Abstract

International audience; The rapidity of the diagnosis of invasive candidiasis (IC) is crucial to allow the early introduction of antifungal therapy that dramatically increases the survival rate of patients. Early diagnosis is unfortunately often delayed because Candida blood culture, the gold standard diagnostic test, is positive in only 50% of cases of IC and takes several days to obtain this result. Complementary non-culture-based methods relying on the detection of Candida cell wall polysaccharides in the serum, β-glucans and mannans, by enzymatic and immunological reagents have been successfully developed to allow a more efficient patients care. We have previously demonstrated that detection of circulating glycans by mass spectrometry could provide a reliable and cost-effective early diagnosis method called MS-DS for Mass Spectrometry of Di-Saccharide. Here, by comparing patient's sera and Candida albicans strains deficient in carbohydrates synthesis, we demonstrate that trehalose derived from fungal metabolism can be specifically targeted by MS-DS to allow early diagnosis. In particular, the use of C. albicans strains deficient in the synthesis of trehalose synthesizing enzymes Tps1 and Tps2 show that MS-DS results were correlated to the metabolism of trehalose. Finally, we demonstrate that the performance of the IC diagnosis can be significantly improved by using high resolution mass spectrometry, which opens new perspectives in the management of the disease.

Abstract

International audience; Interlaboratory evaluations of Mucorales qPCR assays were developed to assess the reproducibility and performance of methods currently used. The participants comprised 12 laboratories from French university hospitals (nine of them participating in the Modimucor study) and 11 laboratories participating in the Fungal PCR Initiative. For panel 1, three sera were each spiked with DNA from three different species (Rhizomucor pusillus, Lichtheimia corymbifera, Rhizopus oryzae). For panel 2, six sera with three concentrations of R. pusillus and L. corymbifera (1, 10, and 100 genomes/ml) were prepared. Each panel included a blind negative-control serum. A form was distributed with each panel to collect results and required technical information, including DNA extraction method, sample volume used, DNA elution volume, qPCR method, qPCR template input volume, qPCR total reaction volume, qPCR platform, and qPCR reagents used. For panel 1, assessing 18 different protocols, qualitative results (positive or negative) were correct in 97% of cases (70/72). A very low interlaboratory variability in Cq values (SD = 1.89 cycles) were observed. For panel 2 assessing 26 different protocols, the detection rates were high (77-100%) for 5/6 of spiked serum. There was a significant association between the qPCR platform and performance. However, certain technical steps and optimal combinations of factors may also impact performance. The good reproducibility and performance demonstrated in this study support the use of Mucorales qPCR as part of the diagnostic strategy for mucormycosis.

National audience; Que sont les « communs », les « biens communs », les « patrimoines communs », les « choses communes » ? Ces notions sont fortement mobilisées aujourd’hui dans nombre de disciplines des sciences sociales et actions de citoyens. Leur poussée traduit une évolution des pratiques sociales : sous la pression de la crise écologique et de la transformation numérique notamment, les biens seraient davantage mis en partage. Logiciel libre, encyclopédie et habitat participatif, vélos ou voitures en usage successif, entreprise qui serait le « bien commun » de toutes les parties prenantes : les notions des communs intéressent les domaines de la culture, de la protection de l’environnement, de l’urbanisme, de la santé, de l’innovation, du travail, etc. La mobilisation est intense car elles autorisent à penser le changement social sur la base d’un réinvestissement du collectif, des communautés, de l’usage et du partage. Elles permettent de proposer des réinterprétations des valeurs fondatrices des sociétés contemporaines tels le rôle de l’État, de la propriété et des formes d’expression de la démocratie. Elles appellent à la fois une réflexion théorique, un débat politique et se concrétisent dans des expériences citoyennes. Ce dictionnaire, à mi-chemin entre le vocabulaire et l’encyclopédie, se veut un outil de compréhension de l’ensemble de ces phénomènes.

Abstract

International audience; BACKGROUND: The use of isavuconazole is approved for the management of invasive aspergillosis and mucormycosis, only in adults, as no paediatric pharmacology studies have been reported to date. Very few paediatric cases have been published concerning the use of isavuconazole. Amphotericin B is the only antifungal agent recommended in paediatric mucormycosis, but adverse effects and especially nephrotoxicity, even with the liposomal formulation, could be problematic. In this context, the use of other antifungal molecules active on Mucorales becomes needful.METHODS: We describe a case of mucormycosis with rapid onset dissemination in a 3-year-old girl recently diagnosed with acute lymphocytic leukaemia. She was successfully treated with isavuconazole alone and then in combination with liposomal amphotericin B. Isavuconazole therapy was guided by therapeutic drug monitoring.CONCLUSIONS: This case offers new perspectives on the potential use of isavuconazole in children with mucormycosis, as an alternative or adjunct to liposomal amphotericin B.

Abstract

Ce dictionnaire est né de la rencontre de juristes, d'économistes et d'historiens autour d'un projet de l'Agence nationale de la recherche (ANR) axé sur les communs de la connaissance. Cette recherche, entreprise sous une perspective pluridisciplinaire, impliquait tout d'abord de s'entendre sur les notions qu'utilise chaque discipline dans la réflexion sur les communs. Si nous sommes parties de l'approche développée par Elinor Ostrom et l'École de Bloomington, il ne s'est pas agi de réduire le champ de la confrontation à une seule notion de "commun", mais d'élargir à et de restituer toutes les notions de "commun" ainsi que les notions voisines ou concepts fondamentaux réinvestis à l'aune des communs. L'objectif était aussi de faire une place importante aux expériences novatrices, qu'elles soient savantes ou citoyennes.

Abstract

International audience; This article proposes an assessment of the ingestion doses potentially received by people living in the Japanese areas most severely affected by the radioactive deposits due to the Fukushima nuclear accident. The assessment distinguishes two consecutive periods the first 2 months (March 15th to May 15th 2011) and the rest of the year 2011, according to the two main foodstuff contamination periods identified (Renaud et al., 2013). On the basis of the worst-case hypothesis that can reasonably be made, the estimated doses are much lower than they might have been in other circumstances, on account of generally moderate levels of contamination for most foods, early evacuation of the most severely affected areas and the consumption bans ordered by the Japanese authorities. Thus, a single ingestion of 100 g of leafy vegetables in the early days by a one-year-old child living in non-evacuated localities near Iitate and Kawamata could have led to an equivalent dose to the thyroid of about 25 mSv; and to a thyroid dose of about 140 mSv for the rather unrealistic scenario of a daily consumption. Because of the rapid decrease in the contamination of vegetables and a much weaker contamination of other crops, and thanks to measures taken by the authorities (prohibition of consumption, use of certain fodder, etc.), the ingestion doses potentially received during the remainder of the year 2011 are estimated to be below 1 mSv. In the event that sales limits would have been ignored, only repeated consumption of mushrooms would have led to doses above that level. Even with the worst-case hypotheses, the doses potentially caused by ingestion are much lower than those resulting from outdoor exposure to radioactive deposits approximately several millisieverts in a year for the population living in the non-evacuated localities near Iitate and Kawamata. © EDP Sciences 2013.

International audience; This paper describes a dynamic compartment model (TOCATTA) that simulates tritium transfer in agricultural plants of several categories including vegetables, pasture and annual crops, exposed to time-varying HTO concentrations of water vapour in the air and possibly in irrigation and rainwater. Consideration is also given to the transfer pathways of HTO in soil. Though the transfer of tritium is quite complex, from its release into the environment to its absorption and its incorporation within the organic material of living organisms, the TOCATTA model is relatively simple, with a limited number of compartments and input parameters appropriate to its use in an operational mode. In this paper, we took the opportunity to have data obtained on an ornamental plant - an indoor palm tree - within an industrial building where tritium was released accidentally over several weeks (or months). More specifically, the model's ability to provide hindsight on the chronology of the release scenario is discussed by comparing model predictions of TFWT and OBT activity concentrations in the plant leaves with measurements performed on three different leaves characterized by different developmental stages. The data-model comparison shows some limitations, mainly because of a lack of knowledge about the initial conditions of the accident and when it actually started and about the processes involved in the transfer of tritium. Efforts are needed in both experimental and modelling areas for future evaluation of tritium behaviour in agricultural soil and plants exposed to gaseous HTO releases and/or to irrigation with contaminated water. © 2013 Elsevier Ltd.

Abstract

In food safety, the respect of the cold chain is of prime importance. This paper presents the results of a survey on the French cold chain of chilled products sold in self service counters of super and hyper markets. Continuous temperature monitoring of 314 products all along the cold chain, from the end of production to the consumer, enabled us to create a database representing more than 3800 days of record and describing more than 2500 identified steps of the cold chain, among which 755 refrigerated transports, 113 warehouses, 344 display cabinets, 302 transports after shopping and 251 domestic refrigerators. Coupled with a simple integration model temperature and based on basic concepts of predictive microbiology, these results were used to evaluate the consumed time / temperature capital defined by the industrials as they establish the product shelf life. These results are of high importance as the current way to establish these shelf lives remains empirical today., Pour la sécurité des aliments, le respect de la chaîne du froid est d'importance primordiale. Ce papier présente les résultats d'une étude sur la chaîne du froid en France de produits refrigérés vendus en libre-service dans les super et hypermarchés et les supérettes. L'enregistrement des températures en continu de 314 produits tout au long de la chaîne du froid, de la fin de production jusqu'au consommateur, nous a permis de créer une base de données représentant plus de 3800 jours d'enregistrement et décrivant plus que 2500 maillons identifiés de la chaîne du froid, parmi lequel 755 transports frigorifiques, 113 entrepôts, 344 meubles de vente, 302 transports après l'achat et 251 réfrigérateurs domestiques. Couplé avec un modèle simple d'intégration de température et basé sur des concepts de base de microbiologie prévisionnelle, ces résultats ont été utilisés pour évaluer le capital temps/température consommé en fonction de celui défini par l'industriel quand il établit la DLC du produit. Ces résultats présentent une réelle importance car la façon actuelle d'établir ces DLC reste aujourd'hui empirique.
