Abstract

Background In the general population with coronavirus disease 2019 (COVID-19), obesity is associated with an increased risk of mortality. Given the typically observed obesity paradox among patients on kidney function replacement therapy (KFRT), especially dialysis patients, we examined the association of obesity with mortality among dialysis patients or living with a kidney transplant with COVID-19. Methods Data from the European Renal Association COVID-19 Database (ERACODA) were used. KFRT patients diagnosed with COVID-19 between 1 February 2020 and 31 January 2021 were included. The association of Quetelet's body mass index (BMI) (kg/m2), divided into: Results In 3160 patients on KFRT (mean age: 65 years, male: 61%), 99 patients were lean, 1151 normal weight (reference), 1160 overweight, 525 obese I and 225 obese II/III. During follow-up of 3 months, 28, 20, 21, 23 and 27% of patients died in these categories, respectively. In the fully adjusted model, the hazard ratios (HRs) for 3-month mortality were 1.65 [95% confidence interval (CI): 1.10, 2.47], 1 (ref.), 1.07 (95% CI: 0.89, 1.28), 1.17 (95% CI: 0.93, 1.46) and 1.71 (95% CI: 1.27, 2.30), respectively. Results were similar among dialysis patients (N = 2343) and among those living with a kidney transplant (N = 817) (Pinteraction = 0.99), but differed by sex (Pinteraction = 0.019). In males, the HRs for the association of aforementioned BMI categories with 3-month mortality were 2.07 (95% CI: 1.22, 3.52), 1 (ref.), 0.97 (95% CI: 0.78. 1.21), 0.99 (95% CI: 0.74, 1.33) and 1.22 (95% CI: 0.78, 1.91), respectively, and in females corresponding HRs were 1.34 (95% CI: 0.70, 2.57), 1 (ref.), 1.31 (95% CI: 0.94, 1.85), 1.54 (95% CI: 1.05, 2.26) and 2.49 (95% CI: 1.62, 3.84), respectively. Conclusion In KFRT patients with COVID-19, on dialysis or a kidney transplant, obesity is associated with an increased risk of mortality at 3 months. This is in contrast to the obesity paradox generally observed in dialysis patients. Additional studies are required to corroborate the sex difference in the association of obesity with mortality.

Abstract

BACKGROUND AND AIMS: Studies examining kidney failure patients with COVID-19 reported higher mortality in hemodialysis patients than in kidney transplant recipients. However, hemodialysis patients are often older and have more comorbidities. This study investigated the association of type of kidney replacement therapy with COVID-19 severity adjusting for differences in characteristics. METHOD: Data were retrieved from the European Renal Association COVID-19 Database (ERACODA), which includes kidney replacement therapy patients diagnosed with COVID-19 from all over Europe. We included all kidney transplant recipients and hemodialysis patients who presented between February 1st and December 1st 2020 and had complete information reason for COVID-19 screening and vital status at day 28. The diagnosis of COVID-19 was made based on a PCR of a nasal or pharyngeal swab specimens and/or COVID-19 compatible findings on a lung CT scan. The association of kidney transplantation or hemodialysis with 28-day mortality was examined using Cox proportional-hazards regression models adjusted for age, sex, frailty and comorbidities. Additionally, this association was investigated in the subsets of patients that were screened because of symptoms or have had routine screening. RESULTS: A total of 1,670 patients (496 functional kidney transplant recipients and 1,174 hemodialysis patients) were examined. 16.9% of kidney transplant recipients and 23.9% of hemodialysis patients died within 28 days of presentation. In an unadjusted model, the risk of 28-day mortality was 33% lower in kidney transplant recipients compared with hemodialysis patients (hazard ratio (HR): 0.67, 95% CI: 0.52, 0.85). However, in an age, sex and frailty adjusted model, the risk of 28-day mortality was 29% higher in kidney transplant recipients (HR=1.29, 95% CI: 1.00, 1.68), whereas in a fully adjusted model the risk was even 43% higher (HR=1.43, 95% CI: 1.06, 1.93). This association in patients who were screened because of symptoms (n=1,145) was similar (fully adjusted model HR=1.46, 95% CI: 1.05, 2.04). Results were similar when other endpoints were studied (e.g. risk for hospitalization, ICU admission or mortality beyond 28 days) as well as across subgroups. Only age was found to interact significantly, suggesting that the increased mortality risk associated with kidney transplantation was especially present in elderly subjects. CONCLUSION: In this study, kidney transplant recipients had a greater risk of a more severe course of COVID-19 compared with hemodialysis patients when adjusted for age, sex and comorbidities.

Abstract

In the general population with COVID-19, the male sex is an established risk factor for mortality, in part due to a more robust immune response to COVID-19 in women. Because patients on kidney function replacement therapy (KFRT) have an impaired immune response, especially kidney transplant recipients due to their use of immunosuppressants, we examined whether the male sex is still a risk factor for mortality among patients on KFRT with COVID-19. From the European Renal Association COVID-19 Database (ERACODA), we examined patients on KFRT with COVID-19 who presented between February 1st, 2020, and April 30th, 2021. 1204 kidney transplant recipients (male 62.0%, mean age 56.4 years) and 3206 dialysis patients (male 61.8%, mean age 67.7 years) were examined. Three-month mortality in kidney transplant recipients was 16.9% in males and 18.6% in females (p = 0.31) and in dialysis patients 27.1% in males and 21.9% in females (p = 0.001). The adjusted HR for the risk of 3-month mortality in males (vs females) was 0.89 (95% CI 65, 1.23, p = 0.49) in kidney transplant recipients and 1.33 (95% CI 1.13, 1.56, p = 0.001) in dialysis patients (pinteraction = 0.02). In a fully adjusted model, the aHR for the risk of 3-month mortality in kidney transplant recipients (vs. dialysis patients) was 1.39 (95% CI 1.02, 1.89, p = 0.04) in males and 2.04 (95% CI 1.40, 2.97, p interaction = 0.02). In patients on KFRT with COVID-19, the male sex is not a risk factor for mortality among kidney transplant recipients but remains a risk factor among dialysis patients. The use of immunosuppressants in kidney transplant recipients, among other factors, may have narrowed the difference in the immune response to COVID-19 between men and women, and therefore reduced the sex difference in COVID-19 mortality risk.

Abstract

BACKGROUND: Kidney transplant patients are at high risk for coronavirus disease 2019 (COVID-19)-related mortality. However, limited data are available on longer-term clinical, functional, and mental health outcomes in patients who survive COVID-19.METHODS: We analyzed data from adult kidney transplant patients in the European Renal Association COVID-19 Database who presented with COVID-19 between February 1, 2020, and January 31, 2021.RESULTS: We included 912 patients with a mean age of 56.7 (±13.7) y. 26.4% were not hospitalized, 57.5% were hospitalized without need for intensive care unit (ICU) admission, and 16.1% were hospitalized and admitted to the ICU. At 3 mo follow-up survival was 82.3% overall, and 98.8%, 84.2%, and 49.0%, respectively, in each group. At 3 mo follow-up biopsy-proven acute rejection, need for renal replacement therapy, and graft failure occurred in the overall group in 0.8%, 2.6%, and 1.8% respectively, and in 2.1%, 10.6%, and 10.6% of ICU-admitted patients, respectively. Of the surviving patients, 83.3% and 94.4% reached their pre-COVID-19 physician-reported functional and mental health status, respectively, within 3 mo. Of patients who had not yet reached their prior functional and mental health status, their treating physicians expected that 79.6% and 80.0%, respectively, still would do so within the coming year. ICU admission was independently associated with a low likelihood to reach prior functional and mental health status.CONCLUSIONS: In kidney transplant recipients alive at 3-mo follow-up, clinical, physician-reported functional, and mental health recovery was good for both nonhospitalized and hospitalized patients. Recovery was, however, less favorable for patients who had been admitted to the ICU.

Supplemental Digital Content is available in the text., Background. Endothelial dysfunction is an early and potentially reversible stage in the atherosclerotic process. We assessed endothelial dysfunction noninvasively in kidney transplant recipients (KTRs) and evaluated the association with mortality and graft outcomes. Methods. Flow-mediated dilation (FMD) was measured in arteria brachialis by ultrasound, with baseline diameters obtained at rest and maximal diameters obtained during reactive hyperemia occurring after 5 min of forearm occlusion. FMD% is the percentage difference of flow-mediated dilation relative to baseline. Endpoints on mortality and graft outcomes were collected from The Norwegian Renal Registry. The distribution of risk according to FMD levels was assessed in Cox regression using a restricted cubic spline function. FMD was dichotomized using receiver operating characteristic analysis to identify optimal cut points at maximal sensitivity and specificity. Results. From a total of 269 KTRs in 2012, 152 (56.5%) were eligible and examined 10 wk after transplantation, and 145 had successful FMD measurements. During a mean follow-up of 6.5 y, 26 patients died, 11 lost their graft, and 34 experienced either graft loss or death. This article provides a summary of the 2019 ERA Registry Annual Report, including data from 34 countries and additional age comparisons. Methods: Individual patient data for 2019 were provided by 35 registries and aggregated data by 17 registries. Using these data, the incidence and prevalence of RRT, the kidney transplantation activity and the survival probabilities were calculated. In 2019, a general population of 680.8 million people was covered by the ERA Registry. Overall, the incidence of RRT was 132 per million population (p.m.p.). Of these patients, 62% were men, 54% were ≥65 years of age and 21% had diabetes mellitus as primary renal disease (PRD), and 84% had haemodialysis (HD), 11% had peritoneal dialysis (PD) and 5% had pre-emptive kidney transplantation as an initial treatment modality. The overall prevalence of RRT on 31 December 2019 was 893 p.m.p., with 58% of patients on HD, 5% on PD and 37% living with a kidney transplant. The overall kidney transplant rate was 35 p.m.p. and 29% of the kidney grafts were from a living donor. The unadjusted 5-year survival probability was 42.3% for patients commencing dialysis, 86.6% for recipients of deceased donor grafts and 94.4% for recipients of living donor grafts in the period 2010-14. When comparing age categories, there were substantial differences in the distribution of PRD, treatment modality and kidney donor type, and in the survival probabilities, The ERA Registry is funded by the ERA. 13. The CYP3A biomarker 4β-hydroxycholesterol does not improve tacrolimus dose predictions early after kidney transplantation

Abstract

Aims: Tacrolimus is a cornerstone in modern immunosuppressive therapy after kidney transplantation. Tacrolimus dosing is challenged by considerable pharmacokinetic variability, both between patients and over time after transplantation, partly due to variability in cytochrome P450 3A (CYP3A) activity. The aim of this study was to assess the value of the endogenous CYP3A marker 4β‐hydroxycholesterol (4βOHC) for tacrolimus dose individualization early after kidney transplantation. Methods: Data were obtained from 79 adult kidney transplant recipients who contributed a total of 625 4βOHC measurements and 1999 tacrolimus whole blood concentrations during the first 2 months after transplantation. The relationships between 4βOHC levels and individual estimates of tacrolimus apparent plasma clearance (CL/Fplasma) at different time points after transplantation were investigated using scatterplots and population pharmacokinetic modelling. Results: There was no significant correlation between pre‐transplant 4βOHC levels and tacrolimus CL/Fplasma the first week (r = 0.19 [95% CI −0.03‐0.40]) or between 4βOHC and tacrolimus CL/Fplasma 1 week (r = 0.20 [−0.11‐0.47]), 4 weeks (r = 0.21 [−0.07‐0.46]) or 2 months (r = 0.24 [−0.03‐0.48]) after transplantation (P ≥ 0.06). In the population analysis, time‐varying 4βOHC was not a statistically significant covariate on tacrolimus CL/Fplasma, neither in terms of absolute values (P = 0.11) nor in terms of changes from baseline (P = 0.17). 4βOHC values increased between 1 week and 2 months after transplantation (median change +57% [IQR +22–83%], P < 0.001), indicating increasing CYP3A activity. Contradictorily, tacrolimus CL/Fplasma decreased over the same period (median change −13% [IQR −3 to −26%], P < 0.001). Conclusions: 4βOHC does not appear to have a clinical potential to improve individualization of tacrolimus doses early after kidney transplantation. Abstract

The photovoltaic solar cell industry is growing rapidly, but high cost per watt is still an obstacle. Thin film solar cells, especially thin film solar cells using CIGS absorbers that have the highest proven efficiency, have the potential to reduce the cost through cheap manufacturing. Academic research concerning CIGS solar cells has so far been focused on cells with absorber layers deposited by co-evaporation, which can be used to make very high efficiency cells but is a deposition process ill suited for large scale production. In this thesis a process for depositing CIGS absorber layers by gas flow sputtering, a deposition technique enabling high rate depositions of low energy particles that is potentially easier to apply to a large scale production, has been outlined. Equipment for CIGS-deposition by gas flow sputtering has been prepared, characteristics of the process have been investigated and ultimately a series of first prototype CIGS absorber layers has been deposited as part of complete solar cells. A lot of focus in this thesis is on the practical work and problem solving around the equipment, e.g. pulsed DC power supplies and electrical connections, heating and heating control in a reactive vacuum environment, and on the basic functionality of the gas flow sputter, how process and film properties like deposition rate, thickness uniformity etc. vary with sputter conditions like pressure, gas flow etc. Following the process design the first prototype series produced crystalline CIGS absorbers of desired elemental composition and thickness but having rather small grain sizes, while the complete cells exhibited solar cell IV-characteristics but very poor efficiencies.

16. DTT-behandling av erytrocytter hos pasienter behandlet med DARA

Abstract

Det opprinnelige formålet med oppgaven var å validere en ny metode som kunne tas i bruk ved rutinemessig blodbankarbeid på St. Olavs Hospital i Trondheim. Daratumumab (DARA) er et legemiddel som benyttes ved behandling av myelomatosepasienter. Blodtransfusjon til myelomatosepasienter behandlet med anti-CD38 (DARA), er en utfordring fordi anti-CD38 vil kunne gi falske positive svar ved antistoffscreening og utvidet forlik. Dithiothreitol (DTT)-behandling av erytrocytter er en metode som kan redusere DARA-interferens, og dermed sikre trygg utlevering av blodprodukter til denne pasientgruppen. Ettersom den praktiske delen av oppgaven ikke var mulig å gjennomføre, ble oppgaven omgjort til en litteraturgjennomgang av tema. Formålet med oppgaven ble endret fra å validere en ny metode, til å gjennomgå litteratur som kunne svare på om det var hensiktsmessig for St. Olavs å innføre DTT-metoden. For å innhente litteratur ble det benyttet ulike søkemotorer, tilsendt litteratur fra St. Olavs, og kontaktpersoner ved andre sykehus. Søkemotoren PubMed ble primært brukt til innhenting av litteratur, med Google og Google Scholar som supplerende søkemotorer. Tilsendt litteratur fra fagpersoner inneholdt prosedyrer og presentasjoner som ikke lå offentlig tilgjengelig på nett. I tillegg ble det tatt kontakt med Oslo Universitetssykehus, Stavanger Universitetssjukehus og Haukeland sykehus. Den internasjonale valideringsstudien i 2016, utført av amerikanske forskere, konkluderte med at DTT-metoden kunne innføres verden over. Andre studier støttet opp om at DTTmetoden er en robust og enkel metode. En viktig faktor var også at å innføre metoden kunne redusere kostnadene betraktelig. I senere tid er det også blitt undersøkt om DARA-interferens kan elimineres med andre metoder, slik som BMAP eller ved bruk av DARA-Fab. DTTmetoden har allerede vært i bruk i en rekke andre norske sykehus. DTT-metoden gjør det tryggere å gi blodprodukter til myelomatosepasienter behandlet med DARA. DTT-metoden er bedre enn feno- og genotyping på flere områder. På bakgrunn av våre funn kan vi anbefale å innføre DTT-metoden for St. Olavs. En metodevalidering bør gjøres i forkant, slik som allerede planlagt. En alternativ mulighet er å undersøke andre metoder som også bevarer alle andre antigener på erytrocyttene. The initial purpose of this bachelor thesis was to validate a new method to be used at the blood bank at St. Olavs Hospital in Trondheim, Norway. Daratumumab (DARA), is used as treatment for multiple myeloma patients, and interferes with routine blood bank testing, such as antibody screening and crossmatching. Dithiothreitol (DTT)-treatment of erythrocytes is a method that can mitigate the DARA interference, and therefore make transfusion of blood safer for multiple myeloma patients treated with DARA. The aim of this thesis was changed after starting the writing proses. Instead of validating the DTT-method for St. Olavs Hospital, we conducted a literature review of the topic. Based on the review we were able to conclude on whether it is appropriate for St. Olavs Hospital to adopt the DTT-method. We used three different search engines to collect literature. In addition, we received literature from experts on the subject and contacting people from other hospitals. Primary the search engine PubMed was used for collecting literature, with supplementary content from Google and Google Scholar. The literature we received from experts on the subject was mainly procedure manuals and presentations that were not available on the internet. We also got in contact with Oslo Universitetssykehus, Stavanger Universitetssjukehus and Haukeland sykehus. All information was collected through the internet since libraries were closed during this period. In 2016, the international validation study concluded that the DTT-method is a great approach to reduce DARA interference worldwide. This was confirmed by other studies. The studies concluded that DTT would reduce costs while keeping good specificity and sensitivity in blood bank testing. It has also been examined if other methods could eliminate DARA interference. Examples is the use of DARA-Fab fragments or the BMAP-method. Other blood banks in Norway has already taken the DTT-method in use. The DTT-method makes it possible to distribute suitable blood products for multiple myeloma patients treated with DARA. DTT-treatment is safer than pheno- and genotyping. Based on the literature review and interviews, we conclude that the DTT-method is appropriate for St. Olavs Hospital to introduce. A validation of the method should be done as initially planned. However, one could also consider other methods that also preserves other antigens on the erythrocytes.

Abstract

Sheep are highly adequate models for human renal diseases because of their many similarities in the histology and physiology of kidney and pathogenesis of kidney diseases. However, the lack of a simple method to measure glomerular filtration rate (GFR) limits its use as a model of renal diseases. Hence, we aimed to develop a simple method to measure GFR based on the plasma clearance of iohexol by assessing different pharmacokinetic models: (a) CL2: two-compartment (samples from 15 to 420 min, reference method), (b) CL1: one-compartment (samples from 60 to 420 min), (c) CLlf: CL1 adjusted by a correction formula and (d) SM: simplified CL2 (15 to 300 min). Specific statistics of agreement were used to test the models against CL2. The agreement between CL1 and CL2 was low, but both CL1f and SM showed excellent agreement with CL2, as indicated by a total deviation index of ~5&ndash, 6%, a concordance correlation of 0.98&ndash, 0.99% and a coverage probability of 99&ndash, 100%, respectively. Hence, the SM approach is preferable due to a reduced number of samples and shorter duration of the procedure, two points that improve animal management and welfare.

18. Effect of cyclosporine A and tacrolimus on OATP1B1 mediated transport of atorvastatin in an in vitro whole cell model

Abstract

BACKGROUND: Dyslipidaemia is more frequent in solid organ transplant recipients than in the general population. In order to reduce the overall cardiovascular risk in transplant patients lipid-lowering drugs, especially statins, have become widely used. However, the use of statins in transplant recipients taking cyclosporine A (CsA) results in significantly increased risk for statin induced side-effects such as myopathy and also rhabdomyolysis. Statin plasma concentrations are also increased several-fold and may be responsible for this increased risk. The other calcineurin inhibitor, tacrolimus (Tac), does not present such an effect on statin plasma concentrations during ordinary clinical use. AIM AND METHOD: The present study elucidated the in vitro cellular aspects and the mechanism of the pharmacokinetic interaction between atorvastatin (HMg-CoA reductase inhibitor) and the two calcineurin inhibitors, CsA and Tac. The inhibitory effect of CsA and Tac on cellular uptake of atorvastatin via the organic anion transporter polypeptide 1B1 (OATP1B1) was investigated in an in vitro over expression whole cell model. RESULTS: Atorvastatin was shown to be transported via the OATP1B1 transporter since cellular uptake was higher in OATP1B1 transfected HEK293 cells compared to mock cells. Inhibition of cellular uptake of atorvastatin was observed with CsA in OATP1B1 transfected cells. No clear inhibition of cellular uptake of atorvastatin was however observed for Tac in the OATP1B1 model, at least not in the lower concentration range investigated. CONCLUSION: The present in vitro study indicates that the cellular uptake of atorvastatin is mediated by OATP1B1 transporters and that CsA inhibits this drug transport. Tac on the other hand did not interfere with cellular uptake of atorvastatin, at least not in the concentration range investigated in this thesis. The observed interaction between atorvastatin and CsA is in line with findings in previous clinical studies, however further investigations is necessary in order to further characterize these findings.
