2. Primary postpartum haemorrhage adversely impacts breastfeeding initiation in Victoria, Australia.

Abstract: Breastfeeding has many important benefits for both mother and baby but sustained breastfeeding is sub-optimal. Identifying women who need increased support to establish breastfeeding has the potential to improve this. Analysis of the relationship between primary postpartum haemorrhage (PPH) and primary severe PPH and breastfeeding may prove informative as PPH has potentially negative impacts on breastfeeding. To determine the relationship between PPH and severe PPH and breastfeeding at postnatal discharge and formula use for breastfed babies in hospital. Population-based retrospective cohort study using the Victorian Perinatal Data Collection for all liveborn singleton births at ≥ 37 weeks' gestation (n = 339,854) for 2009–13 in Victoria. Estimated blood loss was categorised as PPH ≥ 500 mL and severe PPH ≥ 1500 mL. Descriptive analysis was conducted and multivariable logistic regression was used to determine the adjusted odds ratio for the relationship between PPH/severe PPH and breastfeeding outcomes after adjustment for relevant confounders. Overall, 94.9% of women initiated breastfeeding. Babies whose mother had a PPH or severe PPH were less likely than others to be exclusively breastfeeding at discharge (aOR 0.88; (95% CI 0.86, 0.90) and aOR 0.57; (95% CI 0.53, 0.61) respectively). Formula – given to 25.9% of all breastfed babies – was more likely for those whose mothers had a PPH or severe PPH (aOR 1.15; (95% CI 1.12, 1.17) and aOR 2.15; (95% CI 2.01, 2.29) respectively. Women have greater challenges establishing exclusive breastfeeding following PPH and severe PPH. Improving support in hospital for women following PPH may increase breastfeeding success. 3. Implementation of an obstetric triage decision aid into a maternity assessment unit and emergency department.

Abstract: The obstetric triage decision aid (OTDA) consists of 10 common pregnancy complaints with key signs and symptoms generating a triage score based on targeted questioning responses. It was developed to provide a standardised approach for obstetric triage conducted by midwives and emergency nurses as neither professional group are expert in the triage of pregnant and postpartum women. To evaluate implementation of the OTDA into an emergency department (ED) and maternity assessment unit (MAU). The OTDA was introduced to the ED and MAU of a hospital in Australia. A range of implementation strategies were utilised and assessed by pre and post staff survey, and a three-month post-audit of unscheduled maternity presentations. The primary outcome was adoption rate of the OTDA. Secondary outcomes were staff confidence and waiting times. Analyses were undertaken using SPSS (v24). Paired analysis was conducted on staff surveys. There were a total of 2829 unscheduled presentations: ED (n = 708) and MAU (n = 2121), 88.1% were triaged using the OTDA, used more in the MAU than the ED (93.2% vs 72.7%; p <.001). In the MAU, women seen within 15 min of arrival improved significantly from 42.0% to 78.0%. There was improvement in the self-rated confidence (p =.002) and competence (p =.004) by nurses and midwives to conduct obstetric triage. The introduction of the OTDA required different approaches to change practice. There were improvements in staff self-rated confidence and competence, a reduction in clinical risk associated with under-triage in the ED and improved prioritisation of care in the MAU. 4. Talking testing: Impact of a training intervention on midwives' antenatal HIV, hepatitis B and hepatitis C screening practice.

Abstract: Midwives play a critical role in ensuring that HIV, hepatitis B and hepatitis C screening occurs during early pregnancy, in accordance with national consensus guidelines and policies. Limited opportunities exist for midwives to gain the knowledge, skills and confidence required to initiate testing discussions at the first antenatal visit. To design, deliver and evaluate a workforce education intervention to build midwives' capacity to initiate testing for HIV and viral hepatitis. Victorian midwives were invited to enrol in an intervention which comprised a pre-learning package and a one-day study day covering clinical, epidemiological and psychosocial aspects of HIV, hepatitis B and hepatitis C testing in early pregnancy. A pre-/post-test design, incorporating a survey with eight knowledge items and four confidence items, was used to measure impact. Of the 69 participating midwives, 55 completed the pre-survey, 69 completed the post-survey and 19 completed a three-month follow up survey. Participant knowledge improved across all domains, with the most significant increases in the areas of HIV and viral hepatitis testing, transmission and treatment. Midwives' confidence levels increased following the intervention, and this was generally sustained among the smaller sample at the three-months. Our findings demonstrate that short educational interventions, designed and delivered by content experts, result in longer-term improvements in clinical practice which are crucial to ensuring women and their partners are given adequate information and recommendations about screening for HIV, hepatitis B and hepatitis C and during pregnancy. 6. Informing the development midwifery standards for practice: A literature review for policy development.

Abstract: • Addresses the gap in evidence to guide the development of midwifery standards for practice. • Identifies concepts central to both clinical and non-clinical midwifery practise. • Highlights interchangeable terms that have markedly different origins. • Presents contemporary midwifery identity, legitimacy and practice as autonomous, woman-centered and focussed on continuity of care, yet often confronted by constraints on being able to work to the full scope of practice. To critically appraise and synthesise the literature regarding the role and scope of midwifery practice, specifically to inform the evidence based development of standards for practice for all midwives in Australia. A structured scoping review of the literature CINAHL Complete, MEDLINE Complete and Cochrane Libraries databases, online and grey literature databases Comprehensive searches of databases used key words and controlled vocabulary for each database to search for publications 2006–2016. Studies were not restricted by research method. There is no substantive body of literature on midwifery competency standards or standards for practice. From 1648 papers screened, twenty-eight papers were identified to inform this review. Eight studies including systematic reviews were annotated with three research papers further assessed as having direct application to this review. To inform the development of Midwife standards for practice, the comprehensive role of the midwife across multiple settings was seen to include: woman centred and primary health care; safe supportive and collaborative practice; clinical knowledge and skills with interpersonal and cultural competence. Midwifery practice is not restricted to the provision of direct clinical care and extends to any role where the midwife uses midwifery skills and knowledge. This practice includes working in clinical and non-clinical relationships with the woman and other clients as well as working in management, administration, education, research, advisory, regulatory, and policy development roles. 7. Responding to violence against women: A qualitative study with midwives in Timor-Leste.

Abstract: The health sector is a critical partner in the response to violence against women, but little is known about how to translate international guidelines and sustainable good practice in remote and under-resourced health systems. This research explores the barriers and enablers that midwives report in responding to domestic and sexual violence in Timor-Leste, a country with a very high rate of violence against women. The aim is to inform a systems approach to health provider training and engagement applicable to Timor-Leste and other low-resource settings. In 2016 we conducted qualitative interviews and group discussions with 36 midwives from rural health settings, community health centres and hospitals in three municipalities of Timor-Leste. A range of individual, health system and societal factors shape midwives' practice. While training provided the foundation for knowing how to respond to cases of violence, midwives still faced significant health system barriers such as lack of time, privacy and a supportive environment. Key enablers were support from colleagues and health centre managers. Health provider training to address violence against women is important but tends to focus on individual knowledge and skills. There is a need to shift toward systems-based approaches that engage all staff and managers within a health facility, work creatively to overcome barriers to implementation, and link them with wider community-based resources. 8. The nature of labour pain: An updated review of the literature.

Abstract: Abstract Background The pain experience associated with labour is complex. Literature indicates psychosocial and environmental determinants of labour pain, and yet methods to support women usually target physiological attributes via pharmacological interventions. Aim To provide an update of our understanding of labour pain based on modern pain science. The review aims to help explain why women can experience labour pain so differently — why some cope well, whilst others experience great suffering. This understanding is pertinent to providing optimal support to women in labour. Method A literature search was conducted in databases Medline, Cumulative Index to Nursing and Allied Health Literature and PsycINFO, using search terms labor/labour, childbirth, pain, experience and perception. Thirty-one papers were selected for inclusion. Findings Labour pain is a highly individual experience. It is a challenging, emotional and meaningful pain and is very different from other types of pain. Key determinants and influences of labour pain were identified and grouped into cognitive, social and environmental factors. Conclusion If a woman can sustain the belief that her pain is purposeful (i.e. her body working to birth her baby), if she interprets her pain as productive (i.e. taking her through a process to a desired goal) and the birthing environment is safe and supportive, it would be expected she would experience the pain as a non-threatening, transformative life event. Changing the conceptualisation of labour pain to a purposeful and productive pain may be one step to improving women's experiences of it, and reducing their need for pain interventions. Abstract: Highlights • Delays to seek antenatal care and care during labour and birth persist in Timor-Leste. • Husbands, history and minimal birth preparedness influence these delays. • Delays are also influenced by emic beliefs, infrastructure limitations and geographical locations. • Perceptions of hospital policies, staff attitudes and the provision of respectful, quality care also contribute to delays seeking care. Abstract Background While global maternal deaths have decreased significantly, hundreds of thousands of women still die from pregnancy and birth complications. Interventions such as skilled birth attendants, emergency transportation to health facilities and birth preparedness have been successful at reducing such deaths, however barriers to seeking, reaching, and receiving respectful care persist. Objective This study aimed to identify what influences people's decisions to seek antenatal care and care during labour and birth in Timor-Leste, a low-middle income newly independent nation in South East Asia with a high maternal death rate. The study aimed to provide emic/local insights to help midwives and maternal health providers tailor care and resources appropriately, thus improving maternal health. Design This qualitative study with a decolonising methodology, was designed to explore the perceptions of reproductive aged Timorese women and men, situating Timorese worldviews in the centre of the research process. Data collection occurred in four municipalities of Timor-Leste in October 2015 and included 9 focus group discussions with 80 men, and 17 individual reproductive history interviews with women. Findings An expanded ‘Three Delays’ model was used to frame the findings. The study found multiple factors impacting on decisions to seek antenatal care and care during labour and birth. Husbands, history, minimal birth preparedness, ethno-physiological beliefs (personal perceptions of how the body works), infrastructure limitations, geographical location, hospital policies and staff attitudes influenced and potentially delayed the decision to seek or reach care. Key conclusion and implications for practice and policy Policies and programs that increase accessibility of midwives and encourage birth preparedness are vital. Given the current locus of power in families in Timor-Leste, it is imperative that men are educated regarding the importance of care from skilled providers, and supported to access such care with their partners. Abstract: Problem The rate and severity of postpartum haemorrhage (PPH) are increasing, according to research reports and clinical anecdote, causing a significant health burden for Australian women giving birth. However, reporting a national Australian rate is not possible due to inconsistent reporting of PPH. Background Clinician concerns about the incidence and severity of PPH are growing. Midwives contribute perinatal data on every birth, yet published population-based data on PPH seems to be limited. What PPH information is contributed? What data are publicly available? Do published data reflect the PPH concerns of clinicians? Aim To examine routine public reporting on PPH across Australia. Methods We systematically analysed routine, publicly reported data on PPH published in the most recent perinatal data for each state, territory and national report (up to and including October 2016). We extracted PPH data on definitions, type and method of data recorded, markers of severity, whether any analyses were done and whether any trends or concerns were noted. Findings PPH data are collected by all Australian states and territories however, definitions, identification method and documentation of data items vary. Not all states and territories published PPH rates; those that did ranged from 3.3% to 26.5% and were accompanied by minimal reporting of severity and possible risk factors. Whilst there are plans to include PPH as a mandatory reporting item, the timeline is uncertain. Conclusions Routinely published PPH data lack nationally consistent definitions and detail. Abstract: Antiplatelet therapy reduces the risk of major vascular events for people with occlusive vascular disease, although it might increase the risk of intracranial haemorrhage. Patients surviving the commonest subtype of intracranial haemorrhage, intracerebral haemorrhage, are at risk of both haemorrhagic and occlusive vascular events, but whether antiplatelet therapy can be used safely is unclear. Abstract: • Staphylococcus aureus on ultrasound (US) heads can survive for as long as 3 days. • Eighty percent of S aureus added to gel on US heads survived for 1 hour. • S aureus on US heads could cause infection if not properly cleaned. • This study advocates for improved cleaning/disinfecting of US heads in clinics. Therapeutic ultrasound (US) is commonly used in the rehabilitation of soft tissue injuries including wounds. US heads and coupling gel come into direct contact with patient skin, increasing the risk for health care-associated infections owing to cross contamination. 18. Data accuracy in the Victorian Perinatal Data Collection: Results of a validation study of 2011 data

Abstract: Objective: Public health data sets such as the Victorian Perinatal Data Collection (VPDC) provide an important source for health planning, monitoring, policy, research and reporting purposes. Data quality is paramount, requiring periodic assessment of data accuracy. This article describes the conduct and findings of a validation study of data on births in 2011 extracted from the VPDC.Method: Data from a random sample of one percent of births in Victoria in 2011 were extracted from original medical records at the birth hospital and compared with data held in the VPDC. Accuracy was determined for 93 variables. Sensitivity, specificity, positive predictive value and negative predictive value were calculated for dichotomous items.Results: Accuracy of 17 data items was 99% or more, the majority being neonatal and intrapartum items, and 95% or more for 46 items. Episodes of care with the highest proportion of items with accuracy of 95% or more were neonatal and postnatal items at 80 and 64%, respectively. Accuracy was below 80% for nine items introduced in 2009. Agreement between medical records and VPDC data ranged from 48% to 100%, the exception being two highly inaccurate smoking-related items. Reasons for discrepancies between VPDC data and medical records included miscoding, missing and inconsistent information.Conclusion: This study found high levels of accuracy for data reported to the VPDC for births in 2011; however, some data items introduced in 2009 and not previously validated were less accurate. Data may be used with confidence overall and with awareness of limitations for some new items.

Abstract: Variation in stillbirth rates across high-income countries and large equity gaps within high-income countries persist. If all high-income countries achieved stillbirth rates equal to the best performing countries, 19 439 late gestation (28 weeks or more) stillbirths could have been avoided in 2015. The proportion of unexplained stillbirths is high and can be addressed through improvements in data collection, investigation, and classification, and with a better understanding of causal pathways. Substandard care contributes to 20–30% of all stillbirths and the contribution is even higher for late gestation intrapartum stillbirths. National perinatal mortality audit programmes need to be implemented in all high-income countries. The need to reduce stigma and fatalism related to stillbirth and to improve bereavement care are also clear, persisting priorities for action. In high-income countries, a woman living under adverse socioeconomic circumstances has twice the risk of having a stillborn child when compared to her more advantaged counterparts. Programmes at community and country level need to improve health in disadvantaged families to address these inequities.

Abstract: This paper focuses on how preservice primary teachers can be supported to embrace digital learning technologies (DLTs) in their teaching of mathematics. The nature of the instruction and the assessment in the final mathematics unit of the bachelor of education program were changed. Despite being tagged as "tech-savvy," preservice students use digital technologies primarily for social networking and information retrieval. These uses of digital technologies do not guarantee any facility for their utilization as learning technologies, which may result in early career teachers being unprepared to enact the effective use of expensive equipment in schools. The provision of a communal constructivism environment supported student learning as they met the challenges of creating interactive digital applications to teach a mathematical concept to their peers. This paper is likely to be of interest to mathematics educators who are trying to steer preservice teachers away from "worksheet maths" as well as other preservice teacher educators who wish to incorporate digital technologies into their content and methodology units. Abstract: Objective: Findings from studies investigating optimal techniques for attenuating propofol-related injection pain are inconsistent. In previous studies, lidocaine pretreatment using a tourniquet has been reported to be superior, inferior, or equivalent to a lidocaine-propofol admixture for reducing pain. This discordance could represent either no meaningful difference in the treatments or underlying methodological differences in the previous studies. We hypothesized that tourniquet-controlled pretreatment with lidocaine would be superior to lidocaine-propofol admixture for reducing propofol injection pain. Methods: This randomized controlled trial compared 3 groups-a control group (saline pretreatment/saline admixture; n = 50), a pretreatment group (lidocaine pretreatment/saline admixture; n = 51), and an admixture group (saline pretreatment/lidocaine admixture; n = 50). The primary outcome was verbal pain score after injection. The incidence of pain on injection was explored as a secondary outcome. Results: The median (interquartile range) verbal pain score after study solution injection were as follows-control group: 3 (0-6), pretreatment group: 0 (0-0), and admixture group: 0 (0-2). The pretreatment group had significantly lower pain scores when compared with the admixture group (P = 0.016), and both groups were superior to the control group. The pretreatment group had fewer subjects experiencing any injection pain than did the admixture group (20% vs. 44%, respectively; P = 0.024). Conclusions: Tourniquet-controlled pretreatment with lidocaine is statistically superior to admixing lidocaine with propofol for reducing propofol injection pain intensity, but the clinical importance of this small effect is questionable. 28. Effect of an extended midwifery postnatal support programme on the duration of breast feeding: A randomised controlled trial.

Abstract: Objective: to evaluate the effects of an extended midwifery support (EMS) programme on the proportion of women who breast feed fully to six months. Design: randomised controlled trial. Setting: large public teaching hospital in Australia. Participants: 849 women who had given birth to a healthy, term, singleton baby and who wished to breast feed. Intervention: participants were allocated at random to EMS, in which they were offered a one-to-one postnatal educational session and weekly home visits with additional telephone contact by a midwife until their baby was six weeks old; or standard postnatal midwifery support (SMS). Participants were stratified for parity and tertiary education. Measurements: the main outcome measures were prevalence of full and any breast feeding at six months postpartum. Findings: there was no difference between the groups at six months postpartum for either full breast feeding [EMS 43.3% versus SMS 42.5%, relative risk (RR) 1.02, 95% confidence interval (CI) 0.87–1.19] or any breast feeding (EMS 63.9% versus SMS 67.9%, RR 0.94, 95%CI 0.85–1.04). Conclusions: the EMS programme did not succeed in improving breast-feeding rates in a setting where there was high initiation of breast feeding. Breast-feeding rates were high but still fell short of national goals. Implications for practice: continuing research of programmes designed to promote breast feeding is required in view of the advantages of breast feeding for all mothers and babies. Abstract: Abstract: Four groups of larval razorback sucker, an endangered fish, were exposed to selenium-laden zooplankton and survival, growth, and whole-body residues were measured. Studies were conducted with 5, 10, 24, and 28-day-old larvae fed zooplankton collected from six sites adjacent to the Green River, Utah. Water where zooplankton were collected had selenium concentrations ranging from <0.4 to 78μg/L, and concentrations in zooplankton ranged from 2.3 to 91μg/g dry weight. Static renewal tests were conducted for 20 to 25 days using reference water with selenium concentrations of <1.1μg/L. In all studies, 80–100% mortality occurred in 15–20 days. In the 28-day-old larvae, fish weight was significantly reduced 25% in larvae fed zooplankton containing 12μg/g selenium. Whole-body concentrations of selenium ranged from 3.7 to 14.3μg/g in fish fed zooplankton from the reference site (Sheppard Bottom pond 1) up to 94μg/g in fish fed zooplankton from North Roadside Pond. Limited information prior to the studies suggested that the Sheppard pond 1 site was relatively clean and suitable as a reference treatment; however, the nearly complete mortality of larvae and elevated concentrations of selenium in larvae and selenium and other elements in zooplankton indicated that this site was contaminated with selenium and other elements. Selenium concentrations in whole-body larvae and in zooplankton from all sites were close to or greater than toxic thresholds where adverse effects occur in fish. Delayed mortality occurred in larvae fed the two highest selenium concentrations in zooplankton and was thought due to an interaction with other elements. [Copyright &y& Elsevier]

Abstract: Abstract: Adult razorback sucker (Xyrauchen texanus) were exposed to various selenium concentrations in ponds and isolated river channels of the Colorado River near Grand Junction, CO, to determine effects on their growth and residue accumulation over an 11-month period. Adults at Horsethief ponds were fed a commercial diet, whereas fish at Adobe Creek channel and North Pond foraged on natural food items. Selenium concentrations at Horsethief were 2.2μg/L in water, 0.1–1.4μg/g in sediment, and 2.3–3.1μg/g in food organisms (1.1μg/g in commercial fish food), at Adobe Creek were 3.8μg/L in water, 0.5–2.1μg/g in sediment, and 4–56μg/g in food organisms, and at North Pond were 9.5μg/L in water, 7–55μg/g in sediment, and 20–81μg/g in food organisms. The selenium concentrations in muscle plugs from adults at Adobe Creek (11.7μg/g, SD=0.4, ) and North Pond (16.6μg/g, SD=1.0, ) were greater than at Horsethief (4.5μg/g, SD=0.2, ). During a depuration period adults from Adobe Creek and North Pond lost 1–2% of their selenium burden in 32 days and 14–19% in 66 days. Selenium accumulated in razorback sucker above toxic thresholds reported in other studies, yet those residues were less than those reported in muscle plugs of 40% of wild razorback sucker caught in the Green River, Utah. There are many different clinical practices that nurses can adopt to perform inotrope and vasopressor infusion line and syringe changes, some of which have the theoretical potential to cause fluctuations in blood pressure. 38. The Impact of Nausea and Vomiting of Pregnancy on Quality of Life

Abstract: Nearly all pregnant women experience nausea and vomiting of pregnancy (NVP) to some degree, yet its effects on quality of life (QOL) have not been studied extensively. The National Voice of Pregnancy survey was undertaken to increase understanding about the symptoms of NVP and their impact on QOL in women in the United States. In all, 621 currently or recently pregnant women completed an online survey about their NVP experiences. Their responses indicated that NVP can occur at any time of day or night and can have negative effects on eating, sleeping, and ability to handle domestic and occupational responsibilities. Relationships with spouse, family, friends, and coworkers can also be adversely affected. The effects of NVP are greater in women with moderate or severe symptoms, but women with mild NVP are also affected. These findings are consistent with previous studies conducted in various countries. Women sometimes fail to discuss NVP with their health care providers, and QOL issues rarely come up if it is discussed. Most women try dietary and other lifestyle modifications first to control their symptoms. Safe and effective medications are also available to help ease the symptoms of NVP and thereby improve QOL.

39. Triage of pregnant women in the emergency department: evaluation of a triage decision aid

Abstract: BackgroundApplying the Australasian Triage Scale to pregnant women presenting to emergency departments (EDs) is difficult as the descriptors may not reflect the urgency of the obstetric condition. This study aimed to examine whether condition-specific algorithms and triage education improved triage assessment and documentation of pregnant women presenting to the ED.MethodAlgorithms with a decision aid for triage with minimum agreed descriptors were developed to triage two pregnancy conditions (pre-eclampsia and antepartum haemorrhage). Triage documentation was then audited before (n=50) and after (n=50) a triage education programme which introduced algorithms for both conditions. Significant differences were examined using χ2test with significance set at p<0.05.ResultsThe quality of documentation of specific clinically significant symptoms of pre-eclampsia improved considerably, including the presence of headache from 58% pre-education to 80% post-education (p=0.002), visual disturbances from 58% to 90% (p<0.001), epigastric pain from 24% to 80% (p=0.002) and the presence of fetal movements from 62% to 90% (p=0.001). Documentation of descriptors for vaginal bleeding >20 weeks gestation improved for estimation of blood loss from 54% to 86% (p<0.001), patient ‘appearance’ from 32% to 62% (p=0.003) and, importantly, descriptions of patient's own assessment of their well-being from 8% to 28% (p=0.009).ConclusionThe introduction of triage education and condition-specific decision aids for triage markedly improved triage assessment and documentation. The application of algorithms may reduce clinical risk resulting from suboptimal triage of pregnant women presenting to EDs.

41. Creative Technologies as a Conduit for Learning in the Early Years

Abstract: THIS PAPER DESCRIBES THEuse of robotics in an Early Years classroom as a tool to aid the development of technological skills in a creative environment rich with literacy and numeracy opportunities. The pilot project described illustrates how a three-phase process can result in: (1) the development of emergent literacy and numeracy, and (2) the development of digital literacy and digital access for Early Years learners. The pilot study was conducted in a combined Prep–Year 1 class over a six-week period, during which students were introduced to and engaged in the creation of robots and simple machines via the use of LEGO WeDo©. The pilot was designed around three distinct phases: modelling, exploring, and evaluating. These phases provided scaffolding for the students to engage with the technology and for the class teacher to develop her own skills. This use of WeDo© is unique to Australia, unique to Early Years, and marries hands-on, fine-motor development with 21st century learning. The outcomes of the project astounded and delighted the teacher, the principal and the researchers.

42. A comparison of preferences for two GLP-1 products – liraglutide and exenatide – for the treatment of type 2 diabetes

Abstract: AbstractObjective:To use time trade-off (TTO) to compare patient preferences for profiles of two glucagon-like peptide (GLP-1) products for the treatment of type 2 diabetes (liraglutide and exenatide) that vary on four key attributes – efficacy (as measured by hemoglobin A1C), incidence of nausea, incidence of hypoglycemia, and dosing frequency (QD vs. BID) – and measure the contribution of those attributes to preferences.Methods:A total of 382 people with T2DM were recruited to participate in an internet-based survey consisting of a series of health-related questions, a conjoint exercise and a set of time trade-off items. In the conjoint exercise, respondents were presented with eight pairs of hypothetical GLP-1 profiles, and completed a time-tradeoff exercise for each pair.Results:The product profile representing liraglutide was preferred by 96 of respondents and resulted in significantly higher health utilities (0.038) than the product profile representing exenatide (0.978 vs. 0.94, p < 0.05). Estimated preference scores from the conjoint analysis revealed that efficacy measured by hemoglobin A1Cis the most important attribute, followed by nausea, hypoglycemia, and dosing schedule.Limitations:On-line participants may not represent ‘typical’ type 2 diabetes patients, and brief product profiles represented results from clinical trials, not clinical practiceConclusion:Based on the four attributes presented, patients prefer liraglutide over exenatide. Preference is based on superior efficacy and less nausea more than less hypoglycemia and once-daily dosing.

43. Trams, trains, planes and automobiles: logistics of conducting a statewide audit of medical records

Abstract: Objective:This paper reports on the logistics of conducting a validation study of a routinely collected dataset against medical records at hospitals to inform planning of similar studies. Method:A stratified random sample of 15 hospitals and two homebirth practitioners was included. Site visits were arranged following consent. In addition to the validation of perinatal data, information was collected regarding logistics. Results:Records at 14 metropolitan and rural hospitals up to 500 km from the research centre, and two homebirth practitioners, were audited. Obtaining consent to participate took between 5 days and 10 months. Auditors visited sites on 101 days, auditing 737 medical record pairs at 16 sites. Median audit time per record was 51.3 minutes; electronic records each took 36 minutes longer than paper. Travel time accounted for nearly one‐quarter of audit time. Conclusions:Delays obtaining consents, long travel times and electronic records prolonged audit duration and expense. Employment of experts maximised use of available audit time. Conducting a validation study is a time‐consuming and expensive exercise; however, confidence in the accuracy of public health data is vital. Implications:Validation studies are unquestionably important. Three alternative strategies have been proposed to make future studies viable.

Abstract: IntroductionMyotonic dystrophies type 1 and 2 (DM1 and DM2) are progressive, inherited multisys- tem disorders characterized by muscle weakness, myotonia and life-threatening cardiac dysrythmias. Treatment of myotonia is important for quality of life, mobility, and functional independence. Mexiletine is a sodium channel blocker, which has so far been shown to be effective in myotonia management in non-dystrophic myotonias. However, cardiac side effects are reported. Its long-term safety and efficacy in DM1 has not been established.Methods25 patients with DM1 or DM2 have been treated with mexiletine in the NHNN cohort. Efficacy and tolerance was determined by patient report. PR interval, QRS duration and corrected QT interval (QTc) were noted.ResultsMean duration of treatment was 32.9 months. 21/25 patients reported efficacy, 3/25 partial efficacy, and 4/25 discontinued treatment due to gastrointestinal side effects.Mexiletine did not precipitate symptomatic arrhythmia in any patient. Paired assessment of ECG param- eters at baseline and at the highest treatment dose revealed no significant change in PR interval, QRS complex duration, or QTc.ConclusionsThis small retrospective analysis suggests long-term efficacy and cardiac safety of mexiletine for treatment of myotonia in patients with myotonic dystrophies.christinamousele92@gmail.com

48. “See one, do one, teach one, have one”: A novel variation on regional anesthesia training

Abstract: Introduction:Is it possible to determine the number of nerve blocks needed for residents to become competent in regional anesthesia? Several studies have focused on this question, and the Residency Review Committee (RRC) for Anesthesiology has now defined a “minimum clinical experience” for some aspects of regional anesthesia training. In our experience, personally being a regional block recipient can also serve to enhance training. Methods:Many residents at Virginia Mason Medical Center have received regional anesthetics as volunteers in research projects. We designed questionnaires to define their perceptions in both performing and receiving regional anesthesia. Results:Twenty-one residents were recipients of a total of 72 regional anesthetic procedures. Many residents' comments focused on the discomfort of local anesthesia infiltration, the value of sedation, a better appreciation of the patients' perspectives, and improved preoperative consultation. Residents experiencing paresthesias were more likely to consider paresthesias as bad (P= .0098). Conclusion:The lessons learned from personally receiving a regional anesthetic are invaluable and can improve the quality of training, as well as the relationship between anesthesiologist and patient. 49. Influences of metamemory on performance predictions for text

Abstract: When predicting future performance on tests over text material, do individuals estimate retention in addition to assessing comprehension? In Experiment 1, participants either rated their comprehension or predicted performance for each text, with lower ratings indicating lower confidence either in comprehension or in eventual performance. Judgement magnitude was significantly lower for performance predictions than for comprehension ratings, suggesting that predictions were based partly on retention estimates. In Experiment 2, predictions varied with anticipated test delay (15 min or 2 weeks) whereas comprehension ratings did not, providing further evidence that individuals estimate retention when predicting performance. Analyses of individual differences suggest that both good and poor performers incorporate retention estimates when predicting performance, but better performers do so in a more discriminative manner. Implications for theory of metacognitive judgements are discussed.
