Purpose: The effect of direct laryngoscopy using a Macintosh blade (MAC) vsGlideScope™ videolaryngoscopy using a Spectrum LoPro blade (GVL) on nociceptive stimulation has not been quantitatively studied. This study used the new nociception level (NOL) index to compare the nociceptive response induced by GVL or MAC during laryngoscopy with or without intubation. Methods: Patients underwent two laryngoscopies at four-minute intervals (L1, L2), one with GVL and the other with MAC (first randomization). A third laryngoscopy (L3) followed by tracheal intubation was performed four minutes after L2 (GVL or MAC, second randomization). Nociception was quantitatively assessed by NOL and standard hemodynamic parameters (heart rate [HR] and mean arterial pressure). For the crossover design, blade comparisons accounted for sequence and blade type. A possible carryover effect between laryngoscopies was assessed. Results: In the 50 patients randomized, there was no carryover effect from one laryngoscopy to the next for all analyzed parameters. Nociception level index peak values were higher with MAC than GVL. Analysis of ΔNOL showed a lower nociceptive response with GVL for L1+L2 combined. Mean peak NOL values were significantly higher after L3+intubation than after L1+L2, for both GVL and MAC groups. Analysis of ΔHR values did not show a significant difference between GVL and MAC for any laryngoscopy. Conclusion: Laryngoscopy alone with GVL induces less nociception than with MAC. The NOL was more sensitive than HR at detecting nociceptive responses to MAC vsGVL. Additionally, and irrespective of which technique/blade was used, the combination of laryngoscopy + tracheal intubation produced a much greater nociceptive response than the laryngoscopy alone. In 2005, the first facial vascularized composite allotransplant was performed in France. In May 2018, our team at Maisonneuve-Rosemont Hospital, Montreal, Quebec, had the privilege to participate in the first facial transplant in Canada. Interdisciplinary collaboration, coordination, and communication formed the cornerstone of this medical undertaking and, ultimately, its success. This report details the anesthetic and organizational considerations of our experience.

Appliquer les principes de récupération optimisée après une chirurgie (Enhanced Recovery After Surgery:ERAS) est une option intéressante pouvant réduire la durée de séjour après un remplacement articulaire de la hanche ou du genou (PTH, PTG). Cette méthode permettrait d’améliorer l’expérience clinique du patient, afin qu’il soit disposé à retourner plus tôt à la maison.

Background: The Pain Monitoring Device (PMD) monitor (Medasense Biometrics Ltd., Ramat Gan, Israel) uses the Nociception Level (NOL) index, a multiple parameter-derived index that has recently shown a good sensitivity and specificity to detect noxious stimuli. The aim of this study was to assess the latest version of the device (PMD200TM) on variations of the NOL response after standardized tetanic stimuli to study the correlation between remifentanil doses and NOL. Methods: Data from 26 patients undergoing midline laparotomy and receiving a desflurane-remifentanil-based anesthetic coupled with low thoracic epidural analgesia were analyzed. A standardized tetanic stimulus was applied to the forearm of the patients at different remifentanil infusion rates. The primary aim was to evaluate the correlation between post-tetanic stimulation NOL values from the PMD200 and remifentanil doses. The NOL index variations after experimental and clinical stimuli were also compared with heart rate (HR), mean arterial pressure (MAP), and Bispectral Index™ (BIS). Results: A correlation between post-tetanic stimulation NOL values and remifentanil doses was found (r = −0.56; 95% confidence interval [CI], −0.70 to −0.44; P< 0.001). The NOL discriminated noxious from non-noxious states with the maximal Youden’s index value of the NOL receiver operating characteristic (ROC) curve showing a specificity of 88% (95% CI, 69.0 to 100) and sensitivity of 79.1% (95% CI, 56.2 to 95.5). The area under the NOL ROC curve (AUC, 0.9; 95% CI, 0.84 to 0.95) was significantly different from the other variables (P< 0.001 vsHR; P< 0.001 vsMAP; P< 0.001 vsBIS). Conclusions: The NOL value after noxious stimulus decreased with incremental remifentanil doses, showing a significant inverse correlation between the NOL index and opioid doses. The sensitivity and specificity of NOL to discriminate between noxious and non-noxious stimuli suggests its interesting potential as a monitor of nociception intensity during anesthesia. Analgesia might be evaluated with simple changes in vital signs, a non-specific and non-sensitive method. Heart rate variability (HRV) correlates with autonomous nervous system activity and can be used to evaluate painful stimuli. Heart rate variability is then transformed into a numeric scale called the analgesia nociception index (ANI), where higher values represent predominant parasympathetic tone, thus low nociception. Under general anesthesia, the ANI decreases following painful stimuli and increases after administration of analgesia, but significant interindividual variability is present. The goal of the present study was to evaluate the ANI as a pain index in healthy awake volunteers. Following research ethics board approval, participants were positioned supine in a calm operating room. The participants’ vital signs and ANI were monitored. After evaluation of all parameters without any stimulation, 23 volunteers received a 2 Hz electrical stimulus at the wrist with increasing current intensity from 0–30 mA (5 mA increments). The current was kept constant for three minutes at each level, and the volunteers rated their pain on a numeric rating scale (NRS) every minute. The Pearson correlation coefficient and linear regression were used to analyze the relationship between the ANI and the NRS score. The ANI absolute values and the variations from baseline were both analyzed. There was a very weak negative correlation between the NRS score and ANImean(Pearson, -0.089; 95% confidence interval [CI], −0.192 to −0.014; P= 0.045) and between the NRS score and ∆ANImean(Pearson, −0.174; 95% CI, −0.272 to −0.072; P< 0.001; regression slope, −0.586; 95% CI, −0.930 to −0.243; P< 0.001). Heart rate, blood pressure, and respiratory rate did not vary significantly throughout the study. These findings provide little evidence to support use of the ANI in awake subjects or in awake patients such as those in the emergency room or in the intensive care unit. Nevertheless, based on an important difference between the expected correlation and the real correlation between the ANI and the NRS scores found in our results, the present study might be underpowered. Studies with a larger sample size would be required to enable firm conclusions about the clinical utility of the ANI in this population of awake volunteers as well as in awake patients. La variabilité de la fréquence cardiaque (HRV) est corrélée à l’activité du système nerveux autonome et peut être utilisée pour évaluer les stimuli douloureux. La variabilité de la fréquence cardiaque est ensuite transformée en une échelle numérique, l’indice ANI (pour analgesia nociception index), sur laquelle les valeurs plus élevées représentent un tonus parasympathique prédominant, soit une nociception moindre. Sous anesthésie générale, l’ANI diminue après des stimuli douloureux et augmente après l’administration d’une analgésie, mais il existe une importante variabilité interpersonnelle. L’objectif de cette étude était d’évaluer l’ANI en tant qu’indice de la douleur chez des volontaires sains éveillés. Après avoir reçu l’approbation du Comité d’éthique de la recherche, les participants ont été placés en décubitus dorsal dans une salle d’opération calme. Les signes vitaux et l’ANI des participants ont été monitorés. Après l’évaluation de tous les paramètres sans aucune stimulation, 23 volontaires ont reçu un stimulus électrique de 2 Hz au poignet avec une intensité de courant augmentant de 0 à 30 mA (en incréments de 5 mA). Le courant a été maintenu pendant trois minutes à chaque niveau, et les volontaires ont évalué leur douleur sur une échelle d’évaluation numérique (EEN) chaque minute. Le coefficient de corrélation de Pearson et la régression linéaire ont été utilisés pour analyser la relation entre l’ANI et le score sur l’EEN. Les valeurs absolues de l’ANI et les variations par rapport aux valeurs de base ont toutes deux été analysées. Il y a eu une très faible corrélation négative entre le score sur l’EEN et l’ANImean(Pearson, −0,089; intervalle de confiance [IC] 95%, −0,192 à −0,014; P= 0,045) et entre le score sur l’EEN et ∆ANImean(Pearson, −0,174; IC 95%, −0,272 à −0,072; P< 0,001; courbe de régression, −0,586; IC 95%, −0,930 à −0,243; P< 0,001). Il n’y a pas eu de variation significative de la fréquence cardiaque, de la tension artérielle et de la fréquence respiratoire tout au long de l’étude. Ces résultats ne sont pas suffisamment probants pour appuyer l’utilisation de l’ANI chez des sujets éveillés ou des patients éveillés tels que ceux de l’urgence ou de l’unité des soins intensifs. Toutefois, étant donné l’importante différence entre la corrélation anticipée et la corrélation véritable entre l’ANI et les scores sur l’EEN dans nos résultats, il est possible que notre étude manque de puissance. Des études portant sur un échantillon plus vaste seraient nécessaires pour parvenir à des conclusions définitives concernant l’utilité clinique de l’ANI chez cette population de volontaires éveillés ainsi qu’auprès de patients éveillés. Waiting five to six minutes before measuring a train-of-four (TOF) after a 50-Hz tetanic stimulation or post-tetanic count (PTC) in order to allow the facilitation of transmission to subside is commonly recommended but is based on limited evidence. The purpose of this study was to measure the TOF responses after PTC in one hand and to compare the responses with those in the contralateral (control) hand. Twenty-two adult patients undergoing elective surgery under opioid-desflurane anesthesia were fitted with sensors to measure displacement of their thumbs in response to ulnar nerve stimulation. Rocuronium 0.6 mg·kg−1was administered and TOF stimulation was applied to both sides. One side was randomized to PTC (50-Hz tetanus followed by a three-second pause and 15 1-Hz stimuli) when the opposite (control) side recovered to 10% first twitch (T1) height. Train-of-four stimulation was continued bilaterally every 20 sec until 30 min after PTC. Comparisons of the T1 and TOF ratio (T4/T1) were made at two, five, ten, 20, and 30 min. The mean (standard deviation [SD]) T1 value in the PTC arm was 11.8 (7.1)% just before PTC. The T1 values were significantly greater in the PTC arm at two, five, and ten minutes, with mean (SD) differences of 6.3 (6.7)%, 9.4 (6.8)%, and 7.4 (3.9)%, respectively (P= 0.008). There were no significant differences in T1 values between groups at 20 and 30 min, and no statistically significant differences in T4/T1 values at any time. A small but clinically insignificant increase in T1 is seen for at least ten minutes after PTC without any detectable change in T4/T1 values. The TOF responses are reliable as early as one minute after PTC. Le fait d’attendre cinq à six minutes avant de mesurer un train-de-quatre (TdQ) après une stimulation tétanique de 50 Hz ou un compte post-tétanique (PTC) afin de permettre à la facilitation de transmission de diminuer est une pratique souvent recommandée, mais qui ne se fonde que sur des données probantes limitées. L’objectif de cette étude était de mesurer les réponses au TdQ après un PTC dans une main et de comparer ces réponses à celles observées dans la main contralatérale (témoin). Nous avons placé des senseurs sur vingt-deux patients adultes subissant une chirurgie non urgente sous anesthésie combinant opioïdes et desflurane afin de mesurer le déplacement de leurs pouces en réponse à une stimulation du nerf cubital. Nous leur avons administré 0,6 mg·kg−1de rocuronium et la stimulation en TdQ a été appliquée des deux côtés. Un côté a été randomisé à un PTC (tétanos à 50 Hz suivi d’une pause de 3 sec puis 15 stimuli à 1 Hz), alors que l’autre côté (témoin) récupérait jusqu’à 10% de l’intensité de la première contraction (T1). La stimulation en train-de-quatre a été poursuivie bilatéralement toutes les 20 sec jusqu’à 30 min après le PTC. Des comparaisons du ratio entre la T1 et le TdQ (T4/T1) ont été effectuées à deux, cinq, dix, 20 et 30 min. La valeur moyenne (écart type [ÉT]) de T1 dans le bras PTC était de 11,8 (7,1) % juste avant le PTC. Les valeurs de T1 étaient significativement plus élevées dans le bras PTC à deux, cinq et dix minutes, avec des différences moyennes (ÉT) de 6,3 (6,7) %, 9,4 (6,8) % et 7,4 (3,9) %, respectivement (P= 0,008). Aucune différence n’a été observée dans les valeurs de T1 entre les groupes à 20 et 30 min, et aucune différence significative d’un point de vue statistique n’a été observée entre les valeurs T4/T1 non plus, à quelque moment que ce soit. Une augmentation légère mais non significative d’un point de vue clinique de la valeur T1 est observée pour au moins dix minutes après le PTC, sans changement détectable des valeurs de T4/T1. Les réponses au TdQ sont fiables dès une minute après le PTC.

Coughing episodes occur frequently at extubation after thoracic surgery, and this may be due in part to the double-lumen tube (DLT). In this study, the DLT was replaced with either a single-lumen endotracheal tube (ETT) or a laryngeal mask airway (LMA) device or left in place, and the incidence of coughing at emergence was compared between the three groups. Fifty-eight adults scheduled for thoracic surgery with a DLT were included. Exclusion criteria were an anticipated difficult airway, obesity, and contraindication to the use of an LMA ProSeal™ (LMA-P). After surgery but before emergence, patients were randomized to having the DLT (1) removed and replaced by an LMA-P (LMA-P Group), (2) removed and replaced by an ETT (ETT Group), or (3) left in place (DLT Group). The primary outcome was the number of coughing episodes at extubation. Among 184 patients screened, 124 did not meet inclusion criteria, and two patients, both in the ETT Group, were excluded after randomization, leaving 20, 18, and 20 patients in the LMA-P, ETT, and DLT Groups, respectively. There were fewer coughing episodes (median [quartiles]) in the LMA-P Group than in the DLT Group (0[0-1] vs2[1-3], respectively; P= 0.01). In the DLT Group, 90% of patients coughed at least once. This incidence was not significantly different in the ETT Group (83%; P= 0.222) but was significantly reduced in the LMA-P Group (35%; P< 0.001). No patient had oxygen desaturation during airway exchange or at extubation. The incidence and severity of hoarseness and sore throat were similar in all groups. Coughing at extubation after thoracic surgery can be reduced if the DLT is replaced by an LMA-P before emergence. The number of patients in this trial was too small to evaluate the risks associated with exchanging the airway device. This trial was registered at ClinicalTrials.gov: NCT00925613. Les épisodes de toux surviennent fréquemment lors de l’extubation après une chirurgie thoracique, et ils pourraient être provoqués en partie par la sonde double lumière (SDL). Dans cette étude, nous avons remplacé la SDL par une sonde endotrachéale (SET) simple lumière, par un masque laryngé (ML) ou l’avons laissée en place, et nous avons comparé l’incidence de toux au réveil dans les trois groupes. Cinquante-huit adultes devant subir une chirurgie thoracique avec une SDL ont été recrutés pour cette étude. Les critères d’exclusion étaient des voies aériennes anticipées comme difficiles, l’obésité et toute contre-indication à l’utilisation d’un masque laryngé ProSeal™ (ML-P). Après la chirurgie mais avant le réveil, les patients ont été aléatoirement attribués à l’un des trois groupes suivants : (1) retrait de la SDL et remplacement par un ML-P (groupe ML-P), (2) retrait et remplacement de la SDL par une SET (groupe SET), ou (3) SDL laissée en place (groupe SDL). Le critère d’évaluation principal était le nombre d’épisodes de toux lors de l’extubation. Parmi les 184 patients évalués, 124 ne répondaient pas aux critères d’inclusion, et deux patients du groupe SET ont été exclus après la randomisation, laissant 20, 18 et 20 patients dans les groupes ML-P, SET et SDL, respectivement. Il y a eu moins d’épisodes de toux (médianes [quartiles]) dans le groupe ML-P que dans le groupe SDL (0 [0-1] vs2 [1-3], respectivement; P= 0,01). Dans le groupe SDL, 90 % des patients ont toussé au moins une fois. Le nombre de patients dans cette étude était trop restreint pour évaluer les risques associés au changement de dispositif pour les voies aériennes. Ideally, preoxygenation is performed using a tight fitting mask either by breathing normally for three to five minutes or with four to eight vital capacity (VC) breaths in 0.5 to one minute, but in practice leaks are frequent and sometimes unavoidable. This study was designed to determine which breathing method provided the best oxygenation in the presence of leak. Twenty volunteers were instructed to breathe from a circle circuit supplied with 6 L·min-1of fresh oxygen. Each subject was tested under four situations selected in random order: 1) normal breathing for three minutes without leak; 2) normal breathing for three minutes with a leak; 3) four VCs in 30 sec without a leak; and 4) four VCs in 30 sec with a leak. The leak was created by a piece of size 18 French nasogastric tube, 5 cm long, taped under the face mask. Inspired and expired O2and CO2were sampled at the nostrils. In the absence of a leak, the end-tidal oxygen fraction (FEO2) was greater after three minutes of tidal breathing (89 ± 3%; mean ± SD) in comparison with the response to four VCs (76 ± 7%; P < 0.001). Introduction of a leak decreased the FEO2significantly (P < 0.001). With a leak, the FEO2was similar with normal breathing (61 ± 8%) and after four VCs (59 ± 11%). Preoxygenation with tidal volume breathing for three minutes yields higher FEO2in comparison to four VCs. If a small leak (4 mm internal diameter) is introduced, the FEO2decreases significantly with both breathing methods to approximately 60%. En pratique, les fuites sont fréquentes et quelquefois incontournables. Cette étude fut réalisée pour déterminer quelle méthode produit la meilleure préoxygénation en présence de fuite. Vingt volontaires ont respiré dans un circuit en cercle fournissant 6 L·min-1ďoxygène frais. Chaque sujet fut soumis à quatre situations sélectionnées de façon aléatoire: 1) respiration normale pour trois minutes sans fuite; 2) respiration normale pour trois minutes avec fuite; 3) quatre CV en 30 sec sans fuite; 4) quatre CV en 30 sec avec fuite. La fuite a été créée avec un segment de tube nasogastrique de taille 18, long de 5 cm, fixé sous le masque. Les fractions inspirées et expirées ďO2et de CO2ont été mesurées aux narines. En ľabsence de fuite, la fraction expirée ďoxygène (FEO2) est plus grande avec trois minutes de respiration normale (89 ± 3%; moyenne ± ET) qu’avec quatre CV (76 ± 7%, P < 0,001). Avec une fuite, la FEO2diminue significativement (P < 0,001). En présence ďune fuite, la FEO2est la même avec respiration normale (61 ± 8%) qu’avec quatre respirations profondes (59 ± 11%). Une préoxygénation avec respiration normale pour trois minutes donne une FEO2plus élevée qu’avec quatre C V. Si une petite fuite (4 mm de diamètre interne) est présente, la FEO2s’abaisse de façon significative à environ 60 % avec les deux méthodes de préoxygénation.

Purpose: To present and discuss a case of opioid-induced rigidity with low-dose fentanyl during recovery from anesthesia. Clinical features: A 41-yr-old woman underwent laparotomy for total abdominal hysterectomy and bilateral salpingooophorectomy under general anesthesia. She received a total of 500 μg of fentanyl by iv intermittent boluses during the three-hour anesthetic. During emergence from anesthesia, while intubated, the patient presented with rigidity. No changes in ventilatory parameters were measured during the episode. The only notable predisposing factor was treatment with venlafexine, an antidepressant that modifies serotonin and norepinephrine levels. She was successfully treated with iv naloxone 20 μg. The rest of the postoperative period was uneventful. Conclusion: We observed an atypical case of opioid-induced rigidity in contrast to the classical syndrome, which presents at induction with high-dose opioids. This syndrome has many clinical presentations with neurologic and ventilatory signs of varying intensity. Early recognition of the syndrome and adequate treatment is crucial. If treated adequately, opioid-induced rigidity is self-limited with few complications.

Advances in the treatment of congenital heart disease have led to a new group of adolescents or adults patients with cardiac anomalies. The anaesthetic management of these patients can be challenging especially when they are scheduled for major noncardiac surgery inducing haemodynamic instability. We report the case of a 14‐year‐old boy scheduled for posterior spinal fusion for idiopathic scoliosis who underwent a Fontan operation in infancy for pulmonary atresia with right ventricle hypoplasia. The preoperative investigations and the anaesthetic management are described.

Purpose: To describe a case of a massive gastric bleeding following emergency coronary artery bypass surgery associated with transoesophageal echocardiographic (TEE) examination. Clinical features: A 50-yr-old man was referred for an acute myocardial infarction and pulmonary edema (Killip class 3). Twelve hours after his myocardial infarction, he was still having chest pain despite anivheparin infusion. Coronary angiography revealed severe three-vessel disease with multifocal stenosis of the left anterior descending, circumflex and total occlusion of the right coronary artery. The patient was transferred to the operating room for emergency coronary artery bypass graft surgery. After total systemic heparinization (3 mg·kg−1) was obtained for cardiopulmonary bypass, a multiplane TEE probe was inserted without difficulty to monitor myocardial contractility during weaning from CPB. During sternal closure, the TEE probe was removed and an orogastric tube was inserted with immediate drainage of 1,200 ml red blood. Endoscopic examination demonstrated a mucosal tear near the gastro-oesophageal junction and multiple erosions were seen in the oesophagus. These lesions were succesfully treated with submucosal epinephrine injections and the patient was discharged from the hospital eight days after surgery. Conclusion: This is a report of severe gastrointestinal hemorrhage following TEE examination in a fully heparinized patient. This incident suggest that, if the use of TEE is expected, the probe should preferably be inserted before the administration of heparin and the beginning of CPB.
