describe our experience of engaging patient advisors to identify an optimal model of consent for the PREP-IT trials. This paper also examines surrogate measures of success for the selected model of consent.

Abstract

Pragmatic trials in comparative effectiveness research assess the effects of different treatment, therapeutic, or healthcare options in clinical practice. They are characterized by broad eligibility criteria and large sample sizes, which can lead to an unmanageable number of participants, increasing the risk of bias and affecting the integrity of the trial. We describe the development of a sampling strategy tool and its use in the PREPARE trial to circumvent the challenge of unmanageable work flow.

Bem's concept of androgyny and Snyder's concept of self-monitoring emphasize traits characterized by behavioral flexibility across situations. In the present study two measures of androgyny were correlated with self-monitoring, but only one of the measures was significantly related to self-monitoring. The findings suggest the need to re-examine the notion of androgyny.
