2. Chemotherapy decision-making in advanced lung cancer: a prospective qualitative study

Abstract: ObjectiveTo study how treatment decisions are made alongside the lung cancer clinical pathway.MethodsA prospective, multicentre, multimethods, five-stage, qualitative study. Mediated discourse, thematic, framework and narrative analysis were used to analyse the transcripts.Results51 health professionals, 15 patients with advanced lung cancer, 15 family members and 18 expert stakeholders were recruited from three UK NHS trusts. Multidisciplinary team (MDT) members constructed treatment recommendations around patient performance status, pathology, clinical information and imaging. Information around patients’ social context, needs and preferences were limited. The provisional nature of MDTs treatment recommendations was not always linked to future discussions with the patient along the pathway, that is, patients’ interpretation of their prognosis, treatment discussions occurring prior to seeing the oncologist. This together with the rapid disease trajectory placed additional stress on the oncologist, who had to introduce a different treatment option from that recommended by the MDT or patient’s expectations. Palliative treatment was not referred to explicitly as such, due to its potential for confusion. Patients were unaware of the purpose of each consultation and did not fully understand the non-curative intent of treatment pathways. Patients’ priorities were framed around social and family needs, such as being able to attend a family event.ConclusionMissed opportunities for information giving, affect both clinicians and patients; the pathway for patients with non-small cell lung cancer focuses on clinical management at the expense of patient-centred care. Treatment decisions are a complex process and patients draw conclusions from healthcare interactions prior to the oncology clinic, which prioritises aggressive treatment and influences decisions.

3. BCG vaccination of healthcare workers for protection against COVID-19: 12-month outcomes from an international randomised controlled trial.

Abstract: Bacille Calmette-Guérin (BCG) vaccine has immunomodulatory effects that may provide protection against unrelated infectious diseases. We aimed to determine whether BCG vaccination protects adults against COVID-19. Phase III double-blind randomised controlled trial. Healthcare centres in Australia, Brazil, the Netherlands, Spain, and the United Kingdom during the COVID-19 pandemic. 3988 healthcare workers with no prior COVID-19 and no contraindication to BCG. Randomised 1:1 using a web-based procedure to receive a single 0.1 mL intradermal dose of BCG-Denmark (BCG group, n = 1999) or saline (placebo group, n = 1989). Difference in incidence of (i) symptomatic and (ii) severe COVID-19 during the 12 months following randomisation in the modified intention to treat (mITT) population (confirmed SARS-CoV-2 naïve at inclusion). Of the 3988 participants randomised, 3386 had a negative baseline SARS-CoV-2 test and were included in the mITT population. The 12-month adjusted estimated risk of symptomatic COVID-19 was higher in the BCG group (22.6%; 95% confidence interval [CI] 20.6 to 24.5%) compared with the placebo group (19.6%; 95% CI 17.6 to 21.5%); adjusted difference +3.0% points (95% CI 0.2 to 5.8%; p = 0.04). The 12-month adjusted estimated risk of severe COVID-19 (mainly comprising those reporting being unable to work for ≥3 consecutive days) was 11.0% in the BCG group (95% CI 9.5 to 12.4%) compared with 9.6% in the placebo group (95% CI 8.3 to 11.1%); adjusted difference +1.3% points (95% CI −0.7 to 3.3%, p = 0.2). Breakthrough COVID-19 (post COVID-19 vaccination) and asymptomatic SARS-CoV-2 infections were similar in the two groups. There were 18 hospitalisations due to COVID-19 (11 in BCG group, 7 in placebo group; adjusted hazard ratio 1.56, 95% CI 0.60 to 4.02, p = 0.4) and two deaths due to COVID-19, both in the placebo group. Compared to placebo, vaccination with BCG-Denmark increased the risk of symptomatic COVID-19 over 12 months among healthcare workers and did not decrease the risk of severe COVID-19 or post-vaccination breakthrough COVID-19. Abstract: Patients with advanced oesophageal cancer have a median survival of 3–6 months, and most require intervention for dysphagia. Self-expanding metal stent (SEMS) insertion is the most typical form of palliation in these patients, but dysphagia deterioration and re-intervention are common. This study examined the efficacy of adjuvant external beam radiotherapy (EBRT) compared with usual care alone in preventing dysphagia deterioration and reducing service use after SEMS insertion.

6. Are We Missing the Opportunity to Measure Muscle Mass on Computed Tomography Thorax?

Abstract: Hand grip strength (HGS) and fat free mass index (FFMI) are important indicators of skeletal muscle mass and correlate with prognosis in patients with respiratory diseases. It is also possible to estimate muscle mass by measuring muscle density and volume on cross sectional imaging. We reviewed all patients of the All Wales Cystic Fibrosis Centre who had a computed tomography thorax as part of routine clinical care between 2013 and 2017. By multiplying the volume and average Hounsfield units of the paraspinal muscles at T4 and T12 levels we were able to estimate the patients skeletal muscle mass. This was compared with their FFMI, HGS and forced expiratory volume in 1 second. Measurements of muscle mass at T4 and T12 showed significant correlation with HGS and FFMI, and T12 also showed significant correlation with forced expiratory volume in 1 second. This method may provide further prognostic information for patient with cystic fibrosis, particularly where equipment for HGS and FFMI assessments are lacking.

7. SMALL MAMMAL ABUNDANCE ON THE STANDING ROCK AND LAKE TRAVERSE INDIAN RESERVATIONS IN NORTH AND SOUTH DAKOTA.

Abstract: Small mammal diversity and abundance were assessed from traps set in various relatively undisturbed habitats on the Lake Traverse Indian Reservation in 2011 and 2012 and on the Standing Rock Indian Reservation in 2014 through 2015. The purpose of these efforts is to document diversity and abundance of mammal species and compare catch rates within two slightly different eco-regions in North and South Dakota. Seven species were captured on the Lake Traverse Indian Reservation; the most common were deer mice (Peromyscus maniculatus) and white footed deer mice (Peromyscus leucopus). On the Standing Rock Indian Reservation, we documented nine species of small mammals: deer mice and meadow voles (Microtus pennsylvanicus) were the most common. Two species of Conservation Priority (Level III) were also captured: the hispid pocket mouse (Chaetodipus hispidus) and the plains harvest mouse (Reithrodontomys montanus). Species abundance between years at each site, as well as between both sites, were not significantly different. Abstract: Antiplatelet therapy reduces the risk of major vascular events for people with occlusive vascular disease, although it might increase the risk of intracranial haemorrhage. Patients surviving the commonest subtype of intracranial haemorrhage, intracerebral haemorrhage, are at risk of both haemorrhagic and occlusive vascular events, but whether antiplatelet therapy can be used safely is unclear. We aimed to estimate the relative and absolute effects of antiplatelet therapy on recurrent intracerebral haemorrhage and whether this risk might exceed any reduction of occlusive vascular events.

Abstract: BackgroundSocial media (SM) have altered the way we live and, for many, the way we die. The information available on even the rarest conditions is vast. Free from restrictions of mobility, time and distance, SM provides a space for people to share experiences of illness, death and dying, and potentially benefit from the emotional and practical support of others n similar positions. The communications that take place in these spaces also create large amounts of ‘data’ which, for any research centre, cannot be ignored. However, for a palliative care research centre the use of this ‘data’ comes with specific ethical dilemmas.MethodsThis paper details the process that we, as a research, went through in constructing a set of ethical guidelines by which to work. This involved conducting two consensus days; one with researchers from within the centre, and one with the inclusion of external researchers with a specific interest in SM.ResultsThe primary themes that emerged from the consensus meetings includes; SM as a public or private space; the status of open and closed groups; the use of historical data; recruiting participants and obtaining informed consent and problems of anonymity associated with dissemination.ConclusionsThese are the themes that this paper will focus on prior to setting out the guidelines that we subsequently constructed.

11. Better than a pound of cure: preventing the development of multidrug-resistant tuberculosis

Abstract: Diagnostic and treatment delays contribute to increased death and disability among the 490,000 adults and children who develop multidrug-resistant (MDR) tuberculosis every year. Since the treatment of MDR tuberculosis is complex, costly and often toxic, tuberculosis control programs should prioritize strategies to prevent drug-resistant tuberculosis. Opportunities to limit transmission and prevent disease progression in close contacts of MDR tuberculosis cases are often neglected. Effective MDR tuberculosis preventive strategies could minimize the costs for patients and healthcare systems. This review characterizes the biological basis for the development of MDR tuberculosis, outlines the evidence for strategies to reduce transmission and highlights programmatic approaches to the management of patients infected with drug-resistant strains of Mycobacterium tuberculosis.

Abstract: ObjectivesEvaluations of new services for palliative care in non-cancer conditions are few. OPTCARE Neuro is a multicentre trial evaluating the effectiveness of short-term integrated palliative care (SIPC) for progressive long-term neurological conditions. Here, we present survey results describing the current levels of collaboration between neurology and palliative care services and exploring the views of professionals towards the new SIPC service.MethodsNeurology and palliative care teams from six UK trial sites (London, Nottingham, Liverpool, Cardiff, Brighton and Chertsey) were approached via email to complete an online survey. The survey was launched in July 2015 and consisted of multiple choice or open comment questions with responses collected using online forms.Results33 neurology and 26 palliative care professionals responded. Collaborations between the two specialties were reported as being ‘good/excellent’ by 36% of neurology and by 58% of palliative care professionals. However, nearly half (45%) of neurology compared with only 12% of palliative care professionals rated current levels as being ‘poor/none’. Both professional groups felt that the new SIPC service would influence future collaborations for the better. However, they identified a number of barriers for the new SIPC service such as resources and clinician awareness.ConclusionsOur results demonstrate the opportunity to increase collaboration between neurology and palliative care services for people with progressive neurological conditions, and the acceptability of SIPC as a model to support this.Trial registration numberISRCTN18337380; Pre-results.

Abstract: ObjectivesThe meaning that patients and carers attribute to their experience of palliative care is fundamental to the provision, practice and evaluation of optimal care. This analysis aims to establish contemporary key practices and priorities from the perspective of patients and carers.MethodsThematic analysis was applied to 594 anonymised free text responses from patients and carers documenting their experiences of palliative care.ResultsThe emotional experience of care is the most significant aspect documented by patients and carers. It refers to the process of care in key domains of respect, renewal, refuge and restorative care. Patients and carers described care as either enabling or directly provided. The emotional experience of care was not confined to psychosocial need and constituted a core practice drawing on professional expertise and interpersonal skills, some of which may be taken for granted by staff themselves.ConclusionsThe emotional experience of care is examined as a practice rather than a topic, acknowledging that patients and carers documented the performance of care and the resulting impact in a variety of situations. The emotional experience of care comprises key aspects of contextual care facilitating autonomy and connectedness. It is embedded in relationships mediated by communication and tenor of care. The perspective of patients and carers places the emotional experience of palliative care centrally. This has implications for future service evaluation and the incorporation of this skill-based outcome alongside more traditional outcome measures such as the amelioration of physical symptoms.

Although expert opinion favouring thrombolysis exists, this is based predominantly on studies demonstrating physiological benefits rather than a mortality benefit. The optimal treatment for massive PE remains unclear. The majority of studies to date have studied medical therapy, and case series of surgical pulmonary embolectomy have also been reported. No studies directly comparing mortality between medical and surgical therapies have been published. In our institution, both medical and surgical therapies are used in the treatment of massive PE.

Abstract: Abstract: Using various chromatographic techniques (size exclusion, anion exchange, and cation exchange) combined with several detectors (neutron activation analysis and atomic fluorescence spectrometry), an attempt was made to characterize selenium compounds in some edible, selenium-accumulating mushrooms (Albatrellus pes-caprae and Boletus edulis). The mushrooms contained mostly low-molecular-weight (6 kDa) selenium compounds. After proteolysis, only a small fraction of the extractable selenium could be identified as selenite (3.0–9.2%, Albatrellus pes-caprae), selenocystine (minor, Albatrellus pes-caprae; 7.5%, Boletus edulis), or selenomethionine (1.0%, Boletus edulis), leaving the form of the bulk still to be elucidated.

Abstract: Survival rates for cancer have improved in recent decades, specifically for younger patients. However for those over 65, who account for over half of all cancer patients, mortality rates remain poor. In those with small-cell lung cancer the achievement of cure is possible in only a minority (5–10%). For the elderly and those with extensive disease we have found that the use of oral etoposide offers similar survival rates to more intensive regimens, with minimal side effects and hospitalisation. Thus the expansion of the elderly population mandates for more specific chemotherapeutic approaches in this group.

The technique was checked against radiotracer techniques using 75Se or 81mSe, and was illustrated by results for different botanical samples and certified reference materials.

Abstract: A comparative study of the determination of iodine in mineral waters is presented. Iodine was first determined by a standard titrimetric method and the results obtained were compared with those obtained by the faster and more sensitive radiochemical neutron activation method. For a series of waters, the results obtained by the two techniques were in fair agreement for the higher concentration levels of iodine, but for very low iodine levels the titrimetric method was insufficiently sensitive. The RNAA procedure was checked by standard addition experiments, and shown to be also valid when iodine was present as iodate.

29. Dementia screening in acute medical and geriatric hospital admissions

Abstract: AIMS AND METHOD We studied a representative cohort of 161 patients over 65 years of age, admitted non-electively to medical and geriatric wards of a large teaching hospital. Assessment for dementia was made using DSM–IV criteria. Psychiatric records were then examined, masked, to determine the involvement of psychogeriatric services. RESULTS: There were 111 possible cases of dementia (69%), of which 30 (27%) had prior local psychogeriatric case notes; in 22 cases (20%) the patient had a prior psychiatric diagnosis of dementia. Of 161 patients, 19 (12%) were seen by psychogeriatric services during their admission, of whom 12 (7%) were already known to psychiatric services. Dementia was diagnosed in 17 (complicated by delirium in 2), depression in 1 and hypomania in 1. Many patients with a possible diagnosis of dementia had no psychiatric assessment. CLINICAL IMPLICATIONS Psychogeriatric assessment was performed on a minority of older people admitted to medical care. This population may include older people with undiagnosed dementia and unmet psychiatric care needs.

A small number of systematic reviews of bereavement interventions are available1 2but these have typically been restricted to evidence from randomised controlled trials (RCTs) and provide little information on support specifically provided in palliative care settings.AimThis mixed method review identifies interventions and services providing bereavement support for adults bereaved through advanced progressive illness and looks at both the quantitative and qualitative evidence for their effectiveness and utility.MethodsKey databases were searched for papers published between 1990 and 2016. Supplementary searches were also conducted. The review includes studies conducted in the UK or comparable countries reporting evaluations of interventions or services delivered to adults bereaved through advanced progressive illness. Study quality was assessed using appropriate checklists.ResultsThirty-nine studies met the inclusion criteria for the review including seven RCTs. Evidence from the trials reported improvements in a range of physical and mental health outcomes. However the studies were generally of poor quality with heterogenous populations and outcomes. Evidence from qualitative studies identified a number of positive impacts for service users including facilitating loss and grief resolution restoration and moving on acquisition of coping strategies and social support.ConclusionsWhilst the qualitative evidence suggests important benefits of bereavement care for service users this is not matched by strong trial-based evidence. This study included a systematic review of the evidence and the development of a core outcome set for bereavement research in palliative care. Public involvement was integral throughout. The collaborative relationship with the research partners (RPs) was fully planned and then documented through reflective log sheets.AimTo detail the research partners’ contribution to the study.MethodologyRPs helped refine the RQ. The study protocol set out the steps needed to ensure that:Research design and methods used were appropriate for bereaved adultsStudy documentation and outputs were accessible to ‘lay’ readersRPs ‘experience based’ expertise was embedded in ongoing project planning outputs and future research designThe experience for RPs was worthwhile leading to new skills and knowledgeReflective log sheets were used to capture how well this was achieved. These were completed independently by the researcher and RPs. Their reflections were shared at quarterly meetings.ResultsThe reflective logs confirmed that steps one to five were carried out. RP inputs achieved the planned impact (e.g. the questionnaires held hardly any missing data reflecting content and face-validity). Enhancement of the bereavement research through planned documented collaboration was thus confirmed.ConclusionWell planned and documented PI enhances the quality of health care research. Researchers felt that the solid collaboration with the research partner broadened their perspective and signalled a clear departure from a simple tick box exercise.

Patients and carers spend time documenting concerns that are often ignored or managed locally in an ad hoc manner. Currently, palliative care experiences of patients and carers in Wales are measured using a service evaluation questionnaire, comprising both closed and open-ended questions. Previous research, exploring free-text responses from this questionnaire, suggests that questionnaire refinement should accommodate service users’ expressed priorities and concerns, and highlights the need to incorporate free-text data analysis strategies during study design.MethodsResults from a previous analysis of 596 free-text responses provided the basis for an expert consensus day, where the current service evaluation questionnaire was refined. The refined version was tested during cognitive interviews with patients (n=10) and carers (n=7) receiving palliative care from 1 of 2 UK hospices. Data were analysed thematically.ResultsInterviews highlighted minor areas for change within the questionnaire and provided broader insight into patients’ experiences of palliative care services. Patients and carers place an emphasis on simplifying language, decreasing the numeric response range and reducing written instructions; relying instead on visual cues, including formatting and layout. Findings highlighted the importance patients attached to providing meaningful free-text contributions.ConclusionsQuestionnaire refinement should use the patient perspective to effectively facilitate respondent understanding, pertinence and usability. The importance of employing data analysis strategies during questionnaire design may reduce research waste, thus enabling a better interrogation of service provision.

The researchers were of varying backgrounds and all reported having experience in interviewing terminally ill patients, and all but one had experience of interviewing patients in their home environment.ResultsThe main areas discussed by researchers included: whether participation in end-of-life research unintentionally becomes a therapeutic experience or an ethical concern; power relationships between terminally ill patients and researchers; researcher reflexivity and reciprocity; researchers’ training needs. Qualitative methods can complement the home environment; however, it can raise ethical and practical challenges, which can be more acute in the case of research undertaken with palliative and patients at the end-of-life.ConclusionsThe ethical and practical challenges researchers face in this context has the potential to place both participant and researcher at risk for their physical and psychological well-being. We present a set of recommendations for researchers to consider prior to embarking on qualitative research in this context and advocate researchers in this field carefully consider the issues presented on a study-by-study basis.

34. The bereavement experiences and support needs of family carers: a supplementary qualitative analysis of free text data from the peolcpsp research priority setting exercise

Abstract: BackgroundAlthough grief is a natural process, in which most people learn to adjust without the need for formal support, the relationship between grief and poor mental and physical health is well established (Agnewet al., 2010). Research also suggests that there may be grief experiences and support needs which are specific to family members who have provided end of life care (eg Stajduhar et al., 2013).MethodsA research priority setting exercise for palliative and end of life care was conducted, involving a public survey of 1403 people. Respondents included patients, current and bereaved carers, health and social care professionals, volunteers and members of the public. To fully explore the views and experiences reported by participants a supplementary thematic analysis was carried out on the free text data. This presentation reports on themes relating to bereavement.ResultsResponses demonstrated a relationship between traumatic deaths, feelings of guilt and bereavement experiences. Respondents also highlighted the “void” caused by the sudden withdrawal of professional support to families after death. Communication and support needs which were identified by participants included more effective communication and emotional support around end of life care planning and decision making and continuity of care and follow up support post-death.ConclusionThis analysis demonstrates some of the specific effects that caring for a loved one at the end of life can have on grief experiences. Abstract: BackgroundDeveloping and implementing novel models of care requires a robust approach to identifying and assessing existing evidence. Rapid reviews have emerged as an efficient approach. There is a growing number of rapid review ‘methods’ but no accepted standardised methodology. We describe the methodological options and an example the Palliative Care Evidence Review Service (PaCERS) where streamlined systematic review methodology is used to conduct rapid reviews requested by clinicians/clinical services.AimsTo describe evidence synthesis options including PaCERS for producing rapid evidence summaries on questions of direct importance to service delivery. To identify related methodological challenges.MethodsWe examine various rapid review methods and describe in full PaCERS methodology. This uses a focussed search strategy across a limited set of databases and other sources. Study selection data extraction and critical appraisal are performed independently by at least two reviewers. Results are published online using a template co-produced with users which describes key clinical messages evidence quality and summaries of included studies.ResultsWe will describe the pros and cons of conducting rapid reviews and highlight methodology development unique to PaCERS. We will discuss specific issues encountered during the review process to produce rigorous reviews in a short time frame for palliative services.ConclusionRapid reviews have tremendous methodological variation however they are an important tool for evidence-informed decision making especially when time resource constraints and health needs are an issue. One example is the patient clinical care pathway for patients diagnosed with advanced non-small cell lung cancer (NSCLC) where there may be misunderstanding of the extent of disease prognosis and aims of treatment.AimTo understand how treatment decisions are made for NSCLC patients and map the decisions pathway.MethodologyPACT is a prospective multi-centre multi-methods five-stage qualitative study. Non-participant observation of MDT meetings (n=12) determined patients’ allocation to treatment. Non-participant observation of patient-clinician consultations (n=14) explored communication of treatment options and decision-making. Interviews with patients/companions and clinicians (n=27) explored perception of treatment options and involvement in decision-making. Mediated discourse thematic framework and narrative analysis were used to analyse the data. An expert consensus meeting (n=33) finalised the content and format of a proposed intervention.ResultsFor NSCLC patients decision making was a process rather than a single event. Patients’ priorities and preferences were not routinely or intrinsically part of the clinical pathway and arose according to the skills/attitudes of the clinician and the confidence/awareness of the patient. The interviews illustrate the uncertainty for patients around treatment considerations and highlighted the importance of knowing patients’ priorities and preferences.ConclusionCurrent pathway for NSCLC patients focuses on clinical management at the expense of patient-centred care. However clinicians’ attitude towards hope for patients with non-curative diagnosis is instrumental to any process change.

However the research evidence is limited and a lack of consistency in the outcomes used to evaluate services makes it difficult to draw conclusions on their relative effectiveness. Core Outcome Sets (COS) represent the ‘minimum that should be measured and reported’ in research into specific conditions or services making it easier to compare evaluation results [www.comet-initiative.org].AimTo develop a COS for evaluating bereavement support in palliative care for adults who have lost adults through terminal illness.MethodologyFour stages were used to identify agreement on the core outcome Set. Stage 1) a systematic review of qualitative and quantitative papers. Stage 2) a first consensus day of 21 people with expertise in bereavement support (including: adults with bereavement experience providers of bereavement support services researchers) who discussed and amended the outcomes emerging from the review of the literature. Stage 3) a two-round DELPHI survey of 238 bereavement experts. Stage 4) a second consensus day of 23 experts who discussed and reached agreement on the ranking of outcome measures emerging form the DELPHI survey.ResultsPreliminary analysis of the consensus days and survey results suggest that the most important outcomes to use are ‘ability to cope’ and ‘mental health and wellbeing’. Core dimensions to explore when assessing these outcomes have also been identified.ConclusionAvailable validated tools are being reviewed for their suitability. Gaps will be highlighted with further work planned to adapt or develop measures as necessary.

38. 27 Understanding public attitudes: a crucial component for developing palliative care services

Abstract: IntroductionA major challenge in developing new models of palliative care is to identify the current needs of the public patients and carer (Dixon 2015). There is a lack of evidence on people’s attitudes towards end of life care (EoLC) in Wales.AimTo increase understanding of people’s feelings views knowledge and preferences around EoLC.MethodAn online survey was conducted using an electronic survey tool. Descriptive statistics and thematic analysis were used to perform data analysis.Results2210 people (Age: Mean +SD; 55+14 years) participated in the survey 43% of those were members of the general public 29% were patients/carers and 23% were health professionals. 49% of respondents thought that available palliative care was inadequate and 92% believed that end of life care for dying people should have equal priority for the NHS. 95% of respondents thought that expressing preferences around EoLC in advance was important but only 13% had done this in practice and 60% did not know how to plan their EoLC. However the top three needs in EoLC were identified as having a trained carer (84%) access to other professionals (59%) and emergency care (44%). The top three preferences for EoLC were being surrounded by loved ones (62%) maintaining dignity (55%) and a feeling of peace (40%). Just 24% respondents would chose to be at home while receiving EoLC.ConclusionsUnderstanding public attitudes is essential to understanding changing contexts of care. Abstract: IntroductionWhilst the majority of seriously ill people express a wish to die at home (79%), only half of those achieve this. The likelihood of patients remaining at home often depends on availability of able and willing informal carers who take on numerous care tasks, often including the responsibility of assisting patients with oral medications. The role of carers administering subcutaneous injections is not widespread practice in the UK, but has proven to be key in achieving home death in other countries.AimTo determine if carer-administration of as-needed subcutaneous medication for four breakthrough symptoms (pain, nausea, restlessness and noisy breathing) in homebased dying patients is feasible and acceptable in the UK.MethodsOne hundred adult patients in the last weeks of life who are likely to lose the oral route and have expressed a preference to die at home will be recruited with their carers to an external randomised pilot trial across three UK sites (North Wales, South Wales and Gloucestershire). Carers randomised to the intervention arm will receive a manualised training package adapted from an existing Australian model and delivered by local district nursing teams. Dyads in the control arm will receive usual care.ResultsThe main outcomes of interest will be feasibility, acceptability, recruitment rates, attrition and selection of the most appropriate outcomes measures.ConclusionFindings will inform a definitive Phase III randomised controlled trial.The study is funded by the National Institute for Health Research (HTA project 15/10/37). Abstract: BackgroundThe iWantGreatCare (iWGC) questionnaire was developed by the Palliative Care Implementation Board and aims to evaluate palliative care service provision in Wales, through the appraisal of care, as encapsulated in 10 questions and a free text box.In order to fully reflect patients' and families' experiences of care, we previously undertook a qualitative analysis of the free text comments of 596 patients and carers, which highlighted areas not fully assessed in the scored results. Following this analysis, we are undertaking a further project to refine the questionnaire to accommodate patients' and carers' priorities and concerns.MethodsThe questionnaire was refined via an expert consensus day with healthcare professionals, researchers and a lay representative, utilising the qualitative free text analysis, and is now being tested using face–to-face, semi-structured, cognitive interviews with patients (n=10) and carers (n=10), who use palliative care services at two hospices in South Wales. The questionnaire comprehensively encompasses the elements of palliative care previously identified as important to patients and carers. Interviews aim to test whether the questions are understandable and pertinent to those completing the questionnaire. Interview transcripts from recorded data are to be analysed using a six phase thematic analysis.ResultsIt is expected that the interviews will inform the final questionnaire revision prior to being presented to the Palliative Care Implementation Board for their consideration. The results of these cognitive interviews and the refined questionnaire will be outlined and discussed in this presentation.

42. P165 Engaging minority ethnic communities: an evaluation of a Marie Curie Hospice project aiming to improve access to palliative care services

Abstract: BackgroundThere is increasing emphasis on ensuring that access to healthcare is equitable in the UK, yet on-going evidence demonstrates that minority ethnic communities are underrepresented in accessing palliative care. A Marie Curie Hospice in Wales initiated a funded outreach project with a keyworker post to engage minority ethnic communities, aiming to improve access to palliative care services.MethodsThe study aim is to evaluate the outreach project and keyworker post. A longitudinal evaluation of the initiative is being undertaken, encompassing interviews with hospice and community healthcare professionals, minority ethnic communities and the project keyworker. Key project documents, such as reports, and hospice ethnicity data are also being analysed. This presentation reports on data from semi-structured interviews with hospice staff and keyworker, and analysis of project reports. Relevant ethical and governance approvals were obtained.ResultsHospice healthcare professionals highlighted challenges when providing palliative care to individuals from minority ethnic communities, particularly language barriers. Minority ethnic communities in South Wales appeared uninformed about local palliative care services and the keyworker implemented a range of initiatives to raise awareness and widen hospice referral routes. Additionally, the keyworker supported healthcare professionals when caring for people from minority ethnic communities, for example meeting religious needs at the end-of-life.Looking ahead, hospice staff are keen to increase their understanding of different ethnic groups and cultures, while the keyworker hopes to work with community healthcare teams to improve referral rates to the hospice.Conclusions and applicationThe evaluation has demonstrated the progress made by the keyworker to engage minority ethnic communities and support hospice staff to holistically care for people from different ethnic groups.The keyworker role appears, at this early stage in the project, to be an excellent way of engaging minority ethnic communities and improving access to palliative care services.

43. ‘Go home with your family and a lot of kit, and we’ll leave you to it’: perceptions of out of hours care by the PeolcPSP respondents

Abstract: IntroductionOut of hours care was the top concern of the 1403 respondents to the PeolcPSP. The respondents of the PeolcPSP were asked to answer big questions. In return, they gave big answers often with highly personal narrative accounts in illustration of their concerns.Aim(s) and method(s)We undertook a supplementary thematic analysis as a valuable opportunity to explore the uncertainties, questions and experiences of respondents to supplement the interventional questions taken forward by the PeolcPSP.ResultsSurvey respondents talked about the need for 24 hour and out of hours care. Most of these responses consider access to good quality home care support and services to be a key priority. In particular, respondents struggled to understand why services were restricted outside normal working hours and expressed anger and frustration. Emergency admissions for crisis care, as the only option, were discussed frequently. 44. Palliative care evidence review service (PaCERS): a rapid systematic approach to identifying high quality evidence on palliative care

Abstract: IntroductionThe importance of relating clinical practice more closely to evidence, is recognised by health and social care professionals and commissioners alike. However a significant obstacle is the lack of time to locate and read the relevant literature. Additionally, the expectation to assess quality and deal with conflicting information can be challenging. Therefore, we have initiated PaCERS to support professionals and other decision makers working in palliative care.AimThe aim of this service is to conduct rapid evidence reviews on questions of current and direct importance to clinical care or service delivery, producing critically appraised summaries of available evidence, in the shortest possible timeframe. Responding directly to external clinical/organisational calls for evidence, the service will also facilitate at-pace integration of research findings into service development, and will help to embed palliative care research into clinical practice.MethodThe service will enable palliative care professionals to gain an overview of literature relevant to the topic of immediate interest in order to inform future practice/service design, confirm that current practice reflects contemporary research evidence and identify areas that require further research. A web-based repository of all reviews produced through PaCERS will be made available to palliative care decision makers.ResultsWe have held a highly productive workshop with clinical colleagues to refine our processes and achieve consensus on how best PaCERS can serve the palliative care community. One full review, which identifies best practice models for end of life and palliative care in rural areas, has been completed.ConclusionThis service will impact directly on palliative care clinicians and other decision makers, and indirectly on patients/carers in receipt of palliative care. This research evidence review service can provide guidance for a variety of palliative care issues including round the clock palliative care.

45. Supporting people bereaved through advanced illness: a systematic review of the evidence

Abstract: BackgroundPalliative care has an important role to play in addressing the bereavement needs of families, although a lack of evidence-based guidance has been noted (Hudson et al., 2012; PeolcPSP 2015). A small number of systematic reviews of bereavement interventions are available, but these have typically been restricted to evidence from randomised controlled trials and provide little information on interventions and support specifically provided for adult populations bereaved through advanced, progressive illnesses (e.g. Forteet al., 2004; Currier et al., 2008).AimTo systematically review the evidence on bereavement support for adults bereaved through advanced, progressive illness.MethodsKey databases have been searched using indexed terms and key words for papers published in English between 1990 and 2016. Supplementary searches will also be conducted. The review will include studies conducted in the UK or comparable countries reporting evaluations of interventions or services, delivered to adults bereaved through advanced, progressive illness. Study quality will be assessed using appropriate checklists. This mixed methods review will identify interventions and services and look at both the quantitative and qualitative evidence for their effectiveness and utility.Results/conclusionsThis review, funded by Marie Curie, will help to identify what type of bereavement support works best for different groups of people, and how such support might be improved. It is also being used to inform the development of a Core Outcome Set for bereavement research in palliative care. 46. Supporting adults bereaved through advanced illness: development of a core outcome set (COS)

Abstract: BackgroundA lack of evidence-based guidance on bereavement support (Hudson et al., 2012) and of consistency in the measures used in clinical practice and research to assess bereavement outcomes (Forte et al., 2004) have been identified. The heterogeneity in outcome measures makes it difficult to assess and benchmark clinical services. It is also challenging to combine and compare research results, and draw robust conclusions on effectiveness to inform clinical practice and service delivery. One step to resolving this situation is to develop a core outcome set (COS). This represents the ‘minimum that should be measured and reported in all clinical trials of a specific condition’ (www.comet-initiative.org).AimThis project, funded by Marie Curie, aims to develop a COS that is specific to bereavement in adults following advanced, progressive illness.MethodsThe COS will be developed through a series of distinct methodologies. First, outcomes utilised in the studies identified by systematic review will be mapped into different domains. Second, an expert consensus day will be convened with up to 30 stakeholders to identify any further outcomes and engage in a consensus methodology (Gallagher et al., 1993) designed to identify the most important. 47. P8 Care across settings: Evaluating an integrated specialist nurse-led palliative care clinic within a general practice setting

Abstract: BackgroundA community nurse-led palliative care clinic was introduced at a UK General Practitioner’s (GP) practice in April 2012. The clinic aimed to provide earlier identification of patients with cancer and non-malignant disease, initial patient assessment, education for GPs and other community professionals, and strengthen relationships with the local multi-disciplinary team. This provides a new model for engaging patients and ensuring that specialist palliative care is delivered across different clinical settings. Usually, clinical nurse specialists (CNS) support patients in the home. This new initiative brings a fortnightly clinic as a complementary resource to consolidate the joint primary/ palliative care approach.MethodsThis study evaluates implementation processes and the impact of the clinic on patient experiences and professional practices. It assesses uptake of palliative services and the management and delivery of palliative care by GPs and CNS.Baseline interviews with GPs (n = 5) and CNS (n = 4) were carried out in February 2012. Semi-structured interviews with patients (n = 6), GPs (n = 6) and CNS (n = 4) are being undertaken to assess perceptions of the clinic.Patient interviews will be analysed using an Interpretive Phenomenological Analysis (IPA) approach. Interviews with GPs and CNS will be analysed using Framework Analysis.ResultsPerceived aspirations for the clinic included: earlier referral; improved communication and continuity of care; psychological and practical benefits for patients/ carers; more targeted use of specialist nursing resources. Baseline interview data highlights the need for improved referral and communication practices, especially for non-cancer patients. Data from patient and follow-up interviews will be reported against these aspirations, with additional themes identified.Conclusions and applicationConclusions will be drawn on the effectiveness of the clinic and implications identified for palliative care provision in primary care settings, including the potential to roll out a similar model more widely.
