The versatile combination of metal nanoparticles with chemotherapy agents makes designing multifunctional drug delivery systems attractive. In this work, we reported cisplatin’s encapsulation and release profile using a mesoporous silica-coated gold nanorods system. Gold nanorods were synthesized by an acidic seed-mediated method in the presence of cetyltrimethylammonium bromide surfactant, and the silica-coated state was obtained by modified Stöber method. The silica shell was modified first with 3-aminopropyltriethoxysilane and then with succinic anhydride to obtain carboxylates groups to improve cisplatin encapsulation. Gold nanorods with an aspect ratio of 3.2 and silica shell thickness of 14.74 nm were obtained, and infrared spectroscopy and ζ potential studies corroborated surface modification with carboxylates groups. On the other hand, cisplatin was encapsulated under optimal conditions with an efficiency of ~58%, and it was released in a controlled manner over 96 h. Furthermore, acidic pH promoted a faster release of 72% cisplatin encapsulated compared to 51% in neutral pH.

Psychology (miscellaneous) ,Music - Abstract
Sensorimotor synchronization (SMS), the coordination of body movements with external stimuli (e.g., dancing, clapping along with music, sport activities), increases throughout childhood when assessed by finger-tapping (FT) tasks following rhythmic auditory cues using metronomes. However, metronomes may support less rhythmic entrainment than naturalistic auditory cues like music, so they may be less effective in showing developmental improvement. This study investigated the effects of age and sex on SMS comparing metronome versus music stimuli in four auditory FT tasks with regular fast (inter-stimulus interval [ISI] = 333 ms) and slow tempi (ISI = 1,000 ms). Participants were 305 typically developing children aged 6 to 11 years. We found improvement (lower variability) in performance with increasing age irrespective of the metronome or music FT and sex, particularly between 6 and 9 years, after which performance stabilized. Nonetheless, mostly irrespective of age, performance for music and metronome stimuli showed differences in synchronization accuracy and variability, indicating that the type of rhythmic stimuli affects performance. A developmental effect was also found in inter-tap variability (but not inter-tap accuracy). Overall, developmental effects were more robust in FT variability (motor implementation) although responses to music and metronome stimuli differed in ways that may be relevant to SMS deficits in clinical populations.

Introduction: Measles is an immunopreventable viral disease, acute exanthematous and extremely transmissible. Infection occurs directly, from person to person, through respiratory secretions. The measles vaccine is low-cost and effective, but measles still affects around 40 million people worldwide and is responsible for at least 800,000 deaths, mainly in developing countries. Objective: To analyze vaccination coverage and measles incidence in Northern Brazil. Methods: This is an ecological study. Secondary data were used regarding vaccination coverage of the viral double, viral triple and viral tetra and incidence of measles in residents in the States of the Northern Region, Brazil, from 2010 to 2018. Results: Confirmed measles cases in 2010 to 2018 in the Northern region of Brazil it registered 10,249, were reported in Amazonas (95.65%), Roraima (3.53%), Pará (0.80%) and Rondônia (0.02%). Outbreaks of measles cases in the Northern Region were reported in 2018, the highest incidence rate was in Amazonas with 237.7. Vaccination coverage in the North Region varied between the lowest rate in 2013 with 56.88% and the highest vaccination coverage achieved in 2010, with 104.02%. Conclusion: It was possible to observe a reduction in vaccination coverage during the study period, not reaching the percentage recommended by the Ministry of Health and increasing the incidence of measles in the Northern Region of Brazil and these indicators are important to determine the strategies to be carried out by the National Program for Immunization and Epidemiological Surveillance.

- Abstract
Active immunization through vaccination is the most effective method to reduce the spread of infectious and contagious diseases. Thus, this study analyzed the vaccination rates in the state of Maranhão in the period 2013–2022, in order to understand the implications of reducing this coverage for the reemergence of diseases that had been eradicated until then and the main outcomes. This is an analytical research of the ecological type of time series carried out in the state of Maranhão. (BCG), human rotavirus vaccine, pneumococcal vaccine 10, pentavalent vaccine (DTP + Hib + HB), meningococcal C vaccine, polio vaccine, and yellow fever (YF) vaccine. in the 19 regional health units of the state (URS). Of the seven immunobiologicals studied, the only ones to reach the recommended target for vaccination coverage were BCG and poliomyelitis, during the period of data analysis. Regarding the trend, none showed an increasing pattern during the analyzed period. Undoubtedly, vaccines are indispensable tools for preventing and combating diseases, in addition to being extremely cost-effective. In view of the results, it can be said that the population is certainly the main protagonist in this scenario, since the success or failure of immunization actions are closely associated with society's adherence. Therefore, supporting and encouraging actions that contribute to community awareness and access to vaccines are fundamental in this context.

Abstract This article presents evidences that there is an ongoing cycle of evictions in the Tucunduba River basin, in the Brazilian city of Belém (PA), where urbanization work has been taken place over several years, which has caused an increasing number of displaced families. It is understood that the housing provision shortage has led to an increase in cases of judicialization, which thereby contributes to further irregular occupations and, thus, a continuation of precariousness and informality. The reasons that contribute to the situation have been classified into four groups: operational/planning; political/administrative; ideological; and extra-legal. Empirical and documentary sources, interviews with technicians from public bodies and with the population threatened by eviction, and an analysis of the judicial processes demonstrate that while evictions intensify the condition of vulnerability of the families, they also serve to maintain structures that support peripheral capitalism.

General Medicine - Abstract

- Abstract
Governance has aroused interest from the most different areas of knowledge. In the legal field, it is not different. Connected to the ideas of transparency, accountability, responsiveness, regulatory improvement, integrity, reliability and participation, good governance, as it has been forged by legislation and doctrine, starts to reflect a change in the decision-making paradigm of Public Administration in the scope of public management, with peculiarities when transposed to the Judiciary. Several public judicial policies have emerged with the express invocation of achieving good governance in the Judiciary, such as the National Policy of Priority Attention to the First Degree of Jurisdiction, instituted by the CNJ in 2014. This dissertation aims to analyze the cycle of this public policy, in order to verify whether, as formulated by the CNJ and executed in the TJSP, it has effectively incorporated elements of good governance in the administration of the Judiciary. To achieve this conclusion, the dissertation will, at first, focus on a dogmatic approach, with a view to establishing a theoretical framework of good governance in the administration of the Judiciary and, secondly, an empirical approach, with research in the Phenomenal field of Policy, based on the compilation of concrete data extracted from official documents and in minutes of Management Committee meetings held at the CNJ and TJSP with their confrontation with the defined theoretical framework. In conclusion, the main points of the exhibition will be picked up and the main contributions and bottlenecks of the Policy will be extracted from the perspective of good governance for the Judiciary.

To assess the prevalence of burnout, anxiety and depression symptoms, and posttraumatic stress disorder (PTSD) in PICU workers in Brazil during the first peak of the COVID-19 pandemic. To compare the results of subgroups stratified by age, gender, professional category, health system, and previous mental health disorders.Multicenter, cross-sectional study using an electronic survey.Twenty-nine public and private Brazilian PICUs.Multidisciplinary PICU workers.None.Self-reported questionnaires were used to measure burnout (Maslach Burnout Inventory), anxiety and depression (Hospital Anxiety and Depression Scale), and PTSD (Posttraumatic Stress Disorder Checklist for Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition [PCL-5]) in 1,084 respondents. Subjects were mainly young (37.1 ± 8.4 yr old) and females (85%), with a median workload of 50 hours per week. The prevalence of anxiety and depression was 33% and 19%, respectively, whereas PTSD was 13%. The overall median burnout scores were high in the emotional exhaustion and personal accomplishment dimensions (16 [interquartile range (IQR), 8-24] and 40 [IQR, 33-44], respectively) whereas low in the depersonalization one (2 [IQR, 0-5]), suggesting a profile of overextended professionals, with a burnout prevalence of 24%. Professionals reporting prior mental health disorders had higher prevalence of burnout (30% vs 22%; p = 0.02), anxiety (51% vs 29%; p0.001), and depression symptoms (32.5% vs 15%; p0.001), with superior PCL-5 scores for PTSD (p0.001). Public hospital workers presented more burnout (29% vs 18.6%, p0.001) and more PTSD levels (14.8% vs 10%, p = 0.03). Younger professionals were also more burned out (p0.05 in all three dimensions). The prevalence of mental health disorders in Brazilian PICU workers during the first 2020 peak of COVID-19 was as high as those described in adult ICU workers. Some subgroups, particularly those reporting previous mental disorders and younger professionals, should receive special attention to prevent future crises.

Executive functions (EFs) are cognitive skills that regulate thoughts and behavior. The seminal EF unity and diversity theoretical framework proposes the existence of three correlated EF latent domains (inhibition, updating, and switching) that become distinguishable from a certain moment during adolescence, but it is unclear how age and socioeconomic status (SES) affect these abilities. Here, we assessed 407 9-15-year-old Iranians of variable SES using an open-access battery of executive function tests that includes two tasks of each EF domain and allows for sociocultural adaptations regarding language and stimuli. Various EF model configurations proposed in the literature were tested (one, two and three EF latent factor, nested and bifactor-S-1 models) using confirmatory factor analyses. In addition, to explore the unbiased effects of age and SES, we performed invariance testing (across age and SES) using multiple indicators multiple causes (MIMIC) model to the best fitting model solution. The three-correlated EF factor model had the best fit and was mostly invariant across age and SES, with all three EF latent traits improving with age, while SES exerted only minimal positive effects on shifting and updating. We concluded that the three separable EF domains, found in adults and adolescents of other ages from different populations, can already be detected from the beginning of adolescence when culturally and psychometrically appropriate EF tasks are used. Additionally, these abilities continue to improve with age and are little affected by SES, suggesting that the unity and diversity framework is useful to study the cross-country generality of EF development.

The umbrella-term ‘executive functions’ (EF) includes various domain-general, goal-directed cognitive abilities responsible for behavioral self-regulation. The influential unity and diversity model of EF posits the existence of three correlated yet separable executive domains: inhibition, shifting and updating. These domains may be influenced by factors such as socioeconomic status (SES) and culture, possibly due to the way EF tasks are devised and to biased choice of stimuli, focusing on first-world testees. Here, we propose a FREE (Free Research Executive Function Evaluation) test battery that includes two open-access tasks for each of the three abovementioned executive domains to allow latent variables to be obtained. The tasks were selected from those that have been shown to be representative of each domain, that are not copyrighted and do not require special hardware/software to be administered. These tasks were adapted for use in populations with varying SES/schooling levels by simplifying tasks/instructions and using easily recognized stimuli such as pictures. Items are answered verbally and tasks are self-paced to minimize interference from individual differences in psychomotor and perceptual speed, to better isolate executive from other cognitive abilities. We tested these tasks on 146 early adolescents (aged 9–15 years) of both sexes and varying SES, because this is the age group in which the executive domains of interest become distinguishable and in order to confirm that SES effects were minimized. Performance was determined by Rate Correct Scores (correct answers divided by total time taken to complete blocks/trial), which consider speed-accuracy trade-offs. Scores were sensitive to the expected improvement in performance with age and rarely/inconsistently affected by sex and SES, as expected, with no floor or ceiling effects, or skewed distribution, thus suggesting their adequacy for diverse populations in these respects. Using structural equation modeling, evidence based on internal structure was obtained by replicating the three correlated-factor solution proposed by the authors of the model. We conclude that the FREE test battery, which is open access and described in detail, holds promise as a tool for research that can be adapted for a wide range of populations, as well as altered and/or complemented in coming studies., Fil: Zanini, Gislaine A. Intracranial hematomas (ICH) are a frequent condition in neurosurgical and neurological practices, with several mechanisms of primary and secondary injury. Experimental research has been fundamental for the understanding of the pathophysiology implicated with ICH and the development of therapeutic interventions. To date, a variety of different animal approaches have been described that consider, for example, the ICH evolutive phase, molecular implications and hemodynamic changes. Therefore, choosing a test protocol should consider the scope of each particular study. The present review summarized investigational protocols in experimental research on the subject of ICH. With this subject, injection of autologous blood or bacterial collagenase, inflation of intracranial balloon and avulsion of cerebral vessels were the models identified. Rodents (mice) and swine were the most frequent species used. These different models allowed improvements on the understanding of intracranial hypertension establishment, neuroinflammation, immunology, brain hemodynamics and served to the development of therapeutic strategies.

Background Human milk is the best food for infants; however, when breastfeeding is not possible, pasteurized milk from human milk banks is the best alternative. Little has been reported about variations in the bacterial microbiota composition of human milk after pasteurization. Research aim To characterize and compare the bacterial microbiota composition and diversity within human milk among Mexican mothers before and after the Holder pasteurization process. Methods A cross-sectional, observational, and comparative design was used. The effect of the pasteurization process on the bacterial composition and diversity of human milk samples of donors ( N = 42) from a public milk bank was assessed before and after pasteurization by high throughput deoxyribonucleic acid sequencing of V3-16S rRNA gene libraries. Sequencing data were examined using the Quantitative Insights into Microbial Ecology software and Phyloseq in R environment. Results A varied community of bacteria was found in both raw and pasteurized human milk. The bacterial diversity of the milk samples was increased by the pasteurization, where some thermoduric bacteria of the phyla Proteobacteria, Firmicutes, and Actinobacteria were more abundant. The source tracker analysis indicated that at most 1.0% of bacteria may have come from another source, showing the safety of the process used to treat milk samples. Conclusion The pasteurization process increased the bacterial diversity. We selected taxa capable of surviving the process, which could proliferate after the treatment without being a risk for infants.

Made available in DSpace on 2021-06-25T10:56:44Z (GMT). No. of bitstreams: 0 Previous issue date: 2021-01-01 Animal models are widely used for studying diabetes in translational research. However, methods for induction of diabetes are conflicting with regards to their efficacy, reproducibility and cost. A comparison of outcomes between the diabetic models is still unknown, especially full-term pregnancy. To understand the comparison, we analyzed the streptozotocin (STZ)-induced diabetes at three life-different moments during the neonatal period in Sprague–Dawley female rats: at the first (D1), second (D2) and fifth (D5) day of postnatal life. At adulthood (90 days; D90), the animals were submitted to an oral glucose tolerance test (OGTT) for diabetic status confirmation. The diabetic and control rats were mated and sacrificed at full-term pregnancy for different analyses. Group D1 presented a higher mortality percentage after STZ administration than groups D2 and D5. All diabetic groups presented higher blood glucose levels as compared to those of the control group, while group D5 had higher levels of glycemia compared with other groups during OGTT. The diabetic groups showed impaired reproductive outcomes compared with the control group. Group D1 had lower percentages of mated rats and D5 showed a lower percentage of a full-term pregnancy. Besides that, these two groups also showed the highest percentages of inadequate fetal weight. In summary, although all groups fulfill the diagnosis criteria for diabetes in adult life, in our investigation diabetes induced on D5 presents lower costs and higher efficacy and reproducibility for studies involving diabetes-complicated pregnancy. BACKGROUND: Acquisition of HIV primary drug-resistant (PDR) infection can lead to poor virologic and clinical outcomes in individuals and hampers public health efforts in epidemic control. Monitoring PDR in HIV-positive blood donors can be used to inform nationwide trends in the spread of drug-resistant HIV strains. METHODS: We conducted a cross-sectional study using genetic sequence analysis to assess HIV pol sequences, PDR, and risk factors for infection using audio computer-assisted structured interviews (ACASI) in four large blood centers in Brazil from 2007 to 2017. RESULTS: Of 716 HIV-positive blood donors, 504 (70.4%) were successfully sequenced. HIV clade B (73.2%) was the most prevalent subtype, followed by a mix of non-B (21.2%) sub-types. A two-fold increase (from 4% to 8%) in recombinants prevalence was observed during the study period. Sixty-four (12.7%) presented PDR. Overall, HIV PDR prevalence remained stable during the study period. Drug resistance mutations for non-nucleoside reverse transcriptase inhibitors were found in 39 (7.7%) donors, while for nucleoside reverse transcriptase inhibitors were found in 26 (5.1%), and for protease inhibitors in 24 (4.8%) of HIV-infected donors. We did not find statistically significant differences in demographics, behavioral risk factors, or HIV genotypes when comparing volunteers with and without PDR. CONCLUSION: The HIV PDR rate among donors remained stable during the study period. HIV-positive blood donors can be an informative population to monitor primary HIV resistance and ultimately may help to increase the knowledge and awareness of HIV risk factors and PDR.

ABSTRACT Dome-shaped macula consists of the convex elevation of the macular region, found mainly in patients with high myopia. The clinical significance and the correlation with other ocular pathologies are still uncertain. This article aims to describe two cases of dome-shaped macula with accumulation of subretinal fluid in high myopic eyes. Those are patients with low visual acuity, myopic fundus and a clear dome-shaped elevation in the macular area at optical coherence tomography. A dome-shaped macula can course with a serous detachment of the sensorineural retina involving the fovea, leading to low visual acuity. Such a pathogenesis is still not well studied. According to the optical coherence tomography, dome-shaped macula can be classified into three morphological types, those two cases being shown in a horizontal, oval arrangement. It is interesting to note that the diagnosis of dome-shaped macula should be considered in patients with high myopia, especially when there is a complaint of low visual acuity, which may be related to the presence of subretinal fluid in cases of dome-shaped macula. As the clinical examination does not allow an adequate diagnosis of the domed macula, the performance of the optical coherence tomography with vertical and horizontal cuts is fundamental in the suspicion of the dome-shaped macula.

ObjectiveTo evaluate whether eculizumab helps patients with anti–acetylcholine receptor–positive (AChR+) refractory generalized myasthenia gravis (gMG) achieve the Myasthenia Gravis Foundation of America (MGFA) post-intervention status of minimal manifestations (MM), we assessed patients' status throughout REGAIN (Safety and Efficacy of Eculizumab in AChR+ Refractory Generalized Myasthenia Gravis) and its open-label extension.MethodsPatients who completed the REGAIN randomized controlled trial and continued into the open-label extension were included in this tertiary endpoint analysis. Patients were assessed for the MGFA post-intervention status of improved, unchanged, worse, MM, and pharmacologic remission at defined time points during REGAIN and through week 130 of the open-label study.ResultsA total of 117 patients completed REGAIN and continued into the open-label study (eculizumab/eculizumab: 56; placebo/eculizumab: 61). At week 26 of REGAIN, more eculizumab-treated patients than placebo-treated patients achieved a status of improved (60.7% vs 41.7%) or MM (25.0% vs 13.3%; common OR: 2.3; 95% CI: 1.1–4.5). After 130 weeks of eculizumab treatment, 88.0% of patients achieved improved status and 57.3% of patients achieved MM status. The safety profile of eculizumab was consistent with its known profile and no new safety signals were detected.ConclusionEculizumab led to rapid and sustained achievement of MM in patients with AChR+ refractory gMG. These findings support the use of eculizumab in this previously difficult-to-treat patient population.ClinicalTrials.gov IdentifierREGAIN, NCT01997229; REGAIN open-label extension, NCT02301624.Classification of EvidenceThis study provides Class II evidence that, after 26 weeks of eculizumab treatment, 25.0% of adults with AChR+ refractory gMG achieved MM, compared with 13.3% who received placebo.

Objective To analyze changes in substantia nigra (SN) iron deposits, assessed by quantitative transcranial sonography (TCS), to obtain and compare substantia nigra echogenicity indices (SNEI) at baseline and after intravenous (IV) iron therapy in patients with restless legs syndrome (RLS)/Willis−Ekbom disease (WED). Methods A total of 30 consecutive subjects diagnosed with RLS/WED were recruited and underwent IV iron treatment. The SNEI, total daily dose of dopamine equivalents, and International Restless Legs Syndrome Rating Scale (IRLS) scores were obtained at baseline and following IV iron administration. Comparative statistics were performed by means of nonparametric testing. Results The sample was stratified into two groups according to the median baseline SNEI and the grade of SN hypoechogenicity: severely hypoechogenic (HE) (n = 13) and moderately HE (n = 17). Following IV iron, the increase in SNEI among severely HE subjects was 19% (0.038 ± 0.046 cm2; P Conclusion Intravenous iron caused changes in SNEI in both groups of patients, reflecting an increase in brain iron stores. However, the increase in SNEI was greater in patients previously defined as severely HE. Furthermore, RLS/WED symptoms also improved more in severely HE subjects, and there was a greater reduction in TDR. This study highlights the role of TCS in quantifying brain iron deposits and in predicting which patients will likely benefit from IV iron.

Objective: The purpose of this study was to analyze the primary and secondary labels of plantbased medicines. Method: This research has a cross-sectional and descriptive. The procedure consisted of analyzing 100 labels of medicinal plant-based products used by elderly people from (in) Macapá-AP, in 2017. A script was prepared to verify their adequacy to health standards based on the Brazilian legislation (commercial name, botanical nomenclature, active ingredients, concentration, route of administration, age-restricted use, quantity and pharmaceutical form, preservation care, company name, CNPJ - Brazils's companies registration number, composition, SAC, expiration date, manufacture and batch), descriptive analysis of the data was performed (protocol number 38400314.9.0000.0003). Results: Only 4% of the products completely met the analyzed criteria. One of the main problems observed was the absence of a uniform standard of presentation of information on the packaging. About 7% of these products did not present any information besides the popular name of the medicinal plant used, 37% of the packages (or labels) contain statements and images that induce self-medication, errors in use, or references to "natural medicine", besides, transmitting the idea that the product has superior properties to drugs on the market. Conclusions: The results obtained suggest that the packaging of medicinal plant-based products induces the irrational use of medicines due to the lack of adequate information, as well as the presence of inadequate makenting strategies, in accordance with current national legislation.

It is unknown whether HTLV-1/2 prevalence has been stable or changing with time in Brazil. We present a 10-year (2007–2016) analysis of HTLV-1/2 infection in first-time blood donors from four blood banks in Brazil. The Brazilian blood centers participating in this multicenter Recipient Epidemiology and Donor Evaluation Study (REDS) are located in Recife in the Northeast and in São Paulo, Rio de Janeiro and Belo Horizonte located in the Southeast of the country. A previous REDS study using the same database from 2007 to 2009 showed that the prevalence per 100,000 donors was 222 in Recife, 83 in Belo Horizonte and 101 in São Paulo. From 2007 to 2016, HTLV-1/2 prevalence was calculated by year, blood center and birth cohort. Covariates included age, gender, schooling, self-reported skin color and type of donation. From 1,092,174 first-blood donations, in the general analysis, HTLV-1/2 infection predominated in females, donors over 50 years of age, black skin color and less educated. The average prevalence was 228 per 100,000 donors in Recife, 222 in Rio de Janeiro, 104 in Belo Horizonte and 103 in São Paulo. In the 10-year analysis, HTLV-1/2 prevalence was stable, but a trend was observed toward an increase in HTLV-1/2 infection among younger people (p < 0.001), males (p = 0.049), those with white skin color (p < 0.001), and higher education (p = 0.014). Therefore, this 10-year surveillance of the infection showed stable HTLV-1/2 prevalence overall but a trend toward increased prevalence among the younger and more educated donors despite Brazilian policies to control sexually transmitted infections being in place for more than 10 years.
