1. Gravitational instability in a planet-forming disk

Abstract: The canonical theory for planet formation in circumstellar disks proposes that planets are grown from initially much smaller seeds1–5. The long-considered alternative theory proposes that giant protoplanets can be formed directly from collapsing fragments of vast spiral arms6–11induced by gravitational instability12–14—if the disk is gravitationally unstable. For this to be possible, the disk must be massive compared with the central star: a disk-to-star mass ratio of 1:10 is widely held as the rough threshold for triggering gravitational instability, inciting substantial non-Keplerian dynamics and generating prominent spiral arms15–18. Although estimating disk masses has historically been challenging19–21, the motion of the gas can reveal the presence of gravitational instability through its effect on the disk-velocity structure22–24. Here we present kinematic evidence of gravitational instability in the disk around AB Aurigae, using deep observations of 13CO and C18O line emission with the Atacama Large Millimeter/submillimeter Array (ALMA). The observed kinematic signals strongly resemble predictions from simulations and analytic modelling. From quantitative comparisons, we infer a disk mass of up to a third of the stellar mass enclosed within 1″ to 5″ on the sky.

2. An Ultra-Low Cost and Multicast-Enabled Asynchronous NoC for Neuromorphic Edge Computing

Abstract: Biological brains are increasingly taken as a guide toward more efficient forms of computing. The latest frontier considers the use of spiking neural-network-based neuromorphic processors for near-sensor data processing, in order to fit the tight power and resource budgets of edge computing devices. However, a prevailing focus on brain-inspired computing and storage primitives in the design of neuromorphic systems is currently bringing a fundamental bottleneck to the forefront: chip-scale communications. While communication architectures (typically, a network-on-chip) are generally inspired by, or even borrowed from, general purpose computing, neuromorphic communications exhibit unique characteristics: they consist of the event-driven routing of small amounts of information to a large number of destinations within tight area and power budgets. This article aims at an inflection point in network-on-chip design for brain-inspired communications, revolving around the combination of cost-effective and robust asynchronous design, architecture specialization for short messaging and lightweight hardware support for tree-based multicast. When validated with functional spiking neural network traffic, the proposed NoC delivers energy savings ranging from 42% to 71% over a state-of-the-art NoC used in a real multi-core neuromorphic processor for edge computing applications.

Abstract: Electrode-confined molecular catalysts are promising systems to enable the efficient conversion of CO2to useful products. Here, we describe the development of an original molecular cathode for CO2reduction to CO based on the noncovalent integration of a tetraazamacrocyclic Co complex to a carbon nanotube-based matrix. Aqueous electrochemical characterization of the modified electrode allowed for clear observation of a change of redox behavior of the Co center as surface concentration was tuned, highlighting the impact of the catalyst microenvironment on its redox properties. The molecular cathode enabled efficient CO2-to-CO conversion in fully aqueous conditions, giving rise to a turnover number (TONCO) of up to 20 × 103after 2 h of constant electrolysis at a mild overpotential (η = 450 mV) and with a faradaic efficiency for CO of about 95%. Post operando measurements using electrochemical techniques, inductively coupled plasma, X-ray photoelectron spectroscopy and X-ray absorption spectroscopy characterization of the films demonstrated that the catalysis remained of molecular nature, making this Co-based electrode a new promising alternative for molecular electrocatalytic conversion of CO2-to-CO in fully aqueous media.

Abstract: Pancreatic ductal adenocarcinoma (PDAC) has a dismal prognosis, mostly due to the high rate of distant dissemination. However, growing evidence shows that isolated lung recurrence or metastases (ILM) from PDAC are not only less common, but also correlated with a better prognosis. Lung-only recurrence after surgery occurs later in time and is associated with more favorable prognostic characteristics of the primary tumor. Moreover, recent findings suggest that this specific site of metastases is characterized by an immunologically “hot” microenvironment and a more favorable molecular profile that could possibly justify its clinical behavior. Thus, ILM from PDAC emerge as a distinct entity, that might also benefit from a different therapeutic approach, possibly with the integration of surgery and de-intensified chemotherapy regimens, especially in selected patients. In this review we delve into the current scientific evidence on the clinical and biological characteristics of isolated LM from PDAC, also focusing on concerns with their diagnostic process and the therapeutic options for the management of this subset of patients.

Abstract: Iron–sulfur (Fe–S) clusters are essential inorganic cofactors dedicated to a wide range of biological functions, including electron transfer and catalysis. Specialized multiprotein machineries present in all types of organisms support their biosynthesis. These machineries encompass a scaffold protein, on which Fe–S clusters are assembled before being transferred to cellular targets. Here, we describe the first characterization of the native Fe–S cluster of the anaerobically purified SufBC2D scaffold from Escherichia coliby XAS and Mössbauer, UV–visible absorption, and EPR spectroscopies. Interestingly, we propose that SufBC2D harbors two iron–sulfur-containing species, a [2Fe-2S] cluster and an as-yet unidentified species. Mutagenesis and biochemistry were used to propose amino acid ligands for the [2Fe-2S] cluster, supporting the hypothesis that both SufB and SufD are involved in the Fe–S cluster ligation. The [2Fe-2S] cluster can be transferred to ferredoxin in agreement with the SufBC2D scaffold function. These results are discussed in the context of Fe–S cluster biogenesis.

11. The multicenter experience in the multidisciplinary Italian breast units: a review and update

Abstract: A breast unit is a multidisciplinary center specialized in the management of women with breast diseases, including breast cancer (BC). It represents a care path, passing from screening activities to diagnostic investigations, from surgery to the definition of the therapeutic strategy, from psychophysical rehabilitation to long-term checks (follow-up), and up to genetic counseling. Since 2006, following a resolution issued by the European Parliament to urge member states to activate multidisciplinary breast centers by 2016, work has been underway throughout Italy to improve the management of women with BC. In Italy, the State-Regions agreement was signed on 18 December 2014, sanctioning the establishment of breast units. These centers must adhere to specific quality criteria and requirements. In 2020, the experts of the EUSOMA group (European Society of Breast Cancer Specialists), in their latest document published, expanded the requirements of the breast units. Furthermore, Senonetwork was founded in 2012 with the aim of allowing BC to be treated in breast units that comply with European requirements to ensure equal treatment opportunities for all Italian women. Indeed, the available data indicate that the BC patient has a greater chance of better treatment in the breast units with a multidisciplinary team, thus increasing the survival rate with a better quality of life, compared to those managed in nonspecialized structures. The present review is a perspective on the current Italian reality of breast units, updated with the available literature and the most recent epidemiological data from Senonetwork and AgeNaS.

Abstract: Influenza B is one of the infective agents that can cause rapid and fatal myocarditis in children. Here, we describe a fatal case of myocarditis in a previously healthy child, after infection with an influenza B/Victoria-lineage virus during the 2022–23 epidemic season in Italy. Influenza B virus was isolated also in a second case, a younger family member showing only a mild influenza-like illness. Genotypic and phenotypic analyses have been performed on both virus samples and results showed that HA1 sequences were identical and genetically and antigenically related to other B viruses circulating in 2022–23 season in Italy. However, a D129N substitution was found in the receptor binding domain of the HA of the two viruses, not detected in other circulating viruses in Italy but only in a proportion of those circulating in other European countries. Phenotypic analyses assessed the susceptibility towards either neuraminidase inhibitors and baloxavir. We estimated the individual and combined prevalence of underweight or thinness and obesity, and their changes, from 1990 to 2022 for adults and school-aged children and adolescents in 200 countries and territories.

Abstract: All classic cardiovascular disease (CVD) risk factors are still relevant in Europe, irrespective of regional area.The differences in the associations of CVD risk factors with overt CVD between regions of Europe are generally small.Minor temporal hazard decreases were observed for non-HDL cholesterol and systolic blood pressure, while a minor hazard increase was observed for body mass index.Graphical Abstract

16. Estimation of a SOPDT process transfer function for PID tuning

Abstract: The paper deals with the estimation of a Second-Order-Plus-Dead-Time (SOPDT) process transfer function for the tuning of a Proportional-Integral-Derivative (PID) controller. In particular, different methodologies for the estimation of the transfer function parameters based on the open-loop process step response are proposed and compared to an existing one that has been recently presented in the literature. A pole-zero cancellation strategy is then employed to tune the PID controller. Advantages and disadvantages of the different methods are then discussed by considering the obtained results and implementation issues.

17. Observations of entomophagy across Benin – practices and potentials

Abstract: Food security is a critical issue for many low-income countries, particularly in Sub-Saharan Africa. Appropriately identifying and utilising local resources can provide sustainable solutions to food security problems. Insects, which are traditionally consumed in many regions of the world, represent one such resource. Insects can be nutritionally rich and therefore could be used to address issues of malnutrition. A first step towards utilising insects as a resource is identifying which ones are traditionally consumed. We present data collected between 2005 and 2012 on insects eaten by communities across Benin, West Africa. A combination of literature research, field collections, community focus groups and targeted interviews were employed. Data on four ethnic groups is presented: the Anii, Fon, Nagot and Waama. Twenty-nine arthropods species are eaten across Benin. The predominant orders are Orthoptera (48 %) and Coleoptera (41 %). New families of edible arthropods in West Africa include: Bradyporidae (Orthoptera), Coreidae (Hemiptera), Dytiscidae (Coleoptera), Ixodidae (Acari). Insect collection is an ancestral tradition in all the described communities: however, there are considerable differences in preferences and collection methods among ethnic groups. Currently there is little valorisation of insects as a food product in Benin, in contrast to neighbouring countries. In light of considerable malnutrition in Benin among young children, promoting this tradition and implementing small scale captive rearing of selected species could improve food security.

18. miR-122 and miR-21 are Stable Components of miRNA Signatures of Early Lung Cancer after Validation in Three Independent Cohorts

Abstract: Several panels of circulating miRNAs have been reported as potential biomarkers of early lung cancer, yet the overlap of components between different panels is limited, and the universality of proposed biomarkers has been minimal across proposed panels. To assess the stability of the diagnostic potential of plasma miRNA signature of early lung cancer among different cohorts, a panel of 24 miRNAs tested in the frame of one lung cancer screening study (MOLTEST-2013, Poland) was validated with material collected in the frame of two other screening studies (MOLTEST-BIS, Poland; and SMAC, Italy) using the same standardized analytical platform (the miRCURY LNA miRNA PCR assay). On analysis of selected miRNAs, two associated with lung cancer development, miR-122 and miR-21, repetitively differentiated healthy participants from individuals with lung cancer. Additionally, miR-144 differentiated controls from cases specifically in subcohorts with adenocarcinoma. Other tested miRNAs did not overlap in the three cohorts. Classification models based on neither a single miRNA nor multicomponent miRNA panels (24-mer and 7-mer) showed classification performance sufficient for a standalone diagnostic biomarker (AUC, 75%, 71%, and 53% in MOLTEST-2013, SMAC, and MOLTEST-BIS, respectively, in the 7-mer model). The performance of classification in the MOLTEST-BIS cohort with the lowest contribution of adenocarcinomas was increased when only this cancer type was considered (AUC, 60% in 7-mer model).

In breast cancer, radiotherapy to regional lymph nodes aims to reduce risks of recurrence and death. Its effects have been studied in randomised trials, some before the 1980s and some after. We aimed to assess the effects of regional node radiotherapy in these two eras.

Abstract: IMPORTANCE: Sentinel lymph node biopsy (SLNB) is the standard of care for axillary node staging of patients with early breast cancer (BC), but its necessity can be questioned since surgery for examination of axillary nodes is not performed with curative intent. OBJECTIVE: To determine whether the omission of axillary surgery is noninferior to SLNB in patients with small BC and a negative result on preoperative axillary lymph node ultrasonography. DESIGN, SETTING, AND PARTICIPANTS: The SOUND (Sentinel Node vs Observation After Axillary Ultra-Sound) trial was a prospective noninferiority phase 3 randomized clinical trial conducted in Italy, Switzerland, Spain, and Chile. A total of 1463 women of any age with BC up to 2 cm and a negative preoperative axillary ultrasonography result were enrolled and randomized between February 6, 2012, and June 30, 2017. Of those, 1405 were included in the intention-to-treat analysis. Data were analyzed from October 10, 2022, to January 13, 2023. INTERVENTION: Eligible patients were randomized on a 1:1 ratio to receive SLNB (SLNB group) or no axillary surgery (no axillary surgery group). MAIN OUTCOMES AND MEASURES: The primary end point of the study was distant disease–free survival (DDFS) at 5 years, analyzed as intention to treat. Secondary end points were the cumulative incidence of distant recurrences, the cumulative incidence of axillary recurrences, DFS, overall survival (OS), and the adjuvant treatment recommendations. RESULTS: Among 1405 women (median [IQR] age, 60 [52-68] years) included in the intention-to-treat analysis, 708 were randomized to the SLNB group, and 697 were randomized to the no axillary surgery group. Overall, the median (IQR) tumor size was 1.1 (0.8-1.5) cm, and 1234 patients (87.8%) had estrogen receptor–positive ERBB2 (formerly HER2 or HER2/neu), nonoverexpressing BC. In the SLNB group, 97 patients (13.7%) had positive axillary nodes. The median (IQR) follow-up for disease assessment was 5.7 (5.0-6.8) years in the SLNB group and 5.7 (5.0-6.6) years in the no axillary surgery group. Five-year distant DDFS was 97.7% in the SLNB group and 98.0% in the no axillary surgery group (log-rank P = .67; hazard ratio, 0.84; 90% CI, 0.45-1.54; noninferiority P = .02). A total of 12 (1.7%) locoregional relapses, 13 (1.8%) distant metastases, and 21 (3.0%) deaths were observed in the SLNB group, and 11 (1.6%) locoregional relapses, 14 (2.0%) distant metastases, and 18 (2.6%) deaths were observed in the no axillary surgery group. CONCLUSIONS AND RELEVANCE: In this randomized clinical trial, omission of axillary surgery was noninferior to SLNB in patients with small BC and a negative result on ultrasonography of the axillary lymph nodes. These results suggest that patients with these features can be safely spared any axillary surgery whenever the lack of pathological information does not affect the postoperative treatment plan. Abstract: Background: Laparoscopic cholecystectomy (LapC) is one of the most frequently performed surgical procedures worldwide. Reaching technical competency in performing LapC is considered one essential task for young surgeons. Investigating the learning curve for LapC (LC-LapC) may provide important information regarding the learning process and guide the training pathway of residents, improving educational outcomes. The present study aimed to investigate LC-LapC among general surgery residents (GSRs). Methods: Operative surgical reports of consecutive patients undergoing LapC performed by GSRs attending the General Surgery Residency Program at the University of Milan were analysed. Data on patient- and surgery-related variables were obtained from the ICD-9-CM diagnosis codes and gathered. A multidimensional assessment of the LC was performed through Cumulative Sum (CUSUM) and Risk-Adjusted (RA)-CUSUM analysis. Results: 340 patients operated by 6 GSRs were collected. The CUSUM and RA-CUSUM graphs based on surgical failures allowed to distinguish two defined phases for all GSRs: an initial phase ending at the peak, so-called learning phase, followed by a phase in which there was a significant decrease in failure incidence, so-called proficiency phase. The learning phase was completed for all GSRs at most within 25 procedures, but the trend of the curves and the number of procedures needed to achieve technical competency varied among operators ranging between 7 and 25. Conclusions: The present study suggested that at most 25 procedures might be sufficient to acquire technical competency in LapC. The variability in the number of procedures needed to complete the LC, ranging between 7 and 25, could be due to the heterogeneous scenarios in which LapC was performed, and deserves to be investigated through a prospective study involving a larger number of GSRs and institutions. 24. Comorbidity and in-hospital mortality in peritoneal dialysis patients: data of the Emilia Romagna region of Italy.

Abstract: OBJECTIVE: Kidney failure increases in-hospital mortality (IHM); however, comorbidity is crucial for predicting mortality in dialysis patients. Our aim was to evaluate the impact of comorbidity, assessed by modified Elixhauser index (mEI), Charlson Comorbidity Index (CCI), and age-adjusted CCI, on IHM in a cohort of peritoneal dialysis patients admitted to hospitals of the Emilia Romagna region (ERR) of Italy. PATIENTS AND METHODS: All hospital admissions of peritoneal dialysis patients recorded between 2007 and 2021 in the ERR database were analyzed. The International Classification of Diseases, 9th Revision, Clinical Modification (ICD-9- CM) was used for detecting diagnoses and procedures, and the inclusion criterion was code 5498. Comorbidity burden was evaluated by three different scores, and hemodialysis (HD) treatment need was considered. IHM was our outcome. RESULTS: During the 15 years of the study, 3,242 hospitalized peritoneal dialysis patients (62.7% males) were evaluated. Mean age was 62.8±20.6 years, 9.6% underwent HD, and IHM was 5.9% (n=192). IHM mortality was stable throughout the study period. Deceased subjects were older, were hospitalized longer, had a higher comorbidity burden, and had a higher percentage of HD treatment needs than survivors. Age, male sex, comorbidity burden, and HD treatment were predictors of IHM. Receiver operating characteristics (ROC) analysis confirmed the impact of comorbidity burden on IHM, especially when age was considered. CONCLUSIONS: We conclude that in male, elderly hospitalized peritoneal dialysis patients with failing dialysis technique, comorbidity burden should be considered being a predictor of IHM. Abstract: Anatomical abnormalities in the thoracic outlet syndrome are very frequent, but radiological images are not always reliable. Therefore, the different features and abilities of CT angiogram and brachial plexus MRI to detect anatomical abnormalities have been investigated. All radiological and surgical records of patients operated for thoracic outlet syndrome between January 2011 and December 2018 were retrospectively analyzed. Considering the CT angiogram and/or brachial plexus MRI reports of the preoperative period, the performance of these investigations was evaluated by calculating the precision (P), sensitivity (Recall) and Hamming loss (L). From a total of 107 patients screened, 84 were eligible for study inclusion. Out of these, 46 had surgical abnormalities (group 1), while 38 did not (group 2). Among them, 30 patients have performed CT angiogram (subgroup A), 32 brachial plexus MRI (subgroup B) and 22 both (subgroup C). The best result is obtained when both diagnostic examinations are performed (subgroup C: P = 0.62; recall = 0.52; L = 0.038). Prescribing both investigations in selected patients increases the probability of finding anatomic anomalies and facilitates preoperative planning. However, as the radiological images are not completely exhaustive, the surgeon should always actively search and remove any potential source of compression on the nervous vascular bundle along the thoracic outlet. Abstract: Synovial sarcomas occurring as primary nerve tumors (SSPN) are rare and only 69 cases of SSPNs are reported in literature. Despite the little data available, SSPNs differ from other SSs in some distinctive aspects such as epidemiology, location, and early onset of symptoms. SSPN are consequently underdiagnosed and easily mistaken for benign or malignant peripheral nerve sheath tumors (PNST). Therefore, cytogenetic or molecular testing becomes essential in order to make a correct diagnosis. This article deals with an extremely rare case of monophasic SSPN of the posterior cords of the right brachial plexus. To our knowledge, this is only the tenth case of intraneural synovial sarcoma involving the brachial plexus. We report the case of a 64-year-old man, who came to our attention due to a slow-growing painful right axillary neoformation, approximately 25 mm in size. The patient did not show any neurological impairments. Ultrasonography and constrast MRI showed a heterogeneous mass arising from the posterior cord of the right brachial plexus, resembling a schwannoma. The patient underwent total resection of tumor and capsule. Histologically, a diagnosis of monophasic synovial sarcoma was made based on histologic features and the immunohistochemical profile. We report a rare primary synovial sarcoma of the brachial plexus. Given its rarity, the diagnosis may be challenging and requires a core biopsy or the surgical specimen to permit immune-molecular analysis. 30. A Risk Model to Predict the Delivery of Adjuvant Chemotherapy Following Lung Resection in Patients With Pathologically Positive Lymph Nodes.

Abstract: To investigate factors associated with the ability to receive adjuvant chemotherapy in patients with pathological N1 and N2 stage after anatomic lung resections for non-small cell lung cancer (NSCLC). Multicenter retrospective analysis on 707 consecutive patients found pathologic N1 (pN1) or N2 (pN2) disease following anatomic lung resections for NSCLC (2014-2019). Multiple imputation logistic regression was used to identify factors associated with adjuvant chemotherapy and to develop a model to predict the probability of starting this treatment. The model was externally validated in a population of 253 patients. In the derivation set, 442 patients were pN1 and 265 pN2. 58% received at least 1 cycle of adjuvant chemotherapy. The variables significantly associated with the probability of starting chemotherapy after multivariable regression analysis were: younger age (p < 0.0001), Body Mass Index (BMI) (p = 0.031), Forced Expiratory Volume in 1 second (FEV1) (p = 0.037), better performance status (PS) (p < 0.0001), absence of chronic kidney disease (CKD) (p = 0.016), resection lesser than pneumonectomy (p = 0.010). The logit of the prediction model was: 6.58 -0.112 x age +0.039 x BMI +0.009 x FEV1 -0.650 x PS -1.388 x CKD -0.550 x pneumonectomy. The predicted rate of adjuvant chemotherapy in the validation set was 59.2 and similar to the observed 1 (59%, p = 0.87) confirming the model performance in external setting. This study identified several factors associated with the probability of initiating adjuvant chemotherapy after lung resection in node-positive patients. This information can be used during preoperative multidisciplinary meetings and patients counseling to support decision-making process regarding the timing of systemic treatment. Abstract: In this work, the role of cardiac biomarkers measured from blood to predict cardiovascular events and death is tested in individuals of the general population and particularly in those with known diabetes. The work is based on a cooperation of different population studies across Europe and includes more than 90 000 individuals, with more than 6000 having diabetes. We could demonstrate that the determination of three cardiac biomarkers helps to identify individuals at highest risk for cardiovascular events (e.g. myocardial infarction or stroke) and death, despite accounting for known cardiovascular risk factors in these individuals. Therefore, these biomarkers should be considered for routine risk assessment for cardiovascular diseases and could improve the early identification of high-risk individuals, consequently leading to an earlier initiation of preventive therapies.

33. Environmental complaint insights through text mining based on the driver, pressure, state, impact, and response (DPSIR) framework: Evidence from an Italian environmental agency

Abstract: Individuals, local communities, environmental associations, private organizations, and public representatives and bodies may all be aggrieved by environmental problems concerning poor air quality, illegal waste disposal, water contamination, and general pollution. Environmental complaints represent the expressions of dissatisfaction with these issues. As the time-consuming of managing a large number of complaints, text mining may be useful for automatically extracting information on stakeholder priorities and concerns. The paper used text mining and semantic network analysis to crawl relevant keywords about environmental complaints from two online complaint submission systems: online claim submission system of Regional Agency for Prevention, Environment and Energy (Arpae) (“Contact Arpae”); and Arpae's internal platform for environmental pollution (“Environmental incident reporting portal”) in the Emilia-Romagna Region, Italy. We evaluated the total of 2477 records and classified this information based on the claim topic (air pollution, water pollution, noise pollution, waste, odor, soil, weather-climate, sea-coast, and electromagnetic radiation) and geographical distribution. Then, this paper used natural language processing to extract keywords from the dataset, and classified keywords ranking higher in Term Frequency-Inverse Document Frequency (TF-IDF) based on the driver, pressure, state, impact, and response (DPSIR) framework. This study provided a systemic approach to understanding the interaction between people and environment in different geographical contexts and builds sustainable and healthy communities. The results showed that most complaints are from the public and associated with air pollution and odor. Factories (particularly foundries and ceramic industries) and farms are identified as the drivers of environmental issues. Citizen believed that environmental issues mainly affect human well-being. Moreover, the keywords of “odor”, “report”, “request”, “presence”, “municipality”, and “hours” were the most influential and meaningful concepts, as demonstrated by their high degree and betweenness centrality values. Keywords connecting odor (classified as impacts) and air pollution (classified as state) were the most important (such as “odor-burnt plastic” and “odor-acrid”). Complainants perceived odor annoyance as a primary environmental concern, possibly related to two main drivers: “odor-factory” and “odors-farms”. The proposed approach has several theoretical and practical implications: text mining may quickly and efficiently address citizen needs, providing the basis toward automating (even partially) the complaint process; and the DPSIR framework might support the planning and organization of information and the identification of stakeholder concerns and priorities, as well as metrics and indicators for their assessment. Therefore, integration of the DPSIR framework with the text mining of environmental complaints might generate a comprehensive environmental knowledge base as a prerequisite for a wider exploitation of analysis to support decision-making processes and environmental management activities.

Abstract: In cystic fibrosis (CF), deletion of phenylalanine 508 (F508del) in the CF transmembrane conductance regulator (CFTR) is associated to misfolding and defective gating of the mutant channel. One of the most promising CF drug targets is the ubiquitin ligase RNF5, which promotes F508del-CFTR degradation. Recently, the first ever reported inhibitor of RNF5 was discovered, i.e., the 1,2,4-thiadiazol-5-ylidene inh-2. Here, we designed and synthesized a series of new analogues to explore the structure–activity relationships (SAR) of this class of compounds. SAR efforts ultimately led to compound 16, which showed a greater F508del-CFTR corrector activity than inh-2, good tolerability, and no toxic side effects. Analogue 16increased the basal level of autophagy similar to what has been described with RNF5 silencing. Furthermore, co-treatment with 16significantly improved the F508del-CFTR rescue induced by the triple combination elexacaftor/tezacaftor/ivacaftor in CFBE41o–cells. These findings validate the 1,2,4-thiadiazolylidene scaffold for the discovery of novel RNF5 inhibitors and provide evidence to pursue this unprecedented strategy for the treatment of CF.

Abstract: Metaplastic breast cancer (MpBC) is a rare and aggressive histologic subtype of breast cancer (BC) characterized by the presence of at least two cellular types, commonly epithelial and mesenchymal components. Despite growing evidence that MpBC is a unique entity, it has long been treated as a variant of nonspecial type (NST) BC. MpBC typically shows the phenotype of triple-negative breast cancer (TNBC), but compared to NST-TNBC, it is a relatively chemorefractory tumor associated with worse outcomes. Therefore, there is an urgent need to develop management guidelines specifically for MpBC to improve the prognosis of patients with early MpBC. This expert consensus aims to guide diagnosis and standardize clinical management of early MpBC among treating physicians. We provide guidance on the challenging radiological and pathological diagnosis of MpBC. Evidence on the involvement of genetic predisposition in the development of MpBC is also explored. We emphasize the importance of a multidisciplinary approach for the treatment of patients with early MpBC. The optimal surgery and radiotherapy approach is presented, as well as the opportunity offered by novel therapeutic approaches to increase treatment response in this chemoresistant subtype. Appropriate management of patients with MpBC is critical to reduce the high risk of local and distant recurrence that characterizes this disease.

Abstract: Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) is an emerging non-Hodgkin’s lymphoma that occurs exclusively in patients with breast implants. The estimated risk of developing BIA-ALCL from exposure to breast implants is largely based on approximations about patients at risk. There is a growing body of evidence regarding the presence of specific germline mutations in patients developing BIA-ALCL, rising interest regarding possible markers of genetic predisposition to this type of lymphoma. The present paper focuses attention on BIA-ALCL in women with a genetic predisposition for breast cancer. We report our experience at the European Institute of Oncology, Milan, Italy, describing a case of BIA-ALCL in a BRCA1mutation carrier who developed BIA-ALCL 5 years after implant-based post mastectomy reconstruction. She was treated successfully with an en-bloc capsulectomy. Additionally, we review the available literature on inherited genetic factors predisposing to the development of BIA-ALCL. In patients with genetic predisposition to breast cancer (mainly TP53and BRCA1/2germline mutations), BIA-ALCL prevalence seems to be higher and time to onset appears to be shorter in comparison to the general population. These high-risk patients are already included in close follow-up programs allowing the diagnosis of early-stage BIA-ALCL. For this reason, we do not believe that a different approach should be followed for postoperative surveillance.

Abstract: The increased use of neoadjuvant chemotherapy (NACT) has changed the approach to breast surgery. NACT allows de-escalation of surgery by both increasing breast conservation rates (up to 40%), the initial goal of this chemotherapy, and in particular it permits reduces axillary surgery. Furthermore, in relation to the molecular characteristics of the tumor we can have a pathological complete response (pCR) ranging from 20 to 80%. In clinically node positive (cN+) patients who converted to clinically node-negative (cN0) various prospective studies have demonstrated that the false negative rate (FNR) of the sentinel node biopsy (SNB) were higher than the acceptable 10% and strategies to reduce the FNR in cN + patients are being investigated. But all the effort to reduce the FNR does not have clinical prognostic significance. This has already been demonstrated in the literature in different randomized trials with long term follow up. The 10-year follow-up of our study confirmed our preliminary data that the use of standard SNB without the use of clip is acceptable in cN1/2 patients who become cN0 after NAT and will not translate into a worse outcome. In fact, the axillary recurrences were less than 2%. Similar positive data with different follow up were also confirmed by other studies that used SNB alone without TAD. Abstract: Orange protein (Orp) is a small bacterial metalloprotein of unknown function that harbors a unique molybdenum/copper (Mo/Cu) heterometallic cluster, [S2MoS2CuS2MoS2]3–. In this paper, the performance of Orp as a catalyst for the photocatalytic reduction of protons into H2has been investigated under visible light irradiation. We report the complete biochemical and spectroscopic characterization of holo-Orp containing the [S2MoS2CuS2MoS2]3–cluster, with docking and molecular dynamics simulations suggesting a positively charged Arg, Lys-containing pocket as the binding site. Holo-Orp exhibits excellent photocatalytic activity, in the presence of ascorbate as the sacrificial electron donor and [Ru(bpy)3]Cl2as the photosensitizer, for hydrogen evolution with a maximum turnover number of 890 after 4 h irradiation. Density functional theory (DFT) calculations were used to propose a consistent reaction mechanism in which the terminal sulfur atoms are playing a key role in promoting H2formation. A series of dinuclear [S2MS2M′S2MS2](4n)–clusters, with M = MoVI, WVIand M′(n+)= CuI, FeI, NiI, CoI,ZnII, CdIIwere assembled in Orp, leading to different M/M′-Orp versions which are shown to display catalytic activity, with the Mo/Fe-Orp catalyst giving a remarkable turnover number (TON) of 1150 after 2.5 h reaction and an initial turnover frequency (TOF°) of 800 h–1establishing a record among previously reported artificial hydrogenases.

Abstract: ABSTRACTEchocardiographic analysis of cardiac chamber volumes throughout the heart cycle is important to assess left- and right-heart function. We have developed and validated a method to simultaneously segment all four heart chambers in 3D echocardiography cine loops. We described the heart using several Doo Sabin models, one for each chamber and one for the pericardium. The model was fitted to 3D echo images using edge detection and a Kalman filter. The average signed error in estimating volume across all chambers was −1.0 ± 12.3 ml. These results show that the model slightly underestimates the volumes. Average signed error for the ejection fraction was −3.2 7.4 pts, with the highest error being the right atrium. This all-chamber automatic segmentation method provides high accuracy and may have utility in understanding chamber interactions.

Abstract: IMPORTANCE: Successful therapeutic cancer prevention requires definition of the minimal effective dose. Aromatase inhibitors decrease breast cancer incidence in high-risk women, but use in prevention and compliance in adjuvant settings are hampered by adverse events. OBJECTIVE: To compare the noninferiority percentage change of estradiol in postmenopausal women with estrogen receptor–positive breast cancer given exemestane, 25 mg, 3 times weekly or once weekly vs a standard daily dose with a noninferiority margin of −6%. DESIGN, SETTING, AND PARTICIPANTS: This multicenter, presurgical, double-blind phase 2b randomized clinical trial evaluated 2 alternative dosing schedules of exemestane. Postmenopausal women with estrogen receptor–positive breast cancer who were candidates for breast surgery were screened from February 1, 2017, to August 31, 2019. Blood samples were collected at baseline and final visit; tissue biomarker changes were assessed from diagnostic biopsy and surgical specimen. Biomarkers were measured in different laboratories between April 2020 and December 2021. INTERVENTIONS: Exemestane, 25 mg, once daily, 3 times weekly, or once weekly for 4 to 6 weeks before surgery. MAIN OUTCOMES AND MEASURES: Serum estradiol concentrations were measured by solid-phase extraction followed by liquid chromatography–tandem mass spectrometry detection. Toxic effects were evaluated using the National Cancer Institute terminology criteria, and Ki-67 was assessed by immunohistochemistry. RESULTS: A total of 180 women were randomized into 1 of the 3 arms; median (IQR) age was 66 (60-71) years, 63 (60-69) years, and 65 (61-70) years in the once-daily, 3-times-weekly, and once-weekly arms, respectively. In the intention-to-treat population (n = 171), the least square mean percentage change of serum estradiol was −89%, −85%, and −60% for exemestane once daily (n = 55), 3 times weekly (n = 56), and once weekly (n = 60), respectively. The difference in estradiol percentage change between the once-daily and 3-times-weekly arms was −3.6% (P for noninferiority = .37), whereas in compliant participants (n = 153), it was 2.0% (97.5% lower confidence limit, −5.6%; P for noninferiority = .02). Among secondary end points, Ki-67 and progesterone receptor were reduced in all arms, with median absolute percentage changes of −7.5%, −5.0%, and −4.0% for Ki-67 in the once-daily, 3-times-weekly, and once-weekly arms, respectively (once daily vs 3 times weekly, P = .31; once daily vs once weekly, P = .06), and −17.0%, −9.0%, and −7.0% for progesterone receptor, respectively. Sex hormone–binding globulin and high-density lipoprotein cholesterol had a better profile among participants in the 3-times-weekly arm compared with once-daily arm. Adverse events were similar in all arms. CONCLUSIONS AND RELEVANCE: In this randomized clinical trial, exemestane, 25 mg, given 3 times weekly in compliant patients was noninferior to the once-daily dosage in decreasing serum estradiol. This new schedule should be further studied in prevention studies and in women who do not tolerate the daily dose in the adjuvant setting. Abstract: ABSTRACTWith the current expansion of vector-based research and an increasing number of facilities rearing arthropod vectors and infecting them with pathogens, common measures for containment of arthropods as well as manipulation of pathogens are becoming essential for the design and running of such research facilities to ensure safe work and reproducibility, without compromising experimental feasibility. These guidelines and comments were written by experts of the Infravec2 consortium, a Horizon 2020-funded consortium integrating the most sophisticated European infrastructures for research on arthropod vectors of human and animal diseases. They reflect current good practice across European laboratories with experience of safely handling different mosquito species and the pathogens they transmit. As such, they provide experience-based advice to assess and manage the risks to work safely with mosquitoes and the pathogens they transmit. This document can also form the basis for research with other arthropods, for example, midges, ticks or sandflies, with some modification to reflect specific requirements.

Abstract: Shared strategies and correct information are essential to guide physicians in the management of such an uncommon disease as Breast Implant–Associated Anaplastic Large Cell Lymphoma (BIA-ALCL). A systematic review of the literature was performed to collect the most relevant evidence on BIA-ALCL reported cases. A panel of multidisciplinary experts discussed the scientific evidence on BIA-ALCL, and updated consensus recommendations were developed through the Delphi process. The lastest reported Italian incidence of BIA-ALCL is 3.5 per 100.000 implanted patients (95% CI, 1.36 to 5.78), and the disease counts over 1216 cases worldwide as of June 2022. The most common presentation symptom is a late onset seroma followed by a palpable breast mass. In the event of a suspicious case, ultrasound-guided fine-needle aspiration should be the first step in evaluation, followed by cytologic and immunohistochemical examination. In patients with confirmed diagnosis of BIA-ALCL confined to the capsule, the en-bloc capsulectomy should be performed, followed by immediate autologous reconstruction, while delayed reconstruction applies for disseminate disease or radically unresectable tumor. 45. Reflectance confocal microscopy as noninvasive tool for monitoring tirbanibulin efficacy in actinic keratosis.

Abstract: • We aimed to investigate the use of in vivo RCM in the assessment of the response of AKs treated with tirbanibulin, as it has never been described in the published literature. • A complete response (total disappearance of AK features) was described by clinical, dermoscopic and in vivo RCM examination in 10 out of 10 lesions, with a recovery of stratum corneum, decrease in atypical honeycomb pattern and changes in dermal collagen. • Based on our experience, we confirm the efficacy and safety of tirbanibulin in treating AKs and the usefulness of RCM in vivo examination for the therapeutic monitoring of such lesions, even in a very early stage. Actinic keratosis (AK) is a common precancerous condition found on sun-damaged skin. Tirbanibulin 1 % ointment has been approved for the topical treatment of non-hyperkeratotic facial and scalp Olsen grade I AKs over a contiguous area of 25 cm2 with a daily application for 5 consecutive days. Our aim was to investigate the use of in vivo RCM in the assessment of the response of AKs treated with tirbanibulin, as it has never been described in the published Literature. A total of 10 AKs in 10 consecutive outpatients were enrolled in the present study in May 2023. The follow-up visit was scheduled after 30 days from last application of tirbanibulin ointment. At follow-up visit, a complete response was described by clinical, dermoscopic and in vivo RCM examination in 10 out of 10 lesions, with a recovery of stratum corneum, decrease in atypical honeycomb pattern and changes in dermal collagen. All patients were followed up for at least 8 months and further recurrences were not registered. Based on our experience, we confirm the efficacy and safety of tirbanibulin in treating AKs and the usefulness of RCM in vivo examination for the therapeutic monitoring of such lesions, even in a very early stage. Second, cancer patients requiring mastectomy at the tumor site and simultaneous risk reducing mastectomy of the healthy breast. Third, cancer patients who have been treated for primary cancer requiring risk reducing mastectomies in a further stage. In this paper, we present a schematic guide for reconstruction for each subpopulation of subjects and their peculiarities.

Abstract: Herein, we demonstrate the formation of polycarbonate (PC) nanoplastics (NPs) through laser ablation of solid PC films in water. We prove that the size of the produced particles depends directly on the laser energy fluence, ranging between a few tens and a few hundreds of nanometers. Focusing on NPs of the smallest size range, the chemical characterization proves that they are composed of PC, while their surface is highly oxidized, similar to the PC fragments derived from natural photo-degradation processes. Together with the PC-NPs, additional photo-degradation by-products are formed, similar to the ones expected from environmentally exposed plastics. With the aim of providing reliable insights into the potentially detrimental effects of PC-based plastic litter on human health, upcyte® human hepatocytes were selected as a hepatic cellular model for assessing the hepatotoxicity of PC-NPs. Although no cytotoxic effects were observed at low concentrations (up to 40 μg mL−1), an alteration of the cytochrome P450 system (CYP3A4, CYP2C9, and CYP1A2) and a reduction of the albumin production were found. Interestingly, we prove that these cytotoxic effects can be attributed to both the nano-particulate matter (i.e., the NPs) and the molecular matter (i.e., the photo-degradation by-products). Although far from the recapitulation of the real fate of NPs in the environment, with this novel approach we demonstrate the possibility of obtaining in a single step an environmentally pertinent composition of PC photo-degradation products, applied on a physiologically meaningful hepatic cellular model for human exposure studies.

Abstract: Silver nanoparticles (AgNP) are heavily used in consumer products and medical devices, including face masks to fight against COVID-19. AgNP effects on mammals have been studied both using cellular and animal models. The former elucidate molecular mechanisms occurring at the subcellular level, while the latter bring insight into the toxicity and accumulation at the organ level. It has not yet been possible to reconcile these two kinds of studies in a translational approach. In the current study, we use 3D HepG2/C3A hepatocyte cultures that partially reproduce liver architecture, leading to the formation of active bile canaliculi and exposure scenarios mimicking parenteral or oral exposure to AgNP. By using a combination of cutting edge imaging techniques, atomic spectroscopy, and cell biology, we reveal the formation of Ag sulphide or Ag–organothiol complexes in the different conditions, the storage of excess Ag into vacuoles, the excretion of Ag(i) ions into bile canaliculi through the ATP7B Cu transporter, and modifications of the mitochondrial network. These findings, made possible by the use of a pseudo-organ as a model, reveal exposure-dependent mechanisms that can inspire future studies on nanomaterial metabolism in mammals and risk-assessment, and thereof regulations. Moreover, they can help bridge the gap between cellular and animal studies, bringing significant advances into the understanding of AgNP fate in mammals.

49. Germline pathogenic variants in metaplastic breast cancer patients and the emerging role of the BRCA1gene

Abstract: Metaplastic breast cancer (MpBC) is a rare, aggressive breast cancer (BC) histotype. Scarce information is available about MpBC genetic predisposition. Previous studies, mainly consisting of case reports, retrospective reviews and others on target therapies, pointed to a possible involvement of the BRCA1gene in increasing MpBC risk, without ever confirming it. In this study, we retrospectively reviewed all BC patients counseled at our Institute for genetic testing of at least BRCA1or BRCA2(BRCA) genes and we found that 23 (23/5226 = 0.4%) were affected by MpBC. About 65% (15/23) of MpBC patients harbored a germline pathogenic variant (PV): 13 in BRCA1(86.7%), including two patients who received genetic testing for known familial PV, one in TP53(6.7%), and one in MLH1(6.7%). We observed a statistically different frequency of MpBC in patients who carried a PV in the BRCAgenes (13/1114 = 1.2%) vs. all other BC patients (10/4112 = 0.2%) (p= 0.0002). BRCAcarriers proved to have an increased risk of developing MpBC compared to all other BC patients who were tested for BRCAgenes (OR = 4.47; 95% CI: 1.95–10.23). Notably, MpBCs were diagnosed in 2.1% (13/610) of BRCA1carriers. No MpBCs were observed in BRCA2carriers (0/498 = 0%), revealing a statistically significant difference between the prevalence of MpBCs in BRCA1and BRCA2carriers (p= 0.0015). Our results confirmed that BRCA1is involved in MpBC predisposition. Further studies on unselected patients are needed to elucidate the authentic role of BRCA1and to explore the possible implication of other genes in MpBC predisposition.

50. Clinical presentation and recurrence of pituitary neuroendocrine tumors: results from a single referral center in Colombia

Abstract: Introduction: Pituitary neuroendocrine tumors (PitNETs) represent 15–18.2% of all intracranial tumors. Their clinical presentation can range from chronic headache, visual defects, hypopituitarism to hormone excess syndromes. PitNETS are commonly classified as functioning neuroendocrine tumors (F-PitNETs) and non-functioning neuroendocrine tumors (NF-PitNETs). At the moment, new classification has emerged based on cell lineages. Almost 50% of all patients with PitNETs require surgical intervention, and about 25% of these have residual and persistent disease that may require additional management. Subjects and methods: A retrospective cohort of medical records of patients with PitNETs, aiming to describe the incidence of recurrence of patients who received surgical treatment over a 12 month follow up period at San Jose Hospital (SJH) in Bogotá, Colombia, over an observation period of 10 years. Furthermore, clinical presentation, biochemical characteristics and immunohistochemistry, postoperative complications are detailed. Results: Eight hundred and eighty-seven patients with pituitary tumors were included in the cohort; 83% (737/887) had a diagnosis of PitNET. Of these, 18.9% (140) received surgical management. The majority 58% (98/140) had nonfunctional-PitNETs (NF-PitNETs), followed by growth-hormone-secreting pituitary adenoma (22.1%; 33/140), adrenocorticotropic- hormone-secreting pituitary adenoma (9.3%; 13/140), and prolactinomas (9.3%; 13/140). A recurrence was found in 45.71% (64/140), subclassified as biochemical in 15.71% (22/140), controlled with medications in 20% (28/140), and remission occurred in 18.57% (26/140). Conclusion: Clinical presentation and incidence of recurrence in patients with PitNETs in a referral center in Colombia are similar to other surgical cohorts with low cure rates and high recurrence.
