We report the recruitment activities and outcomes of a multi-disease neuromuscular patient registry in Canada. The Canadian Neuromuscular Disease Registry (CNDR) registers individuals across Canada with a confirmed diagnosis of a neuromuscular disease. Diagnosis and contact information are collected across all diseases and detailed prospective data is collected for 5 specific diseases: Amyotrophic Lateral Sclerosis (ALS), Duchenne Muscular Dystrophy (DMD), Myotonic Dystrophy (DM), Limb Girdle Muscular Dystrophy (LGMD), and Spinal Muscular Atrophy (SMA). Since 2010, the CNDR has registered 4306 patients (1154 pediatric and 3148 adult) with 91 different neuromuscular diagnoses and has facilitated 125 projects (73 academic, 3 not-for-profit, 3 government, and 46 commercial) using registry data. In conclusion, the CNDR is an effective and productive pan-neuromuscular registry that has successfully facilitated a substantial number of studies over the past 10 years.

Background: ALS is a progressive neurodegenerative disease without a cure and limited treatment options. Edaravone, a free radical scavenger, was shown to slow disease progression in a select group of patients with ALS over 6 months; however, the effect on survival was not investigated in randomized trials. The objective of this study is to describe real-world survival effectiveness over a longer timeframe. Methods: This retrospective cohort study included patients with ALS across Canada with symptom onset up to three years. Those with a minimum 6-month edaravone exposure between 2017 and 2022 were enrolled in the interventional arm, and those without formed the control arm. The primary outcome of tracheostomy-free survival was compared between the two groups, accounting for age, sex, ALS-disease progression rate, disease duration, pulmonary vital capacity, bulbar ALS-onset, and presence of frontotemporal dementia or C9ORF72 mutation using inverse propensity treatment weights. Results: 182 patients with mean ± SD age 60±11 years were enrolled in the edaravone arm and 860 in the control arm (mean ± SD age 63±12 years). Mean ± SD time from onset to edaravone initiation was 18±10 months. Tracheostomy-free survival will be calculated. Conclusions: This study will provide evidence for edaravone effectiveness on tracheostomy-free survival in patients with ALS.

Background: Mutations in the Cu/Zn superoxide dismutase 1 (SOD 1) gene are estimated to cause 20% of familial ALS and 1-2% of sporadic cases. Accurate gene variant classification of novel mutations in amyotrophic lateral sclerosis (ALS) has deepened our understanding of clinical phenotypes, provided pathologic insights, and is crucial to incorporating emerging therapies. Methods: We describe a case of a 75-year-old female who presented with a rapidly progressive lower motor neuron syndrome leading to flaccid quadriparesis and complete loss of independence over a five month period. Results: Genetic testing demonstrated a heterozygous variant of uncertain significance in the SOD1 gene with a g > c point mutation at position 382 that has been described in one other patient in available literature. MR of the lumbar spine demonstrated abnormal smooth nerve root enhancement. Conclusions: This novel mutation in the SOD1 gene may be associated with a rapidly progressive phenotype of sporadic ALS. Ventral nerve root enhancement should not exclude a diagnosis of ALS especially in the absence of nodularity or nerve enlargement.

Carpal tunnel syndrome is the most frequently diagnosed, best understood and most easily treated entrapment neuropathy. During the first half of the 20th century, however, most patients with carpal tunnel syndrome were diagnosed as having compression of either the brachial plexus or thenar nerve motor branch of the median nerve. As late as 1950, only twelve patients with operative release of the transverse carpal ligament for idiopathic carpal tunnel syndrome had been reported. The delay in accurate anatomical localization of this compressive neuropathy can be attributed both to the confusion caused by the diverse manifestations of median nerve compression in the carpal tunnel, and to some interesting developments that altered early investigations in this area.

Zusammenfassung Die O. imp. ist eine klinisch wohl umschriebene Krankheit. Die pathologisch-anatomisch begründete Auffassung, daß es sich bei der O. imp. um eine allgemeine Minderwertigkeit der Stützgewebe des Organismus handelt, findet durch unsere Fälle eine Stütze. Die mannigfaltigen klinischen Symptome lassen sich hieraus einwandfrei erklären. Es handelt sich um eine angeborene Störung, die in verschieden hohen Graden und an verschiedenen Derivaten des Mesenchyms auftreten kann. Je früher die Störung auftritt, um so unangenehmer für das Knochenwachstum. Tritt sie beim Embryo bereits auf, ist das Kind nicht lebensfähig. Blaufärbung der Sklera ist nicht in allen, so den meisten angeführten chirurgischen Fällen, trotz ausgedehnter Knochenerkrankung vorhanden. Das Blutbild zeigt typische Veränderungen in Form von Polyglobulie. Röntgenologisch besteht eine auffallende Strahlendurchlässigkeit der Epiphysen, während die Struktur der Diaphysen normal ist. Die Frakturen treten auch noch im späteren Lebensalter auf, zeigen aber dann geringe Heilungstendenz.

Zusammenfassung: Mit einem aus einer Analysenwaage konstruierten Spezialgert wurden Untersuchungen an je vier Leber- und vier Hirngewebsproben zur Frage der Haftfhigkeit der Cooperschen Kltesonde am Gewebe vorgenommen. Gemessen wurden Zugkraft und Dilatation in Abhngigkeit von der Temperatur. Es erwies sich, da die Haftfhigkeit einmal mit dem Wassergehalt ansteigt und zweitens von gewebsspezifischen Faktoren abhngig ist. Wird eine mechanisch brauchbare Festigkeit der Haftzone und des angrenzenden Gewebes bei der Klteanwendung verlangt, so ist es zweckmig, bis zu −60 C und tiefer abzukhlen, wenn das Gewebe wasserarm ist. Bei wasserreichem Gewebe gengt bereits eine Abkhlung auf unterhalb von −20 C. Soll die Kltehaftung gering sein, so empfiehlt sich, eine Vereisungstemperatur von −20 C nicht zu unterschreiten.

Nach Belastung des Säurebasengleichgewichtes nierengesunder Versuchspersonen mit Bicarbonat zeigte sich deutlich im Harn eine Verschiebung derpH-Kurve zur alkalischen Seite, nach saurem Natriumphosphat zur sauren Seite; ferner im Magensaft meist antagonistisches Verhalten; im Blut analoge Verschiebung der Werte, wie im Harn. Bei Nierenkranken waren die Ergebnisse nicht einheitlich. Bei einigen schweren doppelseitigen Schädigungen fielen geringgradige oder paradoxe Ausschläge auf, die die Annahme einer mangelhaften Nierenfunktion wahrscheinlich machen. Die gleichzeitige Verfolgung derpH-Kurve im Magensaft gab keinen klaren Einblick in den Regulationsmechanismus bei gestörter Nierenfunktion.

By comparing approximate and exact transition probabilities for several new collision systems, we have established lower, less restrictive, values for validity criteria for use in predicting which collinear atom−triatom or diatom−diatom collision systems could be reliably treated in the vibrational sudden approximation. The specific lower bounds for the atom−triatom (A + 1−2−1) validity criterion, 2m1(2m1 + m2 + mA)/[mA(2m1 + m2)], and the diatom−diatom (A2 + B2) validity criterion, mB/3mA, are 14 and 6, respectively.
