The paper presents partial results of the expert cooperation between geologists and historians in the Bohemian Switzerland (Elbe Sandstones) area focused on mapping and documentation of old quarries and mining pits. More than three thousand abandoned quarries for building stone were newly mapped and registered within the study area. Attention was given not only to the preserved relics after mining and their historical context: methods of stone extraction were also described, and the current state of quarries and mining areas was documented. Many of them represent unstable quarry faces prone to rockfall. Catastrophic events have already occurred in some quarries, and some localities had to be technically secured by stabilizing works recently. One of the aims was therefore to create an inventory map of risk related to old quarries and the assessment of the present-day degree of instability. Fresh rockfalls from 2021/2022 were also registered. Three-dimensional models of selected quarry faces were created using modern technology including calculation of cubic capacity of excavated material and collapsed masses. The complete results are included in the map geodatabase, which is under construction.

A multi-element and multivariate geochemical analysis has been carried out at a medieval farm site and village settlement, which belonged to the Cistercian monastery of Plasy (in W Bohemia). The results of our geochemical survey have been evaluated in context of the LiDAR survey covering the same area and the available historic maps (Habsburg Military surveys), which helped to locate relict landscape features and land-use changes. Approximately 300 samples were taken in a grid point pattern within the courtyard of the farm, as well as randomly, in the surrounding areas, in order to identify geochemical signals related to the observable surface phenomena. We have applied different analytical techniques, including PCA, log-transformation and isometrical log-transformation, and through spatial interpolation (IDW) it was possible to link signals of both anthropogenic and geogenic character to archaeological, cultural and land-use phenomena. The results illuminated more intensive anthropogenic impact in connection to the courtyard area, and the intravillain area of the village, and additionally helped to locate different land-use activities in the surrounding area (agricultural and possibly industrial). In that regard, this methodology was successfully applied to trace anthropogenic impact beyond narrowly defined archaeological sites. Site formation processes at ancient tells in the southern Levant have been the focus of several micromorphological studies, contributing to the differentiation of anthropogenic remains from long-term natural sedimentation, occurring post-abandonment. This paper discusses how the study of sedimentary processes and chemical compositions of sediments can be used within the context of an ongoing archaeological project, and how they can contribute to archaeological, historical and geomorphological interpretations. Sedimentary processes were studied implementing POSL, granulometry and PXRF as part of the archaeological research at Tel Burna, Israel. Focusing on the area along the northwestern fortification walls (Area B2), data was collected from multiple strata inside and outside the casemate fortifications dating from the Late Bronze Age to the Late Iron Age. The gradual increase of OSL values obtained inside the casemate wall, indicate accumulation of sediment during a long period of time. Whereas similar values along the entire profile outside the casemate wall indicate sediment accumulation in one-time event. This might be related to defensive preparations, allegedly in response to advances made by Sennacherib's army in 701 BCE. In addition, results from the PXRF studies demonstrated correlation between human activities and the Cu, P, K, Zn, Mn values. Specifically, it was found that as K content increased from younger to older periods, it can be used as a pseudo-dating element. Ca content decreased as sampling descended from the tell's surface, suggesting its origin in long-term aeolian processes. The results show that the use of POSL and PXRF on archaeological contexts are useful for determining sedimentary processes. Furthermore, chemical content enabled pseudo-dating of strata and facilitated the distinction between natural and anthropogenic processes in archaeological sites and landscape.

Infectious diseases, which often result in deadly sepsis or septic shock, represent a major global health problem. For understanding the pathophysiology of sepsis and developing new treatment strategies, reliable and clinically relevant animal models of the disease are necessary. In this review, two large animal (porcine) models of sepsis induced by either peritonitis or bacteremia are introduced and their strong and weak points are discussed in the context of clinical relevance and other animal models of sepsis, with a special focus on cardiovascular and immune systems, experimental design, and monitoring. Especially for testing new therapeutic strategies, the large animal (porcine) models represent a more clinically relevant alternative to small animal models, and the findings obtained in small animal (transgenic) models should be verified in these clinically relevant large animal models before translation to the clinical level.

Uvod: Extrakorporalni membranova oxygenace (ECMO) je metoda mimotělniho oběhu se vzrůstajicim užitim u kardiogenniho soku nebo refrakternich srdecnich zastav. Pece o tyto pacienty je vesměs vazana na centra s kardiochirurgii. Technický vývoj usnadil zavedeni okruhu bez chirurgicke asistence a pro management oběhove podpory neni nutna přitomnost perfuzionisty.

V poslednich letech přitahuje terapeutický přistup s využitim mimotělnich hemadsorpcnich metod u kriticky nemocných stale větsi zajem. Snaha o casný zasah do patofyziologických procesů, ktere doprovazeji řadu inzultů u pacientů v intenzivni peci, představuje nepochybně atraktivni koncept, v soucasnosti pronikajici do klinicke praxe navzdory absenci evidence. Niže uvedený text představuje kritický pohled autorů na soucasný stav problematiky hemadsorpcnich metod a jejich využiti u kriticky nemocných, zejmena septických pacientů.

Archaeological Dark Earth soil is a category of archaeological anthrosols that has gained much attention because it exhibits higher nutrient content and organic matter stocks and a characteristic dark colour, and enables higher crop yields in comparison to neighbouring soil. However, much is unknown about the chemical properties of Archaeological Dark Earth soils in Central Europe. Therefore, we studied a European Dark Earth (EDE) soil, a type of Archaeological Dark Earth soil, in the territory of the early medieval Dřevic hillfort, Czech Republic, aiming to compare the chemical properties of EDE with a control soil unaffected by settlement activities. The black colour of EDE soil contrasted highly with the light grey Cambisol of the control. Soil reaction and total element contents (N, P, Ca, Mn, Fe, Al, Cu, Zn, Sr and Rb) were substantially larger in the EDE soil compared with the control and vice versa in the case of the C/N ratio. Also, the content of plant‐available (Mehlich III) P, Ca, K and Mg was larger in the EDE soil than the control. In contrast to Ca, plant‐available and total P and K were not well correlated. Total element contents are highly suitable for geoarchaeological purposes as enrichment factors can be easily calculated. The increased contents of these elements, together with pieces of charcoal visible in the EDE soil, indicate the deposition of organic waste and biomass ash during the existence of the hillfort. Thus, medieval hillforts can represent extraordinary nutrient‐rich islands in the cultural landscape of Central Europe. The chemical signatures generated by past settlement activities are so intense that they cannot easily be masked by short‐term intensive application of mineral fertilizers. HIGHLIGHTS: Settlement activities increased soil pH and the content of anthropogenic elements. Enrichment by P, Ca, Mn, Fe, Al, Zn, Cu, Sr and Rb may indicate settlement activities. Analysis of total element contents is suitable for geoarchaeological purposes. Application of mineral fertilizers cannot easily mask signatures of medieval settlement activities.

Our research on spatially bounded Iron Age enclosure of the Viereckschanze type in southern Bohemia made it possible to distinguish the anthropogenic influence of the Iron Age from modern human activities. We collected over 456 samples from 200 cores to a depth of up to one metre. Samples were measured using pXRF to determine the content of the following elements: Al, Si, P, K, Ca, Ti, Mn, Fe, Cu, Zn, As, Rb, Sr, Zr, Pb, and LE – ‘light elements’). Subsequent isometric log-transformation of ppm elemental contents and PCA allowed to distinguish the prehistoric anthropogenic influence from the current modern one. The result of the analysis is as follows: 1) the P signal typical for archaeological settlements was found mainly outside of the enclosure; 2) the conventional anthropogenic signal from the inside of the enclosure was only represented by Mn; 3) other elements related to possible anthropogenic activities were revealed only after applying statistical analysis (As, Pb, Zn, Cu); 4) the unusual manifestation of Si and Ti was connected to the archaeological contexts. The combination of these results (Cu, Zn, Pb) and magnetic measurements revealed places of metallurgical activity inside Viereckschanze. It is certain that the site was not only a place of residential activity, but also of production activity. This chapter exposes multiple contextual layers in the historical background of the famous Ossuary at Sedlec in the Czech Republic. The Cistercian monastery of Sedlec was founded in the twelfth century and its importance soon increased together with the expansion of silver mining in the nearby town of Kutná Hora. According to a later legendary narrative, soil brought from the Holy Land was scattered across the monastic cemetery, which made this place an attractive destination for the burial of countless corpses in the course of the following centuries. The enormous amount of human remains in individual as well as mass graves led to the establishment of the Ossuary, which was rearranged several times, most recently in the second half of the nineteenth century. A variety of folk tales tell of the miraculous deeds of the bones and their special powers. Being conveniently located in the buffer zone of the UNESCO World Heritage Site of Kutná Hora, today crowds of tourists appreciate the macabre use of human skeletal elements in its decoration. A large-scale rescue excavation conducted recently around the Ossuary revealed an important archaeological archive that complements historical records.

Coronary artery aneurysm is a relatively rare disorder that is usually discovered as a secondary finding in patients undergoing coronary artery angiography. Coronary artery fistulas are relatively more frequent than rare aneurysms and are often associated with other cardiac abnormalities. The etiology of aneurysms is mostly atherosclerotic, and they are less frequently associated with other acquired or congenital diseases, such as Kawasaki disease, connective tissue diseases, septic emboli, arteritis, and iatrogenic disease. We report a 70-year-old woman with a rare combination of a coronary artery aneurysm associated with a coronary artery fistula, which drained into the pulmonary artery. The diagnosis of our patient was made by selective coronary angiography and confirmed by computed tomography angiography. The patient was treated surgically because of the symptomatic course of the disease.

Bifurkacni leze tvoři 15-20 % vsech intervenovaných koronarnich lezi a jejich osetřovani je i přes výrazný pokrok v intervencni kardiologii i nadale spojeno s technickými obtižemi a horsi klinickou efektivitou. Zakladnim doporucovaným postupem pro osetřovani bifurkacnich lezi je "provisional stenting". Jako alternativni postup lze zvažit některou z dvoustentových technik. V nasem registru jsme se rozhodli posoudit bezpecnost, technickou a klinickou uspěsnost při použiti dedikovaneho bifurkacniho stentu Tryton Side Branch. Do registru bylo zařazeno 201 pacientů neselektovane populace osetřených v obdobi mezi cervnem 2012 a dubnem 2016 v Kardiovaskularnim centru Krajske nemocnice Liberec, u kterých bylo zvažovano užiti bifurkacniho stentu Tryton. Uspěsnost osetřeni bifurkacnich lezi při užiti stentu Tryton byla 85,07 %. Pacienty jsme pote sledovali v obdobi sesti měsiců od osetřeni. K infarktu myokardu doslo u tři pacientů, k revaskularizaci bylo nutno přistoupit pouze u tři pacientů, z toho vsak ani jednou nebylo potřeba osetřit cilovou tepnu nebo lezi. Otazka osetřovani bifurkacnich lezi zůstava do budoucna stale otevřena, jako možna alternativa se jevi užiti dedikovaných bifurkacnich stentů. Bifurkacni stent Tryton ma výhodu v ochraně bocni větve, jejiž osetřeni může být u větsiny standardnich technik problematicke. Stent Tryton lze dle nasich dat s výhodou použivat zejmena u bifurkacnich lezi s velkou bocni větvi (≥ 2,5 mm). Data z naseho registru tak svědci pro vysokou bezpecnost a uspěsnost osetřovani tohoto typu bifurkacnich lezi pomoci stentu Tryton. © 2019, CKS.

The second part of the review deals in detail with the diagnostics and treatment of toxic alcohols poisoning and management and indication of extracorporeal removal techniques in intoxication with other drugs, theophylline, valproic acid, metformin and metformin associated lactic acidosis, respectively. The extracorporeal treatment enhances the clearance of the toxin and corrects patients metabolic disturbances as well. It is necessary to use this treatment in severe intoxications. Indication of this invasive procedure falls within clinicians and nephrologists competence being advised by a toxicologist. This review could help make fast decisions.

Supporting clearance of a toxic substance by an extracorporeal removal technique is one of the advanced treatment methods applied in poisoned patient management. General indications stem from toxicokinetics of the poison while individual indications are determined by poisoning severity. The first part of this review deals in detail with particular options of extracorporeal treatment in toxicology and also with its specific application when treating lithium and salicylates poisoning or dabigatran overdose. The aim of this review is to facilitate the clinicians and nephrologists decision making whether to indicate this invasive procedure, to communicate and summarize the existing recommendations and to highlight the most important ways of how to treat poisoning by specific toxic substances.

Medieval vegetation–human–climate interactions were studied from a sediment profile situated in the centre of a short-lived medieval village located above 800 m a.s.l. on the ridge of the Krusne Mts., NW Bohemia, Central Europe. Analyses of pollen, seeds/fruits, micro- and macro-charcoals, diatoms and concentrations of microelements in connection with written sources revealed a significant human-induced deforestation in the second half of the 14th century. This deforestation occurred sooner than supposed and the area did not revert after ad 1347 as elsewhere in Europe. Arable fields probably enabled basic self-sustaining cultivation of winter cereals even at such elevations in the climatically favourable years of the Medieval Warm Period. The village presumably collapsed due to a combination of weather fluctuations at the onset of the Little Ice Age, simultaneous socioeconomic stagnation in the Czech Lands and exploitation of the surrounding forest. The dynamics of wet stand vegetation and Calthion palustris montane wet meadows were driven by fluctuating human and grazing impacts. Annual and biennial herbaceous species that peaked after village abandonment were rapidly replaced by Filipendula ulmaria and Salix stands. The secondary forest developed towards Picea stands. Only later, mesic montane meadows of medium tall grasses combined with Meum athamanticum and mountain dry pastures developed on nutrient poor patches.

An ancient tell is a multi-period archaeological site, where anthropogenic, and natural sedimentation processes took place. Although a tell is primarily an anthropogenic type of geomorphological feature, it is affected by natural processes as well. This contribution discusses how these processes can be determined within the context of archaeological research and how it is possible to differentiate and interpret past-human activities and natural processes. Tel Burna, a site intensively occupied from the Early Bronze to Iron Ages (3rd millennium BCE – 6th century BCE) located in the southern Levant, was chosen for this study of the studying sedimentary processes and chemical compositions of sediments. The sedimentary processes were studied in the course of an archaeological excavation using POSL (portable optically stimulated luminescence), granulometry and PXRF (portable X-ray fluorescence). Focusing on the area along the fortification walls, data was collected from strata around the casemate fortifications dating from the Late Bronze Age to the Late Iron Age.The gradual increase of OSL values obtained inside the casemate wall, indicate accumulation of sediment during a long period of time. Whereas similar values along the entire profile outside the casemate wall indicate sediment accumulation in one-time event. Porcine model of peritonitis-induced sepsis is a well-established clinically relevant model of human disease. Interindividual variability of the response often complicates the interpretation of findings. To better understand the biological basis of the disease variability, the progression of the disease was compared between animals with sepsis and septic shock. Peritonitis was induced by inoculation of autologous feces in fifteen anesthetized, mechanically ventilated and surgically instrumented pigs and continued for 24 h. Cardiovascular and biochemical parameters were collected at baseline (just before peritonitis induction), 12 h, 18 h and 24 h (end of the experiment) after induction of peritonitis. Analysis of multiple parameters revealed the earliest significant differences between sepsis and septic shock groups in the sequential organ failure assessment (SOFA) score, systemic vascular resistance, partial pressure of oxygen in mixed venous blood and body temperature. Other significant functional differences developed later in the course of the disease. The data indicate that SOFA score, hemodynamical parameters and body temperature discriminate early between sepsis and septic shock in a clinically relevant porcine model. Early pronounced alterations of these parameters may herald a progression of the disease toward irreversible septic shock.

How long decomposed buried human bodies affect soil elemental composition is so far not well-known. Using portable X-ray fluorescence spectrometry, we determined the elemental composition of sandy necrosols from a Corded Ware Culture (ca 2800–2500 BCE) graveyard, Chudeřin, Czech Republic. The elemental compositions of soils in the grave infills were studied and compared with arable and subsoil layers. After removal of the arable layer, infills of graves were identified by the black color in contrast with their reddish surroundings. The burials affected the content of elements even after more than 4500 years. Increased C contents compared with arable and subsoil layers were recorded only close to bones. The lowest P content was in grave infill, where soft tissues decomposed, because of P leaching. Therefore, increased P content in the infill of the prehistoric graves mentioned by previous authors is not a reliable indicator of soft tissue decomposition. In the acidic environment, P and Ca released from bones partly fixed in the grave infill ≤ 5 cm to bones. The high contents of P and Ca in bones and soils close to bones were a source of these elements for plants, indicated by a high density of fine roots. The highest contents of trace elements (Mn, Cu, As, and Pb) were in the arable layer because of recent disposition. The most reliable soft tissue decomposition indicator was an increased Zn content in the grave infill. The multi-elemental analyses of prehistoric necrosols were neglected so far and require further detailed research as the chemical signatures are well-preserved for millennia.

Human settlement activities have caused changes in soil chemical properties that might remain preserved in the soil archive for a very long time. These traces might be close to irreversible on the timescale of known civilizations. Our study explores the potential of an extensive mapping of the multi-elemental composition of soil and buried sediments by a portable XRF spectrometer, using the Tel Burna site in the southern Levant as a case study. The tell, dating from the 3rd to the 1st millennium BCE, is known as a Bronze Age Canaanite settlement, and later in the Iron Age as a stronghold on the historic border between the Kingdom of Judah and Philistia. We compared the results of our geochemical survey conducted on the surface layer of contemporary soil with the data acquired by the same method from the archaeological stratigraphy exposed during excavations of the site. We found that handheld XRF spectrometry can reveal archaeologically meaningful patterns of the multi-elemental composition of soils and sediments in the horizontal plane, as well as in vertical profiles. Such patterns correspond to areas and loci of ancient settlement activities, dated to millennia ago. This methodology, if used critically, provides a rapid and cost-effective analysis of soil chemical composition that can significantly enhance our understanding of archaeological sites in arid environments.

Lovětin was a medieval village located in the Bohemian-Moravian Upland (Czech Republic). It existed approximately between the 14th and the 16th century and was comprised of up to 20 farmsteads. Arable farming based on a short-fallow cultivation system was its main means of subsistence. The fields of the village consisted of two main field areas (128 and 77 ha) and six smaller field areas (up to 8 ha). Our study focused on finding out how human activities have impacted the soil environment imprinted in its geochemical characteristics. The design was based on a 100-m grid (in some cases, a 50-m grid) covering the entire field system (plužina) of the village; soil samples were taken from the B horizon at a depth of 15 cm. The analysis was based on XRF spectrometry, returning near total concentrations of the elements. The statistical and GIS analyses were performed on 338 samples, measuring the following elements: Al, Si, K, Ti, Cr, Mn, Fe, Ni, Cu, Zn, As, Rb, Sr, Zr, Pb, Th, LE (light elements – combined concentrations for H to Na). The results of the analyses were processed by PCA after ilr transformation. The PCA indicated 16 principal components, of which three were interpreted as direct products of past human activities. The main elements reflecting human activities were Mn, Sr, Th, K, Zn; P contributed also to the interpretations, although it was detected and measured only in 132 cases. Some of the elements were spatially connected to the village in terms of their concentrations, but the PCA did not reveal a strong connection (Ti, Cr, Ni, Cu, Zr). The spatial distribution of the principal components was varied, indicating a diversity in the spatial distribution of agricultural activities or of their intensity, as well as differences in land use management. Management was most intensive in the vicinity of the village built-up area up to a distance of 300–400 m. The village was probably deserted due to inadequate soil management strategies.

Human settlement activities are connected with the accumulation of nutrients in archaeological soils. We address the question of whether the large-scale mapping of the elemental composition of the topsoil in contemporary rangeland can be used for the detection of ancient settlement activities. Using portable X-ray fluorescence (pXRF), we mapped the elemental composition of contemporary soils over an area of 67 ha in and around the Bronze and Iron Age settlement of Tel Burna (identified as probably corresponding with biblical Libnah). Ancient settlement activities substantially increased concentrations of nutrients (P, K, S, Zn and Cu) in the contemporary topsoil owing to the deposition of biomass ashes and organic wastes. Increased concentrations of elements were detected 2500 years after the site was abandoned and we can therefore suppose that changes in the elemental composition of the soil caused by ancient settlement activities are irreversible on a timescale in which human societies operate. Ancient settlement activities increased concentrations of nutrients in contemporary soil to the same level as recent intensive fertiliser application on an adjacent arable field used for vegetable production. Concentrations of nutrients higher than those on the tell summit were recorded only in recent cattle resting areas with intensive deposition of cattle faeces. Changes in the elemental composition of the soil caused by ancient settlement activities consequently result in differential nutrient availability for contemporary vegetation, affecting ecosystem functions for thousands of years. Using pXRF, large-scale mapping of the elemental composition of the topsoil layer at archaeological sites can help to identify the extent and provide basic information on the character of past human activities in the affected landscape units.

Can less than a hundred years of past settlement activities in former European settlements substantially increase the concentration of elements and result in the formation of African Dark Earth soil is a question not addressed. We performed multi–element analysis of Africa Dark Earth soil from a late 19th to mid–20th century CE former German–Togoland settlement, Ziavi–Galenkuito in the Volta Region, Ghana. Relatively neutral reaction and black color of soil mainly from charcoal inclusion in the settlement site contrasted highly with moderately acidic brown Ferric Acrisol in the control located on the same metasedimentary/Voltaian bedrock. Organic C and total N, P, K, Ca, Mn, Fe, Cu, Zn, Sr and Rb concentrations were substantially increased and vice–versa for the C/N ratio in the 0–40 cm layer of the settlement soil compared to the control without settlement activities. The concentrations of plant–available P, K, Ca, S, Fe, Cu and Zn were higher in the settlement soil in comparison to the control. The increased concentrations of the elements mentioned above resulted in the disposition of organic waste and biomass ash. Significantly higher concentrations of total and plant–available elements in 0–10, 10–20 and 20–40 cm layers of settlement site compared to their respective control suggested that chemical signatures from past settlement activities can be determined even in the upper soil layer. Past human activities can be reflected in the elemental composition of contemporary soils. We asked how much historical land-use identified according to historical maps is reflected by the multi-elemental signatures of soils in an originally medieval village abandoned after WWII. Using X-ray fluorescence spectrometry, we determined the content of 24 elements in soil samples from former arable fields, field boundaries, forests, built-up area, and permanent grasslands. Previous human activities were connected with the accumulation of 13 elements such as the usually thus interpreted P, Ca, Zn, and Cu, but also with elements rarely used in archaeological studies such as Mg, K, V, Cr, Mn, Fe, Ni, Rb, Zr, and Sr. The content of P, Ca, Cu, Zn, Ni, Fe, V, Cr, and Zr decreased on former fields with the distance from the most enriched built-up area. This can be explained by the most intensive deposition of biomass ashes and manuring of gardens and fields close to the village. With the exception of Pb accumulated sub-recently because of aerial deposition, the lowest content of anthropogenic elements was recorded in continuous forest. The chemical signatures recorded were much stronger than those in previously studied medieval villages in the Czech Republic abandoned in the 15th or 16th centuries. This is because of the long period of the settlement’s existence since medieval times and in addition because of the short time since its abandonment. Although frequently neglected, the multi-elemental composition of soils in deserted settlements can be considered as cultural heritage similarly to the relicts of buildings or still visible field patterns.

Kutna Hora was a centre of medieval mining and remains an important contamination source in the present day. Surprisingly, very little attention has been paid to the associated contamination. Although some studies have been performed, the majority of information regarding contamination is only accessible in the archives and no overview has been published. The purpose of this study is to perform a meta-analysis of all accessible data and to shed light on this topic. The data mainly come from analyses of HNO3 solutions of sediments. We used statistical analyses (exploratory data analysis, PCA). The results were visualised and evaluated in the GIS environment. The complex of heavy metals As, Be, Cd, Co, Cr, Cu, Hg, Pb, V, and Zn can be divided into three main groups of different interpretation: (1) uninfluenced by mining activities—Be, Co, Cr, Hg, and V; (2) smelting processes—Cu, Pb, and Zn; and (3) mining—As and Cd. These groups also show different spatial distribution patterns, absolute concentration values and binding with different environmental types—landscape features. The contamination of Kutna Hora can be characterised by element grouping and also by spatial diversification. This could be used in future research as a bearer of proxy information. Surprisingly, it also seems that the spatial range of contamination of sediments could be shorter than is generally presumed.

We report a case of a successful surgical treatment of congenital heart defect in a 47-year-old man who experienced an atrial septal defect closure seven years ago. Nevertheless even after the operation he still suffered from increasing shortness of breath, which reached grade NYHA III last year. This state was caused by a partial anomalous venous return (PAPVR) from the left lung to the left brachiocephalic vein. This fact was overlooked during the first operation. Surgical treatment of newly diagnosed, at the time undifferentiated concomitant cardiac defect in the field of previous surgery requires a comprehensive assessment of the patient's condition and a choice of optimal tactics and strategies.

The Mordellidae Latreille, 1802 (Coleoptera: Tenebrionoidea) fauna of Iran is summarized. Costa, Mordellistenula Stchegoleva-Barovskaya, Mordellochroa Emery, Paratomoxioda Ermisch, Stenalia Mulsant, and Variimorda Mequignon. Sepsis, newly defined as a life-threatening organ dysfunction caused by a dysregulated host response to infection, is the most common cause of death in ICUs and one of the principal causes of death worldwide. Although substantial progress has been made in the understanding of fundamental mechanisms of sepsis, translation of these advances into clinically effective therapies has been disappointing. Given the extreme complexity of sepsis pathogenesis, the paradigm “one disease, one drug” is obviously flawed and combinations of multiple targets that involve early immunomodulation and cellular protection are needed. In this context, the immune-reprogramming properties of cell-based therapy using mesenchymal stem cells (MSC) represent an emerging therapeutic strategy in sepsis and associated organ dysfunction. This article provides an update of the current knowledge regarding MSC in preclinical models of sepsis and sepsis-induced acute kidney injury. Recommendations for further translational research in this field are discussed.

The objective of our study was to identify changes in the coagulation and serum concentration of soluble P-selectin (sP-sel) after i.v. bolus of 0.75 mg/kg enoxaparin in a group of 33 patients during PCI.As compared to baseline, i.v. enoxaparin increased anti -Xa activity and FIIa inhibition together with APTT and thrombin time tests within 20 min, that persisted for 60 min. At 6 h, the results of all tests had returned to baseline. In contrast, the level of prothrombin fragments (F1 + 2) decreased persistingly for a period of 6 h (baseline 1.19 ± 0.42 nmol/l, after 20 min 1.03 ± 0.46 nmol/l, after 60 min 1.06 ± 0.43 nmol/l, after 6 h 0.95 ± 0.40 nmol/l, p0.001 vs. baseline for all values). In addition, i.v. enoxaparin decreased serum sP-sel level (baseline 111.80 ± 37.05 ng/ml, after 20 min 87.80 ± 33.17 ng/ml, after 60 min 86.45 ± 29.15 ng/ml, after 6 h 92.24 ± 31.34 ng/ml, p0.001 vs. baseline value for all). sP-sel level mildly correlated with both F Xa inhibition (r = -0.275, p0.05) and F1 + 2 level (r = 0.274, p0.05).Intravenous enoxaparin induced target F Xa inhibition (0.6 IU/ml) for 60 min in 82% of study patients. During the 6 h of monitoring, a decrease of thrombin generation (F1 + 2) and sP-selectin levels were observed.

Dilated cardiomyopathy (DCM) represents the third most common cause of heart failure and the most frequent cause of heart transplantation. Infectious, mostly viral, and autoimmune mechanisms, together with genetic abnormalities, have been reported as three major causes of DCM. We hypothesized that Lyme disease (LD), caused by spirochete Borrelia burgdorferi (Bb), might be an important cause of new-onset unexplained DCM in patients living in a highly endemic area for LD such as the Czech Republic. We performed endomyocardial biopsy (EMB) in 39 consecutive patients presenting with symptomatic unexplained left ventricular (LV) systolic dysfunction lasting no more than 12 months. In eight subjects (21%), Bb was detected in the EMB sample by polymerase chain reaction or by electron microscopy. None of these patients exhibited any form of atrioventricular block or other extracardiac manifestation of Bb infection. Serological testing identified IgG antibodies against Bb in only two cases and IgM antibodies in none. All affected patients were treated with intravenous ceftriaxone for 3 weeks. At 6 months follow-up, LV morphology and function as well as functional status of these patients significantly improved. In conclusion, Bb infection may represent an important cause of new-onset unexplained DCM in patients living in endemic regions such as the Czech Republic. Because the antibiotic treatment appears to be markedly effective and serological examination does not provide a tool for diagnosing the disease, EMB focused on the detection of Bb should be performed in all patients from endemic areas with new-onset unexplained DCM not responding to conventional therapy.

Background Surgical revascularisation in patients with acute myocardial infarction with ST-Segment Elevation (STEMI) is usually considered as a second choice when direct angioplasty/stent fails. However, improvements in surgical technique and postoperative care may justify coronary artery bypass grafting (CABG) in STEMI. Methods This was a retrospective analysis of prospectively gathered data of 135 patients with acute STEMI, treated with CABG in our department from February 2008 to December 2012. Patients were divided into two groups – operated up to 6 hours (35 patients) and 6 to 24 hours (100 patients) from onset of symptoms. Results Preoperatively, 18 (13%) patients were in cardiogenic shock, 10 (7.4%) had mechanical ventilation, and 36 (27%) had intra-aortic balloon counterpulsation (IABC). Mean number of distal anastomoses was 3.3 (range, 1 to 5), cardiopulmonary bypass time 122.7 + 52.6 minutes. In hospital (30-day) mortality was 8.1% (11 patients) with no significant difference in both groups (p = 0.541); 45 (33%) patients had one MACE, again with no difference in both groups (p = 0.89). Risk factor analysis revealed that Killip class at admission, cardiogenic shock, preoperative need for catecholamines, ventilation and low ejection fraction are risk factors for early mortality. Conclusions Acute CABG in patients with STEMI can be performed with good results. Risk factors for early mortality and morbidity are cardiogenic shock, poor haemodynamic status and impaired ejection fraction. Time from infarction to reperfusion did not influence the results.
