2. Trunk Control Test as a Main Predictor of the Modified Barthel Index Score at Discharge From Intensive Post-acute Stroke Rehabilitation: Results From a Multicenter Italian Study.

- Abstract
• The control of the trunk is an essential predictor for the recovery of basic ADLs, as measured by the mBI. • Reduced trunk control, pressure ulcers, low functional and communication abilities at admission to post-acute inpatient rehabilitation, as well as premorbid disability can independently affect the global function level at discharge. • Strategies to accelerate TCT recovery during the acute stay and post-stroke inpatient rehabilitation may have a positive effect on global functional recovery at discharge. To verify whether trunk control test (TCT) upon admission to intensive inpatient post-stroke rehabilitation, combined with other confounding variables, is independently associated with discharge mBI. Multicentric retrospective observational cohort study. Two Italian inpatient rehabilitation units. A total of 220 post-stroke adult patients, within 30 days from the acute event, were consecutively enrolled. Not applicable. The outcome measure considered was the modified Barthel Index (mBI), one of the most widely recommended tools for assessing stroke rehabilitation functional outcomes. All variables collected at admission and significantly associated with mBI at discharge in the univariate analysis (TCT, mBI at admission, pre-stroke modified Rankin Scale [mRS], sex, age, communication ability, time from the event, Cumulative Illness Rating Scale, bladder catheter, and pressure ulcers) entered the multivariate analysis. TCT, mBI at admission, premorbid disability (mRS), communication ability and pressure ulcers (P <.001) independently predicted discharge mBI (adjusted R
2 =68.5%). Concerning the role of TCT, the model with all covariates and without TCT presented an R2 of 65.1%. On the other side, the model with the TCT only presented an R2 of 53.1%. Finally, with the inclusion of both TCT and all covariates, the model showed an R2 increase up to 68.5%. TCT, with other features suggesting functional/clinical complexity, collected upon admission to post-acute intensive inpatient stroke rehabilitation, independently predicted discharge mBI. 4. A Tool to Optimize the Participation of BESS to the UK Ancillary Services Market

- Abstract
In the last decade, several applications and studies have demonstrated that, thanks to their fast response time, Battery Energy Storage Systems (BESSs) are a promising technology to incorporate operational flexibility in grids and plants, opening to the possibility of providing a set of different grid services. In this context, this paper defines an optimization model to encode UK market rules and output the optimal set of offers on day-ahead and intraday markets, dynamic frequency response services and imbalance settlement, with a specific focus on a BESS. The proposed mathematical model includes BESS technical constraints and market temporal sequencing, also modelling the State Of Energy Management. Limitations on the number of cycles per day and optimization of the battery use are implemented. A Mixed Integer Linear Programming problem is defined to reduce the computational burden and to integrate the model in the Energy Management System (EMS) developed by the University of Genoa. Specific tests have been performed, allowing to compare scenarios in which ancillary services are enabled or not. According to the results, Dynamic Frequency Response services are the most profitable market. The integration in the EMS of the constraints related to the encoded market sections allows to exploit this tool to evaluate the profitability of investments involving a BESS over long time horizon.

5. Management and breeding for rust resistance in legumes

- Abstract
Rust diseases are a major concern in legume production worldwide causing heavy losses especially in developing countries that depend on grain legumes as staple food. Fungal species from Uromyces, Phakopsoraand Pucciniagenera are the main causal agents of the various legume rust diseases. They induce up to 100% yield losses on susceptible cultivars and are emerging as a substantial threat to global food security. Developing durable resistance to rust has thus become a critical breeding objective alongside efforts to improve cultural and disease management practices. This review specifically focuses on the recent advances in understanding and enhancing genetic rust resistance across diverse legume crops. Key topics covered include: (i) the diversity and host range of the rust species affecting legumes; (ii) the disease management strategies from cultural practices to chemical control; (iii) the available screening methods for identifying new sources of resistance; (iv) the genetic basis of resistance, encompassing both major resistance genes and quantitative trait loci; (v) insights into gene regulation and effector molecules leading to legume-rust interactions; and (vi) emerging genomic-assisted breeding techniques that can accelerate the development of durable rust resistance in legumes. Overall, this review highlights the progress made to date and the remaining challenges in sustainably managing rust diseases across diverse legume crops through integrated approaches spanning pathogen biology, advanced phenotyping, genetic resistance, and molecular breeding.

6. Plastic pollution in a rapidly developing nation: A comprehensive assessment of litter and marine debris surrounding coastal Cambodia.

Cambodia is a rapidly developing country in south-east Asia, a region forecast as an outsized source of plastic pollution into the world's oceans. However, to date there has been no large-scale assessment of plastic pollution in the environment within the country. We conducted a comprehensive assessment of plastic items and hotspots across 243 coastal, river and inland sites along the entire coastline of Cambodia, recording 46,927 items in transects throughout the study area. Plastic densities along coastlines were more than ten times those on riverbanks and more than twenty times that of inland areas. Averaging 70 plastic items per metre of coastline, Cambodia has among the highest average loads of whole plastic items recorded on coastlines globally. Single-use plastic bottles were the most abundant whole item recorded, overrepresented by drinking water bottles. Polystyrene foam was the most common fragmented item. We provide specific recommendations to reduce plastic in Cambodia's environment. • First comprehensive assessment of litter in Cambodia's coastal environment • Scientific surveys following a stratified, representative, randomised design • Among the highest average loads of whole plastics recorded on world's coastlines • Water bottles dominated whole litter, an unusual finding in the Asia region. • Apart from water bottles, plastic pollution was similar to neighbouring countries. 8. Identification of positive modulators of TRPM5 channel from a high-throughput screen using a fluorescent membrane potential assay

- Abstract
Transient Receptor Potential Melastatin 5 (TRPM5) is an intracellular calcium-activated cation-selective ion channel expressed in a variety of cell types. Dysfunction of this channel has recently been implied in a range of disease states including diabetes, enteric infections, inflammatory responses, parasitic infection and other pathologies. However, to date, agonists and positive modulators of this channel with sufficient selectivity to enable target validation studies have not been described, limiting the evaluation of TRPM5 biology and its potential as a drug target. We developed a high-throughput assay using a fluorescent membrane potential dye and a medium- and high-throughput electrophysiology assay using QPatch HTX and SyncroPatch 384PE. By employing these assays, we conducted a primary screening campaign and identified hit compounds as TRPM5 channel positive modulators. An initial selectivity profile confirmed hit selectivity to TRPM5 and is presented here. These small molecule TRPM5 compounds have a high potential both as early tool compounds to enable pharmacological studies of TRPM5 and as starting points for the development of potent, selective TRPM5 openers or positive modulators as novel drugs targeting several pathological states.

Water supply represents a constant worldwide challenge for people and authorities. This issue is significantly severe in semiarid regions such as Gaza Strip, where groundwater constitutes the only fresh water source. Furthermore, Gaza Strip area suffers from water scarcity due to the decrease in water recharge and a constantly groundwater over-pumping, along with seawater intrusion, which cause serious problems in fresh water supply. Treated wastewater reuse represents a sustainable approach to increase water resource availability, to alleviate stressed polluted Gaza's coastal aquifer and to contribute to local agriculture development. On this issue, a pilot-scale plant has been designed to evaluate the feasibility of a municipal wastewater recovery for agricultural purposes in Rafah (Gaza Strip), reproducing the working conditions and performances of a real plant, which will be built by summer 2020. Transient receptor potential cation channel subfamily M member 5 (TRPM5) is a nonselective monovalent cation channel activated by intracellular Ca2+increase. Within the gastrointestinal system, TRPM5 is expressed in the stoma, small intestine, and colon. In the search for a selective agonist of TRPM5 possessing in vivo gastrointestinal prokinetic activity, a high-throughput screening was performed and compound 1was identified as a promising hit. Hit validation and hit to lead activities led to the discovery of a series of benzo[d]isothiazole derivatives. Among these, compounds 61and 64showed nanomolar activity and excellent selectivity (>100-fold) versus related cation channels. The in vivo drug metabolism and pharmacokinetic profile of compound 64was found to be ideal for a compound acting locally at the intestinal level, with minimal absorption into systemic circulation. Compound 64was tested in vivo in a mouse motility assay at 100 mg/kg, and demonstrated increased prokinetic activity.

13. Escherichia coliStrains Isolated from Retail Meat Products: Evaluation of Biofilm Formation Ability, Antibiotic Resistance, and Phylogenetic Group Analysis

Escherichia coliis a ubiquitous organism capable of forming a biofilm. This is an important virulence factor and is critical in certain diseases and in the development of antibiotic resistance, which is increased by biofilm synthesis. In the present study, the potential health risk associated with handling and consumption of foods of animal origin contaminated with E. coli–producing biofilm was evaluated. We analyzed the ability of 182 E. colistrains isolated from pork, poultry, and beef, purchased in three different supermarkets in the area of the “Italian Food Valley” (Parma, northern Italy), to form biofilms. Positive strains were also tested for the presence of 12 biofilm-associated genes. Moreover, the 182 E. coliwere characterized for antibiotic resistance, presence of multidrug resistance, extended-spectrum β-lactamase strains, and phylogenetic diversity through PCR. Twenty-five percent of the isolates produced biofilm. The majority showed weak adherence, five were moderate, and three were strong producers. E. coliwith a strong adherence capability (three of three) harbored eight biofilm-associated genes, while weak and moderate producers harbored only five (frequencies ranging from 80 to 100%). Multidrug resistance was observed in 20 biofilm-producing E. coli,and 15 of these belonged to phylogenetic group D. Among nonbiofilm producers, the percentage of strains belonging to phylogenetic groups B2 and D was approximately 40%, highlighting a potential health risk for consumers and people handling contaminated products. The present study underlines the importance of monitoring the prevalence and characteristics of E. colicontaminating retail meat in relation to the potential virulence highlighted here.

14. Occurrence and Antimicrobial Profile of Bacterial Pathogens in Former Foodstuff Meat Products Used for Pet Diets

- Abstract
European legislation stipulates that food no longer intended for human consumption, due to commercial reasons, manufacturing problems, or some defect, can be used in pet feed. However, the presence of former foodstuffs in pet diets could constitute a public health issue because pets can act as reservoirs of antimicrobial resistance genes. In this study, for the first time, biological hazards due to the presence of Escherichia coliand Salmonellain former foodstuff meat products were evaluated. Among the 112 samples of packaged fresh meat (poultry, pork, and beef) collected from cold storage warehouses of a mass market retailer, the overall prevalence of E. coliand Salmonella,the prevalence of strains with multidrug resistance, the phenotypic and genotypic characteristics of strains that produce extended-spectrum β-lactamase, and the presence of biofilm producers were assessed. A high prevalence of E. coliwas observed in former foodstuffs of poultry (100%), pork (100%), and beef (93.3%). SalmonellaDerby and SalmonellaTyphimurium were found in 11.5% of the poultry samples, and SalmonellaTyphimurium was found in 13.3% of the pork samples. Salmonellawas not recovered from the beef samples. Multidrug resistance was found in E. coliand Salmonellaisolates from poultry, pork, and beef. Overall, 18.2% of E. coliisolates and 20% of Salmonellaisolates were resistant to various types of antibiotics with various mechanisms of action. Salmonellaisolates from pork are an important source of extended-spectrum β-lactamase production. Both E. coli and Salmonellawere carriers of antibiotic resistance marker genes (blaTEM, blaSHV, and blaCTX-M) associated with β-lactamase production in poultry and pork. Approximately 30% of the E. coliisolates from the various types of meat were phenotypically biofilm producers, and all carried the fimHgene. These findings indicate that the use of former foodstuff meat products in pet diets can represent a risk for public health.

17. Human macrophage differentiation induces OCTN2–mediated L-carnitine transport through stimulation of mTOR–STAT3 axis

Monocyte-to-macrophages differentiation associates with upregulation of carnitine transport due to mTOR-dependent activation of STAT3. l-Carnitine, in addition to playing a fundamental role in the ß-oxidation of fatty acids, has been recently identified as a modulator of immune function, although the mechanisms that underlie this role remain to be clarified. In this study, we addressed the modulation of l-carnitine transport and expression of related transporters during differentiation of human monocytes to macrophages. Whereas monocytes display a modest uptake of l-carnitine, GM-CSF–induced differentiation massively increased the saturable Na+-dependent uptake of l-carnitine. Kinetic and inhibition analyses demonstrate that in macrophage l-carnitine transport is mediated by a high-affinity component (Km~4 µM) that is identifiable with the operation of OCTN2 transporter and a low-affinity component (Km> 10 mM) that is identifiable with system A for neutral amino acids. Consistently, both SLC22A5/OCTN2 and SLC38A2/SNAT2 are induced during the differentiation of monocytes to macrophages at gene and protein levels. Elucidation of GM-CSF signaling demonstrates that the cytokine causes the activation of mTOR kinase, leading to the phosphorylation and activation of STAT3, which, in turn, is responsible for OCTN2 transcription. SLC22A5/OCTN2 therefore emerges as a novel member of the set of genes markers of macrophage differentiation.

18. Weed response and crop growth in winter wheat–lucerne intercropping: a comparison of conventional and reduced soil-tillage conditions in northern France

- Abstract
Changing agricultural practices from conventional to conservation tillage generally leads to increased weed populations and herbicide use. To gain information about the possible use of lucerne (Medicago sativa L.) cover crop as an alternative and sustainable weed-control strategy for winter wheat (Triticum aestivum L.), an experiment was performed at Thiverval-Grignon, France, from 2008 to 2010. We compared conventional and reduced tillage as well as the presence and absence of living mulch (i.e. lucerne) on weeds and wheat production. Percentage soil coverage and aboveground biomass of wheat, lucerne and weeds were measured at the end of grain filling. Weed communities were analysed in terms of composition and diversity. During both seasons, wheat biomass did not significantly decrease in reduced-till trials compared with conventional ones (7.0 and 7.2tha-1, respectively, in 2008–09; 6.9 and 7.1tha-1 in 2009–10). Regardless of soil management, the percentage soil coverage by wheat significantly decreased when it was intercropped, although wheat biomass was not significantly reduced compared with the sole crop. To minimise cash-crop losses, we studied the competition between wheat, lucerne and weeds, testing various herbicide strategies. Early control of lucerne allowed better balance between weed control and wheat development. In addition, weed communities varied among treatments in terms of abundance and composition, being reduced but more varied in plots associated with lucerne. A functional group analysis showed that grasses benefited from reduced-till conditions, whereas problematic weeds such as annuals with creeping and climbing morphologies were substantially reduced. In addition, annual and perennial broad-leaf species with rosette morphology were also significantly decreased when lucerne was used as living mulch. Wheat production in reduced-till conditions intercropped with lucerne living mulch may be useful for integrated weed management, reducing the need for herbicides.

- Abstract
The importance of glutamine (Gln) metabolism in multiple myeloma (MM) cells and its potential role as a therapeutic target are still unknown, although it has been reported that human myeloma cell lines (HMCLs) are highly sensitive to Gln depletion. In this study, we found that both HMCLs and primary bone marrow (BM) CD138+ cells produced large amounts of ammonium in the presence of Gln. MM patients have lower BM plasma Gln with higher ammonium and glutamate than patients with indolent monoclonal gammopathies. Interestingly, HMCLs expressed glutaminase (GLS1) and were sensitive to its inhibition, whereas they exhibited negligible expression of glutamine synthetase (GS). High GLS1 and low GS expression were also observed in primary CD138+ cells. Gln-free incubation or treatment with the glutaminolytic enzyme l-asparaginase depleted the cell contents of Gln, glutamate, and the anaplerotic substrate 2-oxoglutarate, inhibiting MM cell growth. Consistent with the dependence of MM cells on extracellular Gln, a gene expression profile analysis, on both proprietary and published datasets, showed an increased expression of the Gln transporters SNAT1, ASCT2, and LAT1 by CD138+ cells across the progression of monoclonal gammopathies. Among these transporters, only ASCT2 inhibition in HMCLs caused a marked decrease in Gln uptake and a significant fall in cell growth. Consistently, stable ASCT2 downregulation by a lentiviral approach inhibited HMCL growth in vitro and in a murine model. In conclusion, MM cells strictly depend on extracellular Gln and show features of Gln addiction. Therefore, the inhibition of Gln uptake is a new attractive therapeutic strategy for MM.

The importance of glutamine (Gln) metabolism in multiple myeloma (MM) cells and its potential role as a therapeutic target are still unknown, although it has been reported that human myeloma cell lines (HMCLs) are highly sensitive to Gln depletion. In this study, we found that both HMCLs and primary bone marrow (BM) CD138+cells produced large amounts of ammonium in the presence of Gln. MM patients have lower BM plasma Gln with higher ammonium and glutamate than patients with indolent monoclonal gammopathies. Interestingly, HMCLs expressed glutaminase (GLS1) and were sensitive to its inhibition, whereas they exhibited negligible expression of glutamine synthetase (GS). High GLS1 and low GS expression were also observed in primary CD138+cells. Gln-free incubation or treatment with the glutaminolytic enzyme l-asparaginase depleted the cell contents of Gln, glutamate, and the anaplerotic substrate 2-oxoglutarate, inhibiting MM cell growth. Consistent with the dependence of MM cells on extracellular Gln, a gene expression profile analysis, on both proprietary and published datasets, showed an increased expression of the Gln transporters SNAT1, ASCT2, and LAT1 by CD138+cells across the progression of monoclonal gammopathies. Among these transporters, only ASCT2 inhibition in HMCLs caused a marked decrease in Gln uptake and a significant fall in cell growth. Consistently, stable ASCT2 downregulation by a lentiviral approach inhibited HMCL growth in vitro and in a murine model. In conclusion, MM cells strictly depend on extracellular Gln and show features of Gln addiction. Therefore, the inhibition of Gln uptake is a new attractive therapeutic strategy for MM.

Ring-fused azacyclic compounds are important building units in the synthesis of biorelevant natural products, pharmaceutical agents, and molecular materials. Herein, we present a new approach to these condensed azacycles by a biomimetic cascade cyclization of arylalkenyl dioxazolones. This cascade reaction was found to proceed with excellent stereoselectivity and a high functional group tolerance. The substrate scope of arylalkenyl dioxazolones turned out to be highly flexible and extendable to additional terminating subunits, such as heteroaryl and alkynyl moieties. This biomimetic cyclization was elucidated to be initiated by an intramolecular transfer of the in situgenerated electrophilic Ir-acylnitrenoid to the tethered olefinic double bond, leading to a key N-acylaziridine intermediate, which is in turn reacted with pendant (hetero)arenes or alkynes in a highly regio- and stereoselective manner to produce ring-fused azacyclic compounds.

Abstract: Rust is a serious disease of pea whose casual agent is not always understood. In this paper we studied reaction of pea accessions to seven rust species infecting closely related legumes, finding that indeed pea can be infected mainly by Uromyces pisi, followed by Uromyces viciae-fabae. Other rust species like Uromyces striatus, Uromyces ciceris-arietini, Uromyces anthyllidis and Uromyces vignae can also infect and reproduce on pea, although in a minor extent. All U. pisi isolates tested were very virulent on pea accessions, but isolates UpPt-03 and UpKeS-05 (from Palmar de Troya, Spain and Kafr-El-Sheik, Egypt respectively) were significantly most infective. In addition to this we studied in detail the host range of U. pisi by inoculating multiple accessions of various legumes with urediospores of seven isolates of U. pisi from different geographical origins. Both experiments were performed under controlled conditions. Based on the evidence presented here the host range of U. pisi is greater than previously recorded, including genotypes belonging to Cicer arietinum, Vicia articulada, Vicia ervilia and Vicia faba, which were not mentioned before. Abstract: Pea rust is reported to be incited by both Uromyces viciae-fabae and U. pisi. However we found that in Córdoba (Spain), pea was more severely infected by U. pisi. We observed that although pea seedlings were slightly susceptible to U. viciae-fabae infection under controlled conditions, pea accessions were little damaged by this pathogen under field conditions, due to high levels of pre-haustorial resistance. The observations with Spanish isolates were confirmed with isolates of U. viciae-fabae from the Netherlands, Syria and Australia. In general, pea accessions displayed a compatible reaction against all the isolates of U. viciae-fabae studied, but levels of infection were very low on peas, markedly reduced compared to the faba bean check. For these reasons, we consider the pathogen U. pisi as the principal agent causing pea rust. However, we cannot discard the possibility of specialization within U. viciae-fabae on pea, as specialization on vetch and lentil has already been documented. [Copyright &y& Elsevier]

29. Oxidative Stress Induced by Copper and Iron Complexes with 8-Hydroxyquinoline Derivatives Causes Paraptotic Death of HeLa Cancer Cells

- Abstract
Here, we report the antiproliferative/cytotoxic properties of 8-hydroxyquinoline (8-HQ) derivatives on HeLa cells in the presence of transition metal ions (Cu2+, Fe3+, Co2+, Ni2+). Two series of ligands were tested, the arylvinylquinolinic L1–L8and the arylethylenequinolinic L9–L16, which can all interact with metal ions by virtue of the N,O donor set of 8-HQ; however, only L9–L16are flexible enough to bind the metal in a multidentate fashion, thus exploiting the additional donor functions. L1–L16were tested for their cytotoxicity on HeLa cancer cells, both in the absence and in the presence of copper. Among them, the symmetric L14exhibits the highest differential activity between the ligand alone (IC50= 23.7 μM) and its copper complex (IC50= 1.8 μM). This latter, besides causing a significant reduction of cell viability, is associated with a considerable accumulation of the metal inside the cells. Metal accumulation is also observed when the cells are incubated with L14complexed with other late transition metal ions (Fe3+, Co2+, Ni2+), although the biological response of HeLa cells is different. In fact, while Ni/L14and Co/L14exert a cytostatic effect, both Cu/L14and Fe/L14trigger a caspase-independent paraptotic process, which results from the induction of a severe oxidative stress and the unfolded protein response.

Macrophage differentiation increases SLC7A7 mRNA, y+LAT1 protein and arginine transport activity through system y+L. l‐arginine metabolism in myeloid cells plays a central role in the processes of macrophage activation and in the regulation of immune responses. In this study, we investigated arginine transport activity and the expression of the related transporter genes during the differentiation of monocytes to macrophages. We show here that the induction of THP‐1 monocyte differentiation by PMA markedly increases the expression of SLC7A7 mRNA and of y+LAT1 protein and consequently, the activity of system y+L‐mediated arginine transport. Conversely, the activity of system y+decreases during macrophage differentiation as a result of a reduction in CAT1 protein expression. The PMA‐induced, macrophage‐differentiated phenotype and the increased activity of system y+L through the induction of SLC7A7 gene are mediated by the specific activation of PKCβ. SLC7A7 gene silencing causes a significant reduction of system y+L activity and a subsequent, marked increase of arginine and lysine cell content, thus suggesting that in macrophagic cells, system y+L activity is mainly directed outwardly. Differentiating agents other than PMA, i.e., VD3 and ATRA, are equally effective in the stimulation of system y+L transport activity through the increased expression of SLC7A7 mRNA and y+LAT1 protein. Moreover, we found that also during differentiation of human monocytes from peripheral blood SLC7A7 mRNA and system y+L activity are increased. These findings point to SLC7A7 gene as a marker of macrophage differentiation.

- Abstract
Thanks to its robustness and reduced sensitivity to vibrations and air turbulence, spatial phase-shift interferometry (SPSI) is a measuring technique of particular value in industrial environments. Making use of a commercial CCD camera connected with a PC we have set up an essential system that acquires and processes the fringe pattern, extracting the relevant features of the phenomenon being observed. The basic algorithms for phase recovery are available from the literature. Here we present a variant of one of such algorithms and describe in detail its implementation in our SPSI system. Experimental results are presented, showing the effectiveness of the overall measuring chain.

Abstract Objective: Treatment with interferon (IFN) of patients affected by chronic hepatitis C (CH-C) may produce alterations in thyroid function, such as hypothyroidism, Graves-like hyperthyroidism and destructive thyrotoxicosis (DT). IFN-induced DT is characterized by suppressed serum TSH levels, normal or elevated FT4 and FT3 concentrations, with the presence or absence of thyroid peroxidase antibodies and antithyroglobulin antibodies, the absence of thyroid receptor antibodies and radioactive iodine uptake suppressed or <5%. Design: IFN-induced DT is a mild clinical disease, because thyroid-destructive processes last for a short time and involve a small portion of the gland. At present, the therapeutic approach in DT suggests IFN withdrawal and 12 months of methylprednisolone treatment. Methods: In consideration of possible untoward side effects of steroid treatment in patients with CH-C, we studied two groups of patients with CH-C who developed DT after treatments with various preparations of recombinant IFN (with or without ribavirin). Patients sequentially entered the study during a 4-year period, at the time of DT diagnosis, when IFN therapy was discontinued. The first 12 subjects (group A) were treated with 816 mg/day methylprednisolone for 3040 days after IFN withdrawal; in the following 15 patients (group B), IFN withdrawal was not followed by any additional treatment. All patients underwent clinical and laboratory controls of thyroid function at 1, 2, 3 and 6 months after DT diagnosis. Results: The results showed restoration of euthyroidism in both group A and group B patients at 6 months after DT diagnosis, regardless of steroid treatment. Conclusions: The simple withdrawal of IFN therapy in patients with CH-C, who had developed DT, appears to be effective in the treatment of the thyroid disease. This study was initiated to evaluate the role of hyperinsulinemia in the regulation of fasting plasma leptin. We measured plasma leptin and insulin concentrations in 404 healthy nondiabetic subjects. For analytical purposes, the population was divided into quartiles on the basis of the lowest (quartile 1) and highest (quartile 4) plasma insulin response to oral glucose, and fasting plasma leptin values in these 2 dichotomous groups were compared. The total plasma integrated insulin response was 4-fold greater in quartile 4, associated with significantly higher (P < .001) fasting plasma leptin (12.60 +/- 0.85 v 8.53 +/- 0.56 ng/mL). Fasting plasma leptin concentrations remained significantly higher in the hyperinsulinemic quartile when comparisons were made after subdividing the population on the basis of gender, body mass index (BMI), or waist to hip ratio (WHR). These results demonstrate that fasting plasma leptin concentrations are significantly higher in hyperinsulinemic individuals, and this difference is independent of either overall or central obesity.

Fine-needle aspiration biopsy (FNAB) is a well-documented procedure for the diagnosis and biologic characterization of breast carcinoma. In order to compare the immunocytochemical expression of biologic parameters on cytology and on histology, estrogen receptor (ER) and progesterone receptor (PgR) status, p53 protein expression, and Ki67 growth fraction were evaluated on presurgical fine-needle aspirates (FNAs) from breast carcinoma patients and on the corresponding surgical samples prior to any systemic therapy. FNAs were performed on 104 patients with primary breast carcinoma at the time of diagnosis and subjected to immunocytochemical evaluation of ER, PgR, p53, and Ki67. The same parameters were immunohistochemically evaluated on the corresponding paraffin embedded sections. ER, PgR, p53, and Ki67 were evaluable on FNAs and on paired tissue sections in 100, 97, 68, and 84 cases, respectively. Concordance between cytology and histology was 89% for ER, 78% for PgR, 79% for p53, and 70% for Ki67. The concordance between the results of immunocytochemical evaluation of ER, PgR, p53, and Ki67, on both cytology and histology, underscores the reliability of the biologic characterization of breast carcinoma by FNAB. This approach could be particularly useful in predicting prognosis and response to treatment in patients who are candidates for neoadjuvant chemotherapy and/or endocrine therapy. Summary: The authors studied the modification of systolic time intervals (STI), pre-ejection period (PEP) and left ventricular ejection time (LVET
c ), before and after isometric exercise, in 294 diabetic patients without clinical evidence of cardiomyopathy and in good metabolic control compared to 132 normal subjects. The study was aimed at detecting preclinical alterations of left ventricular function. Diabetic patients considered together did not show any difference in STI in basal conditions or after isometric exercise compared to normal subjects. When diabetic patients were divided into sub-groups according to their treatment, the insulin-treated diabetics showed modification of STI after isometric exercise, which indicated an alteration of left ventricular function. Also subjects treated with oral hypoglycemic agents showed similar but less evident changes. In diabetic patients on diet only and in those with duration of diabetes of 6 months or less, STI was identical to that of normal subjects. These data do not explain the pathogenesis of myocardial involvement, although they are in accordance with studies which have laid emphasis on the alteration of compliance of the diabetic heart.- Published
