3. Beyond Grid Girls and Checkered Flags: Exploring Sexual Objectification at Motorsport Events.

Motorsport has long been a very male-oriented sport, its events synonymous with conventionally attractive, provocatively dressed "grid girls." This research note examines the issue of sexual objectification in the male-dominated motorsport industry, shedding light on its prevalence and impact through findings from an online survey. While previous studies have focused on objectification in service industries, such as airlines, sports and events environments remain underexplored. The study highlights the mental health implications of employees' objectification, emphasizing the harmful effects of stereotyping individuals, reducing them to objects of desire, and perpetuating gender inequality. Our exploratory study advocates for promoting inclusivity, respect, and empowerment within the industry to combat sexism and objectification, calling for increased awareness and understanding in the broader events sector and specifically in motorsports. 4. Compact Fully Printed GPS/GNSS Antenna Using Embedded MTM-EBGs

- Abstract
A compact and low-profile antenna for global navigation satellite systems (GNSSs), with particular optimization for triband Global Positioning System (GPS) coverage, is presented. A dual-band antenna is developed with the use of metamaterial-based electromagnetic bandgap structures (MTM-EBGs) that covers GPS L1, L2, and L5 bands and a majority of the GNSS spectrum. The patch antenna is fabricated on two sides of a single substrate sheet above a ground plane, with a 3-D printed polylactic acid (PLA) substrate spacer for simple fabrication. The antenna is fed by a wideband and planar feed network below the ground plane, and simulation and measurement results show good performance in terms of matching, gain, pattern shape, and axial ratio, demonstrating that this antenna is an excellent candidate for use with modern high-accuracy multiband and multisystem GPS/GNSS receivers.

6. Development of CD33-Targeted Dual Drug-Loaded Nanoparticles for the Treatment of Pediatric Acute Myeloid Leukemia

- Abstract
Paediatric acute myeloid leukemia (AML) is a heterogeneous hematological malignancy still heavily reliant on traditional chemotherapeutic approaches. Combination treatments have shown to be a superior approach, but their success is often hindered by side effects and different drugs’ pharmacokinetics. Here, we investigated ABT-737 and Purvalanol A as a potential drug pairing for pediatric AML and described the development of CD33-targeted polymeric nanoparticles (NPs) to enable their simultaneous targeted codelivery. Separate drug encapsulation within poly(lactic-co-glycolic acid) (PLGA) NPs was optimized prior to coencapsulation of both drugs at a synergistic ratio in PEGylated PLGA NPs. The therapeutic effects of formulations were evaluated in a panel of pediatric AML cells, and dual drug-loaded NPs (dual NPs) demonstrated significantly enhanced apoptotic cell death. Moreover, conjugation to gemtuzumab resulted in improved NP binding and internalization in CD33-positive cells. Finally, CD33-targeted dual-loaded NPs showed enhanced cytotoxicity to CD33-positive AML cells via CD33-mediated targeted drug delivery.

- Abstract
Men of African descent have the highest prostate cancer incidence and mortality rates, yet the genetic basis of prostate cancer in African men has been understudied. We used genomic data from 3,963 cases and 3,509 controls from Ghana, Nigeria, Senegal, South Africa and Uganda to infer ancestry-specific genetic architectures and fine-map disease associations. Fifteen independent associations at 8q24.21, 6q22.1 and 11q13.3 reached genome-wide significance, including four new associations. Intriguingly, multiple lead associations are private alleles, a pattern arising from recent mutations and the out-of-Africa bottleneck. These African-specific alleles contribute to haplotypes with odds ratios above 2.4. We found that the genetic architecture of prostate cancer differs across Africa, with effect size differences contributing more to this heterogeneity than allele frequency differences. Population genetic analyses reveal that African prostate cancer associations are largely governed by neutral evolution. Collectively, our findings emphasize the utility of conducting genetic studies that use diverse populations.

- Abstract
α-l-(3′-2′)-Threofuranosyl nucleic acid (TNA) pairs with itself, cross-pairs with DNA and RNA, and shows promise as a tool in synthetic genetics, diagnostics, and oligonucleotide therapeutics. We studied in vitroprimer insertion and extension reactions catalyzed by human trans-lesion synthesis (TLS) DNA polymerase η (hPol η) opposite a TNA-modified template strand without and in combination with O4-alkyl thymine lesions. Across TNA-T (tT), hPol η inserted mostly dAMP and dGMP, dTMP and dCMP with lower efficiencies, followed by extension of the primer to a full-length product. hPol η inserted dAMP opposite O4-methyl and -ethyl analogs of tT, albeit with reduced efficiencies relative to tT. Crystal structures of ternary hPol η complexes with template tT and O4-methyl tT at the insertion and extension stages demonstrated that the shorter backbone and different connectivity of TNA compared to DNA (3′ → 2′ versus 5′ → 3′, respectively) result in local differences in sugar orientations, adjacent phosphate spacings, and directions of glycosidic bonds. The 3′-OH of the primer’s terminal thymine was positioned at 3.4 Å on average from the α-phosphate of the incoming dNTP, consistent with insertion opposite and extension past the TNA residue by hPol η. Conversely, the crystal structure of a ternary hPol η·DNA·tTTP complex revealed that the primer’s terminal 3′-OH was too distant from the tTTP α-phosphate, consistent with the inability of the polymerase to incorporate TNA. Overall, our study provides a better understanding of the tolerance of a TLS DNA polymerase vis-à-vis unnatural nucleotides in the template and as the incoming nucleoside triphosphate.

9. A mixed-methods investigation of weight bias reduction in medical students

- Abstract
ABSTRACTWe used a mixed-methods design to test a novel weight bias intervention and understand current attitudes toward people with higher weight and weight-inclusive care among medical students (N = 80). The intervention was embedded in curriculum and uniquely focused on delegitimising the use of weight as a heuristic for health. Almost half of participants (43.8%) responded to both pre- and post-intervention surveys. These participants found the intervention acceptable; and reported significant pre-post reductions in weight bias and endorsement of using weight as a heuristic for health. Less engaged participants reported significantly greater endorsement of using weight as a heuristic for health at pre-intervention. One central theme emerged from pre-intervention qualitative responses, related to how medical students scoped both the problemand the solution. This consisted of three sub-themes: (1) problematizing the system and clinician (and their weight bias), (2) problematizing the patient (who was responsible for their own health), and (3) problematizing the medical condition (weight and health behaviors as problems for the clinician to solve). Participants often showed evidence of several and sometimes contradictory ways of problem-scoping. Problematizing the patient, including weight-stigmatizing assumptions, was particularly pronounced among less engaged participants (i.e. those who did not complete the post-survey). Findings suggest that the intervention may be acceptable and effective in reducing weight bias in medical students who already have lower levels of bias than their peers. Qualitative findings suggest that, even when medical students recognize the harmful impacts of weight stigma and the benefits of weight-inclusive care, approaches to care may remain weight-centric. Together, the findings suggest a key challenge for weight bias reduction among medical students lies in addressing problem-scoping that may encourage weight bias and weight-centric care, and reaching the population most in need of intervention – students with high levels of weight bias.

- Abstract
Research on Traveller education in Ireland is limited. Internationally, studies on the education of the Traveller, Roma or Gypsy community tend to focus on educational participation and exclusion, discrimination, identity and belonging with little focus on the representation of Traveller student across different types of schools and educational settings. This chapter examines the distribution of Traveller students across mainstream and alternative education settings in Ireland, and how this distribution varies across different types of schools and centres. Using data from the Growing Up in Ireland principal survey and survey data from Irish alternative education providers (The National Youthreach Programme), it explores the concentration of Travellers in mainstream provision, in designated disadvantaged schools (Delivering Equality of Opportunity in Schools [DEIS]) compared to non-DEIS schools, and alternative education in Ireland. Findings show a higher representation of Traveller students in schools designated disadvantaged compared to non-disadvantaged settings and greater numbers of Traveller students in large primary and, to a lesser extent, secondary schools. The findings for alternative education show that Traveller young people make up 15% of learners which is higher than their representation in the general population or the school system. There is marked regional variation with higher representation of Travellers in alternative education settings in the West of Ireland compared to other regions. With increasing policy emphasis on inclusive education and the need to increase retention of students from Traveller backgrounds, it is perhaps an opportune time to explore the distribution of Traveller students across different school and alternative educational contexts so that educational supports can be targeted more effectively. 15. A 'giant' pterodactyloid pterosaur from the British Jurassic.

The fossil remains of a pterodactyloid pterosaur from the Kimmeridge Clay Formation (Jurassic: Tithonian) of Abingdon, Oxfordshire, central England are identified as a partial left first wing finger phalanx. The elongation of the phalanx and distinctive morphology of the proximal articular region, in particular the square outline of the extensor tendon process, permit the specimen to be assigned to Ctenochasmatoidea. Although fragmentary, it is sufficiently well preserved to determine accurately its dimensions when complete. Morphometric analysis reveals the specimen to represent one of the largest known examples of a Jurassic pterosaur, with an estimated wingspan of at least 3 m, and is one of the first pterodactyloids to be reported from the Jurassic of the United Kingdom. Acute coronary syndrome and sudden cardiac death are often caused by rupture and thrombosis of lipid-rich atherosclerotic coronary plaques (known as vulnerable plaques), many of which are non-flow-limiting. The safety and effectiveness of focal preventive therapy with percutaneous coronary intervention of vulnerable plaques in reducing adverse cardiac events are unknown. We aimed to assess whether preventive percutaneous coronary intervention of non-flow-limiting vulnerable plaques improves clinical outcomes compared with optimal medical therapy alone.

17. Thermal Scaling Analysis of Large Hybrid Laser Arrays for Co-Packaged Optics

- Abstract
Optical transceivers for data center applications require multi-wavelength light sources, which can either be integrated or external from the transceiver die. Scaling up the number of communication channels implies the need for large laser arrays. Since the energy efficiency of semiconductor lasers is very sensitive to temperature, it is imperative to employ a thermal-aware design and minimize self-heating and thermal crosstalk. In this paper, a thermal scaling analysis is performed on hybrid, flip-chip integrated InP-on-Si lasers. A finite element thermal model of a single gain section laser is validated with experimental measurement of the laser thermal resistance and extrapolated to accomodate multi-section operation. The impact of adding a top-side heat sink as well as increasing laser length and width are investigated. The detailed 3D simulation results are used to build a compact, coupled thermo-optic model of a large array of multiple lasers, considering thermal crosstalk. Finally, this model is applied to a test case with 8 WDM channels and 8 ports. Depending on the configuration (integrated vs. external) and ambient temperature, different optimal designs arise based on both energy efficiency and module footprint. The presented modelling framework is generic; it can be applied to different types of lasers and systems.

18. Autologous Lipo-Dermal Advancement Flap for Inframammary Fold Reconstruction: A Technical Note for Prevention and Management of Caudal Breast Implant Migration

Inferior breast implant displacement, also known as caudal migration, or bottoming-out, is a known complication of breast augmentation. This leads to a loss of upper pole fullness, a more superiorly located nipple-areolar complex, and an unnatural appearance. Avoiding these complaints has been a longstanding challenge in the field of aesthetics and this technical note suggests a novel solution. This technical note describes the process of preoperative assessment, careful markings, and the use of an autologous lipo-dermal advancement flap that has been designed, mobilized, and rotated to be internally fixated to costal margins and the relevant deep fascial structures at the proposed inframammary fold level. This technique creates a robust partitioning hammock-like structure that stabilizes the breast implant and prevents future caudal displacement. The described technique may be implemented as an independent solution, or as an adjunct to other modalities like biological meshes in revision breast surgery.

19. Raman spectroscopic measurements on San Carlos olivine up to 14 GPa and 800 K: Implications for thermodynamic properties

Olivine, the most abundant mineral in the upper mantle, plays a key role in controlling the thermodynamic properties in the Earth’s and planetary interiors, like the temperature distribution along the adiabatic geotherm. In this study, we conducted simultaneous high-pressure (P) and high-temperature (T) Raman measurements on a San Carlos olivine sample in an externally heated diamond-anvil cell (DAC). The intrinsic anharmonic parameters, ai, are calculated as functions of both pressure and temperature, and the isochoric (CV) and isobaric (CP) heat capacities are computed at various P-Tconditions with the anharmonic correction. The harmonic heat capacities are CV= 807.7 J/kg/K and CP= 815.4 J/kg/K at ambient conditions, with anharmonic contribution of ΔC= 7.9 J/kg/K. Relative to the previous vibrational measurements conducted at high-Por high-Tconditions, this simultaneous high-P-Texperiment indicates that the anharmonic contribution to heat capacities is overestimated if the anharmonic parameters (ai) are treated as constants, as done previously. The pressure effect is marginal on the intrinsic anharmonic contribution to thermodynamic properties, whereas it has a much more significant effect on the external anharmonicity (thermal expansivity). The pressure dependence of CP(dCP/dP, in J/kg/K/GPa) increases from –3.14 at 300 K to –1.94 at 700 K, and then decreases smoothly to –5.03 at 1800 K. Combining the derived high-P-Tcapacity with a reliable P-V-Tequation of state (EoS) for olivine, we further modeled the thermodynamic Gruneisen parameter, γ(P-T). The Gruneisen parameter is important for the connection between isothermal and adiabatic compressions of minerals, which can be decreased by approximately 5% with the anharmonic correction at high temperatures. The modeled adiabatic bulk modulus and bulk sound velocity can be expressed as: KS(T,P) (GPa) = 127.5(1) + 4.32(5)·P– 0.018(1)·(T– 300) and VΦ(T-P) (km/s) = 6.22(2) + 0.069(3)·P– [3.74(15)–0.075(13)·P]·10–4·(T– 300). The adiabatic temperature gradient, dTS/dP, which is almost independent of pressure, equals 13.40(16) and 12.35(16) K/GPa in the harmonic and anharmonic models, respectively. This study provides a useful example for modeling the radial temperature distribution in adiabatic planetary mantles.

20. Conformational rigidity of cytochrome c'-α from a thermophile is associated with slow NO binding

Cytochromes c′-α are nitric oxide (NO)-binding heme proteins derived from bacteria that can thrive in a wide range of temperature environments. Studies of mesophilic Alcaligenes xylosoxidanscytochrome c′-α (AxCP-α) have revealed an unusual NO-binding mechanism involving both heme faces, in which NO first binds to form a distal hexa-coordinate Fe(II)-NO (6cNO) intermediate and then displaces the proximal His to form a proximal penta-coordinate Fe(II)-NO (5cNO) final product. Here, we characterize a thermally stable cytochrome c′-α from thermophilic Hydrogenophilus thermoluteolus(PhCP-α) to understand how protein thermal stability affects NO binding. Electron paramagnetic and resonance Raman spectroscopies reveal the formation of a PhCP-α 5cNO product, with time-resolved (stopped-flow) UV-vis absorbance indicating the involvement of a 6cNO intermediate. Relative to AxCP-α, the rates of 6cNO and 5cNO formation in PhCP-α are ∼11- and ∼13-fold lower, respectively. Notably, x-ray crystal structures of PhCP-α in the presence and absence of NO suggest that the sluggish formation of the proximal 5cNO product results from conformational rigidity: the Arg-132 residue (adjacent to the proximal His ligand) is held in place by a salt bridge between Arg-75 and Glu-135 (an interaction not present in AxCP-α or a psychrophilic counterpart). Overall, our data provide fresh insights into structural factors controlling NO binding in heme proteins, including 5cNO complexes relevant to eukaryotic NO sensors.

21. Continuous Flow Approach for Benzylic Photo-oxidations Using Compressed Air

- Abstract
A continuous flow approach for the aerobic photo-oxidation of benzylic substrates to ketone and aldehyde products is presented. The resulting process exploits UV-A LEDs (375 nm) in combination with a Corning AFR reactor that ensures effective gas–liquid mixing and leads to short residence times of 1 min. A variety of benzylic substrates are converted to their corresponding carbonyl products, and scalability is demonstrated to produce multigram quantities of products within a few hours. Overall, this continuous flow approach offers several improvements over alternative oxidation methods due to the combined use of air as an oxidant and SAS (sodium anthraquinone-2 sulfonate) as a water-soluble photocatalyst. The use of greener and safer conditions together with process intensification principles renders this flow approach attractive for further industrial applications.

In recent years, there has been much focus on the use of wastewater‐based epidemiology (WBE) in urban centers, particularly for SARS‐CoV‐2 monitoring. However, less is known about the application of WBE in rural settings or in areas of limited resources. Most WBE programs in low‐resource communities have occurred outside the United States. To reap the benefits, WBE would need to be tailored to better reflect the socioeconomic challenges, technical barriers, communication limitations, and variable wastewater infrastructures associated with rural communities. The objective of this review is to evaluate the potential opportunities and challenges of deploying the current SARS‐CoV‐2 monitoring methodologies in small, rural communities, with a particular focus on rural Texas. For this, we conducted an inventory of rural communities in the state of Texas and their wastewater infrastructure. Based on specific rural examples, we evaluated the potential of current WBE methodologies used in urban settings to monitor for emerging biological agents of concern such as SARS‐CoV‐2. Our findings include an overview of rural wastewater capacity across rural Texas, a look at current WBE efforts to detect SARS‐CoV‐2, and recommendations for future implementation in two cities in rural counties, Kerrville and Valentine. WBE is a rapidly evolving public health tool with several notable advantages associated with cost, access, and adaptability. It is of particular use in resource‐limited communities that often exhibit healthcare disparities. This study presents the first overview of the feasibility of implementing WBE in the rural settings of Texas. We provide several recommendations and suggest alternatives that may be of use when planning an expansion of WBE into these areas.

26. Design and Validation of a 3D Printed Cranio-Facial Simulator: A Novel Tool for Surgical Education

Objective To assess the ability of current 3D printing technology to generate a craniofacial bony and soft tissue anatomical model for use in simulating the performance of a fronto-orbital advancement (FOA) osteotomy and then to further assess the value of the model as an educational tool.Design Anatomic models were designed with a process of serial anatomic segmentation/design, 3D printing, dissection, and device refinement. A validation study was conducted with 5 junior and 5 senior plastic surgery residents. The validation study incorporated a multiple-choice Knowledge Assessment test (KA), an Objective Structured Assessment of Technical skills (OSATs), a Global Rating Scale (GRS) and a Michigan Standard Simulation Experience Scale (MiSSES). We compared the scores of both the junior and senior residents and compared junior resident scores, before and after viewing a lecture/demonstration.Results MiSSES showed high face validity with a score of 85.1/90, signifying high satisfaction with the simulator learning experience. Simulation and the lecture/demonstration improved the junior resident average KA score from 5.6/10 to 9.6/10 (P= .02), OSATs score from 32.4/66 to 64.4/66 (P< .001) and GRS score from 13.9/35 to 27.5/35 (P< .001). The senior residents OSATs score of 56.3/66 was higher than the pre-lecture juniors (32.4/66) (P< .001), but lower than the post-lecture juniors (64.4/66) (P< .001).Conclusion We have successfully fabricated a 3D printed craniofacial simulator capable of being used as an educational tool alongside traditional surgical training. Next steps would be improving soft tissue realism, inclusion of patient and disease specific anatomy and creation of models for other surgical specialties.

However, in exploring recent case law, this article exposes that the rules applied to decide proprietary estoppel cases remain as uncertain as ever. Some commonality can be identified, but only in the persistent favouring of promisors over promisees. This article criticises the practical and theoretical underpinnings of proprietary estoppel and proposes further reform. In particular, the article draws new links from the doctrine of secret trusts to suggest a more just and certain basis for proprietary estoppel’s future application

Objective: Test whether frequent protective behavioral strategies (PBS) users report (a) fewer alcohol-related consequences and (b) less risky alcohol intoxication dynamics (measured via transdermal alcohol concentration [TAC] sensor "features") in daily life. Method: Two hundred twenty-two frequently heavy-drinking young adults (M
age = 22.3 years) wore TAC sensors for 6 consecutive days. TAC features peak (maximum TAC), rise rate (speed of TAC increase), and area under the curve (AUC) were derived for each day. Negative alcohol-related consequences were measured in the morning after each self-reported drinking day. Past-year PBS use was measured at baseline. Results: Young adults reporting more frequent baseline PBS use showed (a) fewer alcohol-related consequences and (b) lower intoxication dynamics on average (less AUC, lower peaks, and slower rise rates). Limiting/stopping and manner of drinking PBS showed the same pattern of findings as the total score. Serious harm reduction PBS predicted fewer negative alcohol-related consequences, but not TAC features. Multilevel path models showed that TAC features peak and rise rate partially explained associations between PBS (total, limiting/stopping, and manner of drinking) and consequences. Independent contributions of PBS subscales were small and nonsignificant, suggesting that total PBS use was a more important predictor of risk/protection than the specific types of PBS used. Conclusions: Young adults using more total PBS may experience fewer alcohol-related consequences during real-world drinking episodes in part through less risky intoxication dynamics (TAC features). Future research measuring PBS at the daily level is needed to formally test TAC features as day-level mechanisms of protection from acute alcohol-related consequences. 33. Gender and socioecological level differences in stakeholders' perceptions of athlete attrition factors in an Australian high-performance pathway system.

This study aimed to build on previous work by the authors. It examines how socioecological level and gender influence high-performance sport system (HPSS) stakeholders' perspectives of the relative importance and feasibility to address athlete attrition factors within an Australian high-performance pathway system (HPPS). Mixed methods. Sub-analysis was conducted of rating data from 30 participants who had contributed to identifying 83 statements in 13 clusters in a previous Concept Mapping study. The 13 clusters were statistically analysed in 'R' using cumulative link mixed models (CLMMs) to determine differences in perceived importance and feasibility between 1) socioecological levels, and 2) genders. Mean ratings for 11 and three of the 13 clusters were statistically significantly different between at least two of the five socioecological levels, for importance and feasibility, respectively. Athletes had the largest variation in mean ratings from the most (athlete health 4.59), to least (performance potential 2.83) important cluster, when compared to the other four socioecological levels. There were statistically significant differences between the ratings between genders (Men/Women) for two clusters for each rating scale: Importance: 'athlete health' (M3.33:W3.84 [p 0.012]); 'performance potential' (M3.35:W2.57; [p 0.001]), Feasibility: 'abuse and mismanagement of health' (M2.97:W3.68; [p 0.000]) and 'athlete health' (M2.54:W3.33; [p 0.000]). This study highlights the need to implement more robust athlete attrition monitoring protocols. It also highlights the importance of listening to youth athletes' voices, and enabling equal gender representation to ensure holistically tailored environments are created to retain talented athletes in high-performance pathway programmes. 34. Burn Pit Smoke Condensate-Mediated Toxicity in Human Airway Epithelial Cells

Burn pits are a method of open-air waste management that was common during military operations in Iraq, Afghanistan, and other regions in Southwest Asia. Veterans returning from deployment have reported respiratory symptoms, potentially from exposure to burn pit smoke, yet comprehensive assessment of such exposure on pulmonary health is lacking. We have previously shown that exposure to condensates from burn pit smoke emissions causes inflammation and cytotoxicity in mice. In this study, we explored the effects of burn pit smoke condensates on human airway epithelial cells (HAECs) to understand their impact on cellular targets in the human lung. HAECs were cultured at the air–liquid interface (ALI) and exposed to burn pit waste smoke condensates (plywood, cardboard, plastic, mixed, and mixed with diesel) generated under smoldering and flaming conditions. Cytotoxicity was evaluated by measuring transepithelial electrical resistance (TEER) and lactate dehydrogenase (LDH) release; toxicity scores (TSs) were quantified for each exposure. Pro-inflammatory cytokine release and modulation of gene expression were examined for cardboard and plastic condensate exposures. Burn pit smoke condensates generated under flaming conditions affected cell viability, with flaming mixed waste and plywood exhibiting the highest toxicity scores. Cardboard and plastic smoke condensates modulated cytokine secretion, with GM-CSF and IL-1β altered in more than one exposure group. Gene expression of detoxifying enzymes (ALDH1A3, ALDH3A1, CYP1A1, CYP1B1, NQO1, etc.), mucins (MUC5AC, MUC5B), and cytokines was affected by several smoke condensates. Particularly, expression of IL6was elevated following exposure to all burn pit smoke condensates, and polycyclic aromatic hydrocarbon acenaphthene was positively associated with the IL-6 level in the basolateral media of HAECs. These observations demonstrate that exposure to smoke condensates of materials present in burn pits adversely affects HAECs and that aberrant cytokine secretion and altered gene expression profiles following burn pit material smoke exposure could contribute to the development of airway disease.

- Abstract
BACKGROUND: Duchenne muscular dystrophy (DMD) is a rare, degenerative, recessive X-linked neuromuscular disease. Mutations in the gene encoding dystrophin lead to the absence of functional dystrophin protein. Individuals living with DMD exhibit progressive muscle weakness resulting in loss of ambulation and limb function, respiratory insufficiency, and cardiomyopathy, with multiorgan involvement. Adeno-associated virus vector-mediated gene therapy designed to enable production of functional dystrophin protein is a new therapeutic strategy. Delandistrogene moxeparvovec (Sarepta Therapeutics, Cambridge, MA) is indicated for treatment of ambulatory pediatric patients aged 4 through 5 years with DMD who have an indicated mutation in the DMDgene.OBJECTIVE: Evidence-based considerations for management of potential adverse events following gene therapy treatment for DMD are lacking in clinical literature. Our goal was to provide interdisciplinary consensus considerations for selected treatment-related adverse events (TRAEs) (vomiting, acute liver injury, myocarditis, and immune-mediated myositis) that may arise following gene therapy dosing with delandistrogene moxeparvovec.METHODS: An interdisciplinary panel of 12 specialists utilized a modified Delphi process to develop consensus considerations for the evaluation and management of TRAEs reported in delandistrogene moxeparvovec clinical studies. Panelists completed 2 Questionnaires prior to gathering for an in-person discussion. Consensus was defined as a majority (=58% ; 7/12) of panelists either agreeing or disagreeing.RESULTS: Panelists agreed that the choice of baseline assessments should be informed by individual clinical indications, the treating provider’s judgment, and prescribing information. Corticosteroid dosing for treatment of TRAEs should be optimized by considering individual risk versus benefit for each indication. In all cases involving patients with a confirmed TRAE, consultations with appropriate specialists were suggested.CONCLUSIONS: The Delphi Panel established consensus considerations for the evaluation and management of potential TRAEs for patients receiving delandistrogene moxeparvovec, including vomiting, acute liver injury, myocarditis, and immune-mediated myositis.

- Abstract
Herein we disclose a telescoped flow strategy to access electronically differentiated bisaryl ketones as potentially new and tunable photosensitizers containing both electron-rich benzene systems and electron-deficient pyridyl moieties. Our approach merges a light-driven (365 nm) and catalyst-free reductive arylation between aromatic aldehydes and cyanopyridines with a subsequent oxidation process. The addition of electron-donating and withdrawing substituents on the scaffold allowed effective modification of the absorbance of these compounds in the UV–vis region, while the continuous flow process affords high yields, short residence time, and high throughput.

The JePPIX InPulse pilot line projects, rooted in the objectives of accelerating InP PIC manufacturing and innovation, present Pilot Photonics with an excellent opportunity to enable a cost‐effective transition from multiproject wafers (MPWs) to dedicated wafers with its two partners Smart Photonics and Fraunhofer HHI, two leading European PIC foundries. The chip quantities produced bolster design optimization and preproduction efforts for the company's flagship tunable and comb laser products.

- Abstract
The International Society of Nephrology (ISN) region of Oceania and South East Asia (OSEA) is a mix of high- and low-income countries, with diversity in population demographics and densities. Three iterations of the ISN-Global Kidney Health Atlas (GKHA) have been conducted, aiming to deliver in-depth assessments of global kidney care across the spectrum from early detection of CKD to treatment of kidney failure. This paper reports the findings of the latest ISN-GKHA in relation to kidney-care capacity in the OSEA region. Among the 30 countries and territories in OSEA, 19 (63%) participated in the ISN-GKHA, representing over 97% of the region’s population. The overall prevalence of treated kidney failure in the OSEA region was 1203 per million population (pmp), 45% higher than the global median of 823 pmp. In contrast, kidney replacement therapy (KRT) in the OSEA region was less available than the global median (chronic hemodialysis, 89% OSEA region vs. 98% globally; peritoneal dialysis, 72% vs. 79%; kidney transplantation, 61% vs. 70%). Only 56% of countries could provide access to dialysis to at least half of people with incident kidney failure, lower than the global median of 74% of countries with available dialysis services. Inequalities in access to KRT were present across the OSEA region, with widespread availability and low out-of-pocket costs in high-income countries and limited availability, often coupled with large out-of-pocket costs, in middle- and low-income countries. Workforce limitations were observed across the OSEA region, especially in lower-middle–income countries. Extensive collaborative work within the OSEA region and globally will help close the noted gaps in kidney-care provision.

40. Assessing Contributions of Synthetic Musk Compounds from Wastewater Treatment Plants to Atmospheric and Aquatic Environments

- Abstract
The high environmental concentrations, persistence, and toxicity of synthetic musk compounds (SMCs) necessitate a better grasp of their fate in wastewater treatment plants (WWTPs). To investigate the importance of WWTPs as pathways of SMCs to the environment, air and wastewater samples were collected at four WWTPs in Ontario, Canada. Polycyclic musks (PCMs) were present at higher concentrations than nitro musks (NMs) and macrocyclic musks (MCMs). Three PCMs [galaxolide (HHCB), tonalide (AHTN), and iso-E super (OTNE)] were the most abundant compounds (0.30–680 ng/m3in air, 0.40–15 μg/L in influent, and 0.007–6.0 μg/L in effluent). Analyses of multiyear data suggest that risk management measures put in place have been effective in reducing the release of many SMCs into the environment. The highest removal efficiency, up to almost 100% of some SMCs, was observed for the plant with the longest solid retention time. A fugacity-based model was established to simulate the transport and fate of SMCs in the WWTP, and good agreement was obtained between the measured and modeled values. These findings indicate that the levels of certain SMCs discharged into the atmospheric and aquatic environments were substantial, potentially resulting in exposure to both humans and wildlife.

- Abstract
The arachnoid barrier delineates the border between the central nervous system and dura mater. Although the arachnoid barrier creates a partition, communication between the central nervous system and the dura mater is crucial for waste clearance and immune surveillance1,2. How the arachnoid barrier balances separation and communication is poorly understood. Here, using transcriptomic data, we developed transgenic mice to examine specific anatomical structures that function as routes across the arachnoid barrier. Bridging veins create discontinuities where they cross the arachnoid barrier, forming structures that we termed arachnoid cuff exit (ACE) points. The openings that ACE points create allow the exchange of fluids and molecules between the subarachnoid space and the dura, enabling the drainage of cerebrospinal fluid and limited entry of molecules from the dura to the subarachnoid space. In healthy human volunteers, magnetic resonance imaging tracers transit along bridging veins in a similar manner to access the subarachnoid space. Notably, in neuroinflammatory conditions such as experimental autoimmune encephalomyelitis, ACE points also enable cellular trafficking, representing a route for immune cells to directly enter the subarachnoid space from the dura mater. Collectively, our results indicate that ACE points are a critical part of the anatomy of neuroimmune communication in both mice and humans that link the central nervous system with the dura and its immunological diversity and waste clearance systems.

- Abstract
Paediatric multisystem inflammatory syndrome temporally associated with SARS-CoV-2 (PIMS-TS), also known as multisystem inflammatory syndrome in children (MIS-C) emerged in April, 2020. The paediatric comparisons within the RECOVERY trial aimed to assess the effect of intravenous immunoglobulin or corticosteroids compared with usual care on duration of hospital stay for children with PIMS-TS and to compare tocilizumab (anti-IL-6 receptor monoclonal antibody) or anakinra (anti-IL-1 receptor antagonist) with usual care for those with inflammation refractory to initial treatment.

- Abstract
Frailty and malnutrition are both associated with worsening morbidity and mortality and become more prevalent in the elderly and as kidney function declines. Anorexia and reduced oral intake are common features of both frailty and malnutrition. However, there are sparse data evaluating the impact of other gastrointestinal (GI) symptoms, such as taste changes, on rates of frailty and malnutrition in people with kidney failure. The aim of this study is to describe the prevalence of frailty and malnutrition and their association with dietary intake and nutrition-related symptoms in people with kidney failure.

- Abstract
Recent series of newborn Oesophageal Atresia (OA) repair continue to report widespread use of chest drains, gastrostomy, routine contrast studies and parenteral nutrition (PN) despite evidence suggesting these are superfluous. We report outcomes using a minimally interventional approach to post-operative recovery. Ethically approved (15/WA/0153), single-centre, retrospective case-note review of consecutive infants with OA 2000–2022. Infants with OA and distal trache-oesophageal fistula undergoing primary oesophageal anastomosis at initial surgery were included (including those with comorbidities such as duodenal atresia, anorectal malformation and cardiac lesions). Our practice includes routine use of a trans-anastomotic tube (TAT), no routine chest drain nor gastrostomy, early enteral and oral feeding, no routine PN and no routine contrast study. Data are median (IQR). Of total 186 cases of OA treated during the time period, 157 met the inclusion criteria of which 2 were excluded as casenotes unavailable. TAT was used in 150 infants. A chest drain was required in 13 (8%) and two infants had a neonatal gastrostomy. Enteral feeds were started on postoperative day 2 (2–3), full enteral feeds established by day 4 (4–6) and oral feeds started on day 5 (4–8). PN was required in 15%. Median postoperative length of stay was 10 days (8–17). Progress was quicker in term infants than preterm. One infant died of cardiac disease prior to neonatal discharge. Two planned post-operative contrast studies were performed (surgeon preference) and a further 7 due to clinical suspicion of anastomotic leak. Contrast study was therefore avoided in 94%. There were 2 anastomotic leaks; both presented clinically at day 4 and day 8 after oral feeds had been started. Our minimally interventional approach is safe. It facilitates prompt recovery with lower resource use, reduced demand on nursing staff, reduced radiation burden, and early discharge home compared to published series without adversely affecting outcomes. Artificial light at night (ALAN) disrupts biological processes across taxa and at all levels of organisation. Despite growing interest in this globally pervasive sensory pollutant, its impact on colour-guided processes remains largely unexplored. This is especially concerning given the rapid transition in recent years away from narrow-spectrum lighting and towards broad-spectrum options such as white LEDs, which are rich in the short wavelengths of light to which many taxa are particularly sensitive. Camouflage is particularly likely to be disrupted by broader spectra of ALAN due to changes in conspicuousness in background matching prey, which may alter prey recognition in visually guided predators. We simulated natural intensities of moonlight with and without ALAN, using both broad-spectrum ('white') ALAN and ALAN filtered to remove the characteristic short (blue) wavelength peak of broad-spectrum LEDs to test whether filtering might mitigate their effects. We tested how exposure to these light treatments impacted predator-prey interactions, using the intertidal crab Carcinus maenas and contrasting colour morphs of the colour-polymorphic snail Littorina obtusata as a model system. Exposure to broad-spectrum ALAN reduced overall predation and reversed the pattern of colour-based prey selection observed under control conditions. Snails were 55 % less likely to be attacked under broad-spectrum ALAN than in control conditions, with likelihood decreasing over 70 % for yellow snails. Yellow snails were over 26 % more likely to be attacked than brown ones under control conditions, but brown snails were over 40 % more likely to be attacked than yellow ones under broad-spectrum ALAN. Exposure to filtered ALAN removed any significant colour-based difference in prey recognition. Our results demonstrate that spectral composition is a crucial aspect of ALAN as a sensory pollutant, capable of instigating profound changes in predator-prey interactions that could drive changes in population demography and increase morphological homogeneity in species that depend on colour polymorphism for camouflage. Background: The precise mechanisms driving poor exercise tolerance in patients with fibrotic interstitial lung diseases (fibrotic ILDs) showing a severe impairment in single-breath lung diffusing capacity for carbon monoxide (D
LCO < 40% predicted) are not fully understood. Rather than only reflecting impaired O2 transfer, a severely impaired DLCO may signal deranged integrative physiologic adjustments to exercise that jointly increase the burden of exertional symptoms in fibrotic ILD. Methods: Sixty-seven subjects (46 with idiopathic pulmonary fibrosis, 24 showing DLCO < 40%) and 22 controls underwent pulmonary function tests and an incremental cardiopulmonary exercise test with serial measurements of operating lung volumes and 0-10 Borg dyspnea and leg discomfort scores. Results: Subjects from the DLCO < 40% group showed lower spirometric values, more severe restriction, and lower alveolar volume and transfer coefficient compared to controls and participants with less impaired DLCO (P < .05). Peak work rate was -45% (vs controls) and -20% (vs DLCO > 40%) lower in the former group, being associated with lower (and flatter) O2 pulse, an earlier lactate (anaerobic) threshold, heightened submaximal ventilation, and lower SpO2 . Moreover, critically high inspiratory constrains were reached at lower exercise intensities in the DLCO < 40% group (P < .05). In association with the greatest leg discomfort scores, they reported the highest dyspnea scores at a given work rate. Between-group differences lessened or disappeared when dyspnea intensity was related to indexes of increased demand-capacity imbalance, that is, decreasing submaximal, dynamic ventilatory reserve, and inspiratory reserve volume/total lung capacity (P > .05). Conclusions: A severely reduced DLCO in fibrotic ILD signals multiple interconnected derangements (cardiovascular impairment, an early shift to anaerobic metabolism, excess ventilation, inspiratory constraints, and hypoxemia) that ultimately lead to limiting respiratory (dyspnea) and peripheral (leg discomfort) symptoms. DLCO < 40%, therefore, might help in clinical decision-making to indicate the patient with fibrotic ILD who might derive particular benefit from pharmacologic and non-pharmacologic interventions aimed at lessening these systemic abnormalities. 49. Building a COVID Field Hospital in Less Than 30 Days: The CNO Story.

Following a phone call from our respective chief nurse executives, two experienced nursing leaders from two large health systems put their 'day' jobs on hold to pivot and help build a COVID field hospital in the Baltimore Convention Center, transforming it into the Baltimore Convention Center Field Hospital (BCCFH). Both leaders, although qualified, had not served as chief nursing officers (CNOs), and the assignment for the BCCFH was to colead nursing as a CNO. Priorities included hiring over 3,000 per diem staff, writing policies and procedures needed to care for up to 250 patients, and designing workflows from patient admission, medication administration, shower schedules, and overall patient care, all of which required infection prevention protocols. Along with other key executives, the CNOs designed, created, and opened the BCCFH in less than 30 days. [ABSTRACT FROM AUTHOR]
