The surgical treatment of parastomal hernias is considered complex and is known to be prone to complications. Traditionally, this condition was treated using relocation techniques or local suture repairs. Since then, several mesh-based techniques have been proposed and are nowadays used in minimally invasive surgery. Since the introduction of robot-assisted surgery to the field of abdominal wall surgery, several adaptations to these techniques have been made, which may significantly improve patient outcomes. In this contribution, we provide an overview of available techniques in robot-assisted parastomal hernia repair. Technical considerations and preliminary results of robot-assisted modified Sugarbaker repair, robot-assisted Pauli technique, and minimally invasive use of a funnel-shaped mesh in the treatment of parastomal hernias are presented. Furthermore, challenges in robot-assisted ileal conduit parastomal hernia repair are discussed. These techniques are illustrated by photographic and video material. Besides providing a comprehensive overview of robot-assisted parastomal hernia repair, this article focuses on the specific advantages of robot-assisted techniques in the treatment of this condition.Die chirurgische Behandlung parastomaler Hernien gilt als komplex und ist bekanntermaßen komplikationsträchtig. In der Vergangenheit wurden diese Hernien durch die Relokation des Stomas oder Nahtverfahren der Austrittstelle versorgt. In den letzten Jahren wurden verschiedene netzbasierte Techniken vorgeschlagen, die heute in der minimal-invasiven Chirurgie eingesetzt werden. Mit der Verbreitung der roboterassistierten Hernienchirurgie wurden die Netzverfahren weiterentwickelt und die Ergebnisse für die Patienten erheblich verbessert. In diesem Beitrag wird ein Überblick über die verfügbaren Techniken der roboterassistierten Versorgung parastomaler Hernien präsentiert. Es werden technische Überlegungen und erste Ergebnisse des roboterassistierten modifizierten Sugarbaker-Verfahrens, der roboterassistierten Pauli-Technik und der Verwendung des trichterförmigen Netzes IPST vorgestellt. Darüber hinaus werden die Herausforderungen bei der roboterassistierten Versorgung parastomaler Hernien am Ileum-Conduit diskutiert. Die Operationstechniken werden durch Foto- und Videomaterial veranschaulicht.

The principle of targeted separation or weakening of individual components of the abdominal wall to relieve tension in the median line during major abdominal reconstruction has been known for over 30 years as anterior component separation (aKS) and is an established procedure. In search of alternatives with lower complication rates, posterior component separation (pKS) was developed; transversus abdominis release (TAR) is a nerve-sparing modification of pKS. With the ergonomic resources of robotics (e.g., angled instruments), TAR can be performed in a minimally invasive manner (r-TAR): hernia gaps of up to 14 cm can be closed and a large extraperitoneal mesh implanted. In this video article, the treatment of large incisional hernias using the r‑TAR technique is presented. Exemplary results of a cohort study in 13 consecutive patients are presented. The procedure is challenging, but our own results-as well as reports from the literature-are encouraging. The r‑TAR is becoming the pinnacle procedure for abdominal wall reconstruction.Das Prinzip der gezielten Trennung bzw. Schwächung einzelner Komponenten der Bauchdecke zur Spannungsentlastung der Medianlinie bei großen abdominellen Rekonstruktionen ist seit über 30 Jahren als anteriore Komponentenseparation (aKS) bekannt und ein etabliertes Verfahren. Auf der Suche nach Alternativen mit geringerer Komplikationsrate wurde die posteriore Komponentenseparation (pKS) entwickelt; der „transversus abdominis release“ (TAR) ist eine nervenschonende Modifikation der pKS. Mit den ergonomischen Ressourcen der Robotik (z. B. abgewinkelte Instrumente) kann der TAR minimal-invasiv durchgeführt werden (r-TAR): Bruchlücken von bis zu 14 cm lassen sich verschließen und ein großes extraperitoneales Netz implantieren. In diesem Videobeitrag wird die Versorgung großer Inzisionalhernien in der r‑TAR-Technik präsentiert. Exemplarisch werden die Ergebnisse einer Kohortenstudie an 13 konsekutiven Patienten vorgestellt. Der Eingriff ist anspruchsvoll, die eigenen Ergebnisse sind – wie auch die Berichte aus der Literatur – ermutigend. Der r‑TAR entwickelt sich zur Königsdisziplin der Bauchdeckenrekonstruktion.

Background 40–50% of patients with colorectal cancer (CRC) will develop liver metastases (CRLM) during the course of the disease. One third of these patients will additionally develop pulmonary metastases. Methods 137 consecutive patients with CRLM, were analyzed regarding survival data, clinical, histological data and treatment. Results were stratified according to the occurrence of pulmonary metastases and metastases resection. Results 39% of all patients with liver resection due to CRLM developed additional lung metastases. 44% of these patients underwent subsequent pulmonary resection. Patients undergoing pulmonary metastasectomy showed a significantly better five-year survival compared to patients not qualified for curative resection (5-year survival 71.2% vs. 28.0%; p = 0.001). Interestingly, the 5-year survival of these patients was even superior to all patients with CRLM, who did not develop pulmonary metastases (77.5% vs. 63.5%; p = 0.015). Patients, whose pulmonary metastases were not resected, were more likely to redevelop liver metastases (50.0% vs 78.6%; p = 0.034). However, the rate of distant metastases did not differ between both groups (54.5 vs.53.6; p = 0.945). Conclusion The occurrence of colorectal lung metastases after curative liver resection does not impact patient survival if pulmonary metastasectomy is feasible. Those patients clearly benefit from repeated resections of the liver and the lung metastases.

Background Multimodal treatment strategies – perioperative chemotherapy (CTx) and radical surgery – are currently accepted as treatment standard for locally advanced gastric cancer. However, the role of adjuvant postoperative CTx (postCTx) in addition to neoadjuvant preoperative CTx (preCTx) in this setting remains controversial. Methods Between 4/2006 and 12/2013, 116 patients with locally advanced gastric cancer were treated with preCTx. 72 patients (62 %), in whom complete tumor resection (R0, subtotal/total gastrectomy with D2-lymphadenectomy) was achieved, were divided into two groups, one of which receiving adjuvant therapy (n = 52) and one without (n = 20). These groups were analyzed with regard to survival and exclusion criteria for adjuvant therapy. Results Postoperative complications, as well as their severity grade, did not correlate with fewer postCTx cycles administered (p = n.s.). Long-term survival was shorter in patients receiving postCTx in comparison to patients without postCTx, but did not show statistical significance. In per protocol analysis by excluding two patients with perioperative death, a shorter 3-year survival rate was observed in patients receiving postCTx compared to patients without postCTx (3-year survival: 71.2 % postCTx group vs. 90.0 % non-postCTx group; p = 0.038). Conclusion These results appear contradicting to the anticipated outcome. While speculative, they question the value of post-CTx. Prospectively randomized studies are needed to elucidate the role of postCTx.

Background The management of rectal cancer (RC) has substantially changed over the last decades with the implementation of neoadjuvant chemoradiotherapy, adjuvant therapy and improved surgery such as total mesorectal excision (TME). It remains unclear in which way these approaches overall influenced the rate of local recurrence and overall survival. Methods Clinical, histological and survival data of 658 out of 662 consecutive patients with RC were analyzed for treatment and prognostic factors from a prospectively expanded single-institutional database. Findings were then stratified according to time of diagnosis in patient groups treated between 1993 and 2001 and 2002 and 2010. Results The study population included 658 consecutive patients with rectal cancer between 1993 and 2010. Follow up data was available for 99.6% of all 662 treated patients. During the time period between 2002 and 2010 significantly more patients underwent neoadjuvant chemoradiotherapy (17.6% vs. 60%) and adjuvant chemotherapy (37.9% vs. 58.4%). Also, the rate of reported TME during surgery increased. The rate of local or distant metastasis decreased over time, and tumor related 5-year survival increased significantly with from 60% to 79%. Conclusion In our study population, the implementation of treatment changes over the last decade improved the patient’s outcome significantly. Improvements were most evident for UICC stage III rectal cancer.

RACIONAL: O uso de telas cirúrgicas para a correção de defeitos da parede abdominal vem ocupando cada vez mais espaço nas operações no mundo todo. OBJETIVO: Estudar duas telas cirúrgicas distintas (Proceed® e Ultrapro®) na cicatrização de defeito produzido em parede abdominal de ratos avaliando-se macroscopia, tensiometria e microscopia. MÉTODO: A amostra foi constituída por 32 ratos Wistar, divididos em dois grupos de 16 animais e quatro subgrupos de oito, submetidos à eutanásia com 15 dias e 30 dias para avaliação. As variáveis macroscópicas foram: presença de hematoma nos bordos da sutura da tela cirúrgica na parede abdominal, união entre a tela e a borda da ferida, presença de infecção no sítio cirúrgico, presença de fístulas de vísceras com a tela cirúrgica, presença de aderências dentro da cavidade abdominal e presença de hérnia incisional. Na microscopia avaliaram-se as fases do processo inflamatório da cicatrização, e na tensiometria a força tênsil necessária para ruptura do material. RESULTADOS: A união entre a tela cirúrgica e a borda da ferida foi melhor no subgrupo Ultrapro 15 dias que no Proceed 15 dias; com 30 dias foi igual nos dois subgrupos. Não houve diferença significativa quanto às demais variáveis da macroscopia. Os dois grupos nos dois momentos tiveram a mesma proporção de casos com processo inflamatório crônico, mas houve maior escore de inflamação do Ultrapro 15 dias que do Ultrapro 30 dias. O subgrupo Ultrapro 15 dias mostrou força de ruptura maior que o subgrupo Proceed 15 dias, mas em 30 dias não houve diferença. Ultrapro mostrou força de ruptura igual para os dois momentos, mas Proceed 30 dias mostrou força de ruptura maior que Proceed 15 dias. CONCLUSÃO: As telas são semelhantes nas variáveis analisadas. BACKGROUND: The use of surgical meshes for the repair of abdominal wall defects has been increasing its share in surgeries worldwide. AIM: To study two different surgical meshes (Proceed® and Ultrapro®) on the healing of defects produced in abdominal wall of rats evaluating the macroscopic, microscopic and tensiometric parameters. METHOD: Thirty two Wistar rats were divided into two groups of 16 animals and four groups of eight animals euthanized within 15 and 30 days for evaluation. The macroscopic variables were: the presence of hematoma at the edges of the sutures of the surgical mesh in the abdominal wall; the knitting of the surgical mesh and the edge of the wound; presence of infection at the surgical site; presence of viscera fistulas on the surgical mesh; presence of adhesions in the abdominal cavity; presence of incisional hernia. During microscopy the stages of the inflammatory process of healing where evaluated, and tensiometry evaluated the tensile force required to rupture the material. RESULTS: The knitting of the mesh and the edge of the surgical wound was better in the 15 days Ultrapro subgroup than in the 15 days Proceed subgroup; within 30 days the knitting was similar in both subgroups. There was no significant difference for the variable of macroscopy. Both groups in both periods had the same proportion of cases with chronic inflammation, but there were higher scores of inflammation in the 15 days Ultrapro subgroup than the 30 days Ultrapro subgroup. The subgroup 15 days Ultrapro showed greater rupture strength than the subgroup 15 days Proceed, but in 30 days there were no difference. Ultrapro showed equal rupture strength in both periods, but 30 days Proceed showed greater rupture strength than 15 days Proceed. CONCLUSION: The meshes are similar in variables.

PURPOSE: To compare polyester with absorbable layer prosthesis with collagen-elastin/polypropylene prosthesis in the repair of abdominal wall defects. METHODS: The 16 studied rabbits were divided in groups A and B (euthanized on the 30th and 60th days, after the implant of the mesh). The animals underwent laparotomy and received a 2cm wall "defect" on each side of the Alba linea. The repair was made with the suture of a polyester mesh with absorbable film on the left side of the Alba Linea and with collagen-elastin/polypropylene mesh on the right side. Adherences were classified according to Nair Score and microscopic evaluation observing types I and III collagen formation and other immunohistochemical analyses. RESULTS: There were no significant differences in adhesion formation. The collagen type I showed higher deposition in polyester with absorbable layer. In group B, the difference between the meshes was significant, with higher collagen III deposition in polyester with absorbable layer (60º P.O.). About the metalloproteinases, the presence of MMP -1 and MMP-8 were about the same; the expression of MMP-13 increased near to the 60th day. CONCLUSIONS: There is no significant difference between the two meshes in adhesion formation and immunohystochemical evaluation. The polyester mesh resorbable film presented a higher deposition of collagen. OBJETIVO: Comparar a tela poliéster com lâmina absorvível e a confeccionada pela aposição da tela de colágeno-elastina/polipropileno no reparo de lesões da parede abdominal. MÉTODOS: Foram avaliados 16 coelhos, divididos aleatoriamente em dois grupos e submetidos a eutanásia no 30º e no 60º dia P.O. Os animais foram submetidos à laparotomia e confecção de dois defeitos triangulares de 2 cm, de espessura total, na parede abdominal ventral. A correção foi realizada através de fixação, na metade esquerda da linha alba, da tela de poliéster com lâmina absorvível e na direita, tela composta por colágeno-elastina/polipropileno. As aderências foram classificadas conforme o escore de Nair e a avaliação microscópica incluiu avaliação do colágeno (tipos I e III) e imunoistoquímica. RESULTADOS: Não houve diferença quanto à formação de aderências. O colágeno tipo I apresentou maior concentração com a tela Poliéster com lâmina absorvível. No grupo B foi encontrada diferença significativa entre as telas, com aumento do colágeno tipo III com a tela Poliéster com lâmina absorvível (60º P.O.). A avaliação das metaloproteinases MMP-1 e MMP-8 não apresentaram diferença significativa; a MMP-13 apresentou aumento significativo na imunoexpressão no 60º P.O. CONCLUSÕES: Não há diferença significativa entre as telas quanto às aderências e avaliação imunoistoquímica. A tela de poliéster com lâmina absorvível apresenta maior formação de colágeno.

RACIONAL: O câncer gástrico continua sendo objeto de grande número de estudos que tentam entender melhor sua gênese, e conseqüentemente propor novos tratamentos. Na atualidade destaque especial está sendo dado na marcação imunoistoquímica onde estão sendo utilizados marcadores diversos sem se saber ainda qual ou quais são os mais efetivos no diagnóstico. OBJETIVO: Identificar e quantificar por citometria de imagem a expressão dos marcadores de angiogênese Fator VIII e CD-34 em tumores gástricos. MÉTODOS:Foram utilizados 29 casos de adenocarcinomas gástricos, todos oriundos de material arquivado e conservado em blocos de parafina. Após a desparafinização, realizou-se coloração com marcadores imunoistoquímicos de angiogênese Fator VIII e CD-34, e as lâminas coradas foram submetidas a estudo citofotométrico de imagem em sistema informatizado SAMBA 4000. RESULTADOS: Foram comparados os dois parâmetros oferecidos pelo método, índice de marcagem e densidade óptica, apenas nos 17 casos em que ocorreu identificação de ambos marcadores. Observou-se expressão numericamente significativa de ambos, porém o Fator VIII apresentou melhor média de densidade óptica, enquanto o CD-34 melhor resultado quanto ao índice de marcagem. CONCLUSÃO:A citometria de imagem foi capaz de identificar e quantificar a expressão do Fator VIII e CD-34 de maneira confiável e satisfatória demonstrando presença de angiogênese, sendo que o Fator VIII marcou menores áreas com melhor qualidade e o CD-34 marcou maiores áreas com menor qualidade. BACKGROUND: Gastric cancer remains a subject of great interest in several studies searching for understanding its genesis and, therefore, proposing new treatments. Currently special credits have been given to immunohistochemistry as various markers are being tested with uncertain efficacy on diagnosis. AIM: To identify and quantify the expression of Factor VIII and CD-34, angiogenesis markers, in gastric tumors by imaging cytometry. METHODS: Twenty-nine 29 gastric adenocarcinomas sampled-tissue in paraffin blocks underwent immunohistochemical angiogenesis markers evaluation for factor VIII and CD-34 and the marked slides were submitted to SAMBA 4000 reading. RESULTS:The investigation method offered two comparison parameters, marking index and optical density, showing up both in 17 cases. Significant numerical expression had been observed for both markers, but Factor VIII showed a better optical density average while CD-34 presented a better result for marking index. CONCLUSIONS: Image cytometry identified and quantified Factor VIII and CD-34 expressions in a reliable and satisfactory manner, revealing the presence of angiogenesis. Factor VIII enhanced in smaller areas with better quality while CD-34 enhanced greater areas with lower quality.

INTRODUÇÃO: A espécie vegetal Jatropha gossypiifolia L., conhecida também como pião roxo, é utilizada na medicina popular como cicatrizante, anti-hipertensivo, purgativo e diurético. OBJETIVO: Avaliar a influência da administração intraperitoneal da Jatropha gossypiifolia L., na cicatrização de suturas da parede abdominal ventral de ratos, observando-se os seus aspectos macroscópicos, tensiométricos e microscópicos. MÉTODOS: Foram utilizados no procedimento 40 ratos da linhagem Wistar, machos, distribuídos em dois grupos de 20. Após incisão da parede e abertura da cavidade abdominal, foi instilado 1 ml/kg/peso de cloreto de sódio a 0,9% no grupo controle e no grupo Jatropha o extrato bruto etanólico da Jatropha gossypiifolia L., na concentração de 1 ml/kg/ peso. Realizou-se a sutura da parede abdominal com fio de polipropileno, com pontos separados. Os animais foram avaliados na sua evolução pós-operatória e mortos em dois subgrupos, no 3º e 7º dia. Analisou-se a parede abdominal ventral macroscopicamente, mediu-se a força de resistência a tensão e foram estudados os aspectos histológicos do reparo cicatricial. RESULTADOS: No exame macroscópico encontraram-se aderências mais intensas nos subgrupos Jatropha no 3º e 7º dia. A avaliação tensiométrica foi significantemente maior nos subgrupos Jatropha no 3º e 7º dia. A avaliação histológica comparativa entre os subgrupos demonstrou que o processo inflamatório agudo foi significantemente maior no subgrupo Jatropha no 3º e 7º dia; a neoformação capilar foi significantemente maior no 3º dia pós-operatório do subgrupo Jatropha sendo os outros parâmetros histológicos semelhantes. CONCLUSÃO: O uso do extrato bruto de Jatropha gossypiifolia L. intraperitoneal não demonstrou melhora significativa no processo de cicatrização da sutura da parede abdominal ventral de ratos com a dose e concentração utilizadas. INTRODUCTION: The Jatropha gossypiifolia L., which is used in popular medicine is considered to have good diuretic effect in hypertension and is also used as a laxative drug. It seems to have a healing effect, although not proved till now. PURPOSE: To evaluate the influence of intraperitoneum administration of Jatropha Gossypiifolia L., in suture healing of ventral abdominal wall of rats, through tensiometric measurement, macro and microscopic aspect of post-operative period. METHODS: Forty wistar male rates were allocated in two groups of 20 animals . After the incision and exposure of abdominal cavity 1 ml/kg/weight of 0,9% sodium chloride solution was injected in control group, and in the other one the injection was of 1 ml/kg/weight of a gross ethanol extract of Jatropha gossypiifolia L. The suture of the abdominal wall was than performed with polypropylene separated stitches. The animals were followed-up and killed in the third and seventh days. The ventral abdominal wall was macroscopically analyzed, the resistance strength to strain was measured and it was also studied the histological aspects. RESULTS: On macroscopic examination more intense adhesion was found on the group of Jatropha in both third and seventh post-operative days. The strain evaluation was meanly greater on Jatropha group also in third and seventh days. CONCLUSION: The histological comparative analysis between the different groups showed that the acute inflammatory process was meanly greater for the Jatropha group in third and seventh post-operative days. The vascular neoformation was significantly greater in third pos-operative day of Jathopha group; the other histological parameters were just alike. The intraperitoneum injection of Jatropha extract did not have any significant improvement for the wound healing on ventral abdominal wall on the evaluated animals in this study, no matter if analyzed at the third or seventh pos-operative days.
