People requiring palliative care should have their needs met by services acting in accordance with their wishes. A hospice in the south of England provides such care via a 24/7 hospice at home service. This study aimed to establish how a nurse-led night service supported patients and family carers to remain at home and avoid hospital admissions. Semi-structured interviews were carried out with family carers (n=38) and hospice-at-home staff (n=9). Through night-time phone calls and visits, family carers felt supported by specialist hospice staff whereby only appropriate hospital admission was facilitated. Staff provided mediation between family carer and other services enabling more integrated care and support to remain at home. A hospice-at-home night service can prevent unnecessary hospital admissions and meet patient wishes through specialist care at home.

Anemia is common in patients with chronic kidney disease (CKD) and those who have received a kidney allograft. Anemia is most prevalent in kidney transplant recipients before and immediately after transplantation, but also can occur months after transplantation if the donor kidney begins to fail. Replacement therapy for CKD-related and posttransplantation anemia is effective through the administration of exogenous erythropoiesis-stimulating proteins. Darbepoetin alfa (Aranesp; Amgen Inc, Thousand Oaks, CA) is a unique erythropoiesis-stimulating protein that can be administered at an extended dosing interval relative to recombinant human erythropoietin because of its approximately 3-fold longer serum half-life. Although darbepoetin alfa has been shown to be an effective treatment for patients with anemia of CKD and anemia after kidney transplantation, limited data have been published showing efficacy in treating women with anemia of these conditions during pregnancy. We report a case of successful darbepoetin alfa treatment for severe anemia in a pregnant transplant recipient.

The X-ray structure of yeast 5-aminolaevulinic acid dehydratase, in which the catalytic site of the enzyme is complexed with a putative cyclic intermediate composed of both substrate moieties, has been solved at 0.16 nm (1.6 Å) resolution. The cyclic intermediate is bound covalently to Lys263 with the amino group of the aminomethyl side chain ligated to the active-site zinc ion in a position normally occupied by a catalytic hydroxide ion. The cyclic intermediate is catalytically competent, as shown by its turnover in the presence of added substrate to form porphobilinogen. The findings, combined with those of previous studies, are consistent with a catalytic mechanism in which the C–C bond linking both substrates in the intermediate is formed before the C–N bond.

For the purposes of this paper, I am defining children with EBD as those who, for reasons of emotional disturbance, cannot make use of their educational opportunities and are difficult to manage. Children with EBD usually have parallel difficulties within their families and communities and much of what I want to say here is based on the experience of working with children and young people who are at the end of the line in more than one area of their lives. Prior to training as a Child Psychotherapist, I worked as a teacher in a variety of mainstream schools and special education settings. The points I want to make and the issues I want to raise, arise out of that experience and have a bearing on the way I now think about working psychotherapeutically with this same client group.

This conference was held at The Roehampton Institute, London on 2-3 September 1992 with its main aim being to bring together those involved in tourism education and those from the industry to discuss the future of tourism marketing. Organised jointly by Tina Bryant (University of Hertfordshire) and John Eade (Roehampton Institute), the event attracted delegates mainly from the academic community, with members of the tourism industry conspicuous by their absence! De novo synthesis of two periplasmic enzymes in Escherichia coli, alkaline phosphatase and acid hexose phosphatase, have been studied in the presence and absence of new phospholipid synthesis. Alkaline phosphatase synthesis was initiated by a temperature shift in a strain carrying a phoA amber mutation and a temperature-sensitive suppressor mutation; acid hexose phosphatase was studied after relief of catabolite repression. Glycerol auxotrophs (gpsA) were used to control phospholipid synthesis. Synthesis of both enzymes proceeded at a normal rate for 0.5 to 1.0 generation of growth, although it was then curtailed. It is concluded that secretion of these enzymes is not obligatorily coupled to new net phospholipid synthesis.

IT has been customary to classify the groups of layer silicate minerals as trioctahedral or dioctahedral according as all the octahedral ion sites in the lattice are occupied, or only two sites out of three. Virtually all minerals in the layer silicate groups fall clearly into one of these two divisions, the number of octahedral sites per unit cell occupied being usually nearly round numbers.

THE iron hydrous layer silicate cronstedtite is unique in having ferric iron substituted for silicon in positions of tetrahedral co-ordination. Previous investigations have been made by Gossner, and Hendricks1, who described its crystallography as a kaolin-type structure with an orthorhombic three-layer unit cell.

THE occurrence of the hydrous ferrous silicate, chamosite, in sedimentary ironstones was described by Hallimond1. Later, with Harvey and Bannister2, he concluded that chamosite differed from the iron-bearing chlorites. Brindley3,4examined chamosites from laterites and other sources, and concluded that they were of kaolin-type. Many of the variations in X-ray diagrams were accounted for by the existence of two kaolin-type structures, one ortho-hexagonal and one monoclinic, with simply related cell dimensions. In common with some other clay minerals, chamosite had random displacements of layers parallel to the b-axis of integral multiples of b/3, giving rise to sharp (hkl) reflexions only for k = 3n. Brindley concluded from the presence of (hkl) reflexions for h ≠ 3n that there were no random a-axis displacements of layers within a single structure, similar to the ‘mistakes’ in the cobalt structure of Wilson5and Edwards, Lipson and Wilson6. The pure ortho-hexagonal form was found in an iron ore from the main oolitic beds at Corby, Northamptonshire, by Brindley and Youell7.

GREENALITE, a hydrous ferrous silicate, was first described in detail by Leith1; heavy contamination by other constituents of the iron-ore in which it occurred made analysis difficult. Discrepancies between analyses of the mineral by Leith and by Jolliffe2were ascribed by Gruner3to different methods of correcting for insoluble silica. Gruner found the X-ray pattern to be very similar to that of chrysotile, but different from chlorite, micas and chamosite. Gruner ascribed to greenalite a unit cell similar to chrysotile, although he found it impossible to tell whether the β-angle differed from that of chrysotile, 93° 16′. Gruner's analysis corresponded to a structural formula of Fe2+4.5Fe3+1.0(Si4O10)(OH)8.
