The fibrotic process of idiopathic pulmonary fibrosis (IPF) is considered to be the consequence of an exaggerated response to an inflammatory lung injury. In a previous report, except for the response to PG-E2, we found no difference in the proliferative profiles of lung fibroblasts between IPF patients and healthy subjects [Mio et al. (1992) Chest, 102:832-837]. In the present study, we hypothesized that lung fibroblasts from IPF patients would not undergo apoptosis as observed in the normal repair process. Additionally, we focused on the protooncogene bcl-2 which prevents apoptosis and the APO-1 (Fas antigen) which induces apoptosis. In order to explore this question, we used immunohistochemical staining to investigate whether apoptotic markers are expressed on lung parenchymal cells of IPF patients obtained by open lung biopsy. Bcl-2 protein was expressed on mononuclear cells in the mantle zone of lymphoid follicules and smooth muscle cells, but it was not expressed on other parenchymal cells. Apo-1 was expressed on epithelial cells, some germinal center cells, and many parenchymal cells including smooth muscle cells, fibrocytes, and myofibroblasts in patients with IPF, findings of which are fundamentally the same as those in normal subjects. Although we could not find any abnormality of lung fibroblasts in IPF patients, the positive staining with anti-Bcl-2 monoclonal antibody and anti-Fas (anti-APO-1) monoclonal antibody in lung lymphoid follicules suggests the continuous activation of B lymphocytes localized in the lung parenchyma in patients with IPF. The role of apoptosis in fibrosis should be further examined.

By use of Astograph, deviced by Takishima, we studied the bronchial responsiveness (sensitivity and reactivity) to inhaled methacholine in 8 patients with diffuse panbronchiolitis (DPB) and compared it to that in 20 patients with bronchial asthma, 2 patients with emphysema, and 17 healthy volunteers. Additional 3 patients with DPB, 3 patients with bronchial asthma, and 2 patients with emphysema were served only for measuring the control respiratory resistance (Rrs. cont.) during inhaling physiological saline. Positive responses to methacholine occurred in 7(87.5%) of 8 patients with DPB, 19(95%) of 20 patients with bronchial asthma, 2(100%) of 2 patients with emphysema, and 9(52.9%) of 17 healthy volunteers. The value of Rrs. cont. in the patients with DPB was significantly higher than that in the patients with bronchial asthma and in the healthy volunteers. Dose-response curves and time course to methacholine challenge differed in the patients with DPB, the patients with bronchial asthma, and the healthy volunteers, as follows. Respiratory sensitivity (Cmin and PC50) in the patients with DPB was as sensitive as that in the patients with bronchial asthma. Respiratory reactivity (St at PC50) of the patients with DPB was, however, as low as that in the healthy volunteers. Bronchoalveolar lavage fluid was analyzed for its liquid compnents following lavage performed in patients with sarcoidosis (Sar, 22cases), hypersensitivity pneumonitis (HP, 6), idiopathic interstitial pneumonitis (IIP, 9), interstitial pneumonitis with collagen vascular disease (CVD, 10), berylliosis (Be, 4), lung cancer (LC, 8), and healthy controls (7). Although albumin levels were markedly higher in Sar (10.2±1.7mg/dl) and HP (13.4±2.5mg/dl) than in the controls (3.3±0.5mg/dl), they were similar to control levels in IIP, CVD, Be, and LC. IgG/albumin was higher in Sar (0.33±0.03) and HP (0.49±0.19) than in the controls (0.16±0.01). In IIP, CVD and Be, it was markedly higher than in Sar or HP. IgA/albumin was higher in HP (0.21±0.06), IIP (0.16±0.03), CVD (0.22±0.05), and Be (0.20±0.08) than in the controls (0.07±0.02). IgM, which was not detected in the controls, was also negligible among the various diseases. ACE/albumin was lower in Sar (0.11±0.01) and HP (0.08±0.02) than in the controls (0.19±0.04). Lysozyme/albumin, lower than the corresponding serum value, was notably lower in HP (0.03±0.02) and in Sar (0.21±0.04) than in the controls (0.49±0.12). It was also slightly lower in IIP, CVD, and Be, but higher in LC (1.03±0.36) than in the controls. CEA (Z)/albumin was considerably higher in IIP (0.66±0.16), CVD (0.87±0.15), and Be (0.76±0.29), as well as in LC than in the controls. Immune Complex/albumin, similary to CEA (Z), was higher in IIP (1.21±0.30), CVD (0.72±0.43), Be (0.60±0.11), and LC (0.74±0.48) than in the controls (0.05±0.03). We classified diffuse interstitial pulmonary diseases into two groups based on the analysis of bronchoalveolar lavage fluid for cellular components : namely, T-lymphocyte alveolitis (Sar, HP, Be) and B-lymphocyte alveolitis (IIP, CVD). Mediastinal germ cell tumor is so rare that only about 60 patients have been reported in Japan. Recently, we have observed one patient with mediastinal germ cell tumor (Seminoma type). This was remitted by treatment with radiation and chemotherapy, but recurred after 12years. The patient was operated upon after the second radiation. Clinical significance of a skin test with 5μg of purified phytohemagglutinin (PHA) was studied by testing 95 subjects, including 45 cases of lung cancer, 27 cases of pulmonary tuberculosis, 14 cases of leprosy, and 9 healthy controls. Forty three of the lung cancer patients were tested simultaneously with 0.05μg of PPD in order to examine the correlation between the PPD test and the PHA test. The PHA skin reaction was determined 24hr after intradermal injection of PHA and was judged as positive when the diameter of erythema was 30mm or more. The mean diameter of erythema was 38.1±8.1mm (negative rate 0%) in healthy controls, 31.2±8.1mm (59.3%) in pulmonary tuberculosis, 28.8±8.1mm (55.6%) in lung cancer, and 13.8±7.2mm (100%) in leprosy. Lung cancer patients with stage III, IV or undifferentiated small cell carcinoma showed reduced PHA skin reactivity. In leprosy, the PHA skin reactivity of the lepromatous type was more markedly imparied than the reactivity of the borderline type. A positive correlation between the PPD skin reaction and the PHA skin reaction was observed in 27 (62.8%) of the 43 lung cancer patients tested. In this study, changes in lysozyme, IgA and IgG found in bronchial secretions were examined from mice whose bronchi were stimulated by smoking inhalation. Because the bronchial secretions are irregular in density, when a comparison over time for percentage of total protein is measured, lysozyme in bronchial secretion increases, reacting a peak after 20 minutes. Thus lysozyme secretions from the bronchial gland were considered to have increased due to smoking inhalation. IgA found in bronchial secretions were extremely thin, less than 1/80 of the IgA found in mouse serum, so could not be measured. As for IgG, after 40 minutes of inhalation, there was hardly any change, density being 1/160 of that found in mouse serum. In order to examine the influence of smoking on mouse immunity, especially regarding immunoglobulin production, as an antigen, sheep red blood cells (SRBC) were used, thus examining their effects on plaque forming cells (PFC) in ddy and C57BL/6 mouse spleens. The fact that smoking worked to suppress or enhance IgM PFC production was here confirmed. 症例は70歳, 女性。64歳時, 胸部X線で腫瘤陰影を発見され, 67歳で摘出術を施行。その結果, 第6肋骨発生の孤立性形質細胞腫と診断され, 術後2年未満で再発, 3年後にさらに4ケ所の骨に播腫を認めた。しかし, 骨髄穿刺所見は正常であった。病理学的には, 頭蓋骨および頸椎の病変は肋骨腫瘤病変と同様に, 形質細胞の増殖を認め, 腫瘍細胞のモノクローナリティはIgM・λタイプであることが, PAP法により証明されたことから, 骨に多発した形質細胞腫と診断された。孤立性形質細胞腫から多発性骨髄腫へと進行する症例は数多く報告されているが, 本例は骨に多発する形質細胞腫へと移行した稀な例であり, 診断に対する考察を加えて報告した。, We recently experienced one case of multiple plasmacytomas of the bone (IgM-λtype), a 70 year-old female who had five foci of bony destruction for a three-year follow-up period after surgery of solitary plasmacytoma of the rib. A diagnosis for plasma cell tumor was discussed in detail.
