Background Neoplasms are common across the animal kingdom and seem to be a feature plesiomorphic for metazoans, related with an increase in somatic complexity. The fossil record of cancer complements our knowledge of the origin of neoplasms and vulnerability of various vertebrate taxa. Here, we document the first undoubted record of primary malignant bone tumour in a Mesozoic non-amniote. The diagnosed osteosarcoma developed in the vertebral intercentrum of a temnospondyl amphibian, Metoposaurus krasiejowensis from the Krasiejów locality, southern Poland. Results A wide array of data collected from gross anatomy, histology, and microstructure of the affected intercentrum reveals the tumour growth dynamics and pathophysiological aspects of the neoplasm formation on the histological level. The pathological process almost exclusively pertains to the periosteal part of the bone composed from a highly vascularised tissue with lamellar matrix. The unorganised arrangement of osteocyte lacunae observed in the tissue is characteristic for bone tissue types connected with static osteogenesis, and not for lamellar bone. The neoplastic bone mimics on the structural level the fast growing fibrolamellar bone, but on the histological level develops through a novel ossification type. The physiological process of bone remodelling inside the endochondral domain continued uninterrupted across the pathology of the periosteal part. Conclusions Based on the results, we discuss our case study’s consistence with the Tissue Organization Field Theory of tumorigenesis, which locates the causes of neoplastic transformations in disorders of tissue architecture.

Stereospondyli are a clade of large aquatic temnospondyls known to have evolved a large dermal pectoral girdle. Among the Stereospondyli, metoposaurids in particular possess large interclavicles and clavicles relative to the rest of the postcranial skel-eton. Because of the large size of these dermal bones, it was first proposed that they served as ballast during hydrostatic buoyancy control which assisted metoposaurids to live a bottom-dwelling mode of life. However, a large bone need not necessarily be heavy, for which determining the bone compactness becomes crucial for under-standing any such adaptation in these dermal bones. Previous studies on the evolu-tion of bone adaptations to aquatic lifestyles such as osteosclerosis, pachyostosis, osteoporotic- like pattern and pachyosteosclerosis have been observed in the long bones of secondarily aquatic amniotes. However, there are no known studies on the analysis of bone compactness in the dermal pectoral girdle of non-amniote taxa in-cluding Temnospondyli. This study looks at evidence of changes in bone mass adapta-tions in the dermal bones of the pectoral girdle of two stereospondyls occurring in the Late Triassic Krasiejόw locality (Southwestern Poland), namely: Metoposaurus krasie-jowensis and Cyclotosaurus intermedius. However, because of lack of research on bone compactness of temnospondyls in general, there is no existent frame of reference to infer bone mass increase (BMI) in the M. krasiejowensis samples, and thus the bone compactness results of this taxon are compared with that of the samples of C. interme-dius. Results of this study indicate that the interclavicles of M. krasiejowensis partially evolved BMI- like condition rendering these bones to be heavy enough to get selected as ballast during hydrostatic buoyancy control. Additionally, M. krasiejowensis shared its habitat with C. intermedius, however, the dermal pectoral girdle sample of the lat-ter taxon does not display signs of BMI- like condition. Furthermore, the absence of variation in hydrostatic buoyancy control in the ontogenetic series of interclavicles of M. krasiejowensis could imply lack of ontogenetic niche shift along the water column.

Temnospondyli are commonly believed to have possessed four digits in the manus and five in the pes. However, actual finds of articulated autopodia are extremely rare. Therefore, an articulated, slightly incomplete forelimb skeleton with preserved manus of Metoposaurus krasiejowensis from the Late Triassic of Poland is important in providing new details about the structure and ossification sequence in the temnospondyl limb. The most important observation is the presence of five metacarpals in this specimen. This allows reconstructing the manus as pentadactyl. The number of phalanges and the distribution of distal articulation facets allow reconstruction of the digit formula as (2?)‐3‐3‐(3?)‐(2?). The well‐developed fifth digit suggests that the Metoposaurus manus shows a unique ossification sequence: the reduction or late ossification of the first digit conforms to the amniote‐frog pattern, and the early development of the second and third digit makes M etoposaurus similar to salamanders. Based on the distribution of pentadactyly vs. tetradactyly in the temnospondyl manus, the number of digits was not phylogenetically constrained in temnospondyls, similar to today's amphibians., Metoposaurus krasiejowensis from the Late Triassic of Poland shows a unique for Temnospondyli structure of the manus with five digits. Eine Uneindeutigkeit von Sachverhalten stellt ein potenzielles Risiko fur Patienten dar. Daher ist es ein Erfordernis, in den zahlreichen zu einer Therapie notwendigen Dokumenten eine Eindeutigkeit zu erlangen, welche Risiken vermeidet und die Sicherheit der Behandlung erhoht. Ziel war es, diese Eindeutigkeit systematisch zu erlangen. Im Laufe eines Projekts wurden alle klinisch relevanten klinikeigenen Dokumente aus einer im EDV-System der Klinik gespeicherten ungelenkten Form in eine SharePoint-basierte gelenkte Form ubertragen. Hierzu wurde eine Systematisierung entwickelt, welche die organisatorischen Gegebenheiten der Klinik widerspiegelt. Die relevanten Dokumente liegen nun ausschlieslich in gelenkter Form vor. Der Zugriff auf gelenkte Dokumente konnte fur alle Mitarbeiter uber ein SharePoint-System erreicht werden. Jede etwaige Anderung in den Dokumenten wird nun protokolliert. Das Projekt kann als exemplarisch gelten, da die Ergebnisse einen multidimensionalen Beitrag zur Patientensicherheit darstellen, der als ein wichtiger Meilenstein in einer Klinikkultur angesehen werden und als Beispiel fur andere, ebenso motivierte Kliniken dienlich sowie fachubergreifend nutzlich und hilfreich sein kann.

The Triassic basal sauropodomorph Plateosaurus trossingensis is well-known from mass accumulations at the German localities of Trossingen and Halberstadt and the Swiss locality of Frick, and is significant especially regarding its ta-phonomy and proposed developmental plasticity. These implications, however, rely on the assumption that this material derives from a single species, which has been questioned. Here we describe new skull material from Frick including eight complete and six partial skulls, more than doubling the number of known skulls of P. trossingensis. This exceptional sample size allows for gaining a deeper understanding of variability that may occur in a single species. The new mate-rial includes the first known juvenile skulls of Plateosaurus, allowing for detecting ontogenetic changes. An attempt is made to distinguish between variability caused by taphonomic plastic deformation and intraspecific variability. Plastic deformation may shorten, but not widen bones. A number of characters commonly included in phylogenetic analyses of basal sauropodomorphs are shown to be variable within P. trossingensis, and possibly require re-evaluation. Although P. trossingensis skulls are highly variable, many of the variable characters include intermediate character states and therefore are continuous. No groupings based on skull features, locality, or stratigraphy are apparent. Consequently, the analyzed skull material from the bonebeds of Frick, Trossingen, and Halberstadt bears no evidence for the presence of more than one species.

Purpose Histological studies on temnospondyl amphibian bones remain rare. A systematic revision of the histology was applied for the purpose of testing the histovariability in the humeri and becoming new information about the growth pattern. Methods The present study includes 12 humeri of Metoposaurus krasiejowensis, which originate from the Late Triassic clay pit near Krasiejo´w, southwestern Poland. The specimens were scanned with a microCT and the histological thin-sections have been obtained with the standard petrographic method. Results The evaluation of the studied bones shows a uniform growth series resulting in one morphotype. Strikingly, the histological analysis reveals a greater diversity with two different histotypes: Histotype I shows a distinct differentiation between alternating zones and annuli with poor to moderate vascularization and increase of bone remodeling during growth. Histotype II does not show any distinct zones and annuli but is characterized by a high vascularization and fast growth coexisting with an extensive bone remodeling. Conclusions Based on these two different growth patterns, several hypotheses can be established: The specimens do not show any pattern of histological variation; humeri represent a taxonomic diversity; the analyzed Metoposaurus bones stem from two different populations separated by space and; or time or sexual dimorphism

Evidenzbasierte Medizin bedeutet, im Rahmen einer medizinischen Behandlung eine patientenorientierte Entscheidung auf der Grundlage von empirisch nachgewiesener Wirksamkeit zu treffen. Hierzu existieren Klassifizierungen, in welchen nach definierten Vorgaben Studien und damit Therapieoptionen hinsichtlich ihrer Evidenz eingeteilt werden konnen. Die Tuba eustachii verbindet den Nasopharynx mit dem Mittelohr. Ihre Hauptaufgabe ist die Gewahrleistung der Beluftung des Mittelohres. Ist diese Beluftungsfunktion eingeschrankt, resultieren entzundliche Erkrankungen des Mittelohres. Fur die Behandlung der gestorten Mittelohrbeluftung sowie der Otitis media sind bisher zahlreiche Therapieoptionen evidenzbasiert. Ein wesentlicher Therapieansatz ist die antibiotische Therapie. Aber auch neuere Verfahren wie die Tubenballondilatation zeigen erste Erfolge, mussen jedoch hoch hinsichtlich ihrer Evidenz weiter evaluiert werden. Weiter liegt z. T. fur langjahrig etablierte Verfahren keine Evidenz vor. Die Klassifikation einer Evidenz fur die unterschiedlichen therapeutischen Ansatze ist aufgrund vielfaltiger Variablen sehr diversifiziert zu betrachten. Daher sind fur die Evidenz der Behandlung der gestorten Mittelohrbeluftung sowie der Otitis media zahlreiche evidenzbeitragende Studien notwendig, um bestehende und auch zukunftige Therapiekonzepte zu evaluieren. Wird dieses angegangen, sind im Laufe der kommenden Jahre zahlreiche Implikationen fur therapeutische Ansatze zu erwarten.

Hintergrund: Die Erbringung von Gesundheitsleistungen basiert in Deutschland auf einer solidarischen Grundpramisse und ist hochreglementiert. Es stellt sich die Frage nach den Optionen fur ein Marketing von Gesundheitsleistungen in Hals-, Nasen-, Ohrenkliniken. Material und Methoden: Die Marketingoptionen werden mithilfe umweltanalytischer Betrachtungen eruiert. Ergebnisse: Das Erreichen der unternehmensgerichteten oder kundengerichteten Marketingziele kann mit Masnahmen der externen Instrumente Leistungspolitik, Preispolitik, Distributionspolitik oder Kommunikationspolitik oder mit Masnahmen des internen Instruments Personalpolitik erreicht werden. Die Rahmenbedingungen sind vor allem gepragt von den gesetzlichen Rahmenbedingungen, welche die Spielraume insbesondere fur die Preispolitik und die Kommunikationspolitik einschranken. Alle Masnahmen mussen sich jedoch an den ethischen Rahmenbedingungen spiegeln, welche als Grundpramisse von Gesundheitsleistungen gelten und ein Spannungsfeld mit okonomischen Faktoren bilden konnen. Preispolitische Gestaltungsansatze bestehen lediglich bedingt im zweiten Gesundheitsmarkt. Damit ist die Bedeutung des Preises im Marketing von Gesundheitsleistungen sehr gering. Als Erfolgsvoraussetzung von Marketingaktivitaten muss eine Marktanalyse durchgefuhrt werden, welche die individuell relevanten Faktoren des Anbieters ergrunden. Die grundlegende Voraussetzung eines Marketings von Gesundheitsleistungen ist Vertrauen. Schlussfolgerungen: Das Marketing von Gesundheitsleistungen unterscheidet sich von einem Marketing betriebswirtschaftlich ausgerichteter Unternehmen anderer Branchen. In Zukunft wird die Wichtigkeit von Marketingaktivitaten in Hals-, Nasen-, Ohrenkliniken zunehmen.

Metoposaurids are representatives of the extinct amphibian clade Temnospondyli, found on almost every continent exclusively in the Late Triassic deposits. Osteohistologically, it is one of the best-known temnospondyl groups, analyzed with a wide spectrum of methods, such as morphology, morphometry, bone histology or computed modelling. The least known member of Metoposauridae isPanthasaurus maleriensisfrom the Pranhita-Godavari basin in Central India, being geographically the most southern record of this family. For the first time the bone histology of this taxon was studied with a focus on the intraspecific variability of the histological framework and the relationship between the observed growth pattern and climatic and/or environmental conditions. The studied material includes thin-sections of five long bones, a rib, an ilium and an intercentrum belonging most likely to eight individuals ranging from different ontogenetic stages. All bones have a large medullary region with progressively increasing remodeling, surrounded by a lamellar-zonal tissue type. The primary cortex consists of parallel-fibered matrix showing various degrees of organization, less organized collagen fibers in the zones and higher organized in the annuli. Growth marks occur in the form of alternating zones and annuli in every bone except the ilium and the intercentrum. The vascularity becomes less dense towards the outermost cortex in all sampled limb bones. Towards the outermost cortex the zone thickness is decreasing, in contrast to the avascular annuli, that become thicker or are of the same thickness. The growth pattern ofP. maleriensisis uniform and represents changes in ontogenetic development. Multiple resting lines are prominent in the outer annuli of the limb bones and the rib and they presumably indicate climatic and environmental influence on the growth pattern. Therefore, a prolonged phase of slowed-down growth occurred during the unfavorable phase, but a complete cessation of growth indicated by Lines of Arrested Growth (LAGs) is not recorded in the studied samples. Based on the histological framework we conclude that the climate had an impact on the growth pattern. As we do not see any LAGs in the Indian metoposaurid, we assume that the local climate was relatively mild in India during the Late Triassic. A similar prolonged phase of slowed down growth without the occurrence of LAGs was observed inMetoposaurus krasiejowensisfrom the Late Triassic of Krasiejów (Poland). This is in contrast to Moroccan metoposauridDutuitosaurus ouazzouifrom the Late Triassic of Argana Basin, where LAGs are regularly deposited throughout ontogeny indicating most likely harsher climatic conditions.

In order to be able to make targeted use of the advantages of AHSS grades in forming processes it is necessary to determine local material properties. Hence, in addition to edge crack sensitivity measures, such as hole expansion ratio according to ISO 16630 (HERISO), it is currently intensively discussed whether measures, that assess the local ductility of the material, should also be specified in material specifications. One representative is given by the parameter true fracture strain (TFS) that is based on the measurement of the fracture area of a cracked flat tensile specimen. By means of 16 different cold- and hot-rolled steel batches, the relation between the parameters TFS and HERISO was investigated. The outcome is that these parameters do not correlate to a satisfying degree when considering all materials and consequently are regarded as representatives of different material properties. Thus, the issue of the reliability of the measures is considered separately. For the regarded steel grades, the overall scattering of TFS, i.e. neglecting the sample orientation, is found to be 13 – 44 % and the scattering of HERISO is within 5 – 35 %. The high scattering of TFS is mainly caused by the anisotropy of the materials. In case of parallel measurements of the same batch the relative standard deviations of TFS and HERISO are on the same level for cold-rolled strips, whereas in case of hot-rolled strips the relative standard deviation of HERISO is significantly higher. By investigating the influence of the tensile specimen shape for a bainitic hot-rolled strip numerically, it could be shown that the width-to-thickness ratio, that takes the hole fracture area into account, influences the TFS value directly.

Zielsetzung: Die ambulante Chirurgie spielt in Deutschland eine zunehmend wichtigere Rolle. Es stellt sich die Frage, ob dafur in den strategischen Planungen von Universitatskliniken Diversifikationsstrategien oder Hedging-Strategien konzipiert werden sollten. Methodik: Vor dem Hintergrund umweltanalytischer Aspekte und exemplarischer Untersuchungen wird eruiert, ob als Planungspramisse eine generelle Einteilung vorgenommen werden kann. Ergebnisse: In Deutschland ist der Anteil ambulanter Operationen an Kliniken derzeit gering, mit einer Zunahme des Bedarfs wird allerdings gerechnet. Demografische und kulturelle Aspekte lassen auf die Notwendigkeit einer Planung als Diversifikationsstrategie schliesen. Volkswirtschaftliche Aspekte sowie ein internationaler Vergleich hingegen lassen den Schluss auf die Notwendigkeit einer Hedging-Strategie als Planungspramisse zu. Die exemplarischen Untersuchungen zeigen keine Tendenz auf. Schlussfolgerung: Ob bei den strategischen Planungen von einer Diversifikations- oder Hedging-Strategie ausgegangen werden sollte, wird individuell unterschiedlich zu beurteilen sein.

Quality management (QM) is a control tool for the implementation of a wide range of organizational measures aimed at achieving process orientation and process optimization. The organizational framework for these measures can be structured to fulfill the requirements of certification to the DIN EN ISO 9001 standard. The objective was to implement this tool within a university otorhinolaryngology department.After drawing up a project plan, the measures required to fulfill the audit requirements were put into practice by a QM core team, which included employees from four areas: healthcare, research, nursing and teaching.The measures carried out resulted in the achievement of certification, provided that annual surveillance audits are performed.The attainment of the certificate requires continuous fulfillment of the required standards. Future requirements, such as the demands placed on knowledge management, necessitate further appropriate measures.

Zusammenfassung Hintergrund: Gruppenphänomene sind gekennzeichnet durch Verhaltensweisen, die von den Handlungen als Einzelperson differieren. Ein solches ist bspw. das Risikoschubphänomen. ­Diese Beobachtung beschreibt, dass Gruppen fast immer risikoreichere Entscheidungen treffen als die Einzelpersonen. Für Ärzte deutscher Kliniken sind Gruppenphänomene bisher nicht untersucht. Material und Methoden: Im Rahmen einer prospektiven Studie erhielten die Studienteilnehmer einen Fragebogen mit einer Beschreibung von 12 klinischen Grenzfällen. Die Teilnehmer wurden gebeten, diesen selbstständig mit einer vorgegebenen numerischen Analogskala von 1 (keine OP- oder Therapieindikation) bis 10 (OP- oder Therapieindikation) zu beantworten. Am Nachmittag wurden die Fragen gemeinsam diskutiert und erneut beantwortet. Ergebnisse: Vor der Diskussion lagen die Antwortmediane der einzelnen Fragen zwischen 3 und 9 (Mittelwert: 6,25), während nach der Diskussion die Mediane Werte zwischen 1 und 10 annahmen (Mittelwert: 4). Diese Änderungen des Antwortverhaltens waren z. T. statistisch signifikant (p Schlussfolgerungen: Die Untersuchung zeigt, dass in einer Gruppe von Assistenzärzten gemeinsame Diskussionen von klinischen Fällen die individuellen Entscheidungen der einzelnen Teilnehmer zum Teil signifikant beeinflussen. Somit muss im klinischen Alltag das ‚Phänomen der Gruppe‘ immer reflektiert werden, um sich bei der Entscheidungsfindung nicht durch (unbewusste) Einflüsse anderer Gruppenmitglieder leiten zu lassen. Der hohe Wert der gemeinsamen Diskussion und der Reflexion von Fällen bleibt unbestritten.

Smooth muscle neoplasms with atypical proliferative behaviour, but without clear histopathological malignancy represent a diagnostic and therapeutic challenge, as distinction from a sarcoma can be difficult and no guaranteed treatment recommendations are available due to the rarity of these changes. In the event of uncertain primary histology, even metastases cannot be assessed as malignancy criteria, but may contribute to the clarification of the histology. Similarities with other smooth muscle proliferations, such as lymphangioleiomyomatosis, are striking. The diagnostic difficulties and treatment options are explained based on the example of a 59-year-old patient, in whom a retroperitoneal mass and pulmonary lesion of such a tumour occurred 4 years after a hysterectomy. Even though the genesis and histological diagnostics have not been conclusively clarified, slow growth and a low recurrence rate for post-menopausal patients allow for a wait-and-see approach, whereby the option for anti-hormonal treatment exists in the event of positive evidence of hormone receptors.Glattmuskuläre Neoplasien mit zwar atypischem Wachstumsverhalten, aber ohne eindeutige histopathologische Malignitätszeichen stellen eine diagnostische und therapeutische Herausforderung dar, da die Abgrenzung zu einem Sarkom schwierig sein kann und es aufgrund der Seltenheit dieser Veränderungen keine gesicherten Therapieempfehlungen gibt. Bei unsicherer Primärhistologie können selbst Metastasen nicht als Malignitätskriterium gewertet werden, aber zur Klärung der Histologie beitragen. Auffällig sind Ähnlichkeiten zu anderen glattmuskulären Proliferationen, wie der Lymphangioleiomyomatose. Anhand des Beispiels einer 59-jährigen Patientin, bei der 4 Jahre nach Hysterektomie eine retroperitoneale Raumforderung sowie pulmonale Herde eines solchen Tumors auftraten, werden die Schwierigkeiten der Diagnostik sowie die therapeutischen Optionen erläutert. Obwohl Genese und histologische Diagnostik nicht abschließend geklärt sind, erlauben ein langsames Wachstum sowie eine niedrige Rezidivrate bei postmenopausalen Patientinnen ein abwartendes Verhalten, wobei bei positivem Nachweis von Hormonrezeptoren die Möglichkeit einer antihormonellen Therapie besteht.

Peritonsillarabszesse gelten als Komplikation einer peritonsillaren Entzundung und sind eine haufig vorkommende Erkrankung. Oftmals wird zur Abszesssicherung und Lokalisation nach Punctio sicca eine Computertomografie durchgefuhrt. Uber den Stellenwert der Computertomografie in der Diagnostik von Peritonsillarabszessen liegen bisher keine gesicherten Daten vor. In einer retrospektiven Betrachtung wurden die Akten von Patienten, die sich zwischen den Jahren 2006 und 2011 mit einem Peritonsillarabszess-Verdacht in einer deutschen Universitatsklinik vorstellten, analysiert. Der Focus wurde insbesondere auf das diagnostische Vorgehen bei Patienten mit einer Punctio sicca gelegt. Die computertomografischen Bilder wurden durch 2 Neuroradiologen ausgewertet. 310 Patienten mit Verdacht auf einen Peritonsillarabszess aus den Jahren 2006–2011 konnten ausgewertet werden. In 51 Fallen (16,5%) fand sich eine Punctio sicca (31 mannliche und 20 weibliche Patienten). Von diesen wurde bei 42 Patienten (82,3%) eine Computertomografie zum Ausschluss oder zur Abszesslokalisation durchgefuhrt. Bei 36 Patienten (85,7%) fand sich im CT der Nachweis eines Abszesses. Im Falle eines klinisch begrundeten Verdachtes auf einen Peritonsillarabszess sowie einer Puncito sicca ist die Durchfuhrung einer Computertomografie zum Nachweis und zur Lokalisation eines Abszesses nach unserer Datenlage ein zielfuhrendes diagnostisches Verfahren. Dennoch sollten zusatzliche diagnostische Verfahren an Bedeutung gewinnen.

Evidence-based medicine is an approach to medical treatment intended to optimise patient-oriented decision-making on the basis of empirically proven effectiveness. For this purpose, a classification system has been established to categorise studies - and hence therapy options - in respect of associated evidence according to defined criteria. The Eustachian tube connects the nasopharynx with the middle ear cavity. Its key function is to ensure middle ear ventilation. Compromised ventilation results in inflammatory middle ear disorders. Numerous evidence-based therapy options are available for the treatment of impaired middle ear ventilation and otitis media, the main therapeutic approach being antibiotic treatment. More recent procedures such as balloon dilation of the Eustachian tube have also shown initial success but must undergo further evaluation with regard to evidence. There is, as yet, no evidence for some of the other long-established procedures.Owing to a multitude of variables, the classification of evidence levels for various treatment approaches calls for highly diversified assessment. Numerous evidence-based studies are therefore necessary in order to evaluate the evidence pertaining to existing and future therapy solutions for impaired middle ear ventilation and otitis media. If this need is addressed, a wealth of implications can be expected for therapeutic approaches in the years to come.

On the long term it is desirable for cochlear implant (CI) users to control their device via brain signals. For an everyday life application it is convenient to use implanted electrodes for recording the signals. In this pilot project we investigate in a first step the feasibility of implanting epidural electrodes temporally and the possibility to record auditory evoked potentials (AEPs) in the course of several days. First recordings in three patients show promising results. Altogether the approach is feasible, safe and well tolerated by the patients, and the AEP waves can be clearly seen.

Die lange Krankengeschichte vom Beginn einer Heiserkeit im Januar 1887 bis zum Tod im Juni 1888 ist aufgrund der prominenten Stellung des Patienten umfangreich dokumentiert und bietet einen Einblick in die Laryngologie des ausgehenden 19. Jahrhunderts. Die unter der Leitung des englischen Laryngologen Dr. Mackenzie mitbehandelnden deutschen Arzte verfassten uber die Behandlung Friedrich III. sowie deren Umstande Berichte, welche im Jahre 1888 durch die Kaiserliche Reichsdruckerei publiziert wurden. Diese Berichte wurden im Hinblick auf die damaligen Behandlungspramissen sowie die diagnostischen und therapeutischen Vorgehensweisen analysiert. Die zitierten Passagen der Niederschriften eroffnen einen lebhaften historischen Blickwinkel in die Behandlungspramissen und Vorgehensweisen bei Kehlkopferkrankungen im ausgehenden 19. Jh. Die Analyse dieses exemplarischen Beispiels legt die Gemeinsamkeiten und Unterschiede der Laryngologie des 19. Jh. mit der heutigen Laryngologie offen. Viele der damals entwickelten Techniken und Pramissen konnen noch heute als Grundlage der Behandlungsoptionen angesehen werden. Andere Vorgehensweisen sind jedoch obsolet.

Ambulante Chirurgie hat in deutschen Hals-, Nasen-Ohrenkliniken zum gegenwartigen Zeitpunkt keinen grosen Stellenwert. Aktuelle Daten uber den Anteil der ambulanten Operationen liegen jedoch nicht vor. Exemplarisch wurden retrospektiv die in Narkose oder Sedierung durchgefuhrten ambulanten Operationen einer deutschen Universitats-HNO-Klinik analysiert. Der Anteil an in Narkose oder Sedierung durchgefuhrten ambulanten Operationen an der Gesamtanzahl von Operationen in Narkose oder Sedierung betrug fur das Jahr 2010 in der untersuchten Klinik 3,1%. Das Spektrum der Operationen war breit angelegt, es wurden jedoch hauptsachlich Adenotomien durchgefuhrt. Angesichts knapper werdender finanzielle Ressourcen im deutschen Gesundheitssystem und in Anbetracht eines internationalen Vergleichs mit anderen Gesundheitssystemen kann eine kunftige Zunahme des Stellenwerts der ambulanten Chirurgie in deutschen HNO-Kliniken angenommen werden. Daher sollten Kliniken in ihren strategischen Planungen das Bedarfsfeld der ambulanten Operationskapazitaten ausreichend berucksichtigen. Grundvoraussetzung ist allerdings eine realistische Kostenerstattung seitens der Kostentrager.

INTRODUCTION: In addition to trauma, risk factors for the formation of auricular hematomas include the influence of tangential shearing forces upon the auricle. Thus far, no data are available concerning the impact of these forces as caused by the putting on and removal of motorcycle helmets. MATERIAL AND METHODS: In a retrospective observational study, all patients presenting with auricular hematoma and auricular seroma at a German Department of Otolaryngology between January 2005 and March 2011 were analyzed as to a possible connection with the wearing of motorcycle helmets. RESULTS: The proportion of auricular hematoma and seroma patients who were helmet-wearing users of motorcycles, motor scooters or mopeds was 41%. However, none of these patients indicated having put on a protective helmet prior to the occurrence of the hematoma. DISCUSSION: According to our findings, the wearing of motorcycle helmets, with the possible associated influence of tangential shearing forces when putting on or removing the helmet is - despite isolated cases having been described - not a cause of the acute formation of auricular hematomas or seromas. However, the proportion of motorcycle/motor scooter/moped users among patients with these conditions appears to be higher than that for the population as a whole. Die Einfuhrung eines Risikomanagements in Kliniken ist durch viele Aspekte begrundet. Neben rechtlichen Gesichtspunkten spielen ethisch-moralische Grunde eine fuhrende Rolle. Vorgestellt wird die Implementierung eines Risikomanagements in einer deutschen Universitats- Hals-, Nasen-, Ohrenklinik. Neben der Einfuhrung verschiedener Instrumente lag das Hauptaugenmerk zum einen auf der Etablierung eines Risikomanagement-Teams, welches sich prospektiv und retrospektiv mit kritischen Situationen im Klinikbetrieb befassten sollte und zum anderen auf einer Kulturanderung in der Klinik im Umgang mit Fehlern, Komplikationen und Schaden. Viele Aspekte der klinikinternen Prozesse wurden kritisch analysiert und Optimierungsstrategien entwickelt, beispielsweise die Einfuhrung von Checklisten zur Patientenidentifikation als Masnahme zur Vermeidung von Eingriffsverwechslungen. Weiter zeigte das eingefuhrt CIRS eine gute Akzeptanz. Eine aussagekraftige Objektivierung der Ergebnisse scheint hingegen schwierig. Ein Hauptgrund ist, dass die Kennzahl „Reduzierung der Schaden und Behandlungsfehler“ fur eine statistische Auswertung zu klein ist. Dennoch ist eine Implementierung eines Risikomanagements in jeder Hals-, Nasen-, Ohrenklinik zu empfehlen.

A 28-year-old female patient with a migrant background presented for surgery with a suspected cholesteatoma in the left ear. The patient reported having had an aural discharge for several months; otoscopic examination revealed a runny ear, and discrete granulation tissue was seen. Pure-tone audiometry showed conduction hearing loss of 30-40 dB across all frequencies in the left ear; high-resolution computed tomography of the temporal bone revealed that the mastoid and tympanic cavity were completely obscured. The intraoperative finding showed a caseous space-occupying mass that completely filled the tympanic cavity. The suspected diagnosis of tuberculosis was corroborated by pathohistological, microbiological and molecular biological tests. Tuberculostatic therapy was initiated at a different location. Although tuberculosis of the middle ear is a rare condition in Germany, it should nevertheless be considered when making a differential diagnosis, especially in high-risk patients where cholesteatoma is suspected on clinical and radiological evidence or in patients with a chronic middle ear process.

A 60-year-old man was hospitalized because of a sudden onset of severe pain in the epigastrium and haematemesis. Acute pancreatitis was diagnosed on the basis of an increased serum amylase concentration (642 U/l). Abdominal ultrasound and computed tomography demonstrated a necrotic zone with central liquefaction in the tail of the pancreas adjoining the stomach wall. Gastroscopy revealed as source of the bleeding an extensive mucosal necrosis at the greater curvature of the stomach. At laparotomy, partial resection of the pancreas, gastrectomy and splenectomy were performed. Histological examination of the resected specimens showed multiple cholesterol emboli in the small arteries of the pancreas and the gastric submucosa.
